Abstract:Summary Background Earlier death among people in socioeconomically deprived circumstances has been found internationally and for various causes of death, resulting in a considerable life-expectancy gap between socioeconomic groups. We examined how age-specific and cause-specific mortality contributions to the socioeconomic gap in life expectancy have changed at the area level in Germany over time. Methods In this ecological study, official German population and cause-of-death statistics provided by the Federal Statistical Office of Germany for the period Jan 1, 2003, to Dec 31, 2021, were linked to district-level data of the German Index of Socioeconomic Deprivation. Life-table and decomposition methods were applied to calculate life expectancy by area-level deprivation quintile and decompose the life-expectancy gap between the most and least deprived quintiles into age-specific and cause-specific mortality contributions. Findings Over the study period, population numbers varied between 80 million and 83 million people per year, with the number of deaths ranging from 818 000 to 1 024 000, covering the entire German population. Between Jan 1, 2003, and Dec 31, 2019, the gap in life expectancy between the most and least deprived quintiles of districts increased by 0·7 years among females (from 1·1 to 1·8 years) and by 0·1 years among males (from 3·0 to 3·1 years). Thereafter, during the COVID-19 pandemic, the gap increased more rapidly to 2·2 years in females and 3·5 years in males in 2021. Between 2003 and 2021, the causes of death that contributed the most to the life-expectancy gap were cardiovascular diseases and cancer, with declining contributions of cardiovascular disease deaths among those aged 70 years and older and increasing contributions of cancer deaths among those aged 40–74 years over this period. COVID-19 mortality among individuals aged 45 years and older was the strongest contributor to the increase in life-expectancy gap after 2019. Interpretation To reduce the socioeconomic gap in life expectancy, effective efforts are needed to prevent early deaths from cardiovascular disease and cancer in socioeconomically deprived populations, with cancer prevention and control becoming an increasingly important field of action in this respect. Abstract:Background While social disparities in mortality have been shown in the majority of high-income countries, research on inequalities in the German population is still limited. This applies especially to studies investigating time trends in social inequalities with respect to life expectancy. The aim of this study is to examine income inequalities in life expectancy and whether inequalities have narrowed or widened over time. Methods The analyses are based on the claims data of a large German health insurance provider, which facilitates the combining of information on individual income and mortality. Life expectancy is calculated separately for three income groups (<60%, 60% to 80% and ≥80% of the average income in Germany) and for sex by applying period life table analyses. Trends are assessed by comparing the time periods 2005–2008 (N = 1 773 122), 2009–2012 (N=1 792 735) and 2013–2016 (N = 1 987 114). Results Trends in life expectancy differed by sex, age and income group. Especially among elderly men, the gap between low- and high-income groups widened over time, disadvantaging men with low incomes. Among women, a slight reduction in inequalities was observed, which was driven by the increases in life expectancy in low-income groups. Conclusion Our study shows that not all population subgroups benefitted equally from the continuing rise in life expectancy. The persisting inequalities emphasise the importance of public health efforts concentrating on reducing mortality risks among individuals in lower socioeconomic positions. Special attention should be paid to elderly men with low incomes. Further research is needed on the mechanisms underlying increasing health inequalities over time.

Abstract:Regional mortality differences are one dimension of health inequalities, but its trends and determinants in Germany are widely unknown. This book examines and illustrates patterns of regional mortality in Germany—with focus on small-area differentials—and their changes over time. It identifies explanatory factors at individual and regional level. Mortality differences between eastern and western Germany exist, but small-area mortality differentials are often greater. Though the main spatial mortality patterns remain, this study provides evidence that some distinct changes in the small-area mortality patterns in Germany—especially among women—occurred within a short period of time. Mortality inequalities at younger ages and in behavior-related causes as well as differences in socioeconomic conditions contribute strongly to regional mortality differences in Germany. The book shows that the complex interplay between individual- and regional-level mortality risk factors requires a multidimensional approach to reduce regional mortality inequalities.

