Abstract:Background: To assess corneal epithelial-thickness (ET) mapping resulting from spectral-domain-optical-coherence-tomography (SD-OCT) by analysing its repeatability and reproducibility and its utility for screening corneal-refractive-surgery (CRS) candidates. Methods: ET was measured in 25-sectors by two-operators. Intra-subject-standard-deviation, coefficient-of-repeatability (CoR) and coefficient-of-variability (CoV) were calculated to evaluate repeatability. Reproducibility was evaluated using a Bland-Altman analysis. Scheimpflug-tomography, refraction, visual acuity, and patient history were used to make a decision on eligibility for CRS. After this decision, the surgeon was shown the patient's ET map and was asked to reconsider his analysis. The percentage of screenings that changed after evaluating the ET maps was determined. Results: Forty-three eyes with normal corneas (CRS-group) and 21 eyes not suitable for CRS (non-CRS-group) were studied. For the CRS-group, CoR ranged from 2.03 (central) to 19.73 μm (outer-inferonasal), with the central-sector showing the highest repeatability (CoV: 1.53-1.80%). For the non-CRS-group, CoR ranged from 3.82 (central-middle-superonasal) to 13.42 μm (middle-inferotemporal), with the inner-superonasal-sector showing the highest repeatability (CoV: 2.86-4.46%). There was no statistically significant difference between operators (p > 0.01). In the CRS-group, the outcomes showed a narrow 95% limits-of-agreement (LoA) for the central-and inner-nasal-sectors (about 4 μm), and wider for the inner-superior, outer-superotemporal and outer-inferonasal (about 10-14 μm). In the non-CRS-group, they were for the outer superonasal (about 4 μm), and for the middle-inferotemporal and outer-temporal (about 10 μm), respectively. Candidacy for CRS changed in 7.82% of patients after evaluation of the ET maps, with all of them screened-out. Conclusion: The SD-OCT provided repeatable and reproducible corneal ET measurements and may alter candidacy for CRS. Abstract:The corneal epithelium (CE) is the outermost layer of the cornea with constant turnover, relative stability, remarkable plasticity, and compensatory properties to mask alterations in the underlying stroma. The advent of quantitative imaging modalities capable of producing epithelial thickness mapping (ETM) has made it possible to characterize better the different patterns of epithelial remodeling. In this comprehensive synthesis, we reviewed all available data on ETM with different methods, including very high-frequency ultrasound (VHF-US) and spectral-domain optical coherence tomography (SD-OCT) in normal individuals, corneal or systemic diseases, and corneal surgical scenarios. We excluded OCT studies that manually measured the corneal epithelial thickness (CET) (e.g., by digital calipers) or the CE (e.g., by confocal scanning or handheld pachymeters). A comparison of different CET measuring technologies and devices capable of producing thickness maps is provided. Normative data on CET and the possible effects of gender, aging, diurnal changes, refraction, and intraocular pressure are discussed. We also reviewed ETM data in several corneal disorders, including keratoconus, corneal dystrophies, recurrent epithelial erosion, herpes keratitis, keratoplasty, bullous keratopathy, carcinoma in situ, pterygium, and limbal stem cell deficiency. The available data on the potential role of ETM in indicating refractive surgeries, planning the procedure, and assessing postoperative changes are reviewed. Alterations in ETM in systemic and ocular conditions such as eyelid abnormalities and dry eye disease and the effects of contact lenses, topical medications, and cataract surgery on the ETM profile are discussed.

