Abstract:Objective To discuss the effect of solution focused approach model on metabolic parameters of patients with the type 2 diabetes. Methods 120 outpatients with type 2 diabetes treated in Department of Health Education,the Affilia-ted Hospital of Nantong University from October 2011 to September 2012 were selected and equally divided into experimental group and control group by means of random number table. The control group received conventional education,while the experi-mental group was intervened with solution focused approach. Patients' metabolic parameters were measured before intervention and after six months' intervention, including body mass index ( BMI ), blood pressure ( BP ), glycosylated hemoglobin (HbA1c),fasting plasma glucose(FBG),2 h postprandial glucose(2 hPBG),total cholesterol(TC),Triglyceride(TG), high density lipoprotein( HDL),low density lipoprotein( LDL),incidence rate of hypoglycemia. Results The differences of patient's BMI,SBP,DBP,HbA1c,FBG,2 hPBG,TC,TG,HDL,LDL and incidence of hypoglycemia between the two groups had no statistically significance before intervention(P>0. 05). After intervention,TC and LDL between the two group had no statistical significance(P>0. 05);BMI,SBP,DBP,HbA1c,FBG,2 hPBG,TG and the incidence rate of hypogly-cemia in experimental group were lower than those in control group and HDL was higher than that in control group(P<0. 05). Conclusion Solution focused approach model can improve patient's HbA1c,FBG,2 hPBG,TG and reduce the incidence of hypoglycemia.
Is adjunctive naturopathy associated with improved glycaemic control and a reduction in need for medications among type 2 Diabetes patients? A prospective cohort study from India

Abstract:BackgroundWith an estimated 65 million Diabetes Mellitus (DM) patients, India ranks second in the world in terms of DM burden. The emphasis of current medical practice has been on pharmacotherapy but, despite the best combination therapies, acheiving glycaemic control (reduction of blood sugar to desirable levels) is a challenge. ‘Integrated Naturopathy and Yoga’(INY) is an alternative system of medicine that lays emphasis on the role of diet and physical exercise. We assessed the short term effect of INY as an adjunct to pharmacotherapy on glycaemic control among type 2 DM patients.MethodsIn this prospective cohort study with a 3 month follow-up, DM patients consecutively admitted to a hospital in India from May-October 2014 for either 15 or 30 days were offered INY - a package of vegetarian diet with no added oil, sugar and salt, yoga-based exercise, patient counselling and rest. A ‘favourable outcome’ was defined as glycaemic control (glycosylated hemoglobin (HbA1c) < 7 % or absolute reduction by 1 %) along with at least 50 % reduction in antidiabetes medication at 3 months relative to baseline. Compliance to diet was scored by self-report on a scale of 0–10 and categorized into poor (0–5), moderate (6–8) and excellent (9–10).ResultsOf 101 patients with 3-month follow-up data, 65(65 %) achieved a favourable outcome – with 19(19 %) stopping medication while sustaining glycemic control. Factors associated with favourable outcome were baseline HbA1c and compliance to diet, which showed a significant linear relationship with mean HbA1c reductions of 0.4 %, 1.1 % and 1.7 % in relation to poor, moderate and excellent dietary compliance respectively.ConclusionINY, adjunctive to pharmacotherapy, was associated with a significant beneficial effect on glycaemic control and reduced the overall need for antidiabetes medications. These early results are promising. Further studies with long-term follow-up and using more rigorous randomized controlled trial designs are needed.

