Abstract:OBJECTIVE:To observe the effects of conventional drug therapy combined with acupuncture and conventional drug therapy on the morbidity of post-stroke depression and recovery of neurological function in patients with acute stroke, and provide new ideas for prevention and treatment of post-stroke depression. METHODS:Sixty patients were randomly divided into an observation group and a control group, 30 cases in each one.Conventional drug therapy was given in the control group;on the basis of conventional drug therapy,acupuncture was applied at Baihui (GV 20), Sishencong (EX-HN1), Neiguan (PC 6), Hegu (LI 4), Taixi (KI 3), etc. The treatment was given once every day, 6 times a week for 4 weeks in the two groups. The 24 Hamilton depression rating scale (HAMD), modified Edinburgh-Scandinavian stroke scale (MESSS) scores were recored before treatment, after 1 week of treatment, and after 4 weeks of treatment in the two groups,and the morbidity of post-stroke depression was compared between the two groups. RESULTS:After 1 week of treatment, the HAMD scale scores of the two groups were higher than those before treatment (both P<0.05), and the MESSS scores were lower than those before treatment in the two groups (both P<0.05). After 4 weeks of treatment, the HAMD scale scores in the observation group were lower than that before treatment (P<0.05), and significantly better than that in the control group (P<0.01), but there was no significant difference in the control group before and after treatment (P>0.05). The MESSS scores were lower than those after 1 week of treatment in the two groups (both P<0.05), and the score in the observation group was significantly lower than that in the control group (P<0.01). After 4 weeks of treatment, the morbidity of post-stroke depression in the observation group was 23.3% (7/30), which was lower than 66.7% (20/30) in the control group (P<0.05). CONCLUSION:Early acupuncture intervention on stroke patients can not only promote the recovery of neurological function, but also reduce the morbidity of post-stroke depression effectively.

Abstract:BACKGROUND: This study aimed to analyze the effect of traditional Chinese medicine-based rehabilitation nursing combined with scalp acupuncture (TCMRN + SA) on negative emotions and the quality of life of patients with stroke.METHODS: 102 patients with stroke admitted to The First People's Hospital of Huzhou from September 2019 to December 2020 were included in this study using the convenience sampling method and split into an observation group and a control group at random (n = 51 in each group). Individuals in the control group received TCMRN, whereas patients in the observation group received TCMRN + SA. Furthermore, the negative emotions and quality of life of the individuals in both groups were evaluated before and after the intervention using the Pittsburgh sleep quality index scale, Self-Rating Depression Scale (SDS), Self-Rating Anxiety Scale (SAS), as well as Activity of Daily Living Scale. Furthermore, the efficiency of the sleep-quality intervention between the 2 groups was compared.RESULTS: After the intervention, the Pittsburgh sleep quality index scale, SDS, as well as self-rating anxiety scale scores of individuals in the observation group were considerably lower in comparison to the individuals in the control group (P < .01). Activity of daily living scale scores in the observation group also differed considerably from those in the control group (P < .01). Moreover, the sleep quality efficiency rate in the observation group (90.19% [46/51]) was substantially higher than that in the control group (70.59% [36/51]) (P < .05).CONCLUSION: TCMRN + SA can effectively improve patients' negative emotions and quality of life and is worthy of clinical promotion and application.

Abstract:Background: Stroke is a type of neurological disorder that can be attributed to a central vascular cause. Stroke can affect the patient’s performance in daily activities and quality of life (QoL). Research indicates a decrease in the quality of life in stroke patients due to cognitive and emotional damage. Objectives: The present study compared the effect of two group approaches, mindfulness training based on stress reduction and Powell’s cognitive rehabilitation, on improving the quality of life of stroke patients. Methods: This semi-experimental study used a pretest-posttest design with a control group and a follow-up period. The sample consisted of 36 eligible stroke patients selected in a targeted and simple random manner. The patients were divided into two intervention groups and one control group. The tools used were a demographic information questionnaire and a Stroke-specific QoL (SS-QOL) scale. Data were analyzed with SPSS software (version. 26). Chi-square, repeated measures analysis of variance, and Bonferroni statistical tests were applied at a significance level of P 0.05). The mindfulness intervention had a higher effect on quality of life on average. Still, statistically, none of the interventions were effective in improving quality of life (P > 0.05), and there was no significant difference between the effect of mindfulness-based methods and cognitive rehabilitation on improving quality of life (P > 0.05). Conclusions: Mindfulness training increases behavioral responses related to attention as the basis of all cognitive activities. Also, mindfulness exercises improve cognitive skills and, by affecting the cognitive system, increase the capacity and ability of the information processing system and improve the quality of life.
The effects of mindfulness-based intervention on quality of life and poststroke depression in patients with spontaneous intracerebral hemorrhage in China

