Abstract:Objective: Unicompartmental knee arthroplasty (UKA) revision is usually due to the degenerative degree of knee articular osteochondral tissue in the untreated compartment. However, it is difficult to simulate the biomechanical behavior on this tissue accurately. This study presents and validates a reliable system to predict which osteoarthritis (OA) patients may suffer revision as a result of biomechanical reasons after having UKA.Design: We collected all revision cases available (n = 11) and randomly selected 67 UKA cases to keep the revision prevalence of almost 14%. All these 78 cases have been followed at least 2 years. An elastic model is designed to characterize the biomechanical behavior of the articular osteochondral tissue for each patient. After calculated the force on the tissue, finite element method (FEM) is applied to calculating the strain of each tissue node. Kernel Ridge Regression (KRR) method is used to model the relationship between the strain information and the risk of revision. Therefore, the risk of UKA revision can be predicted by this integrated model.Results: Leave-one-out (LOO) cross-validation (CV) is implemented to assess the prediction accuracy. As a result, the mean prediction accuracy is 93.58% for all these cases, demonstrating the high value of this model as a decision-making assistant for surgical plaining of knee OA.Conclusions: The results of this study demonstrated that this integrated model can predict the risk of UKA revision with theoretically high accuracy. It combines bio-mechanical and statistical learning approach to create a surgical planning tool which may support clinical decision in the future. Abstract:Unicompartmental knee arthroplasty (UKA) has gained progressive popularity in recent decades, currently comprising approximately 10% of knee arthroplasties in the United States. Nonetheless, UKA has not yet solidified its position as the superior treatment for isolated compartment osteoarthritis, as initially reported implant survivorship was subpar, leading to hesitation in its utilization and stricter patient indications compared to total knee arthroplasty. Patient satisfaction following knee arthroplasty has emerged as a critical metric to gauge patient acceptance and contentment with surgical interventions. Currently, a variety of UKA types exist, differing in bearing design, fixation techniques such as cementless or cemented fixation, and robotic-assisted systems, each with its own merits and drawbacks. Multiple studies have demonstrated the contributions of these innovations to improve clinical outcomes and implant survivorship. However, the abundance of studies has made it challenging to establish a clear overview. This paper provides an overview of the current concepts of UKA, evaluating various aspects of UKA referencing to patient satisfaction and providing a recap of its historical development. Available research demonstrated no significantly universal superior variant of UKA.

Abstract:The preparation of this paper was instigated by the statement of a 75-yearold patient (retired by the Treasury Department because of age, but actively practicing law) that he was frequently taken to be 50 or 60 because his face was free of wrinkles, and his actions were those of a man of that age. Also, he had noticed that all the elderly post-prostatectomy patients whom he had seen in my office when he came for his weekly testosterone injections, had no wrinkles. I n a recent issue of Science the obituary of V. Korenchesky, “Father of Gerontology,” stated that during the RussoJapanese war he visited a Russian infirmary for old people in Moscow and was appalled by their premature and abnormal aging. He tried to estimate the human wastage by comparing these unfortunates with others who enjoyed healthier, more useful, and longer lives, and pictured to himself what these old people might have been. His life during the next fifty-three years was one of striving to discover how people can best realize their full potentialities in their later years. For over forty years he devoted himself mainly to a study of the sex and thyroid hormones. He became primarily interested in the sex hormones, and in his laboratory studies he observed that they had manifold effects on somatic tissues and probably on the process of aging. He investigated the effect of gonadectomy on aging, and was particularly interested in testosterone propionate. I n the January 1962 Journal of the American Geriatrics Society there is an article by Dr. R. A. Shipley on “The Gonads and Aging,” in which he refers to many articles in the literature since 1938 reporting the value of testosterone for increasing muscular strength, endurance and weight in the aged, over short periods of time; but he states that claims regarding permanent dramatic rejuvenation have not been authenticated, as there has been no long-term study. This paper supplements that report since i t presents clinical observations made over a long period of time and involves a comparison of the findings before and after the introduction of testosterone. It also emphasizes the necessity for long-term observations with the use of adequate dosages, to obviate the misleading results of studies conducted for short periods with insufficient dosages.

