Abstract:A lack of consumption of a diversified diet is associated with poor physical and cognitive development in children. Evidence on the relationship between minimum dietary diversity (MDD) and childhood malnutrition remains inconclusive in India. We hypothesized that children aged 6 to 23 months on a diversified diet (five out of eight defined foods and beverages) are less likely to be malnourished (stunting, wasting, and underweight) compared to their counterparts who are not on a diversified diet. This cross-sectional study was based on the 2019-2021 National Family Health Survey of India, comprising a weighted sample of 57,714 children aged 6 to 23 months. Multilevel logistic regression was conducted for data analysis. The results showed a significant protective effect of dietary diversity on underweight (odds ratios [OR] = 0.91; 95% confidence intervals [CI]: 0.86-0.96). In addition, children who did not consume eggs (OR = 1.09; 95% CI; 1.03-1.15), dairy products (OR = 1.22; 95% CI: 1.17-1.27), or fruits and vegetables (OR = 1.11; 95% CI: 1.06-1.17) were more likely to be underweight than children who did. Children who did not consume dairy products, fruits, and vegetables were also more likely to be stunted and wasted. However, we did not find significant associations of MDD with wasting and stunting. Nutritional interventions promoting daily consumption of dairy products, eggs, fruit, and vegetables are recommended to address the growing problem of childhood malnutrition in India. Regions with higher rates of malnutrition and those lacking MDD, such as Uttar Pradesh and Rajasthan, should be prioritized.

Abstract:Low dietary diversity significantly interplays with children’s growth failure. However, evidence of its crucial role in children’s health remains inconclusive in developing countries such as India. This study attempts to find the association between dietary diversity and growth outcomes among children aged between 6 and 23 months in India using the fourth round of the National Family Health Survey (NFHS), 2015–2016. A total of 67,278 mother-child pairs of children between the ages of 6–23 months and mothers aged 15–49 years were included in this study. Pearson’s chi-square significance test and multivariable logistic regression were used to determine the association between dietary diversity and child growth outcomes (stunted, wasted, and underweight). The study found that the prevalence of stunting and severe stunting among children aged between 6 and 23 months were 35.9% and 16.2%; 23.8% and 8.5% represented wasting, and severe wasting; and more than 32%, 10% were underweight and severely underweight respectively. This present study found that having an inadequate minimum dietary diversity (<4 food groups) significantly increases the risk of being stunted (adjusted odds ratio (AOR) = 1.29; 95% confidence interval (CI); 1.21–1.38), wasted (AOR = 1.29; 95% CI; 1.21–1.38), and underweight (AOR = 1.47; 95% CI; 1.39–1.56). Further, it was noted that children who did not intake dairy products, eggs, and other fruits and vegetables were more likely to be stunted, wasted, and underweight and more likely to be severely stunted, wasted, and underweight. Therefore, additional nutrition-specific interventions are urgently needed to strengthen and enhance existing feeding interventions aimed at improving infant and young child feeding (IYCF) practices, including complementary feeding practices among children aged between 6 and 23 months in India. The Government should focus such interventions more on states or regions where the prevalence of adequate minimum dietary diversity (MDD) and malnutrition is high.

Abstract:To understand an emergent spacetime is to understand the emergence of locality. Entanglement entropy is a powerful diagnostic of locality, because locality leads to a large amount of short distance entanglement. Two-dimensional string theory is among the very simplest instances of an emergent spatial dimension. We compute the entanglement entropy in the large-N matrix quantum mechanics dual to two-dimensional string theory in the semiclassical limit of weak string coupling. We isolate a logarithmically large, but finite, contribution that corresponds to the short distance entanglement of the tachyon field in the emergent spacetime. From the spacetime point of view, the entanglement is regulated by a nonperturbative "graininess" of space.
Divergence in time trends between nutrition and health status of Indian children: an enigma?

