Abstract:Abdominal aortic aneurysms (AAAs) are local dilatations in the abdominal aorta occurring due to weakening of arterial wall. The present work investigates the influence of the ratio of maximum transverse diameter to abdominal height (DHr) on rupture risk of AAA, using hemodynamics and AAA wall mechanics simulations. We have considered two idealized AAA geometries, AAA1 of higher DHr than AAA2. Two constitutive models, namely, Newtonian and Carreau-Yasuda models have been used for modelling blood as an incompressible fluid. Additionally, in order to describe the behaviour of AAA wall, three constitutive models, namely, linear elastic, Saint Venant Kirchhoff elastic and a phenomenological finite-strain model called Raghavan-Vorp elastic have been utilised. Numerical simulations of AAA biomechanics have been performed using solids4Foam, an open source package built on finite volume framework. Hemodynamic parameteric study reveals that AAA1 has lower time-averaged wall shear stress (TAWSS) and higher oscillatory shear index (OSI) compared to AAA2. Thus, AAA1 has increased susceptibility to thrombus deposition and, therefore, higher risk of AAA rupture. Furthermore, the peak wall stress of AAA1 is about 8% higher than that of AAA2. It is concluded that higher DHr leads to greater rupture risk.

Abstract:Journal of Endovascular Therapy, Ahead of Print. An abdominal aortic aneurysm (AAA) is a typical aortic disease with serious complications. In clinical practice, the decision to intervene in treatment depends on the rupture risk of AAA. Therefore, monitoring the progression of the disease and accurately assessing the rupture risk is of great importance for its treatment. Studies have shown that the biomechanical indicators based on multi-scale models are more effective in accurately assessing the rupture risk of AAA. However, using computational fluid dynamics (CFD) to simulate the biomechanical environment is a cumbersome and time-consuming process, which is inadequate to meet the needs of clinical monitoring and quick decisions. While the hemodynamic environment of AAA is heavily dependent on geometry, more and more biomechanics-related morphological parameters have been raised and validated. In this review, we summarized typical morphological parameters associated with AAA rupture and their relationships with the mechanical environment, including maximum diameter, deformation rate, saccular index, asymmetry, AAA volume, tortuosity, and intraluminal thrombus (ILT), providing a reference for clinical preoperative risk assessment.Clinical ImpactStudies have shown that the biomechanical indicators based on multi-scale models are more effective in accurately assessing the rupture risk of AAA. To meet the need for clinical monitoring and rapid decision making, the typical morphological parameters associated with AAA rupture and their relationships with the mechanical environment have been summarized, which provide a reference for clinical preoperative risk assessment of AAA.

Abstract:Extended Abstract Abdominal Aortic Aneurysm (AAA) is a disease with a balloon-shaped dilation of blood vessels occurring in the abdominal aorta. It has been known that the prevalence of AAA occurred about 6~8% in men over the age of 65. AAA usually occurs in the infrarenal abdominal aorta [1]. If blood vessels expand abnormally 3 times or more than normal abdominal aortic diameter, the risk of rupture of blood vessel will be increased [2]. The main purposes of the study are to investigate the impact of the risk factors of the abdominal aortic aneurysmal geometric models such as aneurysm size, asymmetry, and neck angle in the aorta proximal the aneurysm bulge using the computational fluid dynamics (CFD). Changes in the blood vessel shape due to AAA not only make alteration of the flow characteristic patterns but also will be varied with hemodynamic stresses acting on the vascular wall [3, 4]. Aneurysmal expansion and the risk of rupture are dependent on the hemodynamic stresses acting on the vessel wall and mechanical stresses. Therefore, it is important to examine the morphological changes of blood flow for the variation in the aneurysm shapes. The parametric study in AAA models is carried out examining effects of the possible risk factors of rupture. The CFD simulations are conducted to analyse the impact of the geometrical shapes such as aneurysm size, asymmetry, and neck angle in the aorta proximal the aneurysm bulge as well as bifurcation angle into the iliac artery. Furthermore, the numerical simulation is performed to understand the impact of the Non-Newtonian effects. The hemodynamic phenomena in AAA vessels are shown by comparing how different compared to the healthy artery. It is found that the neck angle has significant impact on the blood flow patterns and wall shear stress distribution on the surface of AAA. It is also found that the blood flow field in the aorta upstream of the aneurysm is strongly dependent on the AAA geometrical shape. The results show the peak shear stress and the peak pressure within the aneurysm as a function of neck angle, aneurysm size, and asymmetry, respectively. The results of this study provide useful parametric information for the additional diagnostic tool to help clinicians.

