Abstract:Though U-Net has achieved tremendous success in medical image segmentation tasks, it lacks the ability to explicitly model long-range dependencies. Therefore, Vision Transformers have emerged as alternative segmentation structures recently, for their innate ability of capturing long-range correlations through Self-Attention (SA). However, Transformers usually rely on large-scale pre-training and have high computational complexity. Furthermore, SA can only model self-affinities within a single sample, ignoring the potential correlations of the overall dataset. To address these problems, we propose a novel Transformer module named Mixed Transformer Module (MTM) for simultaneous inter- and intra- affinities learning. MTM first calculates self-affinities efficiently through our well-designed Local-Global Gaussian-Weighted Self-Attention (LGG-SA). Then, it mines inter-connections between data samples through External Attention (EA). By using MTM, we construct a U-shaped model named Mixed Transformer U-Net (MT-UNet) for accurate medical image segmentation. We test our method on two different public datasets, and the experimental results show that the proposed method achieves better performance over other state-of-the-art methods. Abstract:Introduction: Ultrasound imaging has become a crucial tool in medical diagnostics, offering real-time visualization of internal organs and tissues. However, challenges such as low contrast, high noise levels, and variability in image quality hinder accurate interpretation. To enhance the diagnostic accuracy and support treatment decisions, precise segmentation of organs and lesions in ultrasound image is essential. Recently, several deep learning methods, including convolutional neural networks (CNNs) and Transformers, have reached significant milestones in medical image segmentation. Nonetheless, there remains a pressing need for methods capable of seamlessly integrating global context with local fine-grained information, particularly in addressing the unique challenges posed by ultrasound images. Methods: In this paper, to address these issues, we propose DDTransUNet, a hybrid network combining Transformer and CNN, with a dual-branch encoder and dual attention mechanism for ultrasound image segmentation. DDTransUNet adopts a Swin Transformer branch and a CNN branch to extract global context and local fine-grained information. The dual attention comprising Global Spatial Attention (GSA) and Global Channel Attention (GCA) modules to capture long-range visual dependencies. A novel Cross Attention Fusion (CAF) module effectively fuses feature maps from both branches using cross-attention. Results: Experiments on three ultrasound image datasets demonstrate that DDTransUNet outperforms previous methods. In the TN3K dataset, DDTransUNet achieves IoU, Dice, HD95 and ACC metrics of 73.82%, 82.31%, 16.98 mm, and 96.94%, respectively. In the BUS-BRA dataset, DDTransUNet achieves 80.75%, 88.23%, 8.12 mm, and 98.00%. In the CAMUS dataset, DDTransUNet achieves 82.51%, 90.33%, 2.82 mm, and 96.87%. Discussion: These results indicate that our method can provide valuable diagnostic assistance to clinical practitioners.

Abstract:Accurate and automatic segmentation of medical images is a key step in clinical diagnosis and analysis. Currently, the successful application of Transformers' model in the field of computer vision, researchers have begun to gradually explore the application of Transformers in medical segmentation of images, especially in combination with convolutional neural networks with coding-decoding structure, which have achieved remarkable results in the field of medical segmentation. However, most studies have combined Transformers with CNNs at a single scale or processed only the highest-level semantic feature information, ignoring the rich location information in the lower-level semantic feature information. At the same time, for problems such as blurred structural boundaries and heterogeneous textures in images, most existing methods usually simply connect contour information to capture the boundaries of the target. However, these methods cannot capture the precise outline of the target and ignore the potential relationship between the boundary and the region. In this paper, we propose the TGDAUNet, which consists of a dual-branch backbone network of CNNs and Transformers and a parallel attention mechanism, to achieve accurate segmentation of lesions in medical images. Firstly, high-level semantic feature information of the CNN backbone branches is fused at multiple scales, and the high-level and low-level feature information complement each other's location and spatial information. We further use the polarised self-attentive (PSA) module to reduce the impact of redundant information caused by multiple scales, to better couple with the feature information extracted from the Transformers backbone branch, and to establish global contextual long-range dependencies at multiple scales. In addition, we have designed the Reverse Graph-reasoned Fusion (RGF) module and the Feature Aggregation (FA) module to jointly guide the global context. The FA module aggregates high-level semantic feature information to generate an original global predictive segmentation map. The RGF module captures non-significant features of the boundaries in the original or secondary global prediction segmentation graph through a reverse attention mechanism, establishing a graph reasoning module to explore the potential semantic relationships between boundaries and regions, further refining the target boundaries. Finally, to validate the effectiveness of our proposed method, we compare our proposed method with the current popular methods in the CVC-ClinicDB, Kvasir-SEG, ETIS, CVC-ColonDB, CVC-300,datasets as well as the skin cancer segmentation datasets ISIC-2016 and ISIC-2017. Abstract:In healthcare, medical image segmentation is crucial for accurate disease diagnosis and the development of effective treatment strategies. Early detection can significantly aid in managing diseases and potentially prevent their progression. Machine learning, particularly deep convolutional neural networks, has emerged as a promising approach to addressing segmentation challenges. Traditional methods like U-Net use encoding blocks for local representation modeling and decoding blocks to uncover semantic relationships. However, these models often struggle with multi-scale objects exhibiting significant variations in texture and shape, and they frequently fail to capture long-range dependencies in the input data. Transformers designed for sequence-to-sequence predictions have been proposed as alternatives, utilizing global self-attention mechanisms. Yet, they can sometimes lack precise localization due to insufficient granular details. To overcome these limitations, we introduce TransDAE: a novel approach that reimagines the self-attention mechanism to include both spatial and channel-wise associations across the entire feature space, while maintaining computational efficiency. Additionally, TransDAE enhances the skip connection pathway with an inter-scale interaction module, promoting feature reuse and improving localization accuracy. Remarkably, TransDAE outperforms existing state-of-the-art methods on the Synaps multi-organ dataset, even without relying on pre-trained weights.