Abstract:Objective: Spontaneous intracerebral hemorrhage (sICH) is a devastating disease that can lead to poststroke depression (PSD) and greatest impact on the quality of life (QOL) of patients. Mindfulness meditation was viewed as one of the effective ways to reduce PSD in patients with cancer. The present study tried to investigate whether mindfulness meditation has potential benefits in PSD and QOL for sICH patients in China. Methods: Two hundred and two patients in West China Hospital, Sichuan University, enrolled from January 2017 to December 2018 were included in a randomized controlled trial. After removing missing values, there were 67 in control group and 67 in intervention group. Patients in intervention group received 2-month mindfulness-based cognitive therapy, and patients in control group received stress management education (ie, an active control). Results: The results suggested that the significant differences of depression, trait mindfulness, social well-being, emotional well-being, and total score of QOL were found in intervention group from time 1 to time 2. Physical well-being and the score of NIH stroke scale experienced significant changes in both control group and intervention group over time. Conclusions: Mindfulness-based intervention has positive effects on sICH patients' depression, social well-being, and emotional well-being. However, the change of trait mindfulness over time could not explain these positive effects. Future studies could explore the mechanism of mindfulness-based intervention on sICH patients' depression and QOL and clarify the boundaries of the positive effects of mindfulness-based intervention.

Abstract:Aim: To explore the effects of brain–computer interface training combined with mindfulness therapy on Hemiplegic Patients with Stroke. Background: The prevention and treatment of stroke still faces great challenges. Maximizing the improvement of patients' ability to perform activities of daily living, limb motor function, and reducing anxiety, depression, and other social and psychological problems to improve patients' overall quality of life is the focus and difficulty of clinical rehabilitation work. Methods: Patients were recruited from December 2021 to November 2022, and assigned to either the intervention or control group following a simple randomization procedure (computer-generated random numbers). Both groups received conventional rehabilitation treatment, while patients in the intervention group additionally received brain–computer interface training and mindfulness therapy. The continuous treatment duration was 5 days per week for 8 weeks. Limb motor function, activities of daily living, mindfulness attention awareness level, sleep quality, and quality of life of the patients were measured (in T0, T1, and T2). Generalized estimated equation (GEE) were used to evaluate the effects. The trial was registered with the Chinese Clinical Trial Registry (ChiCTR2300070382). Results: A total of 128 participants were randomized and 64 each were assigned to the intervention and control groups (of these, eight patients were lost to follow-up). At 6 months, compared with the control group, intervention group showed statistically significant improvements in limb motor function, mindful attention awareness, activities of daily living, sleep quality, and quality of life. Conclusion: Brain–computer interface combined with mindfulness therapy training can improve limb motor function, activities of daily living, mindful attention awareness, sleep quality, and quality of life in hemiplegic patients with stroke. Impact: This study provides valuable insights into post-stroke care. It may help improve the effect of rehabilitation nursing to improve the comprehensive ability and quality of life of patients after stroke. Mindfulness therapy for patients with coronary heart disease: A systematic review and meta-analysis

Abstract:Aim: Coronary heart disease (CHD) is a prevalent cardiovascular disease with high mortality rates worldwide. Patients with CHD often experience adverse psychological stress related to the disease's diagnosis, treatment and recovery phases. This stress can hurt sleep quality and overall quality of life. Mindfulness-based interventions (MBIs) have been studied as a psychotherapeutic approach to alleviating the psychological stress associated with CHD. This study aimed to determine the effectives of MBIs for health outcomes in patients with CHD. Methods: A total of eight English-language databases were searched, and eight relevant studies were included in the analysis. The included studies were assessed for literature quality, and data were extracted and analysed using Review Manager 5.3. Results: A total of eight studies involving 802 participants were included in the analysis. Compared to control groups, MBIs significantly reduced anxiety, depression, perceived stress, and systolic blood pressure. However, there was no significant effect on diastolic blood pressure, quality of life or body mass index. One study reported that MBIs significantly improved sleep quality in patients with acute myocardial infarction after percutaneous coronary intervention but had no significant effect on body mass index. Conclusion: MBIs had significant effects on anxiety and depression in patients with CHD, reduced perceived stress and were associated with reductions in systolic blood pressure and improvements in sleep quality. However, they did not significantly affect diastolic blood pressure, quality of life or body mass index.
Efficacy of Mindfulness-Based Intervention ('Mindfulness-Based Joyful Sleep') in Young and Middle-Aged Individuals with Insomnia Using a Biomarker of Inflammatory Responses: a Prospective Protocol of a Randomised Controlled Trial in China.