Abstract:Background: Type 2 diabetes is a lifestyle-related disorder that affects around 422 million individuals in India. Integration of AYUSH (Ayurveda) with the National Programme for Prevention and Control of Cancer, Diabetes, Cardiovascular Diseases and Stroke (NPCDCS) was conceived on pilot basis at Gaya, Bihar, to provide integrative treatment for non-communicable disease patients and to manage the burden of non-communicable diseases in India. Objectives: The objective of this study was to analyze the effect of Ayurveda intervention, lifestyle modification and Yoga in the management of type 2 diabetes under NPCDCS-AYUSH integration project. Materials and Methods : A multi-centric, open-labeled, prospective, comparative clinical study was conducted at 17 community health centers and 1 district hospital. Population over 30 years of age was screened and prediabetic or type 2 diabetic individuals were enrolled in two cohorts, i.e., pre-diabetic (Cohort A) and type 2 diabetic (Cohort B). Each cohort was further divided into two groups: Group A1 was advised for lifestyle modification and Yoga and group A2 was given Ayurveda medication in addition to lifestyle modification and Yoga . Similarly, group B1 was advised for lifestyle modification and Yoga along with allopathic medication and group B2 was given Ayurveda medication, i.e., Mamajjaka , Amalaki and Guduchi powder in addition to lifestyle modification and Yoga along with allopathic medication. Treatment was given for 6 months. Data were analyzed through paired t -test. Results: A significant reduction was observed in fasting blood sugar level in groups A2 and B2 ( P = 0.001) and also in the postprandial blood sugar level in Groups A2 and B2 ( P = 0.001). Further, improvement in subjective symptoms such as polyuria, polydipsia, polyphagia, blurred vision and weakness was found in all the groups, while non-healing ulcer does not show any improvement. Conclusion: The study reveals that Ayurveda intervention, i.e., Mamajjaka Churna (1 g), Amalaki Churna (3 g) and Guduchi Churna (3 g) two times a day effectively controls blood sugar level in pre-diabetic and type 2 diabetic patients and improves the disease management with lifestyle modification and Yogasana as well as with allopathic treatment.

Abstract:Background Type II diabetes mellitus (T2DM) has become a worldwide public health problem. Although it has been empirically established that physical activity is a promising therapeutical approach to the prevention and management of T2DM, the effectiveness of yoga on T2DM has not yet reached an agreement across studies and also needs an updated synthetic examination. Purpose The purpose of this study was to examine the effect of yoga training on diabetes-related indicators compared with usual care. Methods The review protocol of this study has been registered in the PROSPERO with a registration number CRD42021267868. A systematic literature search through electronic databases was conducted to identify yoga-based intervention (i.e., randomized controlled trial [RCT]; e.g., yogic postures, movements, breathing, and meditation) studies reporting outcomes on glycosylated hemoglobin (HbA1c), fasting blood glucose (FBG), postprandial blood glucose (PPBG), total cholesterol (TC), triglycerides (TG), and body mass index (BMI). A number of two researchers manually reviewed and assessed each article using the Cochrane Risk of Bias Tool 2.0. The literature search identified 296 eligible entries, of which 13 were finalized after screening using predefined inclusion and exclusion criteria. The extracted data (group mean and standard deviation at posttest) were synthesized using random-effects meta-analyses. Finally, potential moderators were explored using subgroup analysis and sensitivity analysis. Results The standardized mean difference for the effects of yoga was significant on HbA1c (MD = −0.47; 95%CI: −0.77, −0.16; Z = 3.02, p = 0.003), FBG (SMD = −0.92; 95%CI: −1.55, −0.29; Z = 2.87, p = 0.004), PPBG (SMD = −0.53; 95%CI: −0.86, −0.21; Z = 3.20, p = 0.001), and TG (SMD = −0.32; 95%CI: −0.54, −0.10; Z = 2.86, p = 0.004). However, yoga effect was not observed on TC (SMD = −0.84; 95%CI: −1.71, 0.04; Z = 1.87, p = 0.06) and BMI (MD = −0.63; 95%CI: −1.42, 0.16; Z = 1.57, p = 0.12). Conclusion The findings suggest that yoga can improve the biochemical indices of blood glucose and the lipid profile of patients with T2DM. Therefore, yoga can be prescribed as an effective and active complementary treatment for T2DM. However, this study only tested yoga as a short-term treatment. In the future, rigorous RCTs with a larger sample size may be carried out to examine the long-term effect of yoga on T2DM. Reversal of type 2 diabetes mellitus through integrated Ayurveda dietary protocol - A case report