Abstract:Purpose To compare agreement of corneal epithelium thickness (ET) between AS-OCT system (RTVue, Optovue, Fremont, USA) and AS-OCT/Placido topographer (MS-39, CSO, Florence, Italy) in different stages keratoconus (KC) eyes, and to assess the repeatability of RTVue AS-OCT. Design Prospective reliability analysis. Methods KC eyes were classified into forme fruste KC (FFKC), mild, moderate and severe KC. Agreement was evaluated with Bland-Altman plots and 95% limits of agreement (LoA). The repeatability of RTVue was assessed via within-subject standard deviation (S w ), test-retest variability (TRT), coefficient of variation (CoV), and intraclass correlation coefficient (ICC). Results Totally, 119 KC eyes were enrolled, with 21 FFKC, 26 mild, 39 moderate, and 34 severe. The 95% LoA ranged between -5.9 and 4.8 μm for center epithelium thickness (CET), between -5.7 and 8.2 μm for thinnest epithelium thickness (TET). At 1mm measuring points, the 95% LoA of superior, inferior, nasal and temporal were -4.2 to 4.7 μm, -5.2 to 6.0 μm, -7.9 to 10.2 μm, -11.2 to 6.0 μm. At 3mm measuring points, the corresponding values were -2.8 to 9.3 μm, -2.0 to 13.0 μm, -4.6 to 9.6 μm, -6.3 to 9.7 μm, indicating the two instruments weren't interchangeable without adjustment. Despite the repeatability of RTVue in KC patients were acceptable, repeatability decreased gradually with the peripheralization of the measurement points. Conclusions The two OCT-based devices, RTVue and MS-39, don't provide interchangeable measurements of ET in KC patients. Repeatability decreases in severer KC, emphasizing the importance of grading before clinical examination to avoid diagnostic errors.

Evaluation of Corneal Epithelial Thickness Imaged by High Definition Optical Coherence Tomography in Healthy Eyes

Abstract:Purpose: To evaluate corneal epithelial thickness (CET) and corneal thickness (CT) in healthy eyes using spectral domain optical coherence tomography. Methods: Thirty-six healthy eyes were imaged using the Cirrus high-definition (HD)-optical coherence tomography device. The average CET and CT were assessed using Cirrus Review Software within predefined concentric corneal ring-shaped zones. Specific regions of CET (superior, inferior, temporal, nasal, superonasal, inferotemporal, superotemporal, and inferonasal) were also assessed. The difference between zones was compared between males and females. Results: The average CET was 48.3, 47.1, 46.1, and 45.8 μm in the 4 concentric zones (0-2, 2-5, 5-7, and 7-9 mm), respectively (P < 0.001). The average CT was 533.5, 550.8, and 579.4 μm in the 3 zones (0-2, 2-5, and 5-7 mm), respectively (P < 0.001). There was no statistically significant correlation between CET and CT in any of the measured zones. Males had thicker corneas than did females in each of the 3 CT zones (P < 0.05), but CET did not differ significantly. The CET superonasal-inferotemporal in 2.0 to 5.0 mm and CET superotemporal-inferonasal in 5.0- to 7.0-mm zones were significantly thinner in males than in females (-1.15 vs. 0.9 μm, -3.5 vs. -1.9 μm), respectively (P < 0.05). Conclusions: Optical coherence tomography-based analysis of CET reveals that it is thinner in the periphery, whereas the total corneal thickness is greater. Although total CT seems to be influenced by sex, CET is not. Regional and sex-based variations in CT may need to be considered when assessing corneal and epithelial alterations in the setting of disease.

Abstract:Abstract Background To determine the repeatability of measurements of ocular surface vessel density in normal and diseased eyes using optical coherence tomography angiography (OCTA). Methods Ten normal eyes, 10 pinguecula eyes, and 10 pterygium eyes of 30 volunteers were subjected to OCTA (AngioVue Imaging System, Optovue, Inc.). For scanning, we used the corneal adapter module. Each eye was scanned three times in the nasal and temporal directions, separately. AngioVue software was used to generate the ocular surface vessel density. Ocular surface vessel density was defined as the proportion of vessel area with blood flow to the total measurement area (3 × 3 mm2). Intersession repeatability of the measurement was summarized as the coefficient of variation (CV), and intraclass correlation coefficients (ICC) were calculated by variance component models. Results The CVs were less than 5% in all subjects, and the ICCs exceeded 0.9; thus, all measurements showed good repeatability. The nasal vessels densities differed significantly between healthy eyes and eyes with pterygium (P < 0.05); however, there was no significant difference between healthy eyes and eyes with pinguecula (P = 0.466). Conclusions These results suggest that measurement of ocular surface vessel density by OCTA in normal eyes and eyes with pterygium and pinguecula is repeatable. This preliminary research describes a quantitative and visual method for assessing vessel density of the ocular surface with a high level of consistency.
Comparison of repeatability of swept-source and spectral-domain optical coherence tomography for measuring inner retinal thickness in retinal disease

