Background:We evaluate the association of migraine before and during pregnancy with the development of major adverse vascular events (MAVE) during an extended postpartum period of 24 weeks.Methods:We identified pregnant women aged between 12 and 44 years from the state inpatient databases of New York, Florida, Georgia, Maryland, and Washington (2016 - 2019). Patients were followed up for up to 24 weeks postpartum. Patients with migraine (ICD-10 code: G43) before or during pregnancy were considered to have a history of migraine. We fit multivariable logistic regression models to evaluate the association between the history of migraine and the development of MAVE (stroke, acute coronary syndrome, systemic thromboembolism, hypertensive encephalopathy, pulmonary heart disease, or death) up to 24 weeks after delivery. Patients with a history of MAVE before or during pregnancy were excluded. Adjusted odds ratios (aOR) and 95% confidence interval (CI) are reported.Results:Among 1,568,646 eligible participants (mean age [SD]: 29 [5.7] years), 43,840 (2.79%) had a documented history of migraine. Women with a history of migraine (vs. those without a history of migraine) had higher odds of experiencing MAVE (268/43840 [0.61%] vs. 4556/1524806 [0.3%], aOR, 95% CI: 1.47, 1.30 - 1.67). Also, older age (aOR, CI: 1.02, 1.02 - 1.03), smoking during pregnancy (2.14, 1.97 - 2.32), and pre-eclampsia (1.87, 1.72 - 2.03) were independently associated with a higher likelihood of developing postpartum MAVE. Additionally, those with a history of kidney failure (2, 1.76 - 2.27), liver disease (1.32, 1.11 - 1.56), and heart failure (1.59, 1.35 - 1.86) had higher odds of experiencing MAVE. Furthermore, those with a history of hypertension (1.23, 1.13 - 1.33), diabetes (1.30, 1.17 - 1.45), obesity (1.31, 1.22 - 1.41), and coagulopathy (2.36, 2.17 - 2.57) were more likely to experience MAVE.Conclusions:Migraine seems to be independently associated with a higher risk of post-partum MAVE. Further studies are needed to assess the utility of extended workup for evaluating the risk of MAVE among pregnant females with migraines.

Abstract:Objective: The aim of the study was to determine if migraine is associated with fetal-type posterior cerebral artery (PCA) in patients with ischemic stroke. Method: In this cross-sectional study, patients with acute ischemic stroke were enrolled from two hospitals. The history of migraine headache was evaluated during a face-to-face interview. The variants of fetal-type PCA were assessed with MRA, CTA, or DSA. Patients with and without migraine were compared in terms of fetal-type PCA status and other clinic characteristics. Multivariate logistic regression analyses were performed to adjust for confounders and provide risk estimates for observed associations. Result: In 750 patients qualified for analysis, 85 (11.3%) were determined with migraine. Patients with migraine had a higher proportion of female gender (51.8% vs. 31.0%, p < 0.001), hypertension (72.9% vs. 57.7%, p = 0.007), and fetal-type PCA (36.5% vs. 20.1%, p = 0.001), while lower proportion of current smoking (25.9% vs. 38.3%, p = 0.025) than patients without migraine. National Institutes of Health Stroke Scale (NIHSS) score (3 vs. 2, p = 0.016) was also higher in migraineurs than in non-migraineurs. After adjustment for confounders, fetal-type PCA status was independently associated with migraine (odds ratio [OR] = 2.06; 95% confidence interval [CI], 1.25-3.38; p = 0.005). Other factors associated to migraine included female gender (OR = 2.03; 95% CI, 1.13-3.62; p = 0.017), hypertension (OR = 1.97; 95% CI, 1.17-3.34; p = 0.011), and NIHSS score (OR = 1.08; 95% CI, 1.01-1.16; p = 0.018). Conclusion: Migraine was associated with fetal-type PCA in patients with ischemic stroke. This finding supported the hypothesis that vascular mechanisms get involved in the migraine-stroke association.

