Abstract:Background Dyslipidemia in pregnancy are associated with gestational diabetes mellitus (GDM), preeclampsia, preterm birth and other adverse outcomes, which has been extensively studied in western countries. However, similar studies have rarely been conducted in Asian countries. Our study was aimed at investigating the associations between maternal dyslipidemia and adverse pregnancy outcomes among Chinese population. Methods Data were derived from 934 pairs of non-diabetic mothers and neonates between 2010 and 2011. Serum blood samples were assayed for fasting total cholesterol (TC), triglycerides (TG), high-density lipoprotein-cholesterol (HDL-C), and low-density lipoprotein-cholesterol (LDL-C) concentrations during the first, second and third trimesters. The present study explored the associations between maternal lipid profile and pregnancy complications and perinatal outcomes. The pregnancy complications included GDM, preeclampsia and intrahepatic cholestasis of pregnancy (ICP); the perinatal outcomes included preterm birth, small/large for gestational age (SGA/LGA) infants and macrosomia. Odds ratios (ORs) and 95 % confidence intervals (95 % CIs) were calculated and adjusted via stepwise logistic regression analysis. Optimal cut-off points were determined by ROC curve analysis. Results After adjustments for confounders, every unit elevation in third-trimester TG concentration was associated with increased risk for GDM (OR = 1.37, 95 % CI: 1.18-1.58), preeclampsia (OR = 1.50, 95 % CI: 1.16-1.93), ICP (OR = 1.28, 95 % CI: 1.09-1.51), LGA (OR = 1.13, 95 % CI: 1.02-1.26), macrosomia (OR = 1.19, 95 % CI: 1.02-1.39) and decreased risk for SGA (OR = 0.63, 95 % CI: 0.40-0.99); every unit increase in HDL-C concentration was associated with decreased risk for GDM and macrosomia, especially during the second trimester (GDM: OR = 0.10, 95 % CI: 0.03-0.31; macrosomia: OR = 0.25, 95 % CI: 0.09-0.73). The optimal cut-off points for third-trimester TG predicting GDM, preeclampsia, ICP, LGA and SGA were separately ≥3.871, 3.528, 3.177, 3.534 and ≤2.530 mmol/L. The optimal cut-off points for third-trimester HDL-C identifying GDM, macrosomia and SGA were respectively ≤1.712, 1.817 and ≥2.238 mmol/L. Conclusions Among Chinese population, maternal high TG in late pregnancy was independently associated with increased risk of GDM, preeclampsia, ICP, LGA, macrosomia and decreased risk of SGA. Relative low maternal HDL-C during pregnancy was significantly associated with increased risk of GDM and macrosomia; whereas relative high HDL-C was a protective factor for both of them.
Impact of multiple obesity metrics on hypertensive disorders of pregnancy: a meta-analysis and Mendelian randomisation study

Abstract:Background: The relationships between various obesity measures and hypertensive disorders of pregnancy (HDP) remain inadequately explored, and their causal links are not well understood. This study aims to clarify these associations and investigate the mediating role of triglycerides. Methods: We conducted a comprehensive meta-analysis of observational studies alongside Mendelian randomisation (MR) analysis to assess the impact of 10 obesity measures on HDP risk. Additionally, we evaluated the mediating effect of triglycerides. Results: Our meta-analysis revealed significant associations between maternal prepregnancy overweight/obesity and increased risks of gestational hypertension (GH) (overweight: OR=1.98, 95% CI 1.83 to 2.15; obesity: OR=3.77, 95% CI 3.45 to 4.13) and pre-eclampsia (overweight: OR=1.78, 95% CI 1.67 to 1.90; obesity: OR=3.46, 95% CI 3.16 to 3.79). Higher maternal waist circumference (WC) was also linked to increased pre-eclampsia risk (OR=1.45, 95% CI 1.14 to 1.83). MR analyses indicated that each 1-SD increase in genetically predicted obesity measures (whole body fat mass, body fat percentage, trunk fat mass, trunk fat percentage, body mass index, WC, hip circumference) was associated with higher risks of GH and pre-eclampsia. Triglycerides mediated 4.3%-14.1% of the total genetic effect of these obesity measures on GH and pre-eclampsia risks. Conclusions: This study demonstrates that various obesity measures are causally linked to increased HDP risk and highlights the mediating role of triglycerides. These findings could inform clinical practices and public health strategies aimed at reducing HDP through targeted obesity and triglyceride management.
Influence of maternal body mass index on pregnancy complications and outcomes: a systematic review and meta-analysis

