Abstract:Background: We aimed to determine the mortality rate and Years of Life Lost (YLL) due to lung cancer in Fars, Iran, during the period from 2004 to 2019. Methods: All deaths due to lung cancer in Fars Province, Iran from the electronic population-based death registration system (EDRS) were obtained. Crude mortality rate, age-standardized mortality rate, YLL and YLL rate data were calculated and trends examined. Results: During 2004-2019, 3346 deaths occurred due to lung cancer in Fars Province, which was 10.8% (3346/30936) of the total cancer deaths in this period. Crude mortality rate of lung cancer had increased 70% and 53% in male and female respectively from 2004 to 2019. The total YLL of lung cancer during the 16-year study period was 28,094 (0.9 per 1000) in men, 14,174 (0.5 per 1000) in women, and 42,268 (0.8 per 1000) in both sexes (sex ratio male/female=2). According to the join point regression, the 16- year trend of YLL rate due to premature mortality was increasing: APC was 2.5% (95% CI 0.9 to 4.2, P=0.005) for males, 1.4% (95% CI 0.3 to 2.6, P=0.017) for females. Conclusion: The mortality rate and YLL due to lung cancer in Fars Province is increasing, although the standardized mortality has a constant trend. Tobacco control is the most effective and least expensive way to reduce the number of lung cancer patients worldwide. National and local media can also play an important role in informing people about the risk factors.

Spatial Variation in Sympathetic Influences on the Vasculature of the Synovium and Medial Collateral Ligament of the Rabbit Knee Joint

Abstract:1 Laser Doppler perfusion imaging was used to assess the role of the sympathetic nervous system in the control of blood flow to the medial collateral ligament and capsule (synovium and overlying fibrous tissues) of the rabbit knee joint. 2 Electrical stimulation of the saphenous nerve (width 1 ms; amplitude 20V; 1–30 Hz) produced a frequency‐dependent vasoconstriction of knee joint vasculature. The response was maximal at 30 Hz and gave the greatest fall in perfusion at the femoral insertion of the ligament (by 33.8 ± 7.4%, mean ±s.e.m.; n= 5–6) and the smallest decrease at the tibial insertion of the ligament (by 10.6 ± 2.9%). 3 Topical application of phentolamine (10−6 mol) had no significant effect on basal knee joint blood flow. However, it abolished the nerve‐mediated constrictor responses in all regions of the medial collateral ligament and synovium at all frequencies. 4 Topical administration of adrenaline (10−14 to 10−7 mol) caused a dose‐dependent decrease in knee joint blood flow with the highest dose producing > 75% reduction in perfusion at all areas. 5 There was no evidence of a reactive hyperaemia in the 5 min following a 5 min period of femoral artery occlusion. Artificial manipulation of arterial blood pressure by intravenous infusion or withdrawal of blood caused a proportional change in ligament and synovial blood flow. These observations may indicate a lack of autoregulation in the joint and its exclusion from baroreflex modulation. 6 These results suggest a potential role for the sympathetic nervous system in the control of knee joint blood flow. Neuromodulation of ligament perfusion appears to predominate at the femoral insertion and this could prove to have functional significance.
Fear of falling and its related factors in older adults following a fall in Kashan, Iran (2023-2024)

