Abstract:Fundus diseases can cause irreversible vision loss in both eyes if not diagnosed and treated immediately. Due to the complexity of fundus diseases, the probability of fundus images containing two or more diseases is extremely high, while existing deep learning-based fundus image classification algorithms have low diagnostic accuracy in multi-labeled fundus images. In this paper, a multi-label classification of fundus disease with binocular fundus images is presented, using a neural network algorithm model based on attention mechanisms and feature fusion. The algorithm highlights detailed features in binocular fundus images, and then feeds them into a ResNet50 network with attention mechanisms to extract fundus image lesion features. The model obtains global features of binocular images through feature fusion and uses Softmax to classify multi-label fundus images. The ODIR binocular fundus image dataset was used to evaluate the network classification performance and conduct ablation experiments. The model's backend is the Tensorflow framework. Through experiments on the test images, this method achieved accuracy, precision, recall, and F1 values of 94.23%, 99.09%, 99.23%, and 99.16%, respectively.

Abstract:Retinal disorders are a major cause of irreversible vision loss, which can be mitigated through accurate and early diagnosis. Conventionally, fundus images are used as the gold diagnosis standard in detecting retinal diseases. In recent years, more and more researchers have employed deep learning methods for diagnosing ophthalmic diseases using fundus photography datasets. Among the studies, most of them focus on diagnosing a single disease in fundus images, making it still challenging for the diagnosis of multiple diseases. In this paper, we propose a framework that combines ResNet and Transformer for multi-label classification of retinal disease. This model employs ResNet to extract image features, utilizes Transformer to capture global information, and enhances the relationships between categories through learnable label embedding. On the publicly available Ocular Disease Intelligent Recognition (ODIR-5 k) dataset, the proposed method achieves a mean average precision of 92.86%, an area under the curve (AUC) of 97.27%, and a recall of 90.62%, which outperforms other state-of-the-art approaches for the multi-label classification. The proposed method represents a significant advancement in the field of retinal disease diagnosis, offering a more accurate, efficient, and comprehensive model for the detection of multiple retinal conditions.

Multi-classification of fundus diseases based on DSRA-CNN

Abstract:There are many kinds of fundus diseases, and early diagnosis is the key to prevent severe visual impairment. In this paper, we propose a deep learning model to automatically detect multiple fundus diseases which includes three parts. The first part is the pre-processing of the data including data screening, black border cropping, data augmentation, and normalization. The second part is a feature extraction network named DSRA-CNN based on Xception architecture, which combines the function blocks of DS block, DSR block, and SE block, among which DS block is composed of depthwise separable convolutions to reduce the amount of calculation, DSR block is composed of DS blocks and residual connections to strengthen the utilization of image features, SE block is used to screen the characteristic information. Lastly, based on the extracted features, we design a classifier to realize the classification of eight different fundus diseases. Experiments are performed on the ODIR dataset and the results of the accuracy, precision, F1 value, and kappa score of the DSRA-CNN network proposed in this paper are respectively 87.90%, 88.50%, 88.16%, and 86.17%. Compared with the original Xception network and existing advanced Convolutional Neural Networks(CNNs), our proposed network performs better.

The application and value evaluation of assisted diagnosis system for five fundus lesion based on artificial intelligence combined with optical coherence tomography

