Abstract:BackgroundSelf-efficacy has been shown to be positively correlated with self-care behaviour and glycaemic control among patients with type 2 diabetes mellitus. However, such evidence is lacking in the Malaysian primary care setting. The objectives of this study were to i) determine the levels of self-efficacy, self-care behaviour and glycaemic control among patients with type 2 diabetes mellitus in the Malaysian primary care setting ii) determine the relationship between self-efficacy, self-care behaviour and glycaemic control iii) determine the factors associated with glycaemic control.MethodsThis was a cross-sectional study involving patients with type 2 diabetes mellitus from two public primary care clinics in Malaysia. Self-efficacy and self-care behaviour levels were measured using previously translated and validated DMSES and SDSCA questionnaires in Malay versions, respectively. Glycaemic control was measured using HbA1c.ResultsA total of 340 patients with type 2 diabetes mellitus were recruited. The total mean (±SD) of self-efficacy and self-care behaviour scores were 7.33 (±2.25) and 3.76 (±1.87), respectively. A positive relationship was found between self-efficacy and self-care behaviour (r 0.538, P < 0.001). Higher self-efficacy score was shown to be correlated with lower HbA1c (r − 0.41, P < 0.001). Multiple linear regression analysis demonstrated that higher self-efficacy scores (b − 0.398; 95% CI: -0.024, − 0.014; P < 0.001), shorter duration of diabetes (b 0.177; 95% CI: 0.002, 0.007; P < 0.001) and smaller waist circumference (b 0.135; 95% CI: 0.006, 0.035; P = 0.006), were significantly associated with good glycaemic control.ConclusionThis study demonstrated that higher self-efficacy was correlated with improved self-care behaviour and better glycaemic control. Findings of this study suggest the importance of including routine use of self-efficacy measures in the management of type 2 diabetes mellitus in primary care.

Abstract:ABSTRACT Background In Indonesia, the number of Type 2 diabetes cases is increasing rapidly, making it the third leading cause of death and among the leading noncommunicable disease healthcare expenditures in the country. Thus, there is a critical need for Indonesians with Type 2 diabetes to perform better self-care to optimize their health and prevent the onset of comorbidities. Purpose This study was designed to investigate the influence of knowledge, depression, and perceived barriers on Type 2 diabetes self-care performance in Indonesia. Methods A cross-sectional study was conducted on 185 patients with Type 2 diabetes, with demographic, diabetes history, obesity status, diabetes knowledge, depression, perceived barriers, and self-care performance data collected. The Indonesian version of the Revised Diabetes Knowledge Test, Depression Anxiety Stress Scale, Perceived Barrier Questionnaire and Self-Care Inventory-Revised were used. Descriptive, bivariate, and multiple linear regression analyses were performed. Results Study participants were found to have moderate diabetes self-care performance scores. Annual eye checks, blood glucose self-monitoring, healthy diet selection, and regular exercise were the least common self-management techniques performed and were consistent with the perceived difficulties of the participants. Being illiterate or having an elementary school education (β = 4.59, p = .002), having a junior or senior high school education (β = 3.01, p = .006), having moderate depression (β = −0.92, p = .04), diabetes knowledge (β = 0.09, p = .006), and perceived barriers (β = 0.31, p < .001) were found to explain 40% of the variance in self-care performance. Educational level, depression, and perceived barriers were the strongest factors that impacted Type 2 diabetes self-care performance in this study. Conclusions/Implications for Practice Nurses should not only provide diabetes education but also identify barriers to diabetes self-care early, screen for the signs and symptoms of depression, and target patients with lower levels of education.

Abstract:<span><br/><b>Purpose:</b> The purpose of this study is to find out the pattern and factors associated with self-care practices among diabetic patients as self-care was considered imperative for the control of disease and enhancement of patient's quality of life. <b>Methods:</b> This is a cross-sectional clinic-based observational study conducted among the type 2 diabetes mellitus patients who came for follow up to the outpatient department of a tertiary hospital of Kolkata from September to December 2019. Self-care practice assessed using questions adapted from the "Summary Diabetes Self-Care Activities Questionnaire" by face-to-face interview. Logistic regression used to find out the predictors of self-care practice. <b>Results:</b> Self-care practice was unsatisfactory among 67.5% of the patients. Near half (56%) of the patients had mild to moderate distress and about 18% were severely distressed. Self-care practice was significantly associated with the educational level, family history, presence of hypertension, advice given regarding self-care activities, and diabetes distress levels of the participants. <b>Conclusions:</b> Primary care physicians should be encouraged to give practical aspects of self-care practices and counseling regarding diabetes distress. To perform self-care practices adequate, support from the family level and community level is imperative.<br/></span>
Relationship Between Self-Care Behavior and Cognitive Function in Hospitalized Adult Patients with Type 2 Diabetes: A Cross-Sectional Study