Abstract:Unicompartmental knee arthroplasty (UKA), an alternative technology that has been prevailed upon Orthope?dics which was proven by time that it's not only an effective treatment for patients with anterior medial knee osteoarthritis, addi?tionally, it plays an important role in the staged treatment of knee osteoarthritis. The advantage of UKA is that patients would re?ceive minimal invasion and faster recovery and it is more suitable for elderly patients. Nevertheless, UKA was also an advisable choice of young patients due to its maximum retention of the knee joint sensation, with a reflect on higher post operation score on FJS (forgetting joint score). The ideal clinical efficacy of unicompartmental knee arthroplasty is relied on the improvements on the latest artificial joint design and surgical skill approaches, and as a consequence, indications and contraindications for UKA have renovated. A subset of patients who were previously thought to be contraindicated to perform UKA (such as medial patellofemoral joint mild arthritis) can be reassessed and performed UKA today which called relative contraindications or Non?essential contrain?dications. The main objective of this article is to provide a new concept of UKA to surgeons, and to provide the best treatment op?tions for patients with different complications according to the latest preoperative evaluation criteria. According to our experience, it is recommended that surgeons should accumulate a certain amount of total joint replacement experience and then to carry out UKA surgery, which will achieve better results.

Abstract:Unicompartmental knee arthroplasty (UKA) is in many ways the ultimate kinematic operation, as the express aim is to resurface the diseased side of the joint and restore pre-arthritic alignment and balance while maintaining integrity of both cruciate ligaments. An increasing body of knowledge relates the outcomes of UKA to pre-arthritic anatomy rather than an arbitrarily defined neutral. The Coronal Plane Alignment of the Knee (CPAK) classification provides a validated technique for calculating pre-arthritic limb alignment (the arithmetic hip-knee-ankle angle (aHKA)) and joint line obliquity (JLO) and will enable a greater understanding of the interactions between pre-arthritic anatomy, choice of prosthetic position and outcomes. When pre-arthritic alignment is not taken into consideration a post-operative limb alignment of mild to moderate varus for medial UKA and moderate valgus for lateral UKA appears to produce the best outcomes. When pre-arthritic anatomy is taken into account, superior results have been reported with restoration of pre-arthritic limb alignment and joint line obliquity. Restriction boundaries have yet to be clearly defined for tibial component coronal and hip-knee-ankle (HKA) angles when applying this new paradigm, but existing evidence would suggest a 6° varus limit for the tibial coronal angle may be a reasonable starting point. Lateral UKA has inherent differences in terms of tibial component positioning and ligament balance targets. Mobile bearing UKA demands a three-dimensional understanding of the effect of implant position on bearing stability. Modification of technique is necessary to produce anatomic tibia component angles with equipment designed for mechanical alignment. Robotic technology allows accurate understanding of pre-arthritic anatomy, precise reproduction of patient specific virtual planning, equally precise manipulation of soft tissue balance, and future research using these platforms is likely to further clarify in terms of ideal patient-specific component and limb alignment targets.

Abstract:Unicompartmental knee arthroplasty (UKA) faces significant challenges, including lower survival rates and higher revision rates than total knee arthroplasty (TKA). To address these issues, technological advancements like custom-made implants (CMI), patient-specific alignment (PSA), and computer-assisted systems (CAS) are being explored. These innovations aim to tailor procedures to individual joint morphology, soft tissue balance, and limb alignment, moving away from the traditional "one size fits all" approach. Early studies suggest that CMI may improve survival rates and patient-reported outcomes, though conclusive evidence is lacking. PSA shows potential for restoring pre-surgical alignment; however, its long-term benefits are uncertain. CAS improves implant placement precision and ligament balance; nevertheless, long-term survival data remain inconclusive. Moreover, economic and implementation challenges, such as cost and the need for specialized training, remain underexplored. While promising, further research is needed to fully understand the long-term efficacy and practical application of these technologies in UKA.
Decision-making in unicompartmental knee arthroplasty using radiological parameters in South Asian populations

