Abstract:Patients undergoing radiation therapy for the head and neck are susceptible to a significant and often abrupt deterioration in their oral health. The oral morbidities of radiation therapy include but are not limited to an increased susceptibility to dental caries and periodontal disease. They also include profound and often permanent functional and sensory changes involving the oral soft tissue. These changes range from oral mucositis experienced during and soon after treatment, mucosal opportunistic infections, neurosensory disorders, and tissue fibrosis. Many of the oral soft tissue changes following radiation therapy are difficult challenges to the patients and their caregivers and require life-long strategies to alleviate their deleterious effect on basic life functions and on the quality of life. We discuss the presentation, prognosis, and management strategies of the dental structure and oral soft tissue morbidities resulting from the administration of therapeutic radiation in head and neck patient. A case for a collaborative and integrated multidisciplinary approach to the management of these patients is made, with specific recommendation to include knowledgeable and experienced oral health care professionals in the treatment team.

[A case of metastatic Enterococcus faecalis endophthalmitis].

Abstract:We reported a case of successful treatment of early-stage metastatic endophthalmitis caused by Enterococcus faecalis with vitrectomy and lensectomy. The case was a 50-year-old male with poorly controlled diabetes. Following T-tube drainage for a necrotic cholecystitis operation, he developed iridocyclitis in both eyes as well as fever. At the time of his first visit to our clinic, his right eye had already lost light perception. His left eye had visual acuity recognizing of hand movement, marked uveitis, complicated cataract, and dense vitreous opacity. As gram positive cocci were isolated from the aspirated vitreous, we conducted lensectomy and vitrectomy under irrigation of antibiotics. With systemic postoperative antibiotics and human immunoglobulin, the patient showed remarkable improvement in his ocular fundus. By 60 days after the operation, the visual acuity of his left eye recovered to 4/20. Metastatic Enterococcus faecalis endophthalmitis has almost nerve been reported in Japan. The diagnosis and treatment of this disease with a reference to the above findings were discussed.
Progress of Oral Sequelae During Head-Neck Radiotherapy.

Abstract:OBJECTIVE:To evaluate xerostomia, mucositis and dental caries during head and neck radiotherapy.METHODS:Twenty patients with nasopharyngeal carcinoma were included. Oral examinations were conducted before radiotherapy, after dosage of 2000 cGy irradiation, immediately after the termination of radiotherapy, and 1 month and 6 months after termination of the radiotherapy. Oral hygiene instruction, effective oral care and dental intervention were performed during the treatment. Salivary flow rate was evaluated by modified Schirmer's test. Xerostomia, mucositis and dental caries status were evaluated based on oral examinations.RESULTS:Salivary flow rate decreased significantly after the first dosage of 2000 cGy, and was aggravated with the increase in irradiation dosage until the termination of radiotherapy. Xerostomia and mucositis were observed in parallel with the reduction of saliva flow rate, and were aggravated with the increase in irradiation dosage. Mucositis began to recover within 1 month after the termination of radiotherapy and fully recovered within 6 months after the termination of the radiotherapy. Six months after the termination of irradiation, new carious lesions were detected in two patients.CONCLUSION:Oral sequelae developed during radiotherapy of the head and neck. Oral health instructions and effective intervention were essential before, during and after the radiotherapy.

Abstract:Abstract Amongst all the different varieties of cancers diagnosed across the world around 6% is found to be head and neck cancer and radiotherapy is the choice of treatment. Head and neck radiotherapy cause several changes in dentition, saliva, oral microflora and food habits of the patient thus increasing the risk of radiation-induced dental caries a common, yet serious complication. This paper analyses the literature pertaining to radiation-induced caries and discusses the predisposing factors, etiology, structural changes in the tooth, clinical features of radiation caries and management. To review this, the Pubmed and Scopus database was searched using the keywords “radiation-induced caries”, “xerostomia”, “effects of radiology” and “head and neck tumors”, “osteoradionecrosis”, “management of radiation caries”, “structural changes in teeth after radiotherapy”, “prevention of radiation caries”. Radiation caries is a rampant form of caries commonly seen within the first 3 months after the radiotherapy. Thus, early dynamic involvement of the dentist in the preventive and therapeutic strategies are important. Furthermore, educating and motivating patients about the risk factors, maintenance of oral hygiene, stimulation of salivary flow, fluoride use, and nutritional orientation are essential to decrease the incidence of radiation caries and eventually enhances the quality of life of patients.