Abstract:Cephalalgia, Volume 44, Issue 2, February 2024. BackgroundMigraine is common in reproductive aged women. Understanding the impact of migraine and associated treatments on pregnancy outcomes remains very important. An umbrella review of systematic reviews, with or without meta-analyses, examined the link between migraine and pregnancy outcomes.MethodsWe systematically searched Medline, Embase and Cochrane to 27 October 2022. Quality appraisal was carried out using the AMSTAR2 tool. An established framework was used to determine whether included reviews were eligible for update.ResultsFour studies met review criteria. Migraine was reported to be associated with increased odds ratio (OR) of pre-eclampsia, low birth weight and peripartum mental illness (pooled OR = 3.54 (2.24–5.59)). Triptan-exposed women had increased odds of miscarriage compared to women without migraine (pooled OR = 3.54 (2.24–5.59)). In updated meta-analyses, migraine was associated with an increased odds of pre-eclampsia and preterm birth (pooled OR = 2.05 (1.47–2.84) and 1.26 (1.21–1.32) respectively).ConclusionsMigraine is associated with increased odds of pre-eclampsia, peripartum mental illness and preterm birth. Further investigation of the relationship between migraine and placental abruption, low birth weight and small for gestational age is warranted, as well as the relationship between migraine, triptans and miscarriage risk.Systematic Review Registration: Prospero CRD42022357630

We hypothesize that in individuals who are pregnant or within 1 year postpartum (PP), the triad of early PP state (first 6 weeks after delivery), elevated blood pressure (BP), and headache is associated with higher stroke risk.Methods:We conducted a single-institution case-control study. Cases had a maternal stroke (ischemic, hemorrhagic, subarachnoid hemorrhage, or cerebral venous thrombosis) during pregnancy or PP, identified from a stroke registry. Controls were pregnant or PP but without any stroke and admitted for any reason, identified from a cohort of individuals with a high proportion of hypertensive disorders of pregnancy (HDP), a known maternal stroke risk factor. We created multivariable logistic regression models with stepwise backward elimination to evaluate the association between maternal stroke and the triad of (1) early PP; (2) systolic BP ≥140 or diastolic ≥90 mmHg; and (3) headache.Results:We identified 60 cases and 113 controls. Population characteristics are shown in the Table. Cases had a higher proportion of preexisting headache disorders and controls a had higher proportion of HDP. Adjusting for race, HDP, and headache disorder history, stroke risk was higher in those presenting to acute care with the proposed triad than in those without these features (adjusted odds ratio 13.6, 95% confidence interval 3.8-48.5, p<0.0001). In a predictive model, the triad exhibited excellent discrimination between cases and controls (AUC 0.82, sensitivity 30%, specificity 98%, positive predictive value 90%).Conclusions:In a high risk, diverse maternal population delivering at an urban academic medical center, the triad of early PP state, elevated BP, and headache was associated with a more than 13-fold higher risk of stroke. Patients presenting for acute care with this triad could potentially benefit from a comprehensive stroke evaluation.