Abstract:Purpose: To investigate the relationship between diabetes self-care behavior and cognitive function of hospitalized young and middle-aged Chinese patients with type 2 diabetes mellitus (T2DM). Patients and Methods: In this cross-sectional study, young and middle-aged T2DM patients (age range, 35-65 years) were recruited at 4 tertiary hospitals between July 2016 and January 2017. Data pertaining to self-care behavior and cognitive function were collected using two questionnaires (the Summary of Diabetes Self-care Activities [SDSCA] and the Montreal Cognitive Assessment [MoCA], respectively). Multivariate linear regression analysis was performed to assess the correlation between cognitive function and self-care activities. Results: A total of 140 patients with diabetes were enrolled (mean age, 53.79 +/- 7.96 years). The mean duration of T2DM was 10.83 +/- 6.76 years. Regarding SDSCA performance, the mean scores for foot care and blood glucose monitoring were 2.20 +/- 2.57 and 1.98 +/- 2.45, respectively, which were the worst; scores for exercise (4.01 +/- 2.58) and diet (3.16 +/- 1.89) were better, while scores for medication administration (5.26 +/- 2.79) were the best. The prevalence of cognitive impairment was 37.9% (53 patients). After variables adjustment, delayed recall showed a significant correlation with blood sugar monitoring behavior (B =0.224, P=0.019); visual space and executive function (B=0.255, P=0.009) and abstraction (B=-0.337, P=0.001) showed a correlation with foot care behavior. Conclusion: Cognitive ability affects the self-care behavior of patients with T2DM. Assessment of cognitive function may help inform patient education interventions to improve the self-care behavior of these patients.

Abstract:Introduction: Diabetes mellitus (DM) is a main, highly prevalent, and challenging public health issue. Suboptimal self-care for type II diabetes can lead to poor glycemic control, complications, and even death. Objective: This study investigated the incidence of distress and its link with self-care habits of patients with diabetes type II. Methods: A correlational, cross-sectional design with a convenient sample of 200 patients was used to conduct this study. Three questionnaires were administered: (A) the demographic and medical data questionnaire; (B) diabetes self-care activities in brief; and (C) the diabetes distress scale in Arabic language. Results: The patients' mean age was 51.78 ± 11.34; 80% of patients practiced lower levels of diabetes self-care, and 37% of them had a high level of diabetes distress. Self-care is associated with diabetes distress (R = -0.152, p-value = .032). Conclusion: Self-care activities can help in the early detection and management of diabetes distress. Sustained self-care education is promising to minimize diabetes distress. The potential advantages of association between diabetes distress and self-care can offer self-care programs that enhance diabetes distress management.

Abstract:Objective To study the self-care level of patients with type 2 diabetes and discuss its related factors.Methods 198 outpatients with diabetes mellitus were evaluated by the self-care measuring scale and basic information questionnaire.Results The self-care scores in patients with type 2 diabetes were significantly low.The self-care level in these patients were correlated with age,vocation,education degree,duration of illness,presence or absence of health-education and economic status.Conclusion Diabetic patients had low self-care level,which is affected by a wide range of influential factors.While patients are receiving medical treatment,nurses should actively develop self-care education,raise their subjective motility,make them assume the responsibility of self-care so as to raise therapeutic efficacy.

