Abstract:We have read the case report entitled “A new technique of reduction for isolated proximal tibiofibular joint dislocation: a case report” with great interest1. We want to thank to appreciate the authors for the contribution to the literature with their new technique. We want also to present our additions with the valuable authors and readers through you. As mentioned in the study, isolated proximal tibiofibular joint dislocations are not frequently seen in emergency departments (ED) but they are orthopedic emergency cases. According to the study, reduction of the talofibular joint dislocations with the new technique has advantages for the patient and physician. Before the reduction, local anesthesia was applied with 10 cc lidocaine for the pain management in the study. We want to mention that regional block also can be used as an alternative to the local anesthesia in these patients. It is known that in recent years, local and regional blocks can be applied safely and quickly in the reduction fractures and dislocations with the widespread use of ultrasonography in ED2. Sensorial and motor innervation of proximal tibiofibular joint is usually provided by common fibular nerve. It can be easily blocked at popliteal region with USG guidance, and it provides a valuable addition to the reduction procedure3. Regional blocks are also superior to procedural sedation with lack of serious complications. Moreover, it is known that vascular and nerve injuries can be seen in these cases. Especially tibial artery and common peroneal nerve injury may accompany4. It should be in mind that cases that have pain at proximal tibiofibular joint region can be associated with isolated proximal tibiofibular joint dislocation after high energy trauma. In these cases, detailed distal extremity sensation, motor and periphery artery examination should be done and findings should be noted. As mentioned in the study, in the presence of vascular and nerve injury, choose of closed reduction or open reduction in the surgery room is controversial.

The confrontation and the connections between French philosophical thought and certain research in neuropsychology, such as that of Kurt Goldstein or Oliver Sacks, allow the possibility of new approaches to the dimension of sickness to emerge, approaches between narration, speculation, and scientific research. A fertile dialogue between disciplines and fields of different research brings to light how every perceptual modality, even when it presents itself as deficient or deviant, constitutes in fact a true re-creation or reconstitution of the world and not only a recording. This process of continuous creation is linked to one of the basic questions raised by the experience of sickness: the question of the modes and conditions of singularity, of the identity of a self in pursuit of recognition or acceptance from another’s gaze or gesture, a gaze or gesture that, in the relation between human subjects, we have the ability to contribute ever in a unique manner.

Abstract:For decades, physicians wrote much of the history of medicine, often "great man" histories that celebrated their colleagues' accomplishments as part of a celebratory historical narrative. Beginning in the 1970s, social historians challenged this type of scholarship, arguing that it was Whiggish, omitted the flaws of the medical profession, left patients out of the story, and ignored issues of gender, race, and class. This Garrison Lecture revisits this history through the prism of my recent book, The Good Doctor: A Father, a Son, and the Evolution of Medical Ethics, which is essentially a biography of my physician father, Phillip Lerner, and an autobiography. In the talk I ask whether there is true historical value to biography or whether it should serve only as an adjunct to "real" social history. I also historicize my own career, something I chose not to do in the book.

Regional adipose tissue metabolism in men and postmenopausal women.

Abstract:The metabolism of subcutaneous abdominal and femoral fat cells was studied in postmenopausal women and matched men of similar age and weight index. Lipoprotein lipase (LPL) activity was similar in both regions in the postmenopausal women. However, a group of women who underwent treatment with estrogen and progestins had considerably higher LPL activity in the femoral cells. Testosterone treatment for 7 days did not change LPL in either region in middle-aged men. Femoral adipocytes from both men and postmenopausal women were markedly unresponsive to catecholamine in comparison with abdominal cells. No clear effect of the sex hormones was seen on lipolysis in the women. Treating men with testosterone increased basal lipolysis in the abdominal cells. Abdominal fat cells were more responsive to the antilipolytic effect of insulin. Both responsiveness and sensitivity to insulin were somewhat greater in the abdominal fat cells from men (ED50 approximately 0.02 nm) than from women (ED50 approximately 0.17 nm). Insulin binding, however, was similar in fat cells from the abdominal and the femoral regions when the results were expressed per unit surface area. Thus, regional differences in fat cell metabolism are seen in both postmenopausal women and elderly men. LPL activity in the femoral region seems to be under the control of female sex hormones while lipolysis appears to be less influenced by sex hormones.

Abstract:The road between a hypothesis about a disease or condition and its cure or palliation is never simply linear. There are many tantalizing tangents to be chased and many seemingly obvious truths with countless exceptions. This is usually a feature, not a bug, as they say in computer programming. In the tangents and exceptions are clues and alternative roads to science and medicine that can provide cures and palliative measures, sometimes for diseases or conditions other than the one being studied. The narrative that follows uses the author's scientific experience in childhood nervous system cancer to illustrate the importance of a robust, bidirectional interaction between the laboratory bench and the clinic bedside in the quest for solutions to problems of health, longevity, and quality of life.

