Abstract:The Multicenter Echo ASD Closure Model study introduces a novel echocardiographic screening model for atrial septal defect (ASD) closure, demonstrating superior predictive capabilities with an AUC of 0.941, potentially reducing the need for invasive procedures. This model utilizes ePASP, PV, TVE, and RA as key predictors for assessing correctable shunts in ASD patients. Background Atrial septal defect (ASD) is a prevalent congenital heart condition in adults, which finally leads to pulmonary hypertension and right heart failure if left untreated. Right heart catheterization (RHC), the current gold standard for determining ASD closure feasibility, is invasive. Thus, a noninvasive prescreening tool is urgently needed. Methods and Results In a multicenter, retrospective study, we assessed 924 ASD patients (2012–2022) to determine their suitability for ASD closure. Using LASSO regression, we identified predictors for a correctable shunt, enabling us to create the ASD model. The ASD model, comprising of estimated pulmonary artery systolic pressure (ePASP), peak velocity through the pulmonary valve (PV), peak E‐wave velocity through the tricuspid valve (TVE), and right atrial longitudinal dimension (RA) by echocardiography, was constructed and exhibited favorable discriminative capability with an area under the curve (AUC) of 0.941 (95% CI: 0.920–0.961) in the derivation group. The model also demonstrated good calibration and discriminative abilities in the validation cohort. When juxtaposed with the earlier congenital heart disease (CHD) model, the newly developed ASD model demonstrated superior predictive capabilities for correctable shunt, supported by the net reclassification index (NRI) [0.063 (95% CI: 0.001–0.127, p = 0.047)] and integrated discrimination improvement (IDI) [0.023 (95% CI: 0.011–0.036, p

Evaluation of Dynamic Morphology of Atrial Septal Defect by Real-Time Three-Dimensional Transesophageal Echocardiography

Abstract:Objective To determine whether real-time three-dimensional transesophageal echocardiography(RT-3D TEE) is an accurate non-invasive technique for defining the morphology of atrial septal defects(ASD). Methods In 20 patients with secundum ASD, mean age 42.2 years (7 male, 13 female) ,live three dimensional surgical views of ASD were acquired,qualitative morphology of ASD such as the shape, presence of fenestrations and the defect margins were noted during the cardiac cycle. The measurements obtained from 2-dimensional transesophageal echocardiography(2D-TEE) and RT-3D TEE were compared to those obtained from stretched balloon diameter(SBD) or surgery. Measurements of the size and area change of ASD were validated during the cardiac cycle by RT-3D TEE,stepwise multiple linear regression analysis was performed to test the correlation between the maximum change ratios of area and diameter of ASD and age, atrial septal length, ASD shunting velocity, the maximum diameter of ASD and right ventricular systolic pressure. Results ① The morphology of ASD was circle-like or elliptical in systole,irregular or elliptical in diastole. ②The area and diameter of ASD measured by RT-3D TEE was minimal in isovolumetric contraction phase, maximal in isovolumetric diastole phase. The correlation coefficient of maximum diameter measured by 3D with SBD was 0.962 , greater than that by 2D and SBD (0.820). ③The change ratio of area and maximum diameter of ASD acquired by RT-3D-TEE was 11.48%-71.12% and 2.80%-43.87% respectively,and the correlation coefficient of them was 0.921. Conclusions RT-3D TEE using live 3D-Zoom mode accurately displayed the varying morphology,dimensions and spatial relations of ASD. RT-3D TEE can offer visualization morphological changing of ASD in different periods of cardiac cycle,providing more information for percutanous catheter intervention and open heart surgery.
Quantitative Evaluation of Left and Right Ventricular Function by Serial Measuring Tei Index in Patients with Atrial Septal Defects Before and After Amplatzer Closure

