Abstract:Objective: To present our classification for peripheral nerve schwannomas as well as explore the surgical strategies and operative management of peripheral nerve schwannomas based on the intraoperative neurophysiological monitoring (INM) technique and to decrease the risk of postoperative neurological deficits in the management of these schwannomas. Materials and methods: A retrospective study was conducted on 92 cases of peripheral nerve microsurgery performed, using the INM technique. We also made the classification for peripheral nerve schwannomas into two types according to operative findings and proceeded corresponding surgical strategies. Results: All tumors were removed completely under microscopy and INM. Three patients developed residual neurological deficits at final follow-up. There were different results about temporary (18/92, 19.6%) and permanent (3/92, 3.3%) neurological deficits. The incidence of temporary and permanent neurological deficits in type II group was significantly higher than that in type I group (p < 0.01). The incidence of permanent neurological deficits in larger size tumors was significantly higher than that of smaller size (p < 0.01). Conclusions: We made the classification for peripheral nerve schwannomas according to operative findings based on INM that is helpful to our surgical strategies. Intracapsular enucleation was the preferred strategy with satisfactory results and low risk of nerve injury. The size and location of tumors seem to be related to the risk of fascicular injury. Abstract:Bielecki and Kumar (1988) show that the threshold or hedging-point production policies are optimal in a continuous manufacturing system, even if production ability is uncertain. Their analysis assumes constant demand and processing time. In this paper, we consider a discrete manufacturing system in which production capacity, demand, and processing time are all nondeterministic. We formulate the problem into a discrete Markovian production model, and explore the most cost-effective control policy for such a system. With two more sources of uncertainty, we find that the threshold control policies are optimal among all feasible policies when the long run average cost is to be minimized. This extends Bielecki and Kumar's result which shows that the threshold policies are optimal among a subset of feasible policies.
Application of intraoperative neuroelectrophysiologic monitoring to gliomas resection in eloquent function brain regions

Abstract:Objective To explore the value of intraoperative neuroelectrophysiologic monitoring (IONM) to the surgery for gliomas in eloquent function brain regions. Method The clinical data of 738 patients with gliomas in eloquent function brain regions, who underwent surgery under IONM from January, 2012 to December, 2014, were analyzed retrospectively. Results Of 738 patients, 608 underwent somatosensory evoked potential or motor evoked potential monitoring under general anesthesia and 130 did cortical and subcortical motor mapping or awake language mapping. Eighty-six patients (11.7%) suffered from early motor deficit after the surgery. Late motor deficit was observed in 28 patients (3.8%). Twenty-one patients (28.8%, 21/73) had early speech deficit and late speech deficit was only observed in 3 patients (4.1%, 3/73). The rate of gross total resection of the tumors was 89.3% in 738 patients. Conclusions IONM can be applied safely and effectively to the resection of gliomas in the eloquent function brain regions because it is helpful to the protection of the eloquent function cortexes and subcortical pathways and total resection of the tumors as possible in the patients with gliomas in eloquent function brain regions.
[Application of Intraoperative Neurophysiological Monitoring in Spinal Cord Surgery].

Abstract:OBJECTIVE:To study application of intraoperative neurophysiological monitoring(IONM) in spinal cord surgery.METHODS:In the study, 24 patients with spinal cord tumors were divided into 4 groups according to the method of monitoring. Group A (ventral tumors) underwent somatosensory evoked potential (SEP) + motor evoked potential (MEP) + electromyography (EMG) monitoring, group B (intramedullary tumors) underwent SEP + MEP monitoring, group C (subdural extramedullary tumors) underwent SEP + EMG monitoring, group D (conus medullary and cauda equina lesions) underwent EMG monitoring. The relationship between changes of IONM and postoperative neurological function was analyzed.RESULTS:All of the 3 patients in group A had IONM alteration which recovered before the end of surgery, but neurological function deteriorated in 1 case. All of the 6 patients in group B had IONM alteration, of whom 5 recovered before the end of surgery, and 1 did not recover and neurological function deteriorated in this case. Seven of the nine patients in group C had IONM alteration which recovered before the end of surgery, but neurological function deteriorated in 1 case. All of the 6 patients in group D had IONM alteration which recovered before the end of surgery, but neurological function deteriorated in 1 case.CONCLUSION:IONM is useful to reduce postoperative neurological deterioration. However, IONM neurophysiological monitoring needs further improvement.