Abstract:In 2020, as a 24-year-old MD PhD student studying cardiovascular population health, I suffered a non-ST elevated myocardial infarction while playing basketball. It was confirmed the cause was familial hypercholesterolemia leading to 95%+ occlusions of both the left anterior descending and left circumflex arteries. The following reflective essay describes my experience as a patient grappling with his own mortality and how those experiences have shaped how I now view my purpose as a future physician. The purpose of this essay is to encourage all physicians and physicians in training to practice reflection and to allow themselves to fully experience the pain and suffering of the patients for whom they care, even amidst times of personal uncertainty.

Abstract:Genetics has become a critical component of medicine over the past five to six decades. Alongside genetics, a relatively new discipline, dysmorphology, has also begun to play an important role in providing critically important diagnoses to individuals and families. Both have become indispensable to unraveling rare diseases. Almost every medical specialty relies on individuals experienced in these specialties to provide diagnoses for patients who present themselves to other doctors. Additionally, both specialties have become reliant on molecular geneticists to identify genes associated with human disorders. Many of the medical geneticists, dysmorphologists, and molecular geneticists traveled a circuitous route before arriving at the position they occupied. The purpose of collecting the memoirs contained in this article was to convey to the reader that many of the individuals who contributed to the advancement of genetics and dysmorphology since the late 1960s/early 1970s traveled along a journey based on many chances taken, replying to the necessities they faced along the way before finding full enjoyment in the practice of medical and human genetics or dysmorphology. Additionally, and of equal importance, all exhibited an ability to evolve with their field of expertise as human genetics became human genomics with the development of novel technologies.

Patient Subjectivity and the Exploration of Intersubjectivity in Clinical Context

Abstract:The exploration of patient subjectivity and doctor-patient intersubjectivity has a long history. The flourishing of narrative medicine has enriched the academic connotation of patient subjectivity in historical narrative and the essential speculation of doctor-patient intersubjectivity in clinical context. On the one hand, the awakening and liberation process of patient subjectivity in the history of medicine is thoroughly reviewed. It is pointed out that modern and contemporary mainstream medical historians have been alert to the obvious bias of doctor-patient subjectivity in the historical narrative, and devote themselves to strengthening the status and value of patients in the medical history narrative. On the other hand, the two basic characteristics of doctor-patient intersubjectivity in clinical circumstances are revealed, namely, the dignity of the weak and the care of others. Based on this, altruistic feelings such as empathy, compassion, compassion and awe are expanded, and care programs of accompanying, witnessing, soothing and settling are enriched, which will provide a new theoretical fulcrum for the harmonious picture of doctors and patients.

Abstract:Introduced to U.S. medical schools in 1972, the field of literature and medicine contributes methods and texts that help physicians develop skills in the human dimensions of medical practice. Five broad goals are met by including the study of literature in medical education: 1) Literary accounts of illness can teach physicians concrete and powerful lessons about the lives of sick people; 2) great works of fiction about medicine enable physicians to recognize the power and implications of what they do; 3) through the study of narrative, the physician can better understand patients' stories of sickness and his or her own personal stake in medical practice; 4) literary study contributes to physicians' expertise in narrative ethics; and 5) literary theory offers new perspectives on the work and the genres of medicine. Particular texts and methods have been found to be well suited to the fulfillment of each of these goals. Chosen from the traditional literary canon and from among the works of contemporary and culturally diverse writers, novels, short stories, poetry, and drama can convey both the concrete particularity and the metaphorical richness of the predicaments of sick people and the challenges and rewards offered to their physicians. In more than 20 years of teaching literature to medical students and physicians, practitioners of literature and medicine have clarified its conceptual frameworks and have identified the means by which its studies strengthen the human competencies of doctoring, which are a central feature of the art of medicine.

The effect of irradiation depends on the oxygenation status of the tissue, while irradiation itself also changes the oxygenation and perfusion status of tissues. A better understanding of the changes in tumor oxygenation and perfusion over time after irradiation will allow a better planning of fractionated radiotherapy in combination with modifiers of blood flow and oxygenation. Vascular architecture (endothelial marker), perfusion (Hoechst 33342) and oxygenation (pimonidazole) were studied in a human laryngeal squamous cell carcinoma tumor line grown as xenografts in nude mice. The effect of a single dose of 10 Gy X rays on these parameters was evaluated from 2 h to 11 days after irradiation. Shortly after irradiation, there was an 8% increase in perfused blood vessels (from 57% to 65%) followed by a significant decrease, with a minimum value of 42% at 26 h after irradiation, and a subsequent increase to control levels at 7 to 11 days after irradiation. The hypoxic fraction showed a decrease at 7 h after treatment from 13% to 5% with an increase to 19% at 11 days after irradiation. These experiments show that irradiation causes rapid changes in oxygenation and perfusion which may have consequences for the optimal timing of radiotherapy schedules employing multiple fractions per day and the introduction of oxygenation- and perfusion-modifying drugs.

