Abstract:Objective To analyze adverse reaction of voriconazole in Zhejiang province,and provide reference for safe and rational clinical use of it. Methods Eighty-four cases of voriconazole ADR reported from 2013 to 2015 were analyzed in terms of sex,age,mode of administration,time of ADR,combined drug use,organ or system involvement,outcome,etc. Results In the 84 cases of voriconazole ADR,the number of males was higher than that of females,aged 22 to 95 years old,and most cases were more than 60 years of age,accounting for 75.00%.Forty-six cases were caused by intravenous infusion,accounting for 54.76%,31 ADR cases were caused by oral administration,accounting for 36.90%,7 ADR cases were caused by intravenous infusion and oral administration,accounting for 8.33%.In 20 cases,ADR occurred immediately or on the administration day, accounting for 23.81%,and ADR in 36 cases occurred 2 to 10 days after the administration,accounting for 42.86%.The clinical manifestations of ADR were mainly digestive system injury,accounting for 31.58%,and the liver function abnormality was most common,mainly caused by oral administration;visual impairment was the second,accounting for 17.54%,mainly caused by the intravenous infusion;the proportion of nervous system injury was 16.67%,and the proportion of systemic reactions accounted for 11.40%. Conclusion During the period of clinical use,doctors,pharmacists,and nurses should strengthen the propaganda and education,recognition and prevention of voriconazole's ADR.The qualified hospitals may monitor the blood concentration and achieve individualized administration,to reduce the occurrence of ADR.

Abstract:Objective To determine the plasma concentrations of voriconazole of inpatients,and access the association between the trough concentrations and clinical effectiveness and adverse drug reactions.Methods The trough concentrations were analyzed by LC-MS/MS.The clinical effectiveness and adverse drug reactions during the pharmacotherapy period were observed.Results A total of 17 patients were enrolled and their trough concentrations of voriconazole were determined.The median concentration was 3.92 μg·mL-1(concentration range was 0.71 7.38 μg·mL-1).There were 14 patients completely or partial responding to the anti-fungal therapy and 3 failing to responding.A number of 7 patients experienced voriconazole-related drug adverse reactions,and 5 of them were liver abnormalities,2 of them were visual disturbances,and 1 was neurotoxicity.The median concentration of those experienced adverse drug reactions was 4.80 μg·mL-1.Conclusion High concentration variability was observed among patients,and routine monitoring of voriconazole blood level may improve the drug safety.Voriconazole concentration over 4.80 μg·mL-1 is related to a higher proportion of adverse drug reaction.

Abstract:Aims: The gene polymorphism of voriconazole metabolism-related liver enzyme is notable in East Asia population. It casts a significant influence on the rational use of voriconazole. We conducted this study to investigate the relationship between steady-state voriconazole trough concentration (C-trough) and adverse effects (AEs), especially hepatotoxicity. Methods: We conducted a real-world study in the Jinling Hospital from January 2015 to June 2020. A total of 140 patients receiving voriconazole were enrolled in this study. The determination and scoring of voriconazole-associated hepatotoxicity were performed according to the Roussel Uclaf Causality Assessment Method scoring scale and the severity of hepatotoxicity was graded according to the Common Terminology Criteria for Adverse Events (CTCAE). Results: Elevated steady-state voriconazole C-trough with concomitant AEs are the most common reason for dose adjustments during treatment. Compared with the group without any AEs, voriconazole C-trough was significantly higher in the hepatotoxicity and neurotoxicity groups, and the incidence of both events showed an overall increasing trend with increasing voriconazole C-trough. Hepatotoxicity occurred in 66.7% of patients within 7 days of the first dose of voriconazole and 94.4% within 15 days of the dose. Steady-state voriconazole C-trough >3.61 mg/l was associated with an increased incidence of hepatotoxicity (area under the curve = 0.645, p = 0.047). Logistic regression analysis showed that timely voriconazole dose adjustment was a predictor of attenuated hepatotoxicity after adjustment for confounders, but hepatotoxicity was not associated with voriconazole C-trough measured at a single time point. Conclusion: Hepatotoxicity and neurotoxicity correlate with voriconazole C-trough, and dose reduction in patients with elevated steady-state voriconazole C-trough may prevent hepatotoxicity. In patients with early occurrence of hepatotoxicity, initial therapeutic drug monitoring (TDM) might predict the risk of hepatotoxicity. Follow-up TDM may be necessary to predict late onset hepatotoxicity.

