She would hold up a brown letter ‘‘E’’ and ask the kids what it was: a ‘‘Brownie.’’ She’d hold up another, with a red letter ‘‘E’’ and say ‘‘Are youIRed E?’’ Lots of word play. There was a teenager sitting in a corner. He hadn’t been saying anything before this, but during this game, he was the first to get it, each and every time. An ‘‘I’’ written in the color black? A black eye. Every time Dr. Lilly Pad held one up, he got it. He showed us who he was and what he knew, and afterwards the staff told us that that was when they learned who this kid was.V Dr. Chester Drawers (personal communication, 2008) In this Clinical Perspectives column, we consider what clowns might teach us about working with children and adolescents and what roles clowning might play in pediatric mental health. After all, laughter, joy, and play are an essential part of childhood, and clowns are a nearly ubiquitous source of laughter, now even performing on a regular basis in some hospitals. Mental health providers must be either formally or intuitively attuned to a child’s laughter, in part because most of the disorders of childhood and adolescence will in some way affect the core human behavior of laughter. Children who do not smile because they are anhedonic or who do not engage in developmentally expected types of play because they are anxious may find themselves in our offices. The ease with which a child smiles or laughs is a natural part of an assessment of a child’s affect. How a child engages in play and jokes gives us insight into their development. Attention to such characteristics may also be crucial to the therapeutic process. 1
Three-Month Follow-Up of the Post-COVID Syndrome after Admission to a Specialised Post-COVID Centre—A Prospective Study Focusing on Mental Health with Patient Reported Outcome Measures (PROMs)

Abstract:Background and objective: The impairments and duration of PASC (post-acute sequelae of COVID-19) symptoms in mental health have, to date, not been comprehensively examined. Our objective is to provide longitudinal data on the mental health of Post-COVID patients and to identify risk and protective factors associated with a severe or prolonged course. Methods: The mental health of 265 Post-COVID patients of the outpatient Post-COVID centre of the University Hospital Erlangen was assessed 17.1 (T0) and 22.5 months after infection (T1). An online survey with validated questionnaires for Post-COVID symptoms (Post-COVID Syndrome Score), depression (Patient Health Questionnaire-9), somatic symptoms (Patient Health Questionnaire-15), anxiety (Generalized Anxiety Disorder-7), fatigue (Fatigue Severity Scale) and Post-Exertional Malaise (PEM) (DePaul Post-Exertional Malaise Screening) was conducted in the home environment. Results: In total, 80% of patients experienced severe PASC at follow-up. Clinically relevant symptoms of depression, persistent somatic symptoms, anxiety and fatigue were reported by 55.8%, 72.5%, 18.9% and 89.4% of patients, respectively. Depressive, anxiety and somatic symptom severity decreased significantly over time; fatigue and PEM remained at an unchanged high level. The risk factor for higher depression scores was older age; prior psychiatric illness treated with psychotherapy was associated with more severe depressive, somatic, anxiety and PASC symptoms. PEM symptoms were significantly associated with longer duration between acute infection and initial presentation in the Post-COVID centre. Conclusions: Our findings align with previous research, claiming severe mental health symptoms in PASC syndrome, lasting for months after infection. In-depth assessment of risk and protective factors for the mental health implications of PASC is needed for the planning of health services and disease prevention.

Abstract:Post-COVID syndrome (PCS) is characterized by a variety of non-specific symptoms. One of the leading symptoms is fatigue. So far, there is no evidence-based causal therapy established and treatment of PCS is primarily symptom-oriented. The Clinic for Internal and Integrative Medicine in Bamberg, Germany, offers a comprehensive multimodal integrative inpatient therapy for PCS patients. Within a prospective uncontrolled observational study, the results of N = 79 patients were analysed. Post-COVID fatigue patients were hospitalized for up to 14 days. The treatment consists of individual modules depending on the patient's needs. It includes a wide range of integrative non-pharmacological treatment modalities. Outcomes were assessed before and after the inpatient treatment as well as 6 months after discharge from the hospital. Results show that fatigue of post-COVID patients in this study ( M = 76.30, SD = 10.18, N = 64) was initially significantly higher than in the subsample "women aged 60–92 years" of the general German population ( M = 51.5, Schwarz et al. [Schwarz et al. in Onkologie 26:140–144, 2003]; T(63) = 19.50, p < .001). Fatigue was significantly and clinically relevant reduced directly after discharge (M T1 = 76.21, SD = 11.38, N = 42; M T2 = 66.57, SD = 15.55, N = 42), F(1, 41) = 19.80, p < .001, partial eta squared = .326, as well as six months after discharge (M T3 = 65.31, SD = 17.20, N = 42), F(1, 41), p < .001, partial eta squared = .371. Additionally, self-reported ability to work (NRS, 0–10) improved significantly from admission (M T1 = 2.54, SD = 2.23, N = 39) to discharge (M T2 = 4.26, SD = 2.60, N = 39), F(1, 38) = 26.37, p < .001, partial eta squared = .410), as well as to six months later (M T3 = 4.41, SD = 3.23, N = 39), F(1, 38) = 15.00, p < .001, partial eta squared = .283. The study showed that patients suffering from chronic post-COVID syndrome for several months can achieve a significant improvement in their leading fatigue symptoms and a significant improvement in the subjective assessment of their ability to work through a comprehensive two-week multimodal integrative inpatient program.

