Abstract:Background and Purpose— Advanced imaging to identify tissue pathophysiology may provide more accurate prognostication than the clinical measures used currently in stroke. This study aimed to derive and validate a predictive model for functional outcome based on acute clinical and advanced imaging measures. Methods— A database of prospectively collected sub-4.5 hour patients with ischemic stroke being assessed for thrombolysis from 5 centers who had computed tomographic perfusion and computed tomographic angiography before a treatment decision was assessed. Individual variable cut points were derived from a classification and regression tree analysis. The optimal cut points for each assessment variable were then used in a backward logic regression to predict modified Rankin scale (mRS) score of 0 to 1 and 5 to 6. The variables remaining in the models were then assessed using a receiver operating characteristic curve analysis. Results— Overall, 1519 patients were included in the study, 635 in the derivation cohort and 884 in the validation cohort. The model was highly accurate at predicting mRS score of 0 to 1 in all patients considered for thrombolysis therapy (area under the curve [AUC] 0.91), those who were treated (AUC 0.88) and those with recanalization (AUC 0.89). Next, the model was highly accurate at predicting mRS score of 5 to 6 in all patients considered for thrombolysis therapy (AUC 0.91), those who were treated (0.89) and those with recanalization (AUC 0.91). The odds ratio of thrombolysed patients who met the model criteria achieving mRS score of 0 to 1 was 17.89 (4.59–36.35, P<0.001) and for mRS score of 5 to 6 was 8.23 (2.57–26.97, P<0.001). Conclusions— This study has derived and validated a highly accurate model at predicting patient outcome after ischemic stroke.
Machine-learning-based Performance Comparison of Two-Dimensional (2D) and Three-Dimensional (3D) CT Radiomics Features for Intracerebral Haemorrhage Expansion.

Abstract:AIM: To investigate the value of non-contrast CT (NCCT)-based two-dimensional (2D) radiomics features in predicting haematoma expansion (HE) after spontaneous intracerebral haemorrhage (ICH) and compare its predictive ability with the three-dimensional (3D) signature.MATERIALS AND METHODS: Three hundred and seven ICH patients who received baseline NCCT within 6 h of ictus from two stroke centres were analysed retrospectively. 2D and 3D radiomics features were extracted in the manner of one-to-one correspondence. The 2D and 3D models were generated by four different machine-learning algorithms (regularised L1 logistic regression, decision tree, support vector machine and AdaBoost), and the receiver operating characteristic (ROC) curve was used to compare their predictive performance. A robustness analysis was performed according to baseline haematoma volume.RESULTS: Each feature type of 2D and 3D modalities used for subsequent analyses had excellent consistency (mean ICC >0.9). Among the different machine-learning algorithms, pairwise comparison showed no significant difference in both the training (mean area under the ROC curve [AUC] 0.858 versus 0.802, all p>0.05) and validation datasets (mean AUC 0.725 versus 0.678, all p>0.05), and the 10-fold cross-validation evaluation yielded similar results. The AUCs of the 2D and 3D models were comparable either in the binary or tertile volume analysis (all p>0.5).CONCLUSION: NCCT-derived 2D radiomics features exhibited acceptable and similar performance to the 3D features in predicting HE, and this comparability seemed unaffected by initial haematoma volume. Predicting the 90-day prognosis of stereotactic brain hemorrhage patients by multiple machine learning using radiomic features combined with clinical features

