Abstract:dermatologist who reported that the lesion had no granulomatous features then, but a biopsy was not done. She presented with a clinically granulomatous lesion ofthe nose with some possible scarring. There was a nasal perforation. There was no previous history of T.B. and she was not on systemic corticosteroids. Investigations. Histology showed an ulcerated lesion with epithelioid follicles and giant cells. Mantoux test was positive at i in 10,000; Chest X-ray, no abnormality; Culture of skin biopsy for A.F.B.'s was negative; Guinea-pig inoculation was negative. Treatment. Response to para-aminosalicylic acid and isoniazid was excellent. Case III: Mrs W.P. aged 74 years. History. She had a 6-month history of the development of crusted lesions around the nose and upper lip and left cheek, which gradually became a very proliferative, vegetating granuloma, associated with a large anterior nasal perforation. She was not on systemic corticosteroids. She had no previous history of tuberculosis. Investigations. Histology showed warty hyperplasia with underlying tuberculoid granuloma. There was no caseation and no A.F.B.'s were seen. Mantoux test: markedly positive at i in 10,000; Chest X-ray, no abnormality; Bacteriology, Staphylococcus aureus and acid fast baciUi isolated on culture of biopsy material. Treatment. The response to rifampicin, ethambutol and isoniazid was excellent. Case IV: Mrs E.B. aged yi years. History. She presented with a 6-month history of ulceration and inflammation of the tip of the nose. She had had a tuberculous gland in the neck at the age of 20 years. Investigations. Three separate biopsies were entirely non-specific. Culture and guinea-pig inoculation were negative for A.F.B.'s. There was a very heavy growth of staphylococcus aureus. Treatment. Because of the development of apple jelly nodulation, she was treated with para-amino salicyclic acid and isoniazid. The response was excellent. A fifth case will be discussed. Comment. These cases are discussed to emphasize how difficult it can be to diagnose tuberculosis of the skin when it involves the nose. Delay in diagnosis can lead to unpleasant scarring. There are cases when a trial of anti-tuberculous therapy seems to be entirely justified in spite of not having a definite tuberculous clinical or histological picture, even if the culture is negative too.
Team clinician variability in return-to-play decisions

Abstract:Objective: To describe the variability in the return-to-play (RTP) decisions of experienced team clinicians and to assess their clinical opinion as to the relevance of 19 factors described in a RTP decision-making model. Design: Survey questionnaire. Setting: Advanced Team Physician Course. Participants: Sixty-seven of 101 sports medicine clinicians completed the questionnaire. Main outcome measures: Results were analyzed using descriptive statistics. For categorical variables, we report percentage and frequency. For continuous variables, we report mean (SD) if data were approximately normally distributed and frequencies for clinically relevant categories for skewed data. Results: The average number of years of clinical sports medicine experience was 13.6 (9.8). Of the 62 clinicians who responded fully, 35% (n = 22) would "clear" (vs "not clear") an athlete to participate in sport even if the risk of an acute reinjury or long-term sequelae is increased. When respondents were given 6 different RTP options rather than binary choices, there were increased discrepancies across some injury risk scenarios. For example, 8.1% to 16.1% of respondents who chose to clear an athlete when presented with binary choices, later chose to "not clear" an athlete when given 6 graded RTP options. The respondents often considered factors of potential importance to athletes as nonimportant to the RTP decision process if risk of reinjury was unaffected (range, n = 4 [10%] to n = 19 [45%]). Conclusions: There is a high degree of variability in how different clinicians weight the different factors related to RTP decision making. More precise definitions decrease but do not eliminate this variability.
Validation of the Keele STarT MSK Tool for Patients with Musculoskeletal Pain in United States-Based Outpatient Physical Therapy Settings

