Abstract:The success of the promotion policies of the Medicines Derived from Traditional Pharmacopoeia (MDTPs) requires a regulation that is adapted to realities and guarantees their quality, efficiency and safeness. This study aims to analyze the legal texts and the current guiding principles obtained from the heads of the departments in charge of traditional medicine in Burkina Faso and Benin. The documents collected from the two countries have been analyzed comparatively and also in relation to WAEMU regulations and the WHO recommendations. Several texts, dealing with the activity, products, facilities and advertisement related to traditional medicine, have been recorded in both countries. The regulation battery of Burkina Faso is more extensive than that of Benin, especially on traditional medicine and pharmacopoeia facilities. In addition, unlike biomedicines, the West African Economic and Monetary Union (WAEMU) and the West African Health Organization (WAHO) have not yet passed community laws on MDTPs. To limit disparities in legal frameworks between the countries of the same sub-region, it is important that the WAEMU or WAHO be involved in the harmonization of pharmaceutical regulations by setting Community rules in the domain.
Public and private sector responses to essential drugs policies: a multilevel analysis of drug prescription and selling practices in Mali

Abstract:Many African countries have introduced cost recovery mechanisms based on the sale of drugs and measures aimed at improving drug supply. This study compares prescribing and selling practices in Mali, in 3 cities where the public sector contributes differentially to the supply of drugs on the market. Multilevel models are used to analyse the content and cost of 700 medication transactions observed in 14 private and public legal points of sale. Results show that the objective of improving access to drugs seems to have been achieved in the sites studied. Costs of prescriptions were lower where public health services had been revitalized. Affordable generic drugs were accessible and widely used, even in the private sector. However, measures intended to rationalize the prescription and delivery of drugs did not always have the desired effect. While agents in the public sector tended to prescribe fewer antibiotics, injectables, or brand-name drugs, the data confirm the virtual absence of advice concerning the use or the side effects of the drugs in both public and private sectors. In addition, data supported the notion that the public and private sectors are closely intertwined. Notably, availability of drugs in the public sector contributed to diminishing the prices charged in the private sector. Similarly, the use that agents in the public sector made of the opportunities afforded by the presence of the private pharmaceutical sector provided another illustration of interrelatedness. Finally, the data showed that the presence of a private sector, which has not been affected by measures aimed at rationalizing prescription and sales practices, limits the effects of measures implemented in the public sector. More assertive policies, based on strategies encompassing actors in the private sector, are needed to increase the safety and effectiveness of prescription and sales practices.

SIMULTANEOUS DETERMINATION OF METOPROLOL, PROPRANOLOL AND PHENOL RED IN SAMPLES FROM RAT IN SITU INTESTINAL PERFUSION STUDIES

Abstract:Single-pass intestinal perfusion technique (SPIP) is the most used classic technique employed in the study of intestinal absorption of compounds in which a non-absorbable marker such as phenol red is used to correct the water flux. A simple and rapid reversed-phase high performance liquid chromatographic method with UV detection at 227 nm was developed for simultaneous quantitation of propranolol and metoprolol along with phenol red for in-situ permeability studies. The mobile phase was a mixture of 55% methanol, 45% of 0.05 M KH2PO4 aqueous solution (adjusted to pH 6) and 0.2 % (v/v) triethylamine. Analysis was run at a flow rate of 1 ml/min with a 9 min run time. The calibration curves were linear for all three compounds (r > 0.999) across the concentration range of 7.5-125 µg/ml with a limit of detection of 4.24, 2.18 and 8.57 ng/ml and limit of quantification of 14, 7.2 and 28.3 ng/ml for metoprolol, propranolol and phenol red respectively. The coefficient of variation for intra-assay and interassay precision was less than 8% and the accuracy was between 93.6-107%. Using the SPIP technique and the suggested HPLC method for sample analysis, the mean values of 0.49 e -4 (±0.19) cm/sec and 0.32 e -4 (± 0.09) cm/sec were obtained for propranolol and metoprolol intestinal permeability coefficients