Abstract:To investigate the alpha 1-adrenoceptor subtype(s) mediating contraction in rat renal artery, we have compared the effect of the alpha 1-adrenoceptor antagonists, 5-methylurapidil, BMY 7378 (8-(2-(4-(2-methoxyphenyl)-1-piperazinyl) ethyl) 8-azaspiro (4.5) decane-7,9-dione 2HCl) and chloroethylclonidine on functional responses to noradrenaline. A clear blockade by chloroethylclonidine (10(-4) M) of noradrenaline-induced contraction was observed and, along with this effect. pKB values of 9.12 and 8.40 for BMY 7378 and 9.75 and 10.06 for 5-methylurapidil were obtained, indicating that the renal artery expresses the alpha 1D-adrenoceptor subtype as the one involved in contraction and not only the alpha 1A subtype as has been reported.
Alcohol use in hazardous situations: implications for DSM-IV and DSM-5 alcohol use disorders.

Abstract:BACKGROUND The use in hazardous situations criterion (hazard) is 1 of 4 criteria related to alcohol abuse in the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV), and 1 of the 11 criteria related to the new alcohol use disorder (AUD) proposed for DSM-5. The current study aims to evaluate the hazard criterion in the context of both DSM-IV alcohol abuse and DSM-5 AUD. METHODS Data came from the 2007 Australian National Survey of Mental Health and Well-Being (n = 8,841) as a stratified, multistage area probability sample of persons aged 16 to 85 years. Logistic regressions were used to: (i) compare the clinical characteristics of those with alcohol abuse including hazard and those with alcohol abuse because of other criteria; (ii) investigate the relationship between the hazard criterion and indices of socioeconomic status (SES); and (iii) investigate the effect of eliminating the hazard criterion on the epidemiology and correlates of the proposed DSM-5 AUD diagnosis. RESULTS When compared with the other abuse subgroup, those with abuse including hazard (irrespective of other abuse criteria endorsed) were more likely to report another drug use disorder. The 2 abuse subgroups could not be differentiated by any other clinical characteristics. There were no systematic relationships between the hazard criterion and indices of SES. The elimination of the hazard criterion would lead to a considerable decrease in the prevalence of AUD, with those no longer receiving a diagnosis more likely to be young males with drug use disorders and suicidal behaviors. CONCLUSIONS The current study failed to replicate previous analyses that indicated problems with the hazard criterion when assessed in the U.S. population. Many of the problems identified in the hazard criterion appear to be due to operationalizations of this criterion that includes items specifically related to drink-driving. The current results indicate that the elimination of the hazard criterion would lead to a considerable decline in the prevalence of DSM-5 AUD and risk excluding a potentially clinically significant subtype of AUD from future diagnosis.
Effect of Progestagen/PMSG Treatment for Oestrus Synchronization of Tuj Ewes to be Bred after the Natural Breeding Season

Acta Vet. Brno 74, 2005: 385-393. The aim of this study was to investigate the effect of pregnant mare serum gonadotrophin (PMSG) and progestagen sponge on oestrus synchronization of Tuj ewes mated after breeding season. Body weights (b.w.) and condition scores (BCS) of seventeen mature fat-tailed Tuj ewes were recorded prior to the experiment and they were divided into two compared groups. All the ewes were treated by 30 mg Chronolone-impregnated intravaginal sponges for 14 days. Ewes in the first group (Group I, n = 9) were then injected with i.m. 600 I.U. PMSG upon sponge withdrawal while others, as control (Group II, n = 8) received placebo injection only. Ewes were monitored (at 2 h intervals) for both the signs of oestrus behaviour (classified as attractivity and receptivity) and their durations over the 102 h. Non-return rates were monitored (twice daily) during day 14-21 with the aid of a fertile ram. Lambing rates and litter sizes were recorded following the 150 ± 5 days of inseminations (hand-mating). Results showed that oestrus period was significantly prolonged (P < 0.05) in Group I. Additionally, a positive significant correlation (P = 0.021, R2 = 0.345) was found between BCS in situ and the litter size, regardless of PMSG administration. Furthermore, regardless of the nature of oestrus (induced or natural) in all the ewes, there was a profound effect of BCS per se on the overall litter size (P = 0.000, R 2 = 0.633). It was significantly higher in ewes that had BCS higher than both 2.0 (P = 0.001, R2 = 0.633) and 2.5 units (P = 0.001, R2 = 0.654). In conclusion, PMSG combined with progestagen sponge administration for oestrus synchronization prolonged the duration of oestrus period but with no marked effects on the non- return rate, lambing rate or the litter size in Tuj ewes mated after breeding season. Social protection expenditure on health in later life in 20 European countries: Spending more to reduce health inequalities

