Abstract:Abstract Background The major emerging infectious diseases (MEIDs) have occurred frequently and become increasingly serious in the world. Sufficient personal emergency preparedness is critical for the general people in efficiently responding to and recovering from MEIDs. Nevertheless, few specific indicators are available for assessing the individual emergency preparedness of the general public during these periods. Therefore, the aim of this study was to construct an index system for comprehensively evaluating the personal emergency preparedness of the public regarding MEIDs. Methods Based on the global national-level emergency preparedness index framework and a literature review, a preliminary index system was constructed. From June 2022 to September 2022, a panel of 20 experts from nine provinces and municipalities across multiple research areas participated in this Delphi study. They rated the importance of pre-defined indicators using a five-point Likert scale and provided their qualitative comments. According to the feedback of each round of experts, the indicators of the evaluation index system were revised. Results After two rounds of expert consultation the evaluation index system reached a consensus, containing five first-level indicators, cooperating with prevention and control work, improving emergency response capacity, securing supplies and equipment, preparing economic resources, maintaining physical and mental health with affiliated 20 s-level indicators and 53 third-level indicators. The expert authority coefficient of consultation was 0.88 and 0.90. The Kendall’s coefficient of concordance of expert consultations was 0.294 and 0.322, respectively. The differences were statistically significant ( P < 0.05). Conclusion A valid, reliable and scientific evaluation index system was established. This personal emergency preparedness index system, as a precursor form, will further lay the foundation for the formation of an assessment instrument. At the same time, it could provide a reference for future education and training of emergency preparedness for the general public.

Abstract:BACKGROUND:The severity of an outbreak is a priority in decision-making for human infection control. However, there have been no reports on how to quantify the severity of an outbreak.METHODS:We propose a simple method to measure the severity of an infectious disease outbreak. It involves scoring the severity of clinical signs, the transmission of the infection, the number of cases, and the infection source.RESULTS:The method was evaluated using the data available at the early stage of some recent outbreaks of infectious diseases, including the influenza A (H1N1) pandemic in 2009, and the evaluation supports the design idea.CONCLUSION:The method is practical for rating the severity of an infectious disease outbreak, though it should be optimized. It could also be used to judge whether an event constitutes a public health emergency of international concern (PHEIC) or not.

Abstract:Abstract Aim and objective This study was to establish an index system for the evaluation of Chinese infectious disease specialist nurses’ core competence. Background The index system for the evaluation of infectious disease specialist nurses’ core competence has not been established. Design A two-round Delphi survey was conducted to seek opinions from experts about the index system for the evaluation of infectious disease specialist nurses’ core competence. Methods The study adopted several research methods, including literature retrieval, theoretical analysis and qualitative research. Based on the above method, the draft of core competence evaluation index system of infectious disease specialist nurses was constructed. A Delphi survey was used for the study of 30 infectious disease experts from 8 provinces and cities around China. A modified recommendation for the Conducting and Reporting of Delphi studies (CREDES) was also used to guide this study. A STROBE checklist was used. Results The Core Competence Evaluation Index System of Infectious Disease Nurses is composed of 6 primary indicators, namely, Nursing Abilities for Infectious Diseases, Infection Prevention and Control Abilities, Responsiveness to Infectious Diseases, Professional Development Abilities, Communication and Management Abilities, and Professionalism and Humanistic Accomplishment, 16 secondary indicators and 47 tertiary indicators. The authority coefficient, judgment coefficient and familiarity degree of Delphi experts were 0.923, 0.933 and 0.913 respectively. Conclusions The evaluation index system of core competence of diseases specialist nurses is scientific and reliable. It can be reference for future training and assessment of Chinese infectious disease specialist nurses. Relevance to clinical practice Infectious disease specialist nurses are the main force for infectious disease nursing. Their core competence is related to the quality of infectious disease nursing and treatment. The core competence of the nurses is important for identification of training strategies and can be regarded as reference for nurse assessment and promotion. The construction of the index system is based on the consensus of infectious disease experts, which is not only helpful to standardize the training strategies and selection standards of infectious disease specialist nurses in the future, but also meet the society’s needs in clinical infectious disease nursing.

