Abstract:INTRODUCTION: Blepharophimosis-ptosis-epicanthus inversus syndrome (BPES) is a rare autosomal dominant genetic disorder characterized by complex orbito-palpebral anomalies. We report a rare case of BPES associated with bilateral congenital cataract.OBSERVATION: This study reports the case of a 6-month-old infant with BPES in whom a bilateral congenital cataract was diagnosed, after the parents noticed leukocoria and signs of poor vision in their child. No other ophthalmologic manifestations commonly associated with this syndrome were found. The infant underwent cataract surgery first, with lens phacoaspiration and posterior capsulotomy coupled with anterior vitrectomy and placement of a 3-piece foldable hydrophobic posterior chamber lens in the capsular bag. The surgery was a real challenge due to the orbito-palpebral anomalies that limited a small surgical space, and the placement of the IOL was a matter of discussion.DISCUSSION: Publications on the association of congenital cataract with BPES are very rare. The link between these two anomalies is difficult to establish since different genes on different chromosomes code for the two diseases. A lateral canthotomy can be considered to overcome the surgical difficulties due to the reduced working space. The surgical management of pediatric cataract varies in the literature.CONCLUSION: This case highlights the difficulty of cataract surgery in children, even more so when associated with BPES, and the challenge of improving vision in these children given the high risk of amblyopia.
Blepharophimosis, Ptosis, and Epicanthus Inversus Syndrome: A Simple Remedy for Challenging Cases

Abstract:A 14-year-old male presented to the outpatient department of ophthalmology with complaints of visual impairment. The patient was assessed with a detailed history and physical examination. Marked amblyopia was observed on inspection, and his best-corrected vision was 6/36 in both eyes with no further improvement. Both the anterior and posterior segments of the eyes were normal. A diagnosis of blepharophimosis, ptosis, and epicanthus inversus syndrome (BPES) was suspected. Surgery was initiated in two stages, with the first stage utilizing Mustarde's double Z-plasty to correct the epicanthus inversus and telecanthus. The second stage was done three months later, involving a tarsofrontalis sling with prolene sutures to correct ptosis. The success of this operation speaks to the efficacy of a two-stage procedure for remedying a syndrome as complex as BPES.
Brown McLean syndrome after congenital glaucoma surgery, unique case report and a literature review

Abstract:Purpose: Brown-McLean Syndrome (BMS) was first documented by Brown and McLean in 1969. To date, BMS is typically described in primarily cataract-related complications, and it is inextricably associated with long-term aphakia. Our purpose is to report a case that describes a unique association of BMS in the presence of patent peripheral iridectomy. Observation: A 35-year-old male known case of congenital glaucoma who has a history of bilateral trabeculectomy, presented to our clinic with a long history of mild progressive blurriness of vision for years. Upon examination, the patient was bilaterally phakic, with bilateral 360-degree of peripheral corneal edema sparing the center of the cornea with endothelial pigmentation and bilateral patent peripheral iridectomy in an otherwise stable ophthalmic examination. His uncorrected visual acuity was 20/20 in the right eye and 20/30 in the left eye with normal intraocular pressure. Our plan was to prescribe topical medications and schedule him for regular follow-up. At the last follow-up, our patient reported no improvement in his vision, and there were no signs of clinical improvement. Conclusion and importance: This case of BMS demonstrates the importance of the potential of such a disease to develop, albeit rarely, in phakic patients who are undergoing intraocular surgery. Abstract:Abstract Blepharophimosis, ptosis, epicanthus inversus syndrome (BPES) is a rare genetic disorder characterized primarily by four distinct features including telecanthus. BPES is an autosomal dominant disorder caused by mutations in the FOXL2 gene located on chromosome 3q23. Two types of BPES has been described, type 1 BPES with ovarian insufficiency and type 2 with only major ocular features. Various other associations including lid dysplasia, strabismus, refractive error and lacrimal duct anomalies are noted. This review article aims to provide a comprehensive overview of BPES syndrome, including its genetics, clinical manifestations, differential diagnosis, and management options.

