Abstract:Abstract The external validity of trial results is a matter of debate, and no strong evidence is available to support whether a trial may have a positive or a negative effect on the outcome of patients. Methods. We compared the results of stage IV colorectal cancer patients treated within a large Dutch phase III trial (CAIRO), in which standard chemotherapy and standard safety eligibility criteria were used, to patients treated outside the trial during the trial accrual period in a representative selection of 29 Dutch hospitals. Non-trial patients were identified by the Netherlands Cancer Registry (NCR), and were checked for the trial eligibility criteria. Results. The NCR registered 1946 stage IV colorectal cancer patients who received chemotherapy, of whom 394 patients were included in the CAIRO trial and 30 patients in other trials. Thus, the CAIRO trial participation rate was 20%. In the 29 hospitals, 162 patients received chemotherapy in the trial and 396 patients received chemotherapy outside the trial. Of the non-trial patients, 224 patients fulfilled the trial eligibility criteria. The overall survival of eligible non-trial patients was comparable to trial patients (HR 1.03, p = 0.70). However, non-eligible non-trial patients had a significantly worse outcome (HR 1.70, p < 0.01). Conclusion. These data provide evidence in a common tumor type that trial results have external validity, provided that standard eligibility criteria are being observed. Our finding of a worse outcome for patients not fulfilling these criteria strongly argues against the use of cancer treatments in other patient categories than included in the original trials in which these treatments were investigated.
Wiki-health: A big data platform for health sensor data management

Abstract:Quickly evolving modern technologies such as cloud computing, Internet of things, and intelligent data analysis have created great opportunities for better living. The authors visualize the role these technological innovations will play in the healthcare sector as they spearhead a shift in focus from offering better healthcare services only to people with problems to helping everyone achieve a healthier lifestyle. In this chapter, the authors first discuss the existing and potential barriers followed by an in-depth demonstration of a service platform named Wiki-Health that takes advantage of cloud computing and Internet of things for personal well-being data management. It is a social platform, which is designed and implemented for data-driven and context-specific discovery of citizen communities in the areas of health, fitness, and well-being. At the end of the chapter, the authors analyse a case study to illustrate how the Wiki-Health platform can be used to serve a real world personal health training application.

Abstract:Do academics, both directly and indirectly involved with healthcare, have a moral mandate to ensure that Wikipedia has the most accurate, up-to-date and understandable information? From the perspective of a physician who is also a long-time Wikipedia editor, the ethical, moral, and power dynamics of the medical community's interaction with Wikipedia are explored in this paper. An attempt is made to reconcile and identify the key stakeholders affected by Wikipedia's accuracy and credibility, including medical institutions such as peer-reviewed journals, medical schools, research funders and academic reward systems. These stakeholders act as the true guardians of <i>Primum non nocere</i> – first to do no harm. Finally, suggestions are made on how to increase the sharing of medical knowledge on Wikipedia by focusing advocacy towards these 'true guardians' rather than relying on the benevolence of individual academics.

Abstract:Background: Several health institutions developed strategies to improve health content on Wikimedia platforms given their unparalleled reach. The objective of this study was to compare an online volunteer-based Wikimedia outreach campaign with university course Wikipedia assignments (both focused on improving hearing health content in Wikimedia's public digital knowledge archives), in terms of the reach of the contributions and the extent of the participants' input. A secondary objective was to examine the feasibility and the implementation of the different strategies. Methods: The research team partnered for the (1) coordination of improvements in hearing and healthcare content through educational programs using Wikimedia platforms, (2) participation in the global campaign Wiki4WorldHearingDay2023 and (3) evaluation of the proposed strategies. Metrics used in the comparison of the two strategies included the number of articles edited, number of views of the edited articles (as reach) and the extent of edits, captured as the number of words. The feasibility evaluation included assessing recruitment success and the implementation of the proposed plan among faculty, students from various university programs, and volunteers representing different constituencies. Results: The effort increased the availability of quality plain language information on hearing conditions and hearing care. Both strategies demonstrated to be feasible by their success in recruiting participants who contributed to the effort and by measurable outputs as edits. The contribution of content to Wikimedia platforms as part of education activities provided a more robust result. Wiki4WorldHearingDay2023 145 participants (78 from educational programs) contributed 167,000 words, 258 + references and 140 images to 322 Wikipedia articles (283 existing and 39 new ones), which were viewed 16.5 million times. Contributions occurred in six languages. Edits in Portuguese, mainly by those involved in educational programs, led the number of articles (226 or 70.2%) that were expanded or created during the 5-month tracking period. Conclusions: The elements that contributed to the success of the studied strategies include an impact topic, coordination with educational programs, international multidisciplinary collaborations, the dissemination of the initiative in several platforms, connection with a robust local Wikimedia affiliate, and the use of a technical infrastructure that provides metrics and coordination mechanisms.

