Abstract:BACKGROUND:Ophthalmic segment artery aneurysms (OSAs) are difficult to clip; therefore, improvement of the surgical method is of great significance to the prevention of complications, and the classification of the aneurysms is essential to formulate a reasonable surgical plan.OBJECTIVE:To explore the strategies and effects of surgery for OSAs using a modified subdural Dolenc approach.METHODS:The clinical data of 38 patients (12 men and 26 women, aged 48-73 years) with OSA were analyzed retrospectively. A total of 44 aneurysms were identified, 40 of which were OSAs. The 40 aneurysms were divided into types Ia(1) (n = 2), Ia(2) (n = 2), Ib (n = 6), IIa (n = 4), IIb (n = 4), IIIa (n = 0), IIIb (n = 4), IIIc (n = 16), and IV (n = 2) based on preoperative images. Thirty-nine OSAs were operated successfully through pterional craniotomy combined with the modified subdural Dolenc approach, and 1 aneurysm was clipped through the contralateral approach. Clinical outcomes were evaluated using the Glasgow Outcome Scale (GOS).RESULTS:Thirty-nine OSAs were clipped, and one was wrapped. Visual dysfunction, headache, and dizziness improved after the operation in 18 patients. One patient had new visual impairment, and there were no deaths. At discharge, the GOS score was 5 in 36 cases, 4 in 1 case, and 3 in 1 case. Thirty-seven patients had a GOS score of 5, and 1 patient had a score of 3 at 6 months after the operation.CONCLUSION:The modified subdural Dolenc approach (Zheng approach) for clipping OSAs may be associated with less trauma and good postoperative outcomes.
Intracranial Aneurysms Surgery Assisted by Rigid Endoscope

Abstract:Objective To evaluate the usefulness of endoscope in the microsurgical treatment of intraeranial aneurysms and analyze its benefits.risksand disadvantages.Mehtods We retrospectively study 63 patients with 66 aneurysms,treated between Janurary 2007 and October 2007,for whom the endoscope were used to assist the mierosurgical treatment of lesions.63 aneurysms were ruptured.In most of the cases,the endoscope was used in addition to microsurgical dissection and clipping(before,during and after clipping).Observation of the anatomic features of necks,perforators and selection of optimal clip position were performed.Results In the majority of cases,the endoscope was used.The clip Was repositioned in five aneurysm operations.including incomplete neck obliteration in 1 case of intracarotidophathalmic aneurysm,incomplete neck obliteration in 1 ICA-PCoA aneurysms,involvement of perforating vessels with the clip in 1 ICA-PCoA aneurysms,involvement of perforating vessels with the clip in 2 ACoA aneurysms.One patient experienced a mild temporal lobe contusion during the application of endoscope.There Was no intra-operative aneurysm rapture due to endoscope manipulation.Conclusioas Endoscope is vary useful during cerebral aneurysm surgery.It can provide valuable information for the assessment of regional anatomic features around aneurysm and improve the quality of aneurysm surgery.
Surgical Treatment for Posterior Inferior Cerebellar Artery Aneurysms

Abstract:Objective To explore the clinical characteristics and surgical managements of posterior inferior cerebellar artery (PICA) aneurysms. Methods Retrospective analysis of the clinical features, imaging characteristics and clipping results of 21 PICA artery aneurysms. This series included 9 men and 12 women, mean age was 40.6 years. One presented with mass pressure effects of infrotentoria fossa; 20 patients presented with severe and sudden onset of symptoms, reflecting a hemorrhage from a PICA aneurysm. Hunt-Hess and Hess grades at presentation were: Grade Ⅰ in 1 patient, Grade Ⅱ in 14 patients, Grade Ⅲ in 5 patients, Grade Ⅳ in 1 patient. The bleeding was confined to the infrotentoria subarachnoid space (SAH) and/or the fourth ventricle. Arteriographies revealed 13 proximal PICA aneurysms and 8 distal PICA aneurysms. Results Lateral suboccipital and midline suboccipital surgical approaches were used to secure the aneurysms in all patients, either by direct clipping in 19 cases, parent artery's occlusion in 1 case and parent artery's sacrifice in 1 case. Transient CSF leak occurred in 3 patents; paresis and lower cranial nerve dysfunction and long-term coma occurred in 1 patient; mild lower cranial nerve dysfunctions occurred in 2 patients, and had good recovery at 3 months of follow-up; no complication in other patients. Conclusions The favorable outcomes and low postoperative morbidity in our patients argues that surgical clipping be considered a first-line therapeutic option. The protection of the low cranial nerves and the preservations of PICA are major contributors to good long-term results.

