Abstract:Background: The UK is legally committed to reduce its greenhouse gas emissions to net zero by 2050. We aimed to understand the potential impact on population health of two pathways for achieving this target through the integrated effects of six actions in four sectors. Methods: In this multisectoral modelling study we assessed the impact on population health in England and Wales of six policy actions relating to electricity generation, transport, home energy, active travel, and diets relative to a baseline scenario in which climate actions, exposures, and behaviours were held constant at 2020 levels under two scenarios: the UK Climate Change Committee's Balanced Pathway of technological and behavioural measures; and its Widespread Engagement Pathway, which assumes more substantial changes to consumer behaviours. We quantified the impacts of each policy action on mortality using a life table comprising all exposures, behaviours, and health outcomes in a single model. Findings: Both scenarios are predicted to result in substantial reductions in mortality by 2050. The Widespread Engagement Pathway achieves a slightly greater reduction in outdoor fine particulate matter air pollution of 3·2 μg/m3 (33%) and, under assumptions of appropriate ventilation, a greater improvement in indoor air pollution (a decrease in indoor-generated fine particulate matter from 9·4 μg/m3 to 4·6 μg/m3) and winter temperatures (increasing from 17·8°C to 18·1°C), as well as appreciably greater changes in levels of active travel (27% increase in metabolic equivalent hours per week of walking and cycling) by 2050. Additionally, the greater reduction in red meat consumption (50% compared with 35% under the Balanced Pathway) by 2050 results in greater consumption of fruits (17-18 g/day), vegetables (22-23 g/day), and legumes (5-7 g/day). Combined actions under the Balanced Pathway result in more than 2 million cumulative life-years gained over 2021-50; the estimated gain under the Widespread Engagement Pathway is greater, corresponding to nearly 2·5 million life-years gained by 2050 and 13·7 million life-years gained by 2100. Interpretation: Reaching net zero greenhouse gas emissions is likely to lead to substantial benefits for public health in England and Wales, with the cumulative net benefits being correspondingly greater with a pathway that entails faster and more ambitious changes, especially in physical activity and diets. Abstract:Nitrogen dioxide (NO2) is associated with mortal-ity and many other adverse health outcomes. In 2021, the World Health Organization established a new NO2 air quality guideline (AQG) (annual average <10 mu g/m3). However, the burden of diseases attributable to long-term NO2 exposure above the AQG is unknown in China. Nitrogen oxide is a major air pollutant in populous cities, which are disproportionately impacted by NO2; this represents a form of environmental inequality. We conducted a nationwide risk assessment of premature deaths attributable to long-term NO2 exposure from 2013 to 2020 based on the exposure-response relationship, high-resolution annual NO2 concentrations, and gridded population data (considering sex, age, and residence [urban vs rural]). We calculated health metrics including attributable deaths, years of life lost (YLL), and loss of life expectancy (LLE). Inequality in the distribution of attributable deaths and YLLs was evaluated by the Lorenz curve and Gini index. According to the health impact assessments, in 2013, long-term NO2 exposure contributed to 315,847 (95% confidence interval [CI]: 306,709-319,269) premature deaths, 7.90 (7.68-7.99) million YLLs, and an LLE of 0.51 (0.50-0.52) years. The high-risk subgroup (top 20%) accounted for 85.7% of all NO2-related deaths and 85.2% of YLLs, resulting in Gini index values of 0.81 and 0.67, respectively. From 2013 to 2020, the estimated health impact from NO2 exposure was significantly reduced, but inequality displayed a slightly increasing trend. Our study revealed a considerable burden of NO2-related deaths in China, which were disproportionally frequent in a small high-risk subgroup. Future clean air initiatives should focus not only on reducing the average level of NO2 exposure but also minimizing inequality.

Abstract:Air pollution, greenspace and noise are interrelated environmental factors with the potential to influence human health outcomes. Research has measured these exposures in diverse ways across the globe, but no study has yet performed a country-wide analysis of air pollution, greenspace, and noise in England. This study examined cross-sectional PM2.5, NO2, greenness, accessible greenspace, road noise, and rail noise exposure data at all residential postcodes in England (n = 1,227,681). Restricted cubic spline models were fitted between each environmental exposure and a measure of socioeconomic status, the Index of Multiple Deprivation (IMD) rank. Population-weighted exposures by IMD deciles, urbanicity, and region were subsequently estimated. Restricted cubic spline models were also fitted between greenness and each other environmental exposure in the study. The results show some evidence of inequalities in exposure to air pollutants, greenspace, and noise across England. Notably, there is a socioeconomic gradient in greenness, NO2, PM2.5, and road noise in London. In addition, NO2, PM2.5, and road noise exposure decrease as greenness increases in urban areas. Concerningly, almost all air pollution estimates in our study exceed international health guidelines. Further research is needed to elucidate the socioeconomic patterns and health impacts of air pollution, greenspace, and noise over time.

