Abstract:Segmentation of the femoral head and proximal acetabulum from three dimensional (3D) CT data is essential for patient specific planning and simulation of hip surgery whereas it still remains challenging due to deformed shapes and extremely narrow inter-bone regions. In this paper, we present an accurate, automatic and fast approach for simultaneous segmentation of the femoral head and proximal acetabulum in the hip joint from 3D CT data. First valley-emphasized image is constructed from original images so that valleys stand out in high relief and initial thresholding segmentation is performed to divide the image set into bone (femoral head and acetabulum) and non-bone classes. It is employed as an initial boundary of the femoral head and acetabulum for further processing in the segmentation procedures. In the subsequent iterative process, the bone regions are further segmented with consideration of the narrow joint space, the neighborhood information and the partial volume effect. Finally, the segmented bone boundaries are corrected based on the normal direction of vertices of the 3D bone surface. Evaluation of the method is performed on the 110 hips including pathologies. Experimental results indicate that our method rapidly leads to very accurate segmentations of the femoral head and acetabulum in the hip joint and can be applied as a tool in the clinical practice.
An Automatic Method for Elbow Joint Recognition, Segmentation and Reconstruction

Abstract:Elbow computerized tomography (CT) scans have been widely applied for describing elbow morphology. To enhance the objectivity and efficiency of clinical diagnosis, an automatic method to recognize, segment, and reconstruct elbow joint bones is proposed in this study. The method involves three steps: initially, the humerus, ulna, and radius are automatically recognized based on the anatomical features of the elbow joint, and the prompt boxes are generated. Subsequently, elbow MedSAM is obtained through transfer learning, which accurately segments the CT images by integrating the prompt boxes. After that, hole-filling and object reclassification steps are executed to refine the mask. Finally, three-dimensional (3D) reconstruction is conducted seamlessly using the marching cube algorithm. To validate the reliability and accuracy of the method, the images were compared to the masks labeled by senior surgeons. Quantitative evaluation of segmentation results revealed median intersection over union (IoU) values of 0.963, 0.959, and 0.950 for the humerus, ulna, and radius, respectively. Additionally, the reconstructed surface errors were measured at 1.127, 1.523, and 2.062 mm, respectively. Consequently, the automatic elbow reconstruction method demonstrates promising capabilities in clinical diagnosis, preoperative planning, and intraoperative navigation for elbow joint diseases.
A Deep Learning-Based Approach to Extracting Periosteal and Endosteal Contours of Proximal Femur in Quantitative CT Images

Abstract:Automatic CT segmentation of proximal femur is crucial for the diagnosis and risk stratification of orthopedic diseases; however, current methods for the femur CT segmentation mainly rely on manual interactive segmentation, which is time-consuming and has limitations in both accuracy and reproducibility. In this study, we proposed an approach based on deep learning for the automatic extraction of the periosteal and endosteal contours of proximal femur in order to differentiate cortical and trabecular bone compartments. A three-dimensional (3D) end-to-end fully convolutional neural network, which can better combine the information between neighbor slices and get more accurate segmentation results, was developed for our segmentation task. 100 subjects aged from 50 to 87 years with 24,399 slices of proximal femur CT images were enrolled in this study. The separation of cortical and trabecular bone derived from the QCT software MIAF-Femur was used as the segmentation reference. We randomly divided the whole dataset into a training set with 85 subjects for 10-fold cross-validation and a test set with 15 subjects for evaluating the performance of models. Two models with the same network structures were trained and they achieved a dice similarity coefficient (DSC) of 97.87% and 96.49% for the periosteal and endosteal contours, respectively. To verify the excellent performance of our model for femoral segmentation, we measured the volume of different parts of the femur and compared it with the ground truth and the relative errors between predicted result and ground truth are all less than 5%. It demonstrated a strong potential for clinical use, including the hip fracture risk prediction and finite element analysis.
Dual output feature fusion networks for femoral segmentation and quantitative analysis of the knee joint