Abstract:The STarT MSK tool was developed to enable risk stratification of patients with common musculoskeletal (MSK) pain conditions and help identify individuals who may require more targeted interventions or closer monitoring in primary care settings, however its validity in US-based outpatient physical therapy settings has not been investigated. The 10-item Keele STarT MSK risk stratification tool was tested for construct (convergent and discriminant) and predictive validity using a multicenter, prospective cohort study design. Participants (n=141) receiving physical therapy for MSK pain of the back, neck, shoulder, hip, knee, or multisite regions completed intake questionnaires including the Keele STarT MSK tool, Functional Comorbidity Index (FCI), OSPRO-ROS and OSPRO-YF tools. Pain intensity, pain interference, and health related quality of life (SF-8 physical [PCS] and mental [MCS] component scores) were measured at 2- and 6-month follow-up. Participants were classified as STarT MSK tool low (44%), medium (39%), and high (17%) risk. Follow-up rates were 70.2% (2 months) and 49.6% (6 months). For convergent validity, fair relationships were observed between the STarT MSK tool and FCI and SF-8 MCS (r = 0.35-0.37) while moderate-to-good relationships (r = 0.51-0.72) were observed for seven other clinical measures. Abstract:Injuries occur when an athlete performs a greater amount of activity (workload) than what their body can absorb. To maximize the positive effects of training while avoiding injuries, athletes and coaches need to determine safe workload levels. The International Olympic Committee has recommended using the acute:chronic workload ratio (ACRatio) to monitor injury risk, and has provided thresholds to minimize risk. However, there are several limitations to the ACRatio which may impact the validity of current recommendations. In this review, we discuss previously published and novel challenges with the ACRatio, and possible strategies to address them. These challenges include 1) formulating the ACRatio as a proportion rather than a measure of change, 2) its use of unweighted averages to measure activity loads, 3) inapplicability of the ACRatio to sports where athletes taper their activity, 4) discretization of the ACRatio prior to model selection, 5) the establishment of the model using sparse data, 6) potential bias in the ACRatio of injured athletes, 7) unmeasured confounding, and 8) application of the ACRatio to subsequent injuries.

Abstract:Overuse injuries are a serious problem in junior tennis. Gaining insight in age-specific risk factors can contribute to prevention. The developmental cognitive processes that take place during adolescence make talented players more inclined to take risks. This may be even more pronounced in the high performance culture in which they move. Therefore, this study focuses on the relationship between risk-taking and overuse injuries in talented tennis players. Seventy-three talented tennis players (45 boys and 28 girls, age 11–14 years) were monitored for 32 weeks, using the Oslo Sports Trauma Research Centre Questionnaire on Health Problems. Risk-taking was measured at the start of the season with the Iowa Gambling Task. Linear regression analyses were executed to predict (a) overuse injuries, (b) time loss overuse injuries and (c) overuse severity, by risk-taking, exposure time, and injury history. In boys, risk-taking contributed significantly to time loss overuse injuries [F(1,39) = 7.764, P = 0.008, R2 = 0.15] and to overuse severity [F(1,39) = 5.683, P = 0.022, with an R2 of 0.13] In girls, time loss overuse injuries [F(1,23) = 6.889, P = 0.018, R2 = 0.20] and overuse severity [F(1,23) = 7.287, P = 0.013, R2 = 0.24] were predicted by exposure time. Coaches and trainers should be aware that talented male tennis players who are inclined to take risks, are more likely to maintain risky behavioral patterns related to overuse injuries.