Abstract:Type 2 Diabetes Mellitus (T2DM) is a chronic metabolic disorder of hyperglycemia has close link with obesity and insulin resistance. Standard approaches in T2DM management are lifestyle management and Pharmacotherapy. Dietary management in T2DM was found to be safe and effective. In India, studies on reversal of T2DM through diet were less found. Presenting a case of female aged 31 years with T2DM (HbA1c-13.5) was successfully reversed diabetes with dietary principles of Indian traditional medicine (Ayurveda diet) and therapeutic yoga practices. Pre and post assessment of Glycaemic parameters (HbA1C, FBS, and PPBS), Lipid profiles, Insulin resistance parameter (HOMA IR) were done. Major outcome of this case is greater reduction of HbA1c from inadequate control to near normal. Marked changes observed in weight, BMI and Waist circumference. A change in Homa IR indicates improvement in insulin resistance. Ayurveda dietary management training include Therapeutic fasting [Two meal method], meals include nutrient dense wholesome food, Indian millet based diet, herbal recipes [therapeutic food/herbal drinks, smoothies and plant juices] and eat only when hungry method. Yoga include complete sequel of body loosening practices, surya namaskara, yogic postures and breathing and meditative techniques practiced for specified period [60 min].This case showed effectiveness of Ayurveda diet and Yoga practices in reversing the insulin resistance, help to maintain the glycemic parameters .In addition, patient was able to loose body weight, waist circumference & serum triglyceride levels. Patient can able to withstand her diabetic profile in normal without any pharmacotherapy intervention.

Abstract:Objectives: The objective of the study is to evaluate the short-term effect of yoga on plasma glucose, lipid profile, blood pressure and insulin requirement in a patient with type 1 diabetes mellitus (T1DM). Case presentation: A 28-years old female was diagnosed with T1DM at the age of seven. She was under Human Mixtard insulin 30-70, thrice/day for 15-15-15 units. She had a history of poor glycemic control, diabetic ketoacidosis and hypoglycemic shock. At the age of 27, she got admitted with complaints of general weakness, joints stiffness and weight loss in our holistic health centre and underwent two weeks of yoga. Results showed a better reduction in the plasma glucose levels, lipid profile, blood pressure and insulin dose and an increase in hemoglobin levels with no episodes of hypoglycemia after yoga. Conclusions: Yoga might be considered as a safer and effective adjuvant in the management of T1DM.
Effect of Yoga and Naturopathy-Based Lifestyle Intervention on Inflammatory Markers on Obese Individuals: A Prospective Randomized Controlled Trial

Abstract:Background: Obesity is characterized by abnormal accumulation of fat in adipose tissue. It is a major risk factor for metabolic syndrome caused by chronic inflammatory changes. Naturopathy and yogic intervention are a holistic approach. So, the present study aims to assess the effect of naturopathy and yogic intervention on inflammatory markers among obese individuals. Methods: In this randomized controlled trial, 262 subjects were recruited and randomized into either an experimental or control group with 131 subjects in each group aged 35 ± 9.6 years in experimental and 39.3 ± 9.6 years in control group. The experimental group received 10 days of naturopathy and yogic treatment. The control group didn't receive any naturopathy or yogic treatment. Complete blood count (CBC) was tested at baseline and after 10 days of intervention. Results: The study showed a significant reduction in white blood cells (WBC), Neutrophils, and Lymphocytes with mean values of 1.63 ± 0.38, 1.14 ± 0.25, 0.4 ± 0.09 and an increase in Platelet lymphocyte ratio (PLR) with mean value 19.84 ± 4.97 respectively with a P < .001 in experiment group when compared to the control group. The experimental group also showed a reduction in Platelet and Neutrophil lymphocyte ratio (NLR) with a mean value of 11.28 ± 3.5 & 0.18 ± 0.03 with a P = .01 when compared to the control group. Conclusion: Obesity being a major risk factor for metabolic syndrome could be safely and effectively treated with naturopathy and yogic intervention by alleviating the inflammatory markers.
Yoga and nutritional therapies for promoting health-related quality of life in persons with metabolic syndrome-An interventional observational study