Abstract:Abstract Background The performance evaluation of the Centers for Disease Control and Prevention (CDC) is crucial for enhancing the quality of public health services. With the ongoing reform of the CDC system in China, the existing performance evaluation system faces challenges. This study used the Delphi method to develop a new performance evaluation system for China’s provincial, city, and county-level CDC. Methods Following the “Structure-Process-Outcome” model, assessment indicators were systematically collected. Indicators were modified and screened through two Delphi rounds based on CDC responsibilities, health development, and national policies. Twenty-four experts provided ratings and recommendations, and the research team evaluated questionnaire reliability, expert positivity, expert authority, and opinion consistency. Results The preliminary index system identified through the literature review and pre-survey included 11 primary, 30 secondary, and 64 tertiary indicators. After the first round of consultation, two secondary indicators and 11 tertiary indicators were removed and 22 tertiary indicators were added. After the second round of consultation, three secondary indicators and 11 tertiary indicators were removed and three tertiary indicators were added, at which point the p -value of the test for Kendall’s coefficient of concordance W was < 0.001 and the coefficient of variation was within acceptable limits (< 0.25), so the consultation was concluded. The final index system included 11 primary, 25 secondary, and 67 tertiary indicators. Conclusions This study responded to the CDC system reform by developing a comprehensive performance evaluation index system for provincial, city, and county-level CDC in China. The index system is both scientifically grounded and practical, serving as an effective tool for promoting the high-quality work of CDC organizations.

Abstract:Risk assessment plays an essential role in the prevention and control of infectious diseases. A sound index system is critical to obtain accurate risk assessment results. The spread of different types of infectious diseases in various situations has complex influencing factors. Thus, the results of different risk assessment index system of infectious disease transmission could be varying. This paper summarizes the risk assessment index systems of infectious disease transmission established at home and abroad according to the transmission route and the specific situations in which they occur. This paper also quoted China's references to formulate a new index system for risk assessment of infectious disease transmission.

Abstract:Background: A crucial goal of infectious disease surveillance is the early detection of epidemics, which is essential for disease control. In China, the current surveillance system is based on confirmed case reports. In rural China, it is not practical for health units to perform laboratory tests to confirm disease and people are more likely to get 'old' and emerging infectious diseases due to poor living conditions and closer contacts with wild animals and poultry. Syndromic surveillance, which collects non-specific syndromes before diagnosis, has great advantages in promoting the early detection of epidemics and reducing the necessities of disease confirmation. It will be especially effective for surveillance in resource poor settings.Methods/Design: This is a field experimental study. The experimental tool is an innovative electronic surveillance system, combining syndromic surveillance with the existing case report surveillance in four selected counties in China. In the added syndromic surveillance, three types of data are collected including patients' major symptoms from health clinics, pharmaceutical sales from pharmacies and absenteeism information from primary school. In order to evaluate the early warning capability of the new added syndromic surveillance, the timelines and validity of the alert signals will be analyzed in comparison with the traditional case reporting system. The acceptability, feasibility and economic evaluation of the whole integrated surveillance system will be conducted in a before and after study design.Discussions: Although syndromic surveillance system has mostly been established in developed areas, there are opportunities and advantages of developing it in rural China. The project will contribute to knowledge, experience and evidence on the establishment of an integrated surveillance system, which aims to provide early warning of disease epidemics in developing countries.

Abstract:Objective To build a comprehensive screening indicator system of public health dissemination material using Delphi Method.Methods On the basis of the literature review method,Delphi method was applied to finalize the screening indicator system of public health dissemination material by two rounds of investigation done by 18 experts specialized in community health,health education,disease prevention and clinical medicine.Results By two rounds of consulting experts using Delphi Method,the screening indicator system of public health dissemination material was determined.Level 1 had 6 items,Level 2 had 17 items,and Level 3 had 26 items.The coordination factors of the experts in two indicator systems(books and videos) were respectively 0.08 and 0.11 for the first round,0.17 and 0.20 for the second round(P<0.05).The average level of experts' authority was 0.80.Conclusion A comprehensive screening indicator system of public health dissemination material is established using Delphi Method.
Comprehensive evaluation of health effect of notifiable infectious diseases in China