Abstract:Automatic segmentation of medical images provides a reliable scientific basis for disease diagnosis and analysis. Notably, most existing methods that combine the strengths of convolutional neural networks (CNNs) and Transformers have made significant progress. However, there are some limitations in the current integration of CNN and Transformer technology in two key aspects. Firstly, most methods either overlook or fail to fully incorporate the complementary nature between local and global features. Secondly, the significance of integrating the multi-scale encoder features from the dual-branch network to enhance the decoding features is often disregarded in methods that combine CNN and Transformer. To address this issue, we present a groundbreaking dual-branch cross-attention fusion network (DCFNet), which efficiently combines the power of Swin Transformer and CNN to generate complementary global and local features. We then designed the Feature Cross-Fusion (FCF) module to efficiently fuse local and global features. In the FCF, the utilization of the Channel-wise Cross-fusion Transformer (CCT) serves the purpose of aggregating multi-scale features, and the Feature Fusion Module (FFM) is employed to effectively aggregate dual-branch prominent feature regions from the spatial perspective. Furthermore, within the decoding phase of the dual-branch network, our proposed Channel Attention Block (CAB) aims to emphasize the significance of the channel features between the up-sampled features and the features generated by the FCF module to enhance the details of the decoding. Experimental results demonstrate that DCFNet exhibits enhanced accuracy in segmentation performance. Compared to other state-of-the-art (SOTA) methods, our segmentation framework exhibits a superior level of competitiveness. DCFNet’s accurate segmentation of medical images can greatly assist medical professionals in making crucial diagnoses of lesion areas in advance.

TEC-Net: Vision Transformer Embrace Convolutional Neural Networks for Medical Image Segmentation

Abstract:The hybrid architecture of convolution neural networks (CNN) and Transformer has been the most popular method for medical image segmentation. However, the existing networks based on the hybrid architecture suffer from two problems. First, although the CNN branch can capture image local features by using convolution operation, the vanilla convolution is unable to achieve adaptive extraction of image features. Second, although the Transformer branch can model the global information of images, the conventional self-attention only focuses on the spatial self-attention of images and ignores the channel and cross-dimensional self-attention leading to low segmentation accuracy for medical images with complex backgrounds. To solve these problems, we propose vision Transformer embrace convolutional neural networks for medical image segmentation (TEC-Net). Our network has two advantages. First, dynamic deformable convolution (DDConv) is designed in the CNN branch, which not only overcomes the difficulty of adaptive feature extraction using fixed-size convolution kernels, but also solves the defect that different inputs share the same convolution kernel parameters, effectively improving the feature expression ability of CNN branch. Second, in the Transformer branch, a (shifted)-window adaptive complementary attention module ((S)W-ACAM) and compact convolutional projection are designed to enable the network to fully learn the cross-dimensional long-range dependency of medical images with few parameters and calculations. Experimental results show that the proposed TEC-Net provides better medical image segmentation results than SOTA methods including CNN and Transformer networks. In addition, our TEC-Net requires fewer parameters and computational costs and does not rely on pre-training. DA-TransUNet: Integrating Spatial and Channel Dual Attention with Transformer U-Net for Medical Image Segmentation