Abstract:Objective:To establish an artificial intelligence robot-assisted diagnosis system for fundus diseases based on deep learning optical coherence tomography (OCT) and evaluate its application value.Methods:Diagnostic test studies. From 2016 to 2019, 25 000 OCT images of 25 000 patients treated at the Eye Center of the Second Affiliated Hospital of Zhejiang University School of Medicine were used as training sets and validation sets for the fundus intelligent assisted diagnosis system. Among them, macular epiretinal membrane (MERM), macular edema, macular hole, choroidal neovascularization (CNV), and age-related macular degeneration (AMD) were 5 000 sheets each. The training set and the verification set are 18 124 and 6 876 sheets, respectively. Through the transfer learning Attention ResNet structure algorithm, the OCT image was characterized by lesion identification, the disease feature was extracted by a specific procedure, and the given image was distinguished from other types of disease according to the statistical characteristics of the target lesion. The model algorithms of MERM, macular edema, macular hole, CNV and AMD were initially formed, and the fundus intelligent auxiliary diagnosis system of five models was established. The performance of each model-assisted diagnosis in the fundus intelligent auxiliary diagnostic system was evaluated by applying the subject working characteristic curve, area under the curve (AUC), sensitivity, and specificity.Results:With the intelligent auxiliary diagnosis system, the diagnostic sensitivity of the MERM was 93.5%, the specificity was 99.23%, and AUC was 0.983 7; the diagnostic sensitivity of macular edema was 99.02%, the specificity was 98.17%, and AUC was 0.994 6; the diagnostic sensitivity of macular hole was 98.91%, the specificity was 99.91%, AUC was 0.996 2; the diagnostic sensitivity of CNV was 97.54%, the specificity was 94.71%, AUC was 0.987 5; the diagnostic sensitivity of AMD was 95.12%, the specificity was 97.09%, AUC was 0.985 3.Conclusions:The artificial intelligence robot-assisted diagnosis system for fundus diseases based on deep learning for OCT images has accurate and efficient diagnostic performance for assisting the diagnosis of MERM, macular edema, macular hole, CNV, and AMD.

Abstract:Deep learning and computer vision (CV) can easily recognize pictures that the human doctor would have to spend a lot of time and labor to process. Thus, they have a wide range of applications in the field of ophthalmology, which is highly dependent on fundus images. However, training a robust and highly accurate model requires a large number of labeled high-quality images, which is often difficult for a single institution to provide. A natural solution is to use data from multiple medical centers to train the model. But due to privacy protection, data heterogeneity, and other problems, these data cannot be shared directly. Based on a real case of ophthalmic disease diagnosis with multiple data sources, this paper proposes solutions to the heterogeneity of data amount and features between datasets, establishes a heterogeneous federated learning framework, and applies federated learning to the diagnosis of multiple types of ophthalmic diseases for the first time. The experimental results show that federated learning can greatly improve the diagnostic accuracy of clients with less data, and the accuracy of clients with more data can also be slightly improved. The ablation experiment has demonstrated the effectiveness of the proposed method for handling data heterogeneity. Our research shows that through heterogeneous federated learning, we can use multi-source, non-independent-identical data to build accurate disease diagnosis models, and make clients who cannot obtain effective models get a practical level disease diagnosis model.

Abstract:Early detection and treatment of retinal disorders are critical for avoiding irreversible visual impairment. Given that patients in the clinical setting may have various types of retinal illness, the development of multi-label fundus disease detection models capable of screening for multiple diseases is more in line with clinical needs. This article presented a composite model based on hybrid graph convolution for patient-level multi-label fundus illness identification. The composite model comprised a backbone module, a hybrid graph convolution module, and a classifier module. This article established the relationship between labels via graph convolution and then employed a self-attention mechanism to design a hybrid graph convolution structure. The backbone module extracted features using EfficientNet-B4, whereas the classifier module output multi-label using LightGBM. Additionally, this work investigated the input pattern of binocular images and the influence of label correlation on the model’s identification performance. The proposed model MCGL-Net outperformed all other state-of-the-art methods on the publicly available ODIR dataset, with F1 reaching 91.60% on the test set. Ablation experiments were also performed in this paper. Experiments showed that the idea of hybrid graph convolutional structure and composite model designed in this paper promotes the model performance under any backbone CNN. The adoption of hybrid graph convolution can increase the F1 by 2.39% in trials using EfficientNet-B4 as the backbone. The composite model had a higher F1 index by 5.42% than the single EfficientNet-B4 model.