Abstract:Background: The prevalence of obesity among women of reproductive age is increasing worldwide, with implications for serious pregnancy complications. Methods: Following PRISMA guidelines, a systematic search was conducted in both Chinese and English databases up to December 30, 2020. Pregnancy complications and outcomes including gestational diabetes mellitus (GDM), gestational hypertension (GHTN), pre-eclampsia, cesarean section (CS), induction of labor (IOL), and postpartum hemorrhage (PPH) were analyzed. Random-effects or fixed-effects models were utilized to calculate the odds ratio (OR) with 95% confidence intervals (CIs). Results: Women with overweight and obesity issues exhibited significantly higher risks of GDM (OR, 2.92, 95%CI, 2.18-2.40 and 3.46, 95%CI, 3.05-3.94, respectively) and GHTN (OR, 2.08, 95%CI, 1.72-2.53 and 3.36, 95%CI, 2.81-4.00, respectively) compared to women of normal weight. Pre-eclampsia was also significantly higher in women with overweight or obesity, with ORs of 1.70 (95%CI, 1.44-2.01) and 2.82 (95%CI, 2.66-3.00), respectively. Additionally, mothers with overweight or obesity issues had significantly higher risks of CS (OR, 1.44, 95%CI, 1.41-1.47, and 2.23, 95%CI, 2.08-2.40), IOL (OR, 1.33, 95%CI, 1.30-1.35 and 1.96, 95%CI, 1.85-2.07), and PPH (OR, 1.67, 95%CI, 1.42-1.96 and 1.88, 95%CI, 1.55-2.29). Conclusion: Women with overweight or obesity issues face increased risks of pregnancy complications and adverse outcomes, indicating dose-dependent effects.

Abstract:Background Although research indicates an association between hypertensive disorders of pregnancy (HDP) and congenital heart defects (CHDs) in offspring, consistency is still lacking. Therefore, we aimed to synthesize the updated published epidemiologic evidence to estimate the association of maternal HDP with the risk of total CHDs and its phenotypes in offspring. Methods A systematic search of Web of Science Database, PubMed, and Embase were searched from inception through April 30, 2021 based on a preprepared protocol, and the reference lists were also manually searched. The combined risk estimates were calculated using either the fixed-effect models or random-effect models. Possible heterogeneity moderators were detected by subgroup, sensitivity analyses, and Galbraith plot. Results Twenty-four studies involving 477,839 CHDs cases among 40,394,699 participants were included in our meta-analysis. Mothers who had HDP exposure were significantly associated with an increased risk of total CHDs compared with non-exposure. When maternal HDP exposure was further subdivided into pre-eclampsia ( OR = 1.79, 95% CI: 1.50–2.13), gestational hypertension ( OR = 1.16, 95% CI: 1.02–1.31), and chronic hypertension ( OR = 1.68, 95% CI: 1.49–1.89), a significantly increased risk of total CHDs were still presented. Furthermore, a statistically significant increased association was found between maternal HDP exposure and most CHD phenotypes. Besides, relevant heterogeneity moderators have been identified by subgroup and sensitivity analyses. Conclusion Our study suggested that maternal HDP exposure may be associated with an increase in the risk of CHDs in offspring. These findings highlight the need for greater surveillance of pregnant women with HDP exposure to allow early prevention that may be good for reducing the risk of CHDs in offspring. Abstract:Maternal adiposity deleteriously affects obstetrical health and has been associated with long-term adverse consequences in offspring. Here we conducted an umbrella review encompassing 194 observational meta-analyses, 10 Mendelian randomization studies and 748 interventional meta-analyses to appraise the published evidence on the associations between maternal adiposity and perinatal and offspring outcomes. Evidence grading suggested that 17 (8.8%) observational meta-analyses were supported by convincing evidence for 12 outcomes: maternal adiposity was associated with an increased risk of caesarean delivery following labour induction, infant mortality, Apgar score <7 at 1 min, antenatal depression, offspring overweight and obesity, early timing of puberty onset in daughters, attention deficit hyperactivity disorder, cerebral palsy, congenital heart disease and spina bifida (OR/RR ranging from 1.14 to 2.31), as well as increased offspring body fat percent and fat mass (SMD 0.31 and 0.35, respectively). Among these outcomes, interventional meta-analyses supported that maternal weight loss interventions significantly reduced the risk of antenatal depression but not low Apgar scores; these interventions also could not reduce offspring fat mass or body fat percent. Evidence from Mendelian randomization studies supported a causal relationship between maternal adiposity and gestational diabetes mellitus, preeclampsia, birth size and offspring adiposity. Our findings highlight that while observational meta-analyses reveal associations between maternal adiposity and various adverse perinatal and offspring outcomes, convincing, unbiased evidence or support from Mendelian randomization studies is limited. Maternal pre-conceptional and prenatal weight loss interventions can reduce some, but not all, of these adverse effects.