Abstract:Background: Many patients who visit orthopedic surgeons mainly complained of knee pain, which is often diagnosed as osteoarthritis affecting the medial compartment, whereas the lateral compartment and patello-femoral joint remain relatively unaffected. AIM: This study aimed to establish criteria for patient selection and validate an evidence-based approach for selecting candidates for unicompartmental knee arthroplasty (UKA). Key considerations in patient selection for UKA include identifying the presence of bone-on-bone osteoarthritis in the medial compartment, ensuring a functionally normal anterior cruciate ligament, maintaining full-thickness cartilage in the lateral compartment, verifying a functionally normal medial collateral ligament, and confirming the absence of severe damage lateral to the patello-femoral joint. MATERIALS AND METHODS: From a consecutive cohort of 390 patients with medial knee pain, preoperative radiographs of bilateral knee including anteroposterior/lateral/Rosenberg/20° valgus stress views were collected, and results were tabulated. Patients were categorized into appropriate groups. The suitability for UKA was determined based on the Oxford radiological decision aid, history, examination, and radiographic assessment including stress radiographs. RESULTS: The Oxford radiological decision aid demonstrated 92% sensitivity and 88% specificity. According to the radiographic assessment, 49% of the knees were considered suitable for Oxford UKA (OUKA), whereas 51% were deemed unsuitable. Among the 51 knees identified as unsuitable for OUKA, 40% did not meet one radiographic criterion, 38% did not meet two criteria, 22% did not meet three criteria, and 1% did not meet four criteria. CONCLUSION: The Oxford radiographic decision aid safely and reliably identifies the appropriate patients for meniscal-bearing UKA and achieves good results in this population. The widespread use of this radiological decision aid should improve the results of UKA.

Abstract:Patients with isolated medial gonarthrosis benefit from a treatment with a unicompartmental knee arthroplasty (UKA) compared to a total knee arthroplasty (TKA). They have a shorter rehabilitation time, better function, lower morbidity and a higher satisfaction rate. However, the revision rate is higher than in patients with TKA. Patient selection and the resulting incorrect indication may be one reason. Hamilton et al.'s research group prepared a radiological "decision aid" for the indication of a UKA. The aim of this study was to validate this uncomplicated diagnostic procedure based on intraoperative findings. In this prospective study, 85 patients received routinely conventional x-rays. Strictly lateral and anterior-posterior view as well as varus/valgus stress x-rays were taken. Using Hamilton et al.'s "desiscion aid", the x-ray images were preoperatively evaluated with regard to medial and lateral cartilage damage and the function of the anterior cruciate ligament. The cartilage damage of the individual tibial and femoral compartments was also scaled and documented intraoperatively. The results of the decision aid were validated with regard to the intraoperative findings. Indication-justifying cartilage damage was defined as focal complete cartilage loss. The sensitivity (SEN) and specificity (SPE) of decision aid with regard to cartilage damage in the medial and lateral compartments and the functionality of the anterior cruciate ligament were determined. In the area of the medial compartment, decision aid has a SEN and PES of 79.4% and 82.4% respectively. The SEN and SPE for lateral compartments are 98.5% and 55.0%, respectively. Intact ACLs are correctly diagnosed at 94.7%. Insufficient ACLs at 33.3%. These data suggest that according to our findings, only about 2 out of 10 patients are not reliably diagnosed with medial gonarthrosis. However, about 2 out of 10 patients are diagnosed with medial gonarthrosis, which does not yet have to be treated with UKA. Furthermore, it could be shown that almost all patients with a completely laterally preserved cartilage are also recognized as such. However, every second patient with relevant cartilage damage is not radiologically identified. These data refer only to the radiological findings. Coupled with the patient's medical history and clinic, the "Desicion Aid" is a very good aid for the correct indication for a UKA.