Abstract:External beam radiotherapy is an integral part of the management of head and neck cancer. Radiotherapy for head and neck cancer lead to many treatment-related toxicities, including a wide range of dental morbidities. These complications depend on the various patient and treatment-related factors. Patients with poor oral hygiene, pre-existing periodontal disease, and the conventional mode of radiotherapy delivery are major factors leading to acute and severe late dental problems. Most of the complications are preventable with proper oral and dental care before, during and after radiotherapy. Pre radiotherapy dental evaluation to diagnose patients with potential risk and timely intervention can reduce long-term morbidities like carries, osteoradionecrosis, etc., thereby improving patients' quality of life. This article highlights the various aspects of dental care and management in head neck cancer patients before, during and after radiation therapy.

Abstract:The number of head and neck cancer (HNC) patients is steadily increasing, and the use of radiotherapy for treatment in this patient group has been associated with side effects such as mucositis, trismus, xerostomia, dental caries, periodontal disease, and osteoradionecrosis (ORN). Radiation caries (RC) can be atypical, and their treatment can be challenging. Preventing and treating RC become crucial as post-radiotherapy tooth extraction can pave the way for ORN. The aim of this review is to evaluate the restorative dental treatment for HNC patients before, during, and after radiotherapy. This includes examining the impact of radiotherapy on tooth decay and demineralization, and providing solutions to address these effects.

Abstract:Objective To review our experience with a standardized dental management approach in patients with planned radiotherapy of the head and neck region based on preradiation and follow‐up data. Material and Methods Records of patients who underwent radiotherapy between June 2016 and November 2020 were reviewed. Data on dental findings and therapeutic recommendations were extracted from a prospectively managed database. Hospital records were used to obtain follow‐up data. Results Two hundred eighty‐one patient records were identified. After the exclusion of 81 patients because of incomplete data, 200 patients were included in the study. Dental findings relevant to radiotherapy were found in 144 cases (72.0%). Teeth extractions were recommended in 112 (56.0%) patients. Follow‐up data were available for 172 (86.0%) patients (mean follow‐up: 16.8 ± 10.7 months). Radiodermatitis was the most frequently observed sequela of radiotherapy (42.4%), followed by dysphagia (38.4%) and stomatitis (36.6%). Osteoradionecrosis was observed in only 2.3% of the patients. Conclusion Dental findings relevant to planned radiotherapy were frequent and in many cases resulted in recommendations for teeth extraction. Based on our standardized dental management protocol, we observed low rates of late oral complications after radiotherapy of the head and neck region.

Abstract:The ultraviolet spectrum of methyl isothiocyanate (CH3NCS) and the quantum yield for its dissociation into methyl isocyanide (CH3NC) and atomic sulfur at 308 nanometers, Φ = 0.98 � 0.24, were measured. Methyl isothiocyanate is widely used as an agricultural fumigant and readily enters the atmosphere during and after application. The results indicate that photodissociation by sunlight is an effective pathway for its removal from the atmosphere.