Abstract:Hypertensive Intracerebral Hemorrhage (HICH) is one of the most common types of cerebral hemorrhage with a high mortality and disability rate. Currently, preoperative non-contrast computed tomography (NCCT) scanning-guided stereotactic hematoma removal has achieved good results in treating HICH, but some patients still have poor prognoses. This study collected relevant clinical and radiomic data by retrospectively collecting and analyzing 432 patients who underwent stereotactic hematoma removal for HICH from January 2017 to December 2020 at the Liuzhou Workers Hospital. The prognosis of patients after 90 days was judged by the modified Rankin Scale (mRS) scale and divided into the good prognosis group (mRS ≤ 3) and the poor prognosis group (mRS > 3). The 268 patients were randomly divided into training and test sets in the ratio of 8:2, with 214 patients in the training set and 54 patients in the test set. The least absolute shrinkage and selection operator (Lasso) was used to screen radiomics features. They were combining clinical features and radiomic features to build a joint prediction model of the nomogram. The AUCs of the clinical model for predicting different prognoses of patients undergoing stereotactic HICH were 0.957 and 0.922 in the training and test sets, respectively, while the AUCs of the radiomics model were 0.932 and 0.770, respectively, and the AUCs of the combined prediction model for building a nomogram were 0.987 and 0.932, respectively. Compared with a single clinical or radiological model, the nomogram constructed by fusing clinical variables and radiomic features could better identify the prognosis of HICH patients undergoing stereotactic hematoma removal after 90 days.

Predicting Intraventricular Hemorrhage Growth with a Machine Learning-Based, Radiomics-Clinical Model

Abstract:We constructed a radiomics-clinical model to predict intraventricular hemorrhage (IVH) growth after spontaneous intracerebral hematoma. The model was developed using a training cohort (N=626) and validated with an independent testing cohort (N=270). Radiomics features and clinical predictors were selected using the least absolute shrinkage and selection operator (LASSO) method and multivariate analysis. The radiomics score (Rad-score) was calculated through linear combination of selected features multiplied by their respective LASSO coefficients. The support vector machine (SVM) method was used to construct the model. IVH growth was experienced by 13.4% and 13.7% of patients in the training and testing cohorts, respectively. The Rad-score was associated with severe IVH and poor outcome. Independent predictors of IVH growth included hypercholesterolemia (odds ratio [OR], 0.12 [95%CI, 0.02-0.90]; p=0.039), baseline Graeb score (OR, 1.26 [95%CI, 1.16-1.36]; p<0.001), time to initial CT (OR, 0.70 [95%CI, 0.58-0.86]; p<0.001), international normalized ratio (OR, 4.27 [95%CI, 1.40, 13.0]; p=0.011), and Rad-score (OR, 2.3 [95%CI, 1.6-3.3]; p<0.001). In the training cohort, the model achieved an AUC of 0.78, sensitivity of 0.83, and specificity of 0.66. In the testing cohort, AUC, sensitivity, and specificity were 0.71, 0.81, and 0.64, respectively. This radiomics-clinical model thus has the potential to predict IVH growth.
CT-based Intra-thrombus and Peri-thrombus Radiomics for Prediction of Prognosis After Endovascular Thrombectomy: A Retrospective Study Across Two Centers

Abstract:Background and purpose: Complications from endovascular thrombectomy (EVT) can negatively affect clinical outcomes, making the development of a more precise and objective prediction model essential. This research aimed to assess the effectiveness of radiomic features derived from pre-surgical CT scans in predicting the prognosis post- EVT in acute ischemic stroke patients. Materials and methods: This investigation included 336 acute ischemic stroke patients from two medical centers, spanning from March 2018 to March 2024. The participants were split into a training cohort of 161 patients and a validation cohort of 175 patients. Patient outcomes were rated with the mRS: 0-2 for good, 3-6 for poor. A total of 428 radiomic features were derived from intra-thrombus and peri-thrombus regions in non-contrast CT and CT angiography images. Feature selection was conducted using a least absolute shrinkage and selection operator regression model. The efficacy of eight different supervised learning models was assessed using the area under the curve (AUC) of the receiver operating characteristic curve. Results: Among all models tested in the validation cohort, the logistic regression algorithm for combined model achieved the highest AUC (0.87, with a 95% confidence interval of 0.81 to 0.92), outperforming other algorithms. The combined use of radiomic features from both the intra-thrombus and peri-thrombus regions significantly enhanced diagnostic accuracy over models using features from a single region (0.81 vs 0.70, 0.77), highlighting the benefit of integrating data from both regions for improved prediction. Conclusions: The findings suggest that a combined radiomics model based on CT imaging serves as a potent approach to assessing the prognosis following EVT. The logistic regression model, in particular, proved to be both effective and stable, offering critical insights for the management of stroke. Predicting Outcome of Patients With Cerebral Hemorrhage Using a Computed Tomography–Based Interpretable Radiomics Model: A Multicenter Study