Abstract:SESSION TITLE: Infections: Fungal SESSION TYPE: Original Investigation Poster PRESENTED ON: Saturday, April 16, 2016 at 11:45 AM - 12:45 PM PURPOSE: The part was to study the pharmacokinetics of voriconazole following a single-administration and the multiple-administration of voticonazole and the correlation between voriconazole plasma concentration and biochemical indicators as well as concurrent drug in elderly patients with fungal infection. METHODS: There are 10 elderly patients with fungal infection, every patient was given once 300mg voriconazole instilled with 100mLliquid for 1h twice a day, 3-5 ml blood was taken from side antebrachium for every patient at the following time point: before administration 0.5h, and after administration 30 min,1.0 h, 2.0 h, 3.0h, 4.0h, 6h, 8h, 12h, 16h, 18h, 24h, 36h and 48h. All plasma samples was determined by validated HPLC method and the pharmacokinetics parameter was calculated by DAS2.0 soft. RESULTS: The 10 elderly patients with fungal infection patients had been given single dose injection voriconazole, the pharmacokinetics parameter are:Tmax: 0.90±0.21h, Cmax:3.38±0.68mg/L; t1/2:5.23±0.64h; AUC0-t:23.37±6.41mg g·h·L-1; AUC0-∞: 48.91±26.96 mg·h·L-1. The plasma level of dose voriconazole was significantly associated with weight, age, alanine aminotransferase, direct bilirubin, glucocorticoid and proton pump CONCLUSIONS: The results showed that pharmacokinetics of voriconazole of elderly patients had coincidence with non-compartment model. These parameter are coincident to the pharmacokinetics parameter reported in the literature. Increasing age and combined with proton pump inhibitor in patients (omeprazole) can significantly improve the voriconazole concentration, while the increasing of body weight of patients and the combined application of Glucocorticoid (methylprednisolone or prednisone / prednisolone, dexamethasone) can reduce the voriconazole drug concentration, the combination of voriconazole glucocorticoid can significantly reduce the concentration of drug. CLINICAL IMPLICATIONS: Monitoring of blood concentration of voriconazole for patients, not only can guide clinical individualized medication scheme optimization, but also can reduce the incidence of adverse reactions, and improve drug safety and efficacy. Abstract:Objectives: The optimum trough concentration of voriconazole for clinical response and safety is controversial. The objective of this review was to determine the optimum trough concentration of voriconazole and evaluate its relationship with efficacy and safety. Methods: MEDLINE, EMBASE, ClinicalTrials.gov, the Cochrane Library and three Chinese literature databases were searched. Observational studies that compared clinical outcomes below and above the trough concentration cut-off value were included. We set the trough concentration cut-off value for efficacy as 0.5, 1.0, 1.5, 2.0 and 3.0 mg/L and for safety as 3.0, 4.0, 5.0, 5.5 and 6.0 mg/L. The efficacy outcomes were invasive fungal infection-related mortality, all-cause mortality, rate of successful treatment and rate of prophylaxis failure. The safety outcomes included incidents of hepatotoxicity, neurotoxicity and visual disorders. Results: A total of 21 studies involving 1158 patients were included. Compared with voriconazole trough concentrations of >0.5 mg/L, levels of <0.5 mg/L significantly decreased the rate of treatment success (risk ratio = 0.46, 95% CI 0.29-0.74). The incidence of hepatotoxicity was significantly increased with trough concentrations >3.0, >4.0, >5.5 and >6.0 mg/L. The incidence of neurotoxicity was significantly increased with trough concentrations >.4.0 and >.5.5 mg/L. Conclusions: A voriconazole level of 0.5 mg/L should be considered the lower threshold associated with efficacy. A trough concentration >.3.0 mg/L is associated with increased hepatotoxicity, particularly for the Asian population, and >.4.0 mg/L is associated with increased neurotoxicity.

