Abstract:Abstract Background Autism Spectrum Disorder (ASD) is a neurodevelopmental disorder marked by characteristic impairments in social communication and interaction, as well as restricted and repetitive behavior. There is a continued need for exploring effective interventions and evaluating treatment options for ASD. Transcranial magnetic stimulation (rTMS) and transcranial direct current stimulation (tDCS) are non-invasive neurostimulation techniques which have shown clinical benefits in adult psychiatric conditions. While in ASD patients, no guideline has so-far recommended on the application of NIBS due to a lack of high-quality synthetic evidence. Therefore, objective of this study will be to systematically evaluate the evidence on clinical effects of non-invasive neurostimulation in patients with ASD. Methods We designed and registered a study protocol for a systematic review. A comprehensive search from database inception onwards will be conducted in PubMed, EMBASE and Cochrane library. Randomized and non-randomized sham-controlled studies assessing the effects of noninvasive neurostimulation in patients with ASD will be identified. Eligibility of citations retrieved will be independently screened by two reviewers. The risk of bias and quality of included studies will be appraised using appropriate tools. A narrative synthesis around the features of the evidences will provided. There may be a chance for meta-analysis to pool the estimates of studies included if three studies or more meet the requirements for meta-analysis. If so, a random-effects model maybe applied. And we will evaluate heterogeneity between studies using the I2 statistic. Discussion This systematic review will provide a broad and comprehensive evaluation of the evidence on clinical effects of TMS and tDCS in patients with ASD. Our findings will be reported according to the PRISMA guidelines and may add more confidence when healthcare professionals are making informed decisions about the choice of this therapy. The effect of music therapy on language communication and social skills in children with autism spectrum disorder: a systematic review and meta-analysis

Abstract:Background: Studies have shown that music therapy can be used as a therapeutic aid for clinical disorders. To evaluate the effects of music therapy (MT) on language communication and social skills in children with autism spectrum disorder (ASD), a meta-analysis was performed on eligible studies in this field. Methods: A systematic search was conducted in eight databases: PubMed, Embase, Web of Science, Cochrane Library databases, the China National Knowledge Infrastructure (CNKI), Wanfang Data, the Chinese Biomedical Literature (CBM) Database, and the VIP Chinese Science and Technology Periodicals Database. The standard mean difference (SMD) values were used to evaluate outcomes, and the pooled proportions and SMD with their 95% confidence intervals (CIs) were also calculated. Results: Eighteen randomized controlled trial (RCT) studies were included, with a total of 1,457 children with ASD. This meta-analysis revealed that music therapy improved their language communication [SMD = −1.20; 95%CI –1.45, −0.94; χ 2 (17) = 84.17, I 2 = 80%, p < 0.001] and social skills [SMD = −1. 13; 95%CI –1.49, −0.78; χ 2 (17) = 162.53, I 2 = 90%, p < 0.001]. In addition, behavior [SMD = −1.92; 94%CI –2.56, −1.28; χ 2 (13) = 235.08, I 2 = 95%, p < 0.001], sensory perception [SMD = −1.62; 95%CI –2.17, −1.08; χ 2 (16) = 303.80, I 2 = 95%, p < 0.001], self-help [SMD = −2. 14; 95%CI –3.17, −1.10; χ 2 (6) = 173.07, I 2 = 97%, p < 0.001] were all improved. The clinical effects of Orff music therapy on children with autism spectrum disorder: a comprehensive evaluation

