Abstract:After a year of transition to a new publisher, Creative Nursing 2024 will build on our legacy of nurturing novice authors with a Student Article of the Year initiative; presenting courageous discourse as reflected in our overarching theme for the year, The Impact of Social Forces on Nursing and Health; and publishing even more new knowledge in the form of research and discussion of nursing theories and models. This issue's theme, Activism, Advocacy, and Allyship, is exemplified by articles about paths to a nursing workforce that reflect the people we serve, and about concepts such as hygge, dialectical pluralism, acculturative stress, shared decision-making for breast health, nature immersion, iceberg demographics, and self-care in palliative care.
Innovation Reports: Successes and Limitations for Promoting Innovation in Medical Education

Abstract:In 2013, Academic Medicine introduced a new article type, Innovation Reports, with the intent to promote innovation by providing a forum for publishing promising new ideas at an early stage of development. In this article, the authors examine Innovation Reports as a means for promoting innovation within the medical education community.The authors undertook a 2-part analysis. In July 2018, they examined submission data, Altmetric scores, and citations for the first 5 years of the feature. To explore authors' perceptions of the impact of publishing an Innovation Report, in October 2018, they conducted a web-based survey of corresponding authors.Between October 2013 and May 2018, 920 manuscripts were submitted under the Innovation Report category, of which 335 were sent for review and 151 were published (16% overall acceptance rate). The mean citation rate for the published Innovation Reports was 4.3 (range 0-47). The mean Altmetric Attention Score was 14.3 (range 0-441). An Altmetric score of 14 places an article in the top 20% of articles published in Academic Medicine.The 151 published Innovation Reports had 148 unique corresponding authors, of whom 85 (57%) responded to the survey. Almost all respondents (n = 82; 96%) reported that publishing an Innovation Report promoted their individual career growth. For many corresponding authors, the publication of early ideas in an Innovation Report appeared to be an end point rather than a springboard for further development and innovation, as only 14 (16%) reported publication of a subsequent related study.Reflecting on the successes and limitations of Innovation Reports over the first 5 years, the authors suggest that soliciting input from more stakeholders and being explicit about the goals of this article type would help inform how Innovation Reports should evolve in the future so they can better launch creative thought and spur innovation.

New registered nurse and registered psychiatric nurse graduates' transition-to-practice amid the pandemic: An art-based grounded theory study

Abstract:Background: According to the Transition Shock theory, new Registered Nurses face formidable obstacles within their first year of practice. However, little is known about the experience of new Registered Psychiatric Nurses entering the workforce. Objectives: The aim was to explore how new graduates from nursing and psychiatric nursing experience their transition into the workplace that included identifying mediating factors that assist or challenge these entry-level professionals within their transition. Design: Constructivist grounded theory methodology developed by Charmaz was utilized to explore the first-year transition period using art-based methods. Participants and setting: A total of 13 new graduates practicing as Registered Nurses and Registered Psychiatric Nurse in a western Canadian province were recruited to participate in the study. Three art-based workshops occurred at four, eight, and 12-months starting in the fall of 2020 through the Spring of 2021 during the novel coronavirus pandemic with subsequent member checks in the Spring of 2022. Methods: A total of 38 interview and focus group data were collected. Art-based activities included sand tray and figurine object exercises, collaging, reflective and thematic writing, as well as group concept mapping. Through co-creation of knowledge using Charmaz's methodology, participants selected the major themes throughout their first year of transition with guided researcher facilitation. Data were analyzed using the constant comparative method of analysis for each workshop drawing on the art creations, discussion, and narrative reflective writing pieces generated by participants. Results: Participants were in a basic psychosocial process of growing personal and professional self. Through co-construction of knowledge with the participants, three substantive interconnected categories were identified: (1) Being thrown into the fire; (2) Adjusting and adapting to the team environment; and (3) Nurturing self and others. Conclusions: The findings revealed many recommendations for employers hiring new graduates and nursing education programs preparing graduates for the practice arena.
Temporal Exploration of New Nurses' Field Adaptation Using Text Network Analysis

