Abstract:Background: Previous research has shown that young people in the child protection system experience different levels of participation in decision-making on important issues. Nonetheless, researchers agree that children's participation in decision-making is likely to affect critical factors in their lives, such as educational and employment performance. Objectives: This study aimed to examine (1) to what extent the level of participation in decision-making is related to this group of young people's subsequent education and employment, and (2) how the relation varies between different dimensions of the measure of participation. Participants and setting: Participants were a sample of 1477 young Danish individuals who received out-of-home care or preventive in-home care in 2019 and who answered a questionnaire regarding their participation in decision-making. Results: This study found a positive association between overall perceived participation and entering upper secondary education, even after controlling for individual-, parental- and case-specific background characteristics. However, no association with school-leaving exams or employment was identified. Unfolding the underlying dimensions of our measure of participation showed that the influence dimension drives a positive association with upper secondary education. At the same time, a better relationship with the caseworker is positively related to leaving the labor market. Conclusions: Young people's participation in decision-making in the child protection system does not relate as strongly to their subsequent performance as anticipated. However, the measure of participation is complex. The evidence suggests that the lack of association is partly driven by non-linear associations and the causal ambiguity of the dimension concerning the relationship with the caseworker.

Abstract:A 42-years old woman has been suffering from psychomotor retardation since early childhood. She needed permanent assistance from her family for all basic activities of daily life and never developed neither language skills, nor sphincter control. She has a peculiar face and walks with small steps, without motor deficit. She also presents traits of altered behavior, smiles continuously without apparent cause, and scratches when she is upset. She has severe constipation since childhood (1 deposition/week), and requires frequent intake of laxatives. The body mass index was normal (21 kg/m2). At age of 11 she presented a picture of herpetiform dermatitis. She has suffered from epilepsy since adolescence. At first, she had frequent convulsive seizures, mixed with episodes of absence with a myoclonic component. She was also diagnosed of hypothyroidism at age 37, and has been treated since then with substitution therapy. The diagnosis of Angelman syndrome (AS) was established based on clinical features, and confirmed by fluorescence in situ hybridization (FISH) on the affected region, which showed the presence of a microdeletion in the q11-q13 region of one of the chromosomes 15. The patient came to our clinic 2.5 years ago with the chief complaint of marked food intolerance with frequent nausea and repeated episodes of vomiting and acid reflux. She was unresponsive to antiemetics or PPIs. She had a significant ferropenic anemia (Hb = 10.4 g/dL), which was refractory to oral iron preparations. A colonoscopy and an upper gastrointestinal (GI) endoscopy were normal. Duodenal biopsies showed an increase of intraepithelial lymphocytes (LIEs) (> 40%), without villous atrophy (Marsh I). A slightly increased tissue transglutaminase (3 U/ml) was detected, being DQ2-positive in one allele and DQ8-positive in both. Based on the histological, serological, and genetic findings a celiac disease (CD) was diagnosed, and the family was advised to feed the patient with a gluten-free diet (GFD). We also administered parenteral iron, once/week during 3 months, with total resolution of anemia. The follow-up examinations to date (five in two years and a half overall) show that the patient remains asymptomatic, without anemia and improved control of the hypothyroidism and epilepsy, after the first year of the GFD. Constipation has notably improved (1 deposition/2 days). Upper GI endoscopy was repeated after 2 years of the GFD, showing that the lymphocytic enteritis had resolved with normal appearance of the intestinal mucosa (Marsh 0). The follow-up laboratory tests performed during these 2 years are shown in table I. The AS is different from the one caused by abnormal paternal contribution to the same locus, known as Prader-Willi syndrome. The genetic basis for AS was described for the first time

Abstract:The United Nations Convention on the Rights of the Child emphasizes the importance of allowing children and adolescents to influence decisions that are important to them following their age and maturity. This paper explores the principles, practices, and implications around using parental versus child/adolescent consent when participating in social science research and policy development. Experiences from two studies are presented: The Confronting Obesity: Co-creating policy with youth (CO-CREATE) and the Health Behaviour in School-aged Children (HBSC) study, a World Health Organization (WHO) Collaborative Cross-National study. Although parental consent may be an important gatekeeper for protecting children and adolescents from potentially harmful research participation, it may also be considered an obstacle to the empowerment of children and adolescents in case they want to share their views and experiences directly. This paper argues that evaluation of possible harm should be left to ethics committees and that, if no harm related to the research participation processes is identified and the project has a clear perspective on collaborating with the target group, adolescents from the age of 12 years should be granted the legal capacity to give consent to participate in the research project. Collaboration with adolescents in the development of the research project is encouraged.