Abstract:Objective This study attempted to quantitatively assess left and right ventricular function by serially measuring Tei indices in patients with atrial septal defects before and after Amplatzer closure.Methods Fifteen patients with secondary atrial septal defect undergoing transcatheter Amplatzer closure were prospectively investigated.Serial Tei indices were obtained by measuring the ratio of isovolumic time (isovolumic contraction time IVCT and isovolumic relaxation time IVRT) to the ejection time of the left ventricle (LV) and right ventricle (RV) before,during and 30 days after Amplatzer device implantation.Results The LV Tei index was significantly decreased in the patients of secondary atrial septal defects between 30 days after and before Amplatzer device implantation (0.30±0.07 vs 0.22±0.09,P<0.05).The RV Tei index has no obvious change in the patients of secondary atrial septal defects before,during and 30 days after Amplatzer device implantation.Conclusions The LV function has a tendency of improvement after Amplatzer device implantation in the patients with secondary atrial septal defects.The changes of volume load has no significant influence to the RV Tei index.
Non-fluoroscopic percutaneous transcatheter closure of atrial septal defects in children under transesophageal echocardiographic guidance

Abstract:Background This study sought to investigate the feasibility, safety and effectiveness of transcatheter closure of atrial septal defects (ASDs) under the guidance of transesophageal echocardiography (TEE) in children. Methods We reviewed the medical records of patients who underwent percutaneous ASD closure at our center from August 2016 to December 2017. For a total of 88 patients who were identified as having a single-hole defect and were undergoing percutaneous transcatheter ASD closure, a procedure completely guided by TEE was performed. There were 31 male patients and 57 female patients. The patients’ mean age was 60.09 ± 36.42 months (13–182 months), and their mean body weight was 20.16 ± 10.04 kg (9–77 kg). Patients were followed up by performing transthoracic echocardiography and obtaining chest X-rays and electrocardiograms. Results The transcatheter closure of ASDs was successful in all patients. The mean ASD size was 11.58 ± 5.31 mm (3–28 mm), and the mean size of the occlusion device was 16.07 ± 5.29 mm (6–36 mm). The mean procedural times were 13.33 ± 2.82 minutes (6–16 minutes). The mean hospitalization costs were 27,259.66 ± 2507.04 RMB (25,200.00–33,911.45 RMB). The mean postoperative hospital stay was 3.22 ± 0.53 days (3–5 days). Residual shunt, occlusion device shedding or displacement, and pericardial effusion were not observed during or after the operation. Conclusion Percutaneous transcatheter ASD closure completely guided by TEE is a feasible, safe, non-invasive and easy procedure.

Abstract:This study introduces the P‐echo model, a noninvasive tool for predicting shunt closure feasibility in adult congenital heart disease patients with post‐tricuspid valve shunt defects. Developed using retrospective analysis of 1474 VSD or PDA patients and key echocardiographic parameters, the P‐echo model boasts excellent predictive performance and high accuracy, potentially reducing the need for invasive procedures. It categorizes patients into low, medium, and high‐risk groups for shunt closure feasibility, providing critical guidance for clinical decision‐making and individualized treatment strategies. The P‐echo model offers a promising alternative to invasive right heart catheterization, highlighting its potential for wide clinical use. Background As right heart catheterization (RHC) is invasive and not always accessible, this study developed a noninvasive model (P‐echo) to predict shunt closure feasibility in adult congenital heart disease (ACHD) patients with post‐tricuspid valve shunt defects (PTD), specifically isolated ventricular septal defects (VSD) and patent ductus arteriosus (PDA). Methods A retrospective analysis of 1474 VSD or PDA patients from 2012 to 2022 was conducted. Echocardiographic parameters were assessed, and key variables identified via LASSO regression. The P‐echo model incorporated left to right velocity (LRv), right to left velocity (RLv), tricuspid regurgitation (TR), pulmonary artery diameter (PA), and RV/LV ratio. Its predictive performance was evaluated using ROC curve analysis. Results The P‐echo model demonstrated excellent predictive performance with AUC values of 0.975 (95% CI: 0.965–0.984) in the derivation set, 0.963 (95% CI: 0.937–0.989) in the validation set, and high accuracy in both PDA (AUC 0.975, 95% CI: 0.965–0.984) and VSD (AUC 0.958, 95% CI: 0.936–0.980) subsets. In the derivation set, the model categorized patients into low (9.1% closure rate), medium (70.9% closure rate), and high‐risk groups (99.7% closure rate) for shunt closure feasibility. Calibration plots confirmed the model's accuracy. Decision curve analysis showed a higher net benefit across a range of threshold probabilities, indicating the clinical usefulness of the model. Conclusions The P‐echo model is a robust and reliable tool for predicting the feasibility of shunt closure in patients with PTD, offering a noninvasive alternative to RHC. This model can guide clinical decision‐making and support individualized treatment strategies in ACHD management.