Abstract:To improve abdominal aortic aneurysm (AAA) rupture risk assessment, a large, longitudinal study on AAA hemodynamics and biomechanics is necessary, using personalized fluid-structure interaction (FSI) modeling. 3-dimensional, time-resolved ultrasound (3D+t US) is the preferred image modality to obtain the patient-specific AAA geometry for such a study, since it is safe, affordable and provides temporal information. However, the 3D+t US field-of-view (FOV) is limited and therefore often fails to capture the inlet and aorto-iliac bifurcation geometry. In this study, a framework was developed to add parametric inlet and bifurcation geometries to the abdominal aortic aneurysm geometry by employing dataset statistics and parameters of the AAA geometry. The impact of replacing the patient-specific inlet and bifurcation geometries, acquired using computed tomography (CT) scans, by parametric geometries was evaluated by examining the differences in hemodynamics (systolic and time-averaged wall shear stress and oscillatory shear index) in the aneurysm region. The results show that the inlet geometry has a larger effect on the AAA hemodynamics (median differences of 7.5 to 18.8%) than the bifurcation geometry (median differences all below 1%). Therefore, it is not feasible to replace the patient-specific inlet geometry by a generic one. Future studies should investigate the possibilities of extending the proximal FOV of 3D+t US. However, this study did show the feasibility of adding a parametric bifurcation geometry to the aneurysm geometry. After extending the proximal FOV, the obtained framework can be used to extract AAA geometries from 3D+t US for FSI simulations, despite the absence of the bifurcation geometry.

Objective:Extensive research has focused on the evaluation of rupture risks in abdominal aortic aneurysms (AAAs) through comprehensive morphologic and hemodynamic analyses, primarily considering the AAA as a whole entity. This study tried to identify the high-risk rupture sites of AAAs more precisely before the fatal process based on morphologic and hemodynamic analyses at the local segment.Methods:Computed tomography angiography of a specific AAA patient was conducted at the follow-up 4 months before rupture, 1 day before rupture, the day of the rupture, and 15 days after endovascular aortic repair. The evolution of local morphology and the hemodynamic characteristics at these critical timepoints were investigated based on patient-specific reconstructions and computational fluid dynamics.Results:The morphologic and hemodynamic parameters of the rupture region vary continuously in the process of AAA development and rupture. The surface area and volume of the rupture segment were gradually enlarged at the follow-up 4 months before rupture (47.33 cm2; 67.35 mL), 1 day before rupture (57.23 cm2; 85.24 mL), and on the day of the rupture (62.41cm2; 104.73ml). A prominent decrease in time-averaged wall shear stress and velocity for the rupture segment is observed. The percentages of the lowest time-averaged wall shear stress (<0.1 Pa) area are increased in the AAA region (20.42%, 33.85%, and 53.00%, separately).Conclusions:The results based on precisely rebuilt geometries for the complete follow-ups of patient-specific computed tomography angiography demonstrate that notable morphologic and hemodynamic evolutions have occurred in the local segment of the AAA, which was further proved at the rupture site. The significant changes occurring at the local segment may provide valuable information for the evaluation of aneurysm rupture risk and locate the most probable site of rupture.Clinical ImpactCapturing the entire process of AAA rupture through CTA imaging is a rare occurrence in clinical practice. The evolution of morphology and hemodynamic characteristics observed in the illustrated results provides valuable insights for clinicians to monitor the state of AAA from a different perspective. These findings suggest that variations in morphology and hemodynamics within the local segment of the AAA might serve as an alternative approach for predicting the rupture risk of AAA.