Abstract:BACKGROUND: Aneurysms of the posterior cerebral artery (PCA) are rare and often associated with anterior circulation aneurysms. The lateral supraorbital approach allows for a very fast and safe approach to the ipsilateral lesions Circle of Willis. A technical note on the successful clip occlusion of two aneurysms in the anterior and posterior Circle of Willis via this less invasive approach has not been published before. The objective of this technical note is to describe the simultaneous microsurgical clip occlusion of an ipsilateral PCA-P1 and an internal carotid artery - posterior communicating artery (ICA-PCoA) aneurysm via the lateral supraorbital approach.CASE DESCRIPTION: The authors present a technical report of successful clip occlusions of ipsilateral located PCA-P1 and ICA-PCoA aneurysms. A 59-year-old female patient was diagnosed with a PCA-P1 and an ipsilateral ICA-PCoA aneurysm by computed tomography angiography (CTA) after an ischemic stroke secondary to a contralateral ICA dissection. The patient underwent microsurgical clipping after a lateral supraorbital craniotomy. The intraoperative indocyanine green (ICG) videoangiography and the postoperative CTA showed a complete occlusion of both aneurysms; the parent vessels (ICA and PCA) were patent. The patient presents postoperative no new neurologic deficit.CONCLUSION: The lateral supraorbital approach is suitable for the simultaneous microsurgical treatment of proximal anterior circulation and ipsilateral proximal PCA aneurysms. Compared to endovascular treatment, direct visual control of brainstem perforators is possible.

Abstract:A 72-year-old female patient suddenly lost consciousness due to a subarachnoid hemorrhage (SAH) and a large intracerebral hematoma (ICH) stemming from the rupture of an aneurysm on the small anterior communicating artery (AcomA). The poor clinical condition of the patient meant that the ICH needed to be surgically removed. The small wide-necked aneurysm was not suitable for coil occlusion. Since there was no escaping surgery, the decision was made to also clip the aneurysm. As the aneurysm was hidden on the posterior wall of the AcomA and therefore not actually visible during the operation, a fenestrated clip was used. CTA following the operation confirmed that this had completely obliterated the aneurysm while leaving the afferent and efferent vessels patent. When the patient was referred on to a rehabilitation facility, she was awake, aphasic, and with no motor deficit. Most ruptured anterior communicating artery (AcomA) aneurysms are nowadays eliminated by endovascular therapy. However, in selected cases in which the aneurysm is not suitable for endovascular therapy, surgical clipping is the suggested route. In this chapter, we address the surgical approach, the as yet unsolved issue of which side to approach from, and radiological follow-up for ruptured AcomA aneurysms which have been microsurgically clipped. Microsurgical clipping of ruptured AcomA aneurysms is the main topic of this chapter.

Abstract:Approximately 25% of intracranial aneurysms originate at the internal carotid artery (ICA) and posterior communicating artery (PCoA) junction. 1 Unlike typical PCoA aneurysms, which are usually saccular, a subset known as "true" PCoA aneurysms arise directly from the PCoA. These represent about 1.3% of all intracranial aneurysms and 6.8% of PCoA aneurysms. 1 The first report of a true PCoA aneurysm dates back to 1979. 2 This video article illustrates the microsurgical clipping of a true PCoA aneurysm in a 27-year-old male with subarachnoid hemorrhage and left-sided ophthalmoplegia. CT angiography revealed a large true PCoA aneurysm, leading to the decision for microsurgical clipping. Our surgical strategy included: 1) an extended pterional approach; 2) early brain relaxation through basal cisterns and third ventricle opening; 3) Sylvian fissure dissection; 4) partial uncus resection; 5) tracing the PCoA to the aneurysm; 6) pilot clipping and thrombectomy; 7) careful aneurysm dissection and definitive clipping. The patient had an uncomplicated recovery, discharged on the fifth day with resolved third nerve dysfunction. The literature review from 2022 documented only 47 cases of true PCoA aneurysms, predominantly presenting with rupture. 3 Some studies suggest these aneurysms may have a higher rupture risk than typical ICA-PCoA junction aneurysms. 4 Microsurgical clipping is a primary treatment, often in cases associated with a fetal PCA variant. 5 Ensuring the patency of the PCoA and thalamoperforating arteries is crucial, with careful visualization of the clip's distal ends to avoid impacting nearby neurovascular structures.

