Compliance of Adolescent Friendly Health Clinics with National and International Standards: Quantitative findings from the i-Saathiya study

Abstract:Objective Indian adolescents experience several health challenges requiring acceptable, equitable, appropriate and effective healthcare services. Our objective was to assess the compliance of Adolescent Friendly Health Clinics (AFHCs) in two of India's largest states, using both national benchmarks (under Rashtriya Kishor Swasthya Karyakram-RKSK) and global standards (by WHO). Design Cross-sectional study comprising structured observations and interactions (November 2021 to June 2022). Setting Fourteen AFHCs across all levels of health system were included from two districts of Maharashtra (n=8) and Madhya Pradesh (n=6). These AFHCs were observed using checklist, and few items of checklist were verified by interactions with AFHC's health workers (medical officers/auxillary nurse midwives/counsellors) handlings adolescents. The developed checklist included 57 items based on adapted global standards and 25 items using national benchmarks. Result High compliance of AFHCs with RKSK's benchmarks was attributed to various items including the accessibility through local transport (n=14, 100%), clean surroundings (n=11, 78.5%), presence of signage (n=10, 71.4%), convenient operating days and time (n=11, 78.5%), and secure storage of records (n=13, 92.9%). Concurrently, items that showed low compliance encompassed, the availability of Information, Education and communication (IEC) resources, which were deficient in 57.1% of AFHCs (n=8). Similarly, designated areas for clinical services (n=10, 71.4%) and commodity disbursement (n=9, 64.3%) lacked in more than half of the recruited AFHCs. Additionally, lack of guidelines for referrals (n=13, 92.9%), as well as standard operating procedures to ensure equity, non-judgemental attitude, competence, confidentiality and referral as per WHO standards. Conclusion Evidence spotlights the strengths and gaps in AFHCs, aligning with, government's priorities on adolescent health. Addressing the identified gaps is crucial to creating healthcare facilities that are adolescent-friendly, easily accessible and effectively navigate adolescent health challenges. This concerted effort would contribute to their development and transformation, playing a pivotal role in India's progress.

Review of the India Adolescent Health Strategy in the context of disease burden among adolescents