Abstract:BACKGROUND: Self-care for diabetes is very important in preventing complications of diabetes and also improving quality of life. This study aims to find the various self-care practices of type 2 diabetes patients who are being treated at a rural health center of a medical college and the factors influencing self-care.MATERIALS AND METHODS: This is a hospital-based cross-sectional study. Type 2 diabetes patients registered and receiving treatment from the noncommunicable disease clinic of a rural health center were administered a pretested questionnaire, developed based on different diabetes self-care questionnaires making changes as appropriate and including most aspects of self-care practices. The proportion of compliant was then cross-tabulated with background characteristics and their glycated hemoglobin (HbA1c) levels. SPSS version 16.0 was used for the analysis.RESULTS: Among 390 patients with a mean age 56 years, 25.5% adhered to at least four dietary modifications, 46% were physically active, and 57% had good compliance to drugs; hypoglycemia prevention practices ranged from 21% to 51%. Except for avoiding barefoot walking (90%), other foot-care practices were followed by only a quarter of them. Among ever users, 69.2%, 64.3%, and 29.4% have quit smoking, alcohol, and tobacco use, respectively. Adherence to dietary modifications and drug compliance were associated with a lower HbA1c level of ≤7 g% (P < 0.05).CONCLUSION: In this study, the compliance to physical activity or medications is much better than dietary changes and foot-care practices. Focused education programs and monitoring during follow-up visits will improve self-care in the less adhered to aspects. Those who are adherent to dietary modifications and drugs have better glycemic control.

Abstract:Context: Diabetes has emerged as a major chronic public health problem throughout the world. Self-management by diabetes patients is very important for controlling blood sugar levels and preventing complications of diabetes. Aims: The present study was conducted to study self-management practices among diabetes patients and to analyse socio-demographic factors associated with them. Methods and materials: A cross-sectional analytical study was conducted among 230 randomly selected diabetes patients in the Andaman and Nicobar Islands. The self-management practices were measured by the diabetes self-management questionnaire (DSMQ). Statistical analysis used: The association of the DSMQ score with socio-demographic factors and blood sugar level was analysed by statistical tests like the T-test, analysis of variance (ANOVA) test, Tukey's honestly significant difference, and Chi-square test. Results: The mean DSMQ score of the diabetes patients was 29.55 ± 5.98. There was a significant difference between the mean DSMQ score and the educational level (P value = 0.009), residential status (P value = 0.037), and duration of diabetes (P value = 0.006). The subcomponent analysis of the DSMQ score revealed that the glucose management score of rural people (9.38 ± 3.36) was significantly higher (P value = 0.006) than that of urban people (8.32 ± 2.46), and the diet control score was significantly higher (P value = 0.02) in patients with normal post-prandial blood sugar (PPBS) (7.64 ± 2.18) than in patients with raised PPBS (6.96 ± 2.12). Conclusions: Higher educational level, rural background, and long duration of diabetes were associated with better self-management practices. The patients with normal blood sugar levels showed higher diet control scores than the patients with raised blood sugar levels.

Abstract:Franck and Hetrz in 1914 were the first to demonstrate that free electrons can be decelerated by mercury atoms in discerete energy quanta. In 1930 Latyscheff and Leipunksy have demonstrated the inverse effect namely, free electrons can be accelerated by energy stored in the mercury atoms (collision of the second kind). It was only in 1958 that Schawlow and Townes have used multiple collisions between photons and excited atoms to amplify radiation (MASER and LASER). In 1995 Schachter suggested to use excited atoms for coherently accelerate particles. The results of a proof-of-principle experiment (2006) demonstrating the PASER scheme are reported here. Performed at the BNL-ATF, the essence of the experiment is to inject a 45 MeV density modulated electrons' beam, bunched by its interaction with a high- power CO2 laser pulse within a wiggler, into CO2 excited molecules cell. The electrons experienced 0.15% relative change in their kinetic energy, in less than 40 cm long interaction region. The experimental results indicate that a fraction of these electrons have gained 200 keV each, implying that such an electron has undergone two-million collisions of the second kind. Hence, this is the first experimental demonstration of coherent collisions of the second kind.

