Abstract:The optimal surgical management of patients presenting with an aortic coarctation together with other cardiovascular disorders is unclear. In this study, we report the case of an adult male with an aortic coarctation associated with a bicuspid aortic valve and an ascending aortic aneurysm. The patient underwent single-stage repair involving the Bentall technique and total arch replacement combined with stented elephant trunk implantation, which was performed through median sternotomy. We consider this procedure to be a suitable alternative for treating these complex cases.

Abstract:We report the case of a 9-year-old male with severe congenital pulmonary valve stenosis referred to our centre for percutaneous valvotomy. On admission, trans-thoracic echocardiogram confirmed a unicuspid pulmonary valve with a peak/mean pulmonary valve gradient of 91/53 mmHg and a pulmonary annulus of 13.8 mm (−0.8 Z Score). It also showed an enlarged RV (RV/LV ratio 0,9). During cardiac catheterisation, an additional atrial septal defect (secundum) with significant left to right shunt (Qp/Qs > 2) was diagnosed, which was not amenable to percutaneous closure. The patient was referred for surgical repair. The atrial septal defect was closed by a direct running suture. The repair of the unicuspid valve consisted in bicuspidisation by a large commissurotomy to the left anterior wall of the pulmonary artery. The neo-commissure was created with two separate patches of autologous pericardium secured to the wall of the pulmonary root. The adjustment of the effective height of the pulmonary valve leaflets was done by trimming the patches and a triangular plication of the newly created posterior leaflet. Perioperative echocardiogram showed a peak gradient of 15 mm Hg and trivial pulmonary regurgitation. The total cross-clamp time was 92 min and the bypass time 123 min with a favourable evolution after the surgery. The particularity of the case is represented by the complexity of the bicuspidisation procedure. Using this technique, a tailored approach is needed for every patient.

Abstract:This study aimed to investigate the safety, feasibility and availability of doubly committed ventricular septal defect (DCVSD) closure via an ultraminimal intercostal incision under the guidance of transoesophageal echocardiography in children. From August 2014 to August 2016, 35 children with DCVSDs (a parts per thousand5 mm in diameter) were enrolled in this study. A left parasternal ultraminimal intercostal incision (a parts per thousand1 cm) and a pericardium hanging technique were employed without sternal incision. DCVSDs were closed through a short delivery sheath assembled with an eccentric occluder device. Transoesophageal echocardiography was used to guide and monitor the entire procedure. All patients were followed up. All 35 children had complete closures with an operation success rate of 100%. The average size of DCVSDs was 3.50 +/- 0.79 (range 2.2-5.0) mm, and the average device size was 5 +/- 2 (range 4-9) mm. The average operation duration was 45.42 +/- 11.77 (range 25-70) min, and the average hospital stay was 8 +/- 2 (range 7-16) days. The median follow-up period was 17 months (range 6 months-2.5 years). Pre-existing aortic regurgitation disappeared after surgery in 1 patient and remained the same in 4 patients. No other complications were found during the operation or during follow-up. Under transoesophageal echocardiography guidance, DCVSD closure using an eccentric occluder via an ultraminimal intercostal incision is feasible, safe and effective in children. The use of this approach is recommended.
Efficacy of arterial duct stenting in neonatal pulmonary atresia with intact ventricular septum]

Abstract:To evaluate the feasibility and efficacy of arterial duct stenting in neonates with pulmonary atresia and intact ventricular septum.Eleven neonatal pulmonary atresia with intact ventricular septum patients received arterial duct stenting in our hospital from December 2007 to September 2010 were involved in this study. The average age was (8.20 +/- 2.90) days (ranged from 3 to 13 days). The average weight was (3.41 +/- 0.29) kg (ranged from 3.00 to 3.88 kg). The stents were selected according to digital subtracted angiography measurements. After checking for correct position by angiography, the balloon was inflated to expand the stent to desired diameter. Oxygen saturation was monitored, echocardiography was measured and stent diameter and location were observed by chest Xray. Patients were followed up at 1, 3, 6 and 12 months post procedure.Stents were successfully implanted in all 11 patients. The preoperative peripheral oxygen saturation was (63.27 +/- 8.47)%, while increased to (82.73 +/- 5.59)% after alprostadil application and to (86.18 +/- 3.19)% after operation (all P < 0.01). After the operation, the peripheral oxygen saturation was higher than alprostadil application (P < 0.05). The intraoperative narrowest diameter of patent ductus arteriosus was (1.69 +/- 0.37) mm, the length was (16.72 +/- 2.37) mm. The internal diameter of implant stents was 4 mm, the length was (20.18 +/- 3.40) mm. After the operation, surgical B-T shunt operation was performed in one patient due to stent shift and pulse oxygen saturation decrease. One patient died post operation with unknown reason, another patient received stent balloon dilatation due to pulse oxygen saturation decrease at 4 months after the surgery. Pulmonary atresia with intact ventricular septum surgeries were performed in 2 patients at 5 and 7 months after stent implantation.The neonatal pulmonary atresia with intact ventricular septum arterial stent implantation was a feasible and effective procedure and this method could be used as preferred treatment in pulmonary atresia and intact ventricular septum for neonates.