Abstract:India has been food secure and children had access to essential health and nutrition services for four decades but under-nutrition rates in children continue to be high. Despite the high prevalence of low birthweight (LBW) and under-nutrition, under-five mortality rates are relatively low: the ‘South Asian enigma’. Data from research studies and large-scale nutrition and health surveys were analysed and reviewed to assess: dimensions, determinants and time trends of under-nutrition in under-five children, functional decompensations associated with LBW and under-nutrition, factors responsible for ‘South Asian enigma’, and progress towards Sustainable Development Goals (SDG) for child nutrition and health. Neonates born with low weight and length, follow a lower trajectory of growth throughout childhood; this was the major factor responsible for the high under-nutrition rates in children. Breast feeding was nearly universal; about 2/3rd of infants were solely breast-fed at 5 months; but both complementary feeds and adult food given to children were inadequate and lacked diversity. This resulted in a rise in under-nutrition rates between 12-23 months. In 24-59 months there was no further rise in under-nutrition rates. Moderate under-nutrition is not associated with increase in risk of infections; health care for infection was available and utilised; therefore, mortality rates were low. India will not be able to achieve the SDG targets for reduction in low birthweight or under-nutrition but is likely to achieve the targets for reduction in food insecurity and under-five mortality.

Abstract:Dietary diversity among children is a crucial factor influencing their nutritional status; therefore, this paper uses data from four rounds of the Cambodia Demographic and Health Survey (CDHS) to examine the minimum dietary diversity among children aged 6-23 months. Multilevel binary regression is used to evaluate the variation in minimum dietary diversity at the cluster and province levels. The results show that nearly half of Cambodian children consistently lacked access to vitamin A-rich fruits and vegetables. Although the prevalence of inadequate minimum dietary diversity (MDD) among children significantly dropped from 76% in 2005 to 51% in 2021-2022, it is still high and needs attention. A decomposition analysis (Blinder-Oaxaca decomposition) was further used to understand the drivers of this temporal change in dietary diversity. The empirical results show that clusters represented the most significant source of geographic variation with respect to all eight food groups and MDD. Nutritional policy should improve education and awareness, reduce socio-economic disparities, leverage media, and promote full antenatal care to improve dietary diversity in Cambodia. Initiatives targeting the enhancement of insufficient minimum dietary diversity intake should encompass individual aspects and be customized to suit geographic and community settings.

Abstract:There has been a notable reduction in moderate and severe forms of child anthropometric failure in India since 1993. However, the rate and direction of change vary widely among states and union territories (UTs) and between moderate and severe categories within each of the states and UTs. Though child anthropometric failure (CAF) is a persistent problem in India, previous studies have often neglected state‐level variance and aggregated moderate and severe CAF categories. This study addresses this gap by examining moderate and severe malnutrition across India's states and union territories (UTs) from 1993 to 2021. Data of children under 2 years old from five waves of National Family Health Surveys, a representative cross‐sectional survey of Indian households, were analysed. Outcomes included prevalence of moderate and severe stunting, underweight and wasting, as per the 2006 World Health Organization growth standards. Percentage prevalence and standardized absolute change (SAC) were calculated nationally and by region for each wave. From 1993 to 2021, there was a notable reduction in the nationwide prevalence of moderate stunting, underweight and wasting, with rates dropping from 20% to 16%, 23% to 18%, and 15% to 12%, respectively. Severe stunting and underweight declined considerably from 23% to 16% and 18% to 11%, respectively; severe wasting marginally increased from 8% to 9%. From 2016 to 2021 moderate underweight was noted to have the highest SAC across all regions, although 15 regions saw an increase in the prevalence of moderate underweight. In the 2016–2021 period, severe wasting has increased in 13 of the 36 regions. While there has been a nationwide reduction in most indicators of CAF since 1993, the rate and direction of change vary widely among states and UTs and between moderate and severe categories within each of the states and UTs. Understanding these patterns of change can direct context‐specific interventions for improving child nutrition and health. A greater focus on reducing severe wasting, which has increased since 1993, is also crucial.