Abstract:After 1 hr. of continuous infusion of L-(35S)methionine the specific activities of L-methionine in plasma and tissue-free and tRNA-bound L-methionine in brain were in the same range. This result indicates that, under steady-state conditions, dilution of the precursor pool for protein synthesis (L-methionyl-tRNA) by L-methionine derived from a source other than plasma can be considered as negligible.
Voriconazole Therapeutic Drug Monitoring and Hepatotoxicity in Critically Ill Patients: A Nationwide Multicenter Retrospective Study

Abstract:Objectives The aim of this study was to characterize voriconazole trough concentrations ( C min ) and associated hepatotoxicity, and to determine predictors of hepatotoxicity and identify high-risk groups in critically ill patients. Methods This was a nationwide, multicenter, retrospective study. We studied C min and hepatotoxicity in 363 critically ill patients from 2015 to 2020 who received voriconazole treatment. Logistic regression and classification and regression tree (CART) models were used to identify the high-risk patients. Results A large interindividual variability was observed in the voriconazole initial C min and concentrations ranging from 0.1 mg/L to 18.72 mg/L. Voriconazole-related grades ≥2 hepatotoxicity developed in 101 patients, including 48 patients with grades ≥3 hepatotoxicity. The median time to hepatotoxicity occurrence was 3 days (range 1–24 days), and 83.2% of hepatotoxicity cases occurred within 7 days after voriconazole initiation. Voriconazole C min was significantly associated with hepatotoxicity. The CART model showed that the significant predictors of grades ≥2 hepatotoxicity were C min >3.42 mg/L, receiving trimethoprim-sulfamethoxazole or tigecycline treatment simultaneously, and suffering from sepsis shock. The model predicted that grades ≥2 hepatotoxicity incidence among these high-risk patients was 48.3–63.4%. The significant predictors of grades ≥3 hepatotoxicity were C min >6.87 mg/L, concomitating use of ≥3 hepatotoxic drugs, and complicating with sepsis shock, and the predictive incidence among these high-risk patients was 22.7–36.8%. Conclusion Patients with higher voriconazole C min and sepsis shock, as well as concomitant use of hepatotoxic drugs were the strongest predictors of hepatotoxicity. Voriconazole plasma concentration monitoring should be performed early to avoid hepatotoxicity.

Abstract:Introduction: The application of voriconazole in patients with liver dysfunction lacks pharmacokinetic data. In previous study, we proposed to develop voriconazole dosing regimens for these patients according to their total bilirubin, but the regimens are based on Monte Carlo simulation and has not been further verified in clinical practice. Besides, there are few reported factors that significantly affect the efficacy of voriconazole. Methods: We collected the information of patients with liver dysfunction hospitalized in our hospital from January 2018 to May 2022 retrospectively, including their baseline information and laboratory data. We mainly evaluated the efficacy of voriconazole and the target attainment of voriconazole trough concentration. Results: A total of 157 patients with liver dysfunction were included, from whom 145 initial and 139 final voriconazole trough concentrations were measured. 60.5% (95/157) of patients experienced the adjustment of dose or frequency. The initial voriconazole trough concentrations were significantly higher than the final (mean, 4.47 versus 3.90 μg/mL, p = 0.0297). Furthermore, daily dose, direct bilirubin, lymphocyte counts and percentage, platelet, blood urea nitrogen and creatinine seven covariates were identified as the factors significantly affect the voriconazole trough concentration. Binary logistic regression analysis revealed that the lymphocyte percentage significantly affected the efficacy of voriconazole (OR 1.138, 95% CI 1.016–1.273), which was further validated by the receiver operating characteristic curve. Conclusion: The significant variation in voriconazole trough concentrations observed in patients with liver dysfunction necessitates caution when prescribing this drug. Clinicians should consider the identified factors, particularly lymphocyte percentage, when dosing voriconazole in this population.