Abstract:Since its introduction, the internet has played a major role in reshaping patient-physician communication and interactions, having fostered a shift from a paternalistic to a patient-centered model. Because of its dynamic nature, the internet has been used as a platform to not only disseminate knowledge—favored by improved access to an increasing wealth of available resources—but also to spread advocacy and awareness, contribute to fund-raising, and facilitate open, public self-disclosure of one’s own disease, thus eliminating any taboo and reducing the stigma associated with it. The era of Medicine 2.0 is characterized by openness, collaboration, participation, and social networking. The current situation is completely different from the time when Lorenzo Odone’s parents, after his diagnosis of adrenoleukodystrophy, decided to attend medical school in order to collect information about a devastating, unknown disease and had to contend with medical authorities at that establishment to convince them of the alleged effectiveness and safety of their discovered therapeutics. Orphan and rare neurological diseases have currently received recognition on web-based resources. However, while the intention is not to ridicule Odone’s family legacy and the “complicated lessons” they have reported, some issues should be carefully addressed by health authorities, such as the reputability, reliability, and accuracy of material available on the internet and prevention of the dissemination of material that could instill illusions and unjustified hopes in individuals seeking medical treatment. Neurologists should be aware of such digital resources, participate in web-based activities, and recommend select high-quality websites to their patients.

Abstract:DNA sequence database search revealed that most of Tn5053/Tn402 family transposons inserted into natural plasmids were located in putative res regions upstream of genes encoding various resolvase‐like proteins. Some of these resolvase genes belonged to Tn3 family transposons and were closely related to the tnpR genes of Tn1721 and a recently detected Tn5044. Using recombinant plasmids containing fragments of Tn1721 or Tn5044 as targets in transposition experiments, we have demonstrated that Tn5053 displays striking insertional preference for the res regions of these transposons: more than 70% of Tn5053 insertion events occur in clusters inside the target res regions, while most remaining insertion events occur no further than 200 base pairs away from both sides of the res regions. We demonstrate that Tn5053 insertions (both into and outside a res region of the target plasmid) require the presence of a functional cognate resolvase gene either in cis or in trans. To our knowledge, this is the first case when a site‐specific recombination system outside a transposon has been shown to be involved in transposition.
The voices of neurosurgeons: doctors' non-medical writing

Abstract:Biomedical publishing is an integral part of medicine--both to those who produce it and those who consume it to improve the care of their patients. Non-medical writing by surgeons usually takes the form of creative non-fiction, generally reflective essays on moving and emotionally charged situations such as working in the trenches in war-time or in natural disasters, or dealing with individual patients. Such writing is both creative and cathartic for neurosurgeons, and can help educate patients thus improving the doctor-patient relationship. The purpose of this article is to encourage fellow neurosurgeons to pursue this enjoyable and valuable endeavour, to utter a call to arms so to speak.
What Does Financial Literacy Training Teach Us?

Abstract:The authors use data from a finance-related theme park to explore how financial education changes investment, financing, and consumer behavior. Students were assigned fictitious life situations and asked to create household budgets. Some students received a 19-hour financial literacy curriculum before going to the park, and some did not. After controlling for demographic variables, the authors show that the treatment effects of the financial literacy program are strong. Students were more frugal, delayed gratification, paid off debt faster, and relied less on credit financing after training. Students who attended training showed greater uptake of decision support that was offered in the park, which indicates that decision support and financial literacy training are complements, not substitutes.

Abstract:Neuropalliative care developed to address the needs of patients living with life‐limiting neurologic disease. One critical consideration is that disease‐related changes to cognition, communication, and function challenge illness experiences and care practices. We conducted an ethnography to understand neuropalliative care as a phenomenon; how it was experienced, provided, conceptualized. Personhood served as our conceptual framework; with its long philosophical history and important place in nursing theory, we examined the extent to which it captured neuropalliative experiences and concerns. Personhood contextualized complex losses, aligning the impact of functional and relational changes. Cognition, communication, and functional alterations stretched conceptions of personhood, insinuating it can be relational, fluid, adaptive. Although normative conceptions of personhood guided research and decision‐making, ethical considerations suggested personhood could be transformed, remade. We consider the implications of our findings through three themes. First, we examine how literature on illness experience fails to integrate the realities of people living with and dying from neurologic disease; we counter this by interrogating the concept of experience. Second, we turn to Ricoeur's work on recognition to illuminate relational conceptions of personhood to inform care practices. Finally, we reflect on how personhood can bridge the gap left by functional changes, enhance relational engagement, and promote dignity at the end of life.