Abstract:Background: Falling is a significant challenge in old age, often leading to isolation, self-imposed limitation, reduced movement, and muscle strength. This study aimed to explore the fear of falling and its related factors in older adults following a fall in Kashan, Iran. Methods: This cross-sectional study followed 200 older adults who visited medical centers in Kashan from July 2023 to May 2024. Continuous sampling was carried out. Data collection involved a background information questionnaire, the shortened version of the falls efficacy scale-international in older adults, the independence scale of activities of daily living, and the short-form state-trait anxiety inventory. Data were gathered before, one month, and three months after the fall. Data analysis utilized SPSS-24, including t-test, ANOVA, Pearson's correlation coefficient, and repeated measures analysis of variance. Results: Participants had a mean age of 71.34 ± 8.36 years (ranging from 65 to 96 years). There was a significant increase in fear of falling scores one month and three months after the fall (P < 0.001). Multiple linear regression revealed that factors such as illiteracy, old age, and previous falls were predictors of fear of falling before the fall (P < 0.05). One month after the fall, predictors included illiteracy, hip fracture, and high anxiety scores (P < 0.05). Three months after the fall, predictors encompassed illiteracy, hip fracture, high anxiety score, and internal fixator placement as a treatment intervention (P < 0.05), explaining 15% of the variance in fear of falling. Conclusions: Identifying predictive factors for fear of falling can assist health policymakers in developing a holistic care plan to enhance the quality of life for older adults post-fall. It is essential to screen for fear of falling levels, particularly after trauma, offer educational counseling services, particularly in mental health, after discharge, and prepare training programs related to fall prevention.

Abstract:Background: Gastrointestinal cancers can cause major health problems globally since their burden is increasing in many countries. We aimed to investigate the trend of years of life lost due to gastrointestinal cancers in Fars Province, southern Iran during the 2004-2019. Methods: In this cross-sectional survey study, we obtained the information regarding all deaths due to gastrointestinal cancers in Fars Province from the electronic population-based death registration system (EDRS). Years of Life Lost (YLL) was calculated using the YLL template of 2015 by the WHO. To examine the trend for different years, join point regression based on the log-linear model was used. Joinpoint regression analysis describes changing trends over successive periods of time and the increasing or decreasing rate within each period. Results: During the years of 2004-2019, 9742 deaths due to gastrointestinal cancers occurred in Fars Province. 6013 (61.72%) cases were male and 3729 (38.28%) cases were female (Male / Female Sex Ratio: 1.61). Overall, 4152 cases (42.63%) were due to gastric cancer and 2112 cases (21.68%) were due to liver cancer. Total years of life lost due to premature death from gastrointestinal cancers during the 16-year study period was 73565 yr (2.33 per 1000 persons) in men, 52766 yr (1.71 per 1000 persons) in women, and 126331 yr (2.02 per 1000 persons) in both sexes. Conclusion: Among all cancers, the highest mortality rates in both sexes belong to gastric cancer. This study showed the trend of YLL rate of malignant neoplasms of liver and intrahepatic bile ducts, esophagus, oral cavity, and colon cancer were increasing in both sexes, however, the trend of YLL rate for malignant neoplasms of the small intestine was decreasing in both sexes. Variation of GI cancers patterns and trends around the Fars Province indicated that a more comprehensive control plan is needed to control these variations.

A decadal analysis of road traffic accident-related mortality among women of reproductive age: A cross-sectional study in East Azerbaijan Province, Iran

Abstract:Background: Given the extensive impact of road traffic accidents (RTAs) consequences and their potential ramifications on the health of both current and future generations, this study examines the social and demographic factors that influence RTA-related mortality among women of reproductive age. Methods: The study population consisted of cases retrieved from the database of the Legal Medicine Organization, encompassing all women aged 15-49 who succumbed to road accidents between 2011 and 2021. Results: The mean age of women in the reproductive age group from East Azerbaijan province between 2011 and 2021 was 33.67 years, with a standard deviation of 9.18. RTAs on main roads accounted for the majority of incidents (395 cases, 50.8%), with 93.7% (728 cases) attributed to road traffic. In 54.4% of these cases, the affected organ was the head and neck, and the primary cause of death in 52.1% was head trauma. Across all age groups, injuries to the neck and head were the most common, followed by injuries to the abdomen, chest, back, and sternum. Conclusion: The higher incidence of road accidents in the 25-29 age group highlights the need for targeted interventions to address risky behaviors, inexperience, and peer influences in this demographic. Our observation of passengers experiencing the highest mortality rate emphasizes the vulnerability of road users, particularly pedestrians, in traffic accidents. Pedestrian violations in the 17-30 age group further emphasize the importance of education and awareness campaigns aimed at reducing risky behavior.