Abstract:Background: A nuanced understanding of the health needs of adolescents in the context of the India Adolescent Health Strategy (IAHS) is needed to inform policy interventions for improving the health and well-being of adolescents in India. Methods: Using data from the Global Burden of Diseases, Injuries, and Risk Factors Study 2019, we identified the top ten causes of years of life lost (YLLs), years lived with disability (YLDs), and disability-adjusted life years (DALYs) disaggregated by sex and age group (10-14 and 15-19 years) for India and its states in 2019. To inform the IAHS of refinement or expansion in focus needed to improve adolescent health in India, we reviewed the extent to which the top 10 causes of disease burden are addressed in the IAHS, and the availability of and age- and sex-disaggregation in the service utilisation data for adolescents captured in the Adolescent Friendly Health Clinic monitoring information system (AFHC MIS) and Health Management Information System (HMIS). We also reviewed the availability of and age-and sex-disaggregation in the data capture at the population level for the IAHS outcome indicators in the data sources identified in the IAHS operational framework. Findings: Females in the 10-14 and 15-19 years age groups suffered 6.75 million and 9.25 million DALYs, respectively, 39.1% and 44.2% of which were YLLs; the corresponding DALYs for males were 6.71 million and 9.65 million (42.3% and 41.1% YLLs), respectively. Within the 6 thematic areas of the IAHS, most strategies and indicators identified are for sexual and reproductive health followed by nutrition, and broadly these conditions accounted for YLDs and not YLLs in adolescents. Significant gaps in the IAHS in comparison to the disease burden for fatal diseases and conditions were seen across injuries, communicable diseases, and non-communicable diseases. Injuries accounted for 65.9% and 45.3% of YLLs in males and females aged 15-19 years, and 40.8% in males aged 10-14 years. Specifically, road injuries (15.3%, 95% UI 11.0-18.0) and self-harm (11.3%, 95% UI 8.7-14.2) accounted for most of the injury deaths in 15-19 years whereas drowning (7.7% 95% UI 5.8-9.6) and road injuries (6.9%, 95% UI 4.7-8.6) accounted for the most injury deaths in 10-14 years males. However, only self-harm and gender-based violence are specifically addressed in the IAHS with non-specific interventions for other injuries. Diarrhoea, lower respiratory infections, malaria, encephalitis, tuberculosis, typhoid, cirrhosis, and hepatitis are the other disease conditions accounting for YLLs and DALYs in adolescents but these are neither addressed in the IAHS nor in service provision under the AFHC MIS. There is no age- or sex-disaggregation in the cause of death data captured in the HMIS to allow an understanding of mortality in adolescents. For the IAHS outcome indicators at the population level, data capture for the 10-14 years irrespective of sex was largely missing from the population surveys and none of the surveys captured data for either females or males aged 15-19 years for physical inactivity and mental health indicators. Interpretation: The considerable differences seen in the IAHS thematic focus as compared with the leading causes of fatal and non-fatal disease burden in adolescents in India, and in the availability of population-level data to monitor the outcome indicators of the IAHS can pose substantial limitations for improving adolescent health in India. Abstract:Objectives Treatment for mental health problems is determined by cultural, health infrastructure, and illness-related factors. Literature is sparse from India, particularly from the mental health resources-deficient regions of the country such as central India. Therefore, the current study is aimed at assessing the profile of the patients visiting the psychiatry outpatient facility (OPF) of a tertiary-care general hospital setting (GHS), their referral patterns, and their determinants. Materials and Methods A retrospective chart review of the newly registered individuals (October 2019 to March 2020) in the psychiatry OPF of the GHS from Central India was used in the study. Data (sociodemographic profiles, illness characteristics, and referral patterns) were extracted as per the standard guidelines. Statistical Analysis Descriptive statistics were used to represent sociodemographic, illness-, and past treatment-related characteristics of the participants. Chi-squared test was used to compare the referral characteristics of the two groups (self-referred patients vs. those referred by others, dependent variable) with regard to characteristics of the patients (independent variables). Results A total of 418 individuals were registered in the clinic. Most individuals suffered from the neurotic, stress-related, and somatoform group of disorders ( n = 231, 39.5%). More than halves were self-referred; most were referred from the internal medicine and allied departments. Being male, having at least graduate degree (χ 2 df(1) = 4.25 to 6.79, p < 0.05), suffering from organic mental-, psychotic-, and recurrent affective-disorders, and positive family history (χ 2 df(1) = 4.91 to 21.76, p < 0.05 to <0.001) along with first treatment attempt or previous treatment from the traditional healers, and absence of co-occurring medical illness were associated with self-referral (vs. referred by others) (χ 2 df(1) = 4.64 to 17.6, p < 0.05 to <0.001). Conclusions GHS has a characteristic referral pattern. The referral patterns of the patients for various psychiatric problems are determined by their sociodemographic, illness, and cultural characteristics; particularly, poor mental health literacy (among the patients-caregivers), stigma related to mental disorders, and unavailability of the mental health services act as major determinants. Sensitizing the patients-caregivers and health-care professionals concerning this could facilitate an early engagement with the psychiatric treatment. Future research needs to explore this phenomenon in greater detail, maybe by qualitative methods. Social Security Measures of Reproductive Health Among Adolescents in India: A Narrative Review

Abstract:Adolescence is one of life's most complex stages, associated with unique reproductive health requirements. Adolescents have individual sexual and reproductive health requirements (whether or not they are sexually active or married). Young and developing children have limited knowledge and awareness of the biological changes that occur through adolescence and the illnesses that affect them. We aim to discuss adolescent problems related to reproductive health, estimate the social security measures and associated factors among adolescents, and find why healthcare services are gravely limited in terms of accessibility and availability. This paper was based on previously available data through multiple sources and literature searches on PubMed, Research Gate, Sci-Hub, and Google Scholar. Adolescent health programs are currently fragmented, with no comprehensive program addressing all of the needs of adolescents. The major barriers are a lack of reliable information, a lack of proper guidance, parental ignorance, a lack of skills, and inadequate services from the healthcare system. We need a multidimensional approach that addresses all young adults' health issues, focusing on reproductive health, social security, mental well-being, and interventions involving communication toward a healthy lifestyle.
Obesogenic behaviours in South Asian adolescents in England: a literature review and quantitative investigation