Abstract:Objective: This study aimed to investigate the clinical effects of Orff music therapy on children with Autism Spectrum Disorder (ASD) from the perspectives of parents, evaluators, and therapists. Methods: 93 children with ASD aged 3–6 years participated in the study. They were divided into an observation group ( n = 48) receiving comprehensive rehabilitation intervention including Orff music therapy, and a control group ( n = 45) receiving only comprehensive rehabilitation intervention. The Autism Behavior Checklist (ABC), Childhood Autism Rating Scale (CARS), and Psycho-educational Profile-3rd edition (PEP-3) were used for assessments before and after the intervention. Results: There were no significant demographic differences between the two groups. Both groups showed significant improvements in Sensory, Relating, Language, CVP, EL, RL, VMI, AE, SR, and CARS scores at T1, T2, and T3 (T1 vs. T2, T2 vs. T3, T1 vs. T3) (all p < 0.05). The observation group demonstrated significant changes in Body and Object use and FM, while the control group showed some changes in these domains. Social and self-help, GM, CMB, and CVB also significantly improved in both groups after 6 months of intervention (all p < 0.05). In terms of different time intervals, the observation group showed greater improvements in Sensory, Relating, Language, CARS scores, EL, RL, and SR compared to the control group (all p < 0.05). The improvement levels in Body and Object use, CVP, FM, VMI, and AE did not differ significantly between the two groups in the T1–T2 interval, but were significantly higher in the observation group in the T2–T3 and T1–T3 intervals (all p < 0.05). The magnitude of changes in Social and self-help, GM, CMB, and CVB did not differ significantly between the groups. Conclusion: Orff music therapy showed significant improvements in language expression, language comprehension, social skills, cognitive abilities, imitation abilities, emotional expression and fine motor in children with ASD. These findings provide support for the use of Orff music therapy as an effective intervention for children with ASD.

Abstract:The notion of a connection between autism and music is as old as the first reported cases of autism, and music has been used as a therapeutic tool for many decades. Music therapy holds promise as an intervention for individuals with autism, harnessing their strengths in music processing to enhance communication and expression. While previous randomized controlled trials have demonstrated positive outcomes in terms of global improvement and quality of life, their reliance on psychological outcomes restricts our understanding of underlying mechanisms. This paper introduces the protocol for the Music for Autism study, a randomized crossover trial designed to investigate the effects of a 12-week music therapy intervention on a range of psychometric, neuroimaging, and biological outcomes in school-aged children with autism. The protocol builds upon previous research and aims to both replicate and expand upon findings that demonstrated improvements in social communication and functional brain connectivity following a music intervention. The primary objective of this trial is to determine whether music therapy leads to improvements in social communication and functional brain connectivity as compared to play-based therapy. In addition, secondary aims include exploring various relevant psychometric, neuroimaging, and biological outcomes. To achieve these objectives, we will enroll 80 participants aged 6-12 years in this international, assessor-blinded, crossover randomized controlled trial. Each participant will be randomly assigned to receive either music therapy or play-based therapy for a period of 12 weeks, followed by a 12-week washout period, after which they will receive the alternate intervention. Assessments will be conducted four times, before and after each intervention period. The protocol of the Music for Autism trial provides a comprehensive framework for studying the effects of music therapy on a range of multidimensional outcomes in children with autism. The findings from this trial have the potential to contribute to the development of evidence-based interventions that leverage strengths in music processing to address the complex challenges faced by individuals with autism. Music therapy improves engagement and initiation for autistic children with mild intellectual disabilities: A randomized controlled study

Abstract:This study aimed to evaluate the effectiveness of music therapy (MT) for children with co‐occurring autism spectrum disorder (ASD) and intellectual disabilities (ID) and explore whether pre‐intervention quantitative electroencephalogram (qEEG) responses can predict outcomes. A randomized controlled trial was conducted with 33 children receiving MT and 34 receiving an active control therapy. Participants received either MT or a non‐musical social skills intervention for 45 min weekly over 12 weeks. Primary outcomes were measured using the Childhood Autism Rating Scale (CARS‐2), along with the parent‐rated Social Responsiveness Scale (SRS‐2) and video coding of social behaviors. Both conditions significantly improved in CARS‐2 scores at 2 weeks and 4 months post‐intervention, with no differences between MT and control conditions. No changes were found in SRS‐2 scores. While both conditions showed reduced disengagement after intervention, only the MT condition showed increased engagement and initiation. Strong qEEG responses to social scenes and music predicted increased initiation, indicating its potential to help tailor interventions. These results support incorporating MT into standard services and further research on qEEG predictors.