Abstract:Background Dietary diversity is an indicator of nutritional adequacy, which plays a significant role in child growth and development. Lack of adequate nutrition is associated with suboptimal brain development, lower school performance, and increased risk of mortality and chronic diseases. We aimed to determine the prevalence and determinants of meeting minimum dietary diversity (MDD), defined as consuming at least five out of eight basic food groups in the previous 24-h in three sub-Saharan African countries. Methods A weighted population-based cross-sectional study was conducted using the most recent Demographic and Health Surveys (DHS). MDD data were available between 2019 and 2020 for three sub-Saharan African countries (Gambia, Liberia, and Rwanda). The study population included 5,832 children aged 6–23 months. A multivariable logistic regression model was developed to identify independent factors associated with meeting MDD. Results Overall, the weighted prevalence of children who met the MDD was 23.2% (95% CI: 21.7–24.8%), ranging from 8.6% in Liberia to 34.4% in Rwanda. Independent factors associated with meeting MDD were: age of the child (OR) = 1.96, 95% CI: 1.61, 2.39 for 12–17 months vs. 6–11 months], mothers from highest households' wealth status (OR = 1.86, 95% CI: 1.45–2.39) compared with the lowest, and mothers with secondary/higher education (OR = 1.69, 95% CI: 1.35–2.12) compared with those with no education. Mothers who were employed, had access to a radio, and those who visited a healthcare facility in the last 12 months were more likely to meet the MDD. There was no significant association between the child's sex and the odds of fulfilling the MDD. Conclusions There is substantial heterogeneity in the prevalence of MDD in these three sub-Saharan African countries. Lack of food availability or affordability may play a significant role in the low prevalence of MDD. The present analysis suggests that policies that will effectively increase the prevalence of meeting MDD should target poor households with appropriate materials or financial assistance and mothers with lower literacy. Public health interventions working with sectors such as education and radio stations to promote health education about the benefits of diverse diets is a critical step toward improving MDD in sub-Saharan Africa and preventing undernutrition.

Abstract:Background: This study examines the levels and predictors of malnutrition in Indian children under 5 years of age. Methods: Composite Index of Anthropometric Failure was applied to data from the India National Family Health Survey 2019-2021. A multivariable logistic regression model was used to assess the predictors. Results: 52.59% of children experienced anthropometric failure. Child predictors of lower malnutrition risk included female gender (adjusted odds ratio (AOR) = 0.881) and average or large size at birth (AOR = 0.729 and 0.715, respectively, compared to small size). Higher birth order increased malnutrition odds (2nd-4th: AOR = 1.211; 5th or higher: AOR = 1.449) compared to firstborn. Maternal predictors of lower malnutrition risk included age 20-34 years (AOR = 0.806), age 35-49 years (AOR = 0.714) compared to 15-19 years, normal BMI (AOR = 0.752), overweight and obese BMI (AOR = 0.504) compared to underweight, and secondary or higher education vs. no education (AOR = 0.865). Maternal predictors of higher malnutrition risk included severe anemia vs. no anemia (AOR = 1.232). Protective socioeconomic factors included middle (AOR = 0.903) and rich wealth index (AOR = 0.717) compared to poor, and toilet access (AOR = 0.803). Children's malnutrition risk also declined with paternal education (primary: AOR = 0.901; secondary or higher: AOR = 0.822) vs. no education. Conversely, malnutrition risk increased with Hindu (AOR = 1.258) or Islam religion (AOR = 1.369) vs. other religions. Conclusions: Child malnutrition remains a critical issue in India, necessitating concerted efforts from both private and public sectors. A 'Health in All Policies' approach should guide public health leadership in influencing policies that impact children's nutritional status.
Effect of social and behavioral change interventions on minimum dietary diversity among pregnant women and associated socio-economic inequality in Rajasthan, India