Abstract:Objective To investigate the degree and trend of health hazards of notifiable infectious diseases from 2004 to 2013 and to identify the key infectious diseases in the current and near future in China. This study almed to provide the evidence for the health policy making and health resources allocation. Methods The surveillance data of infectious diseases from 2004 to 2013 were collected from China CDC Information System. The comprehensive index method was used to combine the index mortality and morbidity, and then the health effect index (HEI) was first created to compre-hensively measure the hazard degree of the infectious diseases. Descriptive statistical analysis and regression analysis were used to investigate the characteristics and changing trend of the HEI over the past ten years. Two-dimensional quadrant method was used to identify the key infectious diseases that with high HEI and significant increased trend of the HEI. Results The infectious diseases with top 3 HEI were tuberculosis, viral hepatitis and alDS from 2004 to 2013, of which HEI were 1.29, 1.08 and 0.80 respectively. The infectious diseases, whose HEI increase most signifi-cantly, were hand foot and mouth diseases (HFMD), alDS and syphilis (b=0.17, 0.09, 0.02, P<0.05). Results of two-di-mensional quadrant method showed that alDS in quadrantII(HEI>range/2 and increased significantly), tuberculosis and viral hepatitis in quadrant Ⅳ (HEI>range/2), and HFMD, infectious diarrhea disease and syphilis in quadrant Ⅰ(HEI increased significantly), which were all the key infec-tious diseases in China. Conclusion The comprehensive index method and two-dimensional quadrant method can be used to evaluate and identify health hazard degree effectively. The key infectious diseases in China are tu-berculosis, viral hepatitis, alDS, HFMD, and syphilis in the current and near future.
Establishment of performance assessment index system for control of infection and diseases in hospital departments based on principle of balanced scorecard

Abstract:OBJECTIVE To establish the performance assessment index system for infection control in hospital departments so as to provide a comprehensive, systematic and quantifiable standard for assessment of infection control. METHODS The first draft of index system was drawn up based on literature research, semi-structured interview and group discussion, the questionnaire was compiled. Two rounds of correspondence were conducted with 27 experts in Beijing by Delphi method, and the weights of indexes at all levels were determined by means of analytic hierarchy process. RESULTS The recovery rates of valid questionnaires were 100% for both rounds of the correspondence. The authority coefficients were 0.925 and 0.940, respectively, the judgement bases were 0.936 and 0.944, respectively, and the familiarities were 0.914 and 0.936, respectively.Kendall’s W coordination coefficients of the first-level, second-level and third-level indexes of the second round inquiry were 0.245, 0.342 and 0.437, respectively. The performance assessment index system for infection control in hospital departments that was finally determined included 4 first-level indexes, 12 second-level indexes and 31 third-level indexes. CONCLUSION The performance assessment index system for infection control in hospital departments is established based on the experts′ opinions, the results are scientific and reliable, and it can provide objective and fair assessment for the infection control in the hospital departments.

Abstract:OBJECTIVE To establish the evaluation index system for core competence of infection management professionals so as to provide objective basis for training and evaluation of full-time staff.METHODS From Aug 2016 to Oct 2016,the Delphi expert consultation was conducted to 34 experts who were engaged in control of nosocomial infections,nursing,clinical work,and medical technique in 22 hospitals of 7 cities of China.The indexes were screened out,the index weight was determined by using analytic hierarchy process,and the evaluation index system for core competence of infection management professionals was established.RESULTS After two rounds of consultation,the positive coefficients of experts were 94.44％ and 100.00％,respectively;the authority coefficients were 0.84 ％ and 0.88 ％,respectively.The index system for evaluation of core competence of infection management professionals,including 5 primary indexes,15 secondary indexes,and 71 tertiary indexes,was established after two rounds of expert consultation.CONCLUSION The index system for evaluation of core competence of infection management professionals is scientific and reliable,it can provide quantified bases for appointment,selection,training,assessment,and evaluation of the infection management professionals and set the goals for the infection management professionals to achieve through practice.
Establishment of the Evaluation Index System for Priority Setting for Essential Public Health Programs

Abstract:OBJECTIVE To establish a set of index system for priority setting for essential public health programs.METHODS The improved Delphi method was applied to carry out a three-round consultation.RESULTS Experts'authority coefficients was 0.87,and the corresponding coefficients of experts'consistent opinion were 0.33,0.39 and 0.66,respectively.After experts appraisal,a primary system with 5 indexes for priority setting for essential public health programs was established.Index was disease severity,program's effectiveness,cost-effective,supporting conditions and social effects,and their weights were 26.00%,24.00%,19.00%,15.00% and 16.00%,respectively.CONCLUSIONS The establishment of this appraisal index system took every aspects of public health programs into considerations,moreover,it's easy to be used,and it has a certain value in practical application.