Abstract:Background: Autism spectrum disorder (ASD) is a developmental impairment characterized by persistent deficits in social communication and interactions, and over half of children with ASD possess below average intellectual ability (IQ < 85). The social development and response to social skill interventions among children with ASD and comorbid intellectual disability (ID) is not well understood. Music therapy is a systematic process of intervention, wherein a therapist may help clients promote their social skills by using musical experience. The proposed study will address limited research evidence on music therapy as an intervention for social functioning in children with ASD with mild to borderline ID. Method: A randomized controlled trial (RCT) with two parallel groups of 40 children each (1:1 allocation ratio) is planned. Participants will receive 45 min of music therapy or non-musical intervention targeting social skills once a week for 12 weeks. Primary outcome measures will be independent ratings on the Childhood Autism Rating Scale and parent ratings on the Social Responsiveness Scale-2. Linear mixed-effects models for these two outcome measures will be created for data collected at 2-week pre-intervention, 2-week post-intervention, and 4-month post-intervention sessions. In-session behaviors at the first and last intervention will be videotaped and coded offline and compared. Pretreatment neural response of quantitative electroencephalograms (qEEG) to social scenes will be used to predict the outcomes of musical and non-musical social skill interventions, whereas qEEG responses to music will be used to predict the effectiveness of musical social skill intervention. Discussion: If neural markers of social skill development are found, then the long-term goal is to develop individualized intervention based on pre-treatment markers to maximize treatment efficacy. The proposed study's results may also suggest directions to development and provision of music therapy services in Hong Kong. Trial registration: ClinicalTrials.gov ( NCT04557488 ). Effects of Auditory Integration Training Combined with Music Therapy on Children with Autism Spectrum Disorder

Abstract:Objective: This study explored the effects of Auditory Integration Training combined with Music Therapy on Autistic Children. Methods: Eighty Autistic Children who received treatment at a Rehabilitation Center for Children with Autism in Guangzhou from January to June 2023 were selected as participants. They were randomly allocated to an Experimental or a Control group, giving 40 in each condition. The Control group received Auditory Integration Training, while the Experimental group received Auditory Integration Training combined with Music Therapy. Sociability, Language, Behavior, and Neuron Development of the two groups of children were compared before and six months after treatment. Results: By comparing the Sociability, Language, Behavior, and Neuron Development scores of two groups of children before and after six months of treatment, the combination of these two treatments has positively impacted the rehabilitation of children with autism. Conclusion: The combination of Auditory Integration Training and Music Therapy has shown promising results in its application for Children with Autism Spectrum Disorder (ASD).

Abstract:Importance: Music therapy may facilitate skills in areas affected by autism spectrum disorder (ASD), such as social interaction and communication. Objective: To evaluate effects of improvisational music therapy on generalized social communication skills of children with ASD. Design, setting, and participants: Assessor-blinded, randomized clinical trial, conducted in 9 countries and enrolling children aged 4 to 7 years with ASD. Children were recruited from November 2011 to November 2015, with follow-up between January 2012 and November 2016. Interventions: Enhanced standard care (n = 182) vs enhanced standard care plus improvisational music therapy (n = 182), allocated in a 1:1 ratio. Enhanced standard care consisted of usual care as locally available plus parent counseling to discuss parents' concerns and provide information about ASD. In improvisational music therapy, trained music therapists sang or played music with each child, attuned and adapted to the child's focus of attention, to help children develop affect sharing and joint attention. Main outcomes and measures: The primary outcome was symptom severity over 5 months, based on the Autism Diagnostic Observation Schedule (ADOS), social affect domain (range, 0-27; higher scores indicate greater severity; minimal clinically important difference, 1). Prespecified secondary outcomes included parent-rated social responsiveness. All outcomes were also assessed at 2 and 12 months. Results: Among 364 participants randomized (mean age, 5.4 years; 83% boys), 314 (86%) completed the primary end point and 290 (80%) completed the last end point. Over 5 months, participants assigned to music therapy received a median of 19 music therapy, 3 parent counseling, and 36 other therapy sessions, compared with 3 parent counseling and 45 other therapy sessions for those assigned to enhanced standard care. From baseline to 5 months, mean ADOS social affect scores estimated by linear mixed-effects models decreased from 14.08 to 13.23 in the music therapy group and from 13.49 to 12.58 in the standard care group (mean difference, 0.06 [95% CI, -0.70 to 0.81]; P = .88), with no significant difference in improvement. Of 20 exploratory secondary outcomes, 17 showed no significant difference. Conclusions and relevance: Among children with autism spectrum disorder, improvisational music therapy, compared with enhanced standard care, resulted in no significant difference in symptom severity based on the ADOS social affect domain over 5 months. These findings do not support the use of improvisational music therapy for symptom reduction in children with autism spectrum disorder. Trial registration: isrctn.org Identifier: ISRCTN78923965.
The influence of music technology on the academic behavior of preschool children with autism spectrum disorder