The incidence of pregnancy-related stroke: A systematic review and meta-analysis

Abstract:Background Stroke risk is increased during pregnancy, but estimates of pregnancy-related stroke incidence vary widely. Aims A systematic review and meta-analysis was conducted to assess the incidence of stroke during pregnancy and the puerperium. Ovid Medline, EMBASE, and ISI Web of Science were searched for studies published between 1990 and January 2017 reporting stroke incidence during pregnancy and postpartum, from defined pregnancy populations. Pooled analyses were conducted using a random effects approach and expressed as an incidence rate per 100,000 pregnancies, with 95% confidence intervals. Subgroup analyses of stroke type and timing were conducted. Summary of review Eleven studies met inclusion criteria. Variation in estimated rates was noted based on geography and study methodology. The pooled crude rate of pregnancy-related stroke was 30.0 per 100,000 pregnancies (95% confidence interval 18.8-47.9). The pooled crude rates from nonhemorrhagic stroke (arterial and cerebral venous sinus thrombosis) were 19.9 (95% confidence interval 10.7-36.9) and from hemorrhage 12.2 (95% confidence interval 6.4-23.2) per 100,000 pregnancies. For studies separately reporting cerebral venous sinus thrombosis, the rates were roughly equal between ischemic stroke (12.2, 95% confidence interval 6.7-22.2), cerebral venous sinus thrombosis (9.1, 95% confidence interval 4.3-18.9), and hemorrhage (12.2, 95% confidence interval 6.4-23.2). The crude stroke rate for antenatal/perinatal stroke was 18.3 (95% confidence interval 11.9-28.2), and for postpartum stroke was 14.7 (95% confidence interval 8.3-26.1). Conclusions Stroke affects 30.0 per 100,000 pregnancies, with ischemia, cerebral venous sinus thrombosis, and hemorrhage causing roughly equal numbers and with highest risk peripartum and postpartum. Organized approaches to the management of this high-risk population, informed by existing evidence from stroke and obstetrical care are needed.

Abstract:Migraine and stroke are two common and heterogeneous neurovascular disorders responsible for a significant burden for those affected and a great economic cost for the society. There is growing evidence that migraine increases the overall risk of cerebrovascular diseases. In this review, based on available literature through a PubMed search, we found that ischaemic stroke in people with migraine is strongly associated with migraine with aura, young age, female sex, use of oral contraceptives and smoking habits. The risk of transient ischaemic attack also seems to be increased in people with migraine, although this issue has not been extensively investigated. Although migraine appears to be associated with haemorrhagic stroke, the migraine aura status has a small influence on this relationship. Neuroimaging studies have revealed a higher prevalence of asymptomatic structural brain lesions in people with migraine. They are also more likely to have unfavourable vascular risk factors; however, the increased risk of stroke seems to be more apparent among people with migraine without traditional risk factors. The mechanism behind the migraine-stroke association is unknown. In light of the higher risk of stroke in people with migraine with aura, it is important to identify and modify any vascular risk factor. There is currently no direct evidence to support that a migraine prophylactic treatment can reduce future stroke in people with migraine.

Trends in the prevalence and pharmacological management of migraine during pregnancy in the UK, 2000–2018

Abstract:Background Migraine is common in women of reproductive age. This study aimed to (1) describe the prevalence of migraine in pregnant women in the UK, (2) identify drugs commonly prescribed for migraine during pregnancy and (3) identify characteristics associated with being prescribed medication for migraine during pregnancy. Methods The Clinical Practice Research Datalink pregnancy register, a database of pregnancy episodes identified in anonymised primary care health records, was used. Crude and age-standardised prevalence of migraine during pregnancy and the proportion of women with migraine prescribed drugs used for migraine management were calculated for each year between 2000 and 2018. Logistic regression was used to describe the relationship between patient characteristics and being prescribed migraine medication during pregnancy. Results 1 377 053 pregnancies were included, of which 187 328 were in women with a history of migraine. The age-adjusted prevalence increased from 11.4% in 2000 to 17.2% in 2018. There was an increase in the rates of prescription for numerous medications for the management of migraine. Older women (adjusted OR (aOR) 1.41 (1.20 to 1.66)), women of black (aOR 1.40 (1.32 to 1.48)) and South Asian ethnicity (aOR 1.48 (1.38 to 1.59)), those living in the most deprived areas (aOR 1.60 (1.54 to 1.66)), women who were obese (aOR 1.39 (1.35 to 1.43)), smokers (aOR 1.15 (1.12 to 1.18)) and those with comorbid conditions were more likely to receive a prescription during pregnancy. Conclusions Rates of recorded migraine have increased over the past two decades as well as rates of prescribing in women with migraine. Higher prescribing rates are seen in certain groups, which has the potential to exacerbate health inequalities.