Abstract:The interaction between POCl(3) or POBr(3) and pyridine or DMAP has been reinvestigated to clarify the discrepancies between previously published results concerning the Lewis acidity of phosphoryl halides and their behavior toward pyridine bases. The obtained results show that POCl(3) virtually does not react with pyridine, while it does with 4-(dimethylamino)pyridine (DMAP), even in SO(2) solution, to yield an ionic compound [(DMAP)(2)PO(2)]Cl.3SO(2) (1.3SO(2)). Its recrystallization from acetonitrile gives [(DMAP)(2)PO(2)]Cl.CH(3)CN (1.CH(3)CN). The POBr(3) reacts readily with both DMAP and pyridine forming the analogous tribromides, [(DMAP)(2)PO(2)]Br(3) (2) and [(py)(2)PO(2)]Br(3) (3), respectively. Treatment of 3 with Me(3)SiOSO(2)CF(3) in acetonitrile solution led to [(py)(2)PO(2)][CF(3)SO(3)].CH(3)CN (4), while the reaction between 1.CH(3)CN and Me(3)SiOPOF(2) gave [(DMAP)(2)PO(2)][PO(2)F(2)] (5). The crystal structures of 1.CH(3)CN, 1.3SO(2), 2, and 4 revealed that all four compounds are ionic containing the distorted tetrahedral cations [(DMAP)(2)PO(2)](+) and [(py)(2)PO(2)](+). Both ions represent a donor-stabilized form of the so far unknown cation [PO(2)](+). The geometry of [(DMAP)(2)PO(2)](+), optimized by density functional calculations at the B3LYP/6-31G(d,p) level, is in good agreement with X-ray structural data. The NBO analysis of natural atomic charges shows an extensive delocalization of the [PO(2)](+) intrinsic positive charge and indicates a contribution of the electrostatic attraction to the formation of N-P donor-acceptor bonds. According to a (31)P NMR study, the reactions of both phosphoryl halides with DMAP proceed via successive formation of the intermediates [(DMAP)POX(2)](+) and (DMAP)PO(2)X to give an equimolar mixture of [(DMAP)(2)PO(2)](+) and PX(5) (X = Cl, Br) as the end products. The NMR spectroscopic identification of the cations [(DMAP)POX(2)](+) and [(DMAP)(2)PO(2)](+) was supported by ab initio calculations of their chemical shifts.

Abstract:Objective To investigate the status of xerostomia,which is the symptoms of the Nasopharyngeal Carcinoma pa-tients after the face and neck radiotherapy,in order to guide the clinical practice and improve the patients quality of life.Methods 70 Nasopharyngeal Carcinoma patients were investigated by the Wijers OB,etc-designed questionnaire of xerostomia,and analyze the incidence and the effects on daily life.Results The incidence of xerostomia is 100%.Those from moderate to high degree comes to 68.6%.Because of xerostomia,70% of patients have to change their living habits.Conclusions The symptoms of xe-rostomia take a serious impact on the patients’quality of life.Health education and appropriate nursing care can alleviate the symp-toms and improve the quality of life.

Abstract:Radiotherapy to the head and neck region (HN) bears the risk of a rampant development of caries, making intensified prevention necessary. Aim of this systematic review was to summarize the evidence on the efficacy of caries preventive measures in these patients. Clinical studies investigating caries in patients with radiotherapy in the HN with at least one caries preventive intervention compared to any control were included. Reports in languages other than English or German were excluded. Records were identified on PubMed, Web of Science, Google Scholar and Cochrane Library mid-January 2024. Risk of bias was assessed with RoB2. Results were summarized. Planned meta-analyses could not be performed, because of heterogenous data. Five studies were included, with a total of 355 participants. They were irradiated with up to 70Gy and received different caries preventive interventions, including sodium fluoride or stannous fluoride gels, remineralizing solutions, an "Intraoral Fluoride Release System" and sucrose restricted diet. Caries score increased between 0.48 DMF-S and 9.2 DF-S per year. Largest differences in caries increments were measured between groups with insufficient and with rigorous fluoride application. The main limitations were compromised randomisation, heterogeneity of patients and small sample sizes. Clinical studies on caries prevention after radiotherapy to the HN are lacking and the existing ones bear extensive limitations. However, the large effect size and broad consensus suggest the use of fluoride to be indispensable in preventing caries in these patients. The study was funded by the Medical Center - University of Freiburg and was not registered.