Abstract:Color fundus photographs enable the observation of numerous critical biomarkers and early-onset lesions associated with illnesses. Due to its non-invasive and cost-effective nature, this approach can be used for large-scale screening of fundus disorders. In recent years, most applications of data-driven deep learning approaches to fundus illnesses have been based on color fundus photographs. However, current screening methods for fundus illnesses are mainly focused on identifying particular diseases. The majority of published models for multi-disease recognition are ensembles of several binary classification networks, and an ensemble network consumes more resources than a single network. Additionally, many fundus illness recognition models make direct use of established networks for natural picture processing without performing structural optimization, which may reduce disease classification accuracy. To address these issues, we optimized the network structure to build a single multi-label fundus disease recognition model. Specifically, we began by selecting EfficientNet-B4 as a backbone network, then modified the output layer to create a multi-label recognition model. Based on this model, the spatial attention structure was extended to improve the network’s feature extraction capability. The focal loss was used to increase classification accuracy by mitigating the training effects of data imbalance. Our proposed model significantly improves recognition performance on a publicly available dataset compared to other baseline networks. When spatial attention and focal loss function are introduced into the backbone model, the baseline values of all relevant evaluation indicators are improved without significant increase in computational cost. Additionally, this article provides two error correction strategies for multi-label classification issues: mutual exclusion and super label space. When a more robust error correction strategy is employed with the model, F1 increases by 2.86% in comparison to the original performance of EfficientNet-B4.

Abstract:Expanded use of nurse practitioners (NPs) and physician assistants (PAs) is a potential solution to workforce issues, but little is known about how NPs and PAs can best be used. Our study examines whether medical and social complexity of patients is associated with whether their primary care provider (PCP) type is a physician, NP, or PA. In this national retrospective cohort study, we use 2012-2013 national Veterans Administration (VA) electronic health record data from 374 223 veterans to examine whether PCP type is associated with patient, clinic, and state-level factors representing medical and social complexity, adjusting for all variables simultaneously using a generalized logit model. Results indicate that patients with physician PCPs are modestly more medically complex than those with NP or PA PCPs. For the group having a Diagnostic Cost Group (DCG) score >2.0 compared with the group having DCG <0.5, odds of having an NP or a PA were lower than for having a physician PCP (NP odds ratio [OR] = 0.83, 95% confidence interval [CI]: 0.79-0.88; PA OR = 0.85, CI: 0.80-0.89). Social complexity is not consistently associated with PCP type. Overall, we found minor differences in provider type assignment. This study improves on previous work by using a large national dataset that accurately ascribes the work of NPs and PAs, analyzing at the patient level, analyzing NPs and PAs separately, and addressing social as well as medical complexity. This is a requisite step toward studies that compare patient outcomes by provider type.

Abstract:Purpose: We aim to present effective and computer aided diagnostics in the field of ophthalmology and improve eye health. This study aims to create an automated deep learning based system for categorizing fundus images into three classes: normal, macular degeneration and tessellated fundus for the timely recognition and treatment of diabetic retinopathy and other diseases. Methods: A total of 1,032 fundus images were collected from 516 patients using fundus camera from Health Management Center, Shenzhen University General Hospital Shenzhen University, Shenzhen 518055, Guangdong, China. Then, Inception V3 and ResNet-50 deep learning models are used to classify fundus images into three classes, Normal, Macular degeneration and tessellated fundus for the timely recognition and treatment of fundus diseases. Results: The experimental results show that the effect of model recognition is the best when the Adam is used as optimizer method, the number of iterations is 150, and 0.00 as the learning rate. According to our proposed approach we, achieved the highest accuracy of 93.81% and 91.76% by using ResNet-50 and Inception V3 after fine-tuned and adjusted hyper parameters according to our classification problem. Conclusion: Our research provides a reference to the clinical diagnosis or screening for diabetic retinopathy and other eye diseases. Our suggested computer aided diagnostics framework will prevent incorrect diagnoses caused by the low image quality and individual experience, and other factors. In future implementations, the ophthalmologists can implement more advanced learning algorithms to improve the accuracy of diagnosis.