Abstract:Objective: To explore the clinical features, surgical treatment, and effectiveness of neurofibromas associated with neurofibromatosis type 1 (NF1). Methods: A clinical data of 41 patients with NF1 admitted between December 2018 and April 2024 was retrospectively analyzed. There were 15 males and 26 females, with an average age of 27.5 years (range, 5-61 years). Only one type of neurofibroma existed in 3 patients and the rest of the patients had more than two types of neurofibromas. Fourteen patients had total resection of multiple cutaneous neurofibromas (CNF). Eighteen patients of diffuse neurofibromas underwent total, near-total, or subtotal resection. Among the 13 patients of localized nodular neurofibromas, 9 of benign tumors underwent total sub-capsular resection and 4 of malignant peripheral nerve sheath tumor (MPNST) underwent maginal resection, and only 1 underwent postoperative radiotherapy and chemotherapy. Among the 15 patients of plexiform neurofibromas (PNF), 5 patients underwent both superficial and deep PNF resection, 2 underwent the superficial PNF resection, and 8 underwent the large nodular lesions in the deep PNF resection. There were 8 MPNST, of which 7 cases underwent total sub-capsular resection and large tumor capsule resection under neurophysiological monitoring, and 1 case with the tumor located on the top of the head underwent wide resection and skin grafting. One patient underwent proton knife therapy after surgery, 2 patients did not receive radiotherapy, and the remaining patients received conventional radiotherapy. Results: All patients were followed up after surgery, and the follow-up time was 3-66 months, with an average of 25.0 months. Patients with CNF recovered satisfactorily after surgery, and there was no recurrence during follow-up. Patients with diffuse neurofibromas relieved preoperative symptoms after surgery. Three patients with diffuse neurofibromas located in the head and face recurred during follow-up. The patients with benign localized nodular neurofibromas recovered well after surgery, and only 1 patient had transient regional neuralgia after surgery. Among the patients with MPNST, 2 patients died of recurrence and lung metastasis, while the remaining 2 patients had no recurrence and metastasis during follow-up. All preoperative symptoms disappeared in patients with benign PNF, and no tumor recurrence was observed during follow-up. Two patients with PNF located in the brachial plexus had difficulty in shoulder abduction after surgery, 1 patient with PNF located in vagus developed hoarseness after surgery. Among the 8 patients with MPNST in PNF, 1 died of lung metastases and 1 died of systemic failure. The remaining 6 patients were in stable condition during follow-up, and no tumor recurrence or metastasis was observed. Conclusion: According to the clinical features of neurofibromas in patients with NF1, choosing appropriate surgical approaches can obtain good effectiveness. Because of the difficulty of completely resection, diffuse neurofibromas, especially those located in the head and face, are prone to recurrence after surgery. MPNST has the worst prognosis, high incidence of recurrence/metastasis, and short survival period. Total resection combined with radiotherapy can decrease local recurrence.
Long-Term Functional and Oncologic Outcomes of Glioma Surgery with and Without Intraoperative Neurophysiologic Monitoring: A Retrospective Cohort Study in a Single Center