Abstract:Abstract Background WHO has stated that childhood obesity is one of the most serious challenges facing public health in the 21st century, with obese children and adolescents facing a plethora of health complications and increased risk of many chronic diseases. Using a literature review and quantitative analysis of the WHO Health Behaviour in School-Aged Children (HBSC) data, we aimed to better understand obesogenic behaviours in South Asian adolescents in England. Methods We searched electronic databases (Embase, Scopus, Medline, Web of Science) from inception of the database to April 10, 2016, and the references of relevant papers. Grey literature including reports and non-peer reviewed sources were also included. Inclusion criteria were: adolescents aged 11–18 years of age, articles in English language, and studies based in England. Quantitative analysis of HBSC data from 2010 examined variables such as physical activity, dietary behaviours, and mental health indicators. We compared adolescents from Bangladeshi, Indian, and Pakistani groups for each variable, by sex, with white British adolescents. Pearsonu0027s χ 2 test was carried out to compare all groups against each other. Ethics approval was given by the University of Hertfordshire Ethics committee for Health and Human Sciences (HSK/SF/UH/00007). Ethical sensitivity was also enhanced through the work of reference groups with young people, which informed the conduct of the study within schools. Findings The HSBC 2010 sample consisted of 51 Bangladeshi, 96 Indian, 111 Pakistani, and 3476 white British adolescents. Two (8·7%) of 23 Bangladeshi girls undertook 5 or more daysu0027 physical activity, compared with 744 (44·1%) of 1688 white British girls, 13 (27·7%) of 47 Indian girls, and 16 (33·3%) of 48 Pakistani girls. South Asian adolescents had significantly worse outcomes in terms of number of times physical activity was undertaken per week (p=0·023) or any physical activity in the previous 7 days (p=0·002). 11 (20·0%) of 55 Pakistani girls never ate vegetables or ate them less than once a week, compared with 94 (5·3%) of 1771 white British girls (consumption of vegetables, p Interpretation Key findings from the literature review and HBSC 2010 data were that South Asian adolescents have lower physical activity levels compared with their white peers, have worse dietary behaviours, and have barriers to engaging in healthier lifestyles, compared with their white British counterparts. These negative lifestyle behaviours will need careful consideration of upstream policy and local health service provision and interventions in South Asian adolescents in England. Taking this evidence into action will be crucial to address the obesity epidemic and reduce existing health inequalities. Funding None.

Abstract:Background:-Adolescence is the transitional period between childhood and adulthood. As per the World health organization (WHO), adolescence includes the age group of 10-19 years. Adolescents constitute 20.0% of our total population and represent almost one-fth of the world's population The objective of these studies was to evaluate various causes of visiting adolescent girls in gynecology OPD and to assess their awareness about reproductive health problems. Material And Method:- This cross sectional study was conducted in a private clinic in district Shivpuri from 1 st April 2017 to 30 march 2022 In this study 668 adolescents were studied during this period. Detailed history, general examination, laboratory investigation, and pelvic USG were done to evaluate their health. Awareness of girls about reproductive and sexual health was interviewed. Results:- In this study, 668 adolescents were studied. The majority 54.04% of adolescent girls were between the ages of 14-16 years. Most 48.05% of them were educated up to 10th standard and most 74.10% of them were between BMI KG/m2 19-25 and the most 75.44% common age of menarche was between 11-13 years. Menstrual irregularities were the most common (58.68%) type of gynecological problem seen in adolescent girls. Frequency and percentage of other gynecological problems like breast problems, per vaginal discharge, Vulval itching, Hirsutism, Acne Lower abdominal pain, Dysurea & frequency of micturition, Bartholins cyst, contraceptive advise, Pregnancy/abortion, and post abortal bleeding in adolescent girls were 9%, 6.14%.,2.99%, 1.95%, 1.95%, 2.99%, 2.99%, 5.99% 2.10% and 1.05 respectively. Puberty menorrhagia (30.14%) was the most common type of menstrual disorder found among adolescent girls Frequency and percentage of other menstrual problems like Oligomenorrhea, and polymenorrhea. Primary amenorrhea, secondary amenorrhea, and Dysmenorrhea were 25.84% 12.92% 4.07% 18.18%, and 8.85% of adolescent girls respectively. AUB was the most common cause of puberty menorrhagia. polycystic ovaries were diagnosed in 37.30% of cases. Hypothyroidism was present in 3.97% and bleeding disorders were diagnosed in 1.59% of cases. Mullerian agenesis was the main cause (58.82%) of primary amenorrhea, and PCOD was the main cause of secondary amenorrhea 57.04% were aware of the physical science of pubertal development. Awareness about cancer cervix, HPV vaccination, and pap smear was found in only 17.96% of cases. 40.87% of cases were aware of cancer or breast and self-breast examination. STD awareness, HIV awareness, modes of transmission, awareness about contraceptives, awareness about the physiology of menstruation, awareness about anemia, awareness about menstrual hygiene, and awareness about tobacco and drug addiction were found in 50.90%, 25.00%, 53.89%, 20.96%, 61.98%, 25.00%, and 20.96% respectively. Conclusion:- In our study, menstrual disorders were the commonest gynecological problems among adolescents To create awareness and to promote knowledge regarding various health issues, health education classes and counseling of adolescent girls regarding reproductive health problems should be done Community-level health services should be well equipped with drugs, supplies of blood, and facilities for laboratory investigation to deal with reproductive health problems in adolescent
Obesogenic diet and metabolic syndrome among adolescents in India: data-driven cluster analysis

