Abstract:Objectives Mental health disorder is highly prevalent worldwide, and factors attributable to its development have not been fully understood. We aimed to explore the association of adverse childhood experiences (ACEs) with mental health disorder using data from the China Health and Retirement Longitudinal Study (CHARLS). Methods CHARLS is an ongoing nationwide survey enrolling 17,708 residents aged ≥45 years in China. Total 20 ACE indicators were extracted. Unmeasured confounding and potential mediation were assessed, and effect-size estimates are expressed as odds ratio (OR) with 95 % confidence interval (CI). Results Of 10,961 assessable participants with complete information, 3652 were reported to have mental health disorder. The risk for mental health disorder was increased by 20.7 % for participants with one to less than four events of ACEs (OR: 1.207, 95 % CI: 0.711 to 2.049), and this number reached 102.7 % for participants with four or more events (OR: 2.027, 95 % CI: 1.196 to 3.436) relative to those with no ACE. In subgroup analyses, between-group risk estimates differed significantly for residence, marital status, personal asset, life satisfaction, and living standard. The E -value of 2.35 for ACEs of 4 or more events indicated the low likelihood of unmeasured confounders. In mediation analyses, the proportion eliminated was 5.1 % for continuous ACEs and 6.1 % for categorical ACEs (both P < 0.001). Discussion Our findings indicate that ACEs, especially with four or more events, can predict the significant risk of mental health disorder, underscoring the necessity of screening mental illness based on ACEs to reduce the resultant morbidity and mortality in China.

Abstract:Importance: Associations between adverse childhood experiences (ACEs) and chronic diseases among middle-aged or older Chinese individuals have not been well documented. In addition, whether demographic and socioeconomic characteristics modify any such associations has been underexplored.Objectives: To examine associations between ACEs and subsequent chronic diseases and to assess whether age, sex, educational level, annual per capita household expenditure level, and childhood economic hardship modify these associations.Design, Setting, and Participants: This population-based cross-sectional study used data from the China Health and Retirement Longitudinal Study (CHARLS), a survey of residents aged 45 years or older in 28 provinces across China; specifically, the study used data from the CHARLS life history survey conducted from June 1 to December 31, 2014, and a CHARLS follow-up health survey conducted from July 1 to September 30, 2015. The study population included 11 972 respondents aged 45 years or older who had data on at least 1 of 14 specified chronic diseases and information on all 12 of the ACE indicators included in this study. Data analysis was performed from December 1 to 30, 2020.Exposures: Any of 12 ACEs (physical abuse, emotional neglect, household substance abuse, household mental illness, domestic violence, incarcerated household member, parental separation or divorce, unsafe neighborhood, bullying, parental death, sibling death, and parental disability), measured by indicators on a questionnaire. The number of ACEs per participant was summed and categorized into 1 of 5 cumulative-score groups: 0, 1, 2, 3, and 4 or more.Main Outcomes and Measures: Hypertension, dyslipidemia, diabetes, heart disease, stroke, chronic lung disease, asthma, liver disease, cancer, digestive disease, kidney disease, arthritis, psychiatric disease, and memory-related disease were defined by self-reported physician diagnoses or in combination with health assessment and medication data. Multimorbidity was defined as the presence of 2 or more of these 14 chronic diseases. Logistic regression models were used to assess associations of the 12 ACEs with the 14 chronic diseases and with multimorbidity. Modification of the associations by demographic and socioeconomic characteristics was assessed by stratified analyses and tests for interaction.Results: Of the 11 972 individuals included (mean [SD] age, 59.85 [9.56] years; 6181 [51.6%] were females), 80.9% had been exposed to at least 1 ACE and 18.0% reported exposure to 4 or more ACEs. Compared with those without ACE exposure, participants who experienced 4 or more ACEs had increased risks of dyslipidemia, chronic lung disease, asthma, liver disease, digestive disease, kidney disease, arthritis, psychiatric disease, memory-related disease, and multimorbidity. The estimated odds ratios (ORs) ranged from 1.27 (95% CI, 1.02-1.59) for dyslipidemia to 2.59 (95% CI, 2.16-3.11) for digestive disease. A dose-response association was also observed between the number of ACEs and the risk of most of the chronic diseases (excluding hypertension, diabetes, and cancer) (eg, chronic lung disease for ≥4 ACEs vs none: OR, 2.01; 95% CI, 1.59-2.55; P < .001 for trend) and of multimorbidity (for individuals among the overall study population with ≥4 ACEs vs none: OR, 2.03; 95% CI, 1.70-2.41; P < .001 for trend). The demographic or socioeconomic characteristics of age, sex, educational level, annual per capita household expenditure level, or childhood economic hardship were not shown to significantly modify the associations between ACEs and multimorbidity.Conclusions and Relevance: In this population-based, cross-sectional study of adults in China, exposure to ACEs was associated with higher risks of chronic diseases regardless of demographic and socioeconomic characteristics during childhood or adulthood. These findings suggest a need to prevent ACEs and a need for a universal life-course public health strategy to reduce potential adverse health outcomes later in life among individuals who experience them.