Abstract:INTRODUCTION: Falls and fear of falling are considered as the major factors affecting the elderly's disabilities, so that most of these individuals often find their homes as a safe environment.AIM: The aim of this study was to evaluate the relationship between home safety and prevalence of falls and fear of falling among older adults.MATERIALS AND METHODS: This cross-sectional study was conducted on 450 elderly people who lived in Bojnrud, Northeast of Iran, from December 2016 to July 2017 using cluster sampling method. A demographic characteristics questionnaire, Fall Efficiency Scale-International (FES-I)questionnaire, and Home Safety Checklist were employed as research instruments.RESULTS: Out of the study population, 157 individuals (35.7%) had a history of falls in the past one year. The mean score for fear of falling in the elderly people examined was 29.14±11.07 and the same value for home safety status was equal to 11.31±4.17. The mean score for fear of falling was significantly correlated with history of falls in older adults (p<0.0001). There was also a statistically significant relationship between home safety status and prevalence of falls and fear of falling in the elderly (p<0.0001). Besides, the results of logistic regression analysis showed that fear of falling could be estimated by 29-51%, taking the history of falls in the last 12 months and home safety mean score into account (p<0.0001).CONCLUSION: Home safety status and demographic variables could have effects on falls and fear of falling in elderly individuals. Therefore, putting these factors together, older adults at the risk of further falls can be identified and provided with trainings through planning and appropriate interventions to prevent the incidence of falls and their negative consequences among them.
The Trend of Death Rate and Causes of Death Based on the ICD-10 among Young People in Iran within the Last Three Decades

Abstract:Background: Youth is undeniably the most important resource of any country. This study aimed to determine the trend of mortality rate and the cause of death, based on the ICD-10, among young people in Iran and examines significance of the changes in any cause during the last three decades. Methods: The present study is a secondary analysis of data, which examines the trend of death rate and its causes among young adult, aged 15-24 yr, longitudinally during 1990-2019. The data source was the GBD website. Linear Regression analysis was used to measure the slope of changes in mortality rates and causes of death during the period, where "time" was the independent variable and "mortality rate" and "causes of mortality" were dependent variables. Results: The death rate of young people declined by 56% during the period, equal to an average of 2.17 units per year. The most common cause of death has been injuries (69%), then NCDs (25%), and finally communicable diseases (6%). However, death due to injuries (except SUD), communicable diseases (except HIV), and NCDs (except musculoskeletal disorders) declined significantly by 1.43, 0.3, and 0.09 units per year, over 30 yr respectively. Conclusion: Examining past trends in death rates and causes strengthens insights into the state and future trends in health and death-related indicators, which are crucial for policy-making, especially in developing countries with limited resources.

Epidemiological trend of suicide in north of Iran from 2011 to 2018 with a focus on joinpoint regression

Abstract:Background: Deaths due to potential suicide are preventable and this phenomenon is costly for the health care system and contradicts the values and cultural standards of an Islamic country. Methods: This study is a retrospective study. The research population includes all cases of suicide during the years 2011-2018 who were referred to the emergency department of Babol hospitals. The results were analyzed using SPSS v.23 and Joinpoint Trend Analysis software 4.9.0.0 to identify significant changes in the temporal trends of the outbreak. Results: The highest percentage of suicides occurred in summer (27.8%), on Saturdays (13%) and at night (53%). A total of 1.9% of the cases were commit suicides (leading to death). The highest frequency of suicide was shown in 1397 (21.2%) and the lowest in 1392 (5.1%) and was more common in women (68.2% vs. 31.8%). Suicide-related deaths was higher in the second four years (63.5%), but suicide rate was significantly higher in the first four years (2011-2014) and the mortality rate due to suicide was higher in men than women. Conclusion: Suicide attempts were higher in women than men but the death rate was higher in men which means that men attempt suicide more seriously than women. The model also predicted that suicide rates would rise in the coming years. For this reason, this important issue, along with a detailed analysis of the roots of suicidal ideation and preventive measures, should be considered by health officials and social institutions.