Abstract:Introduction: A group of related risk factors known as central obesity, elevated blood pressure, impaired fasting glucose, elevated triglycerides (TGs), and low levels of high-density lipoprotein (HDL) are collectively referred to as metabolic syndrome (MetS). A low quality of life (QoL) in terms of health is linked to MetS. This study sought to determine how lifestyle modifications affected the health-related quality of life (HRQoL) of primary care patients with MetS. Materials and methods: In this study, we randomized 300 subjects diagnosed with MetS as per the National Cholesterol Education Program Adult Treatment Panel III (NCEP ATP III) criteria, into control, yoga, and yoga + diet groups according to their intervention which was given for 6 months. Socio-demographic, anthropometric, and physiological and biochemical parameters were collected at baseline, 3 months, and 6 months. The 36-Item Short Form Health Survey (SF-36) was also used to assess HRQoL. Results: The results of this study indicate that yoga + diet and yoga intervention led to significant decrease in metabolic risk factors, such as waist circumference (WC), systolic blood pressure (SBP), diastolic blood pressure (DBP), fasting blood glucose (FBG), TGs, and increase in HDL at 3 months and 6 months from baseline. SF-36 score also showed increment (from 0 score to 100) at 3 months and 6 months, which indicates improvement in HRQoL. Conclusion: Dietary changes and yoga are useful strategies for reducing the risk of MetS, which improves QoL in relation to health. Incorporating yoga practices and advocating a balanced diet within primary care systems can significantly benefit individuals with or at risk of MetS.

Abstract:Clinical EEG and Neuroscience, Ahead of Print. Introduction. Type 2 diabetes patients are more likely to experience cognitive decline (1.5%) and dementia (1.6%) than healthy individuals. Although cognitive impairment adversely affects Type 2 diabetes mellitus (T2DM) patients, it is the least addressed complication of T2DM patients. Objective. The present study attempts to examine the changes in cognitive performance of T2DM patients and the probable factors contributing to the changes following 12-week yoga practice. Methods. The current study is a parallel group randomized controlled trial that compared the outcomes of the participants randomized to a yoga group (YG) (n = 25) and to a wait-list control group (n = 29). The study assessed N200 and N450 event-related potential (ERP) components following the Stroop task, heart rate variability (HRV) and HbA1c before and after the intervention. Results. The mean amplitude of the N200 ERP component showed a significant group difference after the intervention, demonstrating an improved neural efficiency in the process of conflict monitoring and response inhibition. No differences were present for the N450 component. T2DM patients showed reduced heart rate and increased mean RR following yoga practice without any corresponding changes in other HRV parameters, demonstrating an overall improvement in cardiac activity. Along with that yoga practice also reduced HbA1c levels in T2DM patients, indicating improved glycemic control. Moreover, HbA1c levels were negatively correlated with reaction time after the intervention, indicating an impact of glycemic control on cognitive performance. Conclusion. The 12-week yoga practice improved cognitive performance by enhancing the processes of conflict monitoring and response inhibition. Further, improved cognitive performance postintervention was facilitated by improved glycemic control.