Abstract:This study explored the effects of combining two music technology teaching approaches, holistic music educational approach for young children (HMEAYC) and multisensory musical activities (MSMAs), on children’s academic behavior performances. The venue was a nonprofit early intervention organization and private preschool. Purposive sampling was used to select six children with autism spectrum disorder (ASD) and six without ASD around the age of 40-63 months, and the course arrangement was a 40-minute class each week for 14 weeks. This study demonstrated that the MSMAs teaching method could improve children’s learning habits and performance without ASD. In addition, the intervention of HMEAYC teaching method can improve the academic behavior of both children with ASD, and without ASD, especially for children with ASD; it has more significant benefits on learning habits. In addition, through HMEAYC’s music therapy courses, children’s communication, emotional and social performance can also be improved.
International multicentre randomised controlled trial of improvisational music therapy for children with autism spectrum disorder: TIME-A study

Abstract:Background: Preliminary studies have indicated that music therapy may benefit children with autism spectrum disorders (ASD). Objectives: To examine the effects of improvisational music therapy (IMT) on social affect and responsiveness of children with ASD. Design: International, multicentre, three-arm, single-masked randomised controlled trial, including a National Institute for Health Research (NIHR)-funded centre that recruited in London and the east of England. Randomisation was via a remote service using permuted blocks, stratified by study site. Setting: Schools and private, voluntary and state-funded health-care services. Participants: Children aged between 4 and 7 years with a confirmed diagnosis of ASD and a parent or guardian who provided written informed consent. We excluded children with serious sensory disorder and those who had received music therapy within the past 12 months. Interventions: All parents and children received enhanced standard care (ESC), which involved three 60-minute sessions of advice and support in addition to treatment as usual. In addition, they were randomised to either one (low-frequency) or three (high-frequency) sessions of IMT per week, or to ESC alone, over 5 months in a ratio of 1 : 1 : 2. Main outcome measures: The primary outcome was measured using the social affect score derived from the Autism Diagnostic Observation Schedule (ADOS) at 5 months: higher scores indicated greater impairment. Secondary outcomes included social affect at 12 months and parent-rated social responsiveness at 5 and 12 months (higher scores indicated greater impairment). Results: A total of 364 participants were randomised between 2011 and 2015. A total of 182 children were allocated to IMT (90 to high-frequency sessions and 92 to low-frequency sessions), and 182 were allocated to ESC alone. A total of 314 (86.3%) of the total sample were followed up at 5 months [165 (90.7%) in the intervention group and 149 (81.9%) in the control group]. Among those randomised to IMT, 171 (94.0%) received it. From baseline to 5 months, mean scores of ADOS social affect decreased from 14.1 to 13.3 in music therapy and from 13.5 to 12.4 in standard care [mean difference: music therapy vs. standard care = 0.06, 95% confidence interval (CI) -0.70 to 0.81], with no significant difference in improvement. There were also no differences in the parent-rated social responsiveness score, which decreased from 96.0 to 89.2 in the music therapy group and from 96.1 to 93.3 in the standard care group over this period (mean difference: music therapy vs. standard care = -3.32, 95% CI -7.56 to 0.91). There were seven admissions to hospital that were unrelated to the study interventions in the two IMT arms compared with 10 unrelated admissions in the ESC group. Conclusions: Adding IMT to the treatment received by children with ASD did not improve social affect or parent-assessed social responsiveness. Future work: Other methods for delivering music-focused interventions for children with ASD should be explored. Trial registration: Current Controlled Trials ISRCTN78923965. Using music to assist language learning in autistic children with minimal verbal language: The MAP feasibility RCT