Abstract:Global Health Promotion, Ahead of Print. Fall-related injuries are the leading cause of injury, death and disability among older adults, and their health care costs are high. Therefore, the present study aimed to determine the effect of an educational intervention, based on the health belief model, on the prevention and fear of falling among older people. We conducted this experimental study on 140 older people in Fasa city, Fars Province, Iran, in 2023. We selected the subjects using a simple sampling method. The educational intervention for the experimental group included three 30-min sessions. We analyzed the data using SPSS 22 software, employing chi-square, independent t-tests and paired t-tests. The results showed that before the educational intervention, there was no significant difference between the experimental and control groups in terms of knowledge, perceived sensitivity, perceived intensity, perceived benefits, perceived barriers, self-efficacy, cues to action, and performance; however, three months after the educational intervention, the experimental group showed a significant increase in each of the mentioned variables except the perceived barriers. Additionally, after the educational intervention, the mean score of fear of falling in the experimental group showed a statistically significant decrease. This study demonstrated the effectiveness of an educational intervention based on the health belief model in preventing falls and reducing the fear of falls in older people. Therefore, we suggest using training based on this model to prevent and reduce falls among older people.

Abstract:Suicide is a major public health challenge worldwide with an increasing trend. Identifying risk factors for suicide attempts and suicide deaths may help find useful ways to prevent suicide. We aimed to determine the trend and related factors of suicide attempts and suicide deaths in Rafsanjan. This retrospective study included all suicide cases from 2018 to 2022 in Rafsanjan, a city in the southeast of Iran. The information was extracted from the suicide registration system of Rafsanjan University of Medical Sciences. Univariable and multivariable logistics regression models were used to investigate factors affecting suicide death. A total of 2039 cases of suicide, including 1932 cases (94.75%) of suicide attempts and 107 cases (5.25%) of suicide deaths were recorded during the study period. The frequency of suicide deaths and suicide attempts per 100,000 people increased in 2022 compared to 2018 in both genders. In the adjusted model, the odds of suicide deaths in males was 6.48 (95% CI 3.39–12.42) times higher than in females. Also, the odds of suicide deaths in unemployed subjects and housewives were 2.64 (95% CI 1.50–4.67) and 7.45 (95% CI 3.08–18.07) times higher than employed subjects respectively. Finally, people with education less than a diploma had 10.85 (95% CI 1.48–79.54) times higher odds of suicide deaths compared to people with university education. The present research showed that the pattern of suicide has been increasing since 2018, and we may see an upward trend in the coming years, which requires further investigation and preventive measures. Male gender, low education level, unemployment, and being a housewife were associated with the highest frequency of suicide death.

Long-Term Trends in Unintentional Fall Mortality in China: A Population-Based Age-Period-Cohort Study

Abstract:Background: Unintentional falls seriously threaten the life and health of people in China. This study aimed to assess the long-term trends of mortality from unintentional falls in China and to examine the age-, period-, and cohort-specific effects behind them. Methods: This population-based multiyear cross-sectional study of Chinese people aged 0–84 years was a secondary analysis of the mortality data of fall injuries from 1990 to 2019, derived from the Global Burden of Disease Study 2019. Age-standardized mortality rates of unintentional falls by year, sex, and age group were used as the main outcomes and were analyzed within the age-period-cohort framework. Results: Although the crude mortality rates of unintentional falls for men and women showed a significant upward trend, the age-standardized mortality rates for both sexes only increased slightly. The net drift of unintentional fall mortality was 0.13% (95% CI, −0.04 to 0.3%) per year for men and −0.71% (95% CI, −0.96 to −0.46%) per year for women. The local drift values for both sexes increased with age group. Significant age, cohort, and period effects were found behind the mortality trends of the unintentional falls for both sexes in China. Conclusions: Unintentional falls are still a major public health problem that disproportionately threatens the lives of men and women in China. Efforts should be put in place urgently to prevent the growing number of fall-related mortality for men over 40 years old and women over 70 years old. Gains observed in the recent period, relative risks (RRs), and cohort RRs may be related to improved healthcare and better education.