Abstract:Anterior cruciate ligament (ACL) injuries in female adolescent athletes occur at disproportionately high levels compared to their male counterparts. However, limited prospective data exist on the validity of low-cost screening tools that can proactively identify ACL injury risk, specifically for female athletes. The purpose of this study was to assess the concurrent validity of a three-task injury risk factor assessment by comparing visually derived outcome scores from two-dimensional (2D) video data with dichotomized three-dimensional (3D) biomechanical variables collected using motion capture technology. A total of 41 female club volleyball athletes (14.7 ± 1.4 years) were tested and asked to perform three tasks: double-leg vertical jump (DLVJ), single-leg squat (SLS), and single-leg drop landing (SLDL). One rater was trained on the scoring criteria for the 2D data and independently scored one forward-facing and one side-facing video for each task. Risk factors identified included poor knee position, lateral trunk lean, and excessive trunk flexion/extension. In addition, 3D joint angles were calculated for the trunk and knee in the sagittal and frontal planes and converted to dichotomous variables based on biomechanical thresholds of injury risk. For comparison of 2D and 3D outcomes, percent agreement and Cohen's kappa were calculated for each risk factor individually. Overall, 2D scores were found to exhibit moderate to excellent percent agreement with 3D outcomes for trunk position (69.1–97.1%). Specifically, ipsilateral trunk lean during single-leg tasks exhibited the highest agreement (85.3–88.2%) with moderate reliability (κ = 0.452–0.465). In addition, moderate to substantial reliability was found for trunk flexion during double-leg tasks (κ = 0.521–0.653); however, an evaluation of single-leg tasks resulted in only fair reliability (κ = 0.354). Furthermore, 2D scores were not successful in identifying poor knee position as percent agreement fell below 50% for both the single-leg tasks and averaged 60% agreement across both the phases of the DLVJ. Kappa coefficients further emphasized these trends indicating no to slight concurrent validity (κ = −0.047–0.167) across tasks. Overall, these findings emphasize the potential for valid, low-cost screening tools that can identify high-risk movement patterns. Further study is needed to develop improved assessment guidelines that may be employed through visual assessment in sports environments.

Abstract:YisP is an enzyme involved in the pathway of biofilm formation in bacteria and is predicted to possess squalene synthase activity. A BlastP search using the YisP protein sequence from Bacillus subtilis subsp. subtilis strain 168 shows that it shares 23% identity with the dehydrosqualene synthase from Staphylococcus aureus. The YisP from B. subtilis 168 was expressed in Escherichia coli and the recombinant protein was purified and crystallized. The crystals, which belong to the orthorhombic space group P2(1)2(1)2(1), with unit-cell parameters a = 43.966, b = 77.576, c = 91.378 Å, were obtained by the sitting-drop vapour-diffusion method and diffracted to 1.92 Å resolution. Structure determination using MAD and MIR methods is in progress.
Using a Bayesian network to classify time to return to sport based on football injury epidemiological data

Abstract:Objective The return-to-sport (RTS) process is multifaceted and complex, as multiple variables may interact and influence the time to RTS. These variables include intrinsic factors of the player, such as anthropometrics and playing position, or extrinsic factors, such as competitive pressure. Providing an individualised estimation is often challenging, and yet clinical decision support tools are often rare in the industry. This study aims to demonstrate the functions of a Bayesian network by the use of a set of basic epidemiological data. Methods To exemplify the use of Bayesian network in sports medicine, such as providing an individualised estimation time to RTS for individual players, we applied Bayesian network to a set of basic epidemiological data. Bayesian network was used as a decision support tool to model the epidemiological data and to integrate clinical data, non-clinical factors and expert knowledge. Specifically, we used the Bayesian network to capture the interaction between variables in order to 1) classify days to RTS and 2) injury severity (minimal, mild, moderate and severe). Results Retrospective injury data of 3374 player seasons and 6143 time-loss injuries from seven seasons of the professional German football league (Bundesliga, 2014/2015 through 2020/2021) were collected from public databases and media resources. A total of twelve variables from three main categories (player’s characteristics and anthropometrics, match information and injury information) were included. The key response variables are 1) days to RTS (1-3, 4-7, 8-14, 15-28, 29-60, >60 , and 2) severity (minimal, mild, moderate and severe). As there are more than two categories, producer’s and user’s accuracy was used to reflect the sensitivity and specificity of the model. The producer’s accuracy of the model for days to RTS ranges from 0.24 to 0.97, while for severity categories range from 0.73 to 1.00. The user’s accuracy of the model for days to RTS ranges from 0.52 to 0.83, while for severity categories, it ranges from 0.67 to 1.00. Conclusions The Bayesian network can help to capture different types of data to model the probability of an outcome, such as days to return to sports. In our study, the result from the BN may support coaches and players in predicting days to RTS given an injury, 2) support team planning via assessment of scenarios based on player’s characteristics and injury risk and 3) provide evidence-based support of understanding relationships between factors and RTS. This study shows the key functions and applications of the Bayesian network in RTS, and we suggest further experimenting and developing the Bayesian network into a decision-supporting aid.