Excessive Gestational Weight Gain in Accordance with the IOM Criteria and the Risk of Hypertensive Disorders of Pregnancy: a Meta-Analysis.

Abstract:BACKGROUND:Excessive gestational weight gain (GWG) is a potential risk factor for hypertensive disorders of pregnancy (HDP).METHODS:We systematically reviewed three electronic databases for relevant articles published in English: PubMed, EMBASE and Web of Science. The Newcastle-Ottawa Scale was used to assess study quality. Random-effects meta-analyses were performed to supply a pooled estimation of the OR comparing the risk of HDP among healthy pregnant women with and without excessive GWG.RESULTS:The pooled estimation for the association between excessive GWG and the risk of HDPs yielded an odds ratio (OR) of 1.79 (95% CI: 1.61-1.99). A subgroup analysis showed that women who had excessive GWG were more likely to have an HDP (OR 1.82; 95% CI 1.53-2.17), preeclampsia (OR 1.92; 95% CI 1.36-2.72), or gestational hypertension (OR 1.67; 95% CI 1.43-1.95). The pooled estimation for the association between excessive GWG and the risk of HDPs among pregestational normal weight women yielded an OR of 1.57 (95% CI 1.26-1.96). A subgroup analysis showed that women who had excessive GWG were more likely to have HDP (OR 1.45; 95% CI 1.09-1.92) or gestational hypertension (OR 1.51; 95% CI 1.22-1.86). The summary ORs of pre-gestational underweight women and pre-gestational overweight and obese women were 2.17 (95% CI 1.56-3.02) and 1.32 (95% CI 1.08-1.63), respectively.CONCLUSIONS:The findings of this study suggest that excessive GWG in accordance with the IOM recommendations influences the rate of HDP.

Abstract:BackgroundThe association of gestational hypertension (GH) and preeclampsia (PE) with offspring adiposity outcomes had controversial results in different studies.ObjectiveWe conducted a systematic review and meta-analysis to evaluate the relationship between maternal GH/PE and offspring adiposity outcomes.Search strategyStudies were identified in PubMed, Embase, and Cochrane databases, with keywords including “gestational hypertension”, “preeclampsia”, “offspring”, “weight”, “cohort study”, etc., without year restriction. This study was registered with PROSPERO, CRD42022292084.Selection criteriaWe set the selection criteria for six aspects: population, outcome, time frame, study design, and availability. For the studies included in the meta-analysis, we required the potential confounders in these studies have been adjusted.Data collection and analysisTwo reviewers independently evaluated the data from the included studies. The meta-analyses included mean differences, regression coefficients, and corresponding 95% confidence intervals. Results were performed using RevMan software (version 5.4; Cochrane Collaboration). Heterogeneity among the included studies was assessed using the I2 statistic.Main resultsA total of 16 studies were included in our review, 15 of which were evaluated as high quality. In all offspring, during the early life (28 days-36 months), GH/PE exposure was found to be not or inversely associated with offspring obesity, then become positively associated at larger ages (3-19 years old). In offspring with adverse birth outcomes, the maternal GH/PE-exposed group had a lower weight in the short term (28 days to 18 months), but there was a trend of rapid weight gain as they grew older, compared with the non-exposed group. The meta-analysis showed that the BMI of the female offspring in the maternal PE-exposed group was significantly higher than that of the non-exposed offspring (MD=1.04, 95% CI: 0.67~1.42, P < 0.05).ConclusionsThe systematic review suggested that maternal exposure to de novo hypertension disorders of pregnancy (HDP) was associated with obesity in offspring, extending from early childhood to adolescence. The meta-analysis showed that PE was associated with higher BMI in female offspring. More studies are needed to conduct stratified analyses by PE/GH, the severity of HDP, or gender.Systematic review registrationPROSPERO, identifier CRD42022292084.