Abstract:Background: While death in old age is inevitable, premature death at younger ages is within our control. Premature mortality (death < 70 years) is a crucial indicator of health status and access to healthcare, with variations observed across regions. In North Africa and the Middle East, ischemic heart disease (IHD), road injuries, stroke, and chronic kidney disease are projected to be the main causes of premature mortality. Unfortunately, few studies have been conducted on premature mortality worldwide. This study aimed to analyze the causes of premature death and associated risk factors within the Pars Cohort Study. Methods: The Pars Cohort Study is a prospective cohort study conducted in Fars Province, Iran, involving 9,264 individuals aged 40-75 years, 53.8% of whom were women. We assessed participants from baseline (2012-2014) to 2021. The data were gathered through interviews, biological samples, and physical examinations. The causes of premature mortality, hazard ratios (HRs), and population attributable fraction (PAF) with 95% confidence intervals (95% CIs) for the variables were calculated. Results: Out of 388 deaths, 54% were premature. The most common causes of premature death included IHD (40%), stroke (11%), road traffic injuries (6%), lower respiratory infections (5%), and COVID-19 (3%). The predictive factors [adjusted HRs (95% CIs)] associated with premature mortality included age [year, 1.07 (1.04, 1.10)], tobacco [1.43 (0.96, 2.11)], opium [2.12 (1.39, 3.24)], hypertension [1.52 (1.10, 2.12)], waist circumference [cm, 1.03 (1.00, 1.05)], female sex [0.30 (0.19, 0.47)], education [> 8 years vs. no formal schooling, 0.46 (0.24, 0.88)], being married [0.60 (0.37, 0.97)], physical activity [3rd vs. 1st tertile, 0.38 (0.26, 0.57)], hip circumference [cm, 0.96 (0.92, 0.99)], estimated GFR [mL/min/1.73 m², 0.99 (0.978, 0.999)], and wealth score [4th vs. 1st quartile, 0.54 (0.32, 0.90)]. The PAF (95% CI) for all modifiable predictors was 0.83 (0.62, 0.92). Conclusions: The predominant causes of premature mortality were IHD and stroke. To mitigate premature deaths, it is recommended to address both socioeconomic and behavioral factors simultaneously.
Expression of constitutively active alpha-PAK reveals effects of the kinase on actin and focal complexes

Abstract:The family of p21-activated protein kinases (PAKs) appear to be present in all organisms that have Cdc42-like GTPases. In mammalian cells, PAKs have been implicated in the activation of mitogen-activated protein kinase cascades, but there are no reported effects of these kinases on the cytoskeleton. Recently we have shown that a Drosophila PAK is enriched in the leading edge of embryonic epithelial cells undergoing dorsal closure (N. Harden, J. Lee, H.-Y. Loh, Y.-M. Ong, I. Tan, T. Leung, E. Manser, and L. Lim, Mol. Cell. Biol. 16:1896-1908, 1996), where it colocalizes with structures resembling focal complexes. We show here by transfection that in epithelial HeLa cells alpha-PAK is recruited from the cytoplasm to distinct focal complexes by both Cdc42(G12V) and Rac1(G12V), which themselves colocalize to these sites. By deletion analysis, the N terminus of PAK is shown to contain targeting sequences for focal adhesions which indicate that these complexes are the site of kinase function in vivo. Cdc42 and Rac1 cause alpha-PAK autophosphorylation and kinase activation. Mapping alpha-PAK autophosphorylation sites has allowed generation of a constitutively active kinase mutant. By fusing regions of Cdc42 to the C terminus of PAK, activated chimeras were also obtained. Plasmids encoding these different constitutively active alpha-PAKs caused loss of stress fibers when introduced into both HeLa cells and fibroblasts, which was similar to the effect of introducing Cdc42(G12V) or Rac1(G12V). Significantly dramatic losses of focal adhesions were also observed. These combined effects resulted in retraction of the cell periphery after plasmid microinjection. These data support our previous suggestions of a role for PAK downstream of both Cdc42 and Rac1 and indicate that PAK functions include the dissolution of stress fibers and reorganization of focal complexes.
The study of trends in mortality of respiratory system cancers in Babol, North of Iran (2013-2021)