Abstract:Background Magnetic resonance imaging (MRI) is the preferred imaging modality for diagnosing knee disease. Segmentation of the knee MRI images is essential for subsequent quantification of clinical parameters and treatment planning for knee prosthesis replacement. However, the segmentation remains difficult due to individual differences in anatomy, the difficulty of obtaining accurate edges at lower resolutions, and the presence of speckle noise and artifacts in the images. In addition, radiologists must manually measure the knee's parameters which is a laborious and time‐consuming process. Purpose Automatic quantification of femoral morphological parameters can be of fundamental help in the design of prosthetic implants for the repair of the knee and the femur. Knowledge of knee femoral parameters can provide a basis for femoral repair of the knee, the design of fixation materials for femoral prostheses, and the replacement of prostheses. Methods This paper proposes a new deep network architecture to comprehensively address these challenges. A dual output model structure is proposed, with a high and low layer fusion extraction feature module designed to extract rich features through the cross‐fusion mechanism. A multi‐scale edge information extraction spatial feature module is also developed to address the boundary‐blurring problem. Results Based on the precise automated segmentation results, 10 key clinical parameters were automatically measured for a knee femoral prosthesis replacement program. The correlation coefficients of the quantitative results of these parameters compared to manual results all achieved at least 0.92. The proposed method was extensively evaluated with MRIs of 78 patients' knees, and it consistently outperformed other methods used for segmentation. Conclusions The automated quantization process produced comparable measurements to those manually obtained by radiologists. This paper demonstrates the viability of automatic knee MRI image segmentation and quantitative analysis with the proposed method. This provides data to support the accuracy of assessing the progression and biomechanical changes of osteoarthritis of the knee using an automated process, thus saving valuable time for the radiologists and surgeons.

Abstract:Segmentation of the pelvis and proximal femur in computed tomography (CT) volumes is a prerequisite of patient specific planning and simulation for hip surgery. Existing methods do not perform well due to bone disease and technical limitations of CT imaging. In this article, an accurate framework for segmenting bone in the hip joint is presented. Our approach begins with valley-emphasized image construction using morphological operations so that valleys stand out in high relief, and then, an initial segmentation with optimal threshold is performed to divide the dataset into bone and non-bone regions. Subsequently, bone regions are reclassified based on 3D iterative adaptive thresholding with consideration of the partial volume effect and the spatial information. Finally, we refine the rough bone boundaries based on the normal direction of vertices of the 3D bone surface. Our segmentation approach is automatic and robust. Its performance is evaluated on 35 datasets consisting of 70 hip joints with a status ranging from healthy to severe osteoarthritis and the results have proved to be very successful.
Automating Linear and Angular Measurements for the Hip and Knee After Computed Tomography: Validation of a Three-Stage Deep Learning and Computer Vision-Based Pipeline for Pathoanatomic Assessment

Abstract:Background: Variability in the bony morphology of pathologic hips/knees is a challenge in automating preoperative computed tomography (CT) scan measurements. With the increasing prevalence of CT for advanced preoperative planning, processing this data represents a critical bottleneck in presurgical planning, research, and development. The purpose of this study was to demonstrate a reproducible and scalable methodology for analyzing CT-based anatomy to process hip and knee anatomy for perioperative planning and execution. Methods: One hundred patients with preoperative CT scans undergoing total knee arthroplasty for osteoarthritis were processed. A two-step deep learning pipeline of classification and segmentation models was developed that identifies landmark images and then generates contour representations. We utilized an open-source computer vision library to compute measurements. Classification models were assessed by accuracy, precision, and recall. Segmentation models were evaluated using dice and mean Intersection over Union (IOU) metrics. Contour measurements were compared against manual measurements to validate posterior condylar axis angle, sulcus angle, trochlear groove-tibial tuberosity distance, acetabular anteversion, and femoral version. Results: Classifiers identified landmark images with accuracy of 0.91 and 0.88 for hip and knee models, respectively. Segmentation models demonstrated mean IOU scores above 0.95 with the highest dice coefficient of 0.957 [0.954-0.961] (UNet3+) and the highest mean IOU of 0.965 [0.961-0.969] (Attention U-Net). There were no statistically significant differences for the measurements taken automatically vs manually (P > 0.05). Average time for the pipeline to preprocess (48.65 +/- 4.41 sec), classify/retrieve landmark images (8.36 +/- 3.40 sec), segment images (<1 sec), and obtain measurements was 2.58 (+/- 1.92) minutes. Conclusions: A fully automated three-stage deep learning and computer vision-based pipeline of classification and segmentation models accurately localized, segmented, and measured landmark hip and knee images for patients undergoing total knee arthroplasty. Incorporation of clinical parameters, like patient-reported outcome measures and instability risk, will be important considerations alongside anatomic parameters.
Automatic segmentation of knee CT images of tibial plateau fractures based on three-dimensional U-Net: Assisting junior physicians with Schatzker classification