Abstract:Adverse childhood experiences (ACEs) affect children's development, and their harm to health is pervasive throughout the life course. To identify ACEs and their risk factors in Chinese household with or without parental mental illness. A controlled study was conducted among 181 young adults with parental mental illness (positive group) and 201 demographically matched individuals without parental mental illness (negative group). Univariate and multivariate analyses were performed to study the correlation between ACEs and their risk factors. The positive group suffered emotional abuse, domestic violence, bullying, and cumulative ACEs more frequently than the negative group. In the positive group, living in rural areas and having a low household economic status during childhood were identified as risk factors for cumulative ACEs, whereas a higher education level of the mother was a protective factor for cumulative ACEs in univariate analyses. Low household economic status remained an independent risk factor for cumulative ACEs in the positive group in multivariate analyses. Children living with parental mental illness are more vulnerable to ACEs, and our findings highlight the importance of socioeconomic factors in increasing the risk of ACEs. To alleviate the deleterious impact of parental mental illness on offspring, multidimensional supports are needed.

Abstract:Previous studies have linked poor family support and adverse childhood experiences (ACEs) to increased risk of depression; however, little is known about the interplay between the two when it comes to their effects on depression. Therefore, the current study examined if family support moderated the cumulative effect of ACEs on depression. Based on data from a migrant survey in Shiyan, Hubei Province, in 2019 (N = 1,326), this study used the ordinary least squares method to analyze the effect of ACEs on depression and evaluate whether family support moderated this effect. Higher exposure to ACEs and lower scores of family support were associated with higher depression levels in adulthood. The moderation model indicated that family support significantly moderated the relationship between ACEs and depression. Appropriate interventions to reduce depression should target internal migrants with history of ACEs. Community nurses should consider ACEs as an integral part of psychosocial assessment. Negative effects of ACEs can be reduced through teaching skills that increase effective family interaction and maintain supportive family networks. Abstract:Abstract Background Adverse childhood experiences (ACEs) have been well recognized as risk factors for various adverse outcomes. However, the impacts of ACEs on psychological wellbeing among Chinese children and adolescents are unknown. Methods In total, 27 414 participants (6592 Grade 4–6 and 20 822 Grade 7–12 students) were included and information on ACEs and various psychosocial outcomes was collected. We identified subgroups with distinct psychosocial statuses using cluster analysis and logistic regression was applied to measure the associations of ACEs [individual, cumulative numbers by categories or co-occurring patterns identified by using multiple correspondence analysis (MCA)] with item- and cluster-specific psychosocial difficulties. Results Three and four cluster-based psychosocial statuses were identified for Grade 4–6 and Grade 7–12 students, respectively, indicating that psychosocial difficulties among younger students were mainly presented as changes in relationships/behaviours, whereas older students were more likely featured by deviations in multiple domains including psychiatric symptoms and suicidality. Strongest associations were found for threat-related ACEs (e.g. bullying experiences) with item- or cluster-based psychosocial difficulties (e.g. for cluster-based difficulties, the highest odds ratios = 1.72–2.08 for verbal bullying in Grade 4–6 students and 6.30–12.81 for cyberbullying in Grade 7–12 students). Analyses on cumulative numbers of ACEs and MCA-based ACE patterns revealed similar risk patterns. Additionally, exposure patterns predominated by poor external environment showed significant associations with psychosocial difficulties among Grade 7–12 students but not Grade 4–6 students. Conclusions Chinese adolescents faced different psychosocial difficulties that varied by age, all of which were associated with ACEs, particularly threat-related ACEs. Such findings prompt the development of early interventions for those key ACEs to prevent psychosocial adversities among children and adolescents.