Abstract:PURPOSE: To compare repeatability between SS-OCT and SD-OCT for measurement of macular, macular retinal nerve fiber (mRNFL), and ganglion cell-inner plexiform layer (GC-IPL) thickness in various retinal diseases.METHODS: One hundred and fourteen eyes of 114 subjects were investigated. Seventy-eight eyes with retinal disease and 36 normal eyes underwent two consecutive measurements of macular, mRNFL, and GC-IPL thickness using SS-OCT and SD-OCT. The data were obtained using the Early Treatment Diabetic Retinopathy Study (ETDRS) protocol. The eyes with retinal diseases were divided into three subgroups according to central macular thickness (CMT) for analysis. The intraclass correlation coefficient (ICC) was calculated to determine the repeatability of OCT device.RESULTS: In normal eyes, both OCT devices showed excellent repeatability of macula, mRNFL, and GC-IPL thickness measurements with high ICCs in all ETDRS subfields. In eyes with retinal disease, although SS-OCT showed better repeatability for inner retinal thickness measurements than SD-OCT, the overall ICCs were lower than those in normal eyes. In subgroup analysis, the ICCs in the low CMT group were lower than those in the normal and high CMT groups, particularly when using SD-OCT.CONCLUSIONS: Both OCT devices had comparable repeatability for retinal thickness measurement in normal eyes and eyes with retinal disease. However, the possibility of measurement error should be considered in eyes with a thin and atrophic retina.

Comparison of Automated and Manual Measurement of Corneal Epithelial Thickness in Eyes With Limbal Stem Cell Deficiency

Abstract:Purpose: The purpose of this study was to compare the outcomes of manual and automated corneal epithelial thickness (CET) assessments using anterior segment optical coherence tomography in eyes with limbal stem cell deficiency (LSCD). Methods: Eighty-seven eyes from 64 patients with LSCD and 65 eyes from 39 healthy subjects were included in this cross-sectional study. OCT images of corneas were acquired using a Fourier-domain anterior segment optical coherence tomography. The automated measurement of CET was obtained using the manufacturer's program and the manual measurements were obtained by 2 masked observers. The average CET was obtained from 3 consecutive epithelial profile maps. The average CET of manual measurement was obtained from 3 consecutive cross-line scans using 3-point measurement as previously described. Results: The central CET was thinner in patients with LSCD compared with healthy individuals by both manual and automated measurements. Automated CET measurements were similar to the manually obtained CET in the healthy controls (r = 0.911). However, there is a lower correlation between the automated and manually obtained CET in the LSCD group (r = 0.497). Manual measurements of CET (39.8 ± 11 μm) in eyes with LSCD were significantly lower than the automated measurements (48.8 ± 7.2 μm, P = 0.001, 95% CI -11.0 to -6.9). The correlations between the automated and manually obtained measurements in the mild, moderate, and severe stages of LSCD were 0.623, 0.632, and 0.378, respectively. Conclusions: There is a large discrepancy between the manual and automated measurements in eyes with LSCD but not in normal eyes. Automated measurements in LSCD exhibited a higher rate of errors and nonuniform measurements due to the presence of subepithelial scars. Automated CET measurements appear to be unreliable in eyes with corneal scars in LSCD.
Vertical and horizontal corneal epithelial thickness profiles determined by ultrahigh resolution optical coherence tomography.