Oral and dental health care of oral cancer patients: hyposalivation, caries and infections

Abstract:Oral cancer and its treatment can cause a variety of problems to patients, also as regards maintaining their daily oral hygiene. Surgery mutilates tissues which may hamper cleaning the teeth and mucosal surfaces. The patient may have complicated reconstructive structures that also need continuous attention. Radiotherapy-induced hyposalivation further complicates the situation and decreases the quality of life. Consequently, dental caries, mucosal diseases such as candidosis and sialadenitis become problematic to treat. Hence every effort should be focused on prevention. In caries prevention intensified fluoride therapy together with dietary counseling is needed. Oral cancer patients also need to be frequently referred to dental hygienists for professional cleaning. Drinking enough daily and moisturizing mucosal surfaces with commercial dry-mouth products, vegetable oils, milk products and respective topical agents need to be individually recommended. In addition, patients with severe dry mouth cases may also benefit from the prescription of pilocarpine tablets. In oral candidosis, the microbiological diagnosis must be confirmed before administration of antifungal drugs in order to avoid the selection pressure to resistant strains.
Prevalence and risk factors of chronic oral complications in head and neck cancer therapies: A retrospective study

Abstract:Background: Oncological therapy can trigger various complications and side effects in certain tissues, such as the oral cavity, inducing a direct or indirect impact on basic functions and the patient's quality of life. The aim of the study is to determine the prevalence of chronic oral complications of oncological treatments in patients with head and neck cancer and assess their possible relationship with risk indicators associated with the patient, the tumor, and the treatment. Material and methods: A retrospective, single-center, observational pilot cohort study was designed at the Dental Hospital of the University of Barcelona, involving patients with head and neck cancer who underwent surgery, non-surgical oncological therapy (radiotherapy/chemotherapy/immunotherapy), or combined therapy. Medical histories were analyzed, and data related to demographics, toxic, hygienic, and dietary habits, systemic and oral health status, characteristics of cancer and its treatment, and registered chronic oral complications were collected. The results were expressed in descriptive measures (means, standard deviations, counts, prevalence, and 95% confidence intervals), and for statistical associations, parametric and non-parametric tests were used. Results: The overall prevalence of chronic oral complications was 92.57%. Dental disease (81.14%), periodontal disease (65.14%), and hyposalivation/xerostomia (62.86%) showed the highest prevalence. Advanced age, certain cancer locations, advanced cancer stages, and oncological therapy including radiotherapy were significantly associated with the presence and number of complications. Conclusions: The elevated noticed prevalence necessitates rigorous monitoring and preventive care. The combination of risk factors can significantly contribute to oral complications. Understanding these factors services dentists establish protocols for preventing, diagnosing, and treating oncology patients.
A systematic review of dental disease in patients undergoing cancer therapy

Abstract:Introduction: This purpose of this systematic review was to evaluate the literature and update our current understanding of the impact of present cancer therapies on the dental apparatus (teeth and periodontium) since the 1989 NIH Development Consensus Conference on the Oral Complications of Cancer Therapies. Review method: A systematic literature search was conducted with assistance from a research librarian in the databases MEDLINE/PubMed and EMBASE for articles published between 1 January 1990 and 31 December 2008. Each study was independently assessed by two reviewers. Taking into account predetermined quality measures, a weighted prevalence was calculated for the prevalence of dental caries, severe gingival disease, and dental infection. Data on DMFT/dmft, DMFS/dmfs, plaque, and gingival indexes were also gathered. The level of evidence, recommendation, and guideline (if possible) were given for published preventive and management strategies. Results: Sixty-four published papers between 1990 and 2008 were reviewed. The weighted overall prevalence of dental caries was 28.1%. The overall DMFT for patients who were post-antineoplastic therapy was 9.19 (SD, 7.98; n = 457). The overall plaque index for patients who were post-antineoplastic therapy was 1.38 (SD, 0.25; n = 189). The GI for patients who were post-chemotherapy was 1.02 (SD, 0.15; n = 162). The weighted prevalence of dental infections/abscess during chemotherapy was reported in three studies and was 5.8%. Conclusions: Patients who were post-radiotherapy had the highest DMFT. The use of fluoride products and chlorhexidine rinses are beneficial in patients who are post-radiotherapy. There continues to be lack of clinical studies on the extent and severity of dental disease that are associated with infectious complications during cancer therapy.
Dental rehabilitation of the patient with severe caries after radiation therapy