Abstract:Research has shown that individuals with adverse childhood experiences (ACEs) are more likely to encounter heightened risks of physical and psychological challenges later in life. However, limited research has explored the comprehensive impact on the physical and mental health of young adults. Thus, we conducted a study to investigate the associations of ACEs with the physical and mental health of 18,723 college students in China. Their physical and mental health status was assessed using self-perceived health assessments, self-reported disease diagnoses, and the Generalized Anxiety Disorder-7-item scale. Results showed that 82.6 % of participants had experienced at least one ACE, with 10.7 % reporting exposure to four or more ACEs. Compared to participants with low ACEs exposure (≤ 1), participants with high ACEs exposure (≥ 4) had a nearly twofold increase in the odds of cardiometabolic diseases (OR [95 % CI] = 2.06 [1.55-2.74]) and fractures (2.32 [1.88-2.87]). Additionally, individuals with high ACE exposure exhibited a threefold increased odds of severe anxiety symptoms (3.78 [2.59-5.51]) and bipolar disorder/schizophrenia (3.38 [2.18-5.23]). These findings highlight ACEs as a significant risk factor for poor physical and mental health among young adults. Targeted support and interventions for individuals with multiple ACEs may help mitigate their long-term disease burden.

Abstract:AimThe aims of this study were to examine the prevalence of adverse childhood experiences (ACEs) among health science students in China; associations between the number of ACE exposures and severity of depressive and anxiety symptoms; and the extent to which resilience moderates the effect of ACEs on mental health outcomes. DesignThis descriptive, cross-sectional study was conducted May–August 2020. MethodsFive hundred and sixty-six health science students (18–38 years) from China completed online surveys measuring ACEs using the Simplified Chinese version of the ACE-International Questionnaire, depressive and anxiety symptoms and resilience. Descriptive statistical analysis, ANOVA with Tukey HSD post hoc tests and multiple regression analysis were performed using SPSS 27. Results88.5% of participants reported at least one ACE; 42.6% reported four or more ACEs. Higher number of ACEs was associated with more symptoms of depression and anxiety. Four or more ACEs were associated with significantly worse mental health outcomes than those with no ACEs and those with one to three ACEs. Greater resilience significantly attenuated the effects of ACEs on mental health symptoms. ConclusionsACEs are highly prevalent among Chinese health science students but their impact on mental health can be buffered by higher levels of resilience. ImpactScreening for ACEs and strength-based, trauma-informed interventions on fostering resilience is needed to promote mental health among Chinese young adults.

