Abstract:Objective: To investigate the clinical, radiographic characteristics and prognosis of pulmonary intravascular large B-cell lymphoma(IVLBCL) manifesting as interstitial pneumonias on HRCT. Methods: A retrospective analysis was carried out on clinical data of 2 patients with pulmonary IVLBCL admitted to the Affiliated Drum Tower Hospital of Nanjing University from March 2010 to May 2012. A literature research was performed with "pulmonary intravascular lymphoma" as the key word in Wanfang Database, China Knowledge Resource Integrated Database and Pubmed. The time interval was from January 1980 to June 2015. Related articles of pulmonary IVLBCL were retrieved and the clinical, radiographic characteristics and prognosis were analyzed. Results: The first patient was a 69 year-old female presenting with ground-glass opacities, interlobular septal thickening and patchy consolidation on HRCT, for whom the diagnosis was confirmed by transbronchial lung biopsy (TBLB). The second patient was a 70 year-old male presenting with diffuse ground-glass opacities on HRCT, and the diagnosis was made by pathology on video-assisted thoracoscopic biopsy. The 2 patients all presented with dyspnoea, cough, fever and elevated lactate dehydrogenase(LDH). The pathological study of lung biopsy specimen demonstrated invasion of atypical lymphocytes into small vessels and capillaries. The tumor cells were positive for CD(20).Literature review found 19 articles, all case reports with a total of 22 patients. Conclusions: The clinical manifestation of pulmonary IVLBCL was nonspecific and the disease progressed rapidly. For patients with interstitial pneumonias on HRCT, pulmonary IVLBCL needed to be considered as a differential diagnosis and pathological studies should be obtained as soon as possible, so that better prognosis could be archived through early intervention.

Effective Management of a Rare Case of Primary Renal Intravascular Large B-cell Lymphoma with Modified R-CHOP Regimen: A Case Report

Abstract:Intravascular large B-cell lymphoma, known for its diverse organ involvement, presents significant diagnostic challenges, particularly when it affects the kidneys. This report highlights a rare case of primary renal intravascular large B-cell lymphoma in a 60-year-old male patient, who presented with persistent fever and renal dysfunction. The case underscores the intricacy of diagnosis and the efficacy of personalized treatment. Following the identification of primary renal intravascular large B-cell lymphoma, a modified R-CHOP regimen was administered, resulting in notable amelioration of symptoms and renal function following the initial treatment cycle. The patient achieved sustained complete remission without any complications after completing five subsequent R-CHOP cycles and two additional cycles of rituximab monotherapy, as confirmed by recent assessments. He is currently under regular follow-up for ongoing monitoring and improvement. This case adds to the limited yet expanding pool of knowledge concerning intravascular large B-cell lymphoma, emphasizing the necessity for personalized therapeutic strategies in atypical presentations. It also highlights the importance of early detection and customized intervention in managing rare lymphoma subtypes with unique organ involvement.

Abstract:Objective:To investigate the clinicopathological feature as well as the diagnosis and prognosis of intravascular large B-cell lymphoma(IVLBCL).Methods:Two cases of IVLBCL were studied on their clinical data,histopathology and immunohistochemistry.Results:The age of two patients were 73 and 54 years old.Both patients presented with fever.Histologically,the lesions consisted of dilated vascular,and the vascular channels were filled with neoplastic cells.Immunohistochemically,the tumor cells were positive for CD20,and negative for CD3.The follow-up revealed that one patient died at 2 months from the date of clinical presentation and another died at 7 months.Conclusion:IVLBCL is a rare variant of non-Hodgkin lymphoma,and the diagnosis is based on histopathology.IVLBCL is with high aggression,and the patients have poor prognosis.