Abstract:Purpose: Ectrodactyly-ectodermal dysplasia-cleft (EEC) syndrome is a rare genetic disorder. We present ocular findings and their treatment in patients with EEC. Methods and results: We report on 3 female patients (aged 59, 45, and 11 years) suffering from EEC with varying extraocular and ocular severity of phenotypic expression of the disease. Slit-lamp biomicroscopy, visual acuity, and medical treatment were evaluated over 4 months to 4 years. All patients experienced visual impairment and foreign body sensation. Examination revealed bilateral chronic blepharitis, dry eye syndrome, and corneal vascularization and clouding due to limbal stem cell deficiency (LSCD). Patient #1 presented a corneal ulcer with severe stromal thinning on the right eye. Allogeneic simple limbal epithelial transplantation (allo SLET), penetrating keratoplasty combined with allo SLET, and in total 5 amniotic membrane transplantation were performed to preserve the integrity of the eye. In patients #2 and #3, conservative therapy with lubricant eye drops, topical steroids, and antibiotics was sufficient to stabilize LSCD. In all cases, corneal epithelialization and improvement of visual acuity were achieved. Conclusions and importance: To the best of our knowledge, this is the first report of surgical treatment in a patient with EEC. Allo SLET may be a surgical option to treat LSCD associated with EEC.
The incidence of strabismus and refractive error in patients with blepharophimosis, ptosis and epicanthus inversus syndrome (BPES)

Abstract:A retrospective review was carried out of 204 patients with blepharophimosis, (blepharo) ptosis and epicanthus inversus syndrome (BPES). Of these, 94 (46%) had an autosomal dominant family history of BPES. Forty (20%) had manifest strabismus. Of these, 28 (70%) had esotropia, 10 (25%) had exotropia and 2 (5%) had hypertropia. Twelve (6%) patients had nystagmus. Seventy (34%) patients had a significant refractive error requiring spectacles. Twenty-one (30%) of these patients had anisometropic hypermetropia and 24 (34%) had anisometropic myopia. Forty-three patients had bilateral amblyopia and 40 had unilateral amblyopia, with 26 (65%) of these undergoing occlusion treatment. Of these, 14 had strabismus and refractive error, 7 refractive error only, 2 strabismus only and 3 neither refractive error nor strabismus. We conclude that there is a higher incidence of strabismus and refractive error in patients with BPES than in the normal population.
Identification and Management of a Novel PRDM5 Gene Pathologic Variant in a Family With Brittle Cornea Syndrome

Abstract:Purpose: The aim of this study was to report a novel PRDM5 pathologic variant and ophthalmic findings in a family with 3 children diagnosed with brittle cornea syndrome (BCS). Histopathologic findings and surgical outcome of a child with BCS who underwent full-thickness corneal transplant are described. Methods: This is an observational case report of a nonconsanguineous Laotian family with 3 siblings diagnosed with BCS. Data collected included visual acuity, cycloplegic refraction, slit-lamp biomicroscopy, dilated fundus examination, corneal pachymetry, corneal topography, and general medical findings. Targeted testing through PRDM5 gene sequencing with copy number variation detection was conducted. Results: The 3 siblings included a 12-year-old boy and 8- and 6-year-old sisters, all of whom presented with myopia, blue-tinted sclerae, thin corneas, and variable corneal scarring. All 3 affected children were found to be homozygous for the PRDM5 gene variant c.1117_1123delinsTTTAATGCTTACAAATGTTTG p.Asp373Phefs*57. Coding sequences of PRDM5 and ZNF469 genes were sequenced in their entirety, and this was the only pathologic variant present in this family. The youngest affected sister developed persistent hydrops with severely decreased vision and underwent penetrating keratoplasty. Histopathology revealed severe corneal thinning, diffuse absence of Bowman layer, and ruptured Descemet membrane scrolls. Conclusions: Three siblings with clinical signs of BCS, including corneal thinning, myopia, and blue sclerae, were found to have a novel PRDM5 gene pathologic variant. This pathologic variant has not been previously reported, although 1 downstream nonsense pathologic variant has been reported as pathogenic. The similar phenotypes in all affected patients support the pathogenicity of this variant. Surgical management of BCS presents unique challenges due to severe tissue fragility.

Abstract:Objective Aneurysmal bone cysts (ABCs) are benign but aggressive vascular lesions within bone. Orbital ABCs, though rare, are clinically significant due to the risk of debilitating symptoms such as vision loss. Methods Our systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines using PubMed, Google Scholar, Embase, and SCOPUS databases to identify and assess primary articles exploring orbital ABC cases. Additionally, we present an illustrative case report. Results Among the 54 cases from the 51 studies included, the mean age was 16.87 years, with 23 males, 29 females, and 2 unspecified. The most prevalent symptoms were exophthalmos (N = 36) and diplopia (N = 21). Regarding laterality, the right orbit was most commonly affected (N = 27), followed by the left orbit (N = 21), with 4 cases of bilateral involvement. Within the orbit, the medial wall most frequently impacted location (N = 14), followed by the orbital roof (N = 13), posterior wall (N = 7), lateral wall (N = 3), and orbital floor (N = 2), with 15 cases involving multiple locations. Surgical resection was the primary treatment, resulting in complete resolution in 44 cases. In our case, a 30-year-old male with an orbital ABC was managed surgically through bifrontal craniotomy with superior orbitotomy for lesion excision and orbital roof reconstruction. Conclusions ABCs in the orbit can be challenging, and complete removal of the lesion is essential for achieving optimal patient outcomes.