Abstract:Background. Adverse childhood experiences (ACEs) are recognized as key risk factors linked to poor mental health throughout life. However, research on the specific associations between ACE dimensions and depressive symptoms (DSs) among the Chinese population during mid to late life is rare. Objectives. This study aims to investigate the specific effects of different dimensions of ACEs on the new occurrence of DSs and the number of times with DSs among the middle-aged and elderly Chinese populations. Methods. The analysis included 3979 adults aged ≥45 years with four repeated measurements of the 10-item Center for Epidemiological Studies Depression Scale from the China Health and Retirement Longitudinal Study 2011–2018. Three types of ACE dimensions (total, deprivation, and threat-related ACEs) were conceptualized in accordance with the 15 types of ACEs that occurred before the age of 18 years recorded using the Life History Survey Questionnaire in 2014. In addition, latent class analysis (LCA) was utilized as an additional method for identifying distinct ACE clusters. The Cox regression and ordered logistic regression were used to estimate the risk of ACEs on DSs. Results. Among the 3979 participants, 1656 developed a new occurrence of DSs during follow-up, with 998, 438, and 220 exhibiting DSs one, two, and three times. For total and deprivation-related ACEs, only the group with ≥3 ACEs was significant with the new occurrence of DSs when compared with the no ACE group, and the adjusted hazard ratios (HRs) (95% confidence interval) were 1.562 (1.296, 1.882) and 1.446 (1.221, 1.712), respectively. With regard to threat-related ACEs, all three groups (1 ACE, 2 ACEs, and ≥3 ACEs) were significantly associated with the new occurrence of DSs. The HRs were 1.260 (1.115, 1.425), 1.407 (1.212, 1.634), and 1.585 (1.366, 1.840), respectively. The findings of total, deprivation-related, and threat-related ACEs and their associations with the number of times with DSs represent a similar phenomenon. The LCA revealed five ACE clusters. Compared to the “low risk” cluster, the “poor parent relationship” cluster and the “physical abuse” cluster were linked to an increased risk of the new occurrence of DSs and the number of times with DSs. The results of the subgroup analysis by sex and age were consistent with the total population. Conclusions. Individuals who have experienced higher ACE scores in early life face a higher risk of developing a new occurrence of DSs and multiple detected DSs in mid to late life, particularly in the case of threat-related ACEs. Parsing ACEs is imperative to explore their distinct effects on DSs and the underlying mechanisms. In addition, Incorporating ACE screening into regular health checks among the middle-aged and elderly populations is recommended. Moreover, targeted mental health interventions should be delivered to those who have experienced early life adversities, particularly threat-related ACEs, to promote healthy aging.

Abstract:Abstract Purpose With depression and anxiety becoming one of the major public health problems in China, more evidence shows the long-lasting negative effects of adverse childhood experiences (ACEs) on mental health. This study aimed to explore the ACE exposure patterns in Chinese college students and their associations with mental health problems in adulthood. Methods In this cross-sectional study, data were collected from the Chinese college students aged 18 years old and above. Depressive symptoms and anxiety were measured using the Depression Anxiety and Stress Scale (DASS-21). ACEs were measured with the International Adverse Childhood Experiences Questionnaire (ACE-IQ), and the ACE patterns were identified using latent class analysis (LCA). Then, logit regression models were employed to analyze the association between ACE exposure patterns and anxiety and depressive symptoms. Results A total of 536 Chinese college students were recruited in this study. Nearly 68.7% had at least one ACE and 17.2% were exposed to at least four types of ACEs. Three specific ACE patterns were identified, including “low ACEs” (63.1%), “family dysfunction but low childhood abuse” (20.1%), and “high domestic violence and childhood abuse” (16.8%). Compared with the “low ACEs” class, both the “high domestic violence and childhood abuse” class and the “family dysfunction but low childhood abuse” class showed significantly higher risks of depression and anxiety (P < 0.05). Conclusions The co-occurrence of ACEs was positively associated with higher risks of anxiety and depressive symptoms among Chinese college students and different patterns of ACE exposure may lead to diverse mental health outcomes.
Childhood adversity subtypes and violence victimization and perpetration among early adolescents in Shanghai, China