Abstract:Abdominal aortic aneurysms (AAA) are localized, commonly occurring aortic dilations. Following rupture only immediate treatment can prevent morbidity and mortality. AAA maximal diameter and growth are the current metrics to evaluate the associated risk and plan intervention. Although these criteria alone lack patient specificity, predicting their evolution would improve clinical decision. If the disease is known to be associated with altered morphology and blood flow, intraluminal thrombus deposit and clinical symptoms, the growth mechanisms are yet to be fully understood. In this retrospective longitudinal study of 138 scans, morphological analysis and blood flow simulations for 32 patients with clinically diagnosed AAAs and several follow-up CT-scans, are performed and compared to 9 control subjects. Several metrics stratify patients between healthy, low and high risk groups. Local correlations between hemodynamic metrics and AAA growth are also explored but due to their high inter-patient variability, do not explain AAA heterogeneous growth. Finally, high-risk predictors trained with successively clinical, morphological, hemodynamic and all data, and their link to the AAA evolution are built from supervise learning. Predictive performance is high for morphological, hemodynamic and all data, in contrast to clinical data. The morphology-based predictor exhibits an interesting effort-predictability tradeoff to be validated for clinical translation.

Abstract:Abstract Background Abdominal Aortic Aneurysm (AAA) rupture is the most severe consequence of aneurysmal disease, with an overall death rate of 80%. Early diagnosis and appropriate follow-up and management of AAA will significantly reduce the risk of rupture and mortality. Rupture risk assessment of AAA has a critical role for the early warning. Purpose To Predict of the course of an AAA and determine the rupture risk and location from a biomechanical point of view. AAA hemodynamics were investigated to clarify the relation between the hemodynamic parameters and the AAA rupture. Methods Computed tomography (CT) images of one healthy aorta and 23 patients’ aorta with aneurysms were analyzed. WSS-related hemodynamic parameters of time-averaged wall shear stress (TAWSS), oscillatory shear index (OSI), endothelial cell activation potential (ECAP), and relative residence time (RRT) are investigated in patient-specific AAA geometries by performing computational fluid dynamics (CFD) simulations (Figure 1). Patient-specific AAA geometries are identified using medical imaging techniques and realistic boundary conditions are determined using Doppler ultrasonography. Results AAA rupture locations predicted by CFD simulations were in good agreement with the actual rupture locations of retrospective data. The rupture locations had relatively low TAWSS, high OSI, high ECAP, and high RTT levels. High risk of rupture is expected in regions with a local minimum of TAWSS, and a local maximum of OSI, ECAP, and RRT. Conclusions Examining the spatial distribution of the hemodynamic parameters was a powerful approach for rupture risk assessment of AAA. Predicting the rupture location using a single hemodynamic parameter is not feasible, and the accuracy of AAA rupture risk assessment can be improved by using multiple WSS-related parameters by identifying the common intersection regions of low TAWSS, high OSI, high ECAP, and high RRT.Figure 1

Abstract:In abdominal aortic aneurysm (AAA), the rupture of the aortic tissue is directly related to wall stress. Thus, the investigation of maximum wall stress is a necessary procedure to predict the aortic rupture in AAA. In this study, computational simulations were performed to investigate the correlation between peak wall stress (PWS) and AAA geometry. The Holzapfel model and various orientations of the collagen fibers and thicknesses of the layers of the aorta were employed in the simulation. The material constants used in the Holzapfel model were estimated from the examination and analysis of the biaxial tensile test results of the normal abdominal aorta and AAA. The aneurysm diameter, height, neck angle, and iliac angle were selected as geometric factors affecting the AAA rupture. In addition, a simulation scenario was conceived and created based on the measurement results using the computed tomography data of patients with AAA. Accordingly, the correlation between the PWS and AAA geometry was estimated.