Patient-Reported Experiences of Persistent Post–COVID-19 Conditions After Hospital Discharge During the Second and Third Waves of the Pandemic in Switzerland: Cross-Sectional Questionnaire Study

Abstract:Background: Hospitalized patients infected with SARS-CoV-2 should recover within a few weeks. However, even those with mild versions can experience symptoms lasting 4 weeks or longer. These post–COVID-19 condition (PCC) comprise various new, returning, or ongoing symptoms that can last for months or years and cause disability. Few studies have investigated PCC using self-reports from discharged patients infected with SARS-CoV-2 to complement clinical and biomarker studies. Objective: This study aimed to investigate self-reported, persistent PCC among patients infected with SARS-CoV-2 who were discharged during the second and third waves of the COVID-19 pandemic. Methods: We designed, pretested, and posted an ad hoc paper questionnaire to all eligible inpatients discharged between October 2020 and April 2021. At 4 months post discharge, we collected data on PCC and scores for the Multidimensional Fatigue Inventory (MFI), the Patient Health Questionnaire-4 (PHQ-4), a Brief Memory Screening Scale (Q3PC), and a posttraumatic stress disorder scale (PCL-5). Descriptive, inferential, and multivariate linear regression statistics assessed PCC symptomatology, associations, and differences regarding sociodemographic characteristics and hospital length of stay (LOS). We examined whether our variables of interest significantly predicted MFI scores. Results: Of the 1993 valid questionnaires returned, 245 were from discharged patients with SARS-CoV-2 (median age 71, IQR 62.7-77 years). Only 28.2% (69/245) of respondents were symptom-free after 4 months. Women had significantly more persistent PCC symptoms than men ( P ≤.001). Patients with a hospital LOS ≥11 days had more PCC symptoms as well ( P P=.50) or between intensive care units and other hospitalization units ( P =.09). Patients self-reported significantly higher PHQ-4 scores during their hospitalization than at 4 months later ( P PR 2 =0.09; F 4,7 =1.5; P =.22; adjusted R 2 =0.06). Conclusions: The majority of inpatients infected with SARS-CoV-2 presented with PCC 4 months after discharge, with complex clinical pictures. Only one-third of them were symptom-free during that time. Based on our findings, MFI scores were not directly related to self-reported depression, anxiety, or posttraumatic scores adjusted for age, sex, or LOS. Further research is needed to explore PCC and fatigue based on self-reported health experiences of discharged inpatients infected with SARS-CoV-2.

Abstract:Background Longer-term consequences after SARS-CoV-2 infection are becoming an important burden to societies and healthcare systems. Data on post-COVID-19 syndrome in the general population are required for the timely planning of healthcare services and resources. The objective of this study was to assess the prevalence of impaired health status and physical and mental health symptoms among individuals at least six months after SARS-CoV-2 infection, and to characterize their healthcare utilization. Methods This population-based prospective cohort study (Zurich SARS-CoV-2 Cohort) enrolled 431 adults from the general population with polymerase chain reaction-confirmed SARS-CoV-2 infection reported to health authorities between 27 February 2020 and 05 August 2020 in the Canton of Zurich, Switzerland. We evaluated the proportion of individuals reporting not to have fully recovered since SARS-CoV-2 infection, and the proportion reporting fatigue (Fatigue Assessment Scale), dyspnea (mMRC dyspnea scale) or depression (DASS-21) at six to eight months after diagnosis. Furthermore, the proportion of individuals with at least one healthcare contact after their acute illness was evaluated. Multivariable logistic regression models were used to assess factors associated with these main outcomes. Results Symptoms were present in 385 (89%) participants at diagnosis and 81 (19%) were initially hospitalized. At six to eight months, 111 (26%) reported not having fully recovered. 233 (55%) participants reported symptoms of fatigue, 96 (25%) had at least grade 1 dyspnea, and 111 (26%) had DASS-21 scores indicating symptoms of depression. 170 (40%) participants reported at least one general practitioner visit related to COVID-19 after acute illness, and 10% (8/81) of initially hospitalized individuals were rehospitalized. Individuals that have not fully recovered or suffer from fatigue, dyspnea or depression were more likely to have further healthcare contacts. However, a third of individuals (37/111) that have not fully recovered did not seek further care. Conclusions In this population-based study, a relevant proportion of participants suffered from longer-term consequences after SARS-CoV-2 infection. With millions infected across the world, our findings emphasize the need for the timely planning of resources and patient-centered services for post-COVID-19 care.