Abstract:An asymmetric autocatalytic reaction has been catalyzed by a mixture of chiral and achiral beta-amino alcohols. The absolute configuration of the highly enantioenriched obtained product (>98% ee) was shown to depend not only on the absolute configuration of the chiral catalyst but also on the structure and the amount of achiral catalyst. Even in default versus the chiral catalyst, achiral catalysts were shown to be able to reverse the enantioselectivity of the reaction.
Mental health clinicians' information-seeking and use of Wikimedia platforms

Abstract:Internet sites are increasingly being used to disseminate clinical resources to mental health clinicians. Wikimedia platforms (e.g., Wikipedia and Wikiversity) are open-source, freely available resources that are among the most visited websites and could be an effective way to disseminate mental health information. An important first step in learning how to optimize the impact of such dissemination efforts is to understand how and for what reasons practicing clinicians already engage with these resources. Using a convenience sample of 120 practicing mental health clinicians (82.5% female, 85.6% white, M age = 41.31), we assessed clinician-reported practices about where and how they seek information about psychological science on the internet. Our results showed that freely available resources related to mental health are not a primary source of information for mental health clinicians and that clinicians have low confidence in the veracity of information available through Wikipedia and Wikiversity. Clinicians shared strategies (e.g., implementing verification steps on Wiki platform pages) that could increase clinician confidence in the information provided and their likelihood of using these sites as resources. Overall, this study indicates that mental health clinicians are not regularly using Wiki sites for mental health-related questions, even though they are accessible, regularly updated, and increasingly reliable. Findings suggest that implementing verification steps on Wiki pages could increase clinicians’ confidence in the information provided and their likelihood in using these sites as resources, however, it remains unclear whether verification steps would lead to more frequent use or how such a system would be implemented through existing platforms. Results will help guide future dissemination efforts to increase the availability and utilization of evidence-based psychological science online for clinicians.

Enculturating a Community of Action (CoA): A qualitative study of health professions educators’ perspectives on teaching with Wikipedia

Abstract:Abstract Purpose There is a growing desire for health professions educators to engage learners in more meaningful instruction. Many have tapped Wikipedia to offer an applied approach to engage learners, particularly as it relates to evidence-based medicine (EBM). However, little is known about the benefits and challenges of using Wikipedia as a pedagogical tool from the collective experience of educators who have sought to improve their instructional practice with it. This study aims to uncover and synthesize the perspectives of health professions education (HPE) instructors who have incorporated Wikipedia in their HPE courses. Methods Applying a constructivist approach, the authors conducted semi-structured interviews with 17 participating HPE instructors who had substantively integrated Wikipedia into their curriculum. Participants were interviewed about their experiences of integrating Wikipedia editing into their courses. Thematic analysis was conducted on resulting transcripts. Results Authors observed two broad themes among participants’ expressed benefits of teaching with Wikipedia: 1) provides a meaningful instructional alternative that also benefits society and develops learners’ information literacy and EBM skills, and 2) supports learners’ careers and professional identity formation. Identified challenges included: 1) high effort and time, 2) issues with sourcing references, and 3) challenging interactions with skeptics, editors, and students. Discussion Findings build on known benefits, such as providing a real-world collaborative project that contextualizes students’ learning experiences. They also echo known challenges, such as the resource-intensive nature of teaching with Wikipedia. At the same time, findings extend the current literature by revealing a potential opportunity to approach crowd-sourced information tools, like Wikipedia, as a vehicle to engage and enculturate HPE students within a situated learning context. These findings present implications for HPE programs considering implementing Wikipedia and faculty development needed to help instructors harness crowd-sourced information tools’ pedagogical opportunities as well as anticipate their challenges.
A Quantitative Portrait of Wikipedia's High-Tempo Collaborations during the 2020 Coronavirus Pandemic