Abstract:OBJECTIVE: To evaluate long-term functional and survival outcomes of patients with glioma after intraoperative neurophysiologic monitoring (IONM) application. METHODS: A total of 856 patients with glioma, who underwent tumor resection between October 2010 and March 2016, were included in this retrospective cohort study. All patients were stratified into IONM (439 patients) and non-IONM groups (417 patients). The primary outcome measured was overall survival (OS), and the secondary outcome measured was rate of late neurologic deficits. Analyses were performed using univariate tests and multivariate logistic regression and Cox proportional hazard model. RESULTS: The 2 cohorts were well balanced with respect to baseline characteristics. Univariate survival analysis showed longer OS in the IONM group than that in the non-IONM group (P = 0.036), especially in patients with high-grade astrocytic tumor (P = 0.034). The IONM group showed a lower rate of neurologic deficits than did the non-IONM group. Multivariate analysis showed that IONM was a favorable factor of OS (odds ratio, 0.776; P = 0.046) and late neurologic function (odds ratio, 0.583; P = 0.039). Dominant hemispheric and eloquent location of glioma had no association with OS. CONCLUSIONS: Application of IONM is beneficial to long-term functional and oncologic outcomes of patients with glioma.
Perioperative Observations and Outcome in Surgical Treatment of Malignant Peripheral Nerve Sheath Tumors

Abstract:Background/Objectives: This retrospective observational study aimed to investigate the perioperative outcome in Malignant Peripheral Nerve Sheath Tumors (MPNSTs) with and without relation to Neurofibromatosis Type 1 (NF1) and to detect possible influencing factors. Methods: Clinical reports, histopathological evaluations, imaging, and treatment characteristics were reviewed in 35 operated MPNSTs in 33 patients. Possible predictive valuables included disease type, preoperative tumor volume, SUV and MIB-1 proliferation index, resection margins, the presence of metastasis, and whether radio-/chemotherapy was received. Results: Patients with NF1 were younger (mean age: 29 ± 13, 8–54 years) than sporadic cases (mean age: 45 ± 13, 24–67 years) and exhibited significantly larger preoperative tumor volumes (mean 299 vs. 18 cm3, p = 0.048). Most tumors were located in the facial/cervical/neck area (34%, n = 12), followed by the trunk (31%, n = 11), lower extremity (17%, n = 6), upper extremity (14%, n = 5), and intraspinal area (3%, n = 1). NF1-associated MPNSTs appeared predominantly on the trunk (39%) and sporadically in the facial/cervical/neck area (50%). Complete resection was possible in 66% and an improvement in or stability of function was achieved in most cases (motor 69%, sensory 74%), as well as a decrease in pain intensity (63%). NF1-associated MPNSTs exhibited more severe pain scores (median VRS scale 2, p = 0.002) compared to sporadic tumors (median VRS scale 0.5). Sporadic MPNSTs located at the head/facial/brachial plexus and upper extremities exhibited better preoperative functions compared to those on the lower extremities. In 12 cases with available [18F]FDG PET, the mean preoperative SUV (9.8 ± 7.2) positively correlated with the mean maximum MIB-1 index (34 ± 26%, p = 0.005) and the mean preoperative tumor volume (474.7 ± 68.6 cm3, p = 0.047). The overall survival (OS) was significantly longer in tumors with higher resection extents (R0, p = 0.01) and without accompanying metastasis (p = 0.046), and tended to be longer, but not significantly so, in sporadic MPNSTs. In six and seven tumors, with R1/R2 resection margins and present metastasis, respectively, solid or combined neo-/adjuvant radio-/chemotherapy led to a significantly shorter OS (p = 0.014). Conclusions: NF1-associated MPNSTs have larger tumor volumes, higher SUVs and MIB-1 proliferation indices, and a shorter overall survival period. Nevertheless, surgery can improve symptoms, particularly medication-resistant pain, and should also be considered in advanced disease for symptom control/improvement.

The Intraoperative Utilization of Multimodalities Could Improve the Prognosis of Adult Glioblastoma: A Single-Center Observational Study.