Abstract:Introduction: There is still much uncertainty about why some people develop persistent cognitive and mental health problems after SARS-CoV-2 infection and require additional care while others do not. In this study, we investigated the cognitive and psychological outcomes of non-hospitalized post-COVID-19 patients referred to an outpatient post-COVID-19 clinic for persistent symptoms more than 3 months after infection. Additionally, we aimed to explore the influence of demographic, physical, and personal factors on these outcomes. Methods: This cross-sectional study was conducted at an outpatient post-COVID-19 clinic located at a prominent clinical teaching hospital in the Netherlands. Participants included non-hospitalized patients referred between 2020 and 2022, more than 3 months after SARS-CoV-2 infection, experiencing persistent symptoms. Main outcome measures included levels of anxiety and depression (Hospital Anxiety and Depression Scale), post-traumatic stress symptoms (PTSS) (Post-traumatic Stress Symptoms Checklist 14), and cognitive symptoms (Checklist for Cognitive and Emotional Consequences). Data analysis employed Spearman correlation and hierarchical multiple regression analyses. Results: A total of 265 patients (61% female; mean age of 51.7 ± 13.7 years) were included in the study, with an average of 7.6 ± 4.5 months following SARS-CoV-2 infection. Among them, 104 patients (40%) reported high levels of anxiety, 111 patients (43%) showed high levels depressive symptoms, and 71 patients (31%) demonstrated high levels of PTSS. Additionally, 200 patients (79%) reported experiencing more than 2 cognitive symptoms. Bivariate analyses indicated associations between psychiatric history and increased cognitive and psychological symptoms. Multivariate analyses revealed positive associations between physical symptoms and cognitive and psychological symptoms, and catastrophizing thoughts were associated with higher anxiety levels (β = 0.217, p < 0.001). Conversely, positive refocusing was associated with lower depressive symptoms (β = -0.325, p < 0.001), PTSS (β = -0.290, p < 0.001), and cognitive symptoms (β = -0.220, p < 0.001). Discussion: Among non-hospitalized COVID-19 patients seeking care for persistent symptoms, approximately one-third reported high levels of psychological symptoms, and more than three-quarter experienced cognitive symptoms. Physical symptoms, psychiatric history, and a tendency to catastrophize were identified as potential risk factors for persistent psychological and cognitive symptoms. Conversely, positive refocusing demonstrated a protective effect. These findings contribute to the understanding of long-term COVID-19 outcomes and emphasize the importance of integrating a biopsychosocial perspective into treatment approaches.

Abstract:Leakage current or the I/sub DDQ/ test has been shown to be an effective test screen in combination with traditional test methods. However, leakage current is rising rapidly as semiconductor technology advances. This makes it difficult to distinguish between faulty and fault-free chips using traditional threshold setting methods. This paper presents a method to estimate leakage current using neighboring chip information on a wafer. Outlier chips are rejected, and a least-squares-fit plane through neighboring chips is used to estimate defect-free I/sub DDQ/. Chips that significantly deviate from the estimate are rejected. The proposed method is evaluated using industrial test data.
The Progression of Symptoms in Post COVID-19 Patients: A Multicentre, Prospective, Observational Cohort Study