Abstract:Purpose: This study aimed to analyze the experiences of new nurses during their first year of hospital employment to gather data for the development of an evidence-based new nurse residency program focused on adaptability. Methods: This study was conducted at a tertiary hospital in Korea between March and August 2021 with 80 new nurses who wrote in critical reflective journals during their first year of work. NetMiner 4.5.0 was used to conduct a text network analysis of the critical reflective journals to uncover core keywords and topics across three periods. Results: In the journals, over time, degree centrality emerged as "study" and "patient understanding" for 1 to 3 months, "insufficient" and "stress" for 4 to 6 months, and "handover" and "preparation" for 7 to 12 months. Major sub-themes at 1 to 3 months were: "rounds," "intravenous-cannulation," "medical device," and "patient understanding"; at 4 to 6 months they were "admission," "discharge," "oxygen therapy," and "disease"; and at 7 to 12 months they were "burden," "independence," and "solution." Conclusion: These results provide valuable insights into the challenges and experiences encountered by new nurses during different stages of their field adaptation process. This information may highlight the best nurse leadership methods for improving institutional education and supporting new nurses' transitions to the hospital work environment.

Laser and Particle Beams

Abstract:The start of a new issue in a new year is always an opportunity to look back and to say thank you to the members of the editorial board and the editorial staff who made it possible that we were able to publish four interesting issues in Volume 24 last year. First of all, I want to thank the scientific community for submitting a large number of excellent original research papers to Laser and Particle Beams. In 2006, we were able to publish 71 articles plus editorial material and corrections. The average article in the journal is now 8–9 printed pages, and we have thus achieved the editorial goal to offer the community a place to publish their results without general space restrictions. Authors have accepted this opportunity to present their material with the necessary details. This seems to be very important to readers and authors as well, and we see that for a second consecutive year many articles in Laser and Particle Beams are referenced already during the first year, which is remarkable for a small journal with only four issues per year.
From student to therapist: exploring the first year of practice

Abstract:Objectives: The transition from classroom to clinical practice challenges many health professional students. This study used a phenomenological approach to explore the lived experience of rehabilitation students during their final placement and first year of practice. Method: Students (n = 6) in occupational therapy and physical therapy wrote reflective journals every 2 weeks during their final fieldwork placement and first year of practice. The researchers independently analyzed the journals for common themes. An independent peer completed a blind analysis of two journals. Data were also compared with published first-person accounts of novice practitioners. Results: The lived experience of the first year of practice included four consecutive stages: Transition, Euphoria and Angst, Reality of Practice, and Adaptation. Themes from the journals included great expectations, competence, politics, shock, education, and strategies. Conclusion: Recognizing the stages in the process of transition from student to therapist may assist in educational curriculum development and clinical support and supervision for new graduates. Educators need to continue to make education practice relevant while maintaining a theoretical perspective.

Abstract:There has been sustained research interest in the role of early career researchers in advancing the field and the challenges that they face. However, efforts to document lived experiences of researchers working in a specific research area within science education have been scarce. This paper considers the meaning of innovation in the context of nature of science (NOS) research, drawing from a collective reflection of five early career academics from different backgrounds. After discussing the sources of our motivation to innovate in NOS research, we identify four distinct pathways of innovation. These pathways include (1) delving into specific aspects of NOS in greater depth, (2) exploring the interface of NOS and other established research areas, and (3) using NOS to address pressing social issues, and (4) expanding the methodological repertoire of NOS research. We illustrate these four modes of research innovation using examples from our own work. Barriers to early-career innovation such as the absence of NOS in curricula and initial teacher education, the lack of time to engage with practitioners to develop and implement instructional resources, and the underrepresentation of diverse education systems in NOS research literature are discussed.