Abstract:Deprived communities in many cities are exposed to higher levels of outdoor air pollution, and there is increasing evidence of similar disparities for indoor air pollution exposure. There is a need to understand the drivers for this exposure disparity in order to develop effective interventions aimed at improving population health and reducing health inequities. With a focus on London, UK, this paper assembles evidence to examine why indoor exposure to PM2.5, NOx and CO may disproportionately impact low-income groups. In particular, five factors are explored, namely: housing location and ambient outdoor levels of pollution; housing characteristics, including ventilation properties and internal sources of pollution; occupant behaviours; time spent indoors; and underlying health conditions. Evidence is drawn from various sources, including building physics models, modelled outdoor air pollution levels, time-activity surveys, housing stock surveys, geographical data, and peer-reviewed research. A systems framework is then proposed to integrate these factors, highlighting how exposure to high levels of indoor air pollution in low-income homes is in large part due to factors beyond the control of occupants, and is therefore an area of systemic inequality.POLICY RELEVANCE: There is increasing public and political awareness of the impact of air pollution on public health. Strong scientific evidence links exposure to air pollution with morbidity and mortality. Deprived communities may be more affected, however, with limited evidence on how deprivation may influence their personal exposure to air pollution, both outdoors and indoors. This paper describes different factors that may lead to low-income households being exposed to higher levels of indoor air pollution than the general population, using available data and models for London (i.e. living in areas of higher outdoor air pollution, in poor-quality housing, undertaking more pollution-generating activities indoors and spending more time indoors). A systems approach is used to show how these factors lead to systemic exposure inequalities, with low-income households having limited opportunities to improve their indoor air quality. This paper can inform actions and public policies to reduce environmental health inequalities, considering both indoor and outdoor air.

Abstract:Road traffic has become the leading source of air pollution in fast-growing sub-Saharan African cities. Yet, there is a dearth of robust city-wide data for understanding space-time variations and inequalities in combustion related emissions and exposures. We combined nitrogen dioxide (NO2) and nitric oxide (NO) measurement data from 134 locations in the Greater Accra Metropolitan Area (GAMA), with geographical, meteorological, and population factors in space-time mixed effects models to predict NO2 and NO concentrations at fine spatial (50 m) and temporal (weekly) resolution over the entire GAMA. Model performance was evaluated with 10-fold cross-validation, and predictions were summarized as annual and seasonal (Harmattan vs non-Harmattan) mean concentrations. The predictions were used to examine population level distribution of, and socioeconomic inequalities in, exposure at the census enumeration area. The models explained 88% and 79% of the spatiotemporal variability in NO2 and NO concentrations, respectively. The mean predicted annual, non-Harmattan and Harmattan NO2 levels were 37 (range: 1-189), 28 (range: 1-170) and 50 (range: 1-195) μg/m3, respectively. Unlike NO2, NO concentrations were highest in the non-Harmattan season (41 [range: 31-521] μg/m3). Road traffic was the dominant factor for both pollutants, but NO2 had much higher spatial heterogeneity than NO. For both pollutants, the levels were much higher in the city core, where the entire population (100%) was exposed to annual NO2 levels exceeding the WHO guideline of 10 μg/m3. Significant disparity in NO2 concentrations existed across socioeconomic gradient, with residents in the poorest communities exposed to levels about 15 μg/m3 higher compared to the richest (p < 0.001). The results showed the important role of road traffic emissions in air pollution exposure in the GAMA, with major implication for the health of the city poor. The data could support climate and health impact assessment as well as policy evaluation in the city.

Abstract:The society has high concerns on the inequality that people are disproportionately exposed to ambient air pollution, but with more time spent indoors, the disparity in the total exposure considering both indoor and outdoor exposure has not been explored; and with the socioeconomical development and efforts in fighting against air pollution, it is unknown how the exposure inequality changed over time. Based on the city-level panel data, this study revealed the Concentration Index (C) in ambient PM2.5 exposure inequality was positive, indi-cating the low-income group exposed to lower ambient PM2.5; however, the total PM2.5 exposure was negatively correlated with the income, showing a negative C value. The low-income population exposed to high PM2.5 associated with larger contributions of indoor exposure from the residential emissions. The total PM2.5 exposure caused 1.13 (0.63-1.73) million premature deaths in 2019, with only 14 % were high-income population. The toughest-ever air pollution countermeasures have reduced ambient PM2.5 exposures effectively that, however, benefited the rich population more than the others. The transition to clean household energy sources signifi-cantly affected on indoor air quality improvements, as well as alleviation of ambient air pollution, resulting in notable reductions of the total PM2.5 exposure and especially benefiting the low-income groups. The negative C values decreased from 2000 to 2019, indicating a significantly reducing trend in the total PM2.5 exposure inequality over time.