Abstract:Background: Although the coronavirus disease 2019 (COVID-19) pandemic is no longer a public health emergency of international concern, 30% of COVID-19 patients still have long-term complaints. A better understanding of the progression of symptoms after COVID-19 is needed to reduce the burden of the post COVID-19 condition. Objective: This study aims to investigate the progression of symptoms, identify patterns of symptom progression, and assess their associations with patient characteristics. Methods: Within the P4O2 COVID-19 study, patients aged 40–65 years were recruited from five Dutch hospitals. At 3–6 and 12–18 months post COVID-19, medical data were collected, and pulmonary function tests were performed. In between, symptoms were assessed monthly with a questionnaire. Latent class mixed modelling was used to identify symptom progression patterns over time, with multinomial logistic regression to examine associations with patient characteristics. Results: Eighty-eight patients (aged 54.4 years, 48.9% males) were included. Three trajectories were identified for fatigue and dyspnoea: decreasing, high persistent, and low persistent. The odds of "decreasing fatigue" was higher for never smokers and participants in the lifestyle intervention and lower for those having a comorbidity. The odds of "decreasing dyspnoea" was higher for moderate COVID-19 patients and lifestyle intervention participants and lower for males, mild COVID-19 patients, and those with a higher age. Conclusions: Three distinct trajectories were identified for fatigue and dyspnoea, delineating patterns of symptom persistence following COVID-19. Sex, age, smoking status, participation in lifestyle interventions and COVID-19 severity were associated with the likelihood of belonging to different trajectories. These findings highlight the heterogeneity of the long-term symptoms experienced by post COVID-19 patients and emphasise the importance of personalised treatment strategies.

Abstract:Ziel der Studie Post-COVID zeichnet sich durch eine große Anzahl verschiedener Symptome aus. Die Indikation für eine medizinische Rehabilitation orientiert sich am Hauptsymptom. Versicherte, die unter Erschöpfung leiden und keine relevante organische Störung aufweisen, werden oft in psychosomatischen Kliniken rehabilitiert. In der vorliegenden Studie soll die Effektivität einer psychosomatischen Rehabilitation bei Patient:innen mit Post-COVID untersucht werden. Methodik 91 Patient:innen mit Post-COVID werden mit 124 Patient;innen mit psychischen Störungen sowie 68 Patient:innen mit Tumorerkrankungen hinsichtlich der Verbesserung der Erschöpfung und der Depressivität sowie hinsichtlich der Zufriedenheit und sozialmedizinischer Parameter verglichen. ErgebnisseDas Ausmaß der Erschöpfung bei Aufnahme unterschied sich zwischen den drei Gruppen nicht und konnte gleichermaßen reduziert werden. Bei Patient:innen mit Post-COVID und hohen Depressionswerten bei Aufnahme war die Erschöpfung auch bei Entlassung im klinisch bedeutsamen Bereich. Der Anteil der Patient:innen mit psychischen Störungen, die eine negative Erwerbstätigkeitsprognose aufweisen, war im Vergleich zu den Patient:innen mit Post-COVID erhöht. DiskussionAuch wenn sich durch eine psychosomatische Rehabilitationsbehandlung die Erschöpfung reduzieren lässt, ist der Anteil der unter Erschöpfung leidenden Patient:innen mit Post-COVID bei Entlassung immer noch hoch. SchlussfolgerungDas Behandlungskonzept zur Rehabilitation von Patient:innen mit Post-COVID muss den oftmals individuellen Verlauf der Erkrankung berücksichtigt werden. Das multidisziplinäre Behandlungskonzept einer psychosomatischen Rehabilitation kann hier einen wichtigen Beitrag leisten. Aim of the study Post-COVID is characterized by a large number of different symptoms. The indication for medical rehabilitation is based on the main symptom. Insured individuals who suffer from fatigue and have no relevant organic disorder are often rehabilitated in psychosomatic clinics. In the present study, the effectiveness of psychosomatic rehabilitation in patients with post-COVID will be investigated. Methods 91 patients with post-COVID are compared to 124 patients with mental disorders and 68 patients with cancer regarding the improvement of fatigue and depressiveness as well as satisfaction and socio-medical parameters. Results At admission, the level of fatigue did not differ in the three groups and was equally reduced. Patients with post-COVID and high depression scores at admission had clinically significant levels of fatigue at discharge. The proportion of patients with mental disorders who were discharged with a negative prognosis for employment was significantly increased. Discussion Although psychosomatic rehabilitation can reduce fatigue, the proportion of patients suffering from fatigue with post-COVID at discharge is still high. Conclusion The treatment concept for the rehabilitation of patients with post-COVID must take into account the often individual course of the disease. Abstract:Objective: To follow up patients with post-COVID-19 condition (PCC) 6 months after a multidisciplinary team assessment in specialist care regarding symptoms of pain, anxiety, depression, fatigue and cognition, level of activity, physical activity and sick leave. Methods: A prospective pilot study conducted in a clinical setting of patients (n = 22) with PCC referred from primary healthcare to a specialist clinic for a 2 day-multidisciplinary team assessment followed by a subsequent rehabilitation plan. Data were collected through questionnaires filled in prior to the team assessment and 6 months later. Results: Fifteen of the initial 22 patients participated in the follow-up. No statistically significant improvements were seen in any of the questionnaires after 6 months. However, 76.9% of the participants perceived the intervention as being helpful. This differed between the genders, where all the women 100% (n = 8) perceived it as being helpful, compared with 40% (n = 2) of the men (p = 0.012). Conclusions: Based on these findings, the benefit of a multidisciplinary team assessment of PCC is not fully convincing. However, since the participants themselves perceived the intervention as being helpful, the team assessment seems to be of some value. Further studies with larger populations would be of interest.
Feasibility of an allied health led, workplace delivered Long COVID service for hospital staff: a mixed-methods study