Abstract:BACKGROUND:New materials and devices have been used in the management of cardiac malformations. In this paper, we present our experience with interventional occlusion of congenital vascular malformations.METHODS:Between January 1997 and December 2005, 139 patients with congenital vascular malformations who had undergone interventional occlusion in the Children's Hospital, Zhejiang University School of Medicine were studied. The clinical data of the patients were retrospectively reviewed including pre-operative evaluation, surgical procedures, immediate complete closure rate, short-term complications, and short-term outcome.RESULTS:Of the 139 patients, 126 had patent ductus arteriosus, and successful deployment was achieved in 121 of the 126 patients (96%, 121/126). Six patients had coronary artery fistula and 14 different coils were used for embolization; the immediate complete closure rate was 83.3%, and the complete closure rate after one month was 100%. The abnormal vessels of 3 patients with pulmonary sequestration were completely occluded using four 0.038-inch Gianturco coils. In 3 patients with aortopulmonary collaterals, 14 abnormal vessel branches were occluded with sixteen 0.038-inch Gianturco coils, reaching a closure rate of 100%. One patient with pulmonary arteriovenous fistula was occluded successfully with two 0.038-inch Gianturco coils.CONCLUSIONS:Transcatheter closure using coils is a safe and effective alternative to surgical ligation in the management of congenital vascular malformations in children. Selection of appropriate coils is important to achieve a better outcome.
Rare Quadricuspid Aortic Valve with Acute Early Incapacitation

Abstract:A 39-year-old male patient from a county health facility presented to our department with a one-year history of progressive exertional chest tightness (NYHA class III-IV), which resolved completely within five to six minutes of rest.Both motion and static imaging results revealed a rare solitary Quadricuspid aortic valve (QAV) with severe aortic valve regurgitation.The QAV was replaced with mechanical prosthetic valve via mini-superior sternotomy on cardiopulmonary bypass machine.The patient was reviewed a month after operation, and assessment revealed that he had reverted to NYHA class-I.A rare congenital lesion such as QAV, repair may not be the best option due to lack of long-term data on longevity.Additionally, choice of a mechanical prosthetic valve guarantees unwanted re-operations associated with possible failure of bioprosthetic valves.

Abstract:Truncal valve regurgitation and interrupted aortic arch increase the need for reintervention and mortality rates in neonates undergoing truncus arteriosus repair. This featured case report presents a surgical video demonstrating the tricuspidization of a quadricuspid truncal valve during repair of truncus arteriosus and interrupted aortic arch in a 9-day-old girl. The patient had a quadricuspid and dysplastic truncal valve with moderate to severe regurgitation. Tricuspidization was successfully performed. The Institutional Review Board committee of Children's National approved the study protocol and publication of data (RB Number: Pro00015566, Initial Approval: 7/1/2021). This report highlights the surgical technique and emphasizes the importance of tricuspid valve repair in complex congenital heart diseases.

Abstract:BACKGROUND Congenital aortic and truncal valve disease is challenging in infants and children given the lack of available prostheses in very small sizes and the limited durability of homograft aortic valve replacement. METHODS A comprehensive literature search was performed using the PubMed database. Studies were included either if the report included patients less than 1 year of age or if the technique was tailored to accommodate for somatic growth. RESULTS Techniques for aortic and truncal valve repair addressing each aspect of the aortic valve complex – the aorta, aortic annulus, commissures, and cusps – were reviewed. The incidence of reoperation following aortic or truncal valve repair is significant at 10-years ranging from 30% to 70% depending on the underlying diagnosis and the repair technique utilized. A significant challenge in interpreting the published literature relates to the lack of anatomic data available in the publications limiting the ability to make direct comparisons between operative techniques and also limits the ability to draw conclusions regarding these techniques as applied to varied etiologies. CONCLUSIONS A comprehensive understanding of the aortic valve complex is necessary to achieve adequate results in pediatric aortic valve repair given the high variability in these valves.