Abstract:Background This cross-sectional study aimed to identify adverse childhood experience (ACE) subtypes using variable- and person-centered approaches and examine the possible sex-differentiated associations with violence involvement as victim, perpetrator, and victim-perpetrator. Methods Adolescents aged 10–14 years in three junior high schools in Shanghai, China, were selected using a cluster sampling method in November and December 2017. Participants were surveyed anonymously using a computer-assisted self-interview approach via tablets. Thirteen items modified from the CDC-Kaiser ACE study were used to measure the ACEs. Results show subtypes as neglect, abuse, and household dysfunction by developing cumulative index score from the variable perspective and subgroups identified through the latent class analysis (LCA) from the person perspective. Logistic regression analyses were used to test the association between each ACE subtype and violence victimization and perpetration after adjusting for some demographic characteristics. Results A total of 1,700 participants were included in the final analysis. Approximately 1,322 (77.76 %) participants reported experiencing at least one ACE. The prevalence of neglect, abuse, and household dysfunction was 64.12 % ( n = 1090), 61.29 % ( n = 1042), and 18.24 % ( n = 310), respectively. Three classes were identified through the LCA: low exposure to all ACEs ( n = 854, 50.23 %), high exposure to emotional and physical abuse and neglect (n = 715, 42.06 %), and high exposure to all ACEs ( n = 131,7.71 %). After controlling the covariates, experiencing abuse, neglect, and household dysfunction was significantly related to violence victimization (adjusted odds ratio [aOR] = 3.19, 3.29, 2.37, P < 0.001) and victim-perpetrator (aOR = 3.48, 4.41, 5.16, P < 0.001). Adolescent violence perpetration was only found to be associated with being neglected (aOR = 2.37, P = 0.003) and suffering household dysfunction (aOR = 3.25, P < 0.001). LCA revealed the cumulative effects of ACEs on adolescent violence victimization and perpetration. Sex-stratified analysis indicate that girls were more vulnerable to the negative effects of ACEs, with a higher risk of perpetration among girls exposed to distinctive subtypes or multiple ACEs. Conclusions ACEs were ubiquitous and significantly associated with an elevated risk of violence victimization and perpetration during early adolescence. Future research should examine whether these associations persist over time and the intermediating mechanism from the perspectives of individual neurodevelopment, cognition and resilience ability, and social support.

Abstract:Background Associations between adverse childhood experiences (ACEs) and multimorbidity among middle-aged and older Chinese individuals have not been well documented. Objective We aimed to examine the associations of ACEs and different multimorbidity measures among Chinese middle-aged and older adults. Participants and setting The study population included 6428 participants aged 45 years or older who were obtained from the China Health and Retirement Longitudinal Study (CHARLS). Methods Multimorbidity was assessed by the number of self-reported physician diagnoses of 14 chronic diseases, the Chinese multimorbidity-weighted index (CMWI), multimorbidity trajectories and multimorbidity patterns. ACEs were measured retrospectively by whether the individuals having physical abuse, emotional neglect, household substance abuse, household mental illness, domestic violence, incarcerated household member, parental separation or divorce, unsafe neighborhood, bullying, parental death, sibling death, and parental disability or not, which were characterized by the binarized presence or absence of any ACE. We estimated the associations between ACEs and multimorbidity using a mixed-effects model. Latent growth curve modelling was used to investigate the trajectory of multimorbidity by ACEs. Both models Adjusted for sociodemographic and other health risk factors. Patterns of multimorbidity by ACEs were explored using hierarchical cluster analysis. Results Of the 6428 individuals included (mean [SD] age, 56.67 [8.00] years; 6181 [45.29 %] were females), 81.16 % had experienced ACEs at baseline. ACEs were associated with an increased number of chronic diseases (β = 0.30; 95 % CI, 0.21 to 0.40) and the lower CMWI (β = −0.49; 95 % CI, −0.64 to −0.33). ACEs were associated with an increased number of chronic diseases at the baseline (intercept: 0.28, 95%CI: 0.20 to 0.36) and a more rapid increase in the number of chronic diseases over 7 years (intercept: 0.03, 95%CI: 0.01 to 0.05). The results of continuous variables (the number of ACEs) were consistent with those of binary variables (ACEs). ACEs were associated with lower scores at the baseline (intercept: -0.46, 95%CI: −0.60 to −0.33) but weren't related to a faster decrease (intercept: -0.04, 95%CI: −0.07 to 0.00). The number of ACEs was associated with the lower scores of CMWI at baseline and the faster the decrease in CMWI scores (intercept: -0.17, 95%CI: −0.21 to −0.14; slope: -0.03, 95%CI: −0.04 to −0.02). The above results varied among different types of ACEs. The binary multimorbidity pattern (arthritis and stomach/digestive disease) had the highest prevalence (15.50 %) in the participants with ACEs. There were differences in multimorbidity patterns between individuals exposed to ACE or not. The liver-kidney cluster more likely clustered with the arthritis-stomach cluster in individuals without ACE, but with the cancer-psych cluster in counterparts with ACEs. Conclusions There was an association between ACEs and multimorbidity with its trajectories and patterns after age 45. Patterns of adverse childhood experiences and depressive symptom trajectories in young adults: A longitudinal study of college students in China