Abstract:This study aimed to evaluate the feasibility of a workplace-delivered outpatient multidisciplinary service (ReCOV) for staff experiencing post COVID-19 condition ('Long COVID'). A mixed-methods study of staff at a large, tertiary hospital with Long COVID who attended the service was conducted. Participants completed questionnaires to determine baseline symptoms and were offered allied health appointments for up to 12 weeks each based on clinical indication. Acceptability, implementation, practicality and limited efficacy were evaluated via one-on-one semi-structured interviews and analysed using inductive thematic analysis. Limited efficacy was evaluated via pre- and post-questionnaires and demand via multidisciplinary utilisation. Twenty-three (median age 37 [interquartile range 30–45] years, 52% female) participants were included. Participants had appointments with a median of 4 [3–5] different professions; most commonly exercise physiology ( n = 19, 83%), occupational therapy ( n = 17, 74%) and neuropsychology ( n = 15, 65%). Median time spent on the ReCOV service was 15 [9–19] weeks. Thirteen semi-structured interviews were completed and analysed. Participants valued ReCOV for being a COVID-19 specific, convenient, flexible and multidisciplinary service at their workplace. Participants preferred the service to have been available for longer than 12 weeks to achieve further benefits as many participants perceived little change in physical health. Attending a multidisciplinary service located at their workplace was feasible for staff to manage post COVID-19 symptoms. Further research is required to confirm the efficacy on patient outcomes.

Abstract:Ziel Die Post-COVID-Sprechstunde (PCS) wird im Rahmen eines umfassenden Behandlungsangebotes der gesetzlichen Unfallversicherung für Post-COVID-Patient*innen durchgeführt, um individuelle Empfehlungen für die weitere Versorgung festzulegen. Ziel der Untersuchung war die Erfassung der Hauptsymptome sowie die damit verbundenen Einschränkungen der sozialen und beruflichen Teilhabe, um Konsequenzen für die ambulante Rehabilitation abzuleiten. Methodik Neben einer ärztlichen Untersuchung sowie einem psychologischen Konsil wurden zahlreiche Assessments zur Evaluation des biopsychosozialen Gesundheitszustands durchgeführt. 373 weibliche (82,2%) und 81 männliche Patient*innen im Alter zwischen 40 und 60 Jahren aus den Berufen der Gesundheits- und Pflegedienste, der Erziehung und Pädagogik nahmen seit April 2021 an der PCS teil. Ergebnisse Nahezu alle untersuchten Patient*innen (98,2%) gaben Fatigue als Kardinalsymptom ihrer Post-COVID-Beschwerden an, in über 73% der Fälle in Kombination mit subjektiv erlebten Einschränkungen der Hirnleistungsfähigkeit. Die Dauer der Symptomatik persistierte dabei sowohl bei den weiblichen als auch männlichen Versicherten durchschnittlich seit 14–15 Monaten. Somit sind über 85% der Gesamtstichprobe als Fälle chronischer Erschöpfung (Fatigue Skala) einzustufen. Die Schwere der Fatigue beeinflusst zudem proportional die Lebensqualität (SF-36), Gefühle von Angst und Depression (HADS), die psychische Widerstandskraft (RS-13) sowie motorische Parameter wie die maximale Greifkraft und die Ausdauerleistungsfähigkeit. 