Abstract:Objective The aim of this study was to develop and validate an interpretable and highly generalizable multimodal radiomics model for predicting the prognosis of patients with cerebral hemorrhage. Methods This retrospective study involved 237 patients with cerebral hemorrhage from 3 medical centers, of which a training cohort of 186 patients (medical center 1) was selected and 51 patients from medical center 2 and medical center 3 were used as an external testing cohort. A total of 1762 radiomics features were extracted from nonenhanced computed tomography using Pyradiomics, and the relevant macroscopic imaging features and clinical factors were evaluated by 2 experienced radiologists. A radiomics model was established based on radiomics features using the random forest algorithm, and a radiomics-clinical model was further trained by combining radiomics features, clinical factors, and macroscopic imaging features. The performance of the models was evaluated using area under the curve (AUC), sensitivity, specificity, and calibration curves. Additionally, a novel SHAP (SHAPley Additive exPlanations) method was used to provide quantitative interpretability analysis for the optimal model. Results The radiomics-clinical model demonstrated superior predictive performance overall, with an AUC of 0.88 (95% confidence interval, 0.76–0.95; P < 0.01). Compared with the radiomics model (AUC, 0.85; 95% confidence interval, 0.72–0.94; P < 0.01), there was a 0.03 improvement in AUC. Furthermore, SHAP analysis revealed that the fusion features, rad score and clinical rad score, made significant contributions to the model's decision-making process. Conclusion Both proposed prognostic models for cerebral hemorrhage demonstrated high predictive levels, and the addition of macroscopic imaging features effectively improved the prognostic ability of the radiomics-clinical model. The radiomics-clinical model provides a higher level of predictive performance and model decision-making basis for the risk prognosis of cerebral hemorrhage.

Abstract:Background: The prognosis of aortic intramural hematoma (IMH) is unpredictable, but computed tomography angiography (CTA) plays an important role of high diagnostic performance in the initial diagnosis and during follow-up of patients. In this study, we investigated the value of a radiomics model based on aortic CTA for predicting the prognosis of patients with medically treated IMH.Method: A total of 120 patients with IMH were enrolled in this study. The follow-up duration ranged from 32 to 1,346 days (median 232 days). Progression of these patients was classified as follows: destabilization, which refers to deterioration in the aortic condition, including significant increases in the thickness of the IMH, the progression of IMH to a penetrating aortic ulcer (PAU), aortic dissection (AD), or rupture; or stabilization, which refers to an unchanged appearance or a decrease in the size or disappearance of the IMH. The patients were divided into a training cohort (n=84) and a validation cohort (n=36). Six different machine learning classifiers were applied: random forest (RF), K-nearest neighbor (KNN), Gaussian Naive Bayes, decision tree, logistic regression, and support vector machine (SVM). The clinical-radiomics combined nomogram model was established by multivariate logistic regression. The area under the receiver operating characteristic (ROC) curve (AUC) was implemented to evaluate the discrimination performance of the models. The calibration curves and Hosmer-Lemeshow test were used for evaluating model calibration. DeLong's test was performed to compare the AUC performance of models.Results: Among all of the patients, 60 patients showed destabilization and 60 patients remained stable. A total of 12 radiomic features were retained after application of the least absolute shrinkage and selection operator (LASSO). These features were used for the machine learning model construction. The SVM-radial basis function (SVM-RBF) model obtained the best performance with an AUC of 0.765 (95% CI, 0.593-0.906). In the validation cohort, the combined clinical-radiomics model [AUC =0.787; 95% confidence interval (CI), 0.619-0.923] showed a significantly higher performance than did the clinical model (AUC =0.596; 95% CI, 0.413-0.796; P=0.021) and had a similar performance to the radiomics model (AUC =0.765; 95% CI, 0.589-0.906; P=0.672).Conclusions: A quantitative nomogram based on radiomic features of CTA images can be used to predict disease progression in patients with IMH and may help improve clinical decision-making.