Abstract:<p>Early diagnosis and timely treatment of ocular diseases are vital to prevent irreversible vision loss. Color fundus photography is an effective and economic tool for fundus screening. Since few symptoms are visible in the early disease stages, automatic and robust diagnosing algorithms according to color fundus photographs are in urgent need. Existing studies concentrate on image-level diagnoses treating the eyes independently without utilizing the useful correlation information between the left and right eyes. Besides, they commonly target only one or several ocular disease categories at a time. Considering the importance of both patient-level diagnosis correlating bilateral eyes and multi-label disease classification, we propose a patient-level multi-label ocular disease classification model based on convolutional neural networks. Specifically, a dense correlation network (DCNet) is designed to tackle the problem. DCNet consists of three major modules, a backbone CNN for feature extraction, a spatial correlation module for feature correlation, and a classifier for classification score generation. The backbone CNN extracts two sets of features from the left and right color fundus photographs, respectively. Subsequently, the spatial correlation module captures the pixel-wise correlations between the two feature sets. Then, the processed features are fused to get a patient-level representation. The final disease classification is conducted with the patient-level representation. Adopting a multi-label soft margin loss, the effectiveness of the proposed model is evaluated on a publicly available dataset, and the classification performance is improved with a large margin compared with multiple baseline methods.</p>

Abstract:In clinical practice, several disorders, including as diabetes and hypertension, necessitate a full diagnosis using ophthalmology. However, in many impoverished areas, the large number of patients prohibits early screening and treatment. Therefore, we proposes a intelligent health diagnosis system based on binocular fundus image pairs to replace artificial ophthalmologists for the detection of 8 diseases. First, in order to improve the accuracy of the diagnostic system, this work collects a large number of public fundus image datasets for pre-training, and transfers the model ability to the target task by transfer learning. Then, this paper also proposes multi-scale dilated convolutions to fuse binocular features. In this way, real manual detection scenarios can be simulated to realize binocular intelligent diagnosis. Four alternative Convolutional Neural Network (CNN) architectures are chosen as the backbone of this study and tested on the multi-label classification benchmark to validate its effectiveness. Extensive experimental comparisons show that the proposed modifications improve the performance of all CNN architectures and attain state-of-the-art outcomes.

Abstract:In ophthalmology, early fundus screening is an economic and effective way to prevent blindness caused by ophthalmic diseases. Clinically, due to the lack of medical resources, manual diagnosis is time-consuming and may delay the condition. With the development of deep learning, some researches on ophthalmic diseases have achieved good results, however, most of them are just based on one disease. During fundus screening, ophthalmologists usually give diagnoses of multi-disease on binocular fundus image, so we release a dataset with 8 diseases to meet the real medical scene, which contains 10,000 fundus images from both eyes of 5,000 patients. We did some benchmark experiments on it through some state-of-the-art deep neural networks. We found simply increasing the scale of network cannot bring good results for multi-disease classification, and a well-structured feature fusion method combines characteristics of multi-disease is needed. Through this work, we hope to advance the research of related fields.
Deep Learning for the Detection of Multiple Fundus Diseases Using Ultra-widefield Images

Abstract:Introduction: To design and evaluate a deep learning model based on ultra-widefield images (UWFIs) that can detect several common fundus diseases. Methods: Based on 4574 UWFIs, a deep learning model was trained and validated that can identify normal fundus and eight common fundus diseases, namely referable diabetic retinopathy, retinal vein occlusion, pathologic myopia, retinal detachment, retinitis pigmentosa, age-related macular degeneration, vitreous opacity, and optic neuropathy. The model was tested on three test sets with data volumes of 465, 979, and 525. The performance of the three deep learning networks, EfficientNet-B7, DenseNet, and ResNet-101, was evaluated on the internal test set. Additionally, we compared the performance of the deep learning model with that of doctors in a tertiary referral hospital. Results: Compared to the other two deep learning models, EfficientNet-B7 achieved the best performance. The area under the receiver operating characteristic curves of the EfficientNet-B7 model on the internal test set, external test set A and external test set B were 0.9708 (0.8772, 0.9849) to 1.0000 (1.0000, 1.0000), 0.9683 (0.8829, 0.9770) to 1.0000 (0.9975, 1.0000), and 0.8919 (0.7150, 0.9055) to 0.9977 (0.9165, 1.0000), respectively. On a data set of 100 images, the total accuracy of the deep learning model was 93.00%, the average accuracy of three ophthalmologists who had been working for 2 years and three ophthalmologists who had been working in fundus imaging for more than 5 years was 88.00% and 94.00%, respectively. Conclusion: High performance was achieved on all three test sets using our UWFI multidisease classification model with a small sample size and fast model inference. The performance of the artificial intelligence model was comparable to that of a physician with 2-5 years of experience in fundus diseases at a tertiary referral hospital. The model is expected to be used as an effective aid for fundus disease screening.