Abstract:Primary pulmonary intravascular large B‐cell lymphoma (IVLBCL) is a rare malignant extranodal lymphoma, and it presents symptoms similar to those of lung diseases. Diagnosis of IVLBCL can be challenging and often requires histopathological examination of affected tissues. The present report describes a 65‐year‐old female patient presenting with a fever, without generalized lymph node enlargement. A CT scan revealed subpleural ground‐glass opacities in the upper lobe of the right lung. The lactate dehydrogenase, erythrocyte sedimentation rate and CRP levels were all elevated. The patient had been misdiagnosed with pneumonia by multiple hospitals, and treatments with anti‐infective and anti‐inflammatory therapies proved ineffective. Subsequently, the patient underwent a CT‐guided puncture biopsy of the lesion in the upper lobe of the right lung at the Second Hospital of Dalian Medical University (Dalian, China). Based on the cell morphology, combined with clinical manifestations and other laboratory tests, the possibility of lymphoma was considered. The diagnosis was subsequently confirmed by histopathological examination. To the best of our knowledge, this is the first description of the features of IVLBCL cells under a microscope using an oil‐immersion objective lens.

Abstract:Abstract Background pulmonary primary diffuse large B-cell lymphoma is an extremely rare B cell lymphoproliferative disorder, and cases mimicking an unspecific pulmonary inflammatory lesion are even rarer, which can be easily misdiagnosed.Case presentation: We reported a 33-year-old female patient with multiple ground glass nodules in the lung on chest computed tomography (CT) and Positron emission tomography-computed tomography (PET-CT) scans. Pulmonary biopsy revealed tumor cells growing along the alveolar septum, hiding in an inflammatory background. Although some tumor cells invaded blood vessels, the majority of the tumor cells were distributed outside the vessels. Based on the morphology, immunohistochemistry, and molecular detection, the diagnosis was confirmed as pulmonary primary diffuse large B-cell lymphoma, anaplastic variant. The diagnosis of this type of tumor is challenging due to its clinical and pathological characteristics, requiring differentiation from inflammatory lesions and intravascular large B-cell lymphoma. Tumor cells expressed CD30 and CD20. The patient was treated with brentuximab vedotin (BV) combined with rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone(R-CHOP) regimen for six courses, resulting in almost complete disappearance of the lesions. The patient had a good prognosis, with no involvement of other sites after a six-month follow-up.Conclusions This case emphasizes the rarity of pulmonary primary diffuse large B-cell lymphoma and the significance of being attentive to atypical imaging and pathological morphology that may lead to misdiagnosis. Treatment with BV combined with R-CHOP therapy resulted in complete remission of the lesions. Hence, clinicians and pathologists should carefully observe and consider this diagnosis in such cases.

Abstract:Background: Intravascular large B-cell lymphoma (IVLBCL) is an extremely rare type of malignant lymphoma characterized by exclusive or predominant growth of neoplastic cells within the lumen of blood vessels. Cases in the literature predominantly involve the skin and central nervous system, with special emphasis on the 'cutaneous variant'. Methods: Three cases of IVLBCL with cutaneous manifestations, including two systemic IVLBCL and one cutaneous variant, were described in this study. In all cases, clinical presentation and follow-up data were meticulously evaluated and immunophenotypic and molecular studies were performed. Results: All three cases displayed the B-cell phenotype and showed monoclonality with immunoglobulin heavy chain gene rearrangement. Bcl2 was expressed in the two systemic IVLBCL cases with fatal outcomes. The third patient with the 'cutaneous variant' achieved complete remission and a longer survival time of 15 months after chemotherapy. Conclusions: Skin manifestations and neurological findings, although to different degrees, are important clues to the diagnosis of IVLBCL. As most IVLBCL are grouped into the post-germinal center B-cell subtype of diffuse large B-cell lymphoma, Bcl2 expression may be correlated with a worse prognosis in IVLBCL. The cutaneous variant of IVLBCL has a significantly better outcome than that of systemic IVLBCL.

Abstract:It is unclear whether patients who test positive for anti-hepatitis C virus (HCV) antibodies and have normal alanine aminotransferase (ALT) levels remain infected with the virus. Eleven patients who tested positive for anti-HCV antibodies, had persistently normal ALT levels, and tested negative for HCV RNA by commercial test were studied. Serum and peripheral blood mononuclear cells (PBMCs) were collected 2-3 times at 3-6-month intervals, and PBMCs were cultured with phytohemagglutinin and pokeweed mitogen. HCV RNA was detected in serum samples from 6 (55%) and in PBMCs from 11 (100%) patients. Our results suggest that, in asymptomatic patients who test positive for anti-HCV antibodies, small quantities of HCV RNA commonly persist, even in patients who test negative for HCV RNA in serum by commercial tests.