Thus, the aim of this study was to evaluate the policy governing herbal medicines regulation and its implementation in Ethiopia. Methods and Materials: An archival review, a semi-structured interview with key informants, a cross-sectional study involving traditional healers, and an institution-based cross-sectional survey were conducted from June 15, 2020 to December 25, 2020. The qualitative data was transcribed using Microsoft Word 10, whereas the quantitative data was recorded and analyzed using SPSS 20 computer statistical software. The study's findings are summarized using descriptive statistics. In addition, multiple logistic regressions were performed to identify factors affecting regulation of herbal medicine (HM) in Ethiopia. Variables with p< 0.05 were considered potential predictors. Results: According to all key informants, Ethiopia has yet to adopt distinct policies and laws on herbal medicine that may provide an independent regulatory framework. Similarly, the majority of respondents in an institution-based survey indicated that there were no defined policies (n=52, 57.3%), laws (n=53, 59.6%), or registration systems (n=67, 75.3%) for herbal medicine. However, traditional healers claimed that they are licensed by either the Woreda Health Bureau (n=21, 95.5%) or the Regional Health Bureau (n=1, 4.5%) to legally practise traditional herbal medicine. Besides, no traditional healer is licensed by Ethiopian Food and Drug Administration (EFDA) or Ministry of Health. Conclusion: Ethiopia has yet to adopt distinctive herbal medicine policies and laws to provide an independent herbal regulatory system. Despite the fact that the EFDA has a mandate for herbal medicine regulation, traditional healers are licensed by the woreda and regional health bureaus, thus more investigation is needed. Keywords: herbal medicine, regulation, policy, legal framework, Ethiopia Herbal medicines have a long tradition of use outside of allopathic medicines around the globe. 1 It is estimated that around 80% of the world's population uses herbal medicines, either for maintaining general wellbeing or as part of their primary source of health care. 2 As such, herbal medicines can save many lives, particularly in the developing countries. 3 Despite herbal medicines with an extended history of popular use being normally safe when used appropriately at common therapeutic doses, the absence of evidence of toxicity or adverse drug events can not be taken as confirmation of the lack of toxicity or safety of herbal medicines. 4 Scientifically, it has been proven that herbal medicines and their practices can cause harmful, adverse reactions if the product or therapy is of poor quality or it is taken inappropriately, even though there is a common belief that herbal medicine is safe since they are natural. 5 For instance, data from 260 Asian patented herbal medicines show that 25% of products contain high levels of heavy metal, and 7% contain unspecified drugs which are added intentionally to bring a desired effect. 6 As a result, national health authorities have enacted policies and laws to ensure that all herbal medicines are safe and of good quality, as herbal medicines have a high demand and market value for public health. 7 In a global survey conducted by the World Health Organization (WHO), around 124 member states (64%) responded that they had laws or regulations on herbal medicines whereas around 125 member states or almost 65% of member states responded that they have a registration system for herbal medicines. 6 For instance, regulatory harmonization of European Union member states established legislation (Directive 2001/83/European Commission & Directive 2004/24/European Commission) that has been approved to direct herbal medicine marketing in the European Union member states. 8 In Africa, however, there are a large number of traditional herbal medicines, and a lot of public knowledge about them, -Abstract Truncated-

Abstract:This study explores the traditional knowledge of plants used by traditional health practitioners (THPs) in the treatment of symptoms or syndromes related to mental illnesses in the district of Bamako in Mali, along with the identification of affiliated traditional treating methods. An exploratory and cross-sectional ethnopharmacological survey was conducted in the district of Bamako. The Malian Federation of Associations of Therapists and Herbalists (FEMATH) assisted in the identification and inclusion of the THPs. Data sampling included semi-structured interviews, questionnaires, and in-depth interviews. Quantitative data were evaluated by analysing reports of the use of different medicinal plants and the number of participants. Fifteen THPs belonging to the district of Bamako participated. In total, 43 medicinal plants belonging to 22 plant families were used by the THPs. The most cited plant species was Securidaca longepedunculata (violet tree), followed by Khaya senegalensis (African mahogany) and Boscia integrifolia (rough-leaved shepherds tree). A great number of herbal combinations, preparation methods, and administration routes were used, often with honey as an adjuvant. To our knowledge, this is the first ethnobotanical survey on the use of medicinal plants in the treatment of all types of mental disorders in Bamako.