Abstract:Autism, Ahead of Print. Music has been shown to improve social interaction and attention to verbal stimuli in autism. We report a feasibility randomised controlled trial of an online intervention using music-assisted programmes, compared with best-practice treatment (Social Communication Intervention for Pre-schoolers–Intensive) for language learning in preschool autistic children with minimal verbal language. Minimisation randomisation ensured comparability of groups before intervention. Ninety-one people expressed interest in taking part; 27 met eligibility criteria and were randomised to receive either music-assisted programmes or Social Communication Intervention for Pre-schoolers–Intensive. Children and their parent received two 45-min sessions weekly, over 18 weeks, coached online by a speech and language therapist. A smartphone app was developed to support home-based practice between sessions. Over the study period, 20% of participants completed the intervention and assessments of outcome measures. At 3 months post-intervention follow-up, social responsiveness, understanding of words and phrases and number of words spoken and parent–child interaction improved more in the music-assisted programmes than the Social Communication Intervention for Pre-schoolers–Intensive group. The results demonstrate the feasibility of recruiting this population into a randomised controlled trial and the music-assisted programmes had high perceived acceptability highlighted by parent interviews. A full clinical trial to establish music-assisted programmes' effectiveness in improving early vocabulary learning in autistic children is warranted.Lay abstractResearch has shown that autistic individuals often have unusually good musical skills and that combining words and music helps autistic individuals to focus on spoken words. This study tests the idea that music will help with early language learning of preschool autistic children. The results show that when caregivers sing words to autistic children, the children pay more attention to the caregiver than when the words are spoken and that they learn word combinations more easily.

Abstract:Music therapy is the clinical use of musical interventions to improve mental and physical health across multiple domains, including social communication. Autistic children, who have difficulties in social communication and often increased anxiety, tend to show a strong preference for music, because it can be structured and systematic, and therefore more predictable than social interaction. This makes music therapy a promising medium for therapeutic support and intervention. Previous clinical trials of music therapy compared to traditional therapy for autistic children have shown encouraging but nevertheless mixed results.

Abstract:Music therapy, as an emerging field of rehabilitation, has been studied by many people. However, it only proves the effectiveness of music therapy in addressing emotional and behavioral issues in children with autism, and there are no specific strategies and methods applicable to special education schools for reference. This study attempts to address this issue. This study adopted a cross subject multi trial design in a single subject experimental method, with music therapy as the independent variable. Using the emotional behavior of these four autistic children who cried for no reason as the dependent variable, data was collected during the baseline, intervention, and maintenance periods to verify the intervention effect of music therapy on emotional behavior. The research results indicate that music therapy has immediate, sustained, and generalized effects on typical emotional and behavioral problems in children with autism. After music therapy intervention, the frequency of emotional and behavioral problems in each of the four autistic children decreased significantly, their self-control ability improved significantly, and language interaction was significantly prolonged. Based on the observation of student expressions and actions, participation statistics, and family interviews, it can be seen that students are interested in the environment of visual music, rhythm training, and situational music. The correct grasp of the core concepts of music therapy and timely feedback on intervention content play a crucial role in the treatment effect in this study.