Challenges in management of congenital enophthalmos due to anomalous accessory orbital extraocular muscle bands

Abstract:Introduction: Congenital enophthalmos is a rare condition characterized by posterior displacement of the globe, often associated with bony orbital anomalies or whole globe development defects. The purpose of this report is to present two unrelated cases of congenital enophthalmos secondary to anomalous accessory orbital bands and to describe characteristics of orbital imaging that differentiate this condition from the other causes. Methods: The case records of two patients who presented with congenital enophthalmos and were discovered to have anomalous accessory orbital extraocular muscle bands were reviewed. The clinical features, initial diagnosis, high resolution magnetic resonance imaging (MRI) findings, and surgical outcomes were noted. A 3-dimensional reconstruction model was used to understand the approach and surgical management in one of the cases. Results: Both patients presented with unilateral severe enophthalmos, globe retraction, and restricted ocular motility in all directions since birth. High-resolution MRI of the orbits revealed a short anomalous band, isointense to the muscle, arising from a rectus muscle belly and attaching to the posteroinferior part of the globe adjacent to the optic nerve. The caliber of the extraocular muscles and ocular motor nerves was normal. In one patient, surgery was not pursued due to the extreme posterior location of the band with proximity to the optic nerve. In the other patient, the deviation did not improve, despite successfully severing the accessory band, due to extensive scarring. Conclusion: Anomalous accessory orbital extraocular muscle bands are a rare and often overlooked cause of congenital enophthalmos when associated with limited ocular motility. Imaging the orbit can aid in diagnosis and help differentiate it from other causes. Safe surgical approaches to address the problem are limited, and available approaches may not be effective. These two cases highlight that the management of accessory extraocular muscle bands causing enophthalmos can be extremely challenging and difficult to improve even with intensive surgical intervention.

Abstract:Purpose: To report a rare presentation of bilateral, coexisting ocular surface disease in a case of Xeroderma pigmentosum and its successful management. Methods: Case report. Results: A 21-year-old male with Xeroderma pigmentosum presented with bilateral ocular surface squamous neoplasia (OSSN) along with central guttae in the right eye and corneal decompensation of the left eye. Subsequently, the patient developed dry eyes and lid margin keratinization in both eyes followed by perforation in the left eye. Sequential procedures both medical and surgical, including excision of the tumour, corneal transplantation and mucous membrane grafting addressing each of these ocular surface issues resulted in a successful outcome. There was no recurrence of the tumour over 3 years. Corneal transplantation is preferably done after a minimum of 6 months following excision. Mucous membrane grafting performed for progressive lid margin keratinization resulted in surface stabilization. Conclusions: In Xeroderma Pigmentosum, multiple ocular surface features can rarely coexist and be bilateral. Periodic evaluation of the surface for tumours, progressive dry eyes and endothelial function is recommended as a part of routine evaluation in Xeroderma pigmentosum. Surface procedures should precede intraocular intervention. Sequential management can result in successful outcomes.
Incidence of HHV7 in donors and recipients of allogeneic hematopoietic stem cell transplantation