Abstract:Peripheral neuropathy, a form of nerve damage brought on by diabetes and abnormally high blood sugar, was one of the chronic consequences. Studies reveal that the prevalence of Diabetes Peripheral Neuropathy (DPN) is high and riskier in Males than Females. Also, DPN participants have been examined crucially for physiological variables such as systolic and diastolic blood pressure and body mass index. This study aimed to analyze the potency of Yoga therapy on physiological variables in Males of the age group 35 to 70 having DPN. Yoga connects a person's physical, mental, and spiritual aspects to enhance their health and well-being. It has been proven to treat neurological diseases using Yoga therapy by combining the practice of focused posture (asana), controlled breathing (Pranayama), and meditation. Studies proved 30 males with DPN, 15 in each group, were chosen for the study from the Chennai & Chengalpattu district, Tamilnadu, India. The Experimental Group, Group-I, received a Yoga therapy practice and the control group, Group II, underwent it without any practice. The 12-week training period, six days per week, an hour in the morning. Paired sample t-test was conducted to analyze the data. The calculated 't' value of systolic, diastolic and BMI are 3.486, 2.828, and 1.060, respectively, for Group I and -0.371, -1.547, and -1.792 for Group II. The result shows that the negative t value of Group II shows no significant change in the physiological variables, and Group I data shows a significant improvement in the physiological variables. The calculated 't' value is greater than the table value of 2.14 with a degree of freedom of 14, except for BMI. An extension of the training period is suggested to improve the BMI value. This research study reveals that Yoga Therapy was a significantly more effective treatment for DPN.

Integrated Yoga Practice in Cardiac Rehabilitation Program: A Randomized Control Trial

Abstract:Background: Coronary artery disease (CAD) is a detrimental noncommunicable disease, which is increasing due to sedentary lifestyle and urbanization in the young population. It is further elevated with risk factors such as stress, anxiety, depression, an increase in triglycerides, dyslipidemia, hyperglycemia, hypertension, and so on, which manifests as atherosclerotic disease. Yoga-based lifestyle intervention is a noninvasive effective treatment method to control and prevent cardiac risk factors in CAD patients. Yoga has been used in India as a therapeutic method to manage hypertension and other chronic disorders and is fast gaining popularity as an effective means for the alleviation of stress, improvement of fitness, and enhancement of well-being. This study aimed to determine the feasibility of introducing the integrated approach of yoga therapy (IAYT) in a cardiac rehabilitation center in India and understand its usefulness in improving the cardiac function and managing the cardiac risk factors in acute myocardial infarction patients with left ventricular dysfunction. Methods and Design: Cardiac patients were randomized to a yoga-practicing group (n = 33) and a control group (n = 33). The yoga-practicing group was instructed to attend three supervised IAYT classes 3 days per week for 12 weeks at the hospital yoga center. The control group received standard care that included pharmacologic treatment and the instructions of the cardiologist. The outcome measures were assessed at baseline (T1 = 0) and completion (T2 = 3 months). The primary outcome measure was the left ventricular ejection fraction (LVEF). Results: There was no statistically significant difference in LVEF (U = 420.500, p value = 0.218) between the two groups. However, the yoga-practicing group showed significant reduction in depression (Cardiac Depression Scale [CDS], U = 71, p value = 0.0), anxiety (Hamilton Anxiety Rating Scale [HAM-A], U = 128, p value = 0.0), and a significant increase in quality of life (QOL) scores (Duke Activity Status Index [DASI], U = 146, p value = 0.0; and metabolic equivalents (METs), U = 136, p value = 0.0) at 3 months compared to control. Overall, the CAD patients practicing yoga showed a favorable profile compared to control individuals on CDS, HAM-A, DASI, and MET outcomes. Control and yoga practicing groups did not differ significantly in the lipid levels. Conclusion: This study indicated that the integration of yoga practice in a cardiac rehabilitation program is feasible and has no added benefit in improving the cardiac function. However, the addition of yoga to cardiac rehabilitation may be beneficial in reducing depression and anxiety and improving QOL in patients.

Abstract:Many Indians are at high risk of type 2 diabetes mellitus (T2DM). The blood glucose level can be improved through a healthy lifestyle (such as physical activity and a healthy diet). Yoga can help in T2DM prevention, being a culturally appropriate approach to improving lifestyle. We developed the Yoga Programme for T2DM Prevention (YOGA-DP), a 24-week structured lifestyle education and exercise (Yoga) program that included 27 group Yoga sessions and self-practice of Yoga at home. In this study, the feasibility of undertaking a definitive randomized controlled trial (RCT) was explored that will evaluate the intervention's effectiveness among high-risk individuals in India.