Abstract:Purpose: This study aimed to automatically segment knee computed tomography (CT) images of tibial plateau fractures using a three-dimensional (3D) U-net-based method, accurately construct 3D maps of tibial plateau fractures, and examine their usefulness for Schatzker classification in clinical practice. Methods: We retrospectively enrolled 234 cases with tibial plateau fractures from our hospital in this study. The four constituent bones of the knee were manually annotated using ITK-SNAP software. Finally, image features were extracted using deep learning. The usefulness of the results for Schatzker classification was examined by an orthopaedic and a radiology resident. Results: On average, our model required < 40 s to process a 3D CT scan of the knee. The average Dice coefficient for all four knee bones was higher than 0.950, and highly accurate 3D maps of the tibia were produced. With the aid of the results of our model, the accuracy, sensitivity, and specificity of the Schatzker classification of both residents improved. Conclusions: The proposed method can rapidly and accurately segment knee CT images of tibial plateau fractures and assist residents with Schatzker classification, which can help improve diagnostic efficiency and reduce the workload of junior doctors in clinical practice.

Abstract:Automatic femur segmentation from computed tomography volume is a crucial but challenging task for computer-aided diagnosis in orthopedic surgeries. The main obstacles are weak bone boundaries, narrowness of joint space, variations in femur density and shape, as well as diverse leg postures. In this paper, we presented a novel 3-D feature-enhanced network to address these challenges. The novelty of our approach lies in two feature enhancement modules, including the edge detection task and the multiscale features fusion. First, the edge detection task was embedded into femur segmentation from computed tomography volume to solve the problems of narrow joint space and weak femur boundary. Crucially, a task-specific edge detector was used to optimize the performance of femur segmentation in an end-to-end trainable system. Second, the multi-scale features fusion provided both local and global contexts to handle the problems of large variations in leg postures as well as femur shape and density. The results demonstrated that accurate 3-D femur segmentation with a high Dice similarity coefficient of 96.88% was achieved using the developed method, and the segmentation of computed tomography volume took 0.93 s on an average.
A deep learning-based approach to automatic proximal femur segmentation in quantitative CT images

Abstract:Automatic CT segmentation of proximal femur has a great potential for use in orthopedic diseases, especially in the imaging-based assessments of hip fracture risk. In this study, we proposed an approach based on deep learning for the fast and automatic extraction of the periosteal and endosteal contours of proximal femur in order to differentiate cortical and trabecular bone compartments. A three-dimensional (3D) end-to-end fully convolutional neural network (CNN), which can better combine the information among neighbor slices and get more accurate segmentation results by 3D CNN, was developed for our segmentation task. The separation of cortical and trabecular bones derived from the QCT software MIAF-Femur was used as the segmentation reference. Two models with the same network structures were trained, and they achieved a dice similarity coefficient (DSC) of 97.82% and 96.53% for the periosteal and endosteal contours, respectively. Compared with MIAF-Femur, it takes half an hour to segment a case, and our CNN model takes a few minutes. To verify the excellent performance of our model for proximal femoral segmentation, we measured the volumes of different parts of the proximal femur and compared it with the ground truth, and the relative errors of femur volume between predicted result and ground truth are all less than 5%. This approach will be expected helpful to measure the bone mineral densities of cortical and trabecular bones, and to evaluate the bone strength based on FEA.
A new Level-set based Protocol for Accurate Bone Segmentation from CT Imaging

Abstract:In this work it is proposed a medical image segmentation pipeline for accurate bone segmentation from CT imaging. It is a two-step methodology, with a pre-segmentation step and a segmentation refinement step. First, the user performs a rough segmenting of the desired region of interest. Next, a fully automatic refinement step is applied to the pre-segmented data. The automatic segmentation refinement is composed by several sub-stpng, namely image deconvolution, image cropping and interpolation. The user-defined pre-segmentation is then refined over the deconvolved, cropped, and up-sampled version of the image. The algorithm is applied in the segmentation of CT images of a composite femur bone, reconstructed with different reconstruction protocols. Segmentation outcomes are validated against a gold standard model obtained with coordinate measuring machine Nikon Metris LK V20 with a digital line scanner LC60-D that guarantees an accuracy of 28 $\mu m$. High sub-pixel accuracy models were obtained for all tested Datasets. The algorithm is able to produce high quality segmentation of the composite femur regardless of the surface meshing strategy used.
2D-3D Hierarchical Feature Fusion Network for Segmentation of Bone Structure in Knee Mr Image