Abstract:Background: Metabolic syndrome is on the rise in India and is primarily linked to obesogenic dietary habits. The synergy of both is a prominent risk factor for cardiovascular diseases (CVDs). Hence, the present study aims to unveil clusters at high risk of metabolic syndrome and ascertain cluster characteristics based on dietary patterns among adolescents aged 10-19 years. Data and methods: The study utilizes secondary data, i.e., Comprehensive National Nutrition Survey conducted in 2016-18. The study sample includes children and adolescents aged 10-19 years. An unsupervised learning algorithm was used to ascertain possible clusters in the data based on individuals' dietary patterns. The k-means were used to cluster the data according to their dietary patterns. To determine the number of clusters elbow method was used, and appropriate validation indices were also obtained for the final k. Further, to ascertain the distribution of the obesogenic dietary patterns and metabolic conditions in each cluster was analysed. Bivariate descriptive analysis was used to draw further inferences. Results: The k-means clusters identified five optimum clusters based on 12,318 adolescents (6333 males (mean age:14.2 ± 2.8) and 5985 females (mean age:14.3 ± 2.8)) 17 dietary patterns. Clusters were named based on how prudent these were in terms of consuming a healthy diet. Cluster phenotypic characteristics were defined as follows: a cluster of obesogenic diets (24%) constituted the highest proportion of the total sample and was significantly suffering from obesity (p < 0.001), and greater proportions of lipid anomalies (p = 0.51) and hypertension (p = 0.44) but not statistically significant. In contrast, 21% of the sample comprised a plant-based diet cluster and suffered from all deficiencies but folate (p = 0.625), zinc (p = 0.132), and greater proportion from obesity (p = 0.19; not significant), and diabetes (p < 0.001). A cluster of "convenient" (20%) mainly suffered from lipid anomalies (p = 0.00), diabetes (p = 0.03), and a greater proportion from hypertension (p = 0.56) with deficiencies of all the essential vitamins and minerals but significantly from vitamin A (p < 0.001), folate (p < 0.001), and iron (p = 0.017). Lastly, the cluster of those who follow a "Western diet" (17%) was found to have lipid anomalies (p = 0.003), diabetes (p = 0.016), greater proportion of vitamin B12 (p = 0.136), D (p = 0.002), folate (p < 0.001), and iron deficiencies (p = 0.013). Conclusions and relevance: Adolescents in India show a strong association between obesogenic diet and metabolic syndrome. Therefore, the burden of metabolic syndrome at early ages can be prevented by controlling obesogenic dietary practices and addressing micronutrient deficiencies. This may be done by targeted health promotional campaigns in schools and college-going populations in India.
Progress and challenges in implementing adolescent and school health programmes in India: a rapid review

Abstract:Objectives: To review the overall planning, implementation and monitoring of adolescent and school health programmes currently implemented in India and determine if they are in alignment with the indicators for achieving universal health coverage for adolescents in India. Methods: A rapid review, with key informant interviews and desk review, was conducted using World Health Organization's tool for Rapid Assessment of Implementation of Adolescent Health and School Health Programmes. Operational guidelines, reports and relevant publications (surveys, policy briefs and meeting proceedings) related to India's adolescent and school health programmes were reviewed. Key informant interviews were conducted in New Delhi (India) with senior officials from the health and education departments of the Government of India, representatives from the private health sector and civil society organisations. Data were analysed using World Health Organization's framework for universal health coverage for adolescents and summarised according to the key indicators. Results: Key informant interviews were conducted with 18 participants: four each from health and education department of the government, one clinician from private health sector and nine representatives from civil society organisations. Manuals and operational guidelines of India's existing adolescent and school health programmes were reviewed. India's national adolescent and school health programmes align with many priority actions of the World Health Organization's framework for delivering universal health coverage for adolescents. These programmes require strengthening in their governance and implementation. While adolescent health and school health programmes have robust monitoring frameworks, however, there is a need to strengthen research and policy capacity. Conclusions: Various national health programmes have targeted adolescents as a priority population. A better translation of these programmes into implementation is needed so that the investments provided by the government offer sufficient opportunities for building collective national action for achieving universal health coverage with adolescents as an important section of the population.
Nutrition Transition and Obesity Among Teenagers and Young Adults in South Asia