Abstract:Background: Diabetes mellitus (DM) is a major health problem in the world. DM patients should be able to perform self-care behavior. Diabetic self-care behavior is an effective strategy for controlling diabetes.Objective: The purpose of this study was to examine a health promotion model in patients with type 2 diabetes mellitus.Methods: A cross-sectional design was used, which involved 177 patients with type 2 diabetes from primary health care in Denpasar Bali Indonesia with a stratified random sampling. Independent variables were personal factors (age, sex, education, body mass index, socioeconomic status, self-motivation, illness perception, and belief), self-efficacy, and family support. The dependent variable was self-care behavior. A structural equation model was used to confirm the hypothesis model.Results: Personal factors (education, socioeconomic, and perceptions) (t = 2.891) and family support (t = 5.746) were associated with self-care behavior. Self-efficacy did not affect self-care behavior (t = .139).Conclusion: Diabetes self-care behavior is influenced by socioeconomic status, level of education, perception of the illness, and family support. Therefore, it is suggested for nurses to apply the health promotion model approach to increase self-care behavior of patients with type 2 diabetes mellitus.

Abstract:This study examined the level of adherence to self-care behaviors among individuals with type 2 diabetes in Saudi Arabia and its connection with depression and demographic factors. A cross-sectional survey was conducted among diabetes patients using the Patient Health Questionnaire (PHQ-9) to measure depression and the Summary of Diabetes Self-Care Activities (SDSCA) to evaluate diabetes self-care activities. Among the 252 participants who completed the survey, 43.2% were older than 55 and 59% were men. The ordinal regression model showed an association between the PHQ-9 and SDSCA scores with an OR of 0.83 (95% CI: 0.71 to 0.96, p = 0.013). The PHQ-9 score was significantly associated with blood sugar monitoring (OR: 0.90 [95% CI: 0.82 to 0.99, p = 0.003]), exercise (OR: 0.88 [95% CI: 0.79 to 0.98, p = 0.002]), and diet (OR: 0.94 [95% CI: 0.85 to 1.03, p = 0.045]). Of all the diabetes-related factors, only a history of hospitalization and receiving diabetes education were found to be associated with improved self-care behaviors. In conclusion, a negative association was found between PHQ-9 scores and the SDSCA mean score and most daily diabetic self-care behavior components.

Abstract:Abstract Introduction: The study objective was to determine the levels of self-care and health literacy (HL) and their associations among patients with chronic kidney disease (CKD). Methods: This was a cross-sectional, questionnaire-based study conducted in a public primary care setting in Singapore. A total of 289 participants aged 21–80 years with hypertension were recruited. Self-care profiles were measured using the Hypertension Self-Care Profile (HTN-SCP; range 0–240, domain range 0–80). Health literacy was measured using the Short-Form Health Literacy Scale (HLS-SF12; range 0–50, limited literacy ≤33). Results: The mean self-care score was 182.7 (standard deviation [SD] 23.2). The median HL score was 34.7 (interquartile range [IQR] 31.9–40.3), and 31.1% of participants had limited HL. Self-care was not associated with age, CKD status, household income and education, but was associated with gender and HL score. In the final regression model, lower HL scores (adjusted β = 1.03, 95% confidence interval [CI] 0.7 to 1.36, P < 0.001) and male gender (adjusted β = −5.29, 95% CI −10.56 to −0.03, P = 0.049) were associated with lower self-care scores. The HL scores were associated with self-care domains of self-efficacy (HL: β = 0.30, 95% CI 0.17 to 0.42, P < 0.001), motivation (HL: β = 0.40, 95% CI 0.26 to 0.53, P < 0.001) and behaviour (HL: β = 0.38, 95% CI 0.26 to 0.50, P < 0.001). Conclusion: Thirty-one percent of the participants had limited HL. Self-care was not associated with age, race, CKD status, household income or education. Male gender and limited HL were associated with lower self-care. Self-care was associated with self-efficacy, motivation and behaviour. Future research could focus on more targeted approaches to improve self-care and HL among patients with CKD.