Abstract:This paper examines key challenges and strategies for including older people with dementia in an ethnographic study of quality of life in institutional care settings. The methods of interview and observation are described in relation to meeting four research challenges: verbal communication impairment, memory loss, decision-making capacity, and emotional disposition. A range of strategies for privileging the voice of the person with dementia is recommended which include: using different methods bespoke to each person with dementia; greater flexibility and time; preliminary meetings with the person with dementia; discussions with formal and informal carers; and research training. The researchers also conclude that the use of observation and interview are 'meaning-making occasions' which are qualitatively different but equally valuable for understanding quality of life in care settings.

Abstract:ObjectivesSpontaneous intracerebral hemorrhage remains a major cause of death and disability throughout the world. We tried to establish accurate long-term outcome prediction models for hypertensive intracerebral hemorrhage (HICH) using CT radiomics and machine learning. MethodsIn a retrospective study of 270 patients with HICH between June 2013 and June 2018, CT images and patients' 6-month outcome based on the modified Rankin Scale were collected. Hematomas on CT images were selected as volumes of interests (VOIs), and 1,029 radiomics features of the VOIs were extracted. Based on correlations with patients' outcome, radiomics features underwent dimensionality reduction analyses. Then, the support vector machine (SVM), k-nearest neighbor (KNN), logistic regression (LR), decision tree (DT), random forest (RF), and XGBoost algorithms were applied with the screened features to establish prognostic prediction models of HICH. Accuracies of all models were compared. ResultsEighteen radiomics features were screened as prognosis-associated radiomics signature of HICH based on the variance threshold, SelectKBest, and least absolute shrinkage and selection operator (LASSO) regression models. Patients were randomly allocated into training (n = 215) and validation (n = 55) sets. Accuracies of all 6 machine learning algorithms in the validation set exceeded 80%. The sensitivity, specificity, and accuracy in the validation set were 93.3%, 92.5%, and 92.7% for the RF model and 92.3%, 88.1%, and 89.1% for the XGBoost model, respectively, which were the best two among all models. ConclusionsTaking advantage of radiomics and machine learning, we established accurate prognostic prediction models of HICH. The RF model and XGBoost model returned the best accuracies.

Abstract:Background: Chronic subdural hematoma (CSDH) represents a prevalent medical condition, posing substantial challenges in postoperative management due to risks of recurrence. Such recurrences not only cause physical suffering to the patient but also add to the financial burden on the family and the health care system. Currently, prognosis determination largely depends on clinician expertise, revealing a dearth of precise prediction models in clinical settings. Objective: This study aims to use machine learning (ML) techniques for the construction of predictive models to assess the likelihood of CSDH recurrence after surgery, which leads to greater benefits for patients and the health care system. Methods: Data from 133 patients were amassed and partitioned into a training set (n=93) and a test set (n=40). Radiomics features were extracted from preoperative cranial computed tomography scans using 3D Slicer software. These features, in conjunction with clinical data and composite clinical-radiomics features, served as input variables for model development. Four distinct ML algorithms were used to build predictive models, and their performance was rigorously evaluated via accuracy, area under the curve (AUC), and recall metrics. The optimal model was identified, followed by recursive feature elimination for feature selection, leading to enhanced predictive efficacy. External validation was conducted using data sets from additional health care facilities. Results: Following rigorous experimental analysis, the support vector machine model, predicated on clinical-radiomics features, emerged as the most efficacious for predicting postoperative recurrence in patients with CSDH. Subsequent to feature selection, key variables exerting significant impact on the model were incorporated as the input set, thereby augmenting its predictive accuracy. The model demonstrated robust performance, with metrics including accuracy of 92.72%, AUC of 91.34%, and recall of 93.16%. External validation further substantiated its effectiveness, yielding an accuracy of 90.32%, AUC of 91.32%, and recall of 88.37%, affirming its clinical applicability. Conclusions: This study substantiates the feasibility and clinical relevance of an ML-based predictive model, using clinical-radiomics features, for relatively accurate prognostication of postoperative recurrence in patients with CSDH. If the model is integrated into clinical practice, it will be of great significance in enhancing the quality and efficiency of clinical decision-making processes, which can improve the accuracy of diagnosis and treatment, reduce unnecessary tests and surgeries, and reduce the waste of medical resources.