Abstract:Accurate medical image segmentation is critical for disease quantification and treatment evaluation. While traditional Unet architectures and their transformer-integrated variants excel in automated segmentation tasks. However, they lack the ability to harness the intrinsic position and channel features of image. Existing models also struggle with parameter efficiency and computational complexity, often due to the extensive use of Transformers. To address these issues, this study proposes a novel deep medical image segmentation framework, called DA-TransUNet, aiming to integrate the Transformer and dual attention block(DA-Block) into the traditional U-shaped architecture. Unlike earlier transformer-based U-net models, DA-TransUNet utilizes Transformers and DA-Block to integrate not only global and local features, but also image-specific positional and channel features, improving the performance of medical image segmentation. By incorporating a DA-Block at the embedding layer and within each skip connection layer, we substantially enhance feature extraction capabilities and improve the efficiency of the encoder-decoder structure. DA-TransUNet demonstrates superior performance in medical image segmentation tasks, consistently outperforming state-of-the-art techniques across multiple datasets. In summary, DA-TransUNet offers a significant advancement in medical image segmentation, providing an effective and powerful alternative to existing techniques. Our architecture stands out for its ability to improve segmentation accuracy, thereby advancing the field of automated medical image diagnostics. UTNet: A Hybrid Transformer Architecture for Medical Image Segmentation

Abstract:Transformer architecture has emerged to be successful in a number of natural language processing tasks. However, its applications to medical vision remain largely unexplored. In this study, we present UTNet, a simple yet powerful hybrid Transformer architecture that integrates self-attention into a convolutional neural network for enhancing medical image segmentation. UTNet applies self-attention modules in both encoder and decoder for capturing long-range dependency at different scales with minimal overhead. To this end, we propose an efficient self-attention mechanism along with relative position encoding that reduces the complexity of self-attention operation significantly from O(n2)\documentclass[12pt]{minimal}\usepackage{amsmath}\usepackage{wasysym}\usepackage{amsfonts}\usepackage{amssymb}\usepackage{amsbsy}\usepackage{mathrsfs}\usepackage{upgreek}\setlength{\oddsidemargin}{-69pt}\begin{document}$$O(n^2)$$\end{document} to approximate O(n). A new self-attention decoder is also proposed to recover fine-grained details from the skipped connections in the encoder. Our approach addresses the dilemma that Transformer requires huge amounts of data to learn vision inductive bias. Our hybrid layer design allows the initialization of Transformer into convolutional networks without a need of pre-training. We have evaluated UTNet on the multi-label, multi-vendor cardiac magnetic resonance imaging cohort. UTNet demonstrates superior segmentation performance and robustness against the state-of-the-art approaches, holding the promise to generalize well on other medical image segmentations.

Abstract:To address the problems of under-segmentation and over-segmentation of small organs in medical image segmentation. We present a novel medical image segmentation network model with Depth Separable Gating Transformer and a Three-branch Attention module (DSGA-Net). Firstly, the model adds a Depth Separable Gated Visual Transformer (DSG-ViT) module into its Encoder to enhance (i) the contextual links among global, local, and channels and (ii) the sensitivity to location information. Secondly, a Mixed Three-branch Attention (MTA) module is proposed to increase the number of features in the up-sampling process. Meanwhile, the loss of feature information is reduced when restoring the feature image to the original image size. By validating Synapse, BraTs2020, and ACDC public datasets, the Dice Similarity Coefficient (DSC) of the results of DSGA-Net reached 81.24%,85.82%, and 91.34%, respectively. Moreover, the Hausdorff Score (HD) decreased to 20.91% and 5.27% on the Synapse and BraTs2020. There are 10.78% and 0.69% decreases compared to the Baseline TransUNet. The experimental results indicate that DSGA-Net achieves better segmentation than most advanced methods.