Abstract:Purpose: The purpose of this study was to examine the relationship between self-management behaviors (eg, healthy eating, being active, medication taking, glucose monitoring, feet check), sociodemographic factors, disease-related characteristics, and health literacy among patients with type 2 diabetes in Singapore. Methods: Data were analyzed from a nationwide survey conducted between 2019 and 2020 (n = 387). Self-management behaviors were assessed using the Dietary Approaches to Stop Hypertension questionnaire, the Global Physical Activity Questionnaire, and a diabetes care questionnaire. A linear regression model was generated to examine the association of healthy eating with the variables of interest (sociodemographic factors, disease-related characteristics, and health literacy), and logistic regression models were generated to investigate the significant correlates of the remaining self-care behaviors. Results: Regression models showed that the 5 self-care behaviors have different correlates. Nonetheless, compared to individuals aged 50 to 64 years, those aged 65 years and above were less likely to be active, adhere to their medication prescription, and check their feet. Individuals with a higher number of diabetes-related complications were less likely to be sufficiently active but more likely to monitor their glucose level and check their feet. Moreover, individuals with poor health literacy were more likely to eat healthily and be sufficiently active. Conclusions: Programs related to self-care behaviors can be tailored to specific demographics to improve their uptake in the population. Furthermore, encouraging comprehensive self-care behaviors in those aged 65 years and above is crucial for effective diabetes management.

Abstract:<h3>Introduction</h3><p>Diabetes mellitus is a global public health emergency in the twenty-first century. Diabetes patients who had to adhere to good self-care recommendation can prevent the complication associated with diabetes mellitus. Self-care management of diabetes mellitus in Sub-Saharan Africa was poor including Ethiopia. The aim of this study was to assess factors influencing diabetes self-care practice among type 2 diabetes patients at Debre Tabor General Hospital, Northwest Ethiopia diabetes clinic follow up unit.</p><h3>Methods</h3><p>An institutional based cross-sectional survey was conducted on systematically sampled 405 type 2 diabetes patients at Debre Tabor General Hospital diabetes clinic from June 02/2018 to June 30/2018. Bivariate and multivariable logistic regression was fitted to identify independent predictors of diabetes self-care practice. A <em>p</em> value of less than 0.05 was used to declare statistical significance.</p><h3>Results</h3><p>A total of 385 type 2 diabetes patients participated with a response rate of 95%, of which 243 (63.1%) study participants had good self-care practice. The mean ± SD age of the respondents and the duration of diagnosed for diabetes mellitus was 52.28 ± 12.45 and 5.09 ± 3.80 years respectively. Type 2 diabetes patients who had a glucometer at home (AOR = 7.82 CI (3.24, 18.87)), getting a diabetes education (AOR = 2.65 CI (1.44, 4.89)), and having social support (AOR = 2.72 CI (1.66, 4.47)) were statistically associated with good self-care practice.</p><h3>Conclusion</h3><p>Despite, the importance of diabetes self-care practice for the management of diabetes and preventing its complications, a significant number of type 2 diabetes patients had poor diabetes self-care practice. So, to enhance this poor practice of diabetes self-care, provision of diabetes self-care education and counseling on self-monitoring blood glucose should be promote by health care providers during their follow up.</p>

Levels of compliance of self-care practices of diabetes mellitus type 2 patients: a study from a tertiary care hospital of North India

Abstract:Diabetes management strategies are interdependent and comprise of three basic key elements: self-care activities, effective drug treatment, and adequate follow-up for early detection of complications. There is a need to assess the levels of self-care practices among diabetic patients and factors influencing them for developing better educational strategies to address the gaps identified. A cross-sectional study was conducted to assess the compliance for self-care activities of type 2 diabetes mellitus patients, in a tertiary care hospital by using the Summary of Diabetes Self-Care Activities (SDSCA) scale among 60 patients aged > 18 years. More than 90% of respondents were compliant for not eating high-fat foods, were not smoking, and were monitoring glucose regularly. One third of respondents were not taking proper foot care. After adjusting for confounders, the proportion of individuals who eat five or more servings of fruits and vegetables were significantly lower in middle and low socioeconomic status (SES) individuals as compared to high SES (middle SES OR 0.06, 95% CI 0.01–0.39; lower SES OR 0.06, 95% CI 0.01–0.60). Females were less likely to be involved in physical activity-related self-care activities as compared to males (minimum 30 min of physical activity: OR 0.27, 95% CI 0.08–0.92; specific exercise sessions: OR 0.15, 95% CI 0.04–0.52). The proportion of diabetics who follow proper foot care instructions were significantly lower among middle SES as compared to upper SES (checking of feet: OR 0.08, 95% CI 0.01–0.47; inspection of footwear: OR 0.08, 95% CI 0.01–0.48). There is a need to emphasize the importance of diet, physical activity, and foot care during the counseling sessions of diabetes mellitus type 2 patients.