The cost of time also increases as they need to spend more attention on the care of the autistic child. This is why autism treatment is especially important and relevant in today's society. The most common method used by healthcare providers today is the traditional applied behavior analysis (ABA) therapy. However, traditional applied behavior analysis (ABA) therapy have always ignored the uniqueness of each autistic child and how to integrate into society and increase communication skills. These shortcomings can be solved by the introduction of music therapy, and music therapy can help autistic children better control their emotions and soar in social skills. What is also not to be overlooked is that music therapy reduces the cost of therapy and has greater tolerance and possibilities for the therapeutic environment and equipment. This undoubtedly overcomes some of the drawbacks associated with traditional treatments and improves the efficiency of healthcare while reducing healthcare costs. This paper attempts to analyze the role of music therapy in the treatment of children with autism and to provide feasible recommendations. It also calls on the society to pay more attention to the group of autistic children and light up a light for them.

Abstract:Autism is a prevalent developmental disorder that poses significant challenges for affected children. Music therapy has emerged as a viable strategy with regard to improving speech and communication abilities in autistic children. This article aims to investigate and analyse the effectiveness of three widely used music therapy approaches: Auditory Motor Mapping Training (AMMT), Family-Centred Music Therapy (FCMT), and Melodic Based Communication Therapy (MBCT). Through a comprehensive review of existing studies, the article demonstrates that all three approaches are effective and offer unique benefits in different aspects for children with ASD. These findings suggest the possibility of integrating the three approaches to maximise therapeutic outcomes.
Music improves social communication and auditory-motor connectivity in children with autism

Abstract:Music has been identified as a strength in people with Autism Spectrum Disorder; however, there is currently no neuroscientific evidence supporting its benefits. Given its universal appeal, intrinsic reward value and ability to modify brain and behaviour, music may be a potential therapeutic aid in autism. Here we evaluated the neurobehavioural outcomes of a music intervention, compared to a non-music control intervention, on social communication and brain connectivity in school-age children (ISRCTN26821793). Fifty-one children aged 6-12 years with autism were randomized to receive 8-12 weeks of music (n = 26) or non-music intervention (n = 25). The music intervention involved use of improvisational approaches through song and rhythm to target social communication. The non-music control was a structurally matched behavioural intervention implemented in a non-musical context. Groups were assessed before and after intervention on social communication and resting-state functional connectivity of fronto-temporal brain networks. Communication scores were higher in the music group post-intervention (difference score = 4.84, P = .01). Associated post-intervention resting-state brain functional connectivity was greater in music vs. non-music groups between auditory and subcortical regions (z = 3.94, P < .0001) and auditory and fronto-motor regions (z = 3.16, P < .0001). Post-intervention brain connectivity was lower between auditory and visual regions in the music compared to the non-music groups, known to be over-connected in autism (z = 4.01, P < .00001). Post-intervention brain connectivity in the music group was related to communication improvement (z = 3.57, P < .0001). This study provides the first evidence that 8-12 weeks of individual music intervention can indeed improve social communication and functional brain connectivity, lending support to further investigations of neurobiologically motivated models of music interventions in autism.
Effects of Three Music Therapy Interventions on the Verbal Expressions of Children With Autism Spectrum Disorder: A Combined Single-Subject Design

Abstract:The specific aims of this research study were to (a) examine the differential effect of three different music interventions, namely the interactive music playing therapy ("music and singing"), interaction music singing therapy ("singing"), and receptive music therapy ("listening") studying the varying latency periods in the response time it took 3-year-old children diagnosed with autism spectrum disorder (ASD) to elicit the target word vocally; and (b) assess the index of happiness of children with ASD after the implementation of the three music interventions, which can, in turn, be used to influence their overall quality of life through this specific intervention. This study used a combined single-subject research design consisting of delayed multiple baseline across the participants and a multielement design to compare the effects of each music intervention technique targeting the child's verbal response during playback of a practiced song. Findings demonstrated "singing" to be associated with the lowest latency compared to the other two interventions ("listening" and "singing and music") across participants. Additionally, happiness levels varied from neutral to happy, signifying an overall positive experience during participation in the music applied behavior analysis (ABA) intervention.
Piano with a Twist: A Pilot Study Exploring the Preliminary Effects of a Piano Therapy Program for Children with Autism Spectrum Disorder