Abstract:Transformer-based models have been widely demonstrated to be successful in computer vision tasks by modelling long-range dependencies and capturing global representations. However, they are often dominated by features of large patterns leading to the loss of local details (e.g., boundaries and small objects), which are critical in medical image segmentation. To alleviate this problem, we propose a Dual-Aggregation Transformer Network called DuAT, which is characterized by two innovative designs, namely, the Global-to-Local Spatial Aggregation (GLSA) and Selective Boundary Aggregation (SBA) modules. The GLSA has the ability to aggregate and represent both global and local spatial features, which are beneficial for locating large and small objects, respectively. The SBA module is used to aggregate the boundary characteristic from low-level features and semantic information from high-level features for better preserving boundary details and locating the re-calibration objects. Extensive experiments in six benchmark datasets demonstrate that our proposed model outperforms state-of-the-art methods in the segmentation of skin lesion images, and polyps in colonoscopy images. In addition, our approach is more robust than existing methods in various challenging situations such as small object segmentation and ambiguous object boundaries.
HMDA: A Hybrid Model with Multi-scale Deformable Attention for Medical Image Segmentation

Abstract:Transformers have been applied to medical image segmentation tasks owing to their excellent longrange modeling capability, compensating for the failure of Convolutional Neural Networks (CNNs) to extract global features. However, the standardized self-attention modules in Transformers, characterized by a uniform and inflexible pattern of attention distribution, frequently lead to unnecessary computational redundancy with high-dimensional data, consequently impeding the model's capacity for precise concentration on salient image regions. Additionally, achieving effective explicit interaction between the spatially detailed features captured by CNNs and the long-range contextual features provided by Transformers remains challenging. In this architecture, we propose a Hybrid Transformer and CNN architecture with Multi-scale Deformable Attention(HMDA), designed to address the aforementioned issues effectively. Specifically, we introduce a Multi-scale Spatially Adaptive Deformable Attention (MSADA) mechanism, which attends to a small set of key sampling points around a reference within the multi-scale features, to achieve better performance. In addition, we propose the Cross Attention Bridge (CAB) module, which integrates multi-scale transformer and local features through channelwise cross attention enriching feature synthesis. HMDA is validated on multiple datasets, and the results demonstrate the effectiveness of our approach, which achieves competitive results compared to the previous methods.

Abstract:Medical image segmentation and classification are two of the most key steps in computer-aided clinical diagnosis. The region of interest were usually segmented in a proper manner to extract useful features for further disease classification. However, these methods are computationally complex and time-consuming. In this paper, we proposed a one-stage multi-task attention network (MTANet) which efficiently classifies objects in an image while generating a high-quality segmentation mask for each medical object. A reverse addition attention module was designed in the segmentation task to fusion areas in global map and boundary cues in high-resolution features, and an attention bottleneck module was used in the classification task for image feature and clinical feature fusion. We evaluated the performance of MTANet with CNN-based and transformer-based architectures across three imaging modalities for different tasks: CVC-ClinicDB dataset for polyp segmentation, ISIC-2018 dataset for skin lesion segmentation, and our private ultrasound dataset for liver tumor segmentation and classification. Our proposed model outperformed state-of-the-art models on all three datasets and was superior to all 25 radiologists for liver tumor diagnosis.

Abstract:Automated medical image segmentation is a crucial step in clinical analysis and diagnosis, as it can improve diagnostic efficiency and accuracy. Deep convolutional neural networks (DCNNs) have been widely used in the medical field, achieving excellent results. The high complexity of medical images poses a significant challenge for many networks in balancing local and global information, resulting in unstable segmentation outcomes. To address the challenge, we designed a hybrid CNN-Transformer network to capture both the local and global information. More specifically, deep convolutional neural networks are introduced to exploit the local information. At the same time, we designed a trident multi-layer fusion (TMF) block for the Transformer to fuse contextual information from higher-level (global) features dynamically. Moreover, considering the inherent characteristic of medical image segmentation (e.g., irregular shapes and discontinuous boundaries), we developed united attention (UA) blocks to focus on important feature learning. To evaluate the effectiveness of our proposed approach, we performed experiments on two publicly available datasets, ISIC-2017, and Kvasir-SEG, and compared our results with state-of-the-art approaches. We apply them to the Transformer branch to learn the cross-dimensional long-term dependency for medical images. Experimental results show that our CiT-Net provides better medical image segmentation results than popular SOTA methods. Besides, our CiT-Net requires lower parameters and less computational costs and does not rely on pre-training. Transformer and group parallel axial attention co-encoder for medical image segmentation