Abstract:INTRODUCTION:Insomnia is a prevalent and significant public health concern. Insomnia can lead to increased inflammatory markers associated with chronic diseases such as cardiovascular disease, diabetes and cancer. Studies suggest that mindfulness-based interventions (MBIs) are more easily delivered within the community than cognitive behavioural therapy for insomnia (CBT-I) which was recommended as the preferred non-pharmacological treatment by the American Academy of Sleep Medicine, are effective in insomnia treatment and can reduce inflammatory markers level in older individuals with insomnia. This study aims to compare the effectiveness of an MBI to CBT-I in young and middle-aged individuals with insomnia disorder and explore its effect on nuclear factor kappa B (NF-κB), a transcription factor that controls the expression of genes involved in inflammation.METHODS AND ANALYSIS:This report describes a protocol for a randomised controlled trial. Seventy eligible participants will be assigned to mindfulness-based joyful sleep or CBT-I for 2-hour sessions weekly for 8 weeks. The primary outcome is sleep quality assessed by the Pittsburgh Sleep Quality Index, severity of insomnia symptoms assessed by the Insomnia Severity Index and sleep parameters recorded using sleep diary and polysomnography. Secondary outcomes include perceived stress, anxiety and depression. The exploratory outcome is serum level of NF-κB. Outcomes will be evaluated at baseline, the end of the ntervention period and at a 3 month follow-up. Data will be analysed using general linear models, specifically analysis of covariance and analysis of variance will be used.ETHICS AND DISSEMINATION:Full ethical approval for this study has been obtained from the Ethics Committee of the Third Xiangya Hospital, Central South University, Changsha, China (2018-S236). Effects of Meditation and Breathing Therapy on Anxiety Improvement before and after Percutaneous Coronary Intervention in Patients with Coronary Artery Disease: A Prospective Cohort Study

Abstract:Abstract Background: Coronary artery disease (CAD) is a leading cause of worldwide mortality, and percutaneous coronary intervention (PCI) has long been used to alleviate its symptoms. However, patient anxiety has been noted to exacerbate the negative symptoms of CAD, as well as reduce the effectiveness of PCI. Numerous techniques have been developed for alleviating patient stress, such as mindfulness and breathing exercises, though their efficacy with respect to improving PCI outcomes has still not been fully defined. In this study, we aimed to shed light by examining whether CAD patients who underwent such exercises demonstrated lowered anxiety and improved functional outcomes post-PCI compared to CAD patients who did not undergo these procedures. Methods: One hundred and sixty CAD patients were enrolled and randomly divided into two 80-member groups, one of which (intervention) was prescribed meditation therapies, in the form of 6-min mindfulness videos, breathing therapies carried out 12 times per cycle, for 3 times/day, as well as targeted psychological counseling. The other 80-patient group served as the untreated control (Con). Anxiety was monitored during that time period using the Zung’s Self-rating Anxiety Scale (SAS), while sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI); furthermore, various cardiac functioning parameters, such as heart rate, were measured. In addition, the occurrence of serious adverse events (SAEs) was monitored during the 3-month follow-up period post-PCI. Results: No significant differences were present between the intervention and Con groups for baseline patient characteristics, except for body mass index. However, the intervention group had lower stress levels, in the form of lower SAS, compared to baseline, while the opposite was true for the untreated Con group. Lower SAS also corresponded to lower PSQI pre-PCI in intervention, indicating that this group had improved sleep quality, while PSQI remained unchanged from baseline to PCI in Con. Along with lower stress levels, intervention had lower blood pressures, heart rate, and SAEs, compared to Con, both pre-PCI and during the 3-month follow-up period, illustrating that meditation and respiratory therapies are able to reduce stress pre-PCI, in turn improving PCI outcomes and reducing complications postprocedure. Conclusion: Meditation and respiratory therapies are able to effectively alleviate anxiety and improve sleep quality pre-PCI, in turn improving cardiac functional parameters and facilitating smooth PCI procedures. All of these improvements resulted in increased PCI success rates, lowered hospitalization time, and reduced postsurgical complications in the form of SAEs, enabling these strategies to be of great clinical utility for better preparing CAD patients for the procedure. Influence of insomnia and its correction on the emotional state of post-stroke patients