Abstract:In this work, the impact of modeling techniques on predicting the mechanical behaviors of abdominal aortic aneurysm (AAA) is systematically investigated. The fluid-structure interaction (FSI) model for simultaneously capturing the transient interaction between blood flow dynamics and wall mechanics was compared with its simplified techniques, that is, computational fluid dynamics (CFD) or computational solid stress (CSS) model. Results demonstrated that CFD exhibited relatively smaller vortexes and tends to overestimate the fluid wall shear stress, compared to FSI. On the contrary, the minimal differences in wall stresses and deformation were observed between FSI and CSS models. Furthermore, it was found that the accuracy of CSS prediction depends on the applied pressure profile for the aneurysm sac. A large pressure drop across AAA usually led to the underestimation of wall stresses and thus the AAA rupture. Moreover, the assumed isotropic AAA wall properties, compared to the anisotropic one, will aggravate the difference between the simplified models with the FSI approach. The present work demonstrated the importance of modeling techniques on predicting the blood flow dynamics and wall mechanics of the AAA, which could guide the selection of appropriate modeling technique for significant clinical implications.

A Re-Examination of the Role of the Nucleus in Generating the Circadian Rhythm in Acetabularia

Abstract:The role of the nucleus in the generation of the circadian rhythm in Acetabularia has been unclear. Early experiments showed that the plant could exhibit a circadian rhythm in the absence of a nucleus. However, other experiments appeared to show that the nucleus could impart phase information to the rhythm, and so therefore must be a part of the system that generates the rhythm. We have conducted experiments similar to these—in particular, one in which the nuclear end of the plant was entrained on a light-dark cycle that was opposite that of the rest of the plant. The phase of the free running rhythm of this type of plant is not consistent with the conclusion that the nucleus is part of the circadian oscillator. We have also tried entraining opposite ends of plants with no nuclei on opposite light-dark cycles. The ultimate phases of these plants appear to be nearly random. A possible interpretation of these experiments is discussed.
Geometrical Factors Affect Wall Shear Stress in Saccular Aneurysms of the Infrarenal Abdominal Aorta

Abstract:Background: Low wall shear stress (WSS) is predictive of aortic aneurysm growth and rupture. Yet, estimating WSS in a clinical setting is impractical, whereas measuring aneurysm geometry is feasible. This study investigates the association between saccular aneurysm geometry of the infrarenal aorta and WSS. Methods: Starting with a nonaneurysmal, patient-specific, computational fluid dynamics model of the aorta, saccular aneurysms of varying geometry were created by incrementally increasing the neck width and sac depth from 1 cm to 4 cm. The aspect ratio (the ratio between sac depth and neck width) varied between 0.25 and 4. The peak WSS, time-averaged WSS (TAWSS), and oscillatory shear index (OSI) were measured within the aneurysm sac. Results: Decreasing the neck width from 4 cm to 1 cm decreased the peak WSS by 69% and the TAWSS by 83%. Increasing the sac depth from 1 cm to 4 cm decreased the peak WSS by 55% and the OSI by 37%. The aspect ratio was negatively correlated to peak WSS (Rs -0.85; P < 0.001). Conclusions: In saccular aneurysms of the infrarenal aorta, a smaller neck width, deeper aneurysm sac, and larger aspect ratio are associated with lower peak WSS.