Abstract:Children’s participation in all matters that concern them, particularly child protection decision-making, have many positive effects on children. It is also their right granted by the United Nations Convention on the Rights of the Child. However, in child protection practice there are many obstacles to including children in decision-making processes. First, the article reviews the policy and public discourse in England, Germany, and the Netherlands regarding children’s participation in investigations into suspected child maltreatment. Second, an analysis of 1,207 case files of investigations into suspected child maltreatment unfolds the extent of children’s participation and factors associated with participation within the three countries. Although all three countries grant the right to participate in decision-making to children, documented participation in child protection decision-making is very low even when older children are considered. Children’s participation in decision-making is closely linked to caretakers’ participation in decision-making. Thus, children are almost never included in decision-making when their caretaker is not. Children’s participation is associated with a higher likelihood for individual support for children in the Netherlands and Germany. The results point to the need for research on barriers of children’s participation as well as the need to provide more resources for case workers to be able to facilitate children’s participation.

Abstract:INTRODUCTION. Evidence in child abuse cases can be scarce and is often centred around the child’s testimony. However, child testimony varies with the child’s development. Here, an overview of suspects, case decisions and court verdicts from a cohort of children is presented, stratified across children aged 0-3, 4-7, 8-11, and 12-15 years. METHODS. Children seen at the Department of Forensic Medicine, Aarhus University, Denmark, in 2001-2013 were analysed, including all case files from the police, courts and healthcare services. RESULTS. A total of 647 cases were presented. The most frequent suspect was the child’s parents. The police referred to the prosecutor in 69% of all cases, and 37% were tried in court. The lowest proportion of cases of tried cases was found among children aged 0-3 years (20%) and the highest among children aged 8-11 years (57%). Across ages, no corroborating evidence, the accused’s refusal of guilt and no case to pursue (insufficient strong evidence) were the most frequent reasons for case closure. Cases relating to children aged 0-3 years were frequently dismissed because the fault could not be placed, whereas cases relating to children aged 12-15 were frequently rejected because of lacking evidence of compulsion (non-consent). CONCLUSION. Results show variations across ages regarding children tried in court and case dismissal. A dedicated child court may be considered to ensure equal access to justice. Questioning during the forensic examination and the use of psychologists may strengthen the available evidence. FUNDING. These materials have received financial support from the Danish Victims Fund. Child Maltreatment in China: a Cross-Sectional Study of Prevalence and Attitudes among Young Adults