Abstract:Background: This study aims to examine whether higher social protection expenditure reduces the negative association of life-course socioeconomic disadvantages with subjective and objective health status and trajectories in later life. Methods: We used SHARE data from participants living in 20 European countries aged 50 to 96. Seven waves allowed to examine the trajectories of health inequalities in later life. We used linear mixed-effects models stratified by sex to examine the association between life-course socioeconomic disadvantage and subjective (self-rated health, SRH, N = 55,443) and objective (grip strength, N = 54,718) health. Cross-level interactions between net social protection expenditure as percentage of gross domestic product and life-course socioeconomic disadvantage tested for the moderating effect of social expenditures on the association of disadvantage with SRH and grip strength in later life. Findings: Higher social protection expenditure reduced socioeconomic health inequalities in both men and women for grip strength, and in women but not men for SRH. For SRH, the health-inequality-reducing effect of social protection expenditure became weaker with increasing age. This was not observed in grip strength. Some separate expenditure functions (disability, family and children) were found to have inequality-widening effects in men's and women's SRH, which were either offset or overcompensated by the other functions. No inequality-widening effects were observed in grip strength. Interpretation: Higher social spending reduces life-course socioeconomic inequalities in women's subjective health and in men's and women's objective health. However, some specific social protection policies may have the unintentional effect of increasing inequalities in people's evaluation of their own health.
District-Level Life Expectancy in Germany

Abstract:BACKGROUND: Identifying regions with low life expectancy is important to policy makers, in particular for allocating resources in the health system. Life expectancy estimates for small regions are, however, often unreliable and lead to statistical uncertainties when the underlying populations are relatively small.METHODS: We combine the most recent German data available (2015-2017) with a Bayesian model that includes several methodological advances. This allows us to estimate male and female life expectancy with good precision for all 402 German districts and to quantify the uncertainty of those estimates.RESULTS: Across districts, life expectancy varies between 75.8 and 81.2 years for men and from 81.8 to 85.7 years for women. The spatial pattern is similar for women and men. Rural districts in eastern Germany and some districts of the Ruhr region have relatively low life expectancy. Districts with relatively high life expectancies cluster in Baden-Wuerttemberg and southern Bavaria. Exploratory analysis shows that average income, population density, and number of physicians per 100 000 inhabitants are not strongly correlated with life expectancy at district level. In contrast, indicators that point to particularly disadvantaged segments of the population (unemployment rate, welfare benefits) are better predictors of life expectancy.CONCLUSIONS: We do not find a consistent urban-rural gap in life expectancy. Our results suggest that policies that improve living standards for poorer segment of the population are the most likely to reduce the existing differences in life expectancy.
How socio-political change is associated with the number of individually reported negative life events: a population-based study using the German reunification 1989/1990 as an example

Abstract:Background Socio-political change often leads to disruptions in employment and social networks, which can exacerbate health issues and increase mortality rates. These consequences are likely observed as an increase in negative life events (NLEs), serving as indicators of the broader social and health impacts. Using the German reunification in 1989/1990 as an example, this study investigates changes in reported numbers of NLEs and differences regarding sociodemographic characteristics. Methods We used data from the population-based Study of Health in Pomerania (SHIP-START-0, SHIP-Life-Events and Gene-Environment Interaction in Depression; N=1932). Numbers of NLEs in different categories (work/financial, social/interpersonal, illness (own) and illness/death (others)) were measured retrospectively in 5-year intervals (1980–2004) using a semistructured interview. Pre-reunification and post-reunification changes were modelled using piecewise mixed-effects Poisson regressions with the 1990–1994 interval (reunification) as change point. Interactions with age, sex and education were examined. Results The number of most NLE categories, except social/interpersonal NLEs, increased at reunification. Whereas work/financial NLEs slightly decreased post-reunification, illness-related NLEs continued to increase. Higher numbers of social/interpersonal NLEs were found with younger age. More illness-related NLEs were reported with older age, lower education (illness (own)) and by women (illness/death (others)). However, the majority reported no NLEs at reunification (68.2%–80.7%, varying by category). Conclusion Our findings suggest that although some individuals experience a marked increase in NLEs due to socio-political changes, many remain unaffected, emphasising the need for a differentiated understanding of these effects. This increase in NLEs may partly account for ongoing health and well-being disparities among countries with differing transformation histories.