Abstract:Patient-specific studies on vascular flows have significantly increased for hemodynamics due to the need for different observation techniques in clinical practice. In this study, we investigate aortic aneurysms in terms of deformation, stress, and rupture risk. The effect of Ascending Aortic Diameter (AAD) was investigated in different aortic arches (19.81 mm, 42.94 mm, and 48.01 mm) via Computational Fluid Dynamics (CFD), Two-way coupling Fluid–Structure Interactions (FSI) and deep learning. The non-newtonian Carreau viscosity model was utilized with patient-specific velocity waveform. Deformations, Wall Shear Stresses (WSSs), von Mises stress, and rupture risk were presented by safety factors. Results show that the WSS distribution is distinctly higher in rigid cases than the elastic cases. Although WSS values rise with the increase in AAD, aneurysm regions indicate low WSS values in both rigid and elastic artery solutions. For the given AADs, the deformations are 2.75 mm, 6. 82 mm, and 8.48 mm and Equivalent von Mises stresses are 0.16 MPa, 0.46 MPa, and 0.53 MPa. When the rupture risk was evaluated for the arteries, the results showed that the aneurysm with AAD of 48.01 mm poses a risk up to three times more than AAD of 19.81 mm. In addition, an Artificial neural network (ANN) method was developed to predict the rupture risk with a 98.6% accurate prediction by numerical data. As a result, FSI could indicate more accurately the level of rupture risk than the rigid artery assumptions to guide the clinical assessments and deep learning methods could decrease the computational costs according to CFD and FSI.

Ascending aortic volume: A feasible indicator for ascending aortic aneurysm elective surgery?

Abstract:Diameter-based criterion have been widely adopted for preventive surgery of ascending thoracic aortic aneurysm (ATAA). However, recent and growing evidence has shown that diameter-based methods may not be sufficient for identifying patients who are at risk of an ATAA. In this study, fluid-structure interaction (FSI) analysis was performed on one-hundred ATAA geometries reconstructed from clinical data to examine the relationship between hemodynamic conditions, ascending aortic volume (AAV), ascending aortic curvature, and aortic ratios measured from the reconstructed 3D models. The simulated hemodynamic and biomechanical parameters were compared among different groups of ATAA geometries classified based on AAV. The ATAAs with enlarged AAV showed significantly compromised hemodynamic conditions and higher mechanical wall stress. The maximum oscillatory shear index (OSI), particle residence time (PRT) and wall stress (WS) were significantly higher in enlarged ATAAs compared with controls (0.498 [0.497, 0.499] vs 0.499 [0.498, 0.499], p = 0.002, 312.847 [207.445, 519.391] vs 996.047 [640.644, 1573.140], p < 0.001, 769.680 [668.745, 879.795] vs 1072.000 [873.060, 1280.000] kPa, p < 0.001, respectively). Values were reported as median with interquartile range (IQR). AAV was also found to be more strongly correlated with these parameters compared to maximum diameter. The correlation coefficient between AAV and average WS was as high as 0.92 (p < 0.004), suggesting that AAV might be a feasible risk identifier for ATAAs. STATEMENT OF SIGNIFICANCE: Ascending thoracic aortic aneurysm is associated with the risk of dissection or rupture, creating life-threatening conditions. Current surgical intervention guidelines are mostly diameter based. Recently, many studies proposed to incorporate other morphological parameters into the current clinical guidelines to better prevent severe adverse aortic events like rupture or dissection. The purpose of this study is to gain a better understanding of the relationship between morphological parameters and hemodynamic parameters in ascending aortic aneurysms using fluid-solid-interaction analysis on patient-specific geometries. Our results suggest that ascending aortic volume may be a better indicator for surgical intervention as it shows a stronger association with pathogenic hemodynamic conditions.