Abstract:Objective To apply a surgical approach of minimal invasive closure to atrial septal defect(ASD)without cardiopulmonary bypass.Methods Between Sept.2004 and Apr.2007,46 cases(male 22,female 24)of atrial septal defeet(ASD)were treated by minimal invasive closure without cardiopulmonary bypass.Their age ranged from 8mon tO 16yr(mean 4.7±3.9 yr)and weight ranged from 8.5 to 35 kg.(mean 1 5.2±6.5 kg).The incision was through the right 4th intercostals space after general anesthesia,and the character and size of ASD were confirmed via the transesophageal eehocardiography(TEE).The occluder device suitable to the ASD was placed through the right atrium and released after making sure it's firmly in place of the defect.Results Among them.45 cases of ostium secundum ASD were closed successfully and 1 failure.The size of ASD was from 8 mm to 25 mm,and the shortest distance of the rim of ASD was 7mm to the mitral valve,8 mm to the top of left atrium,6 mm to superior vena cava and 6 mm to inferior vena cava.There were no residual shunts and operative complications.Conclusions The advantages of minimally invasive closure of ASD without cardiopulmonary bypass are less traumas,no radiation,easy and security of manipulation.It is valuable for clinical application.
Transesophageal echocardiography and fluoroscopy for percutaneous closure of atrial septal defects: A comparative study.

Abstract:The aim of the study was to compare transesophageal echocardiography (TEE) and fluoroscopy for percutaneous atrial septal defect (ASD) closure. This was a retrospective analysis of children who underwent percutaneous ASD closure. The procedure was guided by TEE without fluoroscopy in 130 patients (TEE group) and by fluoroscopy in 163 patients (fluoroscopy group). Baseline demographic/ clinical characteristics were recorded. Patients were followed until hospital discharge. Outcomes were procedure duration, peri/ postoperative complications, hospital stay, and costs. The TEE and fluoroscopy groups showed no significant differences in age (71.7 +/- 40.7 vs 62.5 +/- 38.8 months), male/female ratio (54/76 vs 66/97), weight (22.0 +/- 12.0 vs 20.1 +/- 9.0 kg), ASD diameter (9.9 +/- 4.2 vs 9.3 +/- 3.9 cm), distances to the superior vena cava (13.4 +/- 4.6 vs 13.3 +/- 4.2 cm), inferior vena cava (13.4 +/- 4.3 vs 13.9 +/- 4.1cm) and atrial septal roof (12.1 +/- 4.0 vs 12.3 +/- 3.2 cm), or atrial septal size (38.2 +/- 6.2 vs 39.4 +/- 26.6 cm); distance to the mitral valve was greater in the TEE group (13.2 +/- 4.4 vs 11.3 +/- 3.9cm; P<.001). The TEE and fluoroscopy groups showed no significant differences in occlusion device size (14.3 +/- 4.6 vs 13.8 +/- 4.0cm) or sheath size (8.7 +/- 1.8 vs 8.7 +/- 0.9 cm), but procedure duration was shorter in the TEE group (21.5 +/- 14.6 vs 28.6 +/- 10.9 minutes; P<. 001). Postoperative fever (>38 degrees C) occurred less frequently in the TEE group than in the fluoroscopy group (0.8% vs 9.2%; P<. 001); there were no significant differences for the other complications. No patient had postoperative residual shunt, occlusion device shedding/displacement, or pericardial effusion. The TEE group had longer hospital stay (3.2 +/- 0.6 vs 2.9 +/- 0.6 days; P<. 001) and higher procedure cost (29,687 +/- 4218 vs 28,530 +/- 1668 CNY (China Yuan); P=.002) than the fluoroscopy group. TEE-guided percutaneous ASD closure can be used as an alternative to fluoroscopy-guided procedures and avoids the use of radiation or contrast agents.
Model for screening adult congenital heart disease surgery eligibility with echocardiography parameters