Abstract:-OBJECTIVE: In recent years, numerous neurosurgical multimodal techniques have been utilized to maximize tumor resection safely and effectively. However, the synergetic effects of neurosurgical multimodalities on the survival of glioblastoma patients remain unclear. This study evaluated the role of intraoperative utilization of multi -modalities in glioblastoma patients. -METHODS: Data of 912 adult patients with glioblastoma were obtained from the Huashan Glioma Registry. The utilization of fewer than 2 (multimodality value < 2) intra-operative multimodal techniques was defined as the non-multimodal group. In contrast, the utilization of 2 or more (multimodality value >= 2) intraoperative multimodal techniques was regarded as the multimodal group. The prognosis of the 2 cohorts was compared and further stratified based on the diagnosis date (2010-2014 or 2015-2019) to reveal the role of the application of multimodal techniques. -RESULTS: The median overall survival (OS) and progression-free survival of glioblastoma patients were 17.70 months and 12.03 months, respectively. The OS time of the multimodal group was noticeably longer than that of the nonmultimodal group (21.0 months vs. 16.0 months, P < 0.001). Multimodal techniques were more frequently applied in surgery in the 2015-2019 group than in the 2010-2014 group. The popularity of multimodal techniques contributed to significant improvement in the prognosis of glioblastoma patients from 2010-2014 to 2015-2019 (OS, 16.0 months vs. 22.0 months, P < 0.001). -CONCLUSIONS: This study indicated that the utilization of intraoperative multimodal techniques improved the extent of resection and elevated the survival for adult glioblastoma patients.

Abstract:Surgery for spinal cord tumors poses a significant challenge due to the inherent risk of neurological deterioration. Despite being performed at numerous centers, there is an ongoing debate regarding the efficacy of pre- and intraoperative neurophysiological investigations in detecting and preventing neurological lesions. This study begins by providing a comprehensive review of the neurophysiological techniques commonly employed in this context. Subsequently, we present findings from a cohort of 67 patients who underwent surgery for intradural tumors. These patients underwent preoperative and intraoperative multimodal somatosensory evoked potentials (SSEPs) and motor evoked potentials (MEPs), with clinical evaluation conducted three months postoperatively. The study aimed to evaluate the neurophysiological, clinical, and radiological factors associated with neurological outcomes. In univariate analysis, preoperative and intraoperative potential alterations, tumor size, and ependymoma-type histology were linked to the risk of worsening neurological condition. In multivariate analysis, only preoperative and intraoperative neurophysiological abnormalities remained significantly associated with such neurological deterioration. Interestingly, transient alterations in intraoperative MEPs and SSEPs did not pose a risk of neurological deterioration. The machine learning model we utilized demonstrated the possibility of predicting clinical outcome, achieving 84% accuracy.

Abstract:Background:It has been reported that the incidence of recurrent laryngeal nerve (RLN) injury is higher in an operational procedure in the thyroid and parathyroid region. Elevating voice pitch is achieved by the cricothyroid contraction, which in turn is innervated by the external branch of the superior laryngeal nerve (EBSLN). Due to the subtle nature and clinical variability of EBSLN damage, diagnosis may be difficult. The use of intraoperative neuromonitoring (IONM) as a supplement to enhance surgical identification in thyroid and parathyroid operation appeared to have collected momentum with operators. Methods:IONM clinical usage rationale, efficiency, and safety profile are discussed in this overview in thyroid and parathyroid surgery. Results:IONM usage incurred numerous skepticisms in decreasing the frequency of EBSLN or RLN injury. However, a recent article has shown the efficacy and benefits of using IONM in thyroid and parathyroid procedures. IONM facilitates the recognition of RLN and EBSLN, verifies its functional integrity, localizes the site of a nerve injury, and gives postsurgical function feedback. This technique has been successful in reducing rates of bilateral laryngeal paralysis, with the elevated safety profile for surgical patients. Conclusions:It is recommended using IONM in all thyroidectomies and in high-risk parathyroidectomies.
Clinical Application of 3T Intraoperative Magnetic Resonance Image-Guided Functional Neuronavigation Combined with Intraoperative Neurophysiological Monitoring in Resection of Insular Gliomas.