Abstract:Aims Type 2 diabetes mellitus (T2DM) alters brain functional and structural connectivity leading to cognitive dysfunction. This study examined the effect of a 12-week yoga intervention on prefrontal cortex (PFC) oxygenation and working memory in patients with T2DM. Methods Fifty participants were randomized into yoga and waitlist control groups. The yoga protocol specific to T2DM was followed. The pre- (day 1), mid- (6 weeks) and post-intervention (12 weeks) assessments included measurement of PFC oxygenation while performing working memory tasks ( n -back) using functional near-infrared spectroscopy. Results Following a 12-week intervention, the yoga group showed improved performance in working memory [accuracy (geometric mean difference of 3.15%, 95% CI [2.33,3.96], p = 0.001) and reaction time (mean difference of 100.8 milliseconds,95% CI [− 166.6, − 35.1], p = 0.002] in the high task load (2-back) associated with higher oxygenation in dorsolateral PFC ( β coefficient mean difference of 95.6, 95% CI [0.23,191], p = 0.049) and ventrolateral PFC ( β coefficient mean difference of 53.4, 95% CI [7.8,98.9], p = 0.018) regions. Higher oxygenation in dorsolateral PFC during 2-back task was positively correlated with accuracy (r(23) = 0.65, p < 0.001) and negatively correlated with reaction time (r(23) = − 0.47, p = 0.017). Conclusion Integrated yoga practice may improve working memory performance associated with higher PFC oxygenation in patients with T2DM. As 12 weeks of yoga intervention improved working memory performance, the regular practice of yoga may have the potential to prevent decline of cognitive functions in clinical conditions.

Abstract:BACKGROUND: The increasing of diabetes mellitus cases in Indonesia may be due to changes in people’s lifestyles, which is lack of exercise. Factors that may contribute to the high prevalence of diabetes mellitus sufferers other than genetics are exacerbated by environmental factors. Exercise interventions have been effective in counterbalancing diabetes complications. Yoga exerts a beneficial effect on insulin kinetics and the resulting lipid profile. Yoga helps redistribute body fat and reduces obesity which may lead to insulin resistance. In addition, other types of physical activity that can be done by diabetes mellitus patients are adequate morning walking while having scenery leisure for 30 min or more. AIM: This study was conducted to determine the effect of yoga therapy and walking therapy on diabetes mellitus patients in the community. METHODS: Quantitative research using randomized control trial, conducted in Gedongan Village, Baki Subdistrict, Sukoharjo Regency, involving 54 samples divided into three groups. Inclusion criteria: Patients with type 2 diabetes mellitus, fasting glucose levels of 126 mg/dl, no complication, and no insulin therapy or diabetes medication. The outcome measured was fasting blood sugar levels. The intervention group performed therapy for 12 weeks with a duration of 3 times a week. RESULTS: The average fasting sugar levels in the yoga group pre (217.00) post (187.72) p (0.001), the walking group averaged pre (209.89) post (193.83) p (0.001), and the control group averaged pre (221.50) post (225.17) p (0.067). CONCLUSION: There was an effect of yoga therapy and walking therapy on reducing fasting glucose levels.