Abstract:To the Editor: We read with the great interest the article of by Khanani et al. [1] and were surprised with the extremely low incidence of detected Human Herpesvirus 7 (HHV7) in paediatric hematopoietic stem cell transplant (HSCT) recipients, which is not in agreement with our experience. The incidence in the allogeneic setting was 5.5% (9/163) with the median of detection 21 days after HSCT. It would have been interesting to see the total number of tested samples in different categories (blood, plasma, other body fluids, tissue) and the separation of that cohort concerning patients on ganciclovir (GCV) prophylaxis as possible explanation for the low HHV7 incidence. In the article and in the reference about the PCR methods [2] we were not able to find the sensitivity of theHHV7 assay. The lower incidencemay be then also explained by the lower sensitivity of assay. In our institution,we have testedHHV7 in similar clinical setting with the exception of GCV prophylaxis. All patients received acyclovir prophylaxis, while GCV was used to treat CMV preemptively based on the results of weekly PCR monitoring. Viral detection is based on DNA extraction fromwhole blood. The results are normalized to 10,000 human genome equivalents quantified using the albumin gene. The sensitivity of both the HHV7 and albumin gene detection are down to 5 copies per reaction. Up to May 2007, we tested for HHV7 2,546 blood samples obtained from125 patients transplanted between February 2000 and May 2006 (median age at HSCT 9.13 years). Two hundred seventyeight samples from 56 patients (44.8%) contained more than 1 normalized viral copy (NVC), more than 10 NVCs contained samples from 26 patients (20.8%) and more than 100 NVCs were detected in only 5 patients (0.04%). In 72 patients from our cohort, we found no impact of viral reactivation on clinical features (e.g., presence of fever, pneumonia, thrombocytopenia). With regard to the predictors of the reactivation of HHV7 at level 10 NVCs, we have found that a higher dose of CD34þ cells in the graft and use of busulfan increased the probability for HHV7 detection during posttransplant surveillance. HSCT with related donor compared to unrelated resulted in a 4.5-fold increase in risk of HHV7 reactivation. Compared to Khanani et al. [1], we found a positive association of HHV7 and HHV6 detection while there was no relation between HHV7 and CMVand EBV. Interestingly, we were able to detect HHV7 DNA in 9 unrelated and 5 related donors out of 30 donors tested (21 unrelated and 9 related donors) and observed no clinical consequence for the recipient. The quantity of HHV7 DNA in these cases was below 100 NVCs (median 7.5, range 0.6–88). Because the graft for HSCT is obtained in donors without any clinical sign of infection, we suggest this level as a non-clinically important in term of latent HHV7, at least for immunocompetent host. Compared to Khanani et al., higher incidence observed in our cohort is similar to other publishedworks [3,4] andmay be caused by the higher sensitivity of our PCR assay.

Abstract:Blepharophimosis–ptosis–epicanthus inversus syndrome (BPES) is a rare genetic syndrome characterized by dysmorphism of ocular adnexa. We report a rare presentation of BPES and posterior persistent fetal vasculature (PFV) in a 10-year-old male child. A thorough evaluation showed all the classical features of BPES, calcified cataract in the left eye, and an echogenic mass arising from the optic disc with blood flow characteristic consistent with PFV. The coexistence of BPES and posterior PFV has never been reported in the literature before, and their co-occurrence poses a unique surgical challenge.

Abstract:Blepharophimosis, ptosis, and epicanthus inversus syndrome (BPES) is a craniofacial disorder caused by heterozygous variants of the forkhead box L2 (FOXL2) gene. It shows autosomal dominant inheritance but can also occur sporadically. Depending on the mutation, two phenotypic subtypes have been described, both involving the same craniofacial features: type I, which is associated with premature ovarian failure (POF), and type II, which has no systemic features. The genotype–phenotype correlation is not fully understood, but it has been hypothesised that type I BPES involves more severe loss of function variants spanning the whole gene. Type II BPES has been linked to frameshift mutations that result in elongation of the protein rather than complete loss of function. A mutational hotspot has been identified within the poly-alanine domain, although the exact function of this region is still unknown. However, the BPES subtype cannot be determined genetically, necessitating informed genetic counselling and careful discussion of family planning advice in view of the associated POF particularly as the patient may still be a child. Following puberty, female patients should be referred for ovarian reserve and response assessment. Oculofacial features can be managed with surgical intervention and regular monitoring to prevent amblyopia.

Clinical Finding and Management of 12 Orofacial Clefts Cases With Nevoid Basal Cell Carcinoma Syndrome.

Abstract:Objective: To study the phenotype and overview the clinical management on Cleft Lip and/or Palate (CL/P) with Nevoid Basal Cell Carcinoma Syndrome (NBCCS) patients in Peking University School and Hospital of Stomatology. Design: Case series study. Main Outcome Measures: To describe the clinical phenotype of 12 CL/P with NBCCS patients who fulfilled the diagnostic criteria as well as to explore clinical management. Results: Seven cases (7/12, 58.33%) were classified as bilateral complete cleft lip and palate (BCCLP). Two cases (2/12, 16.67%) were classified as unilateral complete cleft lip and palate (UCCLP). Three cases (1/12, 8.33%) were classified as unilateral complete cleft lip (UCCL), submucosa cleft uvula (SCU), and bifid uvula (BU), respectively. The ratio of male/female was 9/3. Keratocystic odontogenic tumors (KCOTs) were presented in all 12 cases. The most common site was the mandible region (12/12, 100%) followed by the maxilla region (7/12, 58.33%). The diagnostic age of 12 NBCCS with CL/P ranged from 11 to 42 years old (usually postponed to the occurring of KCOTs). The delayed diagnosis of NBCCS can be attributed to its complicated clinical manifestations. In some cases, the mutual effect between the surgical therapy of removing KCOTs and alveolar bone grafting made the team approach (TA) of CL/P more complicated. Conclusion: CL/P may become important clinical phenotype in NBCCS. The type of cleft varied, with bilateral cleft lip and palate comprising above 50%. Larger sample sizes are needed to study and confirm this result. KCOTs, as one of the most common clinical feature of NBCCS, make the diagnosis delayed and the TA more difficult because of the occurring time and site. This compels us to improve the diagnostic criteria to make an early diagnosis and explore a better therapeutic protocol for CL/P.