Abstract:Introduction. Stroke is a major cause of morbidity and mortality in various populations. Sleep disturbances are common in patients who have suffered an acute cerebral circulatory disorder, complicating the recovery of motor functions, worsening the emotional state and being associated with poorer health and quality of life in general. Aim. To investigate the impact of sleep disturbance on the emotional state of stroke patients. Materials and methods. The study included 100 people who were divided into two groups. 50 patients who had suffered an acute cerebral circulatory disorder and who suffered from insomnia formed the main group, which was divided into 2 subgroups. MG 1 – main subgroup 1 (26 people), who received a course of short-term (5 sessions) cognitive-behavioural therapy for insomnia in combination with the drug Valocordin-Doxylamine 25 mg as part of complex therapy. – MG 2 main subgroup 2 (24 people), who received only standard treatment of the main disease. Patients in the comparison group who underwent acute cerebral circulatory disorder without concomitant insomnia (CG, 50 people) also received only standard treatment for the primary condition. In addition to clinical examination, the Hospital Anxiety and Depression Scale (HADS), the Pittsburgh Sleep Quality Questionnaire (PSQI) and the Insomnia Severity Scale (ISI) were used to assess patients. Results. Initially, the main group had a statistically significant higher level of depression (clinically expressed depression) than the comparison group (subclinically expressed depression) (U = 54.6; p = 0.012). As a result of treatment in main subgroup 1, where insomnia was corrected, there was not only an improvement in sleep (median PSQI score before treatment 8 (6; 10) and after treatment 5 (5; 6), p = 0.022), but also a reduction in depression (HADS-D score before treatment 12 (10; 14) and after treatment 12 (10; 14)): 12 (10; 14) and post-treatment – 5 (4; 6), p = 0.022) and anxiety (HADS-A pre-treatment: 9 (7; 10) and post-treatment – 5 (4; 6), p = 0.013). In the main subgroup 2, where insomnia was not corrected, there was no improvement in sleep (PSQI before treatment 8 (6; 11) and after treatment 8 (7; 10), p = 0.433), depression (HADS-D before treatment: 9 (8; 11) and after treatment – 8 (6; 9), p = 0.171) and anxiety (HADS-A before treatment: 9 (6; 10) and after treatment – 8 (6; 9), p = 0.064) were less pronounced than in the main subgroup 1 and the comparison group (HADS-D: H (2, N = 100) = 25.73829, p = 0.016; HADS-A: H (2, N = 100) = 28.42621, p = 0.004). Conclusion. Complex therapy for stroke patients suffering from insomnia, including drug and non-drug methods of insomnia correction, increases the effectiveness of treatment by not only improving the quality of sleep, but also reducing the severity of anxiety and depression.
The Feasibility of Tai Chi Exercise as a Beneficial Mind-Body Intervention in a Group of Community-Dwelling Stroke Survivors with Symptoms of Depression

Abstract:Depression is prevalent among one-third to two-thirds of acute and chronic stroke survivors. Despite the availability of pharmacotherapies and/or psychotherapies, depression persists, even for 5–10 years after stroke, reflecting limited treatment responses and/or adherence to this conventional care. Mind-body interventions are commonly used among adults to ameliorate depressive symptoms. Thus, the feasibility of Tai Chi, alongside conventional care, to manage poststroke depression was investigated using a single-group pre-post intervention design. Recruitment and retention, intervention adherence, safety, acceptability, and fidelity were assessed. Symptoms of depression, anxiety, and stress were assessed using standardized questionnaires, objective sleep was assessed via a research-grade triaxial accelerometer, and blood samples were taken to measure oxidative stress, inflammatory markers, and a neurotrophic growth factor using commercially available kits per manufacturer’s protocol. Pre-post intervention changes were assessed using paired t-tests. We enrolled stroke survivors (N = 11, mean age = 69.7 ± 9.3) reporting depression symptoms. After the intervention, we observed significant reductions in symptoms of depression (−5.3 ± 5.9, p = 0.01 ), anxiety (−2.2 ± 2.4, p = 0.01 ), and stress (−4.6 ± 4.8, p = 0.01 ), along with better sleep efficiency (+1.8 ± 1.8, p = 0.01 ), less wakefulness after sleep onset (−9.3 ± 11.6, p = 0.04 ), and less time awake (−9.3 ± 11.6, p = 0.04 ). There was a 36% decrease in oxidative stress ( p = 0.02 ), though no significant changes in the other biomarkers were found (all p values >0.05). Tai Chi exercise is a feasible intervention that can be used alongside conventional care to manage poststroke depression, aid in reducing symptoms of anxiety and stress, and improve sleep.

Improving sleep after stroke: A randomised controlled trial of digital cognitive behavioural therapy for insomnia