Abstract:BackgroundNo formal child protection system exists in China, and redress can be made only through the legal system. Consequently, all forms of child maltreatment are massively under-reported. This study aimed to investigate the prevalence of child maltreatment in China and attitudes towards physical punishment by exploring the experiences of young adults.MethodsA cross-sectional study was carried out in 1255 college students in three colleges in Zhejiang province. A self-administered questionnaire, based on Conflict Tactics Scale—Parent Child and International Society for the Prevention of Child Abuse and Neglect (ISPCAN) Child Abuse Screening Tool, was developed to measure childhood (younger than 18 years) experience of different forms of maltreatment, including physical maltreatment, psychological aggression, and sexual abuse, and attitudes towards discipline. All participants gave informed consent. The study was approved by Zhejiang University Ethics Board. Data was analysed in SPSS 22.0.FindingsParticipants were aged 17–26 years; 640 (51%) were female, 733 (59%) were from rural areas, and 506 (41%) were from urban areas, with 167 (13%) from self-reported high-income backgrounds, 824 (67%) from middle-income backgrounds, and 246 (20%) from low-income backgrounds (some participants did not provide data for residence and family income background). 1014 (81%) had experienced physical maltreatment, 1026 (82%) psychological aggression, and 170 (14%) sexual abuse. Most common forms of maltreatment by parents were hitting or punching (54%), hitting with implements (29%), threatening to spank without doing it actually (29%), insulting (22%), and forcing to stand or kneel in a humiliating or painful way (19%). 473 (38%) were forced to stand or kneel, and 406 (32%) were insulted by teachers. Among college students who experienced physical maltreatment, 763 (75%) thought that it was necessary for good discipline and thus justified. 364 (43%) of 842 thought hitting and punching were not harmful, with 161 (35%) of 464 saying that the use of implements, 137 (39%) of 350 saying that kicking, and 257 (40%) of 644 reporting that being forced to stand or kneel were not harmful. However, experience of psychological aggression was widely perceived as harmful; 544 (89%) of 614 who were insulted, 60 (78%) of 77 who were told they were not loved, 92 (80%) of 115 who wished they were dead, and 118 (67%) of 177 threatened with abandonment felt that such verbal maltreatment had caused short-term and long-term harm.InterpretationChild maltreatment is common in China. Abstract:Abstract Background Residential youth care (RYC) institutions aim to provide care and stability for vulnerable adolescents with several previous and present challenges, such as disrupted attachments, wide-ranging adverse childhood experiences, mental health problems, and poor quality of life (QoL). To the best of our knowledge, the present study is the first to provide knowledge of the associations between perceived social support and QoL and to explore the potential moderating effect of perceived social support on QoL for adolescents who have experienced maltreatment and polyvictimization. Methods All RYC institutions with adolescents between the ages 12–23 in Norway were asked to participate in the study. A total of 86 institutions housing 601 adolescents accepted the invitation, from which 400 adolescents volunteered to participate. The Child and Adolescent Psychiatric Interview was used to gather information on maltreatment histories and degree of victimization; the Kinder Lebensqualität Fragebogen was used to measure QoL through several domains (overall QoL, physical well-being, emotional well-being, and self-esteem); and the Social Support Questionnaire was used to measure perceived social support. Linear regression and independent samples t-test were used to study the associations between perceived social support and QoL as well as the potential moderating effect of perceived social support in the association between maltreatment history and QoL. Results Perceived social support was positively associated with QoL for both girls and boys, with domain-specific findings. A higher number of different types of support persons was associated with overall QoL, emotional well-being, and self-esteem for boys, but only with self-esteem for girls. Individual social support from RYC staff and friends was associated with higher QoL for girls. However, perceived social support did not moderate the association between maltreatment history and reduced QoL for either sex. Conclusions This study emphasizes the importance of maintaining social support networks for adolescents living in RYC, the crucial contribution of RYC staff in facilitating social support, and the potential value of social skills training for these vulnerable adolescents. Furthermore, a wider range of initiatives beyond social support must be carried out to increase QoL among adolescents with major maltreatment and polyvictimization experiences.

Abstract:The United Nations Convention on the Rights of the Child stipulates the need for children and young people to have a say in matters that affect them. Despite being a signatory to this international legislation, Ghanaian children's input in child protection decisions are rarely considered. As a measure to improve child participation in decision making, the possible outcomes for involving children in child protection meetings were explored. Twenty-five parents who have been in contact with a Department of Social Welfare in Ghana took part in the study. This study adapted elements of the constructivist grounded theory approach using an in-depth interview method. The interviews were audio recorded, analysed thematically and managed with NVivo software. Analysis of the interview data revealed positive outcomes of cultural integration and alteration, and children becoming responsible parents in the future. On the other hand, the study revealed escalating maltreatment and obstructing children's education as possible negative outcomes for children when involved in child protection meetings. The findings suggest that child participation in case meetings can present an opportunity to break the cultural barriers of silencing children in decision making. Also, to further protect the child, after-case follow-ups and community sensitisation programmes have to be undertaken by practitioners to mitigate the possibilities of maltreatment from parents following children's involvement in the child protection process.

Abstract:Exploring children and young people’s own understanding of experiences with abuse and maltreatment is an important part of taking their right to participation seriously. By applying a narrative theoretical framework, this paper explores and analyzes young people’s and young adults’ stories of being the target of violence and abuse as children, and their meaning making and definition of such experiences, then and now. The overall findings show the participants’ varied and nuanced perceptions of what constitutes violence were much dependent on contextual, relational, and temporal aspects. Furthermore, many participants reported psychological and emotional abuse and neglect to be the most hurtful of their experiences of maltreatment. Finally, the analysis sheds light on how responses to abuse and maltreatment change and depend on the children and young people’s definition of their situation through time and context. The paper contributes with qualitative knowledge to the social work research and practice field, on how abuse and maltreatment unfold in and affect children and young people’s lives seen from their own points of view.