Abstract:Background: Approximately 80% of the Ethiopian population predominantly depends on herbal medicines (HMs) for their primary healthcare needs. Nevertheless, worries regarding the safety, efficacy, and standard of herbal-based treatments have been escalating due to the lack of strong regulatory frameworks. Therefore, the study aimed to assess the presence of regulatory frameworks for traditional herbal medicines and their enforcement in Ethiopia. Methods: The qualitative–phenomenological study design was conducted from November 2021 to March 2022 G.C. The study included 25 regulatory official key informants (KIs) who work for national and regional medicine regulatory agencies, and 15 traditional herbal medicine (THM) practitioners who work at the regional level were purposefully selected for an in-depth interview (IDI). An in-depth interview guide was developed through the purposive sampling technique. The collected data were analyzed using thematic content analysis techniques. Results: The study found that the current national medicine proclamation is deemed inadequate in the regulation of THM. Both conventional and traditional herbal medicines are regulated by a single agency. Weak legal enforcement, a lack of government commitment and support, resource constraints, and inadequate regulatory tools are the main challenges faced in THM regulation. Conclusion: Overall, the study found inadequate legal frameworks and weak THM regulatory implementations in Ethiopia. Consequently, it is critical for all regulatory authorities in Ethiopia to exert their utmost efforts to effectively regulate THM.

Abstract:Globally, the potential of medicinal plants is increasingly being recognized due to their relative availability, particularly in rural areas. This review explored the ethnobotanical and economic values of six selected medicinal plants widely used to treat and manage childhood diseases in South Africa. Acalypha glabrata, Aloe maculata, Datura stramonium, Gomphocarpus fruticosus, Rhoicissus tridentata and Vachellia karroo were selected based on their high relative frequency of citations for treating a wide range of diseases. Information was obtained from various scientific databases and ethnobotanical books. In addition to being popular for treating childhood diseases, the selected medicinal plants possess diverse applications in traditional medicine for other age groups, highlighting their general therapeutic values. This translates to extensive harvesting, trading and consumption of these plants in order to meet demands on local levels. Currently, empirical data on the economic value of the selected plants remain poorly reported. Even though South Africa has many laws to conserve and promote indigenous knowledge and medicinal plants, their commercialization remains low. Particularly the cultivation of the selected plants needs to be promoted under a participatory management action plan to stimulate the economy of the disadvantaged. A collaborative research framework for the inclusive transformation of indigenous medicinal plants is recommended to reveal their concealed beneficial properties.

Translation of preclinical ethnomedicine data in LMICs: the example of rooibos

Abstract:All disease, but especially non-communicable diseases, are related to dysfunction of one or more regulatory systems. In developing countries, long-term management of patients with chronic diseases has many challenges and is generally not financially viable, but Africa in particular, which is rich in diverse ethnomedicines presents a more feasible long-term therapeutic approach in this niche. However, despite comprehensive preclinical investigations on numerous plant-derived candidate medicines, only a small portion of these reach the patient as recognised medicines. In this review, we use the example of rooibos ( Aspalathus linearis (Burm.f.) R. Dahlgren)–which is globally consumed as aromatic, caffeine-free tea–to illustrate the hurdles that need to be overcome in the low-to middle-income countries, before progression of ethnomedicines to official treatment regimens can be achieved. In terms of methodology, regulatory system focused rooibos papers indexed on PubMed for the past three decades (n = 112) were accessed. Papers reporting duplication of previous results were excluded, as well as review papers. Topics covered includes the high standard of ethnomedicine drug discovery and efficacy testing research performed in Africa (and South Africa in particular in the case of rooibos), the potential bias in terms of preclinical research focus, ethnomedicine ownership and the requirement for independent clinical trial coordination and/or management.