Abstract:Summary Stroke is frequently accompanied by long‐term sleep disruption. We therefore aimed to assess the efficacy of digital cognitive behavioural therapy for insomnia to improve sleep after stroke. A parallel group randomised controlled trial was conducted remotely in participant's homes/online. Randomisation was online with minimisation of between‐group differences in age and baseline Sleep Condition Indicator‐8 score. In total, 86 community‐dwelling stroke survivors consented, of whom 84 completed baseline assessments (39 female, mean 5.5 years post‐stroke, mean 59 years old), and were randomised to digital cognitive behavioural therapy or control (sleep hygiene information). Follow‐up was at post‐intervention (mean 75 days after baseline) and 8 weeks later. The primary outcome was self‐reported insomnia symptoms, as per the Sleep Condition Indicator‐8 (range 0–32, lower numbers indicate more severe insomnia, reliable change 7 points) at post‐intervention. There were significant improvements in Sleep Condition Indicator‐8 for digital cognitive behavioural therapy compared with control (intention‐to‐treat, digital cognitive behavioural therapy n = 48, control n = 36, 5 imputed datasets, effect of group p ≤ 0.02, ηp2 = 0.07–0.12 [medium size effect], pooled mean difference = −3.35). Additionally, secondary outcomes showed shorter self‐reported sleep‐onset latencies and better mood for the digital cognitive behavioural therapy group, but no significant differences for self‐efficacy, quality of life or actigraphy‐derived sleep parameters. Cost‐effectiveness analysis found that digital cognitive behavioural therapy dominates over control (non‐significant cost savings and higher quality‐adjusted life years). No related serious adverse events were reported to the researchers. Overall, digital cognitive behavioural therapy for insomnia effectively improves sleep after stroke. Future research is needed to assess earlier stages post‐stroke, with a longer follow‐up period to determine whether it should be included as part of routine post‐stroke care. Effects of Meditation versus Music Listening on Perceived Stress, Mood, Sleep, and Quality of Life in Adults with Early Memory Loss: A Pilot Randomized Controlled Trial

Abstract:Background: Older adults with subjective cognitive decline (SCD) are at increased risk not only for Alzheimer's disease, but for poor mental health, impaired sleep, and diminished quality of life (QOL), which in turn, contribute to further cognitive decline, highlighting the need for early intervention. Objective: In this randomized controlled trial, we assessed the effects of two 12-week relaxation programs, Kirtan Kriya Meditation (KK) and music listening (ML), on perceived stress, sleep, mood, and health-related QOL in older adults with SCD. Methods: Sixty community-dwelling older adults with SCD were randomized to a KK or ML program and asked to practice 12 minutes daily for 12 weeks, then at their discretion for the following 3 months. At baseline, 12 weeks, and 26 weeks, perceived stress, mood, psychological well-being, sleep quality, and health-related QOL were measured using well-validated instruments. Results: Fifty-three participants (88%) completed the 6-month study. Participants in both groups showed significant improvement at 12 weeks in psychological well-being and in multiple domains of mood and sleep quality (p's≤0.05). Relative to ML, those assigned to KK showed greater gains in perceived stress, mood, psychological well-being, and QOL-Mental Health (p's≤0.09). Observed gains were sustained or improved at 6 months, with both groups showing marked and significant improvement in all outcomes. Changes were unrelated to treatment expectancies. Conclusions: Findings suggest that practice of a simple meditation or ML program may improve stress, mood, well-being, sleep, and QOL in adults with SCD, with benefits sustained at 6 months and gains that were particularly pronounced in the KK group.