Abstract:Objectives This study aimed to screen for the eligibility of correction in cases of adult congenital heart disease (CHD). Pulmonary to systemic flow ratios (Qp/Qs) > 1.5 and pulmonary to systemic vascular resistance ratios (Rp/Rs) < 1/3, acquired by right heart catheterization (RHC), are two essential parameters. Nonetheless, performing RHC at every follow-up is impractical and even harmful. Thus, it is important to establish a model to predict Qp/Qs and Rp/Rs status before a RHC confirmation, using echocardiography parameters. Methods A total of 1,785 patients with adult CHD were enrolled and randomly assigned to the derivation or validation groups. Echocardiogram parameters of the 974 patients in the derivation group were considered candidate predictors for surgery eligibility (Qp/Qs > 1.5 and Rp/Rs < 1/3). Binary logistic regression analyses were performed to identify the independent predictors and establish a scoring system. The scoring system was further examined in the validation group using a receiver operating characteristic (ROC) analysis. Results Estimated pulmonary artery systolic pressure, velocity through the pulmonary valve, and diameters of the left and right atria were identified as independent predictors. The area under the ROC curve of the predictive value in the validation group and its pre- and post-tricuspid valve malformation subgroups were 0.87 (95% confidence interval [CI]: 0.84−0.90, P < 0.01), 0.86 (95% CI: 0.82−0.91, P < 0.01), and 0.85 (95% CI: 0.79−0.90, P < 0.01), respectively. Conclusions This scoring system could augment flexibility and convenience for pre-screening CHD patients' eligibility for surgery, before RHC.

Artificial intelligence-enabled 8-lead ECG detection of atrial septal defect among adults: a novel diagnostic tool

Abstract:Background: Patients with atrial septal defect (ASD) exhibit distinctive electrocardiogram (ECG) patterns. However, ASD cannot be diagnosed solely based on these differences. Artificial intelligence (AI) has been widely used for specifically diagnosing cardiovascular diseases other than arrhythmia. Our study aimed to develop an artificial intelligence-enabled 8-lead ECG to detect ASD among adults. Method: In this study, our AI model was trained and validated using 526 ECGs from patients with ASD and 2,124 ECGs from a control group with a normal cardiac structure in our hospital. External testing was conducted at Wuhan Central Hospital, involving 50 ECGs from the ASD group and 46 ECGs from the normal group. The model was based on a convolutional neural network (CNN) with a residual network to classify 8-lead ECG data into either the ASD or normal group. We employed a 10-fold cross-validation approach. Results: Statistically significant differences ( p < 0.05) were observed in the cited ECG features between the ASD and normal groups. Our AI model performed well in identifying ECGs in both the ASD group [accuracy of 0.97, precision of 0.90, recall of 0.97, specificity of 0.97, F1 score of 0.93, and area under the curve (AUC) of 0.99] and the normal group within the training and validation datasets from our hospital. Furthermore, these corresponding indices performed impressively in the external test data set with the accuracy of 0.82, precision of 0.90, recall of 0.74, specificity of 0.91, F1 score of 0.81 and the AUC of 0.87. And the series of experiments of subgroups to discuss specific clinic situations associated to this issue was remarkable as well. Conclusion: An ECG-based detection of ASD using an artificial intelligence algorithm can be achieved with high diagnostic performance, and it shows great clinical promise. Our research on AI-enabled 8-lead ECG detection of ASD in adults is expected to provide robust references for early detection of ASD, healthy pregnancies, and related decision-making. A lower number of leads is also more favorable for the application of portable devices, which it is expected that this technology will bring significant economic and societal benefits.