Abstract:Automatic segmentation of knee bone structures is an important task in orthopedics diagnosis of knee disease based on MRI images. Inspired by doctors’ diagnosis of knee in sagittal plane of MR image, we propose to first calculate the sagittal maximum intensity projection (MIP) of MR image, then construct a high precision 2D-3D hierarchical feature fusion network for automatic segmentation of knee based on convolutional encoding and decoding architecture. It includes: 1) A 2D bypass network extracting global features based on MIP; 2) A 3D backbone network calculating local details based on MR volume; 3) A feature fusion module integrating 2D global context and 3D local details hierarchically. Particularly, the global features as anchor points will be fused with the local details at each level of the encoding path to enrich the context of local details and improve the segmentation accuracy. Our method is verified on SKI10 dataset. The average dice coefficients of femur, femoral cartilage, tibia and tibia cartilage are 0.978, 0.848, 0.979 and 0.848, respectively, and the segmentation performance is far better than the state-of-the-art methods.
Segmentation of tibiofemoral joint tissues from knee MRI using MtRA-Unet and incorporating shape information: Data from the Osteoarthritis Initiative

Abstract:Knee Osteoarthritis (KOA) is the third most prevalent Musculoskeletal Disorder (MSD) after neck and back pain. To monitor such a severe MSD, a segmentation map of the femur, tibia and tibiofemoral cartilage is usually accessed using the automated segmentation algorithm from the Magnetic Resonance Imaging (MRI) of the knee. But, in recent works, such segmentation is conceivable only from the multistage framework thus creating data handling issues and needing continuous manual inference rendering it unable to make a quick and precise clinical diagnosis. In order to solve these issues, in this paper the Multi-Resolution Attentive-Unet (MtRA-Unet) is proposed to segment the femur, tibia and tibiofemoral cartilage automatically. The proposed work has included a novel Multi-Resolution Feature Fusion (MRFF) and Shape Reconstruction (SR) loss that focuses on multi-contextual information and structural anatomical details of the femur, tibia and tibiofemoral cartilage. Unlike previous approaches, the proposed work is a single-stage and end-to-end framework producing a Dice Similarity Coefficient (DSC) of 98.5% for the femur, 98.4% for the tibia, 89.1% for Femoral Cartilage (FC) and 86.1% for Tibial Cartilage (TC) for critical MRI slices that can be helpful to clinicians for KOA grading. The time to segment MRI volume (160 slices) per subject is 22 sec. which is one of the fastest among state-of-the-art. Moreover, comprehensive experimentation on the segmentation of FC and TC which is of utmost importance for morphology-based studies to check KOA progression reveals that the proposed method has produced an excellent result with binary segmentation

Abstract:Quantitative computed tomography (QCT)-based in silico models have demonstrated improved accuracy in predicting hip fractures with respect to the current gold standard, the areal bone mineral density. These models require that the femur bone is segmented as a first step. This task can be challenging, and in fact, it is often almost fully manual, which is time-consuming, operator-dependent, and hard to reproduce. This work proposes a semi-automated procedure for femur bone segmentation from CT images. The proposed procedure is based on the bone and joint enhancement filter and graph-cut algorithms. The semi-automated procedure performances were assessed on 10 subjects through comparison with the standard manual segmentation. Metrics based on the femur geometries and the risk of fracture assessed in silico resulting from the two segmentation procedures were considered. The average Hausdorff distance (0.03 ± 0.01 mm) and the difference union ratio (0.06 ± 0.02) metrics computed between the manual and semi-automated segmentations were significantly higher than those computed within the manual segmentations (0.01 ± 0.01 mm and 0.03 ± 0.02). Besides, a blind qualitative evaluation revealed that the semi-automated procedure was significantly superior ( p < 0.001) to the manual one in terms of fidelity to the CT. As for the hip fracture risk assessed in silico starting from both segmentations, no significant difference emerged between the two ( R 2 = 0.99). The proposed semi-automated segmentation procedure overcomes the manual one, shortening the segmentation time and providing a better segmentation. The method could be employed within CT-based in silico methodologies and to segment large volumes of images to train and test fully automated and supervised segmentation methods.