Migraine's episodic manifestation and acute and preventive pharmacological treatment options challenge studying drug safety for this condition during pregnancy. To improve such studies, we aimed to develop algorithms to identify and characterize migraines in electronic healthcare registries and to assess the level of care.MethodsWe linked four registries to detect pregnancies from 2009–2018 and used three algorithms for migraine identification: i) diagnostic codes, ii) triptans dispensed, and iii) a combination of both. We assessed migraine severity using dispensed drugs as proxies. ICD-10 diagnostic subcodes of migraine (G43) allowed the allocation of four subtypes: complicated and/or status migrainosus; with aura; without aura; other/unspecified.ResultsWe included 535,089 pregnancies in 367,908 women with available one-year lookback. The prevalence of migraines identified was 2.9%–4.3% before, and 0.8%–1.5% during pregnancy, depending on algorithm used. Pregnant women with migraine were mostly managed in primary care.ConclusionsPrimary care data in combination with drug dispensation records were instrumental for identification of migraine in electronic healthcare registries. Data from secondary care and drug dispensations allow better characterization of migraines. Jointly, these algorithms may contribute to improved perinatal pharmacoepidemiological studies in this population by addressing confounding by maternal migraine indication.

Abstract:Introduction: Migraine is an established risk factor for cerebral ischemic stroke, with an especially robust association in patients with migraine with aura. However, it is not known if migraine is a risk factor for retinal stroke (central or branch retinal artery occlusion; CRAO or BRAO). Methods: We performed a retrospective, observational, cohort study using population-based data from the State Inpatient Databases and State Emergency Department Databases from New York (2006-2015), California (2003-2011), and Florida (2006-2015) to determine the association between hospital-documented migraine and retinal stroke. The primary exposure was hospital-documented migraine (ascertained from admission or emergency department diagnosis codes). The primary endpoint was time to hospital-documented CRAO (ICD-9-CM code 362.31 in the primary diagnosis position) and secondary endpoints included time to BRAO and any retinal artery occlusion (RAO). Cause-specific hazard models were used to model the association between migraine and subsequent CRAO. Results: Of 39,835,024 patients included in the study, 1109,140 had migraine documented during our two year ascertainment window. Patients with migraine were younger (40.2±15.2 vs. 46.9±19.8, standardized difference (SD) 0.38), more likely to be female (81.4 % vs. 54.7 %, SD 0.6), and had a lower burden of atrial fibrillation (4.5 % vs. 6.9 %, SD 0.1), chronic kidney disease (1.9 % vs. 3.6 %, SD 0.2), and congestive cardiac failure (2.7 % vs. 5.1 %, SD 0.12). Migraine was not associated with CRAO in the primary diagnostic position (adjusted hazard rate (aHR) 1.15 (95 % CI: 0.79-1.67). However, it was associated with CRAO in any diagnostic position (aHR 1.39 (95 % CI: 1.08-1.78). As positive controls, we replicated previously established associations of migraine with cerebral ischemic stroke (aHR 1.35 (95 % CI: 1.32-1.38) and embolic ischemic stroke (aHR 1.15 (95 % CI: 1.08-1.22). Conclusions: In a large, nationally-representative, claims-based study of patients from 3 regions in the United States (US), we did not find an adjusted association between migraine and a primary discharge diagnosis of CRAO. Our hypothesis-generating finding that migraine was associated with CRAO when using a broader definition sets the stage for future work leveraging both outpatient and pharmacy based claims to further explore this finding.

A few observational studies from different demographics showed that pregnant women with a history of migraine were at higher risk of developing pre-eclampsia. However, there is no such evidence available from the Indian context. Hence, a hospital-based case–control study was conducted among Indian women to determine the association between migraine and pre-eclampsia.