Abstract:The Guest Editors of this issue of Seminars in Neurology are Drs. Sarah Wahlster and Aaron Berkowitz. Dr. Sarah Wahlster is Associate Professor of Neurology, Neurosurgery (adj), and Anesthesiology (adjunct) at the University of Washington, Seattle. She is the founding fellowship director and section head for Neurocritical Care. She works as a neurointensivist and clinician educator at Harborview Medical Center, where she serves as the medical staff president (as of July 2024). Dr. Wahlster obtained her medical degree at the Ruprecht-Karls Universität Heidelberg, Germany, and completed her neurology residency in the Massachusetts General Hospital/Brigham and Women's Hospital (MGH/BWH) Partners Neurology Program, where she served as chief resident. She then completed a neurocritical care fellowship at the MGH/BWH Partners program before joining the faculty at the University of Washington. Her academic interests include mechanical ventilation in acute brain injury, long-term outcomes of patients and their families after severe acute brain injury, neurological care in resource-limited settings, and medical education. She currently chairs the Neurocritical Care Society Fellowship Director Section. She has collaborated with clinicians in Peru, Ethiopia, Bhutan, Ecuador, Nigeria, and Uganda to build educational material for resource-limited settings. She has published over 40 peer-reviewed articles, and several book chapters, including for Pocket Medicine and Pocket Primary Care. Dr. Aaron Berkowitz is Professor of Clinical Neurology at University of California San Francisco (UCSF) where he serves as a neurohospitalist, general neurologist, and clinician educator at the San Francisco Veterans Administration and San Francisco General Hospital. He previously served as director of global neurology at Brigham and Women's Hospital and Associate Professor of Neurology at Harvard Medical School, where he designed and directed the clinical neuroanatomy/neuroscience course for first year medical students. He completed medical school at Johns Hopkins and neurology residency in the MGH/BWH Partners Neurology Program, where he served as chief resident. As Health and Policy Advisor to Partners In Health, Senior Specialist Consultant to Doctors Without Borders, and prior Chair of the American Academy of Neurology (AAN) Global Health Section and prior Co-Chair of the American Neurological Association Global Health Section, he has worked tirelessly to improve access to neurologic care and education worldwide. This work has been recognized by the Mridha Humanitarian Award from the American Brain Foundation in 2018 and the Viste Patient Advocate of the Year Award from the AAN in 2019. His work as a neurology educator has been recognized by the Residency Teacher of the Year Award from the Harvard Neurology Residency program in both 2018 and 2019, the O'Hara Excellence in Preclinical Teaching award from Harvard Medical School in 2016, appointment to the editorial boards of Continuum and Practical Neurology, and frequent invitations to discuss neurology cases on the popular Clinical Problem Solvers podcast. He has published over 80 peer-reviewed articles, several book chapters including the neurology chapter for the Oxford Manual of Humanitarian Medicine , and four books including the neurology textbook Clinical Neurology and Neuroanatomy: A Localization-Based Approach and One by One by One: Making a Small Difference Amid a Billion Problems about his work caring for patients with brain tumors in rural Haiti. Abstract:The Memory Process, edited by Suzanne Nalbantian, Paul M. Matthews, and James L. McClelland, is an intriguing and well-written book that provides a groundbreaking overview of diverse approaches to understanding memory that sets the agenda for an interdisciplinary approach to the topic. Memory has long been a focus of investigation and interest in both the sciences and the humanities. The way memory enriches and distorts lived experience has been widely explored in literature and the arts. Our fascination with the subject is increasingly evident in popular culture, with the widespread proliferation of novels and movies in which events play out in a nonlinear fashion that reflects how memories of them are woven together in the minds of the characters involved. Scientific approaches to memory have focused on the study of amnesiacs, neuroimaging studies, and cognitive studies of the formation, retrieval, and forgetting of memories. Until now, however, humanistic and scientific investigations of memory have been carried out independently. This book provides an exemplary illustration of how disciplinary boundaries can be transcended, showing how approaches to memory from the humanities and the sciences can revitalize one another. With 19 chapters written by academics from a range of disciplines including neuroscience, psychology, psychiatry, cognitive science, cultural studies, philosophy, bioethics, history of science, art, theatre, film, and literature, it is undoubtedly one of the most interdisciplinary books on library shelves. Abstract:Childhood sinonasal myxomas are frequently misdiagnosed and improperly treated, with consequent adverse results for the patient.