Abstract:In the pursuit of carbon neutrality, China's 2060 targets have been largely anchored in reducing greenhouse gas emissions, with less emphasis on the consequential benefits for air quality and public health. This study pivots to this critical nexus, exploring how China's carbon neutrality aligns with the World Health Organization's air quality guidelines (WHO AQG) regarding fine particulate matter (PM2.5) exposure. Coupling a technology-rich integrated assessment model, an emission-concentration response surface model, and exposure and health assessment, we find that decarbonization reduces sulfur dioxide (SO2), nitrogen oxides (NOx), and PM2.5 emissions by more than 90%; reduces nonmethane volatile organic compounds (NMVOCs) by more than 50%; and simultaneously reduces the disparities across regions. Critically, our analysis reveals that further targeted reductions in air pollutants, notably NH3 and non-energy-related NMVOCs, could bring most Chinese cities into attainment of WHO AQG for PM2.5 5 to 10 years earlier than the pathway focused solely on carbon neutrality. Thus, the integration of air pollution control measures into carbon neutrality strategies will present a significant opportunity for China to attain health and environmental equality.

Abstract:Air pollution generates substantial health damages and economic costs worldwide. Pollution exposure varies greatly, both between countries and within them. However, the degree of air quality inequality and its' trajectory over time have not been quantified at a global level. Here I use economic inequality indices to measure global inequality in exposure to ambient fine particles with 2.5 microns or less in diameter (PM2.5). I find high and rising levels of global air quality inequality. The global PM2.5 Gini Index increased from 0.32 in 2000 to 0.36 in 2020, exceeding levels of income inequality in many countries. Air quality inequality is mostly driven by differences between countries and less so by variation within them, as decomposition analysis shows. A large share of people facing the highest levels of PM2.5 exposure are concentrated in only a few countries. The findings suggest that research and policy efforts that focus only on differences within countries are overlooking an important global dimension of environmental justice.

Abstract:Summary Background Air pollution contributes to a large disease burden and some populations are disproportionately exposed. We aimed to evaluate ethnic and socioeconomic differences in exposure to air pollution in the Netherlands. Methods We did a nationwide, cross-sectional analysis of all residents of the Netherlands on Jan 1, 2019. Sociodemographic information was centralised by Statistics Netherlands and mainly originated from the National Population Register, the tax register, and education registers. Concentrations of NO 2 , PM 2·5 , PM 10 , and elemental carbon, modelled by the National Institute for Public Health and the Environment, were linked to the individual-level demographic data. We assessed differences in air pollution exposures across the 40 largest minority ethnic groups. Evaluation of how ethnicity intersected with socioeconomic position in relation to exposures was done for the ten largest ethnic groups, plus Chinese and Indian groups, in both urban and rural areas using multivariable linear regression analyses. Findings The total study population consisted of 17 251 511 individuals. Minority ethnic groups were consistently exposed to higher levels of air pollution than the ethnic Dutch population. The magnitude of inequalities varied between the minority ethnic groups, with 3–44% higher exposures to NO 2 and 1–9% higher exposures to PM 2·5 compared with the ethnic Dutch group. Average exposures were highest for the lowest socioeconomic group. Ethnic inequalities in exposure remained after adjustment for socioeconomic position and were of similar magnitude in urban and rural areas. Interpretation The variability in air pollution exposure across ethnic and socioeconomic subgroups in the Netherlands indicates environmental injustice at the intersection of social characteristics. The health consequences of the observed inequalities and the underlying processes driving them warrant further investigation. School neighbourhood and compliance with WHO-recommended annual NO2 guideline: A case study of Greater London