Abstract:A bicuspid aortic valve (BAV) refers to a condition in which the valve has two cusps rather than three. Usually ignored, this manifests itself later in life. This bicuspid valve may manifest earlier in children with significant aortic stenosis because they have a severe left ventricular outflow tract restriction that worsens over time. This syndrome commonly results in congestive heart failure in newborns and early neonatal life. There can be a small amount of significant risk in pediatric patients having this BAV causing stenosis and being one of the reasons for sudden, unexpected death. Morphological differences result from a congenital cardiac abnormality called BAV. This paper emphasizes the importance of a multidisciplinary team in managing BAV and critical aortic stenosis and provides evaluation and treatment guidelines for both conditions. Transcatheter or surgical intervention is used for symptomatic individuals or those with a moderate to severe obstruction of the left ventricular outflow tract.

Abstract:The quadricuspid aortic valve (QAV) is a rare congenital disease with a prevalence of 0. 013–0.043% of cardiac cases. Most patients with QAV are treated with aortic valve replacement. A Type B QAV with dilated ascending aorta of 47.9 mm; combined with severe regurgitation is reported here. In this case, considering the patient's cusps are flexible and reservable, the aortic root was reconstructed utilizing tricuspidization and annular banding technique, and dilated ascending aorta was replaced at the same time.

Abstract:A 3‐month‐old baby girl presenting with heart failure was found to have a large persistent ductus arteriosus as well as a bicuspid aortic valve with severe aortic stenosis. She underwent surgical ductal closure following which there was complete resolution of aortic valve gradients without requiring any aortic valve intervention. This case highlights the importance of understanding hemodynamics in efficacious management of congenital heart disease.

Abstract:OBJECTIVES: Transventricular valvotomy is a kind of hybrid therapy that is widely used in developing countries where cardiac catheter intervention is limited in neonates. The purpose of this study was to evaluate the surgical outcomes and effectiveness of transventricular valvotomy in neonates with pulmonary atresia and an intact ventricular septum. METHODS: Between August 2008 and July 2013, 22 neonates with pulmonary atresia and an intact ventricular septum underwent transventricular valvotomy at our institution. The mean age and weight at initial palliation were 14.3 +/- 5.2 days (range 1-24 days) and 3.3 +/- 0.6 kg (range 2.5-3.8 kg), respectively. The mean tricuspid valve Z-score was -1.7 +/- 1.09 (range -3.5 to -0.2). All patients presented with membranous atresia and no evidence of Ebstein's malformation or right ventricle-dependent coronary circulation. RESULTS: There were two hospital deaths and no late deaths. Postoperative echocardiography showed that the gradient over the open pulmonary valve was 26.7 +/- 5.4 mmHg (range 20-32 mmHg). Patients who received patent ductus arteriosus ligation and were treated with a modified Blalock-Taussig shunt experienced a shorter intensive care unit stay than those without patent ductus arteriosus ligation (P = 0.004). The median follow-up time was 32.5 months (range 6-60 months). The freedom from reoperation was 85, 78.9, 56.5% at 6 months, 1 and 5 years, respectively. Fourteen patients underwent definitive repair, 10 underwent biventricular repair, and four received one and one-half ventricle repair. CONCLUSIONS: Transventricular valvotomy can be used successfully as an easy and safe alternative to right ventricular decompression. Furthermore, we note that oxygen saturation is a valuable indicator in determining whether a modified Blalock-Taussig shunt should be performed or not.

Abstract:Anomalous left coronary artery from the pulmonary artery is a congenital coronary artery anomaly comprising 0.5% of all CHDs. Stenosis in the left coronary artery implanted in the sinus valsalva is one of the complications of surgical repair. Management of coronary stenosis after surgery is very critical and difficult, especially in newborns and infants. Transcatheter procedures can be lifesaving. Herein, the coronary stenting procedure applied to a 3-month-old patient with post-surgical coronary stenosis is presented.

Abstract:Background: Aortic atresia with ventricular septal defect is a very rare congenital cardiac anomaly, especially in combination with aortic arch interruption. It is always challenging to choose the optimal treatment tactics for such patients. One of the possible types of intervention is the Yasui procedure. There are only 19 reported cases in the literature of aortic atresia with interruption of the aortic arch type B or C, and not a single clinical case of type A. Case presentation: The proband was a 2-day-old boy with diagnosis: aortic atresia with a ventricular septal defect and interruption of the aortic arch type B. The child underwent a Yasui procedure without serious postoperative complications and with good long-term result. Conclusions: The Yasui procedure in patients with aortic atresia and interrupted aortic arch can be performed with minimal complications, even in low-weight patients.