Abstract:Background: Adverse childhood experiences (ACEs) tend to cluster together in daily life, and most studies focus on the level of depression at certain points, but the dynamic process of depression is often neglected. Thus, research is urgently needed to explore the relationship between ACEs pattern and trajectory of depressive symptom levels at multiple time points in order to provides early targeted interventions to those who are most at risk. Objective: We aimed to explore patterns of ACEs, including types and timing, associated with depression trajectories in college students. Methods: A school-based health survey was used to collect data as part of a longitudinal study in two medical college in Anhui province, China. Questionnaires were issued to 3,662 participants aged 17–22 and recorded details of ACEs (types and timing) and depression. Latent class analysis (LCA) was used to identify "patterns" of ACEs type and timing. Depressive symptom trajectories employed latent class growth analysis (LCGA). Multiple logistic regressions were employed to evaluate the relationships between ACEs patterns and depressive symptom trajectories. Results: We identified five ACEs patterns: "High neglect/emotional abuse/community violence," "High neglect/emotional abuse," "High neglect/family dysfunction," "High neglect," "Low ACEs." We traced three depression trajectories: "High depressive symptom" "Moderate depressive symptom," "Low depressive symptom." "High neglect/emotional abuse/community violence," "High neglect/emotional abuse" and "High neglect/family dysfunction" demonstrated a high risk for "High depressive symptom" and "Moderate depressive symptom." "High neglect" showed a high risk for "Moderate depressive symptom" but not for "High depressive symptom" ( P < 0.05). Conclusions: The findings address the need for a comprehensive consideration of exposure to childhood adversity associated with the risk of depression in young adults through identifying more problematic ACEs patterns amongst exposed children.

Abstract:BackgroundDepression prevalence and adverse childhood experiences (ACEs) in middle-aged and older people has become an increasingly concerned issue, yet it in Chinese population has been less reported, and needs to be further explored.ObjectiveTo investigate the epidemiology of ACEs among middle-aged and elderly Chinese people, and to analyze its association with depression prevalence, providing an accurate delineation of ACEs and depression prevalence in this group of Chinese people.MethodsInformation of 7 035 middle-aged and elderly people≥ 45 years old who met the research requirements were selected during July to August, 2021 from the database of China Health and Retirement Longitudinal Study held by Institute of Social Science Survey, Peking University, in which the questions in the Center for Epidemiologic Studies Depression Scale used in the CHARLS, ACEs (including abuse, neglect, dysfunctional family) and individual characteristics were screened and generated into new dummy variables with the weight reassigned. Covariates of those with ACEs (treatment group) and those without (control group) were included in a propensity score matching model, and propensity scores of them were estimated using robust standard errors in multivariate Logistic regression, then the covariates of two groups were matched using the 1∶1 nearest neighbor matching, and were checked using balance diagnostics and common support assumption. The influence of ACEs on the depression in those with ACEs was quantified by the average treatment effect on the treated.ResultsOf the 7 035 respondents, 6 529 (92.81%) had experienced ACEs, 1 802 (25.61%) had experienced at least four ACEs, and 1 003 (14.26%) had a depressive state. Multivariate Logistic regression model demonstrated that age, gender, and self-rated health status were significantly associated with ACEs (P<0.05) . Individual characteristics of both groups after propensity score matching were comparable (P>0.05) , and went through the check of balance diagnostics and common support assumption. Propensity score matching-based analysis revealed that the risk of depression prevalence in those with ACEs was increased by a factor of 6.9% compared with those without (P<0.001) .ConclusionThe prevalence of ACEs among middle-aged and elderly Chinese people was higher, which may significantly increase the risk of depression prevalence. Therefore, it is urgently needed to establish a system of ACEs screening and intervening to improve the level of healthcare services and well-being for these people.