Abstract:Despite several national and local policies towards cleaner air in England, many schools in London breach the WHO-recommended concentrations of air pollutants such as NO2 and PM2.5. This is while, previous studies highlight significant adverse health effects of air pollutants on children's health. In this paper we adopted a Bayesian spatial hierarchical model to investigate factors that affect the odds of schools exceeding the WHO-recommended concentration of NO2 (i.e., 40 μg/m3 annual mean) in Greater London (UK). We considered a host of variables including schools' characteristics as well as their neighbourhoods' attributes from household, socioeconomic, transport-related, land use, built and natural environment characteristics perspectives. The results indicated that transport-related factors including the number of traffic lights and bus stops in the immediate vicinity of schools, and borough-level bus fuel consumption are determinant factors that increase the likelihood of non-compliance with the WHO guideline. In contrast, distance from roads, river transport, and underground stations, vehicle speed (an indicator of traffic congestion), the proportion of borough-level green space, and the area of green space at schools reduce the likelihood of exceeding the WHO recommended concentration of NO2. We repeated our analysis under a hypothetical scenario in which the recommended concentration of NO2 is 35 μg/m3 – instead of 40 μg/m3. Our results underscore the importance of adopting clean fuel technologies on buses, installing green barriers, and reducing motorised traffic around schools in reducing exposure to NO2 concentrations in proximity to schools. Also, our findings highlight the presence of environmental inequalities in the Greater London area. This study would be useful for local authority decision making with the aim of improving air quality for school-aged children in urban settings.

Abstract:Income inequality and environmental pollution are of great concern in China. It is important to better understand whether the narrowing of income inequality and environmental improvement contradict each other. The study aims to investigate the linkage between income inequality and environmental pollution. To illustrate the interplay between different income groups on environmental issues, we apply a mixed-strategy game. Based on the game-theoretic analytical result, the probability of residents supporting clean energy and environmental protection decreases as income inequality widens and increases as inequality narrows. This empirical study is based on the proportion of coal consumption and urban air pollution data from 113 key environmental protection cities and regions in China. The air quality data are from the National Environmental Air Quality Monitoring Network published in the China Statistical Yearbook from 2014–2018. Convincing results show that regions with higher income inequality suffer severe smog and related pollution and that economies with narrow income disparity experience significant improvements in smog and pollution control, with the expansion of the proportion of clean energy use. The results also provide no evidence of the impact of per capita income on pollution. We studied the relationship between individuals of different wealth levels within an economy, within a repeated-game setting. The finding suggests that the distribution of growth impacts pollution. Imposing higher taxes on air polluters while transferring the revenue to the lower-income group is suggested.

Abstract:To meet the goal of the Paris Agreement, China pledges to realize the "Dual Carbon" targets by 2060. As the capital of China, Beijing plays a leading role in becoming zero-emission or carbon neutral in the future. We project the pollutants emissions of building sector based on current strict clean air policies (PO scenario) and China's carbon neutrality target by 2060 (CN scenario) from 2019 to 2050. Results show that PM 2.5 concentration will increase by 2.62 μg/m 3 under PO scenario; under the CN scenario, ozone concentration will increase by 2.53 μg/m 3 but PM 2.5 concentration will reduce by 9.04 μg/m 3 . It is projected that China carbon neutrality goals could avoid 11.12% of PM 2.5 -related health burden; With strict clean air policies, health burdens of ozone (3.9%) and PM 2.5 (4.1%) could be avoided, respectively. This study highlights the importance of achieving co-benefits of air quality and public health.

Abstract:This article contains a slightly expanded version of the lectures given by the author at the summer school "Algebraic stacks and related topics" in Mainz, Germany from August 31 to September 4, 2015. The content of these lectures is purely expository and consists of two main goals. First, we provide a treatment of Artin's approximation and algebraization theorems following the ideas of Conrad and de Jong which rely on a deep desingularization result due to Neron and Popescu. Second, we prove that under suitable hypotheses, algebraic stacks are etale locally quotients stacks in a neighborhood of a point with a linearly reductive stabilizer.
Changes in PM2.5-related health burden in China's poverty and non-poverty areas during 2000–2020: A health inequality perspective

Abstract:China suffers from severe PM2.5 pollution that has resulted in a huge health burden. Such PM2.5-related health burden has long been suspected to differ between China's poverty-stricken areas (PAs) and non-poverty-stricken areas (NPAs). Yet, evidence-based examination of this long-held belief, which is critical as a barrier of environmental injustice to advancing China's sustainability, is still missing. Here our study shows that the PM2.5 pollution is more serious in China's NPAs than PAs-with their annual averages being respectively 54.83 μg/m3 and 43.63 μg/m3-causing higher premature mortality in the NPAs. Compared to economic inequality, China's total PM2.5-related premature mortality was relatively evenly distributed during 2000-2015 across regions of varying levels of gross domestic product (GDP) per capita but increased slightly in 2015-2020 owing to the dramatic change in age structure. The elderly population increased by 31 %. PM2.5-related premature deaths were more severe for populations of low socioeconomic status, and such environmental health inequalities could be amplified by population aging. Additionally, population migration from China's PAs to developed cities contributed to 638, 779, 303, 954, and 896 premature deaths in 2000, 2005, 2010, 2015, and 2020, respectively. Changes in the age structure (53 %) and PM2.5 concentration (28 %) had the greatest impact on premature deaths, followed by changes in population (12 %) and baseline mortality (8 %). The contribution rate of changes in the age structure and PM2.5 concentration was higher in PAs than in NPAs. Our findings provide insight into PM2.5-related premature death and environmental inequality, and may inform more equitable clean air policies to achieve China's sustainable development goals.