Abstract:Objective To investigate the level of self-care behaviors and self-efficacy among adult type 2 diabetes patients in community and discuss current problems.Methods A convenient sample of 85 patients with type 2 diabetes in Haiyuncang community was interviewed,using diabetes self-care scale and diabetes self-efficacy scale.Totally 85 questionnaires were got and underwent analysis.Results The index score of self-care behaviors and self-efficacy among adult type 2 diabetes patients in community were 66.98% and 69.02%.There were no significant difference in self-care behaviors of patients whether they attended diabetes lectures or not.Self-efficacy correlated significantly with self-care behaviors.Conclusions The self-care behaviors and self-efficacy of diabetes patients need to be improved,and diabetes education in community should be emphasized and carried out effectively.

Abstract:Diabetes mellitus is a leading cause of disability and a major contributor to healthcare costs in most countries. Patient education to facilitate diabetes self-care is a cornerstone of the quality of diabetes care. Adherence to diabetes dietary self-care helps achieve a sense of general well-being and minimize diabetes complications. Patients with type 2 diabetes were surveyed to evaluate demographic, psychological, and environmental factors affecting perceived six dietary self-care behaviors to their prescribed regimens. A cross-sectional evaluation was performed in adult outpatients aged 40 years and older with type 2 diabetes who had received at least one individualized dietitian-led nutrition session. Adherence to six dietary self-care behaviors was assessed by using the Diabetes Self-Care Behavior (DSCB) questionnaire and the influencing factors of each behavior were investigated by using the Factors Affecting Diabetes Self-Care (FADSC) questionnaire. Odd ratios derived from logistic regression analysis were used to identify predictive factors resulting in adherence to dietary self-care behaviors and related factors. A total of 200 patients were completed in this study and the average age was 60.2±12.1 years old. We found that women were more likely to follow reducing fat intake (237 %) and eating more fiber foods (192 %). Patients who received more than 4 nutrition education sessions had better adherence to four different dietary self-care behaviors. Patients with more family support were more likely to have lower fat and more fiber foods. Patients with more self-efficacy showed following meal plans better. Patients’ adherence to six dietary self-care behaviors prescribed for diabetes varied. Many factors are important for supporting adherence to dietary self-care behaviors. The need for individualization of patient characteristics in diabetes nutrition education is the key point to adherence to recommended dietary self-care behaviors. Disclosure C. Ouyang: None.

Abstract:Background: Diabetes is a chronic, progressive disease characterized by elevated levels of blood glucose and is recognized as an important cause of premature mortality and morbidity. Self-care practice in diabetes patient is a critical factor to achieve glycaemic control thus in preventing or delaying its complications.Methods: A cross sectional study was conducted among 140 type 2 diabetic patients attending the outpatient department in Karnataka Institute of Medical Sciences, Hubballi during the period of June-July 2018. Diabetic patients diagnosed more than 3 months were included. Data was collected by interview method using pre-designed, pre-tested and semi-structured questionnaire which consists of information on socio-demographic data, diabetic profile, self-care practices and dietary barriers. Statistical analysis was done using SSPS package.Results: This study showed that 67.9% of study participants were consuming diabetic diet, 17.1% practice exercise for more than 3 days a week, 93.6% were taking medication regularly, 15% practised foot care for >3times a week and 89.3% monitored their blood glucose regularly. The main barriers for dietary practices were lack of knowledge (24%) and lack of motivation (18%). Compliance to diabetic diet was more among the urban population compared to rural (p=0.025).Conclusions: Study concludes that the practice of self-care activities was poor in almost all aspects except for blood sugar monitoring and adherence to medication which necessitates the need to create awareness regarding self-care practices and its importance among diabetic patients.

Abstract:Single-molecule spectroscopies in combination with single-channel patch-clamp measurements have the potential for providing new information on ion channel gating processes. Fluorescent gramicidin derivatives could provide a means for calibrating such experiments since the structure of the open channel is known and the mechanism of gating (peptide dimerization) is generally agreed. We describe here the synthesis and characterization of two pairs of gramicidin derivatives that should prove useful for such studies. They contain robust fluorophores, undergo resonance energy transfer (FRET) when they dimerize, and have single-channel properties close to those of the wild-type channel.