Abstract:Sub 200 nm wafer-to-wafer (w2w) overlay accuracy on the entire 300 mm wafer was successfully demonstrated via wafer level Cu/SiO2 hybrid bonding. Cu bonding pads relevant for back-side illuminated (BSI) complementary metal oxide semiconductor (CMOS) image sensor (CIS) were used for the experiment. Further, a crucial component to improve the overlay accuracy, namely the overlay model which identifies systematic alignment errors, was described.

Abstract:Voriconazole has been widely used in clinical practice for nearly 20 years. The adverse reactions caused by voriconazole have been reported gradually, such as visual impairment, hepatotoxicity, skin rash. At present, there are few reports about triazole antifungal drugs causing the increase of triglyceride and total cholesterol. Thus, the present study reported a case of chronic pulmonary aspergillosis with significantly increased blood lipids after treatment with voriconazole. In this case, the patient's total cholesterol was normal, and triglyceride was 2.64 times of the upper limit of the reference value at the time of admission. On the 30th day after oral administration of voriconazole 200mg q12h, triglyceride and total cholesterol were 4.55 times and 3.31 times of the baseline levels, respectively, with the trough concentration of voriconazole of 6.6 mu g/mL. After 28 days of voriconazole withdrawal and itraconazole administration, triglyceride decreased to 1.45 times of baseline level and total cholesterol decreased to the normal range. After another 24 days of treatment with voriconazole 200mg q12h, triglyceride increased again to 3.25 times of the baseline level and cholesterol was within the normal range. At the same time, the trough concentration of voriconazole was 3.2 mu g/mL. After 14 days of treatment with voriconazole 100mg q12h, the triglyceride level recovered to the baseline level, with the trough concentration of voriconazole of 1.5 mu g/mL. The Naranjo's rating scale was used, the final score was 10 points, indicating that the causal relationship between voriconazole and dyslipidemia was positive, which was likely to be related to the trough concentration of voriconazole.

Abstract:Objective To observe the changes in voriconazole plasma trough concentration in critical patients,and explore its optimal dosage.Methods A retrospective analysis was carried out in critical patients with voriconazole and voriconazole therapeutic drug monitoring (TDM) in intensive care unit (ICU) of the Affiliated Drum Tower Hospital of Nanjing University Medical School from December 2014 to December 2016.The patients' age,sex,dose,and other general information,voriconazole trough concentrations,influence of proton pump inhibitors (PPIs) on plasma trough concentrations,efficacy were analyzed retrospectively.Results 150 times of voriconazole plasma trough concentration in 99 patients were enrolled,and it was found that only 76.00％ (114/150) reached the target concentration (1-5.5 mg/L),13.33％ (17/150) ＜ 1 mg/L and 12.67％ (19/150) ＞ 20 mg/L.Significant difference was observed between old patients (≥60 years old) and other patients (＜ 60 years old) (P ＜ 0.05),age (P =0.000) was found to be correlated to plasma trough concentrations by multiple linear regression analysis,but sex,body weight,weighted dose,APACHE2 score,total bilirubin and albumin don,t;there was no significant difference of plasma trough concentration of voriconazole between the use of PPIs,the concentration tended to increase with pantoprazole (P =0.054),and was statistically different compared with that of esomeprazole (P =0.036);There was no significant difference in mortality between patients with targeting trongh plasma concentrations and those without (x2 =0.059,P =0.809).Conclutions Voriconazole plasma trough concentration has great variation between individuals,so constant monitoring is necessary.Age and the combination of PPIs show a relatively significant influence on the serum trough concentrations,and they should be taken into consideration in dosage to be given.