Abstract:Children with autism spectrum disorder (ASD) show frequent maladaptive behaviors and difficulties in social communication and motricity. Various studies suggest that music-based interventions might help overcome these challenges. Yet, their findings are not conclusive, particularly regarding piano music. The current pilot study aims to present a new music-based intervention – the Piano Therapy program – and to explore its effects on social communication, motricity, self-care, and adaptive behavior skills in children with ASD. The program uses techniques derived from Applied Behavioral Analysis and active and receptive musical methods structured around a single instrument (i.e., piano). Sixty children with ASD were quasi-randomly assigned either to the Piano Therapy group or the control group. In the experimental group, the 4-month program was administered individually, twice a week. Using the Psychoeducational Profile - Third Edition (PEP-3) reports, the examiner and the caregivers assessed participants' skills pre- and post-intervention. The findings indicate that the children enrolled in the Piano Therapy program show significant improvements in all the developmental areas assessed with PEP-3, compared to the control group. Although further research is required, these preliminary results have clinical implications for practitioners and bring new evidence about the beneficial effects of a piano-based intervention in individuals with ASD.

Abstract:Background: Several studies have reported contradictory results regarding the benefits of music interventions in children and adolescents with neurodevelopmental disorders (NDDs), including autism spectrum disorder (ASD). Methods: We performed a systematic review according to the PRISMA guidelines. We searched the Cochrane, PubMed and Medline databases from January 1970 to September 2020 to review all empirical findings, except case reports, measuring the effect of music therapy on youths with ASD, intellectual disability (ID), communication disorder (CD), developmental coordination disorder (DCD), specific learning disorder, and attention/deficit hyperactivity disorder (ADHD). Results: Thirty-nine studies ( N = 1,774 participants) were included in this review (ASD: n = 22; ID: n = 7; CD and dyslexia: n = 5; DCD: n = 0; ADHD: n = 5 studies). Two main music therapies were used: educational music therapy and improvisational music therapy. A positive effect of educational music therapy on patients with ASD was reported in most controlled studies (6/7), particularly in terms of speech production. A positive effect of improvisational music therapy was reported in most controlled studies (6/8), particularly in terms of social functioning. The subgroup of patients with both ASD and ID had a higher response rate. Data are lacking for children with other NDDs, although preliminary evidence appears encouraging for educational music therapy in children with dyslexia. Discussion: Improvisational music therapy in children with NDDs appears relevant for individuals with both ASD and ID. More research should be encouraged to explore whether oral and written language skills may improve after educational music therapy, as preliminary data are encouraging.

Improving a Parent Coaching Model of Music Interventions for Young Autistic Children

Abstract:Parenting a child on the autism spectrum can be rewarding and enriching, but it may also increase risk of parental fatigue, stress, anxiety, and depression. Parent-mediated interventions contribute to increase family satisfaction and child social communication while helping to decrease parental stress and fatigue. Parent coaching, the education of parents in evidence-based strategies, has become common in the autism field. However, parent coaching in music therapy has only recently emerged and has limited research with families with an autistic member. In this study, we attempted to improve a previously published model of parent coaching, adapting only one aspect of the Early Start Denver Model (ESDM), the sensory social routine (SSR) to create a music intervention. Four parents participated in this 6-session parent coaching study. We compared the SSR-based intervention with and without music, in an alternating treatment design. Measures included parental responsiveness, child receptive and initiation joint attention, parent-child similar affect and synchronized gaze, and the Parent Coaching-ESDM (PC-ESDM) parent fidelity rating system. Results from these observational measures were mixed, with better parental responses in the no-music condition, but improved child responses and parent-child synchrony in the music condition for 3 out of the 4 participants. Parent learning increased for all participants, and 3 out of the 4 reached fidelity (a score of at least 80%), according to the PC-ESDM. Although mixed results were observed across participants, implications for practice are possible. Better outcome measures of this complex intervention are needed.