Abstract:Background: Maternal dietary diversity is a key to improving the birth and child health outcomes. Besides socio-economic factors, the nutrition specific program- Social and Behavioural Change Communication (SBCC) interventions aimed to improve maternal dietary diversity has varied levels of impact on the socio-economic groups in poor resource setups. Objective: To measure the factors associated with the minimum dietary diversity (MDD) among pregnant women in selected districts of Rajasthan with special emphasis on the SBCC components. Additionally, it measures the socio-economic gaps in the behaviour of consumption of diversified diet during pregnancy. Methods: Data from a cross sectional survey of 6848 pregnant women, who have received a continuous SBCC counselling and registered under a state introduced conditional cash transfer program, during May to June, 2023, in five intervention districts -Banswara, Baran, Dunagrpur, Pratapgarh and Udaipur in Rajasthan was used. A 24 h recall based food consumption behaviour has been gathered to measure the MDD of pregnant women. Study has used descriptive statistics, multivariate regressions, and multivariate decomposition analysis to address the research objectives. Results: Study finds that only 55.2% of pregnant women are consuming diverse diet in the study duration with mean dietary diversity score is 4.8 (+/- 1.5). Logistic regression finds that SBCC components such as frontline workers (aOR = 1.3, CI: 1.1-1.4), community motivators (aOR = 1.9, CI: 1.7-2.1), and participation in MCHND (aOR = 1.0, CI: 0.9-1.2) have significant and higher likelihood on consumption of MDD food on previous day. A higher education and belonging from richer wealth quintile also show higher association for consumption of MDD. Multivariate decomposition shows, among richest and poorest wealth categories there is 19% point difference (58% difference due to coefficient vs. 42% difference due to composition) in MDD consumption. This is positively contributed by the caste and educational categories of women. Conclusion: Despite a predominant vegetarian diet consuming population, better maternal dietary diversity was observed among those exposed to higher dose of SBCC intervention package. Educational status and caste of the respondent were significantly associated with minimum dietary diversity and contributed to the socio-economic inequality highlighting the importance of tailored and sustained SBCC interventions.

Abstract:Pakistan is facing a high prevalence of malnutrition and Minimum Dietary Diversity (MDD) is one of the core indicators that remain below the recommended level. This study assesses MDD and its associated factors among children aged 6 to 23 months in Pakistan. The study uses a cross-sectional study using the dataset of the latest available Multiple Indicators Cluster Survey (MICS) for all provinces of Pakistan. Multistage sampling is used to select 18,699 children aged 6 to 23 months. The empirical method is the Logistic Regression Analysis and Chi-Square Test. The dataset is freely and publicly available with all identifier information removed, and no ethics approvals are required. About one-fifth (20%) of infants and young children aged 6 to 23 months had met MDD, this number varies from 17 to 29%, highest in Baluchistan and lowest in Punjab province of Pakistan. The age group (18–23) indicates a 2.45 times greater chance of having MDD. Age (< 0.001), diarrhea (0.01), prenatal care (0.06), mother's education (< 0.001), computer access (< 0.001), wealth quantile (< 0.001), and residence (< 0.001) were significantly associated with meeting MDD. However, gender (0.6) and mother's age (0.4) both were statistically insignificant in meeting MDD. Regarding mothers' education, compared to no education, the chance of MDD is 1.45 times greater for highly educated mothers in the Punjab province. Dietary diversity among children aged 6 to 23 months in Pakistan is low. It is recommended that mothers should be aware and encouraged to use dietary diverse food for infants and younger children.