Abstract:Fallot-type absent pulmonary valve is a rare and complex congenital heart disease. Repair surgery for this condition during the neonatal period has a mortality rate of over 50%. We reported a neonate with Fallot-type absent pulmonary valve and occlusion of the left main bronchus. The patient's pulmonary artery had unusual anatomy of a type that has not previously been reported. This case report outlines a successful treatment strategy for patients with complex congenital heart disease and airway occlusion during the neonatal period and the effect of these unusual anatomical conditions on postoperative outcomes.

Abstract:BACKGROUND Congenital heart diseases (CHDs) are the most common form of birth defects, affecting the structure and function of neonatal hearts. Pulmonary valve stenosis (PVS) and ventricular septal defects (VSD) are 2 of the more prevalent forms, both of which can lead to significant morbidity if left untreated. The emergence of transcatheter techniques has revolutionized the therapeutic landscape, presenting minimally invasive yet effective alternatives to open-heart surgery and significantly reducing associated patient morbidity and recovery time. CASE REPORT The presented case details the management of a 19-year-old man with complex CHDs, highlighting the nuanced decision-making process that led to a transcatheter approach. The patient's clinical presentation, marked by symptoms reflective of significant cardiac compromise, demanded a tailored approach that utilized the latest advancements in non-surgical intervention. The successful closure of the VSD with an Amplatzer device and the resolution of PVS via balloon valvuloplasty were achieved without complications, showcasing the potential of these techniques in managing similar cases. The post-intervention period was marked by a noteworthy recovery, confirming the procedural efficacy and enhancing the patient's quality of life. CONCLUSIONS The favorable outcome of this case highlights the pivotal role of transcatheter interventions in treating complex CHDs and suggests a shift towards less invasive approaches in cardiac care. This case contributes valuable insights to the existing body of evidence, reinforcing the potential of transcatheter techniques to become the preferred treatment modality. With promising immediate and short-term results, these techniques highlight the need for continued research into their long-term efficacy and application across diverse patient demographics.
A Midterm Follow-Up Study of the Application of a Confluent Aortic Valve Neocuspidization Technique with Pericardium in Children.

Abstract:Background: The treatment of aortic valve diseases in children remains a great challenge. We aim to report outcomes and midterm follow-up data of our confluent neocuspidization technique with pericardium for aortic valve replacement (AVR) in children.Methods: A retrospective analysis was performed on all 20 children who underwent the confluent neocuspidization technique with pericardium at Children's Hospital of Fudan University from March 2017 to May 2022. Outcome measures included echocardiographic measurements, surgical intervention, and mortality.Results: A total of 20 patients (17 males vs. 3 females), with a median age of 7.5 years [min-max, 0.3-12 years; interquartile range (IQR), 4.4-9.7 years], a median body weight of 24.0 kg (min-max, 6.0-52.3 kg; IQR, 15.6-31.0 kg), and median aortic valve annulus size before surgery of 19.0 mm (min-max, 11.0-25.0 mm; IQR, 17.1-21.5 mm), underwent the neocuspidization technique with pericardium (17 autologous pericardia and 3 bovine patch). With 50% of bicuspid aortic valve and 50% of tricuspid, they were respectively diagnosed as aortic stenosis (AS) (7/20, 35%), aortic regurgitation (AR) (8/20, 40%) and mixed AS and AR (AS & AR) (5/20, 25%). The median postoperative follow-up time was 19 months (min-max, 5-61 months; IQR, 16.3-35 months). The peak pressure gradient across the aortic valve decreased from 81.0 +/- 37.0 mmHg in AS group and AS & AR group before surgery to 25.9 +/- 15.8 mmHg within 24 hours after surgery (P<0.001) and was mostly around 25 mmHg during follow-up. All patients presented mild or less than mild regurgitation within 24 hours after surgery. There were no hospital mortalities. Three patients needed reintervention during follow-up. There was one late death related to mitral valve stenosis.Conclusions: Though the confluent neocuspidization technique with pericardium provided immediate relief of significant AS or regurgitation, the midterm outcome was suboptimal. More research is needed to find the optimal material for AVR.