Abstract:Background: Psychological distress is common among patients with acute coronary syndrome (ACS) and has considerable adverse impacts on disease progression and health outcomes. Mindfulness-based intervention is a promising complementary approach to address patients' psychological needs and promote holistic well-being. Objective: This study aims to examine the effects of a social media–based mindfulness psycho-behavioral intervention (MCARE) on psychological distress, psychological stress, health-related quality of life (HRQoL), and cardiovascular risk factors among patients with ACS. Methods: This study was a 2-arm, parallel-group randomized controlled trial. We recruited 178 patients (mean age 58.7, SD 8.9 years; 122/178, 68.5% male) with ACS at 2 tertiary hospitals in Jinan, China. Participants were randomly assigned to the MCARE group (n=89) or control group (n=89). The 6-week intervention consisted of 1 face-to-face session (phase I) and 5 weekly WeChat (Tencent Holdings Ltd)–delivered sessions (phase II) on mindfulness training and health education and lifestyle modification. The primary outcomes were depression and anxiety. Secondary outcomes included psychological stress, HRQoL, and cardiovascular risk factors (ie, smoking status, physical activity, dietary behavior, BMI, blood pressure, blood lipids, and blood glucose). Outcomes were measured at baseline (T0), immediately after the intervention (T1), and 12 weeks after the commencement of the intervention (T2). Results: The MCARE group showed significantly greater reductions in depression (T1: β=–2.016, 95% CI –2.584 to –1.449, Cohen d =–1.28, P <.001; T2: β=–2.089, 95% CI –2.777 to –1.402, Cohen d =–1.12, P <.001) and anxiety (T1: β=–1.024, 95% CI –1.551 to –0.497, Cohen d =–0.83, P <.001; T2: β=–0.932, 95% CI –1.519 to –0.346, Cohen d =–0.70, P =.002). Significantly greater improvements were also observed in psychological stress (β=–1.186, 95% CI –1.678 to –0.694, Cohen d =–1.41, P <.001), physical HRQoL (β=0.088, 95% CI 0.008-0.167, Cohen d =0.72, P =.03), emotional HRQoL (β=0.294, 95% CI 0.169-0.419, Cohen d =0.81, P <.001), and general HRQoL (β=0.147, 95% CI 0.070-0.224, Cohen d =1.07) at T1, as well as dietary behavior (β=0.069, 95% CI 0.003-0.136, Cohen d =0.75, P =.04), physical activity level (β=177.542, 95% CI –39.073 to 316.011, Cohen d =0.51, P =.01), and systolic blood pressure (β=–3.326, 95% CI –5.928 to –0.725, Cohen d =–1.32, P =.01) at T2. The overall completion rate of the intervention (completing ≥5 sessions) was 76% (68/89). Positive responses to the questions of the acceptability questionnaire ranged from 93% (76/82) to 100% (82/82). Conclusions: The MCARE program generated favorable effects on psychological distress, psychological stress, HRQoL, and several aspects of cardiovascular risk factors in patients with ACS. This study provides clues for guiding clinical practice in the recognition and management of psychological distress and integrating the intervention into routine rehabilitation practice. Despite the availability of pharmacotherapies and/or psychotherapies, depression persists, even 5-10 years post-stroke, reflecting limited treatment responses and/or adherence to this conventional care. Mind-body interventions are commonly used among adults to ameliorate depressive symptoms, thus we investigated the feasibility of Tai Chi, alongside conventional care, to manage post-stroke depression. Purpose Describe changes in symptoms of depression, anxiety and stress, sleep, oxidative stress, and inflammatory markers post-intervention among community-dwelling stroke survivors with depressive symptoms. Methods In this feasibility study, a single-group pre-post intervention design was used. Due to COVID-19, participants attended a 1-hour Tai Chi exercise class three times per week for 8 weeks, instead of the planned 12-weeks. Symptoms of depression, anxiety and stress were assessed using standardized questionnaires (Center for Epidemiological Studies Depression scale, CESD; Generalized Anxiety Disorder Assessment; Perceived Stress Scale), objective sleep was assessed via a research-grade triaxial accelerometer (ActiGraph GT9X), and blood samples were taken to assess oxidative stress (plasma superoxide dismutase, SOD) and inflammatory markers (serum tumor necrosis factor-alpha, interleukin-6, interleukin-10). Pre-post intervention changes were assessed using paired t-tests. Results Community-dwelling stroke survivors (N = 11) on average 69.7 ± 9.3 years old, mainly retired (73%, n = 8), married men (55%, n = 6) with >13 years education (91%, n = 10), reporting depression symptoms (CESD = 17.3 ± 11.4) and 55% taking anti-depressant medications, enrolled. The majority of participants reported having an ischemic stroke (82%, n = 9) with hemiparesis (55%, n = 6), but were able to walk 15 feet without assistance (91%, n = 10). After the Tai Chi intervention, we observed significant reductions in symptoms of depression (-5.3 ± 5.9, p = 0.01), anxiety (-2.2 ± 2.4, p = 0.01) and stress (-4.6 ± 4.8, p = 0.01); along with better sleep efficiency (+1.8 ± 1.8, p = 0.01), less wakefulness after sleep onset (-9.3 ± 11.6, p = 0.04), and less time awake (-9.3 ± 11.6, p = 0.04). In addition, there was a 36% decrease in SOD activity (p = 0.02) indicative of a decreased oxidative environment post-intervention; though no significant changes in any of the inflammatory markers were found (all p-values >0.05). Conclusion Symptoms of depression, anxiety and stress were observed in these community-dwelling stroke survivors along with sub-optimal sleep. Among community-dwelling stroke survivors, Tai Chi exercise is a feasible intervention that can be used alongside conventional care to manage post-stroke depression and may also aid in reducing symptoms of anxiety and stress, and improve sleep. Further research is needed with rigorous study designs and larger samples, before widespread recommendations can be made.

PAM 50 assay and the three-gene model for identifying the major and clinically relevant molecular subtypes of breast cancer