Combined Open Surgical and Endovascular Management of a Giant Posterior Cerebral Artery Aneurysm: 2-Dimensional Operative Video

Abstract:A 17-year-old female patient presented with worsening intractable headaches which were initially diagnosed as migraine headaches. The patient's headaches were refractory to medical management and continued to worsen in both severity and frequency. Computed tomography angiography and digital subtraction angiography demonstrated a giant posterior cerebral artery aneurysm with contributions from the internal carotid artery through the posterior communicating artery. During the surgical planning period, the patient continued to have worsening headaches, intractable nausea, vomiting, left upper extremity weakness, and associated visual obfuscations. Repeated angiography revealed spontaneous thrombosis of the posterior communicating artery contribution with additional sudden onset hemianopsia on balloon test occlusion of the posterior cerebral artery, prompting urgent surgical intervention. A right subtemporal approach was used for a superficial temporal artery to posterior cerebral artery bypass, followed by coil occlusion of the aneurysm inlet at the posterior cerebral artery on postoperative day 1. The patient consented to the procedure. Postoperative angiography demonstrated successful superficial temporal artery to posterior cerebral artery anastomosis with occluded posterior cerebral artery after coiling. At 1-year follow-up, the patient remained neurologically intact with a patent bypass and regression of the trapped aneurysm. We present this case to demonstrate the successful management of a complex posterior cerebral artery aneurysm with both surgical bypass and endovascular parent artery coil occlusion. 1-6

Abstract:Natural organic matter (NOM) was concentrated from Songhua River using a reverse osmosis system. The hydrophobic fraction (HyO), hydrophilic fraction (HyI), fulvic acid (FA) and humic acid (HA) were fractionated by the XAD resin, and a commercial humic acid (CHA) was used for comparison. The FA fraction predominant in natural water and account for the large part of DOC, and the aromaticity of these fractions followed the order HA> HyO> FA> HyI. A weak relationship exists between the E4/ E6 and aromaticity. FTIR spectra revealed that the degree of saturation among these fractions was in the order of HyO = FA > HyI > HAM=CHA. Gel chromatograph results showed that the weaker ultraviolet-adsorption fraction of NOM contributed to the most part of molecular weight. Potentiometric titration indicated that the total organic acidity was in the range of 9.84 - 12.15 meq/g, and carboxylic acidity decrease in the order of CHA> HyI > HyO > NOM > FA > HA. 1H NMR spectra demonstrated that NOM and the isolated fractions were predominated by aliphatic functional groups.
Clip Reconstruction of a Giant Internal Carotid Artery Bifurcation Aneurysm in Hybrid Operating Room: 2-Dimensional Operative Video

Abstract:Giant internal carotid artery (ICA) bifurcation aneurysms are technically difficult to treat because of lack of a definable neck, which makes direct clipping impossible. In addition, these lesions are related to multiple perforating arteries, posing a different challenge to surgical operators. This video demonstrates clip reconstruction of a giant ICA bifurcation aneurysm in a hybrid operating room. A 32-year-old female presented with dizziness for one year. Angiography demonstrated a giant left ICA bifurcation aneurysm. The patient was consented for both clip reconstruction and extracranial-intracranial bypass if needed. A left pterional craniotomy was performed. After dural opening, the sylvian fissure was opened widely until the aneurysm was identified. The proximal ICA was then identified. The aneurysm projected posteriorly, involving the origins of both the A1 and M1 segments. After temporary clipping, the critical perforators were dissected free from the aneurysm wall. Tandem fenestrated clipping was used to reconstruct the parent artery. Finally, curved clips were used to occlude the distal part of the neck. Vessel patency was evaluated using intraoperative Doppler and indocyanine green angiography. Intraoperative somatosensory and motor evoked potential monitoring remained unchanged after final clipping. Intraoperative angiography confirmed complete obliteration of the aneurysm and reconstruction of the parent artery. The patient recovered well without any complications. Giant ICA bifurcation aneurysms are complex lesions that require preoperative planning and intraoperative monitoring. Adherence to general principles, including careful dissection of aneurysmal wall from adjacent perforators and clip reconstruction using tandem fenestrated clipping, these lesions can be treated safely and effectively.