Abstract:Abdominal aortic aneurysm (AAA) is a critical health disorder, where the abdominal aorta dilates more than 50% of its normal diameter. Enlargement in abdominal aorta alters the hemodynamics and flow-induced forces on the AAA wall. Depending on the flow conditions, the hemodynamic forces on the wall may result in excessive mechanical stresses that lead to AAA rupture. The risk of rupture can be predicted using advanced computational techniques such as computational fluid dynamics (CFD) and fluid–structure interaction (FSI). For a reliable rupture risk assessment, formation of intraluminal thrombus (ILT) and uncertainty in arterial material properties should be taken into account, mainly due to the patient-specific differences and unknowns in AAAs. In this study, AAA models are computationally investigated by performing CFD simulations combined with FSI analysis. Various levels of ILT burdens are artificially generated in a realistic AAA geometry, and the peak effective stresses are evaluated to elucidate the effect of material models and ILT formation. The results indicate that increasing the ILT burden leads to lowered effective stresses on the AAA wall. The material properties of the artery and ILT are also effective on the stresses; however, these effects are limited compared to the effect of ILT volume in the AAA sac.

Abstract:Purpose This study aimed to evaluate the association between the outflow morphology and abdominal aortic aneurysm (AAA) rupture risk, to find risk factors for future prediction models. Materials and methods We retrospectively analyzed 46 patients with ruptured AAAs and 46 patients with stable AAAs using a 1:1 match for sex, age, and maximum aneurysm diameter. The chi-square test, paired t -test, and Wilcoxon signed-rank test were used to compare variables. Logistic regression was performed to evaluate variables potentially associated with AAA rupture. Receiver operating characteristic curve analysis and the area under the curve (AUC) were used to assess the regression models. Results Ruptured AAAs had a shorter proximal aortic neck (median (interquartile range, IQR): 24.0 (9.4–34.2) mm vs. 33.3 (20.0–52.8) mm, p = 0.004), higher tortuosity (median(IQR): 1.35 (1.23–1.49) vs. 1.29 (1.23–1.39), p = 0.036), and smaller minimum luminal area of the right common iliac artery (CIA) (median (IQR): 86.7 (69.9–126.4) mm 2 vs. 118.9 (86.3–164.1)mm 2 , p = 0.001) and left CIA (median(IQR): 92.2 (67.3,125.1) mm 2 vs. 110.7 (80.12, 161.1) mm 2 , p = 0.010) than stable AAA did. Multiple regression analysis demonstrated significant associations of the minimum luminal area of the bilateral CIAs (odds ratio [OR] = 0.996, 95 % confidence interval [CI] 0.991 – 0.999, p = 0.037), neck length (OR = 0.969, 95 % CI 0.941 – 0.993, p = 0.017), and aneurysm tortuosity (OR = 1.031, 95 % CI 1.003 – 1.063, p = 0.038) with ruptured AAAs. The AUC of this regression model was 0.762 (95 % CI 0.664 – 0.860, p < 0.001). Conclusions The smaller minimum luminal area of the CIA is associated with an increased risk of rupture. This study highlights the potential of utilizing outflow parameters as novel and additional tools in risk assessment. It also provides a compelling rationale to further intensify research in this area.

Effect of aortic bifurcation geometry on pressure and peak wall stress in abdominal aorta: Fluid-structure interaction study

Abstract:Background and objective: Geometry of aorto-iliac bifurcation may affect pressure and wall stress in aorta and thus potentially serve as a predictor of abdominal aortic aneurysm (AAA), similarly to hypertension. Methods: Effect of aorto-iliac bifurcation geometry was investigated via parametric analysis based on two-way weakly coupled fluid-structure interaction simulations. The arterial wall was modelled as isotropic hyperelastic monolayer, and non-Newtonian behaviour was introduced for the fluid. Realistic boundary conditions of the pulsatile blood flow were used on the basis of experiments in literature and their time shift was tailored to the pulse wave velocity in the model to obtain physiological wave shapes. Eighteen idealized and one patient-specific geometries of human aortic tree with common iliac and renal arteries were considered with different angles between abdominal aorta (AA) and both iliac arteries and different area ratios (AR) of iliac and aortic luminal cross sections. Results: Peak wall stress (PWS) and systolic blood pressure (SBP) were insensitive to the aorto-iliac angles but sensitive to the AR: when AR decreased by 50%, the PWS and SBP increased by up to 18.4% and 18.8%, respectively. Conclusions: Lower AR (as a result of the iliac stenosis or aging), rather than the aorto-iliac angles increases the BP in the AA and may be thus a risk factor for the AAA development.