Abstract:BACKGROUND: The assessment of interatrial septum (IAS) requires a standardized, systematic approach, including two-dimensional transthoracic echocardiography (2D TTE), 2D transesophageal echocardiography (2D TEE), and three-dimensional (3D) TEE. Although 2D TEE has been widely used for the preoperative assessment of atrial septal defect (ASD), its ability to provide reliable information is often limited due to the structural characteristics of IAS. The introduction of 3D TEE provides a unique "en face" view of IAS, which allows the visualization and accurate measurements of diameters, area, and rims of ASD. Hence, appropriate ASD imaging information is particularly important in successful transcatheter closure.METHODS: In this retrospective study, 2D TTE/TEE, and 3D TEE were performed before ASD closure, with 2D minimal and maximal diameters, areas, and residual rims being recorded. Adequate 3D TEE imaging data sets were collected and then analyzed. ASD related parameters were compared using different echocardiography. Patients who underwent ASD closure completed a clinical follow-up.RESULTS: The mean defect maximal diameter and aperture area by 3D TEE was significantly larger than that of the corresponding 2D TEE (P<0.05). There was no statistical difference in the minimal and maximal diameter or area by TEE for circular-shaped ASDs. For oval ASDs, mean minimal diameter on 2D TEE was larger than that on 3D TEE. The mean maximal diameter measured using 2D TEE was smaller than the 3D TEE measurement (16.0±7.1 <i>vs.</i> 19.8±8.6; P<0.05). For complex-shaped defects, there were statistical differences in minimal and maximal diameter between TEEs. Furthermore, 2D and 3D TEE had a longer superior vena cava (SVC) residual rim than did 2D TTE (P<0.05). The 3D TEE residual rims of the inferior vena cava (IVC) was significantly larger than the corresponding 2D TEE. There was a very strong correlation between the residual rim measurements using 3D and 2D TEE. However, the limits of agreement between 2D and real-time 3D TEE measurements were more apparent in the IVC rim group than in the other groups.CONCLUSIONS: Our study confirms the value of 3D TEE in assessing ASD shape and size reported by previous studies, and is also the first to accurately and systematically characterize ASD residual rim in complex ASDs.

Abstract:Objective The current study was the first in China to investigate the value of single-beat real-time three-dimensional echocardiography (sRt-3DE) in assessing the shape and function of right ventricle in Patients with Atrial Septal Defect (ASD). Methods 33 healthy volunteers (11 male, mean age (41.15±17.49 years)) and 18 ASD patients (six male, mean age (34.67±19.50 years)) were enrolled. All the participants received routine echocardiography. sRt-3DE by SIEMENS SC2000 was performed in all to evaluate parameters concerning morphology and systolic function of participants’ right ventricle. All the parameters were indexed by body surface area. Further correlative analysis was calculated between right ventricular parameters and right heart pressures by cardiac catheter as well as right ventricular parameters and body surface area-indexed maximum diameter of ASD (IASD). 21 participants (15 controls and six ASD patients) were randomly selected to explore the inter-observer agreement of this novel technique. Results Rt-3DE was successful in all participants, even in patients with large right ventricle volume up to 250 ml. Statistic analysis showed the technology has good inter-observer correlation, (EDVRV: ICC 0.891 (95% CI 0.731 to 0.956); ESVRV: ICC 0.737 (95% CI 0.731 to 0.893)) Indexed right ventricular end diastolic volume (IEDVRV) and end systolic volume (IESVRV), indexed right ventricular systolic volume (ISVRV) and right ventricular ejection fraction (RVEF) were greater in the ASD group than in controls (p<0.001, p<0.01, p<0.001, p<0.05). IEDVRV, IESVRV and ISVRV had positive relations with pulmonary artery pressures, especially pulmonary artery diastolic pressure measured by cardiac catheter (r=0.61, r=0.79, r=0.83, all p<0.05) and IASD (r=0.57, p<0.05; r=0.6, p<0.01; r=0.55, p<0.05 respectively). At the same time, they had negative relations with LVEF (r=−0.47, r=−0.5, r=−0.52; all p<0.05). Conclusions Left-to-right shunt in ASD patients may lead to an aggressive increase of pulmonary artery pressure and right ventricular volume. The enlargement of right ventricle increases right ventricular contraction and affects the morphology and function of left ventricle. sRt-3DE is a unique new modality to precisely and stably evaluate right ventricular changes in ASD patients.