54,3% der Versicherten erhielten zudem eine psychische Verdachtsdiagnose und 38,1% die Empfehlung zu einer weiterführenden neuropsychologischen Diagnostik. Schlussfolgerung Zur weiteren Behandlung des Leitsymptoms der chronischen Fatigue ist eine multimodale und interdisziplinäre ambulante Rehabilitation zu empfehlen, die sich an der Behandlung der Diagnose des Chronischen Fatiguesyndroms (ME/CFS) und somit insbesondere an einem psychoedukativen und weniger an einem kurativen Therapieansatz orientieren und Nachsorgestrategien berücksichtigen sollte. Gesicherte psychische Störungen und neuropsychologische Defizite sind begleitend zu behandeln. Objective The post-COVID consultation (PCC) is offered as part of a comprehensive range of treatment services provided by the statutory accident insurance for post-COVID patients to determine individual recommendations for further care. The aim of the study was to record the main symptoms and the associated restrictions on social and occupational participation in order to derive consequences for outpatient rehabilitation. Method In addition to a medical examination and a psychological consil, numerous assessments were carried out to evaluate the biopsychosocial state of health. 373 female (82.2%) and 81 male patients aged between 40 and 60 years from the professions of health and care services, education and pedagogy participated in the PCC since April 2021. Results Nearly all patients (98.2%) reported fatigue as a cardinal symptom of their post-COVID complaints, in combination with subjectively experienced limitations in brain functioning in over 73% of cases. The duration of the symptomatology persisted for an average of 14–15 months in both female and male insured persons. Thus, over 85% of the total sample can be classified as cases of chronic fatigue (Fatigue Scale). Abstract:Post-COVID-19 has been recognized as possibly affecting millions of people worldwide. In order to optimize care and ensure equality, we established a multidisciplinary virtual Post-COVID-19 clinic (VPCC) within Maccabi Healthcare Services, the second largest HMO in Israel. This study aims to describe the structure, process and patient satisfaction with this clinic. The multidisciplinary team consisted of physicians, physiotherapists, social workers, occupational therapists and dieticians. Patient entry was to be at least four weeks after COVID-19 infection. A patient satisfaction survey was carried out 7–8 months after the clinic was closed. Demographic data were collected and compared to the general Maccabi COVID-19 population. The clinic treated 1614 patients, aged 16–91, over a period of 18 months. In total, 679 family physicians referred patients. In comparison to the general COVID-19 population, a higher percentage of the VPCC patients lived in the periphery of Israel, South (14.9% compared to 17.8%) and North (17.1% compared to 18.2%). In total, 249 patients answered the survey, and of them, 75% were highly satisfied with the medical care of the physician in the VPCC. A total of 54% of respondents would have preferred a face-to-face consultation, but 50% felt that communication was good in the virtual mode. In conclusion, the VPCC provided a dedicated service for patients, and the virtual format made it equally accessible to all parts of the country.