Abstract:Background: Placental syndromes (PS) cause over 14 million complications and the death of more than 350,000 women each year. Women with preeclampsia (PE) or gestational hypertension (GH) typically exhibit late gestational acute brachial hypertension, making early intervention difficult. Non-invasive aortic waveform assessments provide nuanced insight to global cardiovascular function. Our objective is to delineate cardiovascular differences between complicated and normotensive pregnancies (NP), to identify early vascular markers of at-risk women. Methods: A systematic search (March 2024) for studies reporting both NP and PS pregnancies was performed across five databases: PubMed; Medline; CINAHL Complete; Web of Science; and Scopus. Inclusion criteria required reporting before, during (trimester: T1; T2; T3), or after pregnancy (< 1 year post-delivery) of at least one of the following vascular biomarkers from aortic pulse pressure waveform: aortic systolic blood pressure (aSBP); aortic pulse pressure (aPP); augmentation index normalised to a heart rate of 75 bpm (AIx@75); or arterial stiffness measured by carotid femoral pulse wave velocity (cfPWV). A meta-analysis of continuous random-effects model calculated Hedges’ g with 95% confidence intervals. Results: The systematic search yielded 19 studies. Analysis depicted PS having significantly higher aggregated measures than NP throughout pregnancy (p <0.001) and a stronger effect size of AIx@75, aSBP, cfPWV in each trimester [T1, Hedges g 1.4(CI .64 - 2.0); T2, 1.1(CI .64 - 1.5); T3, 3.4(CI 2.1 – 4.6) p<0.001]. Overall, cfPWV had the highest effect size particularly during T1 and T3 [T1, 2.8(CI 2.5 – 3.1); T2, 1.7(CI 1.2 – 2.1); T3, 5.3(CI 4.6 – 5.9)]. Low aPP reporting allowed only analysis of T3 which showed similar effect sizes to other markers [3.14(CI 1.23 – 5.05)]. Conclusions: Early indication of PS was quantifiable from vascular biomarkers, along with each trimester of pregnancy being elevated compared to NP. Pulse wave analysis and arterial stiffness assessment could enhance risk assessment from as early as the first trimester, improving and informing prenatal care.

Risk factors and prediction model for new-onset hypertensive disorders of pregnancy: a retrospective cohort study

Abstract:Background and aims: Hypertensive disorders of pregnancy (HDP) is a significant cause of maternal and neonatal mortality. This study aims to identify risk factors for new-onset HDP and to develop a prediction model for assessing the risk of new-onset hypertension during pregnancy. Methods: We included 446 pregnant women without baseline hypertension from Liyang People's Hospital at the first inspection, and they were followed up until delivery. We collected maternal clinical parameters and biomarkers between 16th and 20th weeks of gestation. Logistic regression was used to determine the effect of the risk factors on HDP. For model development, a backward selection algorithm was applied to choose pertinent biomarkers, and predictive models were created based on multiple machine learning methods (generalised linear model, multivariate adaptive regression splines, random forest, and k-nearest neighbours). Model performance was evaluated using the area under the curve. Results: Out of the 446 participants, 153 developed new-onset HDP. The HDP group exhibited significantly higher baseline body mass index (BMI), weight change, baseline systolic/diastolic blood pressure, and platelet counts than the control group. The increase in baseline BMI, weight change, and baseline systolic and diastolic blood pressure significantly elevated the risk of HDP, with odds ratios and 95% confidence intervals of 1.10 (1.03–1.17), 1.10 (1.05–1.16), 1.04 (1.01–1.08), and 1.10 (1.05–1.14) respectively. Restricted cubic spline showed a linear dose-dependent association of baseline BMI and weight change with the risk of HDP. The random forest-based prediction model showed robust performance with the area under the curve of 0.85 in the training set. Conclusion: This study establishes a prediction model to evaluate the risk of new-onset HDP, which might facilitate the early diagnosis and management of HDP.