Abstract:Background: Accurate risk stratification of patients with intracerebral hemorrhage (ICH) could help refine adjuvant therapy selection and better understand the clinical course. We aimed to evaluate the value of radio-mics features from hematomal and perihematomal edema areas for prognosis prediction and to develop a model combining clinical and radiomic features for accurate outcome prediction of patients with ICH. Methods: This multicenter study enrolled patients with ICH from January 2016 to November 2021. Their out-comes at 3 months were recorded based on the modified Rankin Scale (good, 0-3; poor, 4-6). Independent clinical and radiomic risk factors for poor outcome were identified through multivariate logistic regression analysis, and predictive models were developed. Model performance and clinical utility were evaluated in both internal and external cohorts. Results: Among the 1098 ICH patients evaluated (mean age, 60 & PLUSMN; 13 years), 703 (64 %) had poor outcomes. Age, hemorrhage volume and location, and Glasgow Coma Scale (GCS) were independently associated with outcomes. The area under the receiver operating characteristic curve (AUC) of the clinical model was 0.881 in the external validation cohort. Addition of the Rad-score (combined hematoma and perihematomal edema area) improved predictive accuracy and model performance (AUC, 0.893), net reclassification improvement, 0.140 (P < 0.001), and integrated discrimination improvement, 0.050 (P < 0.001). Conclusions: The radiomics features of hematomal and perihematomal edema area have additional value in prognostic prediction; moreover, addition of radiomic features significantly improves model accuracy.

Abstract:ObjectivesEdema is a complication of gamma knife radiosurgery (GKS) in meningioma patients that leads to a variety of consequences. The aim of this study is to construct radiomics-based machine learning models to predict post-GKS edema development.MethodsIn total, 445 meningioma patients who underwent GKS in our institution were enrolled and partitioned into training and internal validation datasets (8:2). A total of 150 cases from multicenter data were included as the external validation dataset. In each case, 1132 radiomics features were extracted from each pre-treatment MRI sequence (contrast-enhanced T1WI, T2WI, and ADC maps). Nine clinical features and eight semantic features were also generated. Nineteen random survival forest (RSF) and nineteen neural network (DeepSurv) models with different combinations of radiomics, clinical, and semantic features were developed with the training dataset, and evaluated with internal and external validation. A nomogram was derived from the model achieving the highest C-index in external validation.ResultsAll the models were successfully validated on both validation datasets. The RSF model incorporating clinical, semantic, and ADC radiomics features achieved the best performance with a C-index of 0.861 (95% CI: 0.748–0.975) in internal validation, and 0.780 (95% CI: 0.673–0.887) in external validation. It stratifies high-risk and low-risk cases effectively. The nomogram based on the predicted risks provided personalized prediction with a C-index of 0.962 (95%CI: 0.951–0.973) and satisfactory calibration.ConclusionThis RSF model with a nomogram could represent a non-invasive and cost-effective tool to predict post-GKS edema risk, thus facilitating personalized decision-making in meningioma treatment.Clinical relevance statementThe RSF model with a nomogram built in this study represents a handy, non-invasive, and cost-effective tool for meningioma patients to assist in better counselling on the risks, appropriate individual treatment decisions, and customized follow-up plans.Key Points• Machine learning models were built to predict post-GKS edema in meningioma. The random survival forest model with clinical, semantic, and ADC radiomics features achieved excellent performance.• The nomogram based on the predicted risks provides personalized prediction with a C-index of 0.962 (95%CI: 0.951–0.973) and satisfactory calibration and shows the potential to assist in better counselling, appropriate treatment decisions, and customized follow-up plans.• Given the excellent performance and convenient acquisition of the conventional sequence, we envision that this non-invasive and cost-effective tool will facilitate personalized medicine in meningioma treatment.