Abstract:Nach akuter Infektion mit dem SARS-CoV-2-Virus leiden bis zu 10 % der Erkrankten an längerfristigen gesundheitlichen Beeinträchtigungen, dem sogenannten "Long-COVID-Syndrom". Zur Versorgung dieser Patient*innen wurden in Deutschland spezialisierte Ambulanzen eingerichtet. Eine strukturierte Erfassung der Versorgungssituation ist für eine bedarfsgerechte Versorgung essenziell. Ziel der vorliegenden Studie ist strukturelle und prozessbezogene Versorgungsaspekte von Long-COVID-Ambulanzen in Deutschland darzustellen. Mittels Online-Befragung wurden Aspekte wie Struktur und Organisation der Ambulanzen, Leistungsangebote und Vernetzung der Versorgung erhoben. Insgesamt konnten 95 Ambulanzen identifiziert werden. Die Einladung der Ambulanzleiter*innen zur Teilnahme an der Befragung erfolgte per E-Mail. Die Datenerhebung fand zwischen Februar und Mai 2022 statt. Die Auswertung der Daten erfolgte deskriptiv. Insgesamt haben 28 Ambulanzleiter*innen (29 %) an der Umfrage teilgenommen. Die Befragten waren zwischen 32 und 66 Jahre alt; 61 % davon (n = 17) waren männlich. Die Ambulanzen waren am häufigsten an die Fachbereiche Pneumologie (n = 10; 36 %), Innere Medizin, Psychiatrie und Psychosomatische Medizin sowie Neurologie (jeweils n = 8; 29 %) angegliedert. Nach Aussage der Ambulanzleiter*innen betrug die Wartezeit auf einen Termin in 64 % der Fälle (n = 18) mehr als einen Monat. Die Auslastung (n = 25; 89 %), die Terminnachfrage (n = 26; 93 %) sowie der Bedarf an weiteren Post-COVID-Ambulanzen (n = 20; 71 %) wurden von den Befragten als hoch bewertet. Nahezu alle Ambulanzleiter*innen gaben an, mit klinikinternen Einrichtungen (n = 27; 96 %) sowie Haus- und niedergelassenen Fachärzt*innen (jeweils n = 21; 75 %) vernetzt zu sein. Die Ergebnisse zeigen einen Schwerpunkt der Versorgung im Bereich der Pneumologie sowie zu gleichen Teilen in der Inneren Medizin, Psychiatrie/Psychosomatik und Neurologie. Weiterhin weisen die Ergebnisse auf einen hohen Bedarf an Post-COVID-Ambulanzen und die Notwendigkeit des Ausbaus dieses Versorgungsangebotes hin. After acute infection with the SARS-CoV-2 virus, up to 10 % of affected individuals suffer from long-term health impairments, also referred to as "Post-COVID". In Germany, specialized outpatient clinics have been established to care for patients with Post-COVID. A structured survey of the care situation is not yet available, but essential for a demand-oriented care. The present study aimed to systematically assess and describe structural and process-related aspects of care, and to perform an inventory and needs analysis of Post-COVID outpatient clinics in Germany. An online survey was developed assessing the structure and organization of the outpatient clinics, service offerings and networking of care from the perspective of the outpatient clinic directors. A total of 95 outpatient clinics were identified, and an invitation to participate in the online survey was sent via e-mail to the directors of the outpatient clinics. Data were collected between February and May 2022. Descriptive data analysis was performed. A total of 28 outpatient clinic managers (29 %) took part in the survey. Participants were between 32 and 66 years old, and 61 % (n = 17) were male. The outpatient clinics were most frequently affiliated with the specialties of pneumology (n = 10; 36 %), internal medicine, psychiatric and psychosomatic medicine, and neurology (n = 8; 29 %, respectively). Among the outpatient clinic directors, 64 % (n = 18) stated that the time spent waiting for an appointment was more than one month. Utilization (n = 25; 89 %), appointment demand (n = 26; 93 %), and the need for more Post-COVID outpatient clinics (n = 20; 71 %) were rated as high by the outpatient clinic directors. Nearly all directors reported networking with in-clinic facilities (n = 27; 96 %), with primary care physicians and with specialists in private practice (n = 21; 75 %, respectively). The main focus of care is pneumology. Internal medicine, psychiatry/psychosomatics and neurology are also equally represented. Abstract:Purpose As healthcare workers (HCW) have been disproportionally affected by COVID-19, its post-acute sequelae (PASC) in HCW can impact healthcare systems. We assessed the burden and course of PASC in HCW over a 30-month period. Methods In a prospective multicentre HCW cohort in Switzerland, PASC surveys were conducted in 03/2021, 09/2021, 06/2022, 04/2023, and 10/2023. Stratified by viral variant at first infection, the prevalence of PASC symptoms, self-experienced PASC and the Post-COVID Functional Status (PCFS) were analysed cross-sectionally in 10/2023, self-perceived success of therapeutic measures used was assessed. The evolution of PASC symptoms and PCFS in Wild-type and non-Wild-type infected HCW compared to uninfected controls was analysed longitudinally across all surveys. Results In cross-sectional analysis, 1704 HCW (median age 47 years, 82.2% female) were included. Thereof, 30.7% reported ≥ 1 PASC symptom in 10/2023, with 115 (6.7%) stating to have or have had PASC. Both were most common after Wild-type infection compared to other variants. Overall, 17/115 (15%) indicated relevant/severe restrictions in their daily activities and of 85 (74%) that tried ≥ 1 measure against their symptoms, 69 (81%) reported having benefitted. Longitudinal analysis (n = 653) showed a significantly higher proportion of Wild-type infected HCW to report PASC symptoms compared to controls in 03/2021 (+ 21%, 95% CI 4–39), with decreasing trend (+ 7%, 95%CI -10–25 in 10/2023). This effect was not evident for non-Wild-type infected HCW. Conclusions Over a 30 month period, overall PASC burden in our HCW cohort decreased, although 1% still experience relevant restrictions in their daily life; Wild-type infected individuals show the highest disease burden.