Abstract:China is currently in a crucial stage of air pollution control and has intensive clean air policies. Past strict policies have demonstrated remarkable effectiveness in emission control and fine particulate matter (PM2.5) pollution mitigation; however, it is not clear what the continuous benefits of current policies are for the future. Here, we summarize China's currently implemented, released, and upcoming clean air policies and estimate the air quality and health benefits of the implementation of these policies until 2030. We found that China's current and upcoming clean air policies could reduce major pollutant emissions by 14.3-70.5% under continued socio-economic growth from 2010 to 2030. These policies could decrease the national population-weighted PM2.5 concentrations from 61.6 μg m-3 in 2010 to 26.4 μg m-3 in 2030 (57.2% reduction). These air quality improvements will ensure that over 80% of the population lives in areas with PM2.5 levels below the current annual PM2.5 air quality standard (i.e., 35 μg m-3) and will avoid 95.0 (95% CI, 76.3, 104.2) thousand premature deaths in 2030. We also point out several inadequacies of current clean air policies, suggesting that more ambitious control actions are needed to better protect public health with an increasing ageing population. Our findings could provide quantitative insights that can be used to better address air pollution issues in China and other developing countries.

Abstract:With the increasing number of diagnosed diabetics in the general population, dental health care providers can anticipate a greater number of diabetics in their practices. Familiarity with the systemic and oral signs and symptoms of DM may facilitate its diagnosis, and contribute to the quality of the individual's overall health. It is also important to be aware of the medications used to manage DM and the effect the disease and its treatment may have on dental treatment and scheduling of appointments. The dental provider may also be confronted with emergency management of the diabetic, with the most common condition being hypoglycemia. This paper has also reviewed the clinical signs and symptoms of this state and approaches to management.

Abstract:Abstract Clean air policies in China have substantially reduced particulate matter (PM2.5) air pollution in recent years, primarily by curbing end-of-pipe emissions. However, reaching the level of the World Health Organization (WHO) guidelines may instead depend upon the air quality co-benefits of ambitious climate action. Here, we assess pathways of Chinese PM2.5 air quality from 2015 to 2060 under a combination of scenarios that link global and Chinese climate mitigation pathways (i.e. global 2°C- and 1.5°C-pathways, National Determined Contributions (NDC) pledges and carbon neutrality goals) to local clean air policies. We find that China can achieve both its near-term climate goals (peak emissions) and PM2.5 air quality annual standard (35 μg/m3) by 2030 by fulfilling its NDC pledges and continuing air pollution control policies. However, the benefits of end-of-pipe control reductions are mostly exhausted by 2030, and reducing PM2.5 exposure of the majority of the Chinese population to below 10 μg/m3 by 2060 will likely require more ambitious climate mitigation efforts such as China's carbon neutrality goals and global 1.5°C-pathways. Our results thus highlight that China's carbon neutrality goals will play a critical role in reducing air pollution exposure to the level of the WHO guidelines and protecting public health.

Abstract:Disparities in outdoor air pollution exposure between individuals of differing socio-economic status is a growing area of research, widely explored in the environmental health literature. However, in developed countries, around 80% of time is spent indoors, meaning indoor air pollution may be a better proxy for personal exposure. Building characteristics - such as build quality, volume and ventilation - and occupant behaviour, mean indoor air pollution may also vary across socio-economic groups, leading to health inequalities. Much of the existing literature has focused on inequalities in exposure to outdoor air pollution, and there is thus a lack of an evidence base reviewing data for indoor environments. In this study, a scoping review of the literature on indoor air pollution exposures across different socio-economic groups is performed, examining evidence from both monitoring and modelling studies in the developed world. The literature was reviewed, identifying different indoor pollutants, definitions for socio-economic status and pre- and post- housing interventions. Based on the review, the study proposes a modelling methodology for evaluating the effects of environmental policies on different socio-economic populations. Using a sample size calculation, obstacles in obtaining sufficiently large samples of monitored data are demonstrated. A modelling framework for the rapid quantification of daily home exposure is then outlined as a proof of concept. While significant additional research is required to examine inequalities in indoor exposures, modelling approaches may provide opportunities to quantify exposure disparities due to housing and behaviours across populations of different socio-economic status.