Abstract:Aims Although therapeutic drug monitoring (TDM) of voriconazole is performed in outpatients to prevent treatment failure and toxicity, whether TDM should be performed in all or only selected patients remains controversial. This study evaluated the association between voriconazole trough concentrations and clinical events. Methods We investigated the aggravation of clinical symptoms, incidence of hepatotoxicity and visual disturbances, change in co‐medications and interaction between voriconazole and co‐medications in outpatients receiving voriconazole between 2017 and 2021 in three facilities. Abnormal trough concentrations were defined as 4.0 mg/L (high group). Results A total of 141 outpatients (578 concentration measurements) met the inclusion criteria (treatment, 37 patients, 131 values; prophylaxis, 104 patients, 447 values). The percentages of patients with abnormal concentrations were 29.0% and 31.5% in the treatment and prophylaxis groups, respectively. Abnormal concentrations showed 50% of the concentrations at the first measurement in both therapies. Aggravation of clinical symptoms was most frequently observed in the low treatment group (18.2%). Adverse events were most common in the high group for both therapies (treatment, hepatotoxicity 6.3%, visual disturbance 18.8%; prophylaxis, hepatotoxicity 27.9%). No differences were found in changes to co‐medications and drug interactions. In the prophylaxis group, prescription duration in the presence of clinical events tended to be longer than in their absence (47.4 ± 23.4 days vs 39.7 ± 21.9 days, P = .1132). Conclusions We developed an algorithm based on clinical events for appropriate implementation of TDM in outpatients. However, future interventions based on this algorithm should be validated.

De Lange syndrome: report of 20 cases.

Abstract:Typus Degenerativus Amstelodamensis or Amsterdam dwarfism, a syndrome of unknown etiology characterized by mental retardation, a distinctive face, characteristic hands and feet, defective growth and other minor malformations, was first described by Cornelia de Lange in 1933. Approximately 69 cases, including nine autopsies, have been reported in the literature. In this paper we present a further 20, with illustrations of the syndrome from infancy to puberty (including de Lange's original three cases). The historical, physical, laboratory and radiographic findings of de Lange's three patients and our 20 are tabulated. Autopsy findings in one of our patients are reported and the literature is briefly reviewed.Although some observers have recently reported chromosome abnormalities in de Lange's syndrome, we feel that the diagnosis is made from the history and physical examination and that there are no definitive laboratory aids which can confirm the diagnosis. Chromosome studies in all 20 of our patients were normal and the genetic implications are discussed.

Abstract:Voriconazole (VRCZ) reportedly possesses a broad spectrum of antifungal activity against Aspergillus spp. and Candida spp., and the blood concentration of VRCZ is correlated with both the efficacy and the adverse effects of this drug. Monitoring of the blood concentration target level of VRCZ has not yet been widely adopted in the medical field, and no evidence concerning this target level has been reported. Accordingly, we used a meta-analysis to investigate the optimal blood concentration of VRCZ. Using data from 12 reports, we found that the success rate for fungal infection treatment increased significantly at VRCZ levels greater than 1.0 μg/ml when a graded cutoff value within the range of 1.0-3.0 μg/ml was used as the VRCZ trough blood concentration [odds ratio (OR) 7.23, 95% confidence interval (CI) 2.84-18.37, P < 0.0001]. Concerning the safety evaluation, the incidence of adverse neurological effects increased significantly at a cutoff value of 4.0 μg/ml when a graded cutoff value within the range of 3.0-6.0 μg/ml was used (OR 2.23, 95% CI 1.12-4.46, P = 0.02). However, in all 12 literature sources, an increasing incidence of liver dysfunction was reported at higher blood concentrations, and no accurate cutoff values were obtained. Consequently, a VRCZ trough blood concentration more than 1.0 μg/ml from the perspective of efficacy and less than 4.0 μg/ml from the perspective of safety is recommended.