Abstract:Objective To evaluate the clinical application of 3.0T intraoperative magnetic resonance image(iMRI)-guided functional neuronavigation combined with intraoperative neurophysiological monitoring(IONM) in resection of insular gliomas.Methods From September 2010 to June 2011,18 gliomas involving insular lobe were resected in IMRIS 3.0 T iMRI integrated surgical suite.An intraoperative diffusion tensor imaging(DTI)-based navigation combined with continuing transcortical motor evoked potential(MEP) was performed to localize the pyramidal tracts in all patients.Furthermore,an awake craniotomy with direct cortical electrical stimulation mapping(ESM) was performed to locate the eloquent language area in 10 patients with tumors in dominant hemisphere.The results of intraoperative imaging,IONM and the surgical consequences were analyzed.Results Intraoperative imaging revealed residual tumors in 13 of 18 cases.Six patients with residual tumors further underwent iMR image-Guided resection to remove the tumors.As a result,the total removal rate of tumors increased from 5/18 to 9/18.The Fisher exact test demonstrated a statistically significance(P = 0.046).Tumors in nine cases were not completely removed because DTI navigation and IONM revealed that the lesions were closed to the functional cortex or deep pyramidal tract.Among ten patients with tumors in the primary hemisphere,five developed transient language dysfunction 1 week after operation.Their language function returned to preopoerative level or above by one month after surgery.Among total 18 patients,three developed postoperative motor dysfunction.By one month after operation,two of them had fully recovered their motor function while one patient still had neurologic dysfunction.No side effect or IMRI and the IONM related complications occurred.Conclusion A combination of 3T iMR image-based neuronavigation and IONM helps to maximize the safe removal of the insular cortex glioma.
Multimodal Intraoperative Neurophysiological Monitoring in Intramedullary Spinal Cord Tumors: A 10-Year Single Center Experience

Abstract:Objective: The study aimed at evaluating the efficacy and the ability of D-wave monitoring combined with somatosensory evoked potentials (SSEPs) and motor evoked potentials (MEPs) to predict functional outcomes in intramedullary spinal cord tumor (IMSCT) surgery. Methods: Between December 2011 and December 2020, all patients harboring IMSCT who underwent surgery at our institution were prospectively collected in a surgical spinal registry and retrospectively analyzed. Patient charts and surgical and histological reports were analyzed. The multimodal IONM included SSEPs, MEPs, and-whenever possible-D-waves. All patients were evaluated using the modified McCormick and Frankel grade at admission and 3, 6, and 12 months of follow-up. Results: Sixty-four patients were enrolled in the study. SSEP and MEP monitoring was performed in all patients. The D-wave was not recordable in seven patients (11%). Significant IONM changes (at least one evoked potential modality) were registered in 26 (41%) of the 64 patients. In five cases (8%) where the SSEPs and MEPs lost and the D-wave permanently dropped by about 50%, patients experienced a permanent deterioration of their neurological status. Multimodal IONM (SSEP, MEP, and D-wave neuromonitoring) significantly predicted postoperative deficits (p = 0.0001), with a sensitivity of 100.00% and a specificity of 95.65%. However, D-waves demonstrated significantly higher sensitivity (100%) than MEPs (62.5%) and SSEPs (71.42%) alone. These tests' specificities were 85.10%, 13.89%, and 17.39%, respectively. Comparing the area under ROC curves (AUCs) of these evoked potentials in 53 patients (where all three modalities of IONM were registered) using the pairwise t-test, D-wave monitoring appeared to have higher accuracy and ability to predict postoperative deficits with strong statistical significance compared with MEP and SSEP alone (0.992 vs. 0.798 vs. 0.542; p = 0.018 and p < 0.001). Conclusion: The use of multimodal IONM showed a statistically significant greater ability to predict postoperative deficits compared with SSEP, MEP, and D-wave monitoring alone. D-wave recording significantly increased the accuracy and clinical value of neurophysiological monitoring in IMSCT tumor resection.