Abstract:There is very little evidence whether recalling and answering questions about abuse or interpersonal violence has a positive or negative impact on participants of such research. This is an important ethical dilemma to ensure an appropriate risk-benefit ratio in research with young people is maintained. We assessed reported harms, benefits, and regrets of young adolescents who participated in a sensitive research project, and compared the harms and benefits in those who had and had not been victims and/or perpetrators of abuse or intimate partner violence. Participants were 3,264 adolescents aged 12 to 15 years in 41 public schools in the Western Cape, South Africa, who completed a survey about intimate partner violence, verbal, physical, and sexual abuse, as part of an HIV prevention cluster randomized controlled trial. The majority of participants reported research participation as beneficial (70.3%), while 27.7% reported harms and 14% regrets. Victims of abuse were more likely than non-victims to report benefits (71.9% vs. 67.1%; p = .02) and harms (31% vs. 20.9%; p < .01) and were less likely to report regret (13.1% vs. 16.7%; p = .02). Perpetrators of abuse were less likely than non-perpetrators to report benefits (67.4% vs. 72.8%; p = .01) and more likely to report harms (36.4% vs. 26.1%; p < .01) and regrets (17.4% vs. 13.3%; p = .01). Our findings suggested that research participation was more likely to have a positive rather than a negative emotional impact on young adolescents and that relatively few regretted participating. Victims and perpetrators of abuse were more likely to report benefits than harms, supporting the ethical appropriateness of ongoing research on abuse and violence. We recommend that further research is required to clarify and standardize terminology and instruments to quantify these kinds of evaluations, including measurement of the severity and intensity of reported benefits, harms and regrets, and the longer term impact of participation in sensitive research.

Abstract:This article reviews and compares the use of confinement and other restrictive measures against young people under 18 in child welfare and/or the criminal justice systems in Denmark, Sweden, Finland, and Norway. Young people are confined for a variety of reasons, including protection, care, treatment, and punishment. However, confinement of young people is a contested issue because it can be viewed as necessary but also potentially harmful. Comparison of legislation and practices reveals that while there are some similarities in the service provisions for young people, there are also significant disparities among the four countries regarding the organization, function, and frequency of the use of confinement and restrictive measures. While Denmark and Sweden use secure welfare institutions, Finland and Norway apply other restrictive measures. Despite the differences in approaches to confinement in the Nordic countries, the use of confinement is guided by the principle of the child's best interest, and the child welfare system is the main frame for confinement and intervention. The article discusses these disparate practices from the perspective of children's rights and identifies new avenues for research and practice.

Abstract:Scandinavian Journal of Public Health, Ahead of Print. Aims: This systematic review aims to identify and describe how children of parents with mental illness, substance dependence, or severe physical illness/injury, experience and practise their everyday life. Methods: The review followed the four stepwise recommendations of Harden and colleagues when including quantitative and qualitative studies on peoples' experiences and views. In all, 23 studies with data from Norway (2010–2022) have been included. Brown and Clark's thematic analysis was applied. Results: Three themes were constructed from the reviewed articles: (a) Children practice their relational agency by actively doing practical tasks, occasionally jobs to maintain family economy, and organising fun activities with the ill parent. (b) Emotional ambivalence when their own needs were set aside in favour of the parents. They loved their parents but also felt guilt, anger, disappointment, shame, fear of inheriting the illness and longed for a 'normal' everyday life. (c) Supportive contextual factors were, for example, at least one significant adult recognising them, participating in leisure activities, socialising with friends, and talking with other peers who shared similar experiences as next of kin. Obstructive factors were lack of information and recognition as well as silence and lack of dialogue within the family and/or health professional. Conclusions: There is a strong need for more knowledge and competence on the situation and needs of these children when it comes to professionals, parents and the public. Public health initiatives are needed to honour their agency and recognise their contributions in present time to prevent psychosocial problems later in life.