Abstract:OBJECTIVE:To evaluate right ventricular (RV) global and regional systolic function in patients with atrial septal defect (ASD) before and after percutaneous closure using real time three-dimensional echocardiography (RT3DE).METHODS:RT3DE was performed in 81 patients with ASD within 24 hours before and after percutaneous closure to obtain RV global and regional ejection fraction (EF) in three compartments (inflow, body, and outflow). RV fractional area change (FAC), tricuspid annular plane systolic excursion (TAPSE), peak tricuspid systolic velocity (S), and pulmonary vascular resistance (PVR) were recorded. Forty matched normal adults were included as controls.RESULTS:When compared with controls, RV global and regional EF were decreased in preclosure patients (P < 0.001). FAC was lower while TAPSE and S were higher in preclosure patients than in controls (P < 0.05). After closure, RV systolic function parameters were all reduced (P < 0.001). Regional EF in the body compartment was the lowest among the three compartments in ASD patients (P < 0.05). Procedural percentage changes of RV global EF and regional EF in the inflow compartment were lower than those of two-dimensional systolic function parameters (P < 0.05). RV global and regional EF in the inflow compartment were negatively correlated with PVR in patients after closure (r = -0.601, -0.543, P < 0.001).CONCLUSIONS:RV global and regional systolic functions are impaired in open and closed ASD. RT3DE-derived systolic function parameters are negatively correlated with RV after load. RT3DE has potential value in the evaluation of RV systolic function in patients with ASD.

Abstract:Objective To assess the application value of echocardiography in evaluating minimally invasive Amplatzer closure of great secundum atrial septal defect (ASD) patients. Methods Two hundred and fifty two patients (male, 116) were involved in this study. The defect diameter was 34～38 mm in 230 cases, 22cases were atrial septal aneurysm complicating porosity defects. Each patient was positioned a Amplatzer device to close the defect through parasternal minimized surgical incisions. TEE were used to monitoring the whole procedure, to guide the device placing and evaluate the curative effect instantly after operation. Results All patients were successfully positioned closure devices by echocardiography monitoring. Only 34 cases had micro to small amounts residual shunt instantly after operation,11 cases still had small amounts residual shunt at the time of 48 hours after the operation. In the one year follow-up,6 cases had residual shunt, but the diameters of right chamber heart were significantly reduced than preoperative,and the pulmonary artery systolic pressure were also reduced. Conclusions Echocardiography possesses important role of preoperative patients screening,monitoring the devices deploying intraoperation, and evaluating the curative effect postoperative. Atrial Septal Defect Detection in Children Based on Ultrasound Video Using Multiple Instances Learning