Abstract:Objective: To analyze the relationship between tumors of the musculoskeletal system and adjacent nerves by reconstructing images of peripheral nerves, and explore its value in surgical treatment. Methods: From May 2016 to April 2019, a total of 27 patients were collected in Department of Imaging,Shougang Hospital, who were with skeletal muscle system tumors, including 15 primary soft tissue tumors, 9 primary bone tumors, 3 metastatic tumors, all of them were closely related to nerves. There were 17 males and 10 females, aged 13-67 years, with an average age of 34 years. Before the operation, CT volume scanning was performed, and curved planar reconstruction (CPR) was used to reconstruct the peripheral nerves. All patients were operated within 2 weeks after the examination. According to the image characteristics before the operation, the nerve invasion was judged. The sensitivity, characteristic, positive predictive value and negative predictive value of the tumor invasion (compression) nerve were calculated according to the intraoperative findings as the gold standard. Result: Of the 27 cases, 25 cases (25/27, 92.6%) could show the relationship between the tumors and the adjacent nerves at the same level, and 22 cases (22/25, 88.0%) had the same preoperative image judgments as the intraoperative findings. In the reconstructed images, the peripheral nerve was a continuous strip-like structure on the same level with the tumor. The invaded nerve became thicker and the edge was blurred. Enhanced scan showed enhancement. The sensitivity, specificity, positive predictive value and negative predictive value of neuroimaging reconstruction were 100.0%, 89.5%, 75.0% and 100.0% respectively. The sensitivity, specificity, positive predictive value and negative predictive value of the nerve compression were 92.3%, 100.0%, 100.0% and 80.0% respectively. Conclusions: Neurological reconstructed images can help clinicians evaluate the relationship between lesions and adjacent nerves quickly and intuitively. They can guide the selection of surgical methods, reduce the risk of intraoperative nerve injury, and have high sensitivity and specificity for nerves invasion.
Spinal intradural extramedullary tumors: microscopic keyhole resection with the focus on intraoperative neurophysiological monitoring and long-term outcome

Abstract:Objective: Spinal schwannomas (SS) and spinal meningiomas (SM) account for most intradural extramedullary (IDEM) tumors. These tumors are usually benign lesions, which generally respond favorably to surgical excision. Few studies up to now tried to determine the long-term outcome after minimally invasive surgery (MIS) with multimodal intraoperative neurophysiological monitoring (IONM) for IDEM tumors. The aim of this study was to present one of the largest case series with special regard to IONM findings and long-term outcome after MIS-keyhole surgery with a tubular retractor system. Methods: Between January 2013 and August 2018, 87 patients with IDEM tumors who underwent tumor removal surgery via MIS-keyhole approach under multimodal IONM were retrospectively reviewed. The neurological status was assessed using a modified McCormick grading scale pre- and postoperatively. Multimodal IONM consisted of motor evoked potentials (MEP), somatosensory evoked potentials (SEP), and electromyography (EMG). Both short-term and long-term clinical evaluations as well as patients' medical files were retrospectively analyzed. Results: Surgeries were performed for resection of SS in 49 patients and SM in 38 patients. Tumor locations were cervical in 16.1%, thoracic in 48.3%, thoracolumbar in 4.6%, lumbar 31%. Critical IONM changes were detected in 9 operations (10.3%) in which there were 2 SEPs, 5 MEPs, and 2 EMG events. Three IONM changes (2 MEPs, 1 EMG) were turned out to be transient change in nature since they were resolved in a short time when immediate corrective actions were initiated. Six patients with permanent IONM changes (2SEPs, 3MEPs, 1EMG event), all deficits had resolved during hospitalization or on short -term follow-up evaluation. Sensitivity, specificity, and positive and negative predicted values of IONM were 100, 96, 67, and 100%, respectively. Gross total resection rate was 100%, and a stable or improved McCormick grade exhibited in all patients. No tumor recurrence and no spinal instability were found in the long-term follow-up evaluation (mean 5.2 ± 2.9 years postoperatively). Overall, 94% of patients were either satisfied or very satisfied with their operation, and 93% patients reported excellent or good general clinical outcome according to Odom's criteria. Conclusion: MIS-keyhole surgery with multimodal IONM for IDEM tumors enables a high level of satisfaction and a satisfying long-term clinical and surgical outcome.