Abstract:Introduction: Smoking is a major risk factor for premature death and health problems in which there are significant gender differences in the prevalence of smoking. This ecological study examines the correlation between changes in gender equality and prevalence of smoking among young adults (15–25 years old) in Germany over a period of 45 years (1960–2005). Methods: Gender inequality was measured using the United Nations Gender Inequality Index (GII), which is composed of three dimensions; health, empowerment and labour market. It was calculated for the entire registered German population in five-year intervals with values between 0 and 1 (1 = highest inequality). The smoking prevalence of young women and men in Germany was established using a reconstruction method. A gender smoking ratio (GSR) with values between 0 and 1 was determined (1 = identical smoking prevalence among men and women). The smoking behaviour was illustrated and stratified by education. The correlation between the GII and the GSR was analysed. Results: The GII decreased from 0.98 to 0.56 between 1960 and 2005. The GSR increased from 0.34 to 0.93. There was a strong negative correlation between the GII and the GSR ( r = −0.71). The strength of the correlation fell slightly as the level of education decreased. An increase in gender equality as measured by the GII came along with similarities of smoking prevalence between young women and young men. Conclusion: Successful tobacco prevention among young women and men may benefit from involving experts in gender-specific public health research to develop counter-advertising and gender-specific information as needed.

Is Redistribution Good for Our Health? Examining the Macro-Correlation between Welfare Generosity and Health across EU Nations over the Last 40 Years

Abstract:Abstract Context: Social determinants of health are finally getting much needed policy attention. Yet, their political origins remain underexplored. In this paper, we advance a theory of political determinants as accruing along three pathways of welfare state effects (redistribution, poverty reduction and status preservation) and test these assumptions by examining impacts of policy generosity on life expectancy (LE) over the last 40 years. Methods: We merge new and existing welfare policy generosity data from the Comparative Welfare Entitlement Project (CWEP) with data on LE spanning 1980-2018 across 21 OECD countries. We then examine relationships between five welfare policy generosity measures and LE using cross-sectional differencing and auto-regressive lag models. Findings: We find consistent and positive effects for total generosity (an existing measure of social insurance generosity) on LE at birth across different model specifications in the magnitude of a 0.10-0.15-year increase in LE at birth (p < 0.05) and a measure of status preservation (0.11, p < 0.05). We find less consistent support for our redistribution and poverty reduction measures. Conclusions: We conclude that in addition to generalized effects of policy generosity on health, status-preserving social insurance may be an important, and relatively overlooked, mechanism in increasing life expectancies over time in advanced democracies.

Abstract:Understanding the relationship between life expectancy at birth (e_0) and the gross domestic product per capita (GDPpc) is relevant for cohesion policies in the European Union (EU), because it might imply that economic convergence (or divergence) is accompanied by narrowing (or widening) health gaps. Previous studies have studied the association between GDPpc and e_0 almost exclusively based on national data. However, it is certainly relevant to add a subnational dimension, because levels and trends in both e_0 and GDPpc vary substantially across Europe's regions. Accordingly, the aim of our study is examining whether the economic performance of a region is correlated to their e_0 level. To do so, we collected official mortality and population counts from national statistical offices and information on GDPpc from the Eurostat database for 506 regions in 21 European countries from 2008 to 2019. Using this data, we built Preston curves from regression models. Our results suggest that there is indeed a positive association between GDPpc and e_0. Similarly to Preston's original analysis, we observe an upward shift in the curve, indicating that factors exogenous to a region's GDPpc level also play an important role in explaining e_0 gains. Yet, the relationship differs between geographical areas, and we also find examples, such as women in Germany, Austria, Poland, and the Netherlands, where the relationship/pattern does not seem to hold.