Abstract:The 2020 coronavirus pandemic was a historic social disruption with significant consequences felt around the globe. Wikipedia is a freely-available, peer-produced encyclopedia with a remarkable ability to create and revise content following current events. Using 973,940 revisions from 134,337 editors to 4,238 articles, this study examines the dynamics of the English Wikipedia's response to the coronavirus pandemic through the first five months of 2020 as a "quantitative portrait" describing the emergent collaborative behavior at three levels of analysis: article revision, editor contributions, and network dynamics. Across multiple data sources, quantitative methods, and levels of analysis, we find four consistent themes characterizing Wikipedia's unique large-scale, high-tempo, and temporary online collaborations: external events as drivers of activity, spillovers of activity, complex patterns of editor engagement, and the shadows of the future. In light of increasing concerns about online social platforms' abilities to govern the conduct and content of their users, we identify implications from Wikipedia's coronavirus collaborations for improving the resilience of socio-technical systems during a crisis.

Abstract:Traditional disease surveillance systems suffer from several disadvantages, including reporting lags and antiquated technology, that have caused a movement towards internet-based disease surveillance systems. This study presents the use of Wikipedia article content in this sphere. We demonstrate how a named-entity recognizer can be trained to tag case, death, and hospitalization counts in the article text. We also show that there are detailed time series data that are consistently updated that closely align with ground truth data. We argue that Wikipedia can be used to create the first community-driven open-source emerging disease detection, monitoring, and repository system.

Abstract:Wikipedia has evolved beyond its original function as an online encyclopedia in an increasingly complex data-driven society. The social platform is met with a balancing act between collective intelligence and mass surveillance; processes need to be developed to protect individuals and the community from government mass surveillance without sacrificing the important contributions made through prohibited anonymous communication software. Case studies are provided from NSA government surveillance practices, the anti-SOPA legislation movement, and research that covers Wikipedia's involvement with participatory journalism, disinformation, self-censorship, and the use of Tor. This paper proposes that a common ground can be developed between individuals, public and private institutions through future research in socio-cultural anthropology and policy frameworks around data retention and government accountability. Wikipedia is used as an example within the US intelligence community as a complex organisation that can adapt to changes through its iterative nature, which draws insight into how policy frameworks can be future-proofed. Finally, this paper is a wake-up call to individuals, private institutions, and governments to remain vigilant about the storage and use of personal information as a result of contributing to online communities.

Abstract:Using 16,068 articles in Wikipedia's Medicine Wikiproject, we study the relationship between collaboration and quality. We assess whether certain collaborative patterns are associated with information quality in terms of self-evaluated quality and article viewership. We find that the number of contributors has a curvilinear relationship to information quality, more contributors improving quality but only up to a certain point. Other articles that its collaborators work on also influences the quality of an information artifact, creating an interdependent network of artifacts and contributors. Finally, we see evidence of a recursive relationship between information quality and contributor activity, but that this recursive relationship attenuates over time.

Abstract:While many instructors have reservations against Wikipedia use in academic settings, editing Wikipedia teaches students valuable writing, editing, and critical thinking skills. Wikipedia assignments align with the community of inquiry framework, which focuses on the elements needed for a successful online learning experience. We report on a faculty mentoring network, created by WikiProject Limnology and Oceanography, which helped 14 instructors with little to no prior experience implement a Wikipedia assignment in their classes. We found that Wikipedia assignments increase students' motivation to produce high quality work and enhance their awareness of reliable scientific sources. Wikipedia assignments can be comparable to other writing assignments in length and complexity, but have a far wider audience than a traditional research paper. Participants in our mentoring network reported challenges with implementing this new type of assignment, and here, we share resources and solutions to those reported barriers.