Abstract:Intraoperative neurophysiological monitoring (IONM) is a crucial advancement in neurosurgery, enhancing procedural safety and precision. This technique involves continuous real-time assessment of neurophysiological signals, aiding surgeons in timely interventions to protect neural structures. In addition to inherent limitations, IONM necessitates a detailed anesthetic plan for accurate signal recording. Given the growing importance of IONM in neurosurgery, we conducted a narrative review including the most relevant studies about the modalities and their application in different fields of neurosurgery. In particular, this review provides insights for all physicians and healthcare professionals unfamiliar with IONM, elucidating commonly used techniques in neurosurgery. In particular, it discusses the roles of IONM in various neurosurgical settings such as tumoral brain resection, neurovascular surgery, epilepsy surgery, spinal surgery, and peripheral nerve surgery. Furthermore, it offers an overview of the anesthesiologic strategies and limitations of techniques essential for the effective implementation of IONM.

Abstract:The removal of vestibular schwannomas carries a risk of facial palsy. This study aims to evaluate the usefulness and technical aspects of intraoperative monitoring (IOM) for the facial nerve. A total of 96 patients who underwent surgery for vestibular schwannoma were retrospectively investigated. The cohort was divided into two groups: those with intraoperative facial nerve monitoring (IOM group) and those without IOM (non-IOM group). Preoperative and postoperative facial nerve functions were assessed using the House-Brackmann (HB) scale immediately after surgery, at discharge, and at the 1-year follow-up. HB grade I and II were classified as satisfactory outcomes, HB grade III and IV as intermediate, and HB grade V and VI as poor. Facial nerve functions were compared between the groups. Additionally, the ratio of satisfactory results was investigated in the IOM group, focusing on whether the root exit zone (REZ) was identified at an early or late stage of surgery. Among the 65 (67%) patients in the IOM group and 31 (32%) patients in the non-IOM group, there were no differences in demographic and tumor characteristics. The extent of resection varied from subtotal to gross total removal, with no statistical differences between the groups. Although facial nerve function was more favorably preserved in the non-IOM group immediately after surgery, this trend reversed at discharge and the 1-year follow-up, showing significant statistical differences. In the IOM group, more patients achieved satisfactory outcomes when the REZ was identified early compared to late during tumor resection. Intraoperative facial nerve monitoring provides more satisfactory outcomes in preserving nerve function in vestibular schwannoma surgery. Early recognition of the REZ may contribute to improved surgical outcomes.

Abstract:INTRODUCTION: Intraoperative neuromonitoring (IONM) is used in real-time to alert neurosurgeons of potential neurological changes when resecting intramedullary spinal cord tumors (IMSCTs). Previous studies have reported variable reliability of IONM. METHODS: A multicenter retrospective study was performed on patients undergoing IMSCTs surgery with neuromonitoring at the three Mayo Clinic sites between 2005 to 2022. Demographics, SSEP, MEP changes and postoperative outcomes were recorded. Specificity, sensitivity, positive predictive value (PPV), and negative predictive value (NPV) were calculated. Variables found significant on univariate analysis (p < 0.05) were applied to a multivariate model to identify predictors for IONM SSEP and MEP changes. RESULTS: 135 patients underwent surgical resection for IMSCTs using IONM. The mean age at diagnosis was 45 ± 17.9 years. Laminectomy was the most frequent index operation (96.3%). A total of 8.9% of patients presented with short-term improvement, while 70.4% presented long-term improvement on their baseline McCormick Scale. Sensitivity, specificity, PPV, NPV, accounted for 86.2%, 51%, 5.2%, 99.2% for MEP; 77.8%, 48.2%, 4.4%, 98.6% for SSEP. We found predictors of IONM changes: poorly defined MEP baseline (OR 3.004, 95% CI 1.22-7.397, p = 0.017), WHO grade II tumors (OR 2.334, 95% CI 1.043-5.223, p = 0.039), cervicomedullary junction location (OR 0.103, 95% CI 0.011-0.983, p = 0.048). Only WHO grade II tumors (OR 2.85, 95% CI 1.208-6.727, p = 0.017) was a predictor for SSEP changes. CONCLUSIONS: Our study represents the largest cohort describing sensitivity, specificity, and predictive factors for intraoperative changes and postoperative outcomes of resection of IMSCTs. A poorly defined baseline and cervicomedullary tumor location were predictors of IONM MEP changes, whiIe WHO grade II tumors were a predictor for both IONM SSEP and MEP changes.