Abstract:Knee osteoarthritis is one of the most common joint diseases worldwide, which is characterized by knee pain and structural changes, leading to resulting in disability, diminished quality of life, and financial burden. [ 1 ] Joint replacement surgery is the primary surgical treatment for knee osteoarthritis at present. Compared to traditional total knee arthroplasty, unicompartmental knee arthroplasty (UKA) offers advantages such as faster postoperative recovery, improved postoperative range of motion, preservation of bone stock, lower mortality rate, and fewer complications. [ 2 , 3 ] Despite various advantages, higher revision surgery rates continues to challenge patients who treated with UKA. [ 4 ] Therefore, decrease the risk of revision surgery for patients who underwent UKA has become an important issue to be solved urgently. [ 5 ]

Abstract:Background Almost 50% of the patients requiring knee replacement are suitable for medial unicompartmental knee arthroplasty (UKA). However, national registries have shown a use far below 50%. Aim The aim of this study was to identify the factors that could influence the use of UKA. Methods A questionnaire was conducted among practicing knee surgeons of the Dutch Orthopedic Association. The questionnaire was classified into three domains: surgeon, patient selection, and professional opinion. Associations between the domains and UKA volume and contra-indications were tested using Pearson's chi-square tests. Results Of the 113 included respondents, 40% had no hands-on exposure during residency. Frequently reported contra-indications were obesity (68%), lateral osteophytes (21%), chondrocalcinosis (20%), and age (20%). Medial partial thickness lesions were considered a contra-indication by 49% and lateral joint space narrowing by 61% of the respondents. Respondents perceived UKA to outperform total knee arthroplasty regarding all outcomes, except survival (1.8% versus 42%). The reported barriers for UKA use were insufficient experience (20%), high revision rates (20%), and low hospital volume (16%), whereas 60% did not experience any at all. Hands-on UKA exposure during residency was associated with an increased use of correct indications. Furthermore, the use of correct indications was associated with an increased UKA volume. Conclusion We showed a low UKA exposure during residency and considerable variations in contra-indications. These factors may contribute to an underuse of UKA. Improved UKA training could lead to increased experience and better patient selection among surgeons and consequently a higher use of medial UKA.

Abstract:Medial unicompartmental knee arthroplasty (UKA) has increased in popularity for the treatment of end-stage anteromedial osteoarthritis in part secondary to its quicker recovery, improved function, and decreased risk of medical complications over total knee arthroplasty (TKA). However, despite the success of medial UKA, dilemma still exists over certain patient characteristics as to suitability for undergoing the procedure. In addition to patient age, body mass index (BMI), and the presence, or lack thereof, of an anterior cruciate ligament (ACL), one of the most contentious issues surrounding suitability for medial UKA is the preoperative status of the patellofemoral joint (PFJ). This review aims to look at the historical factors leading to the current dilemma as well as recent evidence surrounding anterior knee pain and preoperative PFJ arthritis on the survivability and outcomes of medial UKA. This article will also evaluate other PFJ factors possibly affecting the outcome of medial UKA such as lateral patellar subluxation and lateral facet grooving, future perspectives that may influence the utilization of medial UKA such as cementless implants, and the choice of mobile versus fixed bearing articulations.
Starting Up a Lateral Unicompartmental Knee Arthroplasty Practice – Is Outcome Affected?

Abstract:Introduction In the past, the utilization of lateral unicompartmental knee arthroplasty (UKA) has been limited at national levels, despite the fact that an estimated 10% of patients who have an indication for arthroplasty, present with isolated lateral compartment osteoarthritis (OA). Units dedicated to UKA have reported good outcomes. Identifying patients for the procedure has been less clear, and the procedure has been perceived to be technically more demanding than medial UKA. This may result in a reluctance to start a lateral UKA practice and challenge the early phase. Therefore, this paper aimed to present the outcomes and learning curve when starting up a lateral UKA practice, as this theme remains unelucidated. Methods There were 85 primary fixed-bearing lateral UKAs, with a minimum of 1-year follow-up, performed between 2016 and 2022 by two arthroplasty surgeons with existing UKA practices. The indications were primary (n = 79) or post-traumatic (n = 6) osteoarthritis. Patient-reported outcome measures (PROMs) were assessed at 3, 12, and 24 months. A cumulative sum (CUSUM) analysis was used to evaluate surgical duration and the 12-month Oxford Knee Score (OKS). Results Median (Interquartile range (IQR)) 12-month OKS, Activity and Participation Questionnaire (APQ), and Forgotten Joint Score (FJS) were 43 (37.5 to 46), 78 (42.5 to 98.5), and 72 (55 to 90), respectively. The OKS outcomes did not reveal adverse effects from the learning curve. A performance shift in surgical duration was observed around case 33. Kaplan-Meier implant survival reached 95.4% at 7 years for the endpoint "implant revision," and 93.5% for "implant revision/implant addition." Conclusion Starting up a lateral UKA practice is safe and efficient for surgeons who have prior medial UKA experience, provided strict adherence to indications. While surgical duration indicated a learning curve over approximately 33 cases, PROMs remained stable, suggesting proficient outcomes irrespective of the learning curve.