Abstract:Purpose: This research aimed to create a machine learning model for clinical-radiomics that utilizes unenhanced computed tomography images to assess the likelihood of malignant cerebral edema (MCE) in individuals suffering from acute ischemic stroke (AIS). Methods: The research included 179 consecutive patients with AIS from two different hospitals. These patients were randomly assigned to training ( n = 143) and validation ( n = 36) sets with an 8:2 ratio. Using 3DSlicer software, the radiomics features of regions impacted by infarction were derived from unenhanced CT scans. The radiomics features linked to MCE were pinpointed through a consistency test, Student's t test and the least absolute shrinkage and selection operator (LASSO) method for selecting features. Clinical parameters associated with MCE were also identified. Subsequently, machine learning models were constructed based on clinical, radiomics, and clinical-radiomics. Ultimately, the efficacy of these models was evaluated by measuring the operating characteristics of the subjects through their area under the curve (AUCs). Results: Logistic regression (LR) was found to be the most effective machine learning algorithm, for forecasting the MCE. In the training and validation cohorts, the AUCs of clinical model were 0.836 and 0.773, respectively, for differentiating MCE patients; the AUCs of radiomics model were 0.849 and 0.818, respectively; the AUCs of clinical and radiomics model were 0.912 and 0.916, respectively. Conclusion: This model can assist in predicting MCE after acute ischemic stroke and can provide guidance for clinical treatment and prognostic assessment.

Abstract:The objective of this study was to predict hematoma expansion (HE) by radiomic models based on different machine learning methods and determine the best radiomic model through the comparison. A total of 108 patients with intracerebral hemorrhage were retrospectively evaluated. Images of baseline non-contrast computed tomography (NCCT) and follow-up NCCT scan within 24 h were retrospectively reviewed. An HE was defined as a volume increase of more than 33% or an increase greater than 12.5 mL from the volume of the baseline NCCT. Texture parameters of the baseline NCCT images were selected by the least absolute shrinkage and selection operator (LASSO) regression. We used support vector machine (SVM), decision tree (DT), conditional inference trees (CIT), random forest (RF), k‑nearest neighbors (KNN), back-propagation neural network (BPNet) and Bayes to build models. Receiver operating characteristic (ROC) analysis and decision curve analysis (DCA) was performed and compared among models. Every model had a relatively high AUC (all > 0.75), SVM and KNN had the highest AUC of 0.91. There were significant differences between SVM and CIT (Z > 2.266, p = 0.02345), KNN and CIT (Z = 2.4834, p = 0.01301), RF and CIT (Z = 2.6956, p = 0.007027), KNN and BPNet (Z = 2.0122, p = 0.0442), RF and BPNet (Z = 1.9793, p = 0.04778). There was no significant difference among SVM, DT, RF, KNN and Bayes (p > 0.05). The SVM obtained the largest net benefit when the threshold probability was less than 0.33, while KNN obtained the largest net benefit when the threshold probability was greater than 0.33. Combined with ROC and DCA, SVM and KNN performed better in all the models for predicting HE. Radiomic models based on different machine learning methods can be used to predict HE and the models generated by SVM and KNN performed best.