Abstract:Purpose: To measure vertical and horizontal thickness profiles of the central and peripheral corneal epithelium and determine if daytime changes occur. Methods: Forty eyes of 20 normal subjects were imaged by ultrahigh resolution spectral domain optical coherence tomography to profile the corneal epithelial thickness from the edge of Bowman layer to the central cornea across the vertical and horizontal meridians. Measurements were made at 10:00 AM and again at 6 and 8 hours later. Results: The baseline vertical meridional epithelial thickness was thinnest, 42.9 +/- 4.1 mu m, at the edge of Bowman layer in the superior region. It increased in thickness (P < 0.01), toward the central cornea. The central epithelium averaged 52.5 +/- 2.4 mu m, becoming thickest, 55.2 +/- 2.5 mu m, in the inferior pericentral region. It thinned toward the inferior periphery, reaching 51.3 +/- 5.1 mu m at the edge of Bowman layer (P < 0.01). Along the horizontal meridian, the epithelium was thickest at the nasal side, 58.6 +/- 5.1 mu m, and temporal side, 59.3 +/- 6.6 mu m, near the edges of Bowman layer. It thinned toward the central cornea. There were no significant changes in the epithelial thickness at any location over 8 hours. Conclusions: Epithelial thickness varied over the horizontal and vertical meridians and appeared stable during the daytime.

Abstract:Purpose This study aims to evaluate the repeatability of anterior segment optical coherence tomography (AS-OCT) in diverse ocular surface disorder (OSD) cohorts, exploring various anterior segment parameters and their accuracy in different disease groups. Methods A total of 239 participants across six distinct OSD groups and healthy controls underwent nonmydriatic AS-OCT imaging using the Tomey CASIA 2 device. Anterior segment parameters including anterior chamber depth, width, angle metrics, corneal thickness, keratometry, lens vault, and others were meticulously assessed. Statistical analyses determined repeatability limits and coefficients of variation for each parameter within the different OSD cohorts. Results Repeatability for anterior chamber and corneal parameters remained consistent across all OSD groups, indicating minimal impact of ocular surface disease on accuracy. The coefficient of variation (CoV) for the trabecular iris-space area was about 20% for all cohorts. Ocular surface inflammation emerged as a key factor in dry eye, affecting immune-mediated and non-immune conditions alongside age-related ocular surface changes. While anterior chamber depth measurements showed variations, particularly in immune (CoV = 2.5%) and non-immune (CoV = 3.8%) OSD groups, parameters like anterior chamber width and angle to angle showed similar values among the cohorts. Keratometry measures remained stable despite OSD (CoV lower than 1%). Conclusion The Tomey CASIA 2 demonstrated reliable repeatability for measuring anterior segment parameters in diverse OSD cohorts. Despite challenges posed by dry eye conditions, this technology holds promise in assessing OSD, suggesting potential clinical protocols similar to those in healthy controls.

Diagnostic Accuracy of Corneal and Epithelial Thickness Map Parameters to Detect Keratoconus and Suspect Keratoconus

Abstract:Aim: To establish the diagnostic accuracy of corneal and epithelial thickness measurements obtained by spectral-domain optical coherence tomography (SD-OCT) in detecting keratoconus (KC) and suspect keratoconus (SKC). Methods: This retrospective study reviewed the data of 144 eyes separated into three groups by the Sirius automated corneal classification software: normal (N) (n = 65), SKC (n = 43), and KC (n = 36). Corneal thickness (CT) and epithelial thickness (ET) in the central (0-2 mm) and paracentral (2-5 mm) zones were obtained with the Cirrus high-definition OCT. Areas under the curve (AUC) of receiver operator characteristic (ROC) curves were compared across groups to estimate their discrimination capacity. Results: ROC curve analysis revealed excellent predictive ability for ET variables: minimum (Min) ET (0_2), minimum-maximum (Min-Max) ET (0_2), superonasal-inferotemporal (SN-IT) ET (2_5), Min-Max ET (2_5), and Min ET (2_5) to detect keratoconus (AUC > 0.9, all). Min-Max CT (0_2) was the only CT parameter with excellent ability to discriminate between KC and N eyes (AUC = 0.94; cutoff = ≤-32 μm). However, both ET and CT variables were not strong enough (AUC < 0.8, all) to differentiate between SKC and N eyes, with the highest diagnostic power for Min-Max ET (2_5) (AUC = 0.71; cutoff = ≤-9 μm) and central corneal thickness (CCT) (AUC = 0.76; cutoff = ≤533 μm). Conclusion: These results demonstrate that OCT-derived CT and ET are able to differentiate between KC and N eyes, with a high level of certainty. However, Min-Max ET (2_5) was the parameter with the highest ability to detect suspect keratoconus.

Age-Related Changes in Human Corneal Epithelial Thickness Measured with Anterior Segment Optical Coherence Tomography

Abstract:PURPOSE. To measure corneal and limbal epithelial thickness (ET) in normal subjects and to evaluate its variation with age by using anterior segment optical coherence tomography (AS-OCT).METHODS. A total of 180 normal subjects (180 healthy eyes) were enrolled and divided into four groups according to age: A (0-20 years), B (21-40 years), C (41-60 years), and D (>60 years). Cornea and limbus were imaged with OCT. Corneal ET (CET) was obtained automatically by the built-in analysis software of the OCT system. Limbal ET (LET) in four quadrants was manually measured from OCT images.RESULTS. Corneal ET of a central 2-mm diameter zone in groups A, B, C, and D were 53.4 +/- 2.8 mu m, 53.4 +/- 2.7 mu m, 53.2 +/- 3.0 mu m, and 52.9 +/- 3.3 mu m, respectively, N showing no significant change with aging. In the paracentral zone extending to 6-mm diameter, correlation analysis suggested that CET was inversely associated with age (P < 0.05). Limbal ET in the nasal and the temporal quadrants were similar and decreased with aging, the averages were 58.3 +/- 8.1 mu m, 54.1 +/- 6.1 mu m, 51.2 +/- 6.1 mu m, 51.6 +/- 5.2 mu m for groups A, B, C and D, respectively; while age seemed to have no effect on LET of the superior and the inferior quadrant.CONCLUSIONS. The paracentral corneal epithelium, as well as the nasal and temporal limbal epithelium, became thinner with aging, while the central CET seemed to remain constant. Measurement with AS-OCT of the corneal and limbal ET could aid in clinical assessment and planning treatments of the cornea.

Abstract:SIGNIFICANCE: Corneal epithelial thickness mapping has a wide range of applications including screening for keratoconus, screening for anterior basement membrane dystrophy, and assessment of dry eye. It also plays an important role in corneal laser refractive surgery. These all require an understanding of the epithelial thickness profile in a normal cornea as a reference.PURPOSE: This study aimed to compare corneal epithelial thickness along different meridians in normal eyes with different types of corneal astigmatism.METHODS: In this cross-sectional study, corneal topography and epithelial thickness mapping by anterior segment optical coherence tomography was performed for 154 normal eyes. Corneal astigmatism was classified based on the orientation of the steepest corneal meridian (with-the-rule [WTR], against-the-rule [ATR], and oblique) and the amount (low, moderate, and high). On a 9-mm epithelial thickness map, the thickness along the horizontal (180°), vertical (90°), and diagonal (45 and 135°) meridians was calculated.RESULT: With-the-rule, oblique, and ATR astigmatisms were observed in 40, 31, and 29% of the eyes, respectively. In all types of astigmatism, the mean epithelial thickness was greater along the 180° meridian and thinner along the 90° meridian. There was no statistically significant difference in mean thickness along different meridians for WTR (P = .24) and oblique (P = .46) astigmatism, whereas the difference along the 180 and 90° meridians (P = .003) for ATR astigmatism was statistically significant. Also, there was a statistically significant difference in thickness in different types of astigmatism separately for each meridian (P < .05) so that the highest and lowest thicknesses were seen for ATR and oblique astigmatisms, respectively. The epithelial thickness showed no statistical difference in various amounts of astigmatism along different meridians.CONCLUSIONS: Thicker epithelium along the horizontal meridian in ATR may point to the surface regularizing functions of the epithelium to create a regular and smooth surface, but thickness assessment along the different meridians in oblique and WTR showed no curvature-dependent changes.

Abstract:1 The capacity of brown adipose tissue to respond calorigenically to catecholamines increases markedly during cold-acclimation of adult rats. To investigate this phenomenon, we have used the potent radioactive ligand (-)-[3H]dihydroalprenolol to directly estimate the number, the density and the affinity of β-adrenergic receptors in brown adipose tissue membranes from control, cold-exposed and cold-acclimated rats. 2 Binding of (−)-[3H]dihydroalprenolol to unfractionated membranes was rapid, stable, saturable and reversible. It displayed the affinity, specificity and stereoselectivity expected of binding to adenylate cyclase-coupled β-adrenergic receptors. β-Adrenergic agonists competed for binding sites with an order of potency typical of the β1 subtype of adrenergic receptors: (−)-Isoproterenol > (−)-norepinephrine ≥ (−)-epinephrine. Binding exhibited a remarkable stereoselectivity, the (−)-isomers of β-adrenergic agonists and antagonists being 34 to 280 times more potent than the (+)-isomers in competing for (−)-[3H]dihydroalprenolol binding sites. 3 Total interscapular brown adipose tissue of the adult, warm-acclimated rat contained 1.12 ± 0.08 pmol of (−)-[3H]dihydroalprenolol binding sites. During cold-acclimation, growth of the tissue was accompanied by a 4–5-fold increase in the total number of recptor sites. However, this increase did not keep pace with the increse in brown adipose tissue cellularity (as estimated by total tissue DNA content), resulting in a 40–50% reduction in receptor density. The decrease in receptor density was associated with cold-exposure rather than with cold-acclimation. 4 The affinity of (−)-[3H]dihydroalprenolol receptor sites was not significantly altered by cold-acclimation. 5 The results of this study are interpreted as indicating that catecholamines released via activation of the sympathetic nervous system regulate both the density and the number of their own receptors in brown adipose tissue of cold-exposed animals. The development of the hyperadrenergic response of this thermogenic tissue during cold-acclimation may result from a marked organ hyperplasia associated with an increased number of β-adrenergic receptor sites and cannot be explained by alterations in receptor density or affinity.
Variability of Corneal Deformation Response in Normal and Keratoconic Eyes

Abstract:PurposeTo compare the corneal deformation response, central corneal thickness (CCT), and intraocular pressure (IOP) measurements and their test-retest variability obtained with an ultrahigh-speed Scheimpflug camera between normal and keratoconus eyes. MethodsThree consecutive measurements were obtained using Corvis ST. The following parameters were analyzed: A1 and A2 length (length of flattened cornea at first and second applanation), A1 and A2 velocity (deformation velocity until first and second applanation), corneal deformation amplitude (deformation amplitude of cornea at the highest concavity), peak distance (distance of two apices of cornea at time of highest concavity), and radius of corneal curvature at the time of maximum deformation. Repeatability coefficient and intraclass correlation coefficient were measured. Linear mix models were used to adjust for the effect of age, CCT, and IOP on corneal deformation response parameters. ResultsTwelve normal subjects and 12 keratoconus patients were included. Data from only one eye of each participant were randomly selected for analysis. Significant differences were found in corneal deformation amplitude (p < 0.001) and radius of corneal curvature (p < 0.001) between normal and keratoconus eyes after adjusting for age, CCT, and IOP. Although there was no significant difference of intraclass correlation coefficient between the groups, repeatability coefficient values of A1 and A2 length, A1 velocity, and peak distance were significantly smaller in normal eyes as compared with keratoconus eyes (p ⩽ 0.023). ConclusionsCorvis ST showed adequate repeatability for measurement of corneal deformation amplitude, CCT, and IOP in normal and keratoconus eyes. It may be used to understand ocular pathologies associated with altered biomechanical properties.

Repeatability and Reproducibility of Superficial Macular Retinal Vessel Density Measurements Using Optical Coherence Tomography Angiography En Face Images

Abstract:Importance: The repeatability and reproducibility of quantitative metrics from optical coherence tomographic angiography (OCTA) must be assessed before these data can be confidently interpreted in clinical research and practice. Objective: To evaluate the repeatability and reproducibility of OCTA-derived retinal vascular quantitative metrics. Design, setting and participants: In this cross-sectional study, 21 healthy volunteers (42 eyes) and 22 patients with retinal disease (22 eyes), including 14 with age-related macular degeneration, 3 with epiretinal membrane, 2 with diabetic retinopathy, 2 with myopic macular degeneration, and 1 with retinal vein occlusion, were enrolled. Participants were recruited from September 1 through November 31, 2016. Each eye underwent 3 repeated scans with 3 instruments for a total of 9 acquisitions. Eyes were randomly assigned to scanning with a 3 × 3-mm or 6 × 6-mm pattern. Eyes were excluded from subsequent analysis if any acquisition had a signal strength of less than 7. Repeatability (defined as the agreement in measurements within a device) and reproducibility (defined as the agreement between devices of the same type) were assessed by intraclass correlation coefficient (ICC) and coefficient of variation. Exposures: All eyes underwent scanning using 3 separate devices. Main outcomes and measures: Vessel length density (VLD) and perfusion density (PD) of the superficial retinal vasculature. Results: A total of 21 healthy volunteers (8 men and 13 women; mean [SD] age, 36 [6] years) and 22 patients with retinal disease (15 men and 7 women; mean [SD] age, 79 [9] years) underwent evaluation. Of these, 40 of 42 normal eyes and 15 of 22 eyes with retinal disease met signal strength criteria and were included in this analysis. The ICC among the 3 consecutive scans ranged from 0.82 to 0.98 for VLD and from 0.83 to 0.95 for PD. The coefficient of variation (CV) ranged from 2.2% to 5.9% for VLD and from 2.4% to 5.9% for PD. For reproducibility, the ICC ranged from 0.62 to 0.95 and the CV was less than 6% in all groups. The agreement was highest for the 3 × 3-mm pattern in the inner ring (ICC range, 0.92 [95% CI, 0.85-0.96] to 0.96 [95% CI, 0.93-0.98]) and 6 × 6-mm pattern in the outer ring (ICC range, 0.93 [95% CI, 0.86-0.97] to 0.96 [95% CI, 0.92-0.98]). Conclusions and relevance: Vessel length density and PD of the superficial retinal vasculature can be obtained from OCTA images with high levels of repeatability and reproducibility but can vary with scan pattern and location.
Exploration of a new method in measurement of choroidal thickness in children and teenagers by EDI-OCT

Abstract:Abstract Purpose: Using EDI-OCT and combining it with a self-built program to achieve choroidal thickness measurement in a simple and easy way. Measurement and analysis of choroidal thickness in normal children and adolescents. Methods: Twenty healthy volunteers (40 eyes) were randomly recruited. The choroidal images of healthy volunteers were obtained by EDI-OCT mode. The anterior choroidal boundary (Bruch membrane) was automatically generated by Heidelberg software. At the same time, the posterior choroidal boundary was manually depicted with the drawing tool in OCT software. A program was written to calculate choroidal thickness to verify the repeatability of the method. OCT images were collected from 99 children and adolescents aged 7-18 years. These choroidal was measured by the method previously verified. Results: 1. ICC was 0.975, 95% CI was [0.959, 0.984], p≤0.001. 2. The average thickness of the choroid in the horizontal direction is 249.17±68.84μm in the 3mm range, and it was 258.24±69.63μm in the vertical direction. The correlation coefficients of choroidal thickness and axial length in horizontal and vertical directions were -0.642, -0.624 (p≤0.001, p≤0.001). The correlation coefficients between choroid thickness and diopter in horizontal and vertical directions were 0.610, 0.583 (p≤0.001, p≤0.001). Conclusion: 1. The experimental method has good repeatability and can calculate the average thickness of the choroid through pixels. 2. There was a weakly significant correlation between choroidal thickness and age, but significant with axial length and diopter. 3. It is simple to operate, requires low computer knowledge, and can be accepted more easily by clinicians and researchers. And it can be widely used in the field of EDI-OCT.
Structural Imaging of Benign Conjunctival Proliferative Diseases Using Optical Coherence Tomography