Abstract:Dental management of the patient with severe caries after radiation therapy can be accomplished by the general practitioner in an office setting. Because of the decrease in vascularity that results from radiotherapy, the general practitioner must keep the following points in mind when treating these patients: Extraction of teeth that were in the field of radiotherapy is contraindicated. If there is a question about a possible extraction, consultation should be made with the patient's radiotherapist and a hospital-trained dentist (preferably, an oral surgeon). Endodontic treatment is the only treatment indicated for pulpally damaged teeth in the field of radiotherapy. When endodontic treatment is done, the following precautions are necessary: all teeth that are an immediate source of infection must be treated first; prophylactic antibiotics should be administered for the duration of treatment'; all endodontic procedures must be administered in an aseptic field of treatment--if a tooth is badly deteriorated as a result of severe caries, a copper band should be used with a rubber dam and no tooth should ever be left "open for drainage" in these patients; temporary fillings should be of sufficient strength to prevent contamination between appointments; tooth-length determination should be done with precision--instrumentation beyond the apex should be avoided; caustic irrigating solutions such as sodium hypochlorite should be avoided--sterile water or sterile saline solution is preferable. For fabrication of prostheses, the general practitioner should be aware that: metallic-oxide impression materials such as zinc-oxide and eugenol should be avoided because of their irritating properties; trismus is common when the masticatory muscles are bilaterally in the field of radiotherapy-therefore, to prevent encroachment on the freeway space for these patients, the vertical dimension of occlusion should be decreased accordingly; monoplane teeth are preferable to cusped teeth for patients with trismus because their border movements are often irregular-monoplane teeth cause less interferences and are easier for the patient to manage; a remount of the overdentures to check the occlusion as well as a careful examination of flange extensions are necessary to prevent tissue ulcerations that could lead to tissue and bony necroses. Cast restorations should be avoided for patients with severe caries after radiation therapy (for example, a cast core and dowel). Prevention of postradiotherapy treatment complications is the primary goal in the management of the patient who has had head and neck cancer. If, however, the patient who has received radiation therapy and as a result has severe caries does come to the general practitioner, a logical and organized treatment plan can be followed that will result in control of the carious disease process and the functional rehabilitation of the patient.

Abstract:Introduction: Head and neck cancer squamous cell carcinoma (HNSCC) is the seventh most common cancer worldwide with around 600,000 new diagnosis each year. Nowadays, in locally advanced disease, radiotherapy (RT) play an important role, this with or without chemotherapy in organ preservation strategies. More specific for early stage localized disease, RT (or surgery) seems to give similar results on locoregional control (LRC) and choice is made according to the organ preservation issue. Despite the fact that technical improvements have been made to optimize the radiation dose delivery and minimize the normal tissue toxicity, RT is associated with potential early and late toxicities. Osteoradionecrosis of the jaw (ORNJ), especially seen after teeth extraction, is one of the associated toxicities and can significantly impair the patient's quality of life. Because of the fear of developing ORNJ, one is very reluctant to extract or place a dental implant post-radiotherapy, especially in high irradiation dose zones (>40 Gy). Hence, it is important to define teeth at risk of future extraction before initiating RT and to handle those in high-risk irradiation zones. In order to optimise extractions, we created a predictive model of the expected irradiation dose, and thus the need for extraction, to the teeth bearing bones. The aim of this study is to validate our model and to define the potential relationship between the radiation dose received by each tooth and the dental complications observed. Material and methods: Between March 2012 and March 2018, patients with HNSCC treated by intensity modulated RT were retrospectively analysed. The mean irradiation dose for each tooth was generated on the administered treatment plan by contouring each tooth separately on each dosimetric scan section using dedicated software (Eclipse, Varian). In order to validate our predictive model, we compared the actual generated/administered teeth irradiation doses with the irradiation doses predicted by our model. Results: Our predictive model was accurate in 69.6% of the cases. In 12.5% of cases the predicted dose was higher than the calculated dose and lower in 17,8% of the cases. A correct- or over-estimation (is the latter being clinically less worrying than an underestimated dose) was achieved in 82% of cases. For the 18% of cases underfitting, the mean margin of error was 5.7 Gy. No statistically significant association was found between the development of caries and doses to the teeth, doses to the parotid glands or dental hygiene. However, a significant association between dental irradiation at more than 40 Gy and the occurrence of dental fractures (p = 0.0002) were demonstrated. Conclusions: Our predictive model seems to be 82% accurate for dose prediction, hence might be helpful for optimizing/minimizing prophylactic extractions. Indeed, following our model, professionals could decide not to extract damaged teeth in areas not at risk of ORNJ, lowering morbidity during and after RT. Contrary to the literature, no relationship was found between the occurrence of dental caries and parotid irradiation and the patient's oral hygiene. However, for the first time, a highly significant correlation between the occurrence of dental fracture and dental irradiation at more than 40 Gy was observed.