Abstract:Objective To develop a scientific evaluation index system for population-based health promotion.Methods After developing the evaluation index system framework with extensive literature review and summary of practical experience,the index system was constructed with Delphi method,and its feasibility was verified by using field assessment in Yuhang.Results 49 experts were selected into two rounds of Delphi consultation research.The effective response rates of two rounds were 93.88% and 78.26% respectively.The authoritative coefficients were 0.746 and 0.742.Based on the field verification,the evaluation index system was composed of 4 primary indicators including formative,implementation,outcome and sustainability.There were 17 secondary indicators and 77 tertiary indicators thereunder.Conclusion The structure of the evaluation index system of health promotion become basically stabilized,and the consultant experts tend to reach a consensus.The evaluation index system embodies the whole process of health promotion,which is scientific,applicable and feasible to a certain degree.
Establishment and Evaluation of Assessment System of Health Literacy Regarding Respiratory Infectious Diseases

Abstract:Objective To establish an assessment system to evaluate individual health literacy regarding respiratory infectious diseases,by selecting its indexes,confirming indexes' weights and combining with a comprehensive sustainable index model.Methods A quota sampling was adopted on considering factors,including participant's gender,age and education background.515 adults in Beijing took part in the survey conducted by computer assistant.Indexes were selected from original items by difficulty,discrimination,reliability and validity test.Besides,normalization method and variation coefficient approach were applied to identify subjective weights and objective weights.Finally,the assessment system was established with the comprehensive index model.Results 39 indexes which constitute of 3 aspects,namely knowledge,behavior or behavioral intention and relevant skills,were selected to evaluate health literacy regarding respiratory infectious diseases.Difficulty coefficients of all indexes ranged from 0.5 to 0.9,and discrimination coefficients were all indexes were above 0.3.Also,the Cronbach's alpha coefficients were 0.782,0.657 and 0.842 for the three aspects respectively.After assessment system established by adding weights to indexes and aspects,the system demonstrated good internal consistency reliability with a Cronbach's alpha of 0.872.As for conduct validity,3 factors were extracted from all indexes,which explained 54.08% of variance of the system accumulatively with KMO=0.821,and the 3 factors well fitted on confirmatory factor analysis(IFI=0.905,TLI=0.873,CFI=0.905,RMSEA= 0.065).Conclusions The assessment system with good reliability and validity meets aims of the study,and can be well applied to evaluate the status of health literacy regarding respiratory infectious diseases.

Abstract:Objective To construct a set of scientific and systemic evaluation system of the vocational aptitude for nurses,and provide a foundation for the further study on the corresponding assessment tools.Methods The Delphi method was used to conduct two rounds of expert consultation for 30 experts,in order to select and modify the index.Results The recycle rate for two-round questionnaires was 100%,which indicated that experts were active in the study response.The authority coefficient for experts was higher than 0.8,which suggested a high authority among experts.This index system constructed the secondary structure,of which 3 items were first-grade and 10 items were second-grade.Conclusion An assessment system for the vocational aptitude of nurses is initially constructed.
Constructing a Competency Training Index System in Public Health Emergencies for Community Nurses

Abstract:Objective A competency training index system was designed in public health emergencies for community nurses. The purpose of this approach is to provide community nurses with guidelines for standardized training programs on how to respond to public health emergencies. Methods Through semi‐structured interviews, a literature review, and team discussion, the first draft of competency training index system in public health emergencies for community nurses was formed. The Delphi method was used for two rounds of expert consultation to screen, modify and determine indicators. Finally, the analytic hierarchy process was utilized to calculate the weights of indicators at all levels. Results A total of 15 expert consultants were included. The effective recovery rate of the two rounds of expert consultation questionnaire was 100%, the expert authority coefficient was 0.880, 0.887, and the Kendall coordination coefficient was 0.172 and 0.171 (p < 0.001). Expert opinions have determined that the final revised competency index system in public health emergencies for community nurses training consists of 2 primary indicators, 10 secondary indicators, and 46 tertiary indicators. Conclusion The study has developed a competency training index system in public health emergencies for community nurses. This system can serve as a foundation for standardized training in community health service institutions. It is characterized by its high demand, scientific approach, reliability, and rationality.