Abstract:OBJECTIVE: In this paper, we describe the development of a novel tool-the Sports Organization Concussion Risk Assessment Tool (SOCRAT)-to assist sport organizations in assessing the overall risk of concussion at a team level by identifying key risk factors.METHODS: We first conducted a literature review to identify risk factors of concussion using ice hockey as a model. We then developed an algorithm by combining the severity and the probability of occurrence of concussions of the identified risk factors by adapting a risk assessment tool commonly used in engineering applications.RESULTS: The following risk factors for ice hockey were identified: age, history of previous concussions, previous body checking experience, allowance of body checking, type of helmet worn and the game or practice environment. These risk factors were incorporated into the algorithm, resulting in an individual risk priority number (RPN) for each risk factor and an overall RPN that provides an estimate of the risk in the given circumstances.CONCLUSION: The SOCRAT can be used to analyse how different risk factors contribute to the overall risk of concussion. The tool may be tailored to organizations to provide: (1) an RPN for each risk factor and (2) an overall RPN that takes into account all the risk factors. Further work is needed to validate the tool based on real data.

Abstract:Osteomyelitis in the adult patient has been associated with failure of eradication, late recurrence, nonunion, and prolonged hospitalization. A staged aggressive approach has been used for the past seven years to treat 53 patients with adult osteomyelitis. This approach includes: evaluation of bone necrosis and identification of the etiologic organisms by deep bone culture; radical surgical debridement of devascularized tissue; intensive systemic antibiotics; and early bone and soft tissue reconstruction. All patients have been followed at least 1 year (mean, 33 months). Lower extremity bones predominated in the series (24 tibias, 13 femurs); and 19 patients had bony instability. Thirty-seven patients had initial successful eradication of their infections with 26 of these returning to full activity status. The remaining 16 patients developed recurrent infection; however, 11 patients totally responded to further aggressive treatment. Of the five failures in the total series, three patients required amputation and two patients have persistent infection. Fifteen of the 19 patients with bony instability healed with initial treatment, and the remaining four patients healed with subsequent treatment. Six patients had primary muscle flap soft tissue reconstruction, and an additional two patients had reconstruction as a secondary procedure. In all these patients with tibial instability, bony union was accelerated compared to those patients with tibial instability not receiving muscle flaps (4 months vs 12 months). The muscle coverage provided by either pedicled flaps or transferred by microvascular anastomoses.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract:Background Lower extremity injuries account for an enormous portion of sports medicine cases in the United States each year. Unfortunately, there are no uniform criteria for athletes to complete prior to returning to sport (RTS) following a lower extremity injury. Therefore, the purpose of this systematic review is to review current literature for joint-specific and global lower extremity testing to determine the most valid functional test that can be utilized to reduce the risk of re-injury as athletes RTS. Methods A systematic search of PubMed, PubMed Central, Cochrane Library, OVID, and Embase databases was conducted for studies prior to May 2024 following PRISMA guidelines. ROBINS-I Tool was utilized for the risk of bias assessment. Results Of 19,189 studies, 114 (0.6%) studies published prior to May of 2024 met inclusion criteria and were analyzed. Eighty five percent of articles discussed RTS for individuals with knee pathology. Furthermore, 82% specifically analyzed RTS following ACL reconstruction. The most common RTS test was isokinetic dynamometry testing which is seen in 73% of studies. Only 6.2% of studies analyzed RTS for individuals with hip pathology and only two studies analyzed RTS for patients with ankle injuries. Conclusion Even with the enormous amount of literature that exists regarding ACL injuries and testing there is no standardized criterion for RTS clearance. The suggested test batteries from this review can serve as a framework for future research and validation for joint-specific RTS functional testing.