Abstract:Introduction: Obesity among teenagers/adolescents and young adults is associated with significant adverse short and longer-term effects on health. To date, no narrative reviews have evaluated nutrition transition and its contribution to the obesity epidemic among adolescents and young adults in the South Asian (SA) region. Method: Data were retrieved by a four-stage systematic search process. A search of the online Pub- Med/Medline, SciVerse Scopus and Web of Science databases was performed. The age groups were defined as follows; teenage:13-19 years, adolescence:10-18 years and young adult:19-24 years. Results: Among teenagers/adolescents, the prevalence of overweight ranged from 11.0% (Sri Lanka) to 19.0% (India), while obesity ranged from 2.4% (Sri Lanka) to 11.0% (Pakistan). In young adults, prevalence of overweight ranged between 7.9% (Nepal) to 15.0% (Pakistan), while obesity showed a much wider variation (0.005%[Nepal] - 22.8%[India]). Nutritional risk factors associated with overweight/ obesity among SAs of this age group included reduced fruit and vegetable consumption, a total vegetarian diet, consumption of fast food and soft drinks, and skipping breakfast. Other contributing factors identified were: adding extra salt to meals, eating meals outside of the home, frequently visiting restaurants and eating while watching television. Daily milk/yoghurt consumption and a family supper have shown a protective effect against overweight/obesity. Conclusion: Overweight and obesity are common amongst teenagers/adolescents and young adults of the SA region. Several food types and habits were identified as being associated with overweight/ obesity in this population. Identifying common protective and contributory factors is very important for the development of a shared regional preventive strategy.
Availability and Utilization of Sexual and Reproductive Health Services among Adolescents of Godawari Municipality, Nepal: a Cross sectional Study

Abstract:Background: Adolescent sexual and reproductive health is getting due attention in developing countries because investing among adolescent can bring multiple benefits. This study aims to assess the availability, utilization and explore different factors affecting the utilization of SRH services by adolescents of Godawari Municipality. Methods: cross-sectional, mixed-method study was conducted using the Anderson's model of health service utilization in 3 government secondary schools and three local health institutions in Godawari Municipality. A self-administered questionnaire was used among adolescents aged 15-19 (n = 416) and Key Informants Interview (KII) and observation were done in health institutions. Quantitative data was analyzed in Statistical Package for Social Sciences (SPSS) version 16 and thematic analysis was done for qualitative data. Results: The mean age of the respondents was 17.31±0.99 years. Nearly one-third of them (30.63%) had not heard about Adolescents Friendly Health Services. Knowledge of ASRH, AFHS (AOR=2.814, 95% C.I=1.129-7.013) and conversation with parents (AOR=2.069, 95% C.I=1.094-3.912), availability of ASRH (AOR=2.801, 95% C.I=0.810-9.687) had significant relationship with utilization of Health Services. The adolescents’ perceived feeling of the need for ASRH services is significantly associated with the utilization of ASRH services (p=< .0001). Only 13.22% of them had ever used ASRH services and the reasons for not using services was the lack of realization of the need of services (60.39%) while 14.55% of them felt that privacy was not maintained at all. The KII found the number of adolescents visiting Health Facility (HF) was very less and those visiting for the SRH related services were rare. The observation showed none of the HF met the adolescents friendly criteria set by government even when they were once certified. Conclusion: Knowledge of respondents and their parents as well as availability of the services had significant role in the utilization of ASRH service by the adolescents. Awareness about the importance of health care services among the adolescent and the availability of services is essential.
Knowledge, attitude, and practices of adolescents and peer educators in relation to the components of the National Adolescent Health Program in India: findings from a cross-sectional survey