Abstract:Background: In Iran, cancers are known as the second leading cause of death, among which respiratory system cancers are particularly important because lung cancer is the second most common cause of death in this country. This study aimed to estimate the crude and age-standardized mortality and its trends during 9 years in the Northern city of Iran, Babol. Methods: In this cross-sectional study, all the recorded deaths due to respiratory system cancers in Babol during the years 2013-2021 on the Classification of Causes of Deaths and Death Registration System of Babol University of Medical Sciences were taken into consideration, and the population estimate was based on the latest census. Finally, the crude and age-standardized rates of mortality and trends of cancer incidents were calculated. Results: In general, 393 deaths with an overall mean age of 67.8±3.9 years have happened due to respiratory system cancers. The crude and standardized rates of respiratory system cancers increased from 6.5 and 5.5 per hundred thousand people in 2013 to 9.1 and 7.8 per hundred thousand people in 2021, respectively. With each decade in age, their trends increased significantly in men (p=0.024) and remained constant in women (p=0.262). In examining the trend of respiratory system cancers, we found an increasing trend for lung cancer and a constant trend for larynx and oropharynx cancers. There was also a decreasing trend for hypopharyngeal cancer. Conclusions: The age-standardized rate and trend of respiratory system cancers are increasing. Therefore, it is important to prevent their prevalence by reducing the risk factors and increasing the general awareness of risks and timely diagnosis.

Abstract:Introduction: Occupational accidents have been regarded as the third cause of mortality around the world. This study examined the rates, types, and causes of occupational accidents in industrial sites. Methods: In a cross-sectional design, 721 accident reports were examined in the manufacturing sites in Semnan which is located in the central part of Iran from 2013 to 2015. This study used the Social Security Organization database on workplace accident investigation reports. Frequency and percentage were used for the prevalence, whereas correlation analyses were used to identify the predictors of occupational accidents. SPSS version 21 was used for data analysis and a p-value less than 0.05 was considered as the significant level. Results: Accident Frequency Rates were 1.46, 1.54, and 1.40 in 2013, 2014, and 2015, respectively. Also, the Safe T-score was calculated between +3 and -3. The majority of accidents occurred in males in the age group 24-45, having the educational level of high school diploma or lower. Furthermore, most accidents occurred during the morning shift. Unsafe actions were determined as the major cause of accidents. The maximum reported statistics related to the type of accidents, injuries, and injured organs of the body were hitting sharp edges or obstacles (25%), bruising (30%), and upper limbs (44%). The results indicated a statistically significant relationship between shift work and occupational accidents (P-value <0.05). Conclusion: This study revealed that workers’ behavior as a main contribution to accidents and injuries in the industry. Health and safety training should be designed according to worker duties.