Abstract:Background: Malnutrition is a major contributor to disease burden in India. To inform subnational action, we aimed to assess the disease burden due to malnutrition and the trends in its indicators in every state of India in relation to Indian and global nutrition targets. Methods: We analysed the disease burden attributable to child and maternal malnutrition, and the trends in the malnutrition indicators from 1990 to 2017 in every state of India using all accessible data from multiple sources, as part of Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2017. The states were categorised into three groups using their Socio-demographic Index (SDI) calculated by GBD on the basis of per capita income, mean education, and fertility rate in women younger than 25 years. We projected the prevalence of malnutrition indicators for the states of India up to 2030 on the basis of the 1990-2017 trends for comparison with India National Nutrition Mission (NNM) 2022 and WHO and UNICEF 2030 targets. Findings: Malnutrition was the predominant risk factor for death in children younger than 5 years of age in every state of India in 2017, accounting for 68·2% (95% UI 65·8-70·7) of the total under-5 deaths, and the leading risk factor for health loss for all ages, responsible for 17·3% (16·3-18·2) of the total disability-adjusted life years (DALYs). The malnutrition DALY rate was much higher in the low SDI than in the middle SDI and high SDI state groups. This rate varied 6·8 times between the states in 2017, and was highest in the states of Uttar Pradesh, Bihar, Assam, and Rajasthan. The prevalence of low birthweight in India in 2017 was 21·4% (20·8-21·9), child stunting 39·3% (38·7-40·1), child wasting 15·7% (15·6-15·9), child underweight 32·7% (32·3-33·1), anaemia in children 59·7% (56·2-63·8), anaemia in women 15-49 years of age 54·4% (53·7-55·2), exclusive breastfeeding 53·3% (51·5-54·9), and child overweight 11·5% (8·5-14·9). If the trends estimated up to 2017 for the indicators in the NNM 2022 continue in India, there would be 8·9% excess prevalence for low birthweight, 9·6% for stunting, 4·8% for underweight, 11·7% for anaemia in children, and 13·8% for anaemia in women relative to the 2022 targets. For the additional indicators in the WHO and UNICEF 2030 targets, the trends up to 2017 would lead to 10·4% excess prevalence for wasting, 14·5% excess prevalence for overweight, and 10·7% less exclusive breastfeeding in 2030. The prevalence of malnutrition indicators, their rates of improvement, and the gaps between projected prevalence and targets vary substantially between the states. Interpretation: Malnutrition continues to be the leading risk factor for disease burden in India. It is encouraging that India has set ambitious targets to reduce malnutrition through NNM. The trends up to 2017 indicate that substantially higher rates of improvement will be needed for all malnutrition indicators in most states to achieve the Indian 2022 and the global 2030 targets. The state-specific findings in this report indicate the effort needed in each state, which will be useful in tracking and motivating further progress. Similar subnational analyses might be useful for other low-income and middle-income countries. Gender Discrimination in Infant and Young Child Feeding Practices in India: Evidence from NFHS-4

Abstract:Undernutrition among children continues to be a serious issue. The study examines the prevalence of the gender gap in the infant and young child feeding (IYCF) practices in India based on recent National Family and Health Survey Data, 2015 (NFHS-4). A binary logistic regression model has been used to determine the factors responsible for achieving exclusive breastfeeding, minimum dietary diversity and minimum meal frequency of the index child, that is, youngest child in the households. A separate regression model was run for both boys and girls. Girls faced discrimination not only in terms of exclusive breastfeeding but also in terms of receiving the required number of complementary meals. Logistic regression results indicate that the birth order of the children and the sex composition of the siblings are the important determinants of IYCF practices. Girls are more deprived in terms of receiving exclusive breastfeeding and other age-appropriate protein-rich food items when born in a household with more than two older daughters. Media exposure, mother’s autonomy and access to health and nutrition-related education are prominent factors in determining IYCF practices. The results suggest that there is a need to improve gender sensitisation in terms of feeding practices within the households.
Trend and determinants of minimum dietary diversity consumption change among children aged 6-23 months in Ethiopia from 2011 to 2019: A multivariate decomposition analysis