Joint Associations of Maternal Gestational Diabetes and Hypertensive Disorders of Pregnancy with Overweight in Offspring

Abstract:Objectives: Either maternal gestational diabetes mellitus (GDM) or hypertensive disorder of pregnancy (HDP) is associated with an increased risk of obesity in the offspring. However, their joint associations with obesity in offspring remain unclear. We investigated the joint associations of maternal GDM and HDP with childhood overweight in offspring. Methods: We performed a large study in 1967 mother-child pairs. Maternal GDM was diagnosed according to the 1999 World Health Organization (WHO) criteria. HDP was defined as self-reported doctor-diagnosed hypertension or treatment of hypertension (including gestational hypertension, preeclampsia, sever preeclampsia or eclampsia) after 20 weeks of gestation on the questionnaire. Body mass index (BMI) for age Z-score and childhood overweight were evaluated according to WHO growth reference. We used the general linear models to compare children's Z score for BMI and logistic regression models to estimate odds ratios of childhood overweight according to maternal different status of GDM and HDP. Results: Offspring of mothers with both GDM and HDP had a higher BMI for age Z-score (0.63 vs. 0.03, P < 0.001) than children born to normotensive and normoglycemic pregnancy. After adjustment for maternal and children's major confounding factors, joint GDM and HDP were associated with increased odds ratios of offspring's overweight compared with normotensive and normoglycemic pregnancy (2.97, 95% confidence intervals [CIs] 1.65-5.34) and GDM alone (2.06, 95% CIs 1.20-3.54), respectively. After additional adjustment for maternal pre-pregnancy BMI and gestational weight gain, joint maternal GDM, and HDP was still associated with an increased risk of offspring's overweight compared with the maternal normotensive, and normoglycemic group but became to have a borderline increased risk compared with the maternal GDM alone group. Conclusions: Maternal GDM alone or joint GDM and HDP were associated with increased ratios of offspring's overweight.

Abstract:OBJECTIVE:Differences in body fat distribution contribute to the metabolic abnormalities associated with overweight and obesity; however, such differences have not been adequately explored during pregnancy. Our aim was to compare longitudinal trends in maternal abdominal adipose tissue deposition during pregnancy in overweight/obese compared with normal weight women.STUDY DESIGN:Pregnant women, classified as normal weight (body mass index (BMI) <25 kg m−2; N=61) or overweight/obese (BMI ⩾25 kg m−2; N=57), were enrolled in a prospective cohort study starting in the first trimester. Maternal subcutaneous (smin) and preperitoneal (pmax) fat were measured by ultrasound at five time points starting between 6 and 10 weeks gestation. The abdominal fat index (AFI), an established marker of visceral adipose tissue, was calculated as the ratio of pmax to smin. The trajectories of smin, pmax, cumulative fat index (smin plus pmax) and the AFI across pregnancy were analyzed using mixed linear models.RESULTS:The rate of maternal weight gain during pregnancy was significantly lower for overweight/obese women compared with their non-overweight counterparts (P<0.05). Accordingly, the rate of change of pmax and smin differed significantly in normal weight compared with overweight/obese women (P=0.0003 and 0.01, respectively). The cumulative fat index did not change across gestation in normal weight women, whereas it decreased for overweight/obese women (P=0.0005). The log AFI increased across pregnancy in both strata, but significantly more rapidly for normal weight compared with overweight/obese women (P=0.06).CONCLUSIONS:Adipose tissue is preferentially deposited in the more metabolically active visceral compartment as pregnancy progresses. However, this process differs in normal weight compared with overweight/obese women and may contribute to metabolic differences between these groups. Our study is a step toward a more refined description of obesity and its consequences during pregnancy.

The Maternal-Fetal Risk Factors of Hypertensive Disorders of Pregnancy and its Effects on Infant Complete Blood Count and Coagulation Factors

Abstract:Objective: Our aim was to analyze the risk factors of hypertensive disorders of pregnancy (HDP) and explore its influence on fetal risk factors, infant's blood cells and markers of inflammation. Methods: A total of 123 patients with HDP were in the HDP group, and 121 healthy pregnant women were selected as the control group. The general clinical data of the participants were recorded. Statistics of maternal and infant outcomes, delivery methods, routine blood lab results and coagulation factors of the newborn were recorded. Univariate analysis and multi-factor analysis were used to explore the risk factors for HDP. Results: The overall incidence of poor maternal outcomes in the HDP group was higher than in the control group. The incidence of premature delivery; postpartum hemorrhage; coagulopathy; placental abruption; heart failure and hemolysis, elevated liver enzymes, and low platelets (HELLP) syndrome was significantly higher in the HDP group than in the control group (P < .05). The cesarean section rate in the HDP group was significantly higher than in the control group (P < .05). The overall incidence of poor outcomes in fetuses and newborns in the HDP group was higher than in the control group. The incidence of infant low birth weight, intraventricular hemorrhage (IVH), neonatal respiratory distress syndrome (NRDS), asphyxia and all-cause neonatal death were higher than in the control group (P < .05). The incidence of small gestational age (SGA), fetal distress and intrauterine death in the HDP group were higher than in the control group (P < .05). In the HDP group, neonatal white blood cells (WBC), neutrophils (NEUT) and platelets (PLT) were significantly lower than in the control group (P < .05), while hemoglobin (Hgb) and hematocrit (Hct) were higher (P < .05). Conclusions: HDP endangers the health of mother and infant; Age, body mass index (BMI) (>24 kg/m2), parity, history of hypertension, family history of hypertension and other factors may be involved in the occurrence and development of HDP.
Meta-analysis of the Relationship between Maternal Body Weight Gain and Poor Pregnancy Outcomes