Abstract:Migraine is a common neurological disorder in clinical practice, characterized by recurrent attacks of chronic neurovascular disease. It is a common primary headache, typically presenting as unilateral or bilateral pain often accompanied by nausea, vomiting, and, in some typical cases, visual, sensory, and motor disturbances known as auras, with a potential family history. Recent epidemiological data shows that the incidence of migraine is approximately 15% to 25%, with an annual incidence of about 250 per 100,000 people, affecting approximately 1 billion individuals globally. [ 1 ] The Global Burden of Disease Study in 2016 identified migraine as one of the leading causes of disability worldwide. [ 2 ] About 90% of migraine patients experience their first attack before the age of 50, characterized by recurrent unilateral or bilateral moderate to severe pulsating headaches, often accompanied by photophobia or gastrointestinal symptoms. Additionally, neurological symptoms such as dizziness and cognitive impairment may also be present. [ 3 ] Studies estimate that the incidence of migraine in the United States is approximately 12%, while the global prevalence is higher, at about 14.4%, with 2.5% of episodic migraines progressing to chronic migraines. [ 4 ] Approximately one-third of migraine patients experience aura symptoms, including visual disturbances, sensory impairments, language or motor symptoms, with visual disturbances being the most common aura symptom. Migraines with aura are referred to as "migraine with aura," while those without are referred to as "migraine without aura."

Abstract:Introduction: Pregnancy is a risk factor for subarachnoid hemorrhage (SAH). Albeit rare, it can be a devastating condition with high rates of mortality and morbidity. We aim to better understand this risk and its impact on in-hospital outcomes over a longitudinal time period in a large population based sample. Methods: We analyzed the 2000-2019 National Inpatient Sample, selecting for aneurysmal SAH (aSAH). Pregnant patients were identified. Demographics, comorbidities, and outcomes were compared between pregnant and not pregnant groups. We used T-tests and chi-squared tests , and created a 1:1 propensity score matched cohort. Variables with notable differences (>0.1 standardized mean) were used for multivariable regression, determining adjusted odds ratios (AOR) and beta coefficients for the impact of pregnancy on outcomes. Results: Of the 662,981 SAH patients identified, 0.82% were pregnant. SAH incidence in these patients increased over our study period. Pregnant patients were more often African American (34.9% vs 15.2%) and were more likely to have Medicaid insurance (43.4% vs 12.5%). They had a lower Elixhauser Comorbidity Score (6.01 vs 8.52). They had less hydrocephalus (8.7% vs 23.0), coma (5.4% vs 11.5%), and ventilator use (16.3% vs 28.4%). However, they experienced seizures more often (18.1% vs 12.3). Their rate of in-hospital mortality was lower (7.3% vs 24.2%), and they were more frequently discharged to home or short term hospitals (72.9% vs 38.8%). After propensity matching, pregnant patients had a reduced risk of in-hospital mortality (AOR 0.47, p<0.001). They were also more likely to be discharged home or to short term hospital although these were not statistically significant to our pre-specified alpha. Conclusion: sSAH in pregnancy appears to be very rare and is associated with better outcome compared to the general population, as the majority of pregnant patients with aSAH present with favorable neurological status.

Abstract:Introduction: Pregnancy and the postpartum period are associated with an increased risk of ischemic and hemorrhagic strokes. The incidence of missed or delayed diagnosis of pregnancy-associated stroke in the emergency setting is unknown. Methods: We conducted a retrospective cohort study of all women aged 12 years or older who were hospitalized for labor and delivery using deidentified administrative claims data from all nonfederal EDs and hospitals in New York from 2006-2016 and Florida from 2005-2018. In this cohort, we then identified women hospitalized for a cerebrovascular event (either ischemic or hemorrhagic stroke) using previously validated ICD-9-CM or ICD-10-CM codes during a 270-day period extending from 6 months before through 3 months after delivery. We defined probable ED misdiagnosis of stroke as having an ED treat-and-release visit for neurological complains (e.g., headache, dizziness, numbness) within the 14 days preceding their stroke hospitalization. Logistic regression was used to compare rates of clinical outcomes in pregnant or postpartum stroke patients with and without probable stroke misdiagnosis after adjusting for demographic and vascular risk factors. Results: Among 3,474,960 women hospitalized for labor and delivery, 350 (0.01%) were hospitalized for an ischemic or hemorrhagic stroke during the study period. Out of the 350 pregnant or postpartum women hospitalized for stroke, 17 (4.9%; median age 30) had at least one preceding ED visit for a neurological symptom (probable misdiagnosis). There was no difference in length of stay (4 versus 5 days, p = 0.97), mortality (0% versus 3.6%, p = 1.00), and discharge to home (70.6% versus 57.1%, p = 0.27) after stroke hospitalization between patients with versus without a probable ED misdiagnosis. Conclusion: Probable ED misdiagnosis occurred in nearly 1 of 20 cases of pregnancy-associated stroke in a large, multistate cohort. However, misdiagnosis was not associated with increased length of stay, higher mortality, or lower rates of discharge to home after stroke.