Abstract:Among the different techniques proposed to integrate the standard cardiopulmonary resuscitation (sCPR) protocol, mechanical CPR (mCPR) and interposed abdominal compression (IAC) were found to be particularly effective for the simplicity of the procedure and the significant results obtained. A case of a 54-year old male with cardiogenic shock following viral infection, in which prolonged mechanical cardiopulmonary resuscitation with interposed abdominal compression was performed, is presented. Five hours after admission in the ICU, the patient's condition worsened with subsequent cardiac arrest with pulseless electrical activity (PEA). Mechanical CPR was promptly started, subsequently associated with IAC and prolonged for 1 hour and 20 minutes. Although the patient survived for only eight hours following cardiac arrest, prolonged IAC-mCPR allowed to start extra corporeal circulation (CPP). The patient was then transferred to the cardiosurgical operating theatre for ventricular assistance by centrifugal pump (VAP). Cardiovascular data obtained from patients monitoring did not shown any cardiac lesions or adverse effects as observed by autoptic examination and suggest the reliability of this mechanical method, which allows a better performance when compared to standard CPR. In prolonged resuscitations a few contraindications to both mCPR and IAC suggest the application of the associated techniques at all times in cardiac arrest, combining the benefits of both procedures.
Fear of falling, quality of life, and daily functional activity of elderly women with and without a history of falling: a cross-sectional study

Abstract:Background: This study aimed to evaluate the fear of falling, quality of life, and daily functional activity of older women aged 60 years or older with or without a history of falling. Materials and methods: Two hundred older adult women were recruited for the cross-sectional study in Iran. This cross-sectional study collected data from July to August 2023 through convenience sampling. The researchers collected data using a five-part questionnaire, that collected information that included demographic characteristics, the Fall Efficacy Scale in the Elderly-International Version (FES-I), 12-item Quality of Life assessment (SF-12), Instrumental Activities of Daily Living (IADL) and Activities of Daily Living (ADL). Results: FES-I score in older women with a history of falls was significantly higher than those without a history of falls [median: 38.0, interquartile range (IQR): 31.5-44.0 versus median: 22.0, IQR: 20.0-30.0; P<0.001]. The median quality-of-life score using the SF-12 was significantly lower in women with a history of falls than in those without a history of falls (median: 25.0, IQR: 21.0-30.0 versus median: 35.0, IQR: 31.0-39.0; P<0.001). The ADL scores were significantly lower among women with a history of falls than those without (P<0.001). A similar result was obtained for IADL scores (P<0.001). Conclusion: Overall, this study's findings highlight the adverse impact of a history of falls on three key factors: fear of falling, quality of life, and daily functional activity (including both basic and instrumental activities). The findings delineate that, ultimately, the history of falls can serve as a valuable indicator for better understanding trends in elderly care and addressing the associated challenges.

Abstract:Background Falls can lead to severe health loss including death. Past research has shown that falls are an important cause of death and disability worldwide. The Global Burden of Disease Study 2017 (GBD 2017) provides a comprehensive assessment of morbidity and mortality from falls. Methods Estimates for mortality, years of life lost (YLLs), incidence, prevalence, years lived with disability (YLDs) and disability-adjusted life years (DALYs) were produced for 195 countries and territories from 1990 to 2017 for all ages using the GBD 2017 framework. Distributions of the bodily injury (eg, hip fracture) were estimated using hospital records. Results Globally, the age-standardised incidence of falls was 2238 (1990–2532) per 100 000 in 2017, representing a decline of 3.7% (7.4 to 0.3) from 1990 to 2017. Age-standardised prevalence was 5186 (4622–5849) per 100 000 in 2017, representing a decline of 6.5% (7.6 to 5.4) from 1990 to 2017. Age-standardised mortality rate was 9.2 (8.5–9.8) per 100 000 which equated to 695 771 (644 927–741 720) deaths in 2017. Globally, falls resulted in 16 688 088 (15 101 897–17 636 830) YLLs, 19 252 699 (13 725 429–26 140 433) YLDs and 35 940 787 (30 185 695–42 903 289) DALYs across all ages. The most common injury sustained by fall victims is fracture of patella, tibia or fibula, or ankle. Globally, age-specific YLD rates increased with age. Conclusions This study shows that the burden of falls is substantial. Investing in further research, fall prevention strategies and access to care is critical.