Abstract:Four cases of pediculosis, two in adults and two in children, caused by the crab-louse, Pthirus pubis Linnaeus, 1758 (Diptera: Anoplura) is reported from peninsular Malaysia. This is the second report of the problem to be documented from the country. Although P. pubis is closely associated with genital hairs, it is, however, also found to occur on the eyelashes, eyebrows, hairs of the body, head and axilla. The few reported cases of pthiriasis probably do not reflect the true situation.

Abstract:Intravascular large B-cell lymphoma, an extranodal large B-cell lymphoma, is a rare hematological malignancy with only a few reports of lung involvement. We report a case of intravascular large B-cell lymphoma with acute hypoxic respiratory failure and interstitial lung disease diagnosed via random skin biopsies. A 54-year-old woman presented with fever, cough, and dyspnea. Computed tomography imaging revealed findings concerning interstitial lung disease. The patient's respiratory status worsened despite the treatment with antibiotics and steroids. Generalized edema and thrombocytopenia also developed. Intravascular large B-cell lymphoma was clinically suspected and ultimately diagnosed by skin biopsy, although she had no apparent skin lesions. The patient's condition considerably improved after chemotherapy. Intravascular large B-cell lymphoma should be considered in patients with acute respiratory failure and interstitial lung lesions.

Abstract:A 68-year-old man was presented with high fever of unknown origin for 3 weeks and non-productive cough for 1 week. A chest computed tomography (CT) scan revealed multiple nodules and ground glass opacities (GGO) in both lungs. The patient was initially diagnosed with hypersensitivity pneumonitis based on the result of bronchoalveolar lavage fluids (BALF). After treatment with methylprednisolone for 2 weeks, the patient's fever recurred, with no resolution of lesions on chest CT. The patient consented to positron emission tomography (PET)/CT. It showed that fluorodeoxyglucose (FDG) metabolism was significantly increased in the spleen, whole skeleton, and both lungs, suggesting a malignant hematological disease. Large B-cell lymphoma was diagnosed by bone marrow puncture and flow cytometry. Transbronchial lung cryobiopsy was performed to evaluate the diffuse lung lesion. Hematoxylin-eosin (HE) staining showed diffuse infiltration of heterotypic cells in the pulmonary interstitial capillaries. Furthermore, immunohistochemical examination results suggested lung infiltration of B lymphohematopoietic system tumors. The patient was finally diagnosed as intravascular large B-cell lymphoma (IVLBCL). IVLBCL with diffuse lung ground glass lesions is very rare and difficult to diagnose. Transbronchial lung cryobiopsy, as an emerging procedure, plays an important role in the diagnosis of interstitial lung disease and has gained popularity for a lower complication rate and acquisition of more tissue samples.

Abstract:Abstract Intravascular large B-cell lymphoma (IVLBCL) is a rare, clinically aggressive lymphoma defined by the proliferation of atypical lymphoma cells in the lumen of all sized blood vessels, particularly capillaries. The reasons for this unusual neoplastic cell proliferation are still only partially understood. IVLBCL is considered stage IV lymphoma and manifests with a variety of nonspecific signs and symptoms. Patients with IVLBCL usually do not present with lymphadenopathy. The tumor cells invade the blood vessels of multiple organs such as the central nervous system, skin, lungs, kidneys, and bone marrow. Common presenting symptoms are based on the organ affected and include mental status changes and fever of unknown origin. Although immunochemotherapy has significantly improved the often-poor prognosis of this kind of lymphoma, a large percentage of patients’ relapse. We present a 63-year-old man who had been diagnosed with large B cell lymphoma in the bone marrow in March 2021 and was in remission state after completing six cycles of chemotherapy. There was no abnormal FDG uptake on a post-chemotherapy PET/CT scan. Patient presented to the emergency room (ER) two months later with fever and dyspnea. The entire workup was completed and showed pancytopenia and elevated ESR. While chest CT scan did not show lymphadenopathy or lesions, PET/CT scans revealed a widespread increase in FDG uptake in both the lungs and spleen. Lung biopsy revealed large, atypical cells within alveolar septae and vessels. Immunohistochemical stains demonstrated that these cells were positive for CD20 and PAX-5 and had high proliferation rate based on Ki67. IVLBCL has a low incidence rate with non-specific clinical presentation. The diagnosis can be easily missed in both clinical, radiological and the corresponding histopathological findings. Radiological finding and CT scan are not sensitive enough and may miss the lesion. Even though the PET/CT scan is more sensitive, the definitive diagnosis of IVLBCL relies mainly on histopathology and immunohistochemistry, at which point awareness of this entity by the pathologist is most necessary.