Abstract:Background: Sustainable Development Goals 2 target 2.2 aimed to end all forms of malnutrition by 2030. However, the prevalence of inadequate minimum dietary diversity (MDD) is high in Ethiopia and the evidence of trends and determinants of MDD consumption change among children over time is limited. Therefore, this study aimed to determine trends and determinants of MDD consumption change among children aged 6 to 23 months in Ethiopia from 2011-2019 Demographic and Health Survey (DHS) data. Method: The study used the data on nationally representative weighted samples of 6,971, 7,467, and 3,154 children aged 6-23 months from the 2011, 2016, and 2019 rounds of the DHS. Trend and Multivariate Decomposition analysis was conducted to determine MDD consumption change from 2011-2016, and 2016-2019 into two components: one that was explained by differences in the level of the determinants (endowment), and the other component was explained by differences in the effect of the covariates on the outcome (coefficient effects). Result: The trends analysis showed that adequate MDD consumption significantly increased from 2% to 10.41% from 2011 to 2016 but decreased from 10.41% to 7.11% from 2016 to 2019 in Ethiopia. The compositional and behavioral change factors like maternal age, occupational status of parents, sex of the household head, wealth index, residence, and sex of child statistically contributed to changes in MDD consumption from 2011 to 2016, and from 2016 to 2019 at p-value <0.05. Conclusion: The trend of minimum dietary diversity consumption among children aged 6 to 23 months in Ethiopia increased from 2011-2016 and decreased from 2016-2019 in the last decade. The study revealed that the changes in behavioral response and population composition contributed to MDD consumption change among children in Ethiopia. The finding highlights the urgent need for targeted interventions and policies to address the issue of MDD consumption change among young children due to population structure changes like wealth status, residence, and behavioral response related to employment, household head, and sex of the child in Ethiopia. Therefore, efforts should be geared to reduce poverty and improve maternal employment status, particularly for women, by producing equitable economic opportunities.

Abstract:Background: Undernutrition among children younger than 5 years is a subtle indicator of a country's health and economic status. Despite substantial macroeconomic progress in India, undernutrition remains a significant burden with geographical variations, compounded by poor access to water, sanitation, and hygiene services. Objective: This study aimed to explore the spatial trends of child growth failure (CGF) indicators and their association with household sanitation practices in India. Methods: We used data from the Indian Demographic and Health Surveys spanning 1998-2021. District-level CGF indicators (stunting, wasting, and underweight) were cross-referenced with sanitation and sociodemographic characteristics. Global Moran I and Local Indicator of Spatial Association were used to detect spatial clustering of the indicators. Spatial regression models were used to evaluate the significant determinants of CGF indicators. Results: Our study showed a decreasing trend in stunting (44.9%-38.4%) and underweight (46.7%-35.7%) but an increasing prevalence of wasting (15.7%-21.0%) over 15 years. The positive values of Moran I between 1998 and 2021 indicate the presence of spatial autocorrelation. Geographic clustering was consistently observed in the states of Madhya Pradesh, Jharkhand, Odisha, Uttar Pradesh, Chhattisgarh, West Bengal, Rajasthan, Bihar, and Gujarat. Improved sanitation facilities, a higher wealth index, and advanced maternal education status showed a significant association in reducing stunting. Relative risk maps identified hotspots of CGF health outcomes, which could be targeted for future interventions. Conclusions: Despite numerous policies and programs, malnutrition remains a concern. Its multifaceted causes demand coordinated and sustained interventions that go above and beyond the usual. Identifying hotspot locations will aid in developing control methods for achieving objectives in target areas.

Abstract:This study analyzes data from India's National Family Health Survey to assess the prevalence of anemia among children aged 6-59 months. Over the past decade, the prevalence increased from 59% to 67%, remaining a significant public health concern. Gujarat has highest rates, and Kerala has lowest among major states in India. The Western region tops in prevalence (75.70%), whereas the Northeast has the lowest (56.20%). Factors such as lower maternal education, poverty, religion, rural residence, poor child nutrition, low birth weight, and stunting contribute to higher odds of anemia. Improved socioeconomic conditions and maternal nutrition are recommended for risk reduction.