Abstract:Against the backdrop of population aging and growing strain on pension systems, monitoring the development of Working Life Expectancy (WLE) is vital to assess whether the policies taken are effective. This is the first study investigating time trends and educational inequalities in WLE based on German health insurance data. The analyses are based on the data of the AOK Lower Saxony (N = 3,347,912) covering three time periods (2006-08, 2011-13, and 2016-18). WLE is defined as years spent in the labor force (i.e. in employment and unemployment) and was calculated for each age between 18 and 69 years for the three periods to depict changes over time using multistate life table analysis. Educational inequalities in 2011-13 are reported for two educational levels (8-11 years and 12-13 years of schooling). WLE increased in both sexes with increases being stronger among women. This holds irrespective of whether WLE at age 18 (35.8-38.3 years in men, 27.5-34.0 years in women) or the older working-age (e.g. at age 50 10.2-11.7 years in men, 7.8-10.5 years in men) is considered. Among women at all ages and men from their mid-20s onwards, WLE was higher among higher-educated individuals. Inequalities were most pronounced among women (e.g. Δ3.1 years in women, Δ1.3 years in men at age 50). The study supports previous research indicating that measures to extend working life are effective, but that noticeable inequalities in WLE exist. Health insurance data represent a valuable source for such research that has so far remained untapped. The data provide a suitable basis to investigate trends and inequalities in WLE. Future research should build on the strengths of the data by broadening the research towards a more comprehensive analysis of the development of WLE from a health perspective.
Determinants of Mental Health Inequalities Among People With Selected Citizenships in Germany

Abstract:Objectives: Mental health is essential for overall health and is influenced by different social determinants. The aim of this paper was to examine which determinants are associated with mental health inequalities among people with selected citizenships in Germany. Methods: Data were derived from the multilingual interview survey "German Health Update: Fokus (GEDA Fokus)" among adults with Croatian, Italian, Polish, Syrian, or Turkish citizenship (11/2021-05/2022). Poisson regressions were used to calculate prevalence ratios for symptoms of depression (PHQ-9) and anxiety disorder (GAD-7). Results: Sociodemographic (sex, income, age, household size) and psychosocial (social support and self-reported discrimination) determinants were associated with symptoms of depression and/or anxiety disorder. The prevalence of mental disorders varied most by self-reported discrimination. Conclusion: Our findings suggest mental health inequalities among people with selected citizenships living in Germany. To reduce these, social inequities and everyday discrimination need to be addressed in structural prevention measures as well as in interventions on the communal level. Protective factors (e.g., social support) are also important to reduce mental health inequalities on the individual and community level.
Shifts in peak month of births and socio-economic factors: a study of divided and reunified Germany 1950–2022

Abstract:The seasonality of conception and birth has intrigued scientists for over 200 years. Since the 1970s, peak birth months in the northern hemisphere have shifted from early spring to late summer, influenced by oral contraceptives and socio-economic factors. The division of Germany from the end of World War II until reunification in 1990 offers a unique opportunity to analyze these influences while both regions had equal access to contraception. This study aims to identify differences in fertility trends and peak birth months between West and East Germany before and after reunification, considering political, socio-economic factors, and contraceptive availability. A population-based study analyzed overall fertility rates, monthly birth data, peak birth months, and average maternal ages from 1950 to 2022. Fertility patterns were similar in both regions until 1970, showing a sharp increase post-1955 followed by a decline around 1965, likely due to contraception. From 1970 to 1990, East German fertility increased while West Germany's remained lower. After reunification, East German fertility fell sharply but later rose to West German levels. The peak birth month shifted from March to August in West Germany by the mid-1980s, while East Germany experienced this change 20 years later, post-reunification. The 20-year delay in East Germany's shift suggests that oral contraception's impact was influenced by other factors, particularly the age of women at childbirth. Increased control over fertility in the West allowed for more choice in conception timing, a trend adopted in the East after gaining similar autonomy.

Abstract:The decline in infant mortality played a crucial role in the health transition in the Western World. This decline among the vulnerable new-borns was however not an evenly dispersed process. Inequalities as a result of regional differences, cultural influences or socioeconomic status shaped the paths towards low mortality rates. The role of socioeconomic status in levels of infant mortality and its decline remain highly debated. In this article, we study the development of socioeconomic disparities in infant mortality in the Dutch town of Maastricht in the period 1864–1955. This study uses unique individual-level cause of death data in order to see when changes in disease patterns took place for different socioeconomic groups. The aim is to identify socioeconomic inequalities by mapping changing epidemiological patterns over time, according to age within the first year of life. By deploying a multinomial logistic regression analysis we can trace the different timing of changes in the epidemiological regime. The results reveal that for most infants the change in mortality patterns from water- and foodborne infectious diseases towards a predominance of airborne infectious diseases occurred simultaneously with the massive decline in infant mortality from the start of the First World War onwards. Infants from the upper classes, however, appeared to have gained an earlier advantage, followed by infants from unskilled workers. Finally, from qualitative data it becomes clear that the awareness of the problematic nature of infant mortality, knowledge on infant care, hygienic practices, breastfeeding practices and the economic situation of World War I were factors aiding to the uneven decline in infant mortality.