Predictors of lower extremity injuries in team sports (PROFITS-study): a study protocol

Abstract:Introduction: Several intrinsic risk factors for lower extremity injuries have been proposed, including lack of proper knee and body control during landings and cutting manoeuvres, low muscular strength, reduced balance and increased ligament laxity, but there are still many unanswered questions. The overall aim of this research project is to investigate anatomical, biomechanical, neuromuscular, genetic and demographic risk factors for traumatic non-contact lower extremity injuries in young team sport athletes. Furthermore, the research project aims to develop clinically oriented screening tools for predicting future injury risk. Methods: Young female and male players (n=508) from nine basketball teams, nine floorball teams, three ice hockey teams, and one volleyball team accepted the invitation to participate in this four-and-half-year prospective follow-up study. The players entered the study either in 2011, 2012 or 2013, and gave blood samples, performed physical tests and completed the baseline questionnaires. Following the start of screening tests, the players will be followed for sports injuries through December 2015. The primary outcome is a traumatic non-contact lower extremity injury. The secondary outcomes are other sports-related injuries. Injury risk is examined on the basis of anatomical, biomechanical, neuromuscular, genetic and other baseline factors. Univariate and multivariate regression models will be used to investigate association between investigated parameters and injury risk.

Abstract:<h3 class="Heading">Background</h3><p class="Para">There has been a recent increase in research examining training load as a method of mitigating injury risk due to its known detrimental effects on player welfare and team performance. The acute:chronic workload ratio (ACWR) takes into account the current training load (acute) and the training load that an athlete has been prepared for (chronic). The ACWR can be calculated using; (1) the rolling average model (RA) and (2) the exponentially weighted moving average model (EWMA).</p><h3 class="Heading">Objective</h3><p class="Para">The primary aim of this systematic review was to investigate the literature examining the association between the occurrence of injury and the ACWR and to investigate if sufficient evidence exists to determine the best method of application of the ACWR in team sports.</p><h3 class="Heading">Methods</h3><p class="Para">Studies were identified through a comprehensive search of the following databases: EMBASE, Medline, SPORTDiscus, SCOPUS, AMED and CINAHL. Extensive data extraction was performed. The methodological quality of the included studies was assessed according to the Newcastle–Ottawa Scale (NOS) for Cohort Studies.</p><h3 class="Heading">Results</h3><p class="Para">A total of 22 articles met the inclusion criteria. The assessment of article quality had an overall median NOS score of 8 (range 5–9). The findings of this review support the association between the ACWR and non-contact injuries and its use as a valuable tool for monitoring training load as part of a larger scale multifaceted monitoring system that includes other proven methods. There is support for both models, but the EWMA is the more suitable measure, in part due to its greater sensitivity. The most appropriate acute and chronic time periods, and training load variables, may be dependent on the specific sport and its structure.</p><h3 class="Heading">Conclusions</h3><p class="Para">For practitioners, it is the important to understand the intricacies of the ACWR before deciding the best method of calculation. Future research needs to focus on the more sensitive EWMA model, for both sexes, across a larger range of sports and time frames and also combinations with other injury risk factors.</p>

[Immunomodulatory properties of statins and cancer risk].

Abstract:Statins are cholesterol-lowering drugs widely prescribed for cardiovascular prevention. They are also immunomodulatory and increase peripheral regulatory T cell numbers and function, in vivo. This may be beneficial by decreasing inflammatory response within the atheroma and stabilizing atherosclerotic plaque, however this novel pleiotropic effect might impair host tumor-specific effector T cell responses. Indeed, in some randomized trials, notwithstanding their short duration, statins have been found to increase cancer incidence especially in the elderly and women. In these situations, the decrease in cardiovascular mortality can be matched by an equal increase in cancer mortality, leaving all-cause mortality unchanged. Furthermore, statins have been shown to interfere with cancer immunotherapy. Preventive pharmacological treatment can sometimes be justified because of reduced cardiovascular events even if total mortality does not change, but if it leads to an increase in cancer incidence, it might be not easily accepted. Physicians need to weigh the global risks and benefits when prescribing statins.
Evaluating the impact of injury prevention interventions in child and adolescent sports using the RE-AIM framework and CERT: A systematic review

Abstract:Participation in sport is a popular pastime for children and adolescents that improves their physical health, mental health and motor skills. Musculoskeletal injuries are a relatively common downside of sports participation and can have negative long-term consequences. Injury prevention programmes have demonstrated effectiveness in child and adolescent sports, provided compliance is adequate. However, little is known about the factors which relate to their impact on the wider community and whether the prevention programmes have been adopted and maintained in the long-term. The objective of this review was to assess the current literature on exercise-based injury prevention interventions in child and adolescent sports (aged under 19 years) against the 'Reach' , 'Effectiveness' , 'Adoption' , 'Implementation' , 'Maintenance' (RE-AIM) framework and Consensus of Exercise Reporting Template (CERT), to ascertain level of reporting for the components which relate to external validity. Seven electronic databases; PubMed, Medline, SPORTDiscus, PsycINFO, CINAHL, Scopus and The Cochrane Library, were searched from date of inception to July 2022 using the themes of: ' Child and Adolescent' , 'Sport' , 'Injury' and 'Prevention' . Eligibility criteria included: Experimental trial design, exercise-based intervention programmes, primary outcome of injury incidence and participants aged under 19 years. Two reviewers assessed each trial independently against the RE-AIM model dimension items checklist (RE-AIM MDIC) and Consensus on Exercise Reporting Template (CERT) before reaching a consensus on reporting. Forty-five unique trials met the eligibility criteria. Mean reporting level for all studies across the whole RE-AIM MDIC was 31% (SD ± 16.2%, Range 7–77%). The domain of 'effectiveness' was the most comprehensively reported (60%), followed by 'implementation' (48%), 'reach' (38%), 'adoption' (26%) and 'maintenance' (7%). The mean reporting score for the CERT was 50% (SD ± 20.8, range 0–81%). Published data on injury prevention in child and adolescent sports is highly focussed on the effectiveness of the intervention, with little consideration given to how it will be adopted and maintained in the long-term. This has led to considerable gaps in knowledge regarding optimal programme implementation, with a lack of data on adoption and maintenance contributing to the gap between research and practice. Future research needs to place greater focus on external validity and consider incorporating the study of implementation and feasibility as part of effectiveness trial design. This approach should provide the data that will help narrow the considerable gap between science and practice. PROSPERO Registration number CRD42021272847.

Abstract:Background: Core stability has been suggested to influence lower extremity functioning and might contribute to the development of lower extremity overuse injuries. However, prospective studies to investigate this relationship are limited. Purpose: To research the role of different components of core stability as risk factors for the development of lower extremity overuse injuries. Study Design: Cohort study; Level of evidence, 2. Methods: A total of 142 first-year physical education students participated in this study. They were tested in 2015 and were prospectively followed for 1.5 years by means of a multilevel injury registration method. Three participants were excluded owing to physical complaints during testing. As such, 139 participants were included in the statistical analysis. At baseline, dynamic postural control, isometric core and hip muscle strength, core muscle endurance, core neuromuscular control and proprioception, and functional movement were measured for all participants. Competing risk regression analyses were performed to identify significant contributors to the development of lower extremity overuse injuries. Results: During the follow-up period, 34 (24%) of the 139 participants developed a lower extremity overuse injury. Significant predictive effects for an overuse injury were found for an increased side-by-side difference in dynamic postural control ( P = .038), decreased isometric hip extension:flexion strength ratio ( P = .046), and decreased abdominal core muscle endurance ( P = .032). Conclusion: This study identified measures for dynamic postural control, core muscle strength, and core muscle endurance as significant risk factors for the development of overuse injuries after statistical model building. However, core neuromuscular control and proprioception and functional movement might not allow clinicians to identify patients at risk. These accessible, reliable screening tools could be used in clinical practice with regard to screening and injury prevention for overuse injuries. Injury prediction based on this model needs to be done with caution given the low relative predictive accuracy (53%).