Abstract:Guided wave focusing has been developed as a promising technique for defect detection in pipeworks, where non‐axisymmetric flexural modes are tuned so that ultrasonic energy can be focused at a target point in a pipe. If a defect is located at the target point, large amplitude reflected waves can be observed. In this study, the focusing phenomenon is analyzed using a semi‐analytical finite element method where a region of a pipe is divided in the thickness direction into the cylindrical subdivisions and is analytically treated in the circumferential and longitudinal directions. Visualization of the calculation results reveals that focusing occurs gradually in the vicinity of the target point.
Pooled Analysis of Global, Regional and Country-Level Prevalence of Childhood Stunting, Wasting and Underweight in 62 Low- and Middle- Income Countries, 2006-2018

Abstract:Background: In 2016, undernutrition as manifested in childhood stunting, wasting and underweight were estimated to cause 1.0 million deaths, 3.9% of years of life lost, and 3.8% of disability-adjusted life years (DALYs) globally. The high disease burden due to child growth failure demands regular monitoring of its prevalence in all populations. Comparative child growth failure up-to-date information on varying geographical regions around the world is essential both to quantify population health and to prompt decision-makers to proportionately allocate interventional resources. The objective of this study is to calculate the prevalence of undernutrition, including stunting, wasting, and underweight, in low- and middle- income countries (LMICs) using the 2006-2018 demographic and health surveys (DHS), and to explore the sources of regional variations.Method: We pooled prevalence from the DHS data in 62 (LMICs) worldwide that included children aged 0-59 months with information on stunting (n=624,734), wasting (n=625,230) and underweight (n=626,130). Anthropometric measurements (length/height and weight) were assessed by trained field-workers. Pooled prevalence of stunting, wasting and underweight were estimated by random-effects models. Sources of heterogeneity were explored through subgroup meta-analyses and meta-regression.Findings: In these 62 LMICs, overall prevalence was 28.4% (95% CI: 26.3%, 30.7%) for stunting, 5.4% (95% CI: 4.5%, 6.6%) for wasting, and 12.3% (95% CI: 10.4%, 14.6%) for underweight. Substantial heterogeneity was evident ( I 2 =99.9%; P for heterogeneity<0.0001). Subgroup and meta-regression analyses suggested that West Africa, South Asia and Southeast Asia had significantly higher estimated prevalence of all three forms of undernutrition. Estimated prevalence of stunting exceeds 40% among children in Burundi, Democratic Republic of Congo (DR Congo), Madagascar, Mozambique, Niger, Zambia, Yemen, Timor-Leste and Guatemala. Nigeria and Niger (West Africa), India (South Asia) and Timor-Leste (Southeast Asia) displayed the highest prevalence of wasting at approximately 20%. Similarly, the prevalence of underweight of 30% or above was observed in Niger, Bangladesh, India, Yemen, and Timor-Leste. Niger and Timor-Leste consistently displayed very high prevalence of all three forms of undernutrition. In multivariable meta-regression, a combination of malaria prevalence, national gross domestic product (GDP) per capita, and region (a proxy for spatial variation) explained 38%, 32%, and 45% of the variation in stunting, wasting and underweight prevalence respectively.Interpretation: Regional, sub-regional and country disparities in undernutrition still exist. Abstract:The article by Karlsson and colleagues 1 provides a sobering look at the high prevalence of young children aged 6 to 23 months in low- and middle-income countries who rely exclusively on breast milk or other fluids (eg, broth, juice, tea, and coffee) as their sole source of nutrition. The authors used the novel metric of zero food to classify children who did not consume any solid or semisolid food items during the 24 hours before each survey to characterize the magnitude of this problem. While exclusive breastfeeding or formula is well established as the most important feeding practice for newborns and infants in the first 6 months of life to promote optimal growth and immunocompetence protecting children against diseases including diarrhea and upper respiratory infections, 2 alone, they do not meet the dietary needs of a growing child older than 6 months. At older ages, infants and young children increase their physiological requirements for energy, protein, and micronutrients as they are increasingly exposed to infectious pathogens. Thus, it is imperative that their diet be complemented by an array of nutritious foods in addition to breastfeeding or formula, ideally a combination of foods from a diversity of food groups: grains, beans or other legumes, dairy products, meats, eggs, and vegetables and fruits, particularly those rich in vitamin A. The consumption of foods from at least 5 of these food groups constitutes a minimum dietary diversity and is associated with micronutrient sufficiency for infants and young children aged 6 to 23 months. 