Abstract:This study investigated voriconazole (VRC) unbound plasma concentration and its relationship with adverse drug reactions (ADRs) in patients with malignant hematologic disease. Plasma samples were collected from patients or spiked in vitro. A time-saving rapid equilibrium dialysis assay was used for the separation of unbound and bound VRC, following a high performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS) analysis method for drug concentration detection. Liver function and treatment details were collected from the electronic medical records of patients. Protein concentration was determined according to instructions. VRC plasma protein binding rate (PPB) in patient is significantly higher [69.5 ± 6.2%] than that in in-vitro samples, influenced by total drug concentration (Ct), plasma protein concentration, and protein type. The α1-acid glycogen (AAG) has the highest affinity with VRC. Relationship between total PPB of VRC with PPB of individual protein is not a simple addition, but a compressive combination. Unbound drug concentration (Cu) of VRC shows significant relationships with Ct, protein concentration, AST level, metabolism type of CYP2C19 and co-administration of high PPB medicines. Unbound plasma concentration of VRC shows a more sensitive relationship with ADRs than Ct.
Population Pharmacokinetics of Voriconazole and Dose Optimization in Elderly Chinese Patients.

Abstract:Voriconazole is commonly recommended as a first-line therapy for invasive aspergillosis infections. Elderly patients are susceptible to infectious diseases owing to their decreased physical function and immune system. Our study aims to establish a population pharmacokinetics model for elderly patients receiving intravenous voriconazole, and to optimize dosing protocols through a simulated approach. An accurate fit to the concentration-time profile of voriconazole was achieved by employing a 1-compartment model featuring first-order elimination. The typical clearance rate of voriconazole was found to be 3.22 L/h, with a typical volume of distribution of 194 L. The covariate analysis revealed that albumin (ALB), gamma-glutamyl transpeptidase, and direct bilirubin had significant impacts on voriconazole clearance. Additionally, body weight was found to be associated with the volume of distribution. Individualized dosing regimens were recommended for different ALB levels based on population pharmacokinetics model prediction. The proposed dosing regimens could provide a rationale for dosage individualization, improve the clinical outcomes, and minimize drug-related toxicities.
Factors Influencing Blood Concentration of Voriconazole and Therapeutic Drug Monitoring in Patients with Child–Pugh Class C Cirrhosis

Abstract:What Is Known and Objective. CYP2C19 is an important influencing factor for voriconazole trough plasma concentration (Cmin); however, it is not verified in Child–Pugh C (CP-C) cirrhosis patients, and no voriconazole dosage regimen is recommended for these patients in the package insert. This retrospective study identified CYP2C19 and other factors influencing voriconazole Cmin for CP-C cirrhosis, and obtained an appropriate method of application of voriconazole for them. Methods. A total of 66 patients with CP-C cirrhosis who accepted voriconazole therapy were involved. The voriconazole Cmin, clinical characteristics, CYP2C19 genotype, and adverse effects (AEs) were recorded and analyzed. Results. Unlike other research studies, voriconazole Cmin was not different among normal metabolizers (NMs), intermediate metabolizers (IMs), and poor metabolizers (PMs) of the CYP2C19 enzyme in CP-C cirrhosis ( P > 0.05). The maintenance dose regimen for voriconazole was the only independent influencing factor for Cmin ( P = 0.045; OR = 3.753; 95% CI, 1.029–13.694). At about 1/3 of the recommended maintenance dose, only 16.7% (8/48) had Cmin >5.5 μg/mL, 4.5% (3/48) had Cmin <1 μg/mL, and only one AE happened. There were four voriconazole-related AEs that happened in this study, and three AEs occurred (3/4, 75%) when the maintenance dose was not adjusted with therapeutic drug monitoring (TDM). What Is New and Conclusion. Voriconazole Cmin did not significantly vary according to CYP2C19 enzyme metabolization status (being an NM, IM, or PM) in CP-C cirrhosis. Reducing the maintenance dose of voriconazole to approximately 1/3 the standard maintenance dose and administering in combination with TDM in patients with CP-C cirrhosis are recommended.