Abstract:Objective: The objective of this study is to develop a model to predicts the postoperative Hunt-Hess grade in patients with intracranial aneurysms by integrating radiomics and deep learning technologies, using preoperative CTA imaging data. Thereby assisting clinical decision-making and improving the assessment and prognosis of postoperative neurological function. Methods: This retrospective study encompassed 101 patients who underwent aneurysm embolization surgery. 851 radiomic features were extracted from CTA images. 512 deep learning features are extracted from last layer of ResNet50 deep convolutional neural network model. The feature screening process pipeline encompassed intraclass correlation coefficient analysis, principal component analysis, U test, spearman correlation analysis, minimum redundancy maximum relevance algorithm and Lasso regression, to identify features most correlated with postoperative Hunt-Hess grading. In the model construction phase, three distinct models were constructed: radiomics feature-based model (RSM), deep learning feature-based model (DLM), and deep learning-radiomics feature fusion model (DLRSCM). The study also calculated the radiomics score and combined it with clinical data to construct a Nomogram for predictive modeling. DLM, RSM and DLRSCM model was constructed by 9 base algorithms and 1 ensemble learning algorithm – Stacking ensemble model. Model performance was evaluated based on the area under the Receiver Operating Characteristic (ROC) curve (AUC), Matthews Correlation Coefficient (MCC), calibration curves, and decision curves analysis. Results: 5 significant radiomic feature and 4 significant deep learning features were obtained through the feature selection process. These features were utilized for model construction. Bootstrap resampling method was used for internal validation of the models. In terms of model evaluation, the DLM model, the stacking ensemble algorithm results achieved an AUC of 0.959 and MCC of 0.815. In the RSM model, the stacking ensemble model AUC was 0.935 and MCC was 0.793. The stacking ensemble model in DLRSCM outperformed others, with an AUC of 0.968 and MCC of 0.820. Results indicated that the ANN performed optimally among all base models, while the stacked ensemble learning model exhibited the highest predictive performance. Conclusion: This study demonstrates that the combination of radiomics and deep learning is an effective approach to predict the postoperative Hunt-Hess grade in patients with intracranial aneurysms. This holds significant value in the early identification of postoperative neurological complications and in enhancing clinical decision-making.

Abstract:Objectives: Preventing the expansion of perihematomal edema (PHE) represents a novel strategy for the improvement of neurological outcomes in intracerebral hemorrhage (ICH) patients. Our goal was to predict early and delayed PHE expansion using a machine learning approach. Methods: We enrolled 550 patients with spontaneous ICH to study early PHE expansion, and 389 patients to study delayed expansion. Two imaging researchers rated the shape and density of hematoma in non-contrast computed tomography (NCCT). We trained a radiological machine learning (ML) model, a radiomics ML model, and a combined ML model, using data from radiomics, traditional imaging, and clinical indicators. We then validated these models on an independent dataset by using a nested 4-fold cross-validation approach. We compared models with respect to their predictive performance, which was assessed using the receiver operating characteristic curve. Results: For both early and delayed PHE expansion, the combined ML model was most predictive (early/delayed AUC values were 0.840/0.705), followed by the radiomics ML model (0.799/0.663), the radiological ML model (0.779/0.631), and the imaging readers (reader 1: 0.668/0.565, reader 2: 0.700/0.617). Conclusion: We validated a machine learning approach with high interpretability for the prediction of early and delayed PHE expansion. This new technique may assist clinical practice for the management of neurocritical patients with ICH. Key points: • This is the first study to use artificial intelligence technology for the prediction of perihematomal edema expansion. • A combined machine learning model, trained on data from radiomics, clinical indicators, and imaging features associated with hematoma expansion, outperformed all other methods.

Abstract:In hypertensive intracerebral hemorrhage (HICH) patients, while emergency surgeries effectively reduce intracranial pressure and hematoma volume, their significant risk of causing postoperative rehemorrhage necessitates early detection and management to improve patient prognosis. This study sought to develop and validate machine learning (ML) models leveraging clinical data and noncontrast CT radiomics to pinpoint patients at risk of postoperative rehemorrhage, equipping clinicians with an early detection tool for prompt intervention. The study conducted a retrospective analysis on 609 HICH patients, dividing them into training and external verification cohorts. These patients were categorized into groups with and without postoperative rehemorrhage. Radiomics features from noncontrast CT images were extracted, standardized, and employed to create several ML models. These models underwent internal validation using both radiomics and clinical data, with the best model's feature significance assessed via the Shapley additive explanations (SHAP) method, then externally validated. In the study of 609 patients, postoperative rehemorrhage rates were similar in the training (18.8%, 80/426) and external verification (17.5%, 32/183) cohorts. Six significant noncontrast CT radiomics features were identified, with the support vector machine (SVM) model outperforming others in both internal and external validations. SHAP analysis highlighted five critical predictors of postoperative rehemorrhage risk, encompassing three radiomics features from noncontrast CT and two clinical data indicators. This study highlights the effectiveness of an SVM model combining radiomics features from noncontrast CT and clinical parameters in predicting postoperative rehemorrhage among HICH patients. This approach enables timely and effective interventions, thereby improving patient outcomes.