Synergistic in vitro antimalarial activity of plant extracts used as traditional herbal remedies in Mali

Abstract:In Mali, where malaria is endemic, plants are extensively used for treating periodic fevers and malaria. According to the advice of traditional medicine, plants are often mixed during the preparation of febrifugal decoctions. In previous studies, we demonstrated the potent in vitro antimalarial activity of extracts isolated from four plants commonly used in traditional remedies: Mitragyna inermis (Willd.) O. Kuntze, Rubiaceae, Nauclea latifolia (Sm.), Rubiaceae, Guiera senegalensis (Gmel.), Combretaceae, and Feretia apodanthera (Del.), Rubiaceae. In the present work, we evaluate the potent in vitro synergistic antimalarial interaction between these extracts, using standard isobologram analysis. Then, we evaluate their cytotoxicity on human monocytes and their mutagenic activity on an in vitro system of two beta-carboline alkaloids isolated from Guiera senegalensis (harman and tetrahydroharman). Three combinations demonstrate a strong, synergistic, inhibitory effect on in vitro plasmodial development and are devoid of cytotoxicity towards human cells. These results justify their use in association in traditional medicine. Moreover, tetrahydroharman, isolated from G. senegalensis, presents interesting antimalarial activity, no cytotoxicity and is not genotoxic in the Salmonella Ames test with and without metabolic activation.
Marketing Authorization Applications Made to the European Medicines Agency in 2018–2019: What was the Contribution of Real‐World Evidence?

Abstract:Information derived from routinely collected real-world data has for a long time been used to support regulatory decision making on the safety of drugs and has more recently been used to support marketing authorization submissions to regulators. There is a lack of detailed information on the use and types of this real-world evidence (RWE) as submitted to regulators. We used resources held by the European Medicines Agency (EMA) to describe the characteristics of RWE included in new marketing authorization applications (MAAs) and extensions of indication (EOIs) for already authorized products submitted to the EMA in 2018 and 2019. For MAAs, 63 of 158 products (39.9%) contained RWE with a total of 117 studies. For 31.7% of these products, the RWE submitted was derived from data collected before the planned authorization. The most common data sources were registries (60.3%) followed by hospital data (31.7%). RWE was mainly included to support safety (87.3%) and efficacy (49.2%) with cohort studies being the most frequently used study design (88.9%). For EOIs, 28 of 153 products (18.3%) contained RWE with a total of 36 studies. For 57.1% of these products, studies were conducted prior to the EOIs. RWE sources were mainly registries (35.6%) and hospital data (27.0%). RWE was typically used to support safety (82.1%) and efficacy (53.6%). Cohort studies were the most commonly used study design (87.6%). We conclude that there is widespread use of RWE to support evaluation of MAAs and EOIs submitted to the EMA and identify areas where further research is required.

Abstract:Background: While extant literature highlights several motivations for Traditional Medicine (TM) usage; few studies have examined how promotions and information asymmetries influence traditional medicine usage behaviors. This study aimed to map out information biases underlying the marketing and promotion of Traditional Medicines in Central Uganda and explored the dimensions of attitudes and usage behaviors of TM users. Methods: The study utilized a qualitative approach by conducting in-depth face-to-face semistructured interviews with TM users in the metropolitan parts of Central Uganda. The study participants were recruited from the Traditional Health Practitioners (THPs) clinics with the help of community development officers under the guidance of the National Council for Traditional Healers and Herbalists' Association (NACOTHA). Study participants gave their written consent, and their identities have since been kept confidential. Saturation point was reached at 34 respondents. Data was recorded and transcribed verbatim and analyzed using content analysis. Results: The study indicates that incomplete, inadequate and inconsistent information especially from TM promotions leads users to overly buy and use TM. Identified themes related to information asymmetry include unverifiable information, usage side effects, and absence of expiry dates on the labels, naturalness of TM, unclear dosage rates, limited diagnosis, limited usage guidelines, and lack of authentic approvals by regulatory bodies among others. Conclusions: The findings raise the need for policy guidelines in the alternative health segment and multi-agency regulation of THPs in Uganda. Further, massive community sensitization and customer education are recommended since the potential for information asymmetries in TM promotions is quite large.
A Legal Analysis on the Current Trade Status of Traditional Chinese Medicinal Product in the E.U. — From the Perspectives of WTO Law and E.U. Pharmaceutical Law