Abstract:Background: Adolescence is a critical period of growth and development. Many adverse health outcomes in adulthood begin during adolescence, often due to insufficient knowledge and attitudes resulting from a lack of education. Therefore, appropriate knowledge, attitudes, and practices (KAP) regarding various aspects of health are essential for holistic adolescent and lifelong health. In India, the Rashtriya Kishore Swasthya Karyakram (RKSK or National Adolescent Health Strategy) has utilized an innovative peer-education approach to engage with adolescents and improve their KAPs. Amid limited evidence, we aimed to assess the KAP of adolescents regarding the six themes of the RKSK, with a particular focus on the status of peer educators (PEs). Our objective was to evaluate these aspects disaggregated by sex and to examine how engagement with the RKSK peer-education program influenced their KAP. Methods: A cross-sectional survey of 238 peer educators and 2885 adolescents enrolled under peer educators was conducted in two localities; Madhya Pradesh and Maharashtra states. KAPs were estimated using descriptive statistics then disaggregated by gender. Practice scores of nutrition and non-communicable disease (NCD) were modelled upon engagement with RKSK (graded as 0, 1, 2, 3). Results: Knowledge was highest regarding substance misuse and lowest in the domains of sexual and reproductive health, and violence and injury. PEs possessed greater knowledge in most domains as compared to adolescents enrolled under them. Attitudes toward abstention from substance misuse were positive, whereas attitudes toward injury and violence, and sexual health, were suboptimal. Boys exhibited better practices related to NCDs, while their nutritional practices were comparatively worse than girls. The RKSK engagement was associated with better nutritional practices: adjusted relative risks (RRs) being 1.04 (95% confidence interval [CI]: 0.94-1.15), 1.12 (1.04-1.21), and 1.21 (1.13-1.31), respectively, for engagement scores 1, 2, and 3 with reference to score 0. The relationship between RKSK engagement and NCD-related practices was restricted to the top engagement group. Conclusion: The knowledge regarding sexual health, and injury and violence, was grossly deficient in adolescents. These components must be prioritized in the program because they are critical for health not only across the life course of individuals but also across generations. However, the RKSK engagement was associated with better practices in a variety of domains, which should be leveraged in the future.
Accurate biopsy-needle depth estimation in limited-angle tomography using multi-view geometry

Abstract:Recently, compressed-sensing based algorithms have enabled volume reconstruction from projection images acquired over a relatively small angle (θ < 20°). These methods enable accurate depth estimation of surgical tools with respect to anatomical structures. However, they are computationally expensive and time consuming, rendering them unattractive for image-guided interventions. We propose an alternative approach for depth estimation of biopsy needles during image-guided interventions, in which we split the problem into two parts and solve them independently: needle-depth estimation and volume reconstruction. The complete proposed system consists of the previous two steps, preceded by needle extraction. First, we detect the biopsy needle in the projection images and remove it by interpolation. Next, we exploit epipolar geometry to find point-to-point correspondences in the projection images to triangulate the 3D position of the needle in the volume. Finally, we use the interpolated projection images to reconstruct the local anatomical structures and indicate the position of the needle within this volume. For validation of the algorithm, we have recorded a full CT scan of a phantom with an inserted biopsy needle. The performance of our approach ranges from a median error of 2.94 mm for an distributed viewing angle of 1° down to an error of 0.30 mm for an angle larger than 10°. Based on the results of this initial phantom study, we conclude that multi-view geometry offers an attractive alternative to time-consuming iterative methods for the depth estimation of surgical tools during C-arm-based image-guided interventions.

Abstract:World Health Organization (WHO) defines adolescence as the period of life between 10and 19 year of age. The adolescent experiences not only physical growth and change but also emotional, psychological, social, and mental change and growth. Physiological changes change lead to sexual maturity and usually occur during the first several year of this period. Adolescence represents a window of opportunity to prepare for a healthy adult life. The world’s adolescent population-1200 million persons, 10-19 year of age or about 19% of the total population-faces series of serious challenges not affecting their growth and development but also their livelihood as adults. Yet adolescents remain a largely neglected, difficult-to-measure, and hard-to-reach population, in which the needs of adolescent girls in particular are often ignored. Adolescence is period of increased risk taking and therefore susceptibility to behavioural problems at the time of puberty and new concerns about reproductive health. Majority of adolescents still do not have access to information and education on sexuality, reproduction, and sexual and reproductive health and rights, nor do they have access to preventive and preventive curative service. Commercial innovation to address health needs at the bottom of the pyramid for more than 800 million men, women and children across India living on USD 1-3 ADAY, the idea if accessible and affordable medicine is often as remote as their rural homes. Arogya Parivar (“health family’’ in Hindi) is a for-profit social initiative developed by Novartis to reach the undeserved million living at the bottom of the pyramid in rural India. After just years, Arogya Parivar is proving to be both a force for improving rural health programme in rural community and a sustainable business.

The interference is much less with bromcresol purple. It can be abolished by addition of hexadimethrine bromide.