Abstract:Objective: To analyze the clinicopathologic characteristics and prognosis of testicular diffuse large B-cell lymphoma (DLBCL) . Methods: A retrospective analysis was performed on 68 patients with testicular DLBCL admitted to Ruijin Hospital affiliated to Shanghai Jiao Tong University School of Medicine from October 2001 to April 2020. The gene mutation profile was evaluated by targeted sequencing (55 lymphoma-related genes) , and prognostic factors were analyzed. Results: A total of 68 patients were included, of whom 45 (66.2% ) had primary testicular DLBCL and 23 (33.8% ) had secondary testicular DLBCL. The proportion of secondary testicular DLBCL patients with Ann Arbor stage Ⅲ-Ⅳ (P<0.001) , elevated LDH (P<0.001) , ECOG score ≥ 2 points (P=0.005) , and IPI score 3-5 points (P<0.001) is higher than that of primary testicular DLBCL patients. Sixty-two (91% ) patients received rituximab in combination with cyclophosphamide, adriamycin, vincristine, and prednisone (R-CHOP) -based first-line regimen, whereas 54 cases (79% ) underwent orchiectomy prior to chemotherapy. Patients with secondary testicular DLBCL had a lower estimated 5-year progression-free survival (PFS) rate (16.5% vs 68.1% , P<0.001) and 5-year overall survival (OS) rate (63.4% vs 74.9% , P=0.008) than those with primary testicular DLBCL, and their complete remission rate (57% vs 91% , P=0.003) was also lower than that of primary testicular DLBCL. The ECOG scores of ≥2 (PFS: P=0.018; OS: P<0.001) , Ann Arbor stages Ⅲ-Ⅳ (PFS: P<0.001; OS: P=0.018) , increased LDH levels (PFS: P=0.015; OS: P=0.006) , and multiple extra-nodal involvements (PFS: P<0.001; OS: P=0.013) were poor prognostic factors in testicular DLBCL. Targeted sequencing data in 20 patients with testicular DLBCL showed that the mutation frequencies of ≥20% were PIM1 (12 cases, 60% ) , MYD88 (11 cases, 55% ) , CD79B (9 cases, 45% ) , CREBBP (5 cases, 25% ) , KMT2D (5 cases, 25% ) , ATM (4 cases, 20% ) , and BTG2 (4 cases, 20% ) . The frequency of mutations in KMT2D in patients with secondary testicular DLBCL was higher than that in patients with primary testicular DLBCL (66.7% vs 7.1% , P=0.014) and was associated with a lower 5-year PFS rate in patients with testicular DLBCL (P=0.019) . Conclusion: Patients with secondary testicular DLBCL had worse PFS and OS than those with primary testicular DLBCL. The ECOG scores of ≥2, Ann Arbor stages Ⅲ-Ⅳ, increased LDH levels, and multiple extra-nodal involvements were poor prognostic factors in testicular DLBCL. PIM1, MYD88, CD79B, CREBBP, KMT2D, ATM, and BTG2 were commonly mutated genes in testicular DLBCL, and the prognosis of patients with KMT2D mutations was poor.
Current progress and future perspectives of research on intravascular large B‐cell lymphoma

Abstract:Intravascular large B-cell lymphoma is a rare disease of the large B cells characterized by selective growth in the lumina of small vessels in systemic organs. Since first reported in 1959, the difficulty of obtaining sufficient tumor cells from biopsy specimens has hampered the elucidation of its underlying biology. Recent progress using xenograft models and plasma cell-free DNA has uncovered genetic features that are similar to those of activated B-cell type diffuse large B-cell lymphoma, including MYD88 and CD79B mutations and frequent alterations in immune check point-related genes such as PD-L1 and PD-L2. Given the improvement in clinical outcomes and a higher risk of secondary central nervous system (CNS) involvement in the rituximab era, a phase 2 trial of R-CHOP combined with high-dose methotrexate and intrathecal chemotherapy as a CNS-oriented therapy has been conducted. This trial, the PRIMEUR-IVL study, has displayed good progression-free survival and a low cumulative incidence of secondary CNS involvement. Long-term follow-up within this trial is still ongoing. Further understanding of the pathophysiology of the disease and improvements in clinical outcomes are still needed.