Abstract:This paper is based on the investigation and research for a project — China and Europe Taking Care of Healthcare Solutions and aims to discuss the trading environment of traditional Chinese medicinal product in European Union from the legal aspect including E.U. pharmaceutical law and World Trade Organization rules. It concerns the general knowledge of healthcare issues of WTO law, basic understandings of Chinese and western people concerning Traditional Chinese Medicine and market access provision of E.U. pharmaceutical law, hereinto to discuss the prospect and development difficulties and trend of TCM in E.U., and economic and political concerns are involved. The methodology of qualitative study is used, mainly including literature review, case study, and interview. It concludes that the E.U. law has set up standards to register traditional herbal medicinal products, yet it is very difficult for TCMP producers to reach the standards at this moment due to the lack of scientific data and references. E.U. has recognized the merit of traditional medicine; by comparing with the full acceptance of the efficacy of western conventional medicine, the writers foresee that traditional Chinese medicine will require a more general treatment in law of trade sooner or later.

Abstract:Ethnopharmacological relevance Knowledge of the high-sale medicinal plants and their authentication are essential parameters to ensure the safety of people using herbal medicine and to plan the safeguarding of medicinal species threatened with extinction. Aims The present study, carried out in Lubumbashi, Democratic Republic of Congo, aimed to geolocate medicinal plant sales points, list the best-selling species, and authenticate samples of the most popular species on the market. Methods A survey was conducted among the medicinal plant sellers in Lubumbashi's markets and other public spaces, to identify the best-selling species. Samples of the species reported as the most sold were purchased for sales unit weight measurement, and authentication. For the 3 most popular plant species, the identity of 92 samples, purchased from some 25 herbalists, was assessed by combining the microscopic characteristics of powdered drugs with the HPTLC fingerprints of methanolic extracts; for each species, these samples were compared with 1 or 2 botanically authenticated reference samples. As abundant starch granules were detected by microscopy, some samples were suspected of heavy flour contamination, which was confirmed by an enzymatic determination of their starch content. Results A total of 108 herbalists (48.1 % women) with a median age of 37 years (range, 20 to 67 years), and a median seniority of 5 years (0.7 to 30) were interviewed. From a total of 514 purchased samples, 396 (77.0 %) corresponded to 56 species that could be identified, including 92 samples representing the 3 most frequently sold plant species. The identities of 118 samples (having 82 different vernacular names), could not be determined, due to the lack of voucher specimen Roots were the most sold organ (56.0 %; n = 514), mainly in powder form (78.7 %), and the median price was 21 USD/kg, at the time of the survey (01/2022). The identified specimens were, predominantly, the roots of Terminalia mollis M.A.Lawson (33.3 %), Securidaca longepedunculata Fresen (28.7 %), and stem barks of Nauclea pobeguinii Hua ex Pobég. (23.1 %); from the recorded sales figures, the amounts of material annually sold for these 3 species are estimated at about 5.7, 6.0, and 3.1 tons, respectively. Some sellers reported problems in identifying and/or preserving S. longepedonculata and T. mollis .Among the 92 samples analyzed for the three species, 18.5 % raised problems, including species substitutions (14/17), dilution with flour (1/17), dilution with flour and species substitution (1/17), and detection of a phytochemical variant (1/17), that could arise either from contamination by another species, a particular plant growth environment, or poor storage conditions. Conclusion The confusion/adulteration rate measured here for highly popular species is quite alarming (18.5 %); but, indeed, roots, especially as powders, are often difficult to differentiate based on the coarse organoleptic examination practiced by herbal traders. Microscopic and phytochemical characteristics reported in this study, quite easy to obtain with basic laboratory equipment, should be systematically applied by the health products regulatory authorities to control the quality of herbals and ensure that patients get the drug desired for their treatment.