Abstract:The invention relates to a disease classification method based on a fundus image. The method comprises the following steps: step 1, carrying out data enhancement on an input image by using an enhancement strategy based on a population algorithm; step 2, carrying out feature extraction on the image after data enhancement by utilizing a double-path basic model so as to obtain features of the extracted image; 3, optimizing the features of the extracted image by using a multi-gradient descent algorithm; and 4, training is carried out in combination with multiple loss functions. According to the invention, picture features of two eyes can be extracted at the same time for training, and the disease classification precision is improved.
Antihistone antibodies in rheumatoid arthritis and Felty's syndrome.

Abstract:Although antihistone antibodies are frequently detected in patients with systemic lupus erythematosus, they appear to be uncommon or present only at low levels in rheumatoid arthritis (RA) patients. We used an enzyme-linked immunosorbent assay to study sera from 42 patients with RA, and we confirmed that antihistone antibodies mainly occurred at low levels in these patients. In contrast, 83% of 24 patients with Felty's syndrome had antihistone antibodies, often at high levels. Thus, antihistone antibodies may be a common autoimmune abnormality in patients with Felty's syndrome but not in other patients with RA. In addition, we demonstrated that some rheumatoid factors have cross-reactive antihistone antibody activity.
Combining convolutional neural networks and self-attention for fundus diseases identification

Abstract:Early detection of lesions is of great significance for treating fundus diseases. Fundus photography is an effective and convenient screening technique by which common fundus diseases can be detected. In this study, we use color fundus images to distinguish among multiple fundus diseases. Existing research on fundus disease classification has achieved some success through deep learning techniques, but there is still much room for improvement in model evaluation metrics using only deep convolutional neural network (CNN) architectures with limited global modeling ability; the simultaneous diagnosis of multiple fundus diseases still faces great challenges. Therefore, given that the self-attention (SA) model with a global receptive field may have robust global-level feature modeling ability, we propose a multistage fundus image classification model MBSaNet which combines CNN and SA mechanism. The convolution block extracts the local information of the fundus image, and the SA module further captures the complex relationships between different spatial positions, thereby directly detecting one or more fundus diseases in retinal fundus image. In the initial stage of feature extraction, we propose a multiscale feature fusion stem, which uses convolutional kernels of different scales to extract low-level features of the input image and fuse them to improve recognition accuracy. The training and testing were performed based on the ODIR-5k dataset. The experimental results show that MBSaNet achieves state-of-the-art performance with fewer parameters. The wide range of diseases and different fundus image collection conditions confirmed the applicability of MBSaNet.

Abstract:Some common physical diseases, such as diabetes and hypertension, require comprehensive diagnosis with ophthalmology in clinical practice. However, the huge number of patients has led to insufficient medical supply in many underdeveloped areas. Therefore, this work proposes a novel deep learning framework based on binocular fundus image pairs for the detection of 8 diseases related to the eyes and body. For the first time, the framework improves its basic capabilities by pre-training external fundus datasets and then uses the Multi-scale Dilated Convolution (MDC) module to fuse binocular features to realize the binocular diagnosis. To verify the effectiveness of the method, 4 different convolutional neural network (CNN) architectures are selected as the backbone and tested on the multi-label classification benchmark. Through extