Abstract:Aim: To determine the circulatory procalcitonin levels, lipid profile and uric acid levels in patients presenting with preeclampsia and to correlate these with clinical severity of the disease. Methodology: This double blinded case-control study was conducted for two years in department of Obstetrics and Gynecology in collaboration with the department of biochemistry, Government Institute of Medical Sciences, Greater Noida, UP. This study included 80 subjects out of 40 were in group I (Control) and other 40 were in group II (PE patients) admitted in the hospital in third trimester of pregnancy. General demographic and clinical data such as age, gestational week, number of gestation, and BMI were recorded, and the results of routine blood cell counts, PCT and other indicators were also obtained. The PE patients were women younger than 40 years old who met the diagnostic criteria for PE in the guidelines for diagnosis and treatment of hypertensive diseases in pregnancy-2014. The healthy pregnant women were between 24 and 40 years old, with a single fetus, normal fetal development, normal liver and kidney function, and without eclampsia, gestational diabetes mellitus, fetal growth restriction, or history of other adverse pregnancy conditions.
Association of Maternal Serum CRP, IL-6, TNF-α, Homocysteine, Folic Acid and Vitamin B12 Levels with the Severity of Preeclampsia and Fetal Birth Weight

Abstract:OBJECTIVE: To assess the levels and clinical significance of high sensitive(hs)-CRP (C-reactive protein), IL-6(interleukin-6), TNF-alpha(tumor necrosis factor-alpha), homocysteine, folic acid and vitamin B12 in normotensive healthy pregnant women, mild and severe preeclamptic patients, and to evaluate the correlations between these markers and the severity of preeclampsia and fetal birth weight.STUDY DESIGN: Using a cross-sectional study design, hs-CRP, IL-6, TNF-alpha, homocysteine and vitamin B12 were measured in the third trimester of pregnancy from normotensive healthy women with uncomplicated pregnancies (n = 62), mild (n = 61) and severe (n = 60) preeclamptic patients.RESULTS: There were statistically significant differences between three groups for hs-CRP (p = 0.012), TNF- alpha (p = 0.046), IL-6 (p = 0.015), homocysteine (p < 0.001) and fetal birth weight (p < 0.001). Fetal birth weights in mild (2477 +/- 746) and severe (2435 +/- 768) preeclamptic patients were significantly lower than controls (3485 +/- 365) (p < 0.001). No significant difference was found between the three groups for folic acid (p = 0.066) and vitamin B12 (p = 0.286). Bonferroni adjusted multiple comparison test showed that the statistical differences with respect to TNF-alpha, IL-6 and homocysteine were mainly created by control and severe preeclampsia groups. Hs-CRP levels still remained higher in severe preeclampsia patients than mild preeclampsia and normotensive patients except for overweight patients in the previous two groups after Bonferroni post hoc adjustment test.CONCLUSION: Elevated maternal serum levels of hs-CRP, TNF- alpha, IL-6 and homocysteine in preeclamptic women correlate with fetal birth weight in the early third trimester.

Abstract:Objectives: Interleukin-18 (IL-18) is a proinflammatory cytokine with pleiotrophic qualities and its roles in pregnancy, labor onset and pregnant complications have been proposed. However, the alterations of serum and placental IL-18 have been less investigated. The objective of the current investigation was to detect IL-18 concentrations in serum and placentas from patients with preeclampsia and women with normal pregnancy.Methods: 27 patients with preeclampsia and 28 women with normal pregnancy were recruited. Blood and placental samples were taken and IL-18 concentrations in serum and placental homogenate were analyzed by enzyme-linked immunosorbent assay (ELISA). The levels of IL-18 in serum and placenta were compared between preeclampsia and control.Results: Both serum and placental levels of IL-18 were significantly increased in preeclampsia as compared with control (P = 0.014 and 0.003, respectively). Serum and placental levels of IL-18 were not significantly different in preeclamptic women who received dexomethasone and those who did not (P = 0.223 and 0.330, respectively). Serum and placental IL-18 levels were not significantly different between preeclamptic women who delivered before 36 complete gestational weeks and those who delivered after 36 complete weeks (P = 0.616 and 0.869, respectively). There were no significant differences in serum and placental IL- 18 between women with mild preeclampsia and those with severe preeclampsia (P = 0.056 and 0.357, respectively).Conclusions: Increased serum and placental levels of IL-18 were observed in preeclampsia. [Relationship Between Serum and Placental Interleukin-18 Levels and Preeclampsia].

Abstract:OBJECTIVE:To investigate the relationship of interleukin (IL)-18 concentrations in serum and placenta and preeclampsia.METHODS:Twenty-seven patients with preeclampsia and 28 women with normal pregnancy were recruited. Blood and placenta samples were taken and serum and placental IL-18 concentrations were determined by enzyme-linked immunosorbent assay (ELISA). The levels of IL-18 in serum and placenta were compared.RESULT:(1) The median of serum IL-18 in patients with preeclampsia was 704 ng/L, which was significantly higher than that of control group (470 ng/L, P<0.05). (2) The median of placental IL-18 in patients with preeclampsia was 105 ng/L, which was also significantly higher than that of control group (31.5 ng/L, P<0.005).CONCLUSION:Increased serum and placental levels of IL-18 were presented in preeclampsia as compared to normal pregnancy, which might offer insight into the pathogenesis of the disease.

Abstract:A generalized inflammatory response has been considered to be the main pathology and has an important role in the pathogenesis of preeclampsia. The immune aberrations per se and immunomodulatory milieu present in serum need to be elucidated. The purpose of the current investigation was to characterize changes in serum levels of interleukin (IL)-15 and IL-16 in preeclampsia. Thirty-seven women with preeclampsia were recruited and 36 age- and gestational age-matched women with normal pregnancy served as control. Levels of IL-15 and IL-16 were detected with immune assays in all serum samples. We found that serum levels of IL-15 and IL-16 were significantly higher in preeclampsia than in normal pregnancy (p<0.001 for both). There were significant differences in serum IL-15 and IL-16 between mild and severe preeclampsia (p<0.01 for both). Our data corroborate the hypothesis of an increased inflammatory response in preeclampsia, as illustrated by the elevated serum levels of IL-15 and IL-16, suggesting their possible role in the pathogenesis of preeclampsia. These associations may offer insight into the pathophysiology of preeclampsia.
Relationship Between Serum Resistin Level and Preeclampsia

Abstract:OBJECTIVE:To investigate the alteration of serum resistin levels in normal pregnancy and preeclampsia. METHODS:Blood samples were take from 28 normal non-pregnant women, 27 women in the 1st, 26 in the 2nd and 26 in the 3rd trimester of normal pregnancy and 25 women with preeclampsia. Serum resistin concentration was determined using ELISA method. RESULT:Serum levels of resistin were not significantly different among non-pregnancy, the 1st and the 2nd trimester of pregnancy (P>0.05 for all). Serum resistin level was significantly elevated in the 3rd trimester of pregnancy as compared with non-pregnancy (P<0.01), the 1st (P<0.001) and the 2nd trimester of pregnancy (P<0.001). Serum resistin level was significantly lower in preeclampsia than in the 3rd trimester of normal pregnancy (P<0.001), but was comparable to that of non-pregnancy, the 1st and the 2nd trimester of pregnancy (P>0.05 for all). CONCLUSION:The decrease of serum resistin level in the preeclampsia may provide insight into the pathogenesis of the disease.
Changes of serum cardiotropin I and cystatin C in preeclampsia and significance thereof

Comparison was made between the 2 groups. Results: The serum cTn I of the preclampsia group was 0.59 μg/L ± 0.42 μg/L, significantly higher than that of the control group (0.09 μg/L ± 0.22 μg/L P < 0.001), and the serum cystatin C of the preclampsia group was 1.64 mg/L ± 0.47 mg/L, significantly higher than that of the control group (1.08 mg/L ± 0.19 mg/L, P < 0.001). And the serum cardiotropin I and cystatin C concentrations of the patients with severe preeclampsia were 0.66 μg /L ± 0.40 μg/L and 1.72 mg/L ± 0.46 mg/L respectively, both significantly higher than those of the patients with mild preclampsia (0.17 μg/L ± 0.26 μg/L and 1.21 mg/L ± 0.37 mg/L respectively, P < 0.001 and 0.05 respectively). Conclusion: Serum cTn I and cystatin C levels are elevated in preeclampsia and the elevation is associated with the severity of preeclampsia, suggesting that serum concentrations of cTn I and cystatin C are useful markers in the early diagnosis of cardiac and renal injury in patients of preeclampsia.

Abstract:Background: Preeclampsia, a pregnancy-specific complication occurring in the second half of human pregnancy and one of the leading causes for perinatal mortality and morbidity, is characterized by the onset of hypertension and proteinuria. We measured circulating adipocyte fatty acid binding protein (AFABP) and retinol binding protein-4 (RBP-4) in patients with preeclampsia.Methods: Twenty-seven healthy non-pregnant women, 27 healthy pregnant women at third trimester and 26 women with preeclampsia were recruited and blood samples were taken. Concentrations of serum AFABP and RBP-4 were measured with ELISA.Results: There were significant differences in serum AFABP (median: 0.99, 0.93 and 1.81 ng/ml, respectively, P < 0.001) and RBP-4 (mean: 3.16, 2.65 and 4.27 ng/ml, respectively, P = 0.022) among non-pregnancy, normal pregnancy and preeclampsia. Serum AFABP was significantly higher in preeclampsia than non-pregnancy (P < 0.001) and normal pregnancy (P = 0.001). Serum RBP-4 was significantly higher in preeclampsia than normal pregnancy (P = 0.007) but not non-pregnancy (P = 0.061). There were no significant differences in serum RBP-4 and AFABP between non-pregnancy and normal pregnancy. Serum RBP-4 was significantly higher in severe than mild preeclampsia (mean: 5.15 vs 2.84 ng/ml, P = 0.046). There was no significant difference in serum AFABP between mild and severe preeclampsia.Conclusion: Increased circulating AFABP and RBP-4 concentrations were demonstrated, suggesting it be an important pathophysiology of preeclampsia. Abstract:Objective: To determine whether serum Follistatin-like-3 was enhanced in second trimester of pregnant women who subsequently developed preeclampsia. Methods: Thirty-nine pregnant women subsequently developed preeclampsia and 73 normal pregnancy women were included. Serum samples taken in 24-28 weeks gestation were used for assay of Follistatin-like-3. Results: Follistatin-like-3 was significantly elevated in preeclampsia group compared with control. However, there was no significant difference of Follistatin-like-3 in women with mild or severe preeclampsia. Conclusion: The elevation of Follistatin-like-3 was demonstrated in late second trimester of pregnancy destined to developing preeclampsia, suggesting its causal role in the pathogenesis and prediction of preeclampsia.
Study of C-reactive Protein Levels in Hypertensive Disorders of Pregnancy

Abstract:Introduction Globally, one of the main causes of maternal and perinatal deaths is hypertensive disorders of pregnancy (HDP). Preeclampsia is a pregnancy-specific syndrome characterized by high blood pressure and proteinuria after 20-week gestation. Women who develop preeclampsia are at increased risk for the development of many systemic complications. Materials and methods A cross-sectional study was conducted in the Department of Biochemistry, SHKM, Nuh, Haryana in collaboration with the Department of Obstetrics & Gynecology. 200 study subjects; Group 1: cases - 100 HDP, Group 2: controls - 100 age-matched normotensive pregnant women. The specimen of 3 mL venous blood was collected under aseptic precautions. The data was evaluated using statistical evaluation tools. Result Out of 200 subjects, mean C-reactive protein (CRP) levels were normal in three cases and 35 controls. Minor elevation of CRP was observed in 11 cases and 29 controls (very significant). Moderate elevation of CRP was observed in 61 cases and 36 controls (highly significant). Marked elevation of CRP was observed in 25 cases. Mean CRP levels increased from preeclampsia without severe features (3.9 mg/dL), preeclampsia with severe features (5.2 mg/dL), and eclampsia (12.7 mg/dL) when compared with the respective control group (1.1 mg/dL, 1.16 mg/dL, and 1.2 mg/dL). Conclusion There is a highly significant association between CRP levels and HDP. Additionally, one useful indicator of the severity of preeclampsia is CRP, a measure of systemic inflammation. Elevated CRP levels in mild PE can be used as an indicator of the progress of the disease and early prevention can be done.
Alteration of Serum Endocan in Normal Pregnancy and Preeclampsia

Abstract:Endocan is a novel marker for inflammation but its significance in preeclampsia remains unknown. Objective: The aims of this study were to delineate the changes of maternal endocan in normal pregnancy and preeclampsia and to explore its possible role in the pathogenesis of preeclampsia. Materials and Methods: Blood samples were taken from 42 non-pregnant women, from 43 normal pregnant women at third trimester, and from 41 preeclamptic women. Serum endocan levels were determined with enzyme-linked immunosorbent assay (ELISA) and compared among the groups. Results: There were no significant differences in serum endocan among non-pregnant women, normal pregnant women, and women with preeclampsia (chi(2)=2.207, p = 0.137) and there was no significant difference in serum endocan between women with mild and severe preeclampsia (Z = 0.368, p = 0. 713). The authors concluded that serum endocan did not change in pregnancy and preeclampsia, indicating endocan may not be involved in the pathogenesis of preeclampsia.
The evaluation of serum homocysteine, folic acid, and vitamin B12 in patients complicated with preeclampsia

Abstract:INTRODUCTION: Increased plasma homocysteine may be associated with adverse pregnancy outcomes, such as preeclampsia. The aim of this study was to determine the plasma homocysteine, serum folate, and vitamin B12 levels in preeclamptic pregnant women.METHODS: This case-control study was conducted in 2016 in Ahwaz on 51 pregnant women with preeclampsia and 51 healthy pregnant women of the same gestational age, who served as controls. The case group also was subdivided into severe and non-severe preeclampsia. Patients' data were collected through a questionnaire and medical records. Serum homocysteine, folic acid, and vitamin B12 were analyzed using chemiluminescent assay. The results were compared between two groups. Statistical analyses were done using IBM-SPSS 20.0. A Kolmogorov-Smirnov test, independent samples t-test, Mann-Whitney test, and Chi-square test were used for data analysis.RESULTS: No different demographic characteristics were found among the groups. Pregnant women complicated with preeclampsia displayed significantly higher serum homocysteine levels (p < 0.001) and lower serum folate (p = 0.005) and vitamin B12 levels (p < 0.001) compared to controls. A statistically significant inverse correlation was evident between serum homocysteine and serum folate levels in preeclamptic patients (p = 0.005; r = -0.389). In addition, an inverse correlation was identified between homocysteine and serum vitamin B12, but it was not statistically significant (p = 0.160; r = -0.200). Significant differences occurred in serum homocysteine and folate levels between the severe and non-severe subgroups (p < 0.001, p < 0.001).CONCLUSION: Women complicated with preeclampsia displayed higher maternal serum homocysteine and lower serum folate and vitamin B12. Further studies are needed to confirm if the prescription of folic acid and vitamin B12 in women with a deficiency of these vitamins could decrease the level of serum homocysteine and, therefore, reduce the risk of preeclampsia or, if it occurred, its severity.

Serum Levels of Angiogenic Factors Distinguish Between Women with Preeclampsia and Normotensive Pregnant Women But Not Severity of Preeclampsia in an Obstetric Center in Turkey

Abstract:BACKGROUND This study aimed to compare serum levels of vascular endothelial growth factor (VEGF) and the VEGF receptors, VEGFR-1 and VEGFR-2, free placental growth factor (fPGF), endostatin, and serum pregnancy-associated plasma protein-A (PAPP-A) levels in women with mild and severe preeclampsia and healthy pregnant women. MATERIAL AND METHODS A included patients diagnosed with mild preeclampsia (n=32), severe preeclampsia (n=32), and healthy pregnant women (n=24). Serum levels of VEGF-A, VEGFR-1, VEGFR-2, fPGF, endostatin, and PAPP-A levels were measured by enzyme-linked immunosorbent assay (ELISA). RESULTS In women with mild and severe preeclampsia, the gestation age at birth and birth weight were found to be significantly lower than the control group (p<0.001). Serum levels of endostatin, VEGFR-1, and VEGF-A levels were significantly increased in pregnant women with preeclampsia compared with healthy pregnant women (p<0.001). Serum levels of PAPP-A, VEGFR-2, and fPGF were significantly higher in healthy pregnant women when compared with women with preeclampsia (p=0.024, p<0.001, and p<0.001, respectively), but there were no significant differences between women with mild and severe preeclampsia. CONCLUSIONS Reduced serum levels of the angiogenic factors PAPP-A, VEGFR-2, and fPGF distinguished between women with preeclampsia and normotensive pregnant women but did not significantly distinguish between mild and severe preeclampsia.

Abstract:Relationship between serum high sensitive C-reactive protein (hs-CRP) and pregnancy-induced hypertension syndrome (PIH) was investigated. One hundred and twenty patients with PIH treated in the First People's Hospital of Chengdu (60 cases of mild preeclampsia, 60 cases of severe preeclampsia) were enrolled in the study. The control group included 60 women with normal singleton pregnancy. Serum hs-CRP and microalbuminuria (mALB) levels were measured by an AU5800 automatic biochemical analyzer (Beckerman Coulter), and the risk factors were analyzed by ROC curve analysis. Patients with PIH had higher levels of serum hs-CRP and mALB than the control group (P<0.01). Serum hs-CRP and mALB levels in the severe preeclampsia group were significantly higher than those in the mild preeclampsia group (P<0.05). ROC curve analysis showed that hs-CRP was a factor of high-risk. Area under the curve was 0.943, and the 95% confidence interval was 0.848-0.974. Detection of serum hs-CRP in patients with PIH can provide references for the prediction of the severity of the disease, and higher level of hs-CRP indicates worse condition.
Serum human pregnancy specific glycoprotein 1 in pregnant women with preeclampsia in comparison with normal pregnancy

Abstract:To compare the serum pregnancy specific glycoprotein 1 values of women with preeclampsia with the values of healthy pregnant women. A total of 90 pregnant women, whom were recruited during their antenatal care visits to the obstetrical department in Hospital, were included. They were divided into two groups: 45 women with diagnosis of preeclampsia and 45 controls who were the women with healthy pregnancy. Blood samples were collected from both groups and the measurement of pregnancy specific glycoprotein 1 was done using an enzyme immunometric assay. The groups of preeclampsia pregnant women had markedly lower pregnancy specific glycoprotein 1 than the women with the uneventful pregnancies (9.8±3.8 vs 14.3±6.0 ng/ml; p value <0.001). Pregnancy specific glycoprotein 1 can significantly predict preeclampsia (P value <0.001) with an odds ratio of 0.839 and 95% confidence interval of 0.763 – 0.924. By application of receiver operating characteristic curve analysis, it was observed that the cut-off value of pregnancy specific glycoprotien1 for predicting preeclampsia was 10.4 ng/ml with 77% sensitivity and 60% specificity, the area under the curve 0.728 with 95% confidence interval between 0.622 and 0.835, P value < 0.001. Pregnancies complicated by preeclampsia are associated with decreased maternal serum level of pregnancy specific glycoprotien. This abnormally decreased levels of might be a reflection of stressed or dysfunctional syncytiotrophoblasts, which is related to the pathogenesis of this placental disorder.

Abstract:BACKGROUND: Preeclampsia is hypertension in pregnancy which are characterized by high blood pressure, proteinuria that occur after 20 weeks of GA. Preeclampsia remains a massive cause of maternal morbidity and mortality that 50.000 death annually. The cause of preeclampsia is still unclear but there is a possibility caused by immunological processes in micro placenta environment during the early age of pregnancy. It is suggested that cytokines such as tumor necrosis factor (TNF-α) has an important role in the pathogenesis of preeclampsia. Preeclampsia is an extreme feature of the systemic inflammatory response during pregnancy. Systemic inflammation in preeclampsia can cause organ damage and induce sepsis. The pathophysiology is initiated by a high level of pro-inflammatory cytokine that released by peripheral blood mononuclear cell (PBMC). Beside pro-inflammatory cytokine, the marker of sepsis can be shown by procalcitonin (PCT) that produced by PBMC which is activated by TNF-α. AIM: The objective of the study is to evaluate profile maternal plasma levels of TNF-α and PCT and analyze their correlation in normotensive pregnant woman, preeclamptic and preeclampsia with sepsis. METHODS: An observational cross-sectional study. The sample were normotensive, preeclamptic, and preeclamptic with sepsis (n = 18) in Bangil Hospital, Pasuruan. The level of TNF-α and PCT was measured by ELISA. The statistical analysis with SPSS 18.0 with p < 0.05. RESULTS: This study showed level of TNF-α and PCT in preeclamptic with sepsis was significantly higher than control (p 0.05). The level of TNF-α and PCT in preeclampsia compared with control was not a significant difference (p > 0.05). This study showed there was no correlation between TNF-α and PCT in patients with preeclampsia with sepsis. CONCLUSION: The plasma level of TNF-α and PCT was statistically different between the control group, preeclampsia and preeclampsia with sepsis. There was no significant difference of TNF-α and PCT plasma level in preeclampsia with sepsis than preeclampsia group. There was no significant correlation between preeclampsia in woman and preeclampsia with sepsis in maternal plasma TNF-α and PCT levels.
Can Blood Inflammatory Markers Identify Pregnant Women with Preeclampsia? Still a Controversial Issue

Abstract:Background: Preeclampsia is a unique vascular disease during pregnancy that generally appears after 20 of weeks gestation or until 6 weeks after delivery. Left undiagnosed, preeclampsia can lead rapidly to death of both mother and fetus. Objectives: To verify the efficacy of peripheral blood inflammatory markers (BIMs)in diagnosing preeclampsia and compare them with results from other studies. Methods: Our retrospective case-control study comprised two patient groups. Pregnant women with preeclampsia and pregnant women without preeclampsia were compared for BIMs: neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR), and mean platelet volume (MPV). The primary endpoint of our research was to assess the predictive power of BIMs for preeclampsia diagnosis. Results: The sample size was calculated based on expected differences of BIMs between the control and study groups. Comparison of quantitative variables was conducted with independent sample t-test or alternatively by Wilcoxon rank sum test. The MPV values were slightly higher in the preeclampsia group, but not statistically significant. NLR and PLR did differentiate between study and control groups. Conclusions: The diagnostic accuracy of BIMs is unsatisfactory for preeclampsia diagnosis. Discrepancies concerning these values need to be clarified. Further large prospective studies are necessary to validate the potential factor accuracy in preeclampsia diagnosis.
Urinary protein and coagulation‐fibrinolysis indicators in preeclampsia: Expression and significance

Abstract:This study investigates the expression and significance of urinary protein and coagulation‐fibrinolysis indicators in preeclampsia, categorized into mild preeclampsia (109 cases) and severe preeclampsia (97 cases) based on disease severity. Additionally, 110 patients with gestational hypertension (gestational hypertension group) were included for comparative analysis. General information, laboratory indicators, urinary protein, and coagulation‐fibrinolysis indicator levels were collected for each group. Significant differences were observed in blood pressure among groups (P .05). Total cholesterol, triglycerides, and low‐density Lipoprotein levels in severe preeclampsia were higher than those in mild preeclampsia and gestational hypertension groups, whereas high‐density lipoprotein, albumin, and platelet levels were lower in severe preeclampsia. No significant differences were observed in prothrombin time or D‐dimer levels among groups (P > .05). Urinary protein, urinary protein quantification, activated partial thromboplastin time, thrombin time, and fibrinogen were identified as influencing factors for adverse maternal and infant outcomes in severe preeclampsia patients. The study concludes that urinary protein and coagulation‐fibrinolysis indicators are elevated in preeclampsia, particularly in severe preeclampsia cases, suggesting their potential use as diagnostic influencing factors for severe preeclampsia.