Abstract:Objective: Impaired glucose control & Insulin resistance are reported to be risk factors for the development of cardiovascular diseases. To find the effects of a structured exercise program on insulin resistance, glycaemic control, functional capacity, and quality of life in patients with Type 2 diabetes mellitus. Design: Randomized, controlled trial. Setting: Diabetic Foot Clinic, Department of Physiotherapy & Department of General Medicine, Kasturba Hospital in Manipal, Karnataka, India. Participants: 160 participants aged between 30-65 years with Type 2 diabetes mellitus. Intervention: A set of structured exercise programs (aerobic, resistance, and combined) along with the standard hospital care was performed 3-5 times weekly for 12 weeks. Measurements: primary outcome measures: Fasting Insulin Level, Homa-IR, Six-minute walk test (6MWT), and WHOQOL-BREF questionnaire at baseline and 12th week. Secondary outcome measures: Body composition analysis, Fasting Blood Sugar, Postprandial Blood Sugar, Glycated Haemoglobin (HbA1c), and GPAQ questionnaire at baseline and 12th week. Results: Significant differences have been observed in Homeostasis model assessment for insulin resistance (Homa-IR) (F (1, 144) = 89.29, p < 0.001); Fasting insulin (FI) (F (1, 144) = 129.10, p < 0.001); Fasting blood sugar (FBS) (F (1, 144) = 12.193, p< 0.001); Post prandial blood sugar (PPBS) (F (1, 144) = 53.015, p< 0.001); glycated haemoglobin (HbA1c) (F (1, 144) = 80.050, p < 0.001); WHOQOL-Physical health (F (1, 144) = 20.008, p< 0.001), Psychological (F (1, 144) = 77.984, p< 0.001), Social relationship (F (1, 144) = 44.866, p< 0.001); Environmental (F (1, 144) = 69.974, p< 0.001); Six minute walk test (6MWT) (F (1, 144) = 84.135, p< 0.001) in the study group when compared with the control group from baseline to 12th week. Conclusions: The study reveals that a 12-week structured exercise training program effectively reduces insulin resistance, improves quality of life, enhances functional capacity, and improves glycaemic control in type 2 diabetes mellitus.

Abstract:Objective: Diabetes mellitus (DM) is known as the silent pandemic. It is hypothesized that other endocrine systems are affected by the metabolic changes occurring due to DM. We aimed to investigate the correlation of thyroid hormones with glycaemic and lipid parameters. Materials and Methods: 81 diabetic patients and 81 non-diabetic age and sex-matched healthy volunteers participated in the study. Their blood samples were analysed for fasting blood glucose (FBG), glycosylated haemoglobin (HbA1C), total tri-iodothyronine (T3), total thyroxine (T4), free T3 (FT3), free T4 (FT4), thyroid-stimulating hormone (TSH), total cholesterol (CHOL), High-Density-Lipoprotein cholesterol (HDLC) and Low-Density-Lipoprotein cholesterol (LDLC). Data was analysed using appropriate statistical tests. Results: Among the cases, 70.37% were euthyroid, while 24.7% had subclinical hypothyroidism, 2.47% had clinical hypothyroidism, 1.23% had subclinical and 1.23% had clinical hyperthyroidism. FBG, HbA1c and TSH (P< 0.05) were significantly higher in diabetics compared to controls. On the other hand, T3, FT3, FT4, and HDLC (P< 0.05) were significantly lower in diabetics compared to controls. A significant negative correlation (P< 0.05) was found when T3 and FT3 were compared against age, FBG and HbA1c. A significant positive correlation (P< 0.05) was found when T3 and FT3 were compared against HDLC, LDLC & CHOL. Conclusion: Our statistics show that high-normal levels of T3 and FT3 are correlated with lower levels of FBG and HbA1c, hence improved glycaemic control. We recommend that thyroid profile of diabetic patients with poor control should be monitored regularly. Early detection of thyroid dysfunction and initiation of therapy for it, can improve the treatment outcome of hypoglycaemic drugs.
Effects of Nonsurgical Periodontal Therapy on Glycemic Control in Diabetic Patients under Systemic Administration of Antidiabetic Ayurvedic Drug