Abstract:Intravascular large B-cell lymphoma(IVLBCL) is a rare form of diffuse LBCL characterized by preferential intravascular growth of malignant lymphocytes,aggressive behavior,and an often fatal course.It displays some differences in clinical presentation among diverse geographical areas.The immunophenotype plays a critical role in diagnosis and differential diagnosis.Recent therapeutic approaches,such as the rituximab plus CHOP regimen,could have a positive impact in IVLBCL patients.The use of high-dose chemotherapy supported by autologous stem-cell transplantation may improve current outcomes.

Abstract:Intravascular large-B-cell lymphoma (IVLBCL) is a unique type of extranodal lymphoma characterized by selective growth of tumor cells in small vessels without lymphadenopathy. Greater understanding of the molecular pathogenesis of IVLBCL is hampered by the paucity of lymphoma cells in biopsy specimens, creating a limitation in obtaining sufficient tumor materials. To uncover the genetic landscape of IVLBCL, we performed whole-exome sequencing (WES) of 21 patients with IVLBCL using plasma-derived cell-free DNA (cfDNA) (n = 18), patient-derived xenograft tumors (n = 4), and tumor DNA from bone marrow (BM) mononuclear cells (n = 3). The concentration of cfDNA in IVLBCL was significantly higher than that in diffuse large-B-cell lymphoma (DLBCL) (P < 0.0001) and healthy donors (P = 0.0053), allowing us to perform WES, and most mutations detected in BM tumor DNA were successfully captured in cfDNA and xenograft. IVLBCL showed a high frequency of genetic lesions characteristic of activated-B-cell-type DLBCL; with the former showing conspicuously higher frequencies (compared to nodal DLBCL) of mutations in MYD88 (57%), CD79B (67%), SETD1B (57%), and HLA-B (57%). We also found that 8 (38%) IVLBCL harbored rearrangements of PD-L1/PD-L2 involving the 3'-UTR; such rearrangements are implicated in immune evasion via PD-L1/PD-L2 overexpression. Our data demonstrate the utility of cfDNA and imply important roles for immune evasion in IVLBCL pathogenesis and PD-1/PD-L1/PD-L2 blockade in therapeutics for IVLBCL.

Abstract:Abstract Background Large B-cell lymphoma (LBCL) with interferon regulatory factor 4 (IRF4) rearrangement (IRF4+LBCL) is a rare and newly discovered subtype of mature B cell neoplasms. Case presentationHere, we describe a patient of 32 years old who was diagnosed IRF4+LBCL. Histological examination showed the normal structure of the lymphoid tissues were destroyed, and slightly crowded follicular or nodal structures instead. There were obvious necrosis on the surface of tonsil and the central part of some follicles. The monomorphic atypical lymphoid cells proliferated and grew consistently, which were of medium size or large, and the nuclear chromatin was opening. Some tumor cells can be seen around the normal striated muscle tissues near the tonsils. Immunohistochemistry (IHC) could show that CD20, CD79a, MUM-1 and BCL6 were positive, Ki-67 was 80%; CD3, CD5, CD10, BCL2, CD30, CD56, CD99, CD38, and CD138 were negative. In situ hybridization (ISH) of EBER was negative. Fluorescence in situ hybridization (FISH) confirmed that IRF4 gene rearrangement was found in tumor cells. The patient was followed up for 18 months without tumor after chemotherapy. ConclusionGenerally speaking, destructive growth patterns with a large number of necrosis, high proliferation index and so on all suggest that the tumor is highly invasive. And in terms of pathological morphology, IRF4+LBCL can be similar to both high-grade follicular lymphoma (FL) and diffuse large B-cell lymphoma (DLBCL). But actually this disease is indolent and significantly different.