Abstract:Objective: To use anterior segment optical coherence tomography (AS-OCT) for structural imaging and accurate measurements of pinguecula and pterygium in order to understand the structural features of these lesions. Methods: This study was a prospective parallel control study. Lateral bulbar conjunctiva of 25 cases (30 eyes) that included pinguecula (10 eyes of 8 cases), pterygium (10 eyes of 9 cases), and normal eyes (10 eyes of 8 cases), were enrolled from the Department of Ophthalmology of the Fourth Affiliated Hospital of Zhejiang Medical University from January to April 2018. The anterior segment module of an Optovue optical coherence tomography angiography (OCTA) was used for measurements. The thickness of the bulbar conjunctiva epithelium and propria tissues at sites at a distance of 0.0 mm, 0.5 mm, 1.0 mm, 1.5 mm, 2.0 mm and 2.5 mm from the scleral process were stratified within the 2.5 mm range, were measured, compared and further analyzed. The data were described by means of mean±standard deviation. Repeated measurement analysis of variance was used for comparison of each point in the group, one-way analysis of variance was used for comparison between groups. Results: Compared with normal eyes and pinguecula eyes, pterygium showed thickened epithelium in 0.5 mm point (P<0.05). In the 2.5 mm range on the nasal side, pterygium and pinguecula showed thickened subepithelium tissues (P<0.05), especially for pterygium, compared to normal eye tissue. Conclusions: AS-OCT can image microstructures in benign conjunctival proliferative diseases, resulting in accurate quantitative diagnosis and evaluation. The thickness of subepithelium tissues increases in pinguecula and pterygium.
Assessment of Corneal Epithelial Thickness in Dry Eye Patients.

Abstract:PURPOSE:To investigate the features of corneal epithelial thickness topography with Fourier-domain optical coherence tomography (OCT) in dry eye patients.METHODS:In this cross-sectional study, 100 symptomatic dry eye patients and 35 normal subjects were enrolled. All participants answered the ocular surface disease index questionnaire and were subjected to OCT, corneal fluorescein staining, tear breakup time, Schirmer 1 test without anesthetic (S1t), and meibomian morphology. Several epithelium statistics for each eye, including central, superior, inferior, minimum, maximum, minimum - maximum, and map standard deviation, were averaged. Correlations of epithelial thickness with the symptoms of dry eye were calculated.RESULTS:The mean (±SD) central, superior, and inferior corneal epithelial thickness was 53.57 (±3.31) μm, 52.00 (±3.39) μm, and 53.03 (±3.67) μm in normal eyes and 52.71 (±2.83) μm, 50.58 (±3.44) μm, and 52.53 (±3.36) μm in dry eyes, respectively. The superior corneal epithelium was thinner in dry eye patients compared with normal subjects (p = 0.037), whereas central and inferior epithelium were not statistically different. In the dry eye group, patients with higher severity grades had thinner superior (p = 0.017) and minimum (p < 0.001) epithelial thickness, more wide range (p = 0.032), and greater deviation (p = 0.003). The average central epithelial thickness had no correlation with tear breakup time, S1t, or the severity of meibomian glands, whereas average superior epithelial thickness positively correlated with S1t (r = 0.238, p = 0.017).CONCLUSIONS:Fourier-domain OCT demonstrated that the thickness map of the dry eye corneal epithelium was thinner than normal eyes in the superior region. In more severe dry eye disease patients, the superior and minimum epithelium was much thinner, with a greater range of map standard deviation.