Abstract:Background Research indicates that prospective and retrospective measures of maltreatment often identify different groups of individuals, yet the reasons for these discrepancies remain understudied. Objective This study explores potential sources of disagreement between prospective and retrospective measures of maltreatment, utilising qualitative data from interviewers' notes. Participants and setting The Environmental Risk Longitudinal Twin Study includes 2232 children followed from ages 5–18. Prospective measures relied on caregiver interviews and researcher observations from ages 5–12, while retrospective measures involved self-reports via the Childhood Trauma Questionnaire at age 18. Methods We purposively sampled written interviewer notes from 36 participants who reported more types of maltreatment retrospectively than prospectively ('new reports' group) and 31 participants who reported fewer types retrospectively than prospectively ('omitted reports' group). We conducted a framework analysis of the notes, comparing between the two groups to explore explanations for measurement disagreement. Results Three categories of themes emerged related to measurement discrepancies: challenges with prospective measures, highlighting reasons given by the 'new reports' group for why maltreatment went undetected or was not adequately responded to prospectively; challenges with retrospective measures that highlight difficulties with openness and accuracy of self-reports; and differences in appraisals of violence or distressing childhood experiences between the two groups that might lead to new or omitted retrospective reports. Conclusions Our findings underscore potential mechanisms underlying the disagreement between prospective and retrospective measures, contributing to better understanding of these different constructs and more balanced interpretation of related findings.

Four- to six-year-old children’s experiences of participating in different physical environments and activities in early childhood education and care institutions in Norway

Abstract:Children's well-being in early childhood education and care (ECEC) is associated with their participation in both planning and assessing ECEC activities. The aim of the study presented here was therefore to explore children's experiences of participating in different physical environments and activities in ECEC institutions. Quantitative data were collected from structured conversations with 3693 4–6-year-old children using the Norwegian ECEC Well-Being Monitor from 2014 to 2019. Among the results, children's experiences of participating in ECEC activities were diverse and depended upon the physical environment, although most children experienced higher degrees of participation when outdoors instead of indoors. Furthermore, going hiking and circle time are activities the majority of children have to attend, even if they do not want to. Increased awareness about how children experience opportunities for participating in ECEC activities is important for ECEC institutions and may influence pedagogical thinking and planning in different environments and activities.

Abstract:Abstract The attention to children’s participation has increased dramatically during the last decade. Compared to the literature in the educational field, the literature on children’s participation in the social and health fields seems to be developing rapidly. The aim of this article is to address the importance of educational psychology service ( eps ) counsellors regarding special educational needs ( sen ) students’ rights to participate in educational decision-making. This article presents an in-depth study of seven Norwegian eps counsellors’ views about children’s participation in educational decision-making. Procedures for a qualitative inductive thematic analysis were followed to reveal four key themes, namely: parents as key persons; children’s presence on teams as an intervention; the importance of children’s age for participation; and the gap between expert assessments and individual education plans ( iep s). The results, which are discussed in relation to Lundy’s (2007) model of participation and other relevant literature, show that there should be more knowledge and interventions to ensure children’s participation in educational decision-making in both research and practice. The results also indicate that structures and procedures in special education need to be considered and further developed from a participation perspective.

Abstract:Bone marrow‐derived stem/progenitor cells (CD34+ progenitor cells) were demonstrated to play an important role in the regeneration of damaged brain tissue. Our aim was to study the influence of CD34+ progenitor cells in the outcome of intracerebral hemorrhage (ICH). Thirty‐two patients with primary ICH (64.0% male, mean age 67.1 ± 10.8 years) were prospectively included in the study within 12 hr of symptom onset. The main outcome variable was good functional outcome at 3 months (modified Rankin scale ≤2). Circulating CD34+ progenitor cell levels were measured by flow cytometry at admission and at 7 ± 1 days, and serum levels of growth factors (determined by ELISA) were measured at admission and at 24 and 72 hr. Circulating levels of CD34+ progenitor cells at day 7 were independently associated with good functional outcome at 3 months (OR 1.17, CI95% 1.06–1.39, P = 0.012). On the other hand, CD34+ progenitor cells at day 7 were negatively correlated with residual cavity volume at 3 months (r = –0.607, P = 0.001). Serum levels of vascular endothelial growth factor (r = 0.386), angiopoietin 1 (r = 0.518), brain‐derived neurotrophic factor (r = 0.484), and stromal cell‐derived factor‐1α (r = 0.837) but not granulocyte‐colony stimulating factor (r = –0.038) at 72 hr showed a strong correlation with CD34+ progenitor cell levels at day 7. These findings suggest that CD34+ progenitor cells may participate in the functional recovery of ICH patients. © 2011 Wiley‐Liss, Inc.