Abstract:New imaging methods have enabled the detection of unruptured abdominal aortic aneurysms (AAA). It is necessary to develop appropriate mathematical models for rupture prediction to allow a proper patient treatment plan. To provide valid hemodynamic parameters, high-fidelity numerical models with patient-specific boundary conditions are needed. Researchers have pointed out in recent research articles and reviews that those morphological parameters, such as shape, dilation ratio, neck angle, common iliac bifurcation angle, and AAA type, consistently correlate with the rupture mechanism. However, it is unclear how morphological indicators affect hemodynamics-based computational fluid dynamics predictions. The present work investigates the influence of AAA shape on local and global hemodynamics parameters and rupture predictions. Five cases of magnetic resonance imaging scan-based data for patient-specific aortofemoral artery modeling are explored. The inflow conditions are patient-specific, and an open loop system has been considered to model all five cases. Hemodynamics parameters in pulsating conditions, such as wall shear stress (WSS), velocity contour, time average WSS (TAWSS), oscillatory shear index (OSI), vorticity, and streamlines, are computed and investigated. Both maximum dilation diameter and aneurysm neck angle are found to have substantial effects on local hemodynamics parameters. The magnitude of WSS, TAWSS, and OSI increases and decreases non-linearly with a change in maximum diameter during the cardiac process. Also, aneurysms with doubly titled and completely saccular shape show complex streamlines, low WSS, and high residence time in the sac area of the wall.

Abstract:Background: Abdominal aortic aneurysm (AAA) is a pathological condition characterized by the dilation of the lower part of the aorta, where significant hemodynamic forces are present. The prevalence and high mortality risk associated with AAA remain major concerns within the scientific community. There is a critical need for extensive research to understand the underlying mechanisms, pathophysiological characteristics, and effective detection methods for abdominal aortic abnormalities. Additionally, it is imperative to develop and refine both medical and surgical management strategies. This review aims to indicate the role of computational analysis in the comprehension and management of AAAs and covers recent research studies regarding the computational analysis approach conducted between 2021 and 2023. Computational analysis methods have emerged as sophisticated and noninvasive approaches, providing detailed insights into the complex dynamics of AAA and enhancing our ability to study and manage this condition effectively. Methods: Computational analysis relies on fluid mechanics principles applied to arterial flow, using the Navier-Stokes equations to model blood flow dynamics. Key hemodynamic indicators relevant to AAAs include Time-Average Wall Shear Stress, Oscillatory Shear Index, Endothelial Cell Activation Potential, and Relative Residence Time. The primary methods employed for simulating the abdominal aorta and studying its biomechanical environment are computational fluid dynamics and Finite Element Methods. This review article encompasses a thorough examination of recent literature, focusing on studies conducted between 2021 and 2023. Results: The latest studies have elucidated crucial insights into the blood flow characteristics and geometric attributes of AAAs. Notably, blood flow patterns within AAAs are associated with increased rupture risk, along with elevated intraluminal thrombus volume and specific calcification thresholds. Asymmetric AAAs exhibit heightened risks of rupture and thrombus formation due to low and oscillating wall shear stresses. Moreover, larger aneurysms demonstrate increased wall stress, pressure, and energy loss. Advanced modeling techniques have augmented predictive capabilities concerning growth rates and surgical thresholds. Additionally, the influence of material properties and thrombus volume on wall stress levels is noteworthy, while inlet velocity profiles significantly modulate blood flow dynamics within AAAs. Conclusions: This review highlights the potential utility of computational modeling. However, the clinical applicability of computational modeling has been limited by methodological variability despite the ongoing accumulation of evidence supporting the prognostic significance of biomechanical and hemodynamic indices in this field. The establishment of standardized reporting is critical for clinical implementation.
Sidewall Aneurysm Geometry As a Predictor of Rupture Risk Due to Associated Abnormal Hemodynamics