Abstract:Unicompartmental knee arthroplasty (UKA) has a long history and has many advantages in some aspects over total knee arthroplasty (TKA) for patients with suitable indications, but it has not been established as a treatment at the same level with TKA. Therefore, 80 members of the British Association for Surgery of the Knee (BASK) and the European Knee Society (EKS) were invited to attend a joint meeting with the aim of creating an evidence-based consensus statement on UKA, in London, UK (December 2019). A formal consensus process was undertaken at the meeting incorporating a multiple round Delphi exercise, with group discussion of areas of agreement and disagreement between rounds. Five consensus statements were issued: ① UKA should be offered as a successful alternative to TKA in patients undergoing arthroplasty who meet agreed indications. ② When consenting a patient for UKA, information including the benefits and risks that are specific to UKA, should be tailored to and discussed with the individual patient. ③ Evidence suggests that surgeons should avoid low-volume use of UKA to optimise outcomes for their patients. ④ Surgeons should use the contemporary evidence-based indications and contraindications for medial UKA. ⑤ Knee arthroplasty surgeons should have exposure to and training in UKA. On the basis of full study of the consensus, combined with the Expert Consensus on Perioperative Management of Unicompartmental Knee Arthroplasty in China in 2020, this paper elaborates the meaning of the final evidence-based consensus for clinicians' reference.

Abstract:: Unicompartmental knee arthroplasty (UKA) which has many potential advantages compared with total knee arthroplasty (TKA) was widely used across the world in recent years. The introduction of the robot systems greatly makes up for the defects of the conventional UKA surgery such as higher complication rates and revision rates. MAKO system, a new image-guided robot system relies on a preoperative computed tomography (CT) scan to assist in preoperative mapping and planning, offers an opportunity to improve the outcome of UKA surgeries. In order to have a more comprehensive and in-depth understanding of MAKO-assisted UKA, the studies on MAKO-assisted UKA were summarized. MAKO-assisted UKA is better than conventional UKA surgery on implant accuracy, soft tissue balance, patient function scores and satisfaction, complications rates, and learning curve in short-term, however, the mid-term and long-term outcomes of MAKO-assisted UKA need to be further studied. Abstract:Improved prosthetic design and application of strict criteria in selection of patients have resulted in wide use of unicompartmental knee arthroplasty (UKA) in surgical treatment of damaged joints. Here, we review the contemporary indications, outcomes, and complications of procedures such as lateral and medial UKA and total knee arthroplasty (TKA), and compare the severity of these complications in UKA and TKA. Patients who received unicompartmental femorotibial osteoarthritis and patients who underwent UKA and ACL reconstruction for deficiency in anterior cruciate ligament (ACL), they all showed good clinical outcomes and survival. Reliable and successful options in the treatment of patients with isolated tibiofemoral osteoarthritis include lateral and medial UKA.