Abstract:Background: Adolescents are generally considered healthy people in the community and their health aspects are usually neglected. Adolescent population is nearly 1.3 billion worldwide accounting to 16% of the world population (UNICEF). Suicide is the fourth leading causes of death among 15-29 years old. Aim: To assess the prevalence of depression among adolescents in Rural area of AP, to determine factors associated with depression and to assess need for motivational speakers/ school psychologists visit to schools. Methodology: This is a cross sectional study done among adolescents in V.kota region of AP,7 schools and the participants in the school were selected by simple random sampling technique, and after taking written consent, participants were interviewed using questionnaire which consists of sociodemographic details, risk factors assumed to be associated with depression, Becks Depression Inventory, students opinion on the need for motivational speakers and psychologists visit to schools and statistical analysis was done using SPSS. Result: In this study, the prevalence of depression among adolescents was 2.7% and 28.57% (2/7), 57.14% (4/7) and 14.28% (1/7) of the depressed individuals have been found to have Mild, Borderline and Moderate depression. Factors like birth order, family history, family environment, stressful events like interpersonal losses, relationship failure, exposure to suicide, social deprivation/rejection, factors which make individuals feel low and disputes with family/friends/others were found to be statistically associated with depression(p<0.05). About 70.35% of the study participants felt the need for motivational speakers and psychologists visits to schools, around 76.28% of the adolescents were motivated by parents/teachers/friends /relatives/others, and 57.14% of the depressed individuals were not willing to reach out for help. Conclusion: Rural adolescents are also exposed to various risk factors which make them equally vulnerable as of their urban counterparts. There is a need for regular psychologists and motivational speakers visit to schools which helps adolescents in coping with stress thereby decreasing the incidence of depression among them

Abstract:Objective: To generate national estimates of key non-communicable disease (NCD) risk factors for adolescents (15-17 years) identified in the National NCD Monitoring Framework and, study the knowledge, attitudes and practices towards NCD risk behaviours among school-going adolescents. Design and setting: A community-based, national, cross-sectional survey conducted during 2017-2018. The survey was coordinated by the Indian Council of Medical Research-National Centre for Disease Informatics and Research with 10 reputed implementing research institutes/organisations across India in urban and rural areas. Participants: A multistage sampling design was adopted covering ages between 15 and 69 years-adolescents (15-17 years) and adults (18-69 years). The sample included 12 000 households drawn from 600 primary sampling units. All available adolescents (15-17 years) from the selected households were included in the survey. Main outcome measures: Key NCD risk factors for adolescents (15-17 years)-current tobacco and alcohol use, dietary behaviours, insufficient physical activity, overweight and obesity. Results: Overall, 1402 households and 1531 adolescents completed the survey. Prevalence of current daily use of tobacco was 3.1% (95% CI: 2.0% to 4.7%), 25.2% (95% CI: 22.2% to 28.5%) adolescents showed insufficient levels of physical activity, 6.2% (95% CI: 4.9% to 7.9%) were overweight and 1.8% (95% CI: 1.0% to 2.9%) were obese. Two-thirds reported being imparted health education on NCD risk factors in their schools/colleges. Conclusion: The survey provides baseline data on NCD-related key risk factors among 15-17 years in India. These national-level data fill information gaps for this age group and help assess India's progress towards NCD targets set for 2025 comprehensively. Though the prevalence of select risk factors is much lower than in many developed countries, this study offers national evidence for revisiting and framing appropriate policies, strategies for prevention and control of NCDs in younger age groups.
Stunting and Underweight among Adolescent Girls of Indigenous Communities in Telangana, India: A Cross-Sectional Study

Abstract:India's indigenous groups remain vulnerable to malnutrition, despite economic progress, reflecting the reliance on traditional agriculture and the problems of poverty and inadequate education and sanitation. This mixed-methods study analyzed the incidence, causes and determinants of chronic malnutrition, measured through stunting, thinness and underweight among adolescent indigenous girls in Telangana. Using 2017 data on 695 girls aged 11–18 years from 2542 households, the analysis showed that 13% had normal nutritional status, while 87% were stunted, underweight or thin. Early adolescents (11–14 years) had higher underweight prevalence (24.4%), while late adolescents (15–18 years) showed greater stunting (30%). Regressions identified key influencing factors. Higher education levels of heads of households and the girls themselves alongside household toilet access significantly improved nutritional status and reduced stunting and underweight. The sociocultural emphasis on starchy staple-based diets and early marriage also impacted outcomes. Tackling this crisis requires mainstreaming nutrition across development agendas via comprehensive policies, education, communication and community participation. Further research can guide context-specific solutions. But, evidence-based investments in indigenous education, livelihoods, sanitation and women's empowerment are the first steps. Nutrition-sensitive development is indispensable for indigenous groups to fully participate in and benefit from India's progress.