Abstract:PURPOSE: To describe and evaluate a new segmentation method using deep convolutional neural network (CNN), 3D fully connected conditional random field (CRF), and 3D simplex deformable modeling to improve the efficiency and accuracy of knee joint tissue segmentation.METHODS: A segmentation pipeline was built by combining a semantic segmentation CNN, 3D fully connected CRF, and 3D simplex deformable modeling. A convolutional encoder-decoder network was designed as the core of the segmentation method to perform high resolution pixel-wise multi-class tissue classification for 12 different joint structures. The 3D fully connected CRF was applied to regularize contextual relationship among voxels within the same tissue class and between different classes. The 3D simplex deformable modeling refined the output from 3D CRF to preserve the overall shape and maintain a desirable smooth surface for joint structures. The method was evaluated on 3D fast spin-echo (3D-FSE) MR image data sets. Quantitative morphological metrics were used to evaluate the accuracy and robustness of the method in comparison to the ground truth data.RESULTS: The proposed segmentation method provided good performance for segmenting all knee joint structures. There were 4 tissue types with high mean Dice coefficient above 0.9 including the femur, tibia, muscle, and other non-specified tissues. There were 7 tissue types with mean Dice coefficient between 0.8 and 0.9 including the femoral cartilage, tibial cartilage, patella, patellar cartilage, meniscus, quadriceps and patellar tendon, and infrapatellar fat pad. There was 1 tissue type with mean Dice coefficient between 0.7 and 0.8 for joint effusion and Baker's cyst. Most musculoskeletal tissues had a mean value of average symmetric surface distance below 1 mm.CONCLUSION: The combined CNN, 3D fully connected CRF, and 3D deformable modeling approach was well-suited for performing rapid and accurate comprehensive tissue segmentation of the knee joint. The deep learning-based segmentation method has promising potential applications in musculoskeletal imaging.

Scheme to Extract Femur Axes Automatically Based on CT Images

Abstract:Objective The identification of lower limb axes plays an important role in the kinematics research and clinical operation of total knee replacement surgery.This paper presents a method to extract the femoral axes based on CT images automatically.Method Using a series of image pre-processing algorithms such as segmentation,smoothing and rotations of the images,to get the 3-D fine mesh of lower limb bones.Then,by analyzing the morphological feature of 3-D femoral surface and based on the definitions of femoral mechanic axis and femoral anatomical axis,three feature points located on femoral head,femoral distal intercondylar notch and femoral shaft centre are selected to define the mechanical and anatomical axes of femur.In the light of the morphological feature of femur head,the center of femoral head was decided according to the changes of the gradient between each cross section of the femoral head.Meanwhile,we got the center of femoral intercondylar notch by calculating the closed-loop zone of cross sections of the bone.Those two points could finally define the mechanical axis of the femur.Result Based on a series of image processing procedures,three dimensional(3-D)surface model of lower limb bone were generated.By analyzing the morphological features of femoral head and knee joint center,a simple and convenient way was proposed to extract the coordinate of three feature points and further the femoral axes automatically.Conclusions Experimental results show that this processing procedure can extract the femoral mechanic axis and femoral anatomical axis precisely with 3-D CT images.
A Feasibility Study of Knee Joint Semantic Segmentation on 3D MR Images

Existing knee joint segmentation methods in the literature focus on only one or few structures out of many. In this paper, we study the feasibility of knee joint segmentation on MR images based on neural networks and deal with the following challenges: (1) end-to-end segmentation of 15 anatomical structures, including bone and soft tissue, of the whole knee on MR images; (2) robust segmentation of small structures such as the anterior cruciate ligament, accounting for about 0.036％ of the volume data. Experiments on the knee joint MR images demonstrate that the average segmentation accuracy of our method achieves 92.92％. The Dice similarity coefficients of 9 structures were above 94％, five structures were between 87％ and 90％, and the remaining one was about 76％.

Abstract:The fate of endogenous vitamin E isomers during production and purification of structured lipids (SLs) was investigated. Two SLs involving tripalmitin, stearidonic acid soybean oil, and docosahexaenoic acid were synthesized by transesterification catalyzed by Novozym 435 (NSL) and acidolysis by Lipozyme TL IM (LDHA) and purified by short-path distillation (SPD). The electron impact and chemical ionization mass spectra of tocopheryl and tocotrienyl fatty acid esters in the distillates measured by GC-MS in synchronous scan/SIM mode demonstrated that these esters were formed during acidolysis as well as transesterification. The predominant esters were tocopheryl palmitate, tocopheryl oleate, and tocopheryl linoleate homologues, and no tocopheryl or tocotrienyl linolenate, stearidonate, or docosahexaenoate was found. Meanwhile, none of these esters were detected in the residues for either NSL or LDHA. Less than 50% of vitamin E isomers were present in residues after SPD. This loss played a major role in the rapid oxidative deterioration of SLs from previous studies with less contribution from the formation of tocopheryl and tocotrienyl esters. The lost tocopherols and tocotrienols present at high concentration in the distillates may be recovered and used to improve the oxidative stability of SLs.
An automatic segmentation algorithm for conglutinated bone fragments in 3D CT images of lower limb fractures based on morphology