Assessing national governance of medicine promotion: an exploratory study in Ghana to trial a structured set of indicators

Abstract:Background: Two billion people worldwide, predominantly in low- and middle-income countries, cannot consistently access required essential medications, thus affecting their ability to attain optimal health outcomes. Access to appropriate medicines may be compromised due to issues involving cost, availability, quality, and prescribing practices, and system-wide factors such as a lack of transparency and accountability. Pharmaceutical promotional practices impact many of these issues, thus influencing the use of appropriate medicines,. Good governance is ultimately the responsibility of national governments through strong health systems with transparent and accountable practices that facilitate appropriate medicine use. We designed a structured set of indicators, based on existing tools, to assess the strength of the national governance of pharmaceutical promotion. In this exploratory study, we trialed the indicators in Ghana. Methods: Two existing tools, one developed by the World Health Organization and the other by Health Action International with the Medicines Transparency Alliance, were adapted to examine the governance of pharmaceutical promotion, resulting in a hybrid framework of 45 indicators of system strength, grouped into four categories: a) Governance of prescription medicines, b) Health care professional codes and regulations, c) Anti-corruption governance, and d) Indexes. Evidence was gathered via desk-based research to establish whether indicator requirements were met. Results: Our desk-based research discovered the following: a) 21 of 45 indicators for the governance of prescription medicines were met in Ghana, including the existence of a national medicines policy, national medicines list, medicines regulatory authority and a national guide for the promotion of prescription pharmaceuticals; b) pharmacists have a code of conduct specific to ethical promotion though co-development with the pharmaceutical industry should be further examined; and c) anti-corruption indicators were met for 10 of 12 criteria; and d) two indexes were available that were relevant to Ghana. Conclusion: Our set of indicators identified gaps and opportunities for the governance of medicines promotion in Ghana. These indicators have the potential to highlight areas requiring improved governance and could therefore form a useful diagnostic tool for identifying key discussion points for policy strengthening within low- and middle-income countries.
Scoping review on lessons learnt on the promotion and use of drugs and traditional medicine in Africa during COVID-19

Abstract:Scientific evidence on the safety and efficacy of pharmaceutical drugs, substances and herbal medicines is important in medical advertising and promotion. Following guidelines for conducting a scoping review, we systematically searched PubMed, SCOPUS and Web of Science to identify in peer reviewed articles medications that were promoted and used widely in Africa during the COVID-19 pandemic. We also searched for information about how safety concerns about untested/or not properly tested drugs were communicated to the public during the pandemic. Of the 2043 articles identified, 41 papers were eligible for inclusion. Most studies were clinical trials ( n = 11 ), systematic reviews (n = 9), quantitative studies (n = 9) the rest were qualitative studies, reviews and reports We found that following global trends, several drugs, traditional and herbal treatments were used and repurposed for the treatment of respiratory symptoms of COVID-19 in Africa. The results highlighted the value of some herbal medicines for treatment during the COVID-19 pandemic, as well as the risks posed by the unregulated sharing of advice and recommendations on treatments in Africa, and globally.

Abstract:Background: The coexistence of traditional healing practices deeply rooted in cultural and historical contexts and the evolving landscape of modern healthcare approaches in West African societies creates a dynamic interplay between tradition and modernity in healthcare. This study aims to comprehensively map the landscape of traditional medicine use for health in West Africa. Methods: A scoping review was conducted following the Joanna Briggs Institute (JBI) methodology and reported according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews (PRISMA-ScR) guidelines. Research questions focused on the links between traditional medicine practices and health in West Africa. The systematic literature search covered PubMed, Web of Science, and CINAHL from database inception to September 2023. A descriptive analysis was conducted highlighting the years of publication, countries of publication, study designs of plant families and plant parts used for making traditional medicines, and the diseases the traditional remedies are for. Results: The search identified 3484 records, with 46 articles meeting the inclusion criteria. Publications spanned from 1979 to 2023, with no observed trend in the number of publications over successive decades. Nigeria had the highest number of publications (54.3%), followed by Ghana (19.6%). The studies employed various designs, including clinical trials, ethnobotanical, ethnopharmacological, and experimental designs. Plant families frequently studied included Combretaceae, Euphorbiaceae, and Rubiaceae. Traditional remedies address various health issues, highlighting their versatility, from general symptoms to specific diseases. Conclusion: This scoping review offers an extensive overview of traditional healing practices in West Africa. The studies highlighted in this review stress the necessity for culturally sensitive healthcare interventions. The widespread use of traditional medicine and the variety of practices underscore the importance of encouraging collaboration between traditional healers and modern healthcare professionals. This review also identifies knowledge gaps and areas needing further research, setting the stage for future exploration into West Africa's intricate healthcare landscape.