The prevalence of anaemia in rural adolescent girls - A cross-sectional study to understand the sociodemographic and dietary determinants in Dharwad District, India

Abstract:Context: Anaemia is one leading cause of morbidity among adolescent girls. Prevention, early detection, and treatment can break the intergeneration cycle of malnutrition. Aims/objectives: The aim of the study was to estimate the prevalence of anaemia and assess its sociodemographic determinants and understand its association with the dietary pattern of rural adolescent girls. Settings and design: A cross-sectional study was conducted among 350 adolescent girls studying in the school of Kalgatigi Taluk, Dharwad district. The school girls were interviewed using a predesigned structured questionnaire by interview method. A food frequency questionnaire was used to assess the dietary pattern. Haemoglobin levels were estimated using Sahli's method. Statistical analysis used: Data were entered in Microsoft Excel and analysed using SPSS version 19 and the Chi-square test was applied to study the significance. Results: Rural adolescent school girls between the age group of 13-16 years were included. The prevalence of anaemia among them was found to be 47.4% and was significantly associated with the educational status of the mother, their socioeconomic status (SES), and type of diet. Vegetarian girls had a higher prevalence of anaemia. The frequency of meat and egg consumption was significantly associated with anaemia. The prevalence of anaemia was higher in thin and severely thin girls based on WHO-BMI. Conclusions: Anaemia in rural adolescent girls is of high burden and public health importance with a prevalence of 47.4% and significant association with dietary habits, SES, and BMI.

Abstract:Background: Youth are consideren to be most vulnerable to health and lifestyle issues (HLS) in India. The current study aims to investigate the factors that contribute to health and lifestyle issues among youth attending mental health promotion clinics (YMHP) in Karnataka. Method: Three-year first-visit data from beneficiaries (aged 15-35 years) attending YMHP clinics in Karnataka between 2017 and 2020 were analyzed. Multivariable logistic regression analysis included beneficiaries reporting any HLS issue as the outcome and a host of 57 hypothesized variables as exposures. Results: Overall, 2,615 (25%) beneficiaries reported HLS issues. Years of schooling (AOR 5-7 years = 0.89; 95% confidence interval [CI] =0.60-1.31), (AOR 8-10 years = 0.65; 95% CI = 0.46-0.91), (AOR >10 years = 0.67; 95% CI = 0.49-0.93)], unemployed youth (AOR = 0.52; 95% CI = 0.45-0.61) business and salaried workers (AOR = 1.69; 95% CI = 1.33-2.13), and other occupations (AOR = 2.11; 95% CI = 1.73-2.56), junk food consumption (AOR = 0.76;95% CI = 0.68-0.84), having issues related to relationships with parents (AOR = 3.01; 95% CI = 2.47-3.68) and intergenerational issues (AOR = 1.71; 95% CI = 1.19-2.45), self-development issues (AOR low-self-awareness = 1.57; 95% CI = 1.33-1.85), (AOR low-self-esteem = 1.29; 95% CI = 1.062-1.57), (AOR emotional issues = 1.57; 95% CI = 1.31-1.89), education and academics (AOR = 1.23; 95% CI = 1.09-1.39), safety issues (AOR = 4.11; 95% CI = 3.07-5.50), gender sex and sexuality issues (AOR = 2.44; 95% CI = 1.43-4.15), suicidal ideation (AOR = 1.91; 95% CI = 1.44-2.54), substance use (AOR tobacco chewing = 1.45; 95% CI = 1.09-1.93), (AOR tobacco-smoking = 1.66; 95% CI = 1.18-2.32), (AOR smoking = 4.94; 95% CI = 3.52-6.93) and experiencing emotions (AOR feel anxious = 1.63; 95% CI = 1.41-1.88), (AOR forgetfulness = 1.50; 95% CI = 1.41-1.98), (AOR difficulty in concentration = 1.37; 95% CI = 1.035-1.81), (AOR anger = 1.61; 95% CI = 1.25-2.07), (AOR feel worthless = 2.21; 95% CI = 1.71-2.86) were associated with HLS issues among beneficiaries. Conclusion: This analysis addresses an important but neglected component of HLS issues among youth highlighting the importance of early intervention among youth to prevent the development of diseases later in life. The study has important implications for youth health promotion in India and countries such as India.