Abstract:Objective: To evaluate the efficacy of the surgery with lateral supraorbital approach for clipping anterior circulation aneurysms. Methods: The clinical data of 99 patients with anterior circulation aneurysms who underwent a clipping via lateral supraorbital approach from October 2012 to September 2014 and 75 patients, who underwent a clipping via pterional approach from January 2010 to December 2011 in the second Affiliated Hospital, Zhejiang University School of Medicine were reviewed. The operative duration, prognosis, residual rate and rupture rate during surgery were compared between two groups. Results: The patients by lateral supraorbital approach had a shorter operative duration, namely 37~61 min less than that in the patients by pterional approach. No difference were found in residual rate and rupture rate between two groups. The Glasgow Outcome Scale (GOS) at 3 months after surgery showed no significant difference between two groups. Conclusion: The lateral supraorbital approach can be used safely and effectively with shorter operative duration and less tissue damage for treatment of patients with anterior circulation aneurysms.

Abstract:Aneurysms of the bifurcation of the internal carotid artery (ICA) represent approximately 5% of intracranial aneurysms and tend to occur in younger patients. 1-3 They typically have a superior orientation, in close relationship with the medial (branches of the anterior cerebral artery, segment A1) and lateral (branches of the middle cerebral artery, segment M1) lenticulostriate arteries (LSA), including recurrent artery of Heubner (RAH). RAH commonly originates in the junction of A1 and A2, courses medially to laterally between LSA and anterior cerebral artery, ICA bifurcation, and middle cerebral artery before entering the lateral portion of the anterior perforated substance. 4-7 Damage to these arteries are catastrophic. This complex vascular anatomy makes treatment challenging, either endovascular or microsurgical. 8-13 We present a video case of microsurgical clipping for the left ICA bifurcation aneurysm. This study was approved by the ethics committee of our institution. The patient, a 46-year-old man, was diagnosed with an unruptured ICA bifurcation aneurysm during workup for intermittent headache. Morphological characteristics of the aneurysm supported intervention over conservative treatment. Digital subtraction angiography showed a prominent proximal RAH crossing between the aneurysm neck and LSA, providing anatomical protection for clip application. Microsurgical clipping was chosen, and the procedure went uneventfully. The patient consented to the procedure and to the publication of his/her image. We discussed and demonstrated throughout the video how we used this anatomic variation and trajectory of the RAH to prevent clipping of LSA unwittingly and achieve complete occlusion of the aneurysm neck.

Abstract:Fetal posterior communicating artery (PComA) is a variant of the cerebral vasculature.1 Woven endobridge (WEB) embolization carries a good safety profile as treatment for ruptured wide neck PComA aneurysms, without the need for antiplatelet therapy. However, the reported occlusion rates are not optimal.2 Flow diversion is suboptimal in treating aneurysms originating from fetal PComA.3 Here we present a case of a 78-year-old female patient with a history of ruptured right fetal PComA aneurysm with wide base. It was initially treated with WEB embolization at an outside hospital. After WEB implantation, the initial follow-up of cerebral angiogram (6 months later) demonstrated a neck recurrence measuring 6 × 3 mm. Approximately 1 year after the initial treatment, pipeline embolization was performed and patient was placed on antiplatelet therapy since. Follow-up images demonstrated a 6 mm × 4 mm persistent neck remnant. Her care was transferred to our institution. Cerebral angiogram obtained 36 months post-WEB implantation showed growth of the neck remnant measuring 9 × 8.5 mm. The WEB device was found to be folded in the aneurysmal fundus. Given this was a growing recurrent previously ruptured fetal PComA aneurysm with a pipeline stent in the internal carotid artery the decision was made to retreat with microsurgical clipping; carotid access at the neck was required for proximal control. We achieved complete aneurysm obliteration through a minimal invasive approach. The patient gave informed consent for surgery and video recording. Institutional Review Board approval was deemed unnecessary.
Bypass-Assisted Microsurgical Clipping for an Endovascular Treatment–Resistant Ruptured Internal Carotid-Posterior Communicating Artery Aneurysm, Recurring Two Decades Postinitial Clipping: 2-Dimensional Operative Video