Abstract:World Journal of Transplantation (WJT) was launched in December 2011. While we are celebrating WJT's 10-year anniversary, we are very proud to share with you that since its first issue, WJT has published 312 articles, which have been cited 2786 times (average cites per article of 9.0). Together with an excellent team effort by our authors, Editorial Board members, independent expert referees, and staff of the Editorial Office, WJT advanced in 2020. In this editorial, we summarize the journal's bibliometrics, including its citation report, published articles in 2020, peer review rate and manuscript invitation metrics, as well as its Editorial Board members and existing problems of WJT. The overall aim of this editorial is to promote the development of WJT in 2021. We appreciate the continuous support and submissions from authors and the dedicated efforts and expertise by our invited reviewers. Endured and prevailed: a phenomenological study of doctors' first year of clinical practice

Abstract:Context: The challenging nature of the transition from medical student to doctor is highlighted by the associated negative consequences to new doctors' mental health and wellbeing. Enhanced understanding of the lived experience of recent medical graduates as they move through the stages of transition over the first year of practice can inform interventions to ease the difficulties encountered. Methods: Using interpretative phenomenological analysis (IPA), a novel approach to this topic, we explored the lived experience of transition from student to doctor over the first year of practice after graduation. Twelve new graduates were purposively recruited. We conducted semi-structured interviews at the end of their first year of practice with respect to their experience over the first year. Results: The experience of transition was characterised by overlapping temporal stages. Participants' initial adjustment period was characterised by shock, coping and stabilisation. A phase of development followed, with growth in confidence and a focus on self-care. Adversity was experienced in the form of interprofessional tensions, overwork, isolation and mistreatment. Finally, a period of reflection and rationalisation marked the end of the first year. Discussion: Following initial anxiety regarding competence and performance, participants' experience of transition was predominantly influenced by cultural, relational and contextual aspects of clinical practice. Solutions to ease this challenging time include stage-specific transitional interventions, curricular change at both undergraduate and postgraduate levels and a re-evaluation of the clinical learning environment to mitigate the difficulties endured.

Abstract:Aims and objectives: The aim was to interview new entrants to a Master's level programme of educational preparation for advanced healthcare practice roles. The objectives were to gain an understanding of their issues and concerns as they looked ahead to their future development and to illuminate issues concerning their experiences as students and as employees. Background: As advanced practice roles become more widespread in the United Kingdom and abroad, nurses and other healthcare professionals are becoming more involved in innovation and service development and experiencing greater autonomy in many areas, particularly in regard to clinical examination and diagnosis. Design: A qualitative design was employed to illuminate issues and concerns of new students on a Master's degree programme of educational preparation for advanced healthcare practitioners. Methods: Two focus group interviews were conducted in 2004. Sixteen students from two programme cohorts participated in the two groups. Findings: Two main themes were outlined. These were: 'opportunities for development' and 'time pressures'. Subcategories within these two themes are also discussed. Conclusions: Students undertaking Master's level preparation for advanced healthcare practitioner roles perceive personal and professional benefits and opportunities to develop innovative services to improve patient care as accruing from the programme. These opportunities and benefits were related to the United Kingdom national health policy agenda. Some role difficulties were experienced. The additional burdens that the programme of study provided were an issue for some, but these were perceived as a temporary problem, worth undergoing for the likely long-term benefits. Relevance to clinical practice: Students undertaking educational preparation for advanced practice roles see themselves as innovators, specifically aiming to develop and improve aspects of service provision. Education for advanced practice roles should not be undertaken lightly, but can equip students with various skills, particularly regarding clinical assessment, and these skills should underpin practitioners' autonomy.

Abstract:Objective: The aim of this study was to understand newer nurses' perception and expectations of their work environment, professional and institutional satisfaction, and motivating/decision-making factors around nursing and intent to leave their positions. Background: Studies have shown newly graduated nurses face many challenges transitioning from a student to an independently practicing nurse. The COVID pandemic complicated this transition and created new stressors resulting in discouragement and turnover for newer nurses. Methods: This study used a qualitative approach through purposive sampling, semistructured qualitative interviews, and content analytic techniques. Transitional Shock Theory was used as the theoretical framework. Results: Four themes emerged from the study: 1) confronting the reality; 2) betrayal/mistrust; 3) relational integrity; and 4) enablers of clinical competence. Conclusions: The findings will help nurse leaders understand the importance of organizational-level supports to improve the transition of newer nurses in typical periods and in times of additional stress.