Abstract:It has recently been proposed that a three-gene model (SCMGENE) that measures ESR1, ERBB2, and AURKA identifies the major breast cancer intrinsic subtypes and provides robust discrimination for clinical use in a manner very similar to a 50-gene subtype predictor (PAM50). However, the clinical relevance of both predictors was not fully explored, which is needed given that a *30 % discordance rate between these two predictors was observed. Using the same datasets and subtype calls provided by Haibe-Kains and colleagues, we compared the SCMGENE assignments and the research-based PAM50 assignments in terms of their ability to (1) predict patient outcome, (2) predict pathological complete response (pCR) after anthracycline/taxane-based chemotherapy, and (3) capture the main biological diversity displayed by all genes from a microarray. In terms of survival predictions, both assays provided independent prognostic information from each other and beyond the data provided by standard clinical–pathological variables; however, the amount of prognostic information was found to be significantly greater with the PAM50 assay than the SCMGENE assay. In terms of chemotherapy response, the PAM50 assay was the only assay to provide independent predictive information of pCR in multivariate models. Finally, compared to the SCMGENE predictor, the PAM50 assay explained a significantly greater amount of gene expression diversity as captured by the two main principal components of the breast cancer microarray data. Our results show that classification of the major and clinically relevant molecular subtypes of breast cancer are best captured using larger gene panels.
Modified-Mindfulness-Based Stress Reduction as a Treatment for Cognitive Recovery in Patients with Minor Stroke: a Randomized Controlled Pilot Study

Abstract:Background: Well-developed rehabilitation paradigms exist for post-stroke language and motor impairments. However, no such recovery program has been identified for commonly disabling cognitive deficits in patients following minor stroke. Mindfulness Based Stress Reduction (MBSR) is thought to engage the frontal lobes, improving concentration and attention, and therefore may be an effective option. Methods: We prospectively enrolled a cohort of patients with subacute minor stroke and randomized them to either an 8-week online modified-MBSR course or online traditional Stroke Support Group (SSG). All patients underwent a battery of cognitive tests and measures of patient reported outcomes (PROs) pre- and post-intervention. ANOVA was used to compare changes in scores over time across both groups, as well as a third group of control patients having received neither intervention (n=128). Results: A total of 30 patients were randomized (n=16 for m-MBSR; n=14 for SSG). The average age of the cohort was 65.9 years. Both groups scored similarly on assessments one-month post-stroke and demonstrated increased T-scores on cognitive tasks at the 3-month visit. However, the m-MBSR group showed moderately elevated levels of improvement, specifically in processing speed, executive, and global cognitive function. Level of engagement was not associated with better clinical scores, though was unexpectedly low for both groups. Conclusions: m-MBSR appears to modestly improve frontal lobe activity and demonstrates some success in increasing cognitive performance. However, further studies are needed to determine if it is more efficacious in the chronic stage of recovery when more patients are able to fully engage and actively participate.

Effect of Tai Chi for post-stroke mental disorders and sleep disorders: A protocol for systematic review and meta-analysis

Abstract:Background: Post-stroke mental disorders (PSMDs) and post-stroke sleep disorders (PSSDs) are very common in stroke patients. Recently, Tai Chi (TC) as a form of Chinese traditional mind-body exercise has been gradually applied to stroke rehabilitation although its efficacy for PSMD and PSSD varies across different studies. The aim of this study is to explore the therapeutic effect of TC training for PSMD and PSSD. Methods: This review will only include randomized controlled trials (RCTs). Search strategy will be performed in 3 English databases, 4 Chinese databases, and the WHO International Clinical Trials Registry Platform. All English or Chinese RCTs, published from inception to February 28, 2019, will be sought. Two reviewers will screen, select studies, extract data, and assess quality independently. Primary outcomes are clinical scales, mainly including "Hamilton depression scale," "Hamilton anxiety scale," and "Pittsburgh sleep quality index." The methodological quality including the risk of bias of the included studies will be evaluated using a modified assessment form, which is based on Cochrane assessment tool and Physiotherapy Evidence Database scale. Review Manager Software (Revman5.3) will be used for heterogeneity assessment, generating funnel-plots, data synthesis, subgroup analysis, and sensitivity analysis. We will use GRADE system to evaluate the quality of our evidence. Results: We will provide some more practical and targeted results investigating the effect of TC exercise for PSMD and PSSD in the current meta-analysis. Meanwhile, we will ascertain study progress of TC for PSMD and PSSD and find out defects or inadequacies of previous studies, so that future researchers could get beneficial guidance for more rigorous study. Conclusion: The stronger evidence about TC's rehabilitative effect and safety for PSMD and PSSD will be provided for clinicians and policymakers. Systematic review registration: PROSPERO CRD42018106608. Ethics and dissemination: We do not apply for formal ethical approval from ethics committee because all of the study data in our review will be obtained in an anonymous way. Findings of this study are projected to be disseminated through peer-review publications.