Abstract:Migraine affects up to 20 percent of the global population and ranks as the second leading cause of disability worldwide. In parallel, ischemic stroke stands as the second leading cause of mortality and the third leading cause of disability worldwide. This review aims to elucidate the intricate relationship between migraine and stroke, highlighting the role of genetic, vascular, and hormonal factors. Epidemiological evidence shows a positive association between migraine, particularly with aura, and ischemic stroke (IS), though the link to hemorrhagic stroke (HS) remains inconclusive. The shared pathophysiology between migraine and stroke includes cortical spreading depression, endothelial dysfunction, and genetic predispositions, such as mutations linked to conditions like CADASIL and MELAS. Genetic studies indicate that common loci may predispose individuals to both migraine and stroke, while biomarkers such as endothelial microparticles and inflammatory cytokines offer insights into the underlying mechanisms. Additionally, hormonal influences, particularly fluctuations in estrogen levels, significantly impact migraine pathogenesis and stroke risk, highlighting the need for tailored interventions for women. The presence of a patent foramen ovale (PFO) in migraineurs further complicates their risk profile, with device closure showing promise in reducing stroke occurrence. Furthermore, white matter lesions (WMLs) are frequently observed in migraine patients, suggesting potential cognitive and stroke risks. This review hopes to summarize the links between migraine and its associated conditions and ischemic stroke, recognizing the profound implications for clinical management strategies for both disorders. Understanding the complex relationship between migraine and ischemic stroke holds the key to navigating treatment options and preventive interventions to enhance overall patient outcomes.

We then identified women hospitalized for stroke (ischemic or hemorrhagic) using validated ICD-9-CM or ICD-10-CM codes during a 270-day period extending from 6 months before through 3 months after delivery. The primary study outcome was possible ED misdiagnosis of PAS, defined as an ED treat-and-release visit for a neurological complaint within the 30 days preceding PAS hospitalization. Standard tests of comparison were used to compare differences in characteristics between PAS patients with a possible ED stroke misdiagnosis versus those without. Results Among 5,308,962 women hospitalized for labor and delivery, 1,656 (0.03%) were hospitalized for a stroke during the study period. Of the pregnant or postpartum women hospitalized for stroke, 79 (4.8%; median age, 30 years) had at least one preceding ED visit for a neurological symptom (possible misdiagnosis). Demographics and vascular comorbidities were similar between those with versus without possible misdiagnosis. There were also no differences in the average length of stay (7.5 versus 9.6 days, p=0.43) or discharge to home (63.3% versus 56.6%, p=0.24) after stroke. Conclusions Possible ED misdiagnosis occurred in nearly 1 of 20 cases of pregnancy-associated strokes in this multistate cohort though there were few of these strokes overall.