Hierarchical Model of Multi-level Index to Assess Local Resilience Capability in COVID-19

Abstract:Background Globally, there have been 212,544,565 confirmed cases of Corona Virus disease 2019 (COVID-19), including 4,441,428 deaths by 24 August 2021, reported to WHO. Facing the global pandemic of COVID-19, countries and regions have implemented different policies and taken different non-pharmacological interventions (NPIs) according to their own circumstances. However, the quantitative assessment of national policies and local resilience capabilities is a huge challenge.MethodsIn order to assess interventions and improve local resilience from a comprehensive perspective, this study aims to establish a multi-dimensional and dynamic prevention and control system. The main body of the system is an index system. To make our evaluation system more scientific and useful, the comparative study with several widely used tools or lessons is conducted to report what they have done. Then analytic hierarchy process (AHP) is used to set up the framework under the concept of a multi-level strategy of public health management. Indicators in the system are determined by literature research and expert interviews.ResultsEmergency capability assessment includes building a well-established system, execution of the system, and measurement. The well-established system exhibits several characteristics: 1) considering indicators about whole-of-society involvement, including country-, city-, local community- and individual-level; 2) improving capability at multi-phases, from the preparedness ability to response ability; 3) at both policy level and implementation level. Categories of containment and closure, response in economic system, and response in public health system constitute the main body of the framework. The well-established system does not necessarily apply to all scenarios, and the actual situation should be taken into consideration in the process of implementation/execution. At the stage of measurement, the case of Wuhan/Hubei response is introduced to implement and test our system. Empirical researches will be conducted to verify the index system quantitatively in our future research. ConclusionsOur index system can assess national policies and capabilities quantitatively. When enough data are available, it will become a tool to assess the local resilience capability for countries or regions.

Abstract:Introduction: Infectious diseases pose a significant global health and economic burden, underscoring the critical need for precise predictive models. The Baidu index provides enhanced real-time surveillance capabilities that augment traditional systems. Methods: Baidu search engine data on the keyword "fever" were extracted from 255 cities in China from November 2022 to January 2023. Onset and peak dates for influenza epidemics were identified by testing various criteria that combined thresholds and consecutive days. Results: The most effective scenario for indicating epidemic commencement involved a 90th percentile threshold exceeded for seven consecutive days, minimizing false starts. Peak detection was optimized using a 7-day moving average, balancing stability and precision. Discussion: The use of internet search data, such as the Baidu index, significantly improves the timeliness and accuracy of disease surveillance models. This innovative approach supports faster public health interventions and demonstrates its potential for enhancing epidemic monitoring and response efforts.

At the same time, the Cronbach coefficient and factor analysis were adopted to test the reliability and validity. What’s more, the analytic hierarchy process and the TOPSIS method were applied to analyze the empirical data of 15 counties and cities in the eastern, central, and western regions of China. The constructed indicator system includes 3 first-level indicators, 6 second-level indicators and 29 third-level indicators. Through reliability and validity tests, the stability and practicability of the index system are demonstrated in empirical research. The evaluation index system constructed in this article can be applied to the performance evaluation of Internet + public health service.

Index System Construction and Application Case Analysis for County Health System Development Focusing on Primary Care

Abstract:Background Focusing on primary care is the primary content of health care policies, however, there is a lack of effective measurement tools in practice. Objective To construct a index system for county health system development focusing on primary care, provide a reference for its monitoring approaches, and perform analysis and presentation with application cases. Methods In the study conducted from 2021 to 2022, the index system, weights and percentage conversion methods were constructed based on literature review and Delphi method, case application and analysis were conducted based on the data of questionnaire survey, institutional survey and medical insurance collected by baseline survey among residents, patients and medical workers, as well as data of key informant interview, in 2 experimental counties of county health system reform in Guangxi Zhuang Autonomous Region from 2020 to 2022. Results The results of Delphi method showed that the authority coefficient of experts was 0.91, the coefficients of variation of two rounds of consultation were 0.14 and 0.13, respectively. The final constructed index system contains 3 domains, 15 dimensions, and 36 indicators, with a comprehensive index calculation method. The results showed that the overall scores for the two counties in Guangxi Zhuang Autonomous Region were 58.62 and 52.57, respectively, both of which were below the current national benchmark (60.00 points) . Conclusion Based on the data availability, the index system constructed in this study can be applied to monitor and evaluate county health system to achieve the goal of focusing on primary care, its application may improve visibility and priority of primary care development, facilitate the publish and implementation policies strengthening primary care.