Abstract:Objective To evaluate the relationship between maternal body weight gain and poor pregnancy outcomes and provide evidence for intervention.Methods The results of 25 domestic cohort studies were analyzed by meta-analysis method in this study.This analysis was performed on Review Manager 4.2 software.Results The maternal body weight gain≥15kg could increase the risk of outcomes as follows:pregnancy induced hypertension(RR=1.18,1.42～2.48),cesarean section(RR=2.12,1.65～2.72),gestational diabetes(RR=3.34,2.20～5.09),operative vaginal delivery(RR=1.63,1.12～2.37),postpartum hemorrhage(RR=3.96,2.56～6.13),abnormal labor(RR=3.05,2.19～4.24),macrosomia(RR=2.73,1.97～3.78),neonatal asphyxia(RR=1.70,1.10～2.60).The maternal body weight gain≥15kg could decrease the risk of low birth weight(RR=0.38,0.18～0.81).Besides,the maternal body weight gain≥15kg could increase the mean labor duration by 5.29(4.81～5.77)hours,and the mean neonatal weight by 378.65(297.80～459.51)g.The maternal inadequate body weight gain could decrease the risk of pregnancy induced hypertension(RR=0.27,0.16～0.48) and macrosomia(RR=0.38,0.36～0.41),but could increase the risk of low birth weight(RR=2.50,2.32～2.69).Conclusion Either maternal excessive body weight gain or maternal inadequate body weight gain will do harm to mother and newborn,so we should control maternal body weight gain strictly,the most appropriate weight gain would be 10～15kg.
Increasing maternal percentage body fat in early second trimester: a risk factor for preeclampsia

Abstract:OBJECTIVE: To determine if maternal percentage body fat (PBF) or fat free mass (FFM) in the early second trimester of pregnancy influenced the development of preeclampsia.METHODS: A matched nested case-control study was conducted from a cohort study of 1668 women at Gansu provincial maternal and child care hospital from July 2007 to August 2011 in China. Maternal PBF and FFM were assessed by bioelectrical impedance analysis during 12th-16th gestational week. The demographic characteristics were all chart abstracted. After childbirth, 70 cases of preeclampsia were matched by race/age with 140 uncomplicated pregnancies women. Multivariate logistic regression analysis was performed to determine the associated risk factors.RESULTS: Pre-pregnancy body mass index were higher in women who subsequently developed preeclampsia compared with controls (p < 0.001). During 12th-16th gestational week, there were nearly 7-fold increase in the odds of preeclampsia (adjusted OR: 6.84, 95% CI: 4.15-41.60) among women with PBF ≥ 40% versus women with PBF < 40%. But FFM were not at further increased risk of the development of preeclampsia (adjusted OR, 1.02; 95% CI, 0.6-3.6).CONCLUSION: Maternal PBF but not FFM is a predictor of preeclampsia in the early second trimester. Excessive adipose tissue possibly played an important role in developing of preeclampsia.