Abstract:The commonly used automatic segmentation algorithms often have over-segmentation and error-segmentation problems when segmenting conglutinated bone fragments in computed tomography (CT) images. And the end face details of the fragments are lost in this process. At present, bone fragments are often manually segmented by doctors. This increases the workload of doctors and takes low efficiency and poor accuracy in bone segmentation. We propose an automatic bone fragment segmentation algorithm based on morphology to segment the conglutinated and separated bone fragments and reconstruct them in 2D and 3D spaces. The pixel classification and clustering method based on morphology are proposed in in the algorithm to identify the pixels in the fracture images. And a concave points detection method is proposed to segment the conglutination area of the fractured bone. This algorithm and two other commonly used algorithms are used in the experiment to segment the three typical fracture images. The segmentation results are quantitatively analyzed to demonstrate the advantages of the morphology algorithm. The morphology algorithm performs well in the metrics of Accuracy (.93 ± .07), Sensitivity (.92 ± .08), Dice coefficient (.92 ± .08) and Mean intersection over union (MIoU) (.92 ± .07). The segmentation results of this algorithm are closest to the manual segmentation results of doctors compared to the other two algorithms. We have demonstrated that the algorithm proposed in this paper has good segmentation accuracy for separated and conglutinated bone fragments. This algorithm significantly improves the segmentation accuracy and efficiency of bone fragments under complex fracture conditions.

Abstract:Unet architectures are being investigated for automatic image segmentation of bones in CT scans because of their ability to address size-varying anatomies and pathological deformations. Nonetheless, changes in mineral density, narrowing of joint spaces and formation of largely irregular osteophytes may easily disrupt automatism requiring extensive manual refinement. A novel Unet variant, called CEL-Unet, is presented to boost the segmentation quality of the femur and tibia in the osteoarthritic knee joint. The neural network embeds region-aware and two contour-aware branches in the decoding path. The paper features three main technical novelties: 1) directed connections between contour and region branches progressively at different decoding scales; 2) pyramidal edge extraction in the contour branch to perform multi-resolution edge processing; 3) distance-weighted cross-entropy loss function to increase delineation quality at the sharp edges of the shapes. A set of 700 knee CT scans was used to train the model and test segmentation performance. Qualitatively CEL-Unet correctly segmented cases where the state-of-the-art architectures failed. Quantitatively, the Jaccard indexes of femur and tibia segmentation were 0.98 and 0.97, with median 3D reconstruction errors less than 0.80 and 0.60 mm, overcoming competitive Unet models. The results were evaluated against knee arthroplasty planning based on personalized surgical instruments (PSI). Excellent agreement with reference data was found for femoral (0.11°) and tibial (0.05°) alignments of the distal and proximal cuts computed on the reconstructed surfaces. The bone segmentation was effective for large pathological deformations and osteophytes, making the techniques potentially usable in PSI-based surgical planning, where the reconstruction accuracy of the bony shapes is one of the main critical factors for the success of the operation.

Abstract:Recent advances in fluorescence microscopy technologies have revolutionized our views on bacterial cell organization. A prolific area of this revolution in tiny cell visualization has been cell division. Most bacteria divide in their middle, implying that they are aware of at least one particular site of themselves, the midcell division site. This raises the fundamental question: How do bacterial cells identify midcell? It all started with the immunolocalization studies of the FtsZ division protein, which showed that FtsZ assembles at midcell in a ring-like structure (1). Since then, additional cell division proteins have been shown to localize to midcell (2). In a recent issue of the Proceedings , Raskin and de Boer (3) extend this concept of site-specific protein targeting by describing the unique dynamic properties of the localization of the MinD division inhibitor. This discovery adds a new dimension to our exploration of the unexpected complexity in the spatial organization of bacterial cells. One cannot talk about bacterial cell division without considering the FtsZ protein, which shares properties with cytoskeletal molecules. FtsZ, which binds GTP and has a GTPase activity (4–6), plays a central role in cytokinesis as a major component of a contractile ring (1). The assembly of the FtsZ ring at midcell occurs well before the constriction is initiated (1). In addition to ftsZ (7), many more genes are specifically involved in cell division (2, 8). The other cell division proteins are later recruited to the FtsZ ring to form the membrane-associated septal ring that mediates septation. The answer to the problem of midsite selection is still largely unknown, but important insights have come from studies on minicell-producing ( min ) mutants (9).