Abstract:Introduction: Children of whose mothers exposed to adverse childhood experiences (ACEs) are at increased risk for developmental problems. This study aims to investigate the relationship between types and patterns of maternal ACEs and preschool children's emotional and behavioral problems (EBPs) in China, and to explore gender differences associated with these problems. Methods: In this cross-sectional study, we selected 9,647 children from 36 preschools in three cities of Anhui province, China. Questionnaires were used to report the details of maternal ACEs and children's EBPs. We used the latent class analysis (LCA) to identify "patterns" in the types of maternal ACEs. Binomial logistic regressions was performed to examine the relationship between types and patterns of maternal ACEs and preschoolers' EBPs. Results: Latent class analysis (LCA) revealed four different classes of maternal ACEs. Logistic regression analysis showed that compared with the low ACEs class, children of mothers in the high abuse and neglect class had the highest risk of developing EBPs (OR = 5.93, 95%CI: 4.70-7.49), followed by moderate ACEs class (OR = 2.44, 95%CI: 1.98-3.00), and high household dysfunction class (OR = 2.16, 95%CI: 1.19-3.90). We found gender differences in the effects of high abuse and neglect/moderate ACEs class and maternal childhood physical abuse/neglect on children's EBPs, which had a stronger impact on EBPs in boys than girls (P<0.05). Discussion: This study supports and refines existing research that confirms an intergenerational association between types and patterns of maternal ACEs and children's EBPs in a large Chinese sample, so as to provide references for the early prevention and control of children's EBPs.

Abstract:Background: Although intergenerational transmission of ACEs among parents and their offspring have been extensively studied in the West, few studies have been conducted in China on preschool children and their parents, and explore the protective fact for the intergeneration transmission. Objective: Using latent class analysis and moderation model, this study examined the associations between patterns of adverse childhood experiences of Chinese preschool parents and behavioural problems in their children and whether coparenting quality plays a protective role in this relationship. Method: A retrospective study was conducted on 3091 parent–child dyads from 11 kindergartens in the northern, central, and southern Anhui provinces in China. Online questionnaires were used to collect the data. Parents reported adverse childhood experiences (ACEs) and provided information on their children's behavioural problems and perceived coparenting quality. Latent class analysis and a moderating model were used to examine the associations between patterns of adverse childhood experiences of Chinese preschool parents and behavioural problems in their children, and the moderating role of coparenting quality. Results: Four classes were identified: a high ACEs group, a violent victimisation group, a child abuse and physical neglect group, and a low ACEs group. Increasing levels of co-parenting quality were associated with reduced parent-reported child behaviour problems for all classes, and that potentially buffering effect was significantly stronger for the low ACEs class than for children whose parents in high ACEs class. Conclusions: Exposure to high ACEs increases the risk of developing behavioural problems in offspring, and coparenting quality may serve as a protective mechanism for intergenerational transmission. Future research should develop and implement interventions to support disadvantaged, at-risk parents and promote coparenting quality. Four classes of parental ACEs were identified: a high ACEs group, child abuse and physical neglect group, violence victimisation group, and a low ACEs group. Children with parents classified in the violence victimisation and low ACEs groups were associated with fewer behavioural problems. With higher levels of coparenting quality those in the low ACEs class reported lower offspring behavioural problems.

Abstract:AIM: Spontaneous pyogenic spinal epidural abscess (SEA) is a rare condition but might be devastating and fatal. Traditional treatment is surgical decompression and antibiotics. A retrospective study was designed to assess the eff ect of clinical findings and treatment methods on the outcome. MATERIAL and METHODS: 14 patients were reviewed (10 male, 4 female, mean age 59.14). Six dorsal, seven ventral and one dorsal with ventral SEA were observed. SEA found in thoracal (5), lumbar (4), cervical (3) regions. One patient showed both cervical and thoracal and one patient showed cervical, thoracal and lumbar involvement. All patients received minimum 3 weeks of I.V., followed by minimum 3 weeks of oral antibiotics. All patients complained of spinal pain. Ten patients presented with fever. Neurological deficit was observed in 9 cases. RESULTS: A total of 22 interventions was performed. Instrumentation was applied in 5 cases. Full recovery was achieved in 7 patients, significant improvement was observed in 5 patients. The neurological findings did not change in one patient. One mortality and one morbidity were observed. CONCLUSION: Spontaneous SEA is a rare disease but might result in catastrophic neurological deficits and fatal even with prompt treatment. Therefore, one should always keep SEA in mind if a patient presents with fever, vague and spinal pain.