Abstract:Salvage treatment of postclipping recurrent aneurysms remains challenging. 1,2 The salvage microsurgical clipping is a possible intervention but sometimes difficult because of tissue adhesions around the aneurysm and previously installed clips; therefore, salvage coil embolization may have certain advantages. 1-5 However, when coil embolization is not applicable, 6 microsurgical clipping is a stand-alone curative treatment, requiring proficient and reliable microsurgical techniques. This article describes a unique case of a 70-year-old female patient complaining of a severe headache with subarachnoid hemorrhage due to a recurrent ruptured left internal carotid-posterior communicating artery aneurysm after microsurgical clipping 23 years ago. An initial attempt at coil embolization proved unsuccessful because of the aneurysm shape. Consequently, the salvage microsurgical clipping was planned. Given the additional time for trapping the internal carotid artery for the old clip removal, a superficial temporal artery-middle cerebral artery bypass was also planned. The operation entailed a superficial temporal artery-middle cerebral artery bypass, a Sylvian fissure dissection, the old clip removal, and aneurysm clipping. Intraoperatively, the complete aneurysm neck clipping was successfully performed without any complication, and patient postoperative course was uneventful. A wide surgical field should be obtained to be able to manipulate the aneurysm and old clip safely under a microscope. It is also important to temporarily trap the main artery to ensure removal of old clips and to prepare for intraprocedural ischemia using bypass after a precise assessment of hemodynamics before surgery. The patient provided informed consent for the procedure and the publication of the case along with its pertinent imaging, and this report was approved by the institutional review board at our hospital.

Abstract:The first examples of samarium, europium, and ytterbium complexes with 3,6-di-tert-butyl-o-benzoquinone(3,6-dbbq) in the form of catecholate have been obtained by reactions of the quinone with the corresponding lanthanocenes, [LnCp2*(thf)n] (n = 1 or 2) in solution. In the course of the reactions lanthanide ions lose one or two Cp* ligands, which take part in reduction of a quinone molecule into a catecholate anion (dbcat, 2(-)). As a result of the reactions, Sm and Yb clearly yield dimeric complexes[(LnCp*)2(dbcat)2], where each Ln ion loses one Cp* ligand. Eu forms a trimeric complex [(EuCp*)-(Eu·thf)2(dbcat)3], in which one Eu ion is coordinated by one Cp* ligand, while two Eu ions have lost all Cp* ligands and are coordinated by THF molecules instead. Magnetic properties corroborate the assignment of oxidation states made on the basis of single-crystal X-ray diffraction: all the quinone ligands are present in the catecholate state; both Sm/Yb ions in the dimers are in the +3 oxidation state, whereas the Eu trimer contains two Eu(II) and one Eu(III) ions. Cyclovoltammetry studies show the presence of two reversible oxidation waves for all complexes, presumably concerned with the redox transitions of the dbcat ligands.
The effect of stimulant and sedative use on treatment outcome of patients admitted to methadone maintenance treatment.

Abstract:While methadone maintenance treatment (MMT) has been demonstrated to be an effective treatment for opiate dependence, its impact on the treatment outcome of other illicit drug abuse is not as clear. Using the initial urine drug screen (UDS) and follow-up UDS at 1, 6, 12, and 24 months, 167 patients consecutively admitted to MMT were evaluated for opiate, sedative (predominantly benzodiazepine), and stimulant (predominantly cocaine) use. Retention for the opiate only group was 97.32 days longer on average than for patients using opiates along with stimulants, sedatives, or both stimulants and sedatives. Patients abusing opiates only had the greatest decrease in drug use; however, MMT was also associated with decreases in cocaine and sedative use over the 24 month follow-up period. There was no evidence that patients "switched" their drugs of abuse with time in treatment. The negative impact of non-opiate drug use on outcome in MMT and its implications for treatment planning are discussed.

Abstract:Anterior communicating artery (ACoA) aneurysms account for the largest proportion of intracranial aneurysms and are the most likely to rupture.[1] The main treatment methods are endovascular embolization and traditional craniotomy clipping.[2] With the rapid development of interventional materials and technologies in the last two decades, many medical centers have adopted endovascular embolization as the preferred treatment for intracranial aneurysms. However, some aneurysms still require traditional craniotomy clipping. Transsphenoidal endoscopy technology has also developed rapidly in recent years.[3] Because intracranial aneurysms are mostly within the circle of Willis and occur on the ventral side of the skull base, a few neurosurgeons have tried to use an endoscopic endonasal approach (EEA) for clipping.[4–6] However, the number of cases reported in the literature is small and more anatomical studies are needed to deeply analyze the approach and technique.

Clipping of Kissing Distal Anterior Cerebral Artery Aneurysms: The Importance of Microsurgical Techniques to Enlarge the Surgical Corridor and Prevent a Premature Aneurysm Rupture

Abstract:Intracranial kissing aneurysms, arising either from the same artery or from 2 adjacent arteries at similar locations, are rare.1,2 The internal carotid artery is most frequently involved; kissing aneurysms rarely affect the distal anterior cerebral artery (DACA). By dint of the close proximity of the aneurysm fundus, these aneurysms can pose unique operative challenges.3,4 A highly fragile aneurysm dome with a high intraoperative rupture rate is a unique management challenge in DACA aneurysms.5 The stakes are higher when there is an aneurysm rupture in the setting of kissing DACA aneurysms requiring an anterior interhemispheric approach. The negotiation of a tight interhemispheric fissure in between the bridging veins and prevention of a premature aneurysm rupture at a narrow space become vital in these situations. Video 1 highlights the surgical steps of clipping bilateral kissing DACA aneurysms in a 60-year-old woman. This surgical video highlights the microneurosurgical nuances of opening a tense interhemispheric fissure and maneuvers for prevention of a premature aneurysm rupture. These nuances are quintessential in the successful surgical clipping of kissing DACA aneurysms. The patient in Video 1 presented with an acute subarachnoid hemorrhage with severe headache of sudden onset and nuchal rigidity (World Federation of Neurological Surgeons grade II). Both aneurysms were located at the A3-A4 junction and successfully clipped through a right-sided anterior interhemispheric approach. She made a satisfactory postoperative recovery (modified Rankin Scale score of 1 at 6-week follow-up and 0 at 6-month follow-up) with an excellent angiographic outcome.
Far Lateral Approach for Clipping of a Posterior Inferior Cerebellar Artery Aneurysm

Abstract:Objectives To describe a far lateral approach for microsurgical clipping of a ruptured posterior inferior cerebellar artery (PICA) aneurysm involving the hypoglossal nerve, with emphasis on the microsurgical anatomy, and technique. Design A far lateral craniotomy is performed in the lateral decubitus position and the transverse and sigmoid sinuses were exposed. After opening the dura, sutures are placed to allow gentle mobilization of the sinuses. The ipsilateral cerebellar tonsil is mobilized and the PICA is followed to its junction with the vertebral artery. Hypoglossal nerve rootlets are draped over the dome of the aneurysm. Mobilization of the PICA and the hypoglossal nerve away from the lateral medulla allows microsurgical clipping of the aneurysm neck. Photographs of the region are borrowed from Dr Rhoton's laboratory to illustrate the microsurgical anatomy. Participants The senior authors performed the surgery. The video was edited by Drs. V.N. and J.B. Chart review and literature review were performed by Drs. W.M. and J.B. Outcome Measures Outcome was assessed with successful clip occlusion and postoperative neurological function. Results There was complete clip occlusion of the PICA aneurysm with no postoperative neurological deficits. The patient was discharged home after an uneventful hospital course. Conclusion The far lateral approach provides an adequate corridor to the ventrolateral brainstem for microsurgical treatment of PICA aneurysms.