Abstract:We describe the first case reported in ophthalmological literature of the surgical management of a 17-month-old boy with bilateral vision-threatening ptosis, tarsomegaly, ectropion, and euryblepharon secondary to suspected overgrowth syndrome. We elaborate on the major challenges associated with surgical management including the natural and asymmetric growth of oversized tissue, the high likelihood of scarring and formation of disorganized tissue, and risks of frequent intubation in these patients who may have lesions that compromise critical structures such as the airway. Ultimately, surgical intervention is encouraged primarily if vision or ocular health is threatened and secondarily to achieve good cosmesis.

Abstract:Objective To explore the clinical features and gene mutations of blepharophimosis-ptosis-intellectual-disability syndrome (BPID). Methods The clinical data, diagnosis and treatment of a child with BPID in neonatal intensive care unit (NICU) were reviewed. Based on the literature retrieved from PubMed database, the common classification, clinical features, diagnosis and genetic counseling of BPID and its affiliated blepharophimosis-mental retardation syndromes (BMR) were reviewed. Results This male infant was 39 weeks of gestational age with birth weight of 1920 g, and was admitted to NICU 15 min after birth due to dyspnea. The main clinical manifestations were facial deformity such as biepharophimosis, ptosis and micromandible, inspiratory dyspnea with laryngeal cartilage softening, malformations of the thorax and feeding difficulties. A heterozygous mutation in UBE3B gene was identified by complete exon sequencing and he was diagnosed of BPID, a rare genetic disorder. Reviewing the literature, there was no relevant report in domestic. While one foreign literature was found to report 5 patients from 4 families having a subtype of BMR, a kind of autosomal recessive diseases caused by mutations in the UBE3B gene. Conclusion BPID is a rare clinical entity of BMR. Complete exon sequencing can be used to diagnose the disease.

Abstract:Purpose: Brown-McLean syndrome (BMS) is a clinical condition characterized by peripheral corneal edema with central corneal transparency. This study aims to document the tomographic and biomechanical characteristics of 3 patients with typical BMS features using the Pentacam® AXL and CORVIS ST® (Oculus Optikgeräte GmbH, Wetzlar, Germany). Observations: Three cases of BMS are presented. Case 1 involves a 26-year-old male, Case 2 a 55-year-old male, and Case 3 a 74-year-old male. The patients in Cases 1 and 3 had bilateral BMS, while the patient in Case 2 had BMS in the right eye and aphakic bullous keratopathy in the left eye. All three patients were aphakic following cataract surgery. Notably, Cases 1 and 2 were first-degree relatives (son and father), both with bilateral microspherophakia and resultant bilateral aphakia from pediatric cataract surgery. Tomographic analysis revealed a consistent increase in corneal thickness from the center to the periphery in BMS eyes, marked by an abrupt rise in the corneal thickness spatial profile (CTSP) and percentage thickness increase (PTI) curves from the thinnest point towards the periphery. There was no loss of parallel isopachs, no displacement of the thinnest point of the cornea, and no evidence of focal posterior corneal surface depression, typical signs of generalized corneal edema. Biomechanically, BMS eyes exhibited relatively normal corneal stiffness, integrated radius, Ambrósio's relational thickness to the horizontal profile (ARTh), and maximum deformation amplitude ratio at 2mm from the corneal apex (DA ratio). However, the left eye of the patient in Case 2, which had aphakic bullous keratopathy, showed altered biomechanical parameters indicative of a softer cornea with loss of rigidity. Conclusions and importance: This case series is the first to evaluate the biomechanical and tomographic features of eyes with BMS. Despite the abrupt rise in CTSP and PTI curves from the thinnest point towards the periphery, the relatively normal central corneal biomechanical indices in these BMS eyes are expected when edema is limited to the periphery. These indices become abnormal when there is progression to central corneal edema with bullous keratopathy.