Abstract:This article analyzes hospitals' strategies to shape physicians' behavior and counter suppliers' power in purchasing physician preference items. Two models of standardization are limitations on the range of manufacturers or products (the "formulary" model) and price ceilings for particular item categories (the "payment-cap" model), both requiring processes to define product equivalencies often with inadequate product comparison data. The formulary model is more difficult to implement because of physicians' resistance to top-down dictates. The payment-cap model is more feasible because it preserves physicians' choice while also restraining manufacturers' power. Hospitals may influence physicians' involvement through a process of orchestration that includes committing to improve clinical facilities, scheduling, and training and fostering a culture of mutual trust and respect.

Abstract:Introduction: In addition to physical symptoms such as dyspnea, fatigue, post-exertional malaise, and pain, a subgroup of patients with Post-COVID-19 syndrome (Post-Acute Sequelae of COVID-19, PASC) suffers from mental illnesses such as anxiety, depression, and neurocognitive impairments. To date, there are no causal treatments available for PASC. While initial studies show that psychotherapy improves psychological symptoms, PASC-related fatigue, and psychosocial functioning, further research is needed to evaluate the effectiveness of psychotherapeutic treatment for PASC. Methods and analysis: This study presents a non-randomized controlled trial aimed at evaluating the effectiveness of a five-week multimodal inpatient psychosomatic treatment program for individuals experiencing PASC symptoms and comorbid mental illness. A total of 118 patients presented at the Post-COVID Center at the Universitätsklinikum Erlangen will be assigned to the intervention group receiving inpatient psychosomatic treatment or the control group receiving treatment as usual. The inclusion criteria for the intervention group are a diagnosis of PASC and at least one condition of mental distress and problems with coping with illness. The primary objective of the intervention is to reduce mental ailments, including depression and anxiety, as well as neurocognitive deficits, and to address PASC symptoms such as fatigue and pain. The core elements of the treatment are psychotherapy in individual and group settings, medical treatment, neurocognitive training, and physical therapy, adapted to the individual's capacity and oriented towards the concept of pacing. After enrollment, participants will undergo a 6-month follow-up to assess long-term results and the sustainability of the intervention effects. Discussion: This study examines the effectiveness of inpatient psychotherapeutic treatment in PASC patients with comorbid mental illness in comparison with a control group based on treatment as usual. The results of the study can contribute to the development of evidence-based interventions to address the complex needs of patients with PASC and comorbid mental illness. Abstract:Introduction Healthcare workers (HCWs) have been of particular relevance for overcoming the SARS-CoV-2 pandemic. At the same time they have been affected by SARS-CoV-2 infections with above average probability. Around 6.5% of the overall infected persons are likely to develop persistent symptoms resulting from the infection, known as long-COVID or post-COVID syndrome (PCS). The aim of this study is (1) to investigate the prevalence, course and characteristics of PCS in German HCWs, (2) to examine its effects on psychosocial variables, (3) to identify rehabilitation and healthcare needs and (4) to analyse treatment experiences. Methods and analysis In a cohort study with a randomised selection of participants (N=20 000) from the Employer's Liability Insurance Association for Health and Welfare Care, the health status of HCWs, who had COVID-19 in their professional context will be examined. There will be two measurement points: baseline (T 1 ) and a 12-month follow-up (T2). The outcome measures are the health status with a particular focus on persistent or newly occurring symptoms after a SARS-CoV-2 infection, health-related quality of life, functional capacity, the subjective need for and utilisation of healthcare services. Pre-existing conditions, the course of the acute infection and sociodemographic factors are considered as predictors. An advisory board made up of affected HCWs supports the study by contributing to the surveys' contents. Ethics and dissemination The study has been approved by the Local Ethics Committee of the Center for Psychosocial Medicine at the University Hospital Hamburg-Eppendorf (LPEK-0518). For dissemination, the results will be published in peer-reviewed journals, presented at conferences and communicated to relevant stakeholders in general and rehabilitation medicine. Barium versus nonbarium stimuli: differences in taste intensity, chemesthesis, and swallowing behavior in healthy adult women.

Abstract:PURPOSE The authors examined the impact of barium on the perceived taste intensity of 7 different liquid tastant stimuli and the modulatory effect that these differences in perceived taste intensity have on swallowing behaviors. METHOD Participants were 80 healthy women, stratified by age group (<40; >60) and genetic taste status (supertasters; nontasters). Perceived taste intensity and chemesthetic properties (fizziness; burning-stinging) were rated for 7 tastant solutions (each prepared with and without barium) using the general Labeled Magnitude Scale. Tongue-palate pressures and submental surface electromyography (sEMG) were simultaneously measured during swallowing of these same randomized liquids. Path analysis differentiated the effects of stimulus, genetic taste status, age, barium condition, taste intensity, and an effortful saliva swallow strength covariate on swallowing. RESULTS Barium stimuli were rated as having reduced taste intensity compared with nonbarium stimuli. Barium also dampened fizziness but did not influence burning-stinging sensation. The amplitudes of tongue-palate pressure or submental sEMG did not differ when swallowing barium versus nonbarium stimuli. CONCLUSIONS Despite impacting taste intensity, the addition of barium to liquid stimuli does not appear to alter behavioral parameters of swallowing. Barium solutions can be considered to elicit behaviors that are similar to those used with nonbarium liquids outside the assessment situation.