Abstract:Voriconazole is a broad-spectrum triazole antifungal agent. A number of studies have revealed that the impact of C-reactive protein (CRP) on voriconazole pharmacokinetics was associated with the CYP2C19 phenotype. However, the combined effects of CYP2C19 genetic polymorphisms and inflammation on voriconazole pharmacokinetics have not been considered in previous population pharmacokinetic (PPK) studies, especially in the Chinese population. This study aimed to analyze the impact of inflammation on the pharmacokinetics of voriconazole in patients with different CYP2C19 genotypes and optimize the dosage of administration. Data were obtained retrospectively from adult patients aged ≥16 years who received voriconazole for invasive fungal infections from October 2020 to June 2023. Plasma voriconazole levels were measured via high-performance liquid chromatography coupled with tandem mass spectrometry (HPLC-MS/MS). CYP2C19 genotyping was performed using the fluorescence in situ hybridization method. A PPK model was developed using the nonlinear mixed-effect model (NONMEM). The final model was validated using bootstrap, visual predictive check (VPC), and normalized prediction distribution error (NPDE). The Monte Carlo simulation was applied to evaluate and optimize the dosing regimens. A total of 232 voriconazole steady-state trough concentrations from 167 patients were included. A one-compartment model with first order and elimination adequately described the data. The typical clearance (CL) and the volume of distribution (V) of voriconazole were 3.83 L/h and 134 L, respectively. The bioavailability was 96.5%. Covariate analysis indicated that the CL of voriconazole was substantially influenced by age, albumin, gender, CRP, and CYP2C19 genetic variations. The V of voriconazole was significantly associated with body weight. An increase in the CRP concentration significantly decreased voriconazole CL in patients with the CYP2C19 normal metabolizer (NM) and intermediate metabolizer (IM), but it had no significant effect on patients with the CYP2C19 poor metabolizer (PM). The Monte Carlo simulation based on CRP levels indicated that patients with high CRP concentrations required a decreased dose to attain the therapeutic trough concentration and avoid adverse drug reactions in NM and IM patients. These results indicate that CRP affects the pharmacokinetics of voriconazole and is associated with the CYP2C19 phenotype. Clinicians dosing voriconazole should consider the patient's CRP level, especially in CYP2C19 NMs and IMs.

Abstract:Aims: We aimed to assess the impact of extracorporeal membrane oxygenation (ECMO) on voriconazole exposure. Methods: Adult critically ill patients with or without ECMO support receiving intravenous voriconazole therapy were included in this retrospective study conducted in a tertiary referral intensive care unit. The first therapeutic drug monitoring (TDM) results of voriconazole in ECMO patients and non-ECMO patients were collected, and the prevalence of subtherapeutic concentrations was analyzed. Multivariate analyses were performed to evaluate the effect of ECMO on voriconazole exposure. Results: A total of 132 patients (including 66 patients with ECMO support) were enrolled and their respective first voriconazole trough concentrations (C min ) were recorded. The median C min of the ECMO group and the non-ECMO group was 1.9 (1.4–4.4) and 4.4 (3.2–6.9) mg/L, respectively ( p = 0.000), and the proportion of the two groups in subtherapeutic concentrations range (<2 mg/L) was 51.5% and 7.6%, respectively ( p = 0.000). Multiple linear regression analysis of voriconazole C min identified that the use of ECMO and coadministration of glucocorticoids were associated with significantly reduced concentrations, while increasing SOFA score and increasing daily dose were associated with significantly increased concentrations. The model accounted for 32.2% of the variability of voriconazole C min . Furthermore, binary logistic regression demonstrated that the use of ECMO was an independent risk factor (OR = 7.78, p = 0.012) for insufficient voriconazole exposure. Conclusion: Our findings showed that, in addition to the known drug interactions, ECMO is a significant covariable affecting voriconazole exposure. In addition, SOFA score was identified as a factor associated with increased voriconazole concentration.