Abstract:Purpose: Congenital heart defect (CHD) is the most common birth defect. Thoracic echocardiography (TTE) can provide sufficient cardiac structure information, evaluate hemodynamics and cardiac function, and is an effective method for atrial septal defect (ASD) examination. This paper aims to study a deep learning method based on cardiac ultrasound video to assist in ASD diagnosis. Materials and methods: We select two standard views of the atrial septum (subAS) and low parasternal four-compartment view (LPS4C) as the two views to identify ASD. We enlist data from 300 children patients as part of a double-blind experiment for five-fold cross-validation to verify the performance of our model. In addition, data from 30 children patients (15 positives and 15 negatives) are collected for clinician testing and compared to our model test results (these 30 samples do not participate in model training). We propose an echocardiography video-based atrial septal defect diagnosis system. In our model, we present a block random selection, maximal agreement decision and frame sampling strategy for training and testing respectively, resNet18 and r3D networks are used to extract the frame features and aggregate them to build a rich video-level representation. Results: We validate our model using our private dataset by five-cross validation. For ASD detection, we achieve 89.33 AUC, 84.95 accuracy, 85.70 sensitivity, 81.51 specificity and 81.99 F1 score. Conclusion: The proposed model is multiple instances learning-based deep learning model for video atrial septal defect detection which effectively improves ASD detection accuracy when compared to the performances of previous networks and clinical doctors.

Abstract:Background: Atrial septal defect (ASD) is a congenital heart disease that often presents without symptoms or murmurs. If left untreated, children with ASD can develop comorbidities in adulthood. In Japan, school electrocardiography (ECG) screening has been implemented for all 1st, 7th, and 10th graders. However, the impact of this program in detecting children with ASD is unknown. Methods: This is a retrospective study that analyzed consecutive patients with ASD who underwent catheterization for surgical or catheter closure at ≤18 years of age during 2009–2019 at a tertiary referral center in Japan. Results: Of the overall 116 patients with ASD (median age: 3.0 years of age at diagnosis and 8.9 years at catheterization), 43 (37%) were prompted by the ECG screening (Screening group), while the remaining 73 (63%) were by other findings (Non-screening group). Of the 49 patients diagnosed at ≥6 years of age, 43 (88%) were prompted by the ECG screening, with the 3 corresponding peaks of the number of patients at diagnosis. Compared with the non-screening group, the screening group exhibited similar levels of hemodynamic parameters but had a lower proportion of audible heart murmur, which were mainly prompted by the health care and health checkups in infancy or preschool period. Patients positive for a composite parameter (rsR' type of iRBBB, inverted T in V4, or ST depression in the aVF lead) accounted for 79% of the screening group at catheterization, each of which was correlated with hemodynamic parameters in the overall patients. Conclusions: The present study shows that school ECG screening detects otherwise unrecognized ASD, which prompted the diagnosis of the majority of patients at school age and >one-third of overall patients in Japan. These findings suggest that ECG screening program could be an effective strategy for detecting hemodynamically significant ASD in students, who are asymptomatic and murmurless.

Abstract:【Abstract】 Aim: To investigate the safety of interventional therapy in patients with secondary atrial septal defect (atrial septal defect, ASD) with complete aortic rim deficiency and explore the predictors of right atrial non-reverse remodeling. Methods: 1011 patients with ASD who underwent transcatheter closure in the Department of Cardiology, Zhongshan Hospital affiliated to Fudan University from June 2017 to June 2022 were enrolled in the study. They were divided into a complete aortic rim deficiency group and without absent aortic rim group. Furthermore, patients who had an enlarged right atrial in the absent aortic rim group were divided into two sub-groups according to whether their right atrial reversed remodeling post-procedure. Multivariate logistic regression was used to determine the predictors of right atrial reversed remodeling. Results: During the 1-year follow-up, no major operative complications occurred in all patients with the absence of an aortic rim and a normal edge. After the operation, the right heart remodeling was significantly reversed, multivariate logistic regression analysis was performed and found that preoperative without coronary heart disease, lower plasma creatinine level, and larger RA and RV dimension were predictive factors for the reverse of right atrial remodeling after treatment. Conclusion: Transcatheter closure of ASD with complete aortic rim deficiency is safe and feasible. The patients without coronary heart disease, the lower the creatinine value and the less tricuspid regurgitation before an operation, the more improvement of right atrial remodeling after the operation.
Open heart surgery or echocardiographic transthoracic or percutaneous closure in secundum atrial septal defect: a developing approach in one Chinese hospital