Abstract:Background: Hemodynamics play an important role in intracranial aneurysm (IA) initiation, growth, and rupture. Yet there remains no definitive quantitative analysis between abnormal hemodynamics and geometrical risk of IA development. Objective: The present study aims to investigate whether abnormal hemodynamics in IA sacs can be predicted by surrogate geometric markers. Methods: Computational fluid dynamics (CFD) simulations were performed on paraclinoid aneurysms derived from digital subtraction angiography (DSA) of 104 IAs in 104 patients. Four basic IA geometric parameters including maximum height, perpendicular height, maximum width, and neck diameter were measured. Abnormal hemodynamics were defined and quantified as the surface area exposed to low wall shear stress (WSS) and high oscillatory shear index (OSI) using objectively-defined thresholds. Relationships between abnormal hemodynamics and specific geometric parameters were analyzed via multiple linear regression. Results: Adjusting for age, sex, and other clinical characteristics, multiple linear regression revealed a significant relationship (p < 0.001) between abnormal hemodynamics and both maximum width (β ≈ 1.2) and neck diameter (β ≈ -0.4), but not maximum height or perpendicular height. These findings were shown to be independent of the choice of abnormal hemodynamic indicators and threshold levels. Conclusions: Maximum width and neck diameter of IA sacs are robust surrogates of exposure to abnormal hemodynamics. Risk of rupture may be increased with wider aneurysms with narrower necks for paraclinoid aneurysms.
A hemodynamic analysis of energy loss in abdominal aortic aneurysm using three-dimension idealized model

Abstract:Objective: The aim of this study is to perform specific hemodynamic simulations of idealized abdominal aortic aneurysm (AAA) models with different diameters, curvatures and eccentricities and evaluate the risk of thrombosis and aneurysm rupture. Methods: Nine idealized AAA models with different diameters (3 cm or 5 cm), curvatures (0° or 30°) and eccentricities (centered on or tangent to the aorta), as well as a normal model, were constructed using commercial software (Solidworks; Dassault Systemes S.A, Suresnes, France). Hemodynamic simulations were conducted with the same time-varying volumetric flow rate extracted from the literature and 3-element Windkessel model (3 EWM) boundary conditions were applied at the aortic outlet. Several hemodynamic parameters such as time-averaged wall shear stress (TAWSS), oscillatory shear index (OSI), relative residence time (RRT), endothelial cell activation potential (ECAP) and energy loss (EL) were obtained to evaluate the risk of thrombosis and aneurysm rupture under different conditions. Results: Simulation results showed that the proportion of low TAWSS region and high OSI region increases with the rising of aneurysm diameter, whereas decreases in the curvature and eccentric models of the corresponding diameters, with the 5 cm normal model having the largest low TAWSS region (68.5%) and high OSI region (40%). Similar to the results of TAWSS and OSI, the high ECAP and high RRT areas were largest in the 5 cm normal model, with the highest wall-averaged value (RRT: 5.18 s, ECAP: 4.36 Pa −1 ). Differently, the increase of aneurysm diameter, curvature, and eccentricity all lead to the increase of mean flow EL and turbulent EL, such that the highest mean flow EL (0.82 W · 10 −3 ) and turbulent EL (1.72 W · 10 −3 ) were observed in the eccentric 5 cm model with the bending angle of 30°. Conclusion: Collectively, increases in aneurysm diameter, curvature, and eccentricity all raise mean flow EL and turbulent flow EL, which may aggravate the damage and disturbance of flow in aneurysm. In addition, it can be inferred by conventional parameters (TAWSS, OSI, RRT and ECAP) that the increase of aneurysm diameter may raise the risk of thrombosis, whereas the curvature and eccentricity appeared to have a protective effect against thrombosis.