Abstract:<h3 class="Heading">Purpose of the Review</h3><p class="Para">Maternal morbidity and mortality is rising in the USA, and maternal stroke is a major contributor. Here, we review the epidemiology, risk factors, and current recommendations for diagnosis and acute treatment of ischemic and hemorrhagic stroke during pregnancy and postpartum, focusing on recent evidence.</p><h3 class="Heading">Recent Findings</h3><p class="Para">The incidence of maternal stroke has risen in recent years, possibly due to increasing rates of hypertensive disorders of pregnancy. The risk of maternal stroke is highest in the peripartum and early postpartum period. Preeclampsia is highly associated with reversible cerebral vasoconstriction syndrome and posterior reversible encephalopathy syndrome and is also associated with long-term increased risk of stroke and vascular dementia. Hypertensive disorders of pregnancy, migraine, and infections are risk factors for maternal stroke. Limited data suggest that thrombolytics and endovascular reperfusion therapy are safe and effective in pregnant women with ischemic stroke, but few data are available regarding safety of thrombolytics in the postpartum period. New consensus guidelines are now available to assist with management of ischemic and hemorrhagic stroke in pregnancy.</p><h3 class="Heading">Summary</h3><p class="Para">Many gaps remain in our understanding of maternal stroke. While risk factors have been identified, there are no prediction tools to help identify which women might be at highest risk for postpartum stroke and require closer monitoring. The risk of recurrent maternal stroke has not been adequately quantified, limiting clinicians' ability to counsel patients. The complex pathophysiology of preeclampsia and its effects on the cerebral vasculature require further targeted study. An increased focus on the prevention, recognition, and optimal treatment of maternal stroke will be critical to reducing maternal morbidity and mortality.</p>

Abstract:Introduction: Pregnant patients are at risk of neurological complications including intracerebral hemorrhage (ICH). We aim to further characterize this risk and its in hospital consequences using a large population based sample over a 20 year study period. Methods: Using the Nationwide Inpatient Sample (2000-2019), we selected for patients who had ICH. We compared characteristics between pregnant and non-pregnant patients using t-tests and chi-squared tests. A 1:1 propensity score matched cohort was generated. Variables with standardized mean differences >0.1 were used in multivariable regression to generate adjusted odds ratios (AOR)/beta coefficients to determine the impact of pregnancy on outcomes. Results: In our study involving 2,000,675 ICH patients, 5,107 were pregnant (0.26%). We observed an increasing trend of ICH among pregnant individuals. Pregnant patients were notably younger (30.63 vs 63.38, p<0.001) and had fewer comorbidities such as cancer and chronic kidney disease, atrial fibrillation, hypertension, hyperlipidemia. Interestingly, a greater percentage of pregnant ICH patients African American (25.0% vs 16.2%, p<0.001). Pregnant patients experienced lower in-hospital mortality (12.4% vs 24.7%, p<0.001) and were more frequently discharged home or to short-term hospital (53.8% vs 23.2%, p<0.001). After propensity matching, pregnant patients shower lower odds of in-hospital mortality (AOR 0.68, p=0.008), but other outcomes including discharge outcomes and hospital charges remained similar. Conclusion: ICH is a rare complication in pregnancy, occurring in 0.26% of cases, and presenting roughly a 14% risk of in-hospital mortality. However, patients they appear to have lower rates of mortality than the overall ICH population. There may be a racial disparity in pregnancy related ICH as well, with African American experiencing this complication at a higher rate than other races.

Abstract:Background and aims: Epidemiological evidence of the association between migraines, severe headaches, and hypertension is contradictory. Hypertension is a critical risk factor for car-diovascular diseases. Migraine is a common neurological disease and a major cause of disability worldwide. Therefore, we aimed to investigate the relationship between migraine, severe head-aches, and hypertension among US adults.Methods and results: Cross-sectional data from 5716 subjects were obtained from the National Health and Nutrition Examination Survey between 1999 and 2004. Weighted logistic regression models investigated the association between migraines, severe headaches, and hypertension. In total, 5716 subjects were enrolled in the present study, of whom 1134 (19.8%) had migraine or severe headaches. Participants with migraine were predominantly younger females and had a higher body mass index (BMI), lower educational level, lower dietary intake of potassium and calcium, lower serum levels of total cholesterol (TC), creatinine, and hemoglobin, as well as a higher estimated glomerular filtration rate (eGFR) (all P < 0.05). After fully adjusting for potential confounders, migraine or severe headaches were positively associated with hypertension (OR 1.25, 95% CI: 1.03-1.53).Conclusion: Our study found a positive association between migraine, severe headaches, and hy-pertension.