Abstract:Background: To study the clinical manifestations and advantages of open-heart surgery and echocardiographic transthoracic or percutaneous closure with secundum atrial septal defect (ASD). The surgeon's learning curve was also analyzed. Methods: In all, 115 consecutive patients with ASD from May 2013 to May 2019 were enrolled. According to the operative procedure, patients were divided into three groups: group one (open repair group) (n = 24), where patients underwent ASD repair (ASDR) under cardiopulmonary bypass (CPB); group two (closed surgical device closure group) (n = 69), where patients (six patients ≤1 y and sixteen ≤10 kg) underwent transthoracic ASD occlusion under transesophageal echocardiographic (TEE) guidance; and group three (transcatheter occlusion group) (n = 22), where patients underwent percutaneous ASD occlusion under echocardiography. The clinical features and results of each group were analyzed. All patients were telephonically followed-up after 3 months. Results: All the three methods treating ASD were successfully performed in our hospital. It was also a typical developing history of congenital heart disease (CHD) surgery in China. One patient in the group two was transferred to emergency surgery for occluder retrieval and CPB-ASDR. Eight patients experienced failed transthoracic or percutaneous occlusion, two of whom underwent unsuccessful percutaneous closure at another hospital. Two patients each in the groups two and three were intraoperatively converted to CPB-ASDR. Two patient in the group three was converted to transthoracic occlusion surgery. All patients were discharged without any residual shunt. The three-month follow-up also did not show any residual shunt and occluder displacement. Conclusion: In low-weight, infants, or huge ASDs with suitable rim for device occlusion, transthoracic ASD closure was successfully performed. Based on knowledge of ASD anatomy and skilled transthoracic occlusion of ASD, surgeons can perform percutaneous occlusion of ASD under echocardiographic guidance.

Cardiovascular outcomes in adult patients with atrial septal defect: a nationwide population-based study

Abstract:Background The beneficial effects of atrial septal defect (ASD) closure on cardiovascular events and mortality in adults remain undetermined. Therefore, this study aimed to investigate the long-term effects of different ASD closure methods on cardiovascular events in adults. Methods A retrospective analysis was conducted using data obtained from the Korean National Health Insurance Service from 2002 to 2020, focusing on patients aged ≥20 years diagnosed with ASD based on the codes provided by the Tenth revision of International Classification of Diseases codes between 2004 and 2015. Participants were categorized into the observation, device closure, and surgery groups. Propensity score matching (PSM) of variables such as sex, age, comorbidities, and medications was employed at a 2:1:1 ratio to mitigate imbalances among the groups. The Cox proportional hazards model was utilized to compare the occurrence of major adverse cardiovascular events (MACE), including stroke, myocardial infarction (MI), coronary revascularization, and all-cause death, and each individual component among the three groups. Results In total, 20,643 patients with ASD were included in this study. After PSM, there were 6,636 in the observation group and 3,318 each in the device closure and surgery group. Over a 5-year follow-up period, the adjusted hazard ratios for MACE were significantly lower in the surgery (0.72; 95% CI: 0.66-0.79) and device closure groups (0.85; 95% CI: 0.78-0.92) than in the observation group. Beneficial effects on stroke and all-cause mortality were observed in both intervention groups. Additionally, a beneficial effect on coronary revascularization was observed in the surgery group, whereas the impact on MI was not significantly different between the groups. Conclusions ASD closure, whether by surgery or using a device, is associated with a decreased incidence of cardiovascular outcomes in adults. The benefits on cardiovascular outcomes vary with the type of closure method, underscoring the need for a tailored approach to manage ASD in adults.