The contribution of avoidable mortality to life expectancy differences and lifespan disparities in the European Union: a population-based study

Abstract:Background: Twenty years after the 2004 European Union (EU) enlargement, life expectancy differences between established (EMS) and new member states (NMS) remain large. Contributing to this gap are deaths that can be avoided through preventive services or adequate medical treatment. We estimate the impact of reducing avoidable mortality on life expectancy and lifespan disparities in the enlarged EU. Methods: Using World Health Organization mortality database data, we analysed the potential of reducing avoidable mortality, as defined by Eurostat and the Organisation for Economic Cooperation and Development, to close the mortality gap between NMS and EMS. We decomposed the changes in life expectancy and lifespan disparity by age and cause using linear integral decomposition. Findings: Averting all avoidable deaths across the EU from 2005 to 2019 would decrease the average life expectancy gap from 5.8 to 2.4 years in men and 3.3-2 years in women and eliminate the lifespan disparity gap. Had NMS achieved the average EMS avoidable mortality rates during the same period, the average life expectancy gap would have been reduced to 1.8 years in men and 1.6 years in women, and the lifespan disparities gap would have been reversed. Avoidable circulatory and injury-related deaths in middle and older age drove the observed mortality changes. Interpretation: Our results suggest that the gap in life expectancy and lifespan disparity across the EU could be reduced by strengthening health systems and investing in averting circulatory and injury-related deaths in middle and older age in NMS. Abstract:BACKGROUND: This longitudinal study builds on the cross-sectional work of Karim et al and examines the influence of welfare state regime on population health with a particular focus on East Asian welfare states (eg, Hong Kong, Japan, Korea, Singapore and Taiwan).METHODS: Data were extracted from the Organisation of Economic Co-operation and Development Data Set, World Development Indicators and Asian Development Bank's key indicators from 1980 to 2006. Infant mortalities and life expectancy were used as health-outcome varables. Thirty-one countries were categorised into six types of welfare regimes: Scandinavian, Anglo-Saxon, Bismarckian, Southern, Eastern European and East Asian. Mixed models were applied to analyse the data with repeated measurements.RESULTS: In keeping with Karim et al, Scandinavian and Eastern European welfare states have lower and higher infant mortalities respectively compared with East Asian welfare states. Eastern European welfare states had a lower life expectancy than East Asian welfare states. Most welfare states had a higher social, health and education expenditure, and higher densities of physicians than East Asian welfare states.CONCLUSION: East Asian welfare states did not have worse health than most welfare states. Future studies should continue to incorporate East Asian countries in the typology of welfare regimes that include more social, economic, political and healthcare system characteristic variables to provide insight on the mechanism by which welfare-state regimes influence population health.

Abstract:Previous cross-sectional studies highlight persistent East–West differences in gender ideologies after German reunification. This study examines the extent to which gender ideologies in the East and West have converged and whether differences are still relevant for younger cohorts who experienced childhood around the time of reunification, or after 1989. Using data from the German Family Panel pairfam (2008–2019) and differences in regime-specific socialization for three cohorts born before and after reunification, results reveal that different dimensions of gender ideologies have only partly converged 30 years after reunification. Attitudes towards housework and female employment converged particularly, yet, in all cohorts, views on maternal employment remain substantially different between East and West. Observed convergence occurred only partly due to contrasting trends of modernization in West Germany and re-traditionalization in East Germany. Moreover, the results highlight smaller attitude changes with increasing age, particularly for the younger cohorts, contributing to further variations in East–West differences. Overall, the findings confirm the existence of long-lasting ideology differences due to regime-specific socialization, and a persistently altered composition of society in East and West Germany. At the same time, they point towards slow convergence among younger cohorts due to a more similar institutional and socialization context following reunification.