Abstract:Aim: This study aimed to monitor the metabolic response of nonsurgical periodontal therapy in type-2 diabetic patients with chronic periodontitis under systemic administration of antidiabetic ayurvedic drug. Materials and methods: About 90 newly diagnosed mild-to-moderate forms of type-2 diabetes mellitus (DM) subjects with generalized chronic periodontitis were selected according to the inclusion and exclusion criteria and were randomly divided into group A and group B with 45 patients in each group. Clinical parameters, including plaque index, probing pocket depth, clinical attachment level, and glycemic status, were assessed at baseline. Following initial periodontal clinical examination, the drug Nishamalaki (NA) 2 gm twice daily for 3 months was prescribed after food by an Ayurvedic physician to all the patients enrolled in group A and group B, and scaling and root planing were completed only for group B patients. Patients were recalled for review, and all the parameters were reassessed at the end of the 1st, 2nd, and 3rd months following interventions. Results: Regarding clinical and metabolic parameters at baseline, no statistically significant differences were displayed between the two groups. However, at the 3-months follow-up period, the patients in group B demonstrated significantly better clinical and metabolic outcomes than patients in group A. Conclusion: Periodontal therapy improved glycemic control in patients with type-2 DM in both groups; however, the reduction in FBS values reached statistical significance only in the group receiving scaling and root planing alone. Clinical significance: Nonsurgical periodontal therapy may have a beneficial effect on the periodontal clinical and glycemic levels in type-2 diabetic patients with chronic periodontitis.

Abstract:oday, among metabolic disorders, diabetes can be considered the most common, increasing, serious, and costly public health problem in the World. Among them, microangiopathies leading to nephropathies, optic nerve damage associated with retinopathies, and diabetic peripheral neuropathy (DPN) associated with pain, tingling, or numbness, loss of feeling in the hands, arms, feet, and legs can occur primarily due to uncontrolled diabetes levels. The main reason to visit in a primary care setting is chronic pain. Moreover, it is difficult to treat because of its severity, chronicity, and resistance to simple analgesics. Non-pharmacological approaches and non-conventional treatments such as naturopathy, yoga, and diet modification are used to relieve pain, improve well-being, and treat acute, chronic, and degenerative conditions of peripheral neuropathy associated with Type 2 Diabetes Mellitus.

Abstract:Background: Type 2 diabetes (T2D) is a chronic metabolic disorder that has a notable influence on mental well-being, contributing to elevated morbidity and mortality rates. Depression and anxiety disorders are the most common mental health concerns among patients with T2D worldwide. Therefore, the present study aimed to assess the impact of an online intensive lifestyle intervention (ILI) on mental health outcomes (depression and anxiety) in patients with T2D in India. Materials and methods: This retrospective pre-post analysis included adult patients (aged > 18 years) diagnosed with T2D who were enrolled in a diabetes management program in India between June 2021 and June 2023. The intervention consisted of lifestyle modifications such as a customized plant-based diet, regular physical activity, psychological support through group and individual therapy, and medical management. Data were extracted from the electronic database of the clinic, including anthropometry, medical history, biochemical parameters, symptoms of depression, and anxiety (assessed using the Patient Health Questionnaire-9 (PHQ-9) and Generalized Anxiety Disorders-7 (GAD-7) scale) at the start and end of the six-month intervention period. Results: Of the 1061 eligible patients (177 with prediabetes), 40.3% were female. The mean age, duration of diabetes, and HbA1c levels were 52 ± 10.4 years, 9.8 ± 7.8 years, and 8 ± 1.7%, respectively. The prevalence of symptoms of depression and anxiety (ranging from mild to severe) was 46% and 44.3%, respectively, which reduced to 28.7% and 29.2%, respectively, post-intervention. Conclusion: Integrated ILI successfully improved the symptoms of anxiety and depression, highlighting the importance of a multidisciplinary approach that includes diet, physical activity, psychological support, and medical management in enhancing mental health outcomes among patients with T2D. Future prospective studies are needed to explore the long-term effects of such interventions and develop effective strategies for promoting mental health in diverse populations. Trial registration: The study was approved by the Freedom from Diabetes Research Foundation Institutional Ethics Committee (approval number FFDRF/IEC/2024/7) and registered in the Clinical Trials Registry of India (CTRI/2024/03/064596, registered on March 21, 2024).