A comparison of anti‐inflammatory and other compounds on the spontaneously contracting pregnant rat uterus

Abstract: The use of tissue from the 17 to 21 day pregnant rat uterus was based on earlier experiments showing that locally produced prostaglandins are the mediators of the spontaneous contractions in the pregnant rat uterus. The isolated uterine tissue provides a simpler method for screening compounds for inhibition of prostaglandin synthetase activity than the biochemical assays or the rat-paw-edema technique. This preparation was studied as a screening method for new antiinflammatory compounds. A 4-5 cm length of tissue from the uterus of 17-21 day pregnant Fullinsdorf rats was suspended in Kreb's bicarbonate solution and gassed with 5% carbon dioxide in oxygen. After 30 minutes to equilibrate, regular spontaneous contractions were recorded up to 8 hours. The sensitivity of this preparation to antiinflammatory agents was 10-100 times lower than reported by some others. Substances tested included papaverine, ketamine, isoprenaline, chloropromazine, desipramine, diazepam, and procaine. Only chlorpromazine had antiinflammatory activity as indicated by the ability to prevent carageenan-induced edema of the rat paw. Low concentrations of papaverine, ketamine, and isoprenaline initially reduced the force but increased the rate of contractions while chlorpromazine, desipramine, and diazepam decreased the force without affecting the rate of contraction. The known potent antiinflammatory agents, iodomethacin, phenylbutazone, acetylsalicyclic acid, and flufenamic acid reduced both rate and force of contractions of the preparation. Contractions of the preparations were completely inhibited by high doses of all these compounds. Results suggest that although isolated pregnant rat uterine tissue is a simple technique for detecting compounds that inhibit prostaglandin synthetase activity, there is lack of specificity of the preparation. Some compounds, e.g., indomethacin, have other independent effects on smooth muscle tissue.
Prognosis of ischemic stroke predicted by machine learning based on multi-modal MRI radiomics

Abstract:Objective: Increased risk of stroke is highly associated with psychiatric disorders. We aimed to conduct the machine learning model based on multi-modal magnetic resonance imaging (MRI) radiomics predicting the prognosis of ischemic stroke. Methods: This study retrospectively analyzed 148 patients with acute ischemic stroke due to anterior circulation artery occlusion. Based on the modified Rankin Scale (mRS) score, patients were divided into good (mRS ≤ 2) and poor (mRS > 2) outcome groups. Segmentation of the infarct region was performed by manually outlining a mask of the lesion on diffusion-weighted images (DWI) using MRIcron software. The apparent diffusion coefficient (ADC), fluid decay inversion recoverage (FLAIR), susceptibility weighted imaging (SWI) and T1-weighted (T1w) images were aligned to the DWI images and the radiomic features within the lesion area were extracted for each image modality. The calculations were done using pyradiomics software and a total of 4,744 stroke-related imaging features were automatically calculated. Next, feature selection based on recursive feature elimination was used for each modality and three radiomic features were extracted from each modality plus one feature from the lesion mask, for a total of 16 radiomic features. At last, five machine learning (ML) models were trained and tested to predict stroke prognosis, calculate the received operating characteristic (ROC) curves and other parameters, evaluate the performance of the models and validate their predictive efficacy by five-fold cross-validation. Results: Sixteen radiomic features were selected to construct the ML models for prognostic classification. By five-fold cross-validation, light gradient boosting machine (LightGBM) model-based muti-modal MRI radiomic features performed best in binary prognostic classification with accuracy of 0.831, sensitivity of 0.739, specificity of 0.902, F1-score of 0.788 and an area under the curve (AUC) of 0.902. Conclusion: The ML models based on muti-modal MRI radiomics are of high value for predicting clinical outcomes in acute stroke patients.