Abstract:Objective To analyse the intra-operative neurophysiological monitoring (IONM) data in patients with dystrophic neurofibromatosis type 1 (NF1) associated scoliosis, and to investigate the possible risk factors for failed IONM monitoring. Methods Patients undergoing posterior spinal fusion from September 2015 to December 2019 were retrospectively reviewed. The latency (P37, N50) and amplitude of somatosensory evoked potentials (SEP) in bilateral lower extremities, latency and amplitude of motor evoked potentials (MEP) in bilateral lower extremities and unilateral upper extremity were recorded. The neurological status, curve pattern, Cobb angle of main curve, vertebral rotatory subluxation and dystrophic features at pre-operation were assessed for each patient. The failed IONM monitoring was defined as no reliable SEP or MEP waveforms of all monitored muscles. Results A total of 92 patients (53 M, 39 F) with an average age of 14.1 +/- 2.7 years were included. Failed IONM monitoring was identified in 17 patients with overall success rates being 87.0 and 94.6% for SEP and MEP. The average P37 latency, N50 latency, SEP amplitude and MEP latency showed no significant difference between concave and convex sides (p > 0.05 for all). The MEP amplitudes of lower extremities were lower on concave side than convex side (334.5 +/- 291.9 mu V VS 417.5 +/- 380.5 mu V, p = 0.030). Higher risk of failed IONM monitoring was found in patients with neurological deficit (p = 0.014) and more dystrophic features (p = 0.002) at pre-operation. Conclusions The overall success rates were 87.0% for SEP and 94.6% for MEP in patients with NF1-associated scoliosis. Neurological deficit and more dystrophic features at pre-operation indicated higher risk of failed IONM monitoring.

Abstract:Objective To investigate the role of intraoperative ultrasound and sodium fluorescein supported by neuronavigation in microsurgery for malignant neoplasm of cerebrum brain.Methods A retrospective analysis was conducted on 9 patients with malignant tumors in the sports area from November 27,2017 to March 12,2018 in the Brain Center of the Shenzhen Second People's Hospital.After diffusion tensor imaging (DTI) combined with neuronavigation,all patients were designed for surgical approach.We injected sodium fluorescein into veins before surgery to highlight brain tumors and used intraoperative ultrasound to determine the location of brain tumors.The consistency of tumor localization was compared by neuronavigation,ultrasound and sodium fluorescein.Under electrophysiological monitoring,the tumor resection was performed by the same surgeon based on the spatial relationship between the tumor and the pyramidal tract as shown by neuronavigation and the tumor boundary displayed by sodium fluorescein.Results The accuracy of neuronavigation to locate brain tumors and pyramidal tract was achieved in 100％ before surgery.The consistency of neuronavigation and ultrasound to locate brain tumors was achieved in 100％ before cutting the dura.The consistency of ultrasound and sodium fluorescein to locate brain tumors was achieved in 100％ after cutting the dura.Eight patients (89％) underwent total resection and one case (11％) underwent subtotal resection.Muscle strength was improved in 2 patients while 4 patients was the same as before.Three patients experienced a transient decline in muscle strength,which gradually retumed to normal after rehabilitation exercise.Conclusion Neuronavigation combined with DTI can well define the spatial relationship between tumo r and pyramidal tract.Intraoperative ultrasound and sodium fluorescein staining can locate the tumor in real time and increase the tumor resection rate and reduce the probability of postoperative dysfunction.