3 The finding that 1 in 10 children aged 6 to 23 months (10.4%) from 92 low- and middle-income countries consumed nothing but breast milk 1 indicates a type of infant starvation during this critical period of rapid physical growth and cognitive development, placing these children at increased risk of stunting and underweight and infectious diseases. Improper infant feeding and undernutrition are associated with developmental delays, cognitive impairment, as well as a heightened chance of developing chronic noncommunicable disorders later in life such as allergic diseases. This figure represents an estimated 13.9 million infants with direct consequences, not only for individual children and their families but for entire societies, as it constrains future economic growth and human capital potential. 4 The study also reveals the important geographic and cultural differences that are associated with diets, as the prevalence of zero-food diets ranged from 0.4% in Tunisia to 21.8% in Guinea and was highest in West and Central Africa (10.5%) and India (19.3%), 1 which corresponds to estimates on the prevalence of acute malnutrition, or wasting. 5 In addition, significant intracountry inequities in infant feeding exist, as differences in zero-food prevalences among the 36 states of India using National Family Health Surveys have been reported, which were associated with rates of low maternal education and household poverty. 6 It should be noted that in that study, 6 virtually all of the nationally representative surveys on child feeding included in the analysis were conducted prior to 2021 and did not fully reflect the adverse impact that the global COVID-19 pandemic and the Russia-Ukraine war may have had on food supplies and childhood malnutrition. 7 The article does a good job to cite some of the limitations of the study, including the use of national aggregate estimates that may have masked disparities and inequities among subgroups within populations, but did not mention the importance of sex, which is likely a major driver of dietary inadequacy and feeding practices in many countries. (Sex was not a significant indicator in the study in India. 6 ) The article amplified the need to identify effective strategies to improve access and affordability of nutritious foods for complementary feeding of young children. While poverty is a key driver of poor diets, it is imperative that countries prioritize efforts to strengthen social behavior change and communications to increase awareness among parents and caregivers of the importance of complementary foods along with culturally specific guidelines on the preparation of nutritious, complementary foods. While achievement of a minimum dietary diversity may be beyond the means of some families, there are ways to increase the nutrient density of diets, such as adding critical vitamins and minerals through fortification of staple foods like wheat, maize, rice, and other cereals that constitute the main source of dietary energy in many low-income households. 8 Similarly, use of iodized salt is a low-cost way to ensure optimal intake of iodine. Abstract:Immune responses to conserved, immunogenic homologues of the mycobacterial 65 kDa stress protein (SP65) have been implicated in inflammatory arthritis. Serum anti-SP65 was measured in AS, RA and healthy controls using an indirect enzyme immunoassay with recombinant SP65. IgA anti-SP65 was elevated in 19 of 59 AS patients, but the elevation in median level was not statistically significant. Anti-SP65 of all isotypes was increased in RA, but achieved significance (P less than 0.01) for IgA only. Adjusting specific antibody results for elevations in total serum Ig levels reduced AS and RA anti-SP65 to near normal levels, suggesting that a major component of the increased anti-SP65 may be secondary to polyclonal activation.