Abstract:U-Net has become baseline standard in the medical image segmentation tasks, but it has limitations in explicitly modeling long-term dependencies. Transformer has the ability to capture long-term relevance through its internal self-attention. However, Transformer is committed to modeling the correlation of all elements, but its awareness of local foreground information is not significant. Since medical images are often presented as regional blocks, local information is equally important. In this paper, we propose the GPA-TUNet by considering local and global information synthetically. Specifically, we propose a new attention mechanism to highlight local foreground information, called group parallel axial attention (GPA). Furthermore, we effectively combine GPA with Transformer in encoder part of model. It can not only highlight the foreground information of samples, but also reduce the negative influence of background information on the segmentation results. Meanwhile, we introduced the sMLP block to improve the global modeling capability of network. Sparse connectivity and weight sharing are well achieved by applying it. Extensive experiments on public datasets confirm the excellent performance of our proposed GPA-TUNet. Abstract:TO THE E D I T O R — I n April 2005, a case of reactivation of hepatitis B virus (HBV) infection occurred in a hemodialysis patient in an Irish tertiary referral center. In the absence of specific Irish dialysis guidelines at this time, most Irish units, including this center, followed UK guidelines; that is, HBV surface antigen (HBsAg) testing was performed prior to dialysis and then monthly, ideally, but at least every 3 months. The majority of units did not test for HBV core antibody (anti-HBc). This patient had tested negative for HBsAg prior to dialysis in November 2004 and was found to be HBsAg positive in April 2005. The patient became immunosuppressed over this period. Testing of an archived sample obtained prior to dialysis (November 2004) revealed that it was positive for anti-HBc. The April 2005 sample was positive for anti-HBc and negative for anti-HBc immunoglobulin (Ig) M.

Abstract:The medical image partition model has a wide range of application prospects in medical diagnosis and treatment and has become an important auxiliary method to improve the diagnostic level by medical imaging analysis. After the feature extraction ability of the convolutional neural network (CNN) reached a bottleneck, the form of feature extraction represented by Transformer has made significant achievements in the medical image domain in recent years. However, the structure of Transformer is relatively fixed, the cost of computer resources is large, and it is difficult to adjust the model structure according to the complex medical imaging segmentation task. To better adapt to the limitation of clinical diagnostic equipment on the parameter scale of the network model, this paper proposed a CK-ATTnet based on the convolutional kernel attention mechanism. CK-ATTnet uses the depthwise separable convolution attention mechanism, which completely innovates the way that the attention mechanism is used on the original image in the traditional model. In addition, CK-ATTnet realizes the new normal form of applying the attention mechanism to the convolutional kernel for feature extraction for the first time. This design further improves the local feature acquisition ability of the convolutional kernel and does not require additional hardware enhancement requirements after applying the attention mechanism to the model. Compared with other CNN models, CK-ATTnet can extract more accurate and fine-grained features; compared with Transformer-based models, it has fewer learning parameters. Experimental results show that CK-ATTnet exhibits better segmentation performance and fewer learning parameters than other models in multiple datasets and has very good application prospects.

Abstract:The convolutional neural networks (CNNs) have been widely proposed in the medical image analysis tasks, especially in the image segmentations. In recent years, the encoder–decoder structures, such as the U-Net, were rendered. However, the multi-scale information transmission and effective modeling for long-range feature dependencies in these structures were not sufficiently considered. To improve the performance of the existing methods, we propose a novel hybrid dual dilated attention network (HD 2 A-Net) to conduct the lesion region segmentations. In the proposed network, we innovatively present the comprehensive hybrid dilated convolution (CHDC) module, which facilitates the transmission of the multi-scale information. Based on the CHDC module and the attention mechanisms, we design a novel dual dilated gated attention (DDGA) block to enhance the saliency of related regions from the multi-scale aspect. Besides, a dilated dense (DD) block is designed to expand the receptive fields. The ablation studies were performed to verify our proposed blocks. Besides, the interpretability of the HD 2 A-Net was analyzed through the visualization of the attention weight maps from the key blocks. Compared to the state-of-the-art methods including CA-Net, DeepLabV3+, and Attention U-Net, the HD 2 A-Net outperforms significantly, with the metrics of Dice, Average Symmetric Surface Distance (ASSD), and mean Intersection-over-Union (mIoU) reaching 93.16%, 93.63%, and 94.72%, 0.36 pix, 0.69 pix, and 0.52 pix, and 88.03%, 88.67%, and 90.33% on three publicly available medical image datasets: MAEDE-MAFTOUNI (COVID-19 CT), ISIC-2018 (Melanoma Dermoscopy), and Kvasir-SEG (Gastrointestinal Disease Polyp), respectively.