Abstract:Stroke is a common cerebrovascular disease with a high disability and case fatality rate, ranking as the second leading cause of death worldwide. With the acceleration of aging, the incidence rate of stroke is rising. [ 1 ] Poststroke depression (PSD) is one of the most common complications after stroke. About one-third of patients will have PSD in different periods after stroke, mainly manifested as depression, anxiety, sleep disorders, loss of interest, memory loss, difficulty concentrating, and other symptoms. [ 2 ] PSD hinders patients' rehabilitation process and functional recovery, diminishes their quality of life, and increases their disability and mortality rates. [ 3 ] Currently, the primary treatment method for PSD are antidepressants, but they have a slow onset. Some patients may not experience immediate mood improvement during the early stage of drugs, resulting in decreased treatment compliance, recurrent illness, and the delay of the best period of rehabilitation treatment. [ 4 ] Studies have confirmed that rational emotive behavior therapy (REBT) can improve the mood and sleep of PSD patients, but the treatment is mainly based on group therapy, which is difficult to achieve individualized precise treatment, and the cost is high, which is not conducive to the later family rehabilitation. In this study, based on the first-line antidepressant escitalopram, the method of combining traditional Chinese and Western medicine was used to combine REBT and traditional Baduanjin training to observe its effect on sleep, mood and serum BDNF, 5-HT and IL-6 levels in PSD patients. In order to develop a more individualized, reasonable, safe, and effective treatment plan for PSD patients, and provide clinical basis for the short-term and long-term prognosis of PSD patients.

Abstract:Background: As a common clinical symptom, insomnia has a high incidence of combined mental illness and it is also a risk factor for the development of depression, anxiety and suicide. As a new concept in the field of health in recent years, mindfulness therapy can improve insomnia, anxiety and depression, which is a new way to solve such diseases. Objective: This study aims to systematically evaluate the effects of mindfulness compared with conventional treatment on scores of the Hamilton Depression Scale (HAMD), Hamilton Anxiety Scale (HAMA), Self-Rating Depression Scale (SDS) and Self-Rating Anxiety Scale (SAS) in people with insomnia and anxiety-depressive symptoms. Methods: Articles published before October 2022 were searched from seven databases and included in randomized controlled trials (RCTs) to evaluate mindfulness therapy. The assessment tool of Cochrane bias risk was used to evaluate the methodological quality of the literature. The main outcome indicators were HAMD and HAMA scores, and the secondary outcome indicators were SDS and SAS scores. Results: Ten randomized controlled trials including 1,058 subjects were systematically evaluated and meta-analyzed in this study. In the main outcome indicators, there was a significant difference between mindfulness therapy and conventional treatment in reducing HAMD score (MD: −3.67, 95% CI: −5.22–2.11, p < 0.01) and HAMA score (MD: −3.23, 95% CI: −3.90–2.57, p < 0.01). In the secondary outcome indicators, mindfulness therapy also showed a significant difference in reducing SDS scores (MD: −6.49, 95% CI: −6.86–6.11, p < 0.01) and SAS scores (MD: −7.97, 95% CI: −9.68–6.27, p < 0.01) compared with conventional treatment. Conclusion: For the people with insomnia, anxiety and depression, the use of conventional treatment with the addition of 4–12 weeks of mindfulness treatment can significantly improve anxiety and depression symptoms of patients. This is a new diagnosis and treatment idea recommended for insomniacs with or without anxiety and depression symptoms. Due to the methodological defects in the included study and the limited sample size of this paper, more well-designed randomized controlled trials are needed for verification.

Abstract:Background: Between 40 and 50% of patients with Parkinson's disease (PD) experience anxiety and depression, associated with impaired physical function, high care dependency and mortality. Recently, the United States National Institutes of Health has urged the implementation of mindfulness practices in chronic illness care. Most research to date has examined the effects on chronically ill patients of complex interventions using a combination of mindfulness techniques. In PD patients, however, such complex modalities appear to hinder the technique mastery. Hence, the aim of this trial is to investigate the effects and underlying mechanism of individual mindfulness techniques among PD patients, as well as exploring participants' experience in using individual mindfulness techniques as a lifestyle intervention for stress and symptom management. Methods: We will conduct an assessor-blind three-arm randomized waitlist-controlled trial with a descriptive qualitative evaluation. Up to 168 PD patients will be recruited from community settings and out-patient clinics, and randomized to meditation, yoga, or usual care group. Meditation and yoga sessions of 90-minute are held weekly for 8 weeks. Primary outcomes include anxiety and depression. Secondary outcomes include PD-related motor and non-motor symptoms and quality-of-life; and level of mindfulness and biomarkers of stress and inflammatory responses will be measured as mediating variables. All outcome evaluations will be assessed at baseline, 8 weeks, and 24 weeks. Following the intention-to-treat principle, generalized estimating equation models and path analysis will be used to identify the treatment effects and the mediating mechanisms. A subsample of 30 participants from each intervention group will be invited for qualitative interviews. Discussion: The study would also generate important insights to enhance the patients' adaptation to debilitating disease. More specifically, symptom management and stress adaptation are highly prioritized healthcare agenda in managing PD. The research evidence will further inform the development of community-based, nurse-led compassionate care models for neurodegenerative conditions, which is complementary to existing health services.