Ultrasonographic abdominal adipose tissue thickness for the prediction of gestational diabetes mellitus: A meta-analysis

Abstract:Obesity has been linked to the risk of gestational diabetes mellitus (GDM). The meta-analysis aimed to assess the predictive role of ultrasonographic measurements of the abdominal adipose tissue thickness for GDM in pregnant women. Cohort studies evaluating the association between abdominal subcutaneous and/or visceral adipose thickness (SAT and/or VAT) and subsequent risk of GDM were retrieved from PubMed, Embase, and Web of Science databases. Only studies with SAT/VAT measured before the diagnosis of GDM were included. Random-effects models incorporating the influence of potential heterogeneity were used to pool the results. A total of 13 studies involving 5616 pregnant women were included. Pooled results showed that both a high abdominal SAT (odds ratio [OR] for per 1-cm increment: 1.23, 95% confidence interval [CI]: 1.07 to 1.41, P = 0.003, I2 = 13%; OR for high versus low category: 3.42, 95% CI: 2.31 to 5.07, P < 0.001, I2 = 0%) and VAT (OR for per 1-cm increment: 1.54, 95% CI: 1.16 to 2.06, P = 0.003, I2 = 63%; OR for high versus low category: 5.73, 95% CI: 3.39 to 9.77, P < 0.001, I2 = 31%) at early stages of pregnancy were associated with a higher subsequent risk of GDM. Subgroup analysis based on study design, timing of ultrasound examination, GDM diagnostic criteria, and study quality score showed consistent results. In conclusion, ultrasound-measured abdominal adipose tissue thickness may be useful for predicting the subsequent risk of GDM in pregnant women.

Abstract:OBJECTIVE: We aim to explore the mediating effect of hypertensive disorders of pregnancy (HDP) on the relationship between pre-pregnancy body mass index (BMI) and the risk of preterm birth (PTB) in women with singleton live births.METHODS: Demographic and clinical data of 3,249,159 women with singleton live births were extracted from the National Vital Statistics System (NVSS) database in this retrospective cohort study. The associations between pre-pregnancy BMI and HDP, HDP, and PTB, and pre-pregnancy BMI and PTB were evaluated using univariate and multivariate logistic regression analyses, with odds ratios (ORs) and 95% confidence intervals (CIs). Structural equation modeling (SEM) was used to explore the mediating effect of HDP on the relationship between pre-pregnancy BMI and PTB.RESULTS: In total, 324,627 women (9.99%) had PTB. After adjustment for covariables, there were significant associations between pre-pregnancy BMI and HDP [OR = 2.07, 95% CI: 2.05-2.09)], HDP and PTB [OR = 2.54, 95% CI: (2.52-2.57)], and pre-pregnancy BMI and PTB [OR = 1.03, 95% CI: 1.02-1.03)]. The effect of pre-pregnancy BMI on PTB was significantly mediated by HDP with a mediation proportion of 63.62%, especially in women of different ages and having gestational diabetes mellitus (GDM) or not.CONCLUSION: HDP may play a mediating role in the effect of pre-pregnancy BMI on PTB risk. Women preparing for pregnancy should pay close attention to BMI, and pregnant women should monitor and develop interventions for HDP to reduce the risk of PTB.

Embracing Semantic Technology for Better Metadata Authoring in Biomedicine

Abstract:The Center for Expanded Data Annotation and Retrieval (CEDAR) has developed a suite of tools and services that allow scientists to create and publish metadata describing scientific experiments. Using these tools and services—referred to collectively as the CEDAR Workbench—scientists can collaboratively author metadata and submit them to public repositories. A key focus of our software is semantically enriching metadata with ontology terms. The system combines emerging technologies, such as JSON-LD and graph databases, with modern software development technologies, such as microservices and container platforms. The result is a suite of user-friendly, Web-based tools and REST APIs that provide a versatile end-to-end solution to the problems of metadata authoring and management. This paper presents the architecture of the CEDAR Workbench and focuses on the technology choices made to construct an easily usable, open system that allows users to create and publish semantically enriched metadata in standard Web formats.
Analysis of the correlation between metabolic risk factors in early pregnancy and neonatal macrosomia