Abstract:Wikipedia is an open-source online encyclopedia and one of the most-read sources of online health information. Likewise, Wikipedia page views have also been analyzed to inform public health services and policies. The present review analyzed 29 studies utilizing Wikipedia page views for health research. Most reviewed studies were published in recent years and emanated from high-income countries. Together with Wikipedia page views, most studies also used data from other internet sources, such as Google, Twitter, YouTube, and Reddit. The reviewed studies also explored various non-communicable diseases, infectious diseases, and health interventions to describe changes in the utilization of online health information from Wikipedia, to examine the effect of public events on public interest and information usage about health-related Wikipedia pages, to estimate and predict the incidence and prevalence of diseases, to predict data from other internet data sources, to evaluate the effectiveness of health education activities, and to explore the evolution of a health topic. Given some of the limitations in replicating some of the reviewed studies, future research can specify the specific Wikipedia page or pages analyzed, the language of the Wikipedia pages examined, dates of data collection, dates explored, type of data, and whether page views were limited to Internet users and whether web crawlers and redirects to the Wikipedia page were included. Future research can also explore public interest in other commonly read health topics available in Wikipedia, develop Wikipedia-based models that can be used to predict disease incidence and improve Wikipedia-based health education activities.

Abstract:Wikipedia, the open crowdsourced encyclopedia that anyone can edit, ranks among the top ten most-visited websites globally. Its integration into university curriculum as an innovative educational tool is a slowly growing trend; however, many higher education institutions have yet to fully grasp its potential. In response, a specific optional module for Wikipedia editing, designed for the selected undergraduate science courses at the School of Advanced Studies, Russia, was implemented as an optional extra credit service-learning activity, a teaching methodology combining meaningful service to the community with curriculum-based learning. Students who chose to participate and those who preferred not to participate in the activity were invited to participate in a research project to explore their perspectives and experiences. In total, five sessions of focus group discussions were conducted with participants (12 females and 2 males) in one set and non-participants (5 females and 4 males) in another to identify students' perspectives on themes such as their interest in science, reasons for their choices, and their expectations before the activity while post-experience focus group discussions were used to identify the perspectives of participant students on themes, encompassing contribution of the service-learning activity, acquisition of new skills, and the development of prosocial behaviors. Students' opinions on integrating social responsibility topics into the curriculum were also explored. The results extracted from these focus group discussions, analyzed through consensual coding, revealed factors promoting student participation, like interest in the subject, novelty of the activity, and grade improvement opportunities, as well as factors deterring participation, such as concerns about academic benefits, workload, and time constraints. Furthermore, the results demonstrated that Wikipedia editing serves as a novel teaching methodology, promoting student learning and development in digital literacy and information literacy, which are among the twenty-first-century skills. Interestingly, at the same time, not all students could address the value of contributing to open, crowdsourced knowledge for public service or interpret this activity as an academic service-learning. These suggest that Wikipedia editing is an innovative teaching approach, fostering students' learning and development while also indicating its potential to enhance students' understanding of responsible citizenship and public service in the digital age.

Abstract:There are over a billion websites on the Internet that can potentially serve as sources of information on various topics. One of the most popular examples of such an online source is Wikipedia. This public knowledge base is co-edited by millions of users from all over the world. Information in each language version of Wikipedia can be created and edited independently. Therefore, we can observe certain inconsistencies in the statements and facts described therein - depending on language and topic. In accordance with the Wikipedia content authoring guidelines, information in Wikipedia articles should be based on reliable, published sources. So, based on data from such a collaboratively edited encyclopedia, we should also be able to find important sources on specific topics. This effect can be potentially useful for people and organizations. The reliability of a source in Wikipedia articles depends on the context. So the same source (website) may have various degrees of reliability in Wikipedia depending on topic and language version. Moreover, reliability of the same source can change over the time. The purpose of this study is to identify reliable sources on a specific topic - the COVID-19 pandemic. Such an analysis was carried out on real data from Wikipedia within selected language versions and within a selected time period.

Abstract:Neurofibromatosis type 1 (NF1), an autosomal dominantly‐inherited disorder, is mainly characterized by the occurrence of multiple dermal neurofibromas and is caused by mutations in the NF1 gene, a tumor suppressor gene. The variable expressivity of the disease and the lack of a genotype/phenotype correlation prevents any prediction of patient outcome and points to the action of genetic factors in addition to stochastic factors modifying the severity of the disease. The analysis of somatic NF1 gene mutations in neurofibromas from NF1 patients revealed that each neurofibroma results from an individual second hit mutation, indicating that factors that influence somatic mutation rates may be regarded as potential modifiers of NF1. A mutational screen of numerous neurofibromas from two NF1 patients presented here revealed a predominance of point mutations, small deletions, and insertions as second hit mutations in both patients. Seven novel mutations are reported. Together with the results of studies that showed LOH as the predominant second hit in neurofibromas of other patients, our results suggest that in different patients different factors may influence the somatic mutation rate and thereby the severity of the disease. Longitudinal Assessment of Reference Quality on Wikipedia