Abstract:Background: The choice of unicompartmental knee arthroplasty (UKA) vs. total knee arthroplasty (TKA) in the surgical treatment of knee osteoarthritis (KOA) remains controversial. This study aimed to perform a systematic review and meta-analysis of randomized controlled trials (RCTs) to compare the clinical results of UKA and TKA for treating unicompartmental KOA. Methods: PubMed, Embase, and the Cochrane Library were systematically searched for articles published up to January 2, 2023. The literature was rigorously screened to include only RCTs comparing UKA and TKA for unicompartmental KOA. A systematic review and meta-analysis were performed to calculate the mean difference (MD), relative risk (RR), and 95% confidence interval (CI) according to the Cochrane standards. Results: Thirteen publications involving 683 UKAs and 683 TKAs were analyzed. Except for one study with a follow-up period of 15 years, all outcome measures reported were within 5 years of follow-up. Meta-analysis showed better knee recovery (MD: 1.23; 95% CI: 1.01-1.45; P <0.00001), greater knee function (MD: 1.78; 95% CI: 0.34-3.22; P = 0.02), less pain (MD: 0.75; 95% CI: 0.43-1.06; P <0.00001), and better health status (MD: 3.75; 95% CI: 0.81-6.69; P = 0.01) after UKA than TKA. However, considering the minimal clinically important difference values for these variables, the findings were not clinically relevant. Moreover, UKA patients had fewer complications (RR: 0.59; 95% CI: 0.45-0.78; P = 0.0002) and shorter hospital stays (MD: -0.89; 95% CI: -1.57 to -0.22; P = 0.009) than did TKA patients. There were no statistically significant differences in terms of postoperative range of movement, revision, failure, operation time, and patient satisfaction. Conclusions: In terms of clinical efficacy, there was no obvious advantage of UKA over TKA in the surgical treatment of knee OA when considering the minimal clinically important difference. The main advantage of UKA over TKA is that it leads to fewer complications and a shorter length of hospital stay. It is ideal to perform prospective studies with longer follow-up periods to fully evaluate the long-term efficacy and safety of the two procedures in the future.

Abstract:Unicompartmental knee arthroplasty (UKA), which has many potential advantages compared with total knee arthroplasty, was widely used across the world in recent years. The introduction of the robot systems greatly makes up for the defects of the conventional UKA surgery such as higher complication rates and revision rates. MAKO system, a new image‐guided robot system relies on a preoperative computed tomography scan to assist in preoperative mapping and planning, offers an opportunity to improve the outcome of UKA surgeries. In order to have a more comprehensive and in‐depth understanding of MAKO‐assisted UKA, the studies on MAKO‐assisted UKA were summarized. MAKO‐assisted UKA is better than conventional UKA surgery on implant accuracy, soft tissue balance, patient function scores and satisfaction, complications rates, and learning curve in short‐term outcome; however, the mid‐term and long‐term outcomes of MAKO‐assisted UKA need to be further studied.
Robotic arm-assisted unicondylar knee arthroplasty resulted in superior radiological accuracy: a propensity score-matched analysis

Abstract:Abstract Introduction Unicompartmental knee arthroplasty (UKA) is an effective surgical treatment for medial compartment arthritis of the knee, yet surgical outcomes are directly related to surgical execution. Robotic arm-assisted surgery aims to address these difficulties by allowing for detailed preoperative planning, real-time intraoperative assessment and haptic-controlled bone removal. This study aimed to compare the clinical and radiological outcomes between conventional manual mobile bearing and robot arm-assisted fixed bearing medial UKA in our local population. Materials and methods This is a retrospective case–control study of 148 UKAs performed at an academic institution with a minimum of 1-year follow-up. 74 robotic arm-assisted UKAs were matched to 74 conventional UKAs via propensity score matching. Radiological outcomes included postoperative mechanical axis and individual component alignment. Clinical parameters included a range of motion, Knee Society knee score and functional assessment taken before, 6 and 12 months after the operation. Results Robot arm-assisted UKA produced a more neutral component coronal alignment in both femoral component (robotic -0.2 ± 2.8, manual 2.6 ± 2.3; P = 0.043) and tibial component (robotic -0.3 ± 4.0, manual 1.7 ± 5.3; P < 0.001). While the postoperative mechanical axis was comparable, robot arm-assisted UKA demonstrated a smaller posterior tibial slope (robotic 5.7 ± 2.7, manual 8.2 ± 3.3; P = 0.02). Clinical outcomes did not show any statistically significant differences. Conclusion Compared with conventional UKA, robotic arm-assisted UKA demonstrated improved component alignment and comparable clinical outcomes. Improved radiological accuracy with robotic-arm assistance demonstrated promising early results.

Abstract:We propose to apply Ramsey's method of separated oscillating fields to the spectroscopy of the quantum states in the gravity potential above a horizontal mirror. This method allows a precise measurement of quantum mechanical phaseshifts of a Schroedinger wave packet bouncing off a hard surface in the gravitational field of the Earth. Measurements with ultracold neutrons will offer a sensitivity to Newton's law or hypothetical short-ranged interactions, which is about 21 orders of magnitude below the energy scale of electromagnetism.