Abstract:Objective The prevalence of obesity and related cardiometabolic comorbidities increased dramatically in Chinese populations at all ages. This study aimed at investigating the prevalence of metabolic syndrome (MS) and components among pregnant women in early gestation and evaluating the associations with macrosomia.Methods A population-based routine data from Kunshan Maternal and Child Health Care Hospital from 2009 to 2010 were retrospectively analyzed. The prenatal informations were analyzed among pregnant women who received the first prenatal examination less than 20 gestation weeks, including maternal anthropometrics and blood pressure at the first prenatal visit, fasting plasma glucose and serum lipids examined in the first trimester. Birth weight and gestational age were obtained from neonates' medical records in the hospital. MS was defined according to the International Diabetes Federation (IDF) criteria 2005. Owing to waist circumferences during pregnancy was imprecise, body mass index (BMI) ≥24 kg·m-2 in first prenatal visit was used to define overweight.Results Statistical analyses were performed among 1 405 subjects with complete prenatal data. The mean of maternal age and BMI at the first prenatal visit was (26.5±3.2) years and (20.8±2.7) kg·m-2, respectively. The mean of triglyceride, HDL-C and fasting glucose was (2.4±1.0), (2.0±0.4) and (4.5±0.5) mmol·L-1, respectively. The mean of SBP and DBP was (107.7±10.0) and (69.3±7.0) mmHg. The overall prevalence of MS, overweight, elevated triglyceride, reduced HDL-C, raised blood pressure and raised glucose among pregnant women in early gestation was 1.8%, 12.0%, 76.2%, 2.8%, 2.6% and 2.3%, respectively. The mean of the birth weight and gestational age was (3 365.0±418.6) g and (39.0±1.4) weeks with the incidence of macrosomia being 7.0%. Stepwise multiple logistic regressions for 497 subjects with the first prenatal visit within 20 gestational weeks and complete prenatal and neonatal birth weight showed in early gestation maternal overweight/obesity (OR=2.4, 95%CI: 1.0~5.5) significantly increased the risk of macrosomia. There was no significant difference in the proportion of maternal overweight in early gestation between analyzed subjects and those missing birth weight data by using Person's chi-square tests.Conclusions About 1.8% of Chinese pregnant women met the criteria of MS in early gestation. The commonest components of MS including raised triglyceride (76.2%) and overweight (12.0%), were the most important risk factors for macrosomia. The findings call for the urgent need for early and throughout management of metabolic risks among pregnant women in order to control the adverse gestation outcomes.

Abstract:Objective This study aimed to examine the associations of maternal obesity in early pregnancy with adverse pregnancy outcomes. Methods A prospective cohort analysis was performed among 18,481 Chinese nulliparous women, using data from a 2006 to 2009 trial of prenatal micronutrient supplementation. Obesity was defined as BMI >= 27.5 kg/m(2). Interested outcomes included fetal loss (spontaneous abortion plus stillbirth), infant death, total mortality, and preterm and birth weight outcomes. Results Compared with normal weight, obesity was associated with total mortality (adjusted relative risks [ARR] 1.34; 95% CI: 1.03-1.74) and fetal loss (ARR 1.51; 95% CI: 1.15-1.99) but not with infant death (ARR 0.53; 95% CI: 0.20-1.46). Further analyses showed that obesity was particularly associated with spontaneous abortion (ARR 1.51; 95% CI: 1.13-2.02) rather than stillbirth (ARR 1.52; 95% CI: 0.65-3.57). Moreover, obesity was associated with preterm birth (ARR 1.59; 95% CI: 1.25-2.02), macrosomia (ARR 3.71, 95% CI: 3.01-4.59), and large for gestational age (ARR 2.93; 95% CI: 2.49-3.47). Conclusions Maternal obesity in early pregnancy is associated with various adverse pregnancy outcomes in Chinese nulliparous women, suggesting the importance of an appropriate weight before and during pregnancy.

Abstract:We aimed to examine the effects of various severities of hypertensive disorders in pregnancy on birthweight, blood pressure (BP), and body mass index in offspring at age 7. In the China Labor and Delivery Survey and the United States Collaborative Perinatal Project (CPP), the relationship of the severity of hypertensive disorders and nutritional and cardiovascular outcomes in offspring was assessed using a multivariable logistic and general linear regression model. In both datasets, those with gestational hypertension were more likely to deliver large for gestational age (LGA) and macrosomia (adjusted odds ratios (aOR) ranged from 1.29 to 1.91), as well as low birth weight (LBW) neonates (aOR ranged from 1.23 to 3.56), compared with normotensive mothers. In the CPP, when gestational hypertension was further stratified into mild and severe, only those with mild gestational hypertension (the mild group) were more likely to deliver macrosomia and LGA (aOR ranged from 1.25 to 1.32). Others (severe gestational hypertension and preeclampsia/eclampsia) were closely related to LBW and small for gestational age (aOR ranged from 1.27 to 2.77). Moreover, children of mothers in the mild group tended to be overweight/obese and had elevated diastolic BP. We concluded that the severity of hypertensive disorders had different effects on birthweight, childhood overweight, and BP.