Abstract:PurposeThis systematic review aimed to assess the literature for management strategies and economic impact of salivary gland hypofunction and xerostomia induced by cancer therapies and to determine the quality of evidence-based management recommendations.MethodsThe electronic databases of MEDLINE/PubMed and EMBASE were searched for articles published in English since the 1989 NIH Development Consensus Conference on the Oral Complications of Cancer Therapies until 2008 inclusive. For each article, two independent reviewers extracted information regarding study design, study population, interventions, outcome measures, results, and conclusions.ResultsSeventy-two interventional studies met the inclusion criteria. In addition, 49 intensity-modulated radiation therapy (IMRT) studies were included as a management strategy aiming for less salivary gland damage. Management guideline recommendations were drawn up for IMRT, amifostine, muscarinic agonist stimulation, oral mucosal lubricants, acupuncture, and submandibular gland transfer.ConclusionsThere is evidence that salivary gland hypofunction and xerostomia induced by cancer therapies can be prevented or symptoms be minimized to some degree, depending on the type of cancer treatment. Management guideline recommendations are provided for IMRT, amifostine, muscarinic agonist stimulation, oral mucosal lubricants, acupuncture, and submandibular gland transfer. Fields of sparse literature identified included effects of gustatory and masticatory stimulation, specific oral mucosal lubricant formulas, submandibular gland transfer, acupuncture, hyperbaric oxygen treatment, management strategies in pediatric cancer populations, and the economic consequences of salivary gland hypofunction and xerostomia.

Oral health considerations in cancer survivors

Abstract:Over the past decade, advances in cancer treatment have helped in prolonging the survival rate for cancer patients. However, the patients who undergo treatment for cancer are potentially at high-risk for developing a number of oral complications, including oral mucositis, infections, hyposalivation, dental caries, and jaw osteonecrosis. Cancer survivors may remain at life-long risk of developing oral complications, and therefore require long-term dental follow-up, well after completion of cancer therapy. Patients should typically undergo thorough oral examination prior to initiation of therapy, during and after therapy to identify any active infection. In addition, and in order to maintain adequate oral health throughout treatment, patients should continue normal oral hygiene with tooth brushing and interproximal cleaning. The aim of this review is to discuss potential oral complications as a result of cancer therapy, and the certain precautions we should be aware of these patients.

Abstract:Tooth extraction in patients who have undergone head and neck radiotherapy can be challenging. This case report describes the conservative clinical management of teeth affected by radiation caries (RadC). A 51-year-old woman with hydrocephalus was referred for the treatment of teeth affected by RadC. Upon clinical examination, it was evident that she experienced severe oral dryness, and her lower canines exhibited extensive caries involving the crown. She had previously received treatment for a myoepithelial tumor in the hard palate, which included surgery and adjuvant radiation therapy.