Abstract:Wikipedia plays a crucial role in the integrity of the Web. This work analyzes the reliability of this global encyclopedia through the lens of its references. We operationalize the notion of reference quality by defining reference need (RN), i.e., the percentage of sentences missing a citation, and reference risk (RR), i.e., the proportion of non-authoritative references. We release Citation Detective, a tool for automatically calculating the RN score, and discover that the RN score has dropped by 20 percent point in the last decade, with more than half of verifiable statements now accompanying references. The RR score has remained below 1% over the years as a result of the efforts of the community to eliminate unreliable references. We propose pairing novice and experienced editors on the same Wikipedia article as a strategy to enhance reference quality. Our quasi-experiment indicates that such a co-editing experience can result in a lasting advantage in identifying unreliable sources in future edits. As Wikipedia is frequently used as the ground truth for numerous Web applications, our findings and suggestions on its reliability can have a far-reaching impact. We discuss the possibility of other Web services adopting Wiki-style user collaboration to eliminate unreliable content.

Content Growth and Attention Contagion in Information Networks: Addressing Information Poverty on Wikipedia

Abstract:Open collaboration platforms have fundamentally changed the way that knowledge is produced, disseminated, and consumed. Although the community governance and open collaboration model of Wikipedia confers many benefits, its decentralized nature can leave questions of information poverty and skewness to the mercy of the system's natural dynamics. In this paper, we leverage a large-scale natural experiment to gain a causal understanding of how exogenous content contributions to Wikipedia articles affect the attention that they attract and how that attention spills over to other articles in the information network. We find a positive feedback loop: content contribution leads to significant and long-lasting increases of attention and future contribution. Unfortunately, this also suggests that impoverished regions of information networks are likely to remain so in the absence of intervention. However, our analysis reveals a potential solution. Articles in impoverished regions of information networks are particularly positioned to benefit from the phenomenon of attention spillovers. Using a simulation that is calibrated with real-world link traffic of the Wikipedia network, we show that an attention contagion policy, which focuses editorial effort coherently on impoverished regions, can lead to as much as a twofold gain in attention relative to unguided contributions.

Abstract:Background In the current era of widespread access to the internet, we can monitor public interest in a topic via information-targeted web browsing. We sought to provide direct proof of the global population’s altered use of Wikipedia medical knowledge resulting from the new COVID-19 pandemic and related global restrictions. Objective We aimed to identify temporal search trends and quantify changes in access to Wikipedia Medicine Project articles that were related to the COVID-19 pandemic. Methods We performed a retrospective analysis of medical articles across nine language versions of Wikipedia and country-specific statistics for registered COVID-19 deaths. The observed patterns were compared to a forecast model of Wikipedia use, which was trained on data from 2015 to 2019. The model comprehensively analyzed specific articles and similarities between access count data from before (ie, several years prior) and during the COVID-19 pandemic. Wikipedia articles that were linked to those directly associated with the pandemic were evaluated in terms of degrees of separation and analyzed to identify similarities in access counts. We assessed the correlation between article access counts and the number of diagnosed COVID-19 cases and deaths to identify factors that drove interest in these articles and shifts in public interest during the subsequent phases of the pandemic. Results We observed a significant (P<.001) increase in the number of entries on Wikipedia medical articles during the pandemic period. The increased interest in COVID-19–related articles temporally correlated with the number of global COVID-19 deaths and consistently correlated with the number of region-specific COVID-19 deaths. Articles with low degrees of separation were significantly similar (P<.001) in terms of access patterns that were indicative of information-seeking patterns. Conclusions The analysis of Wikipedia medical article popularity could be a viable method for epidemiologic surveillance, as it provides important information about the reasons behind public attention and factors that sustain public interest in the long term. Moreover, Wikipedia users can potentially be directed to credible and valuable information sources that are linked with the most prominent articles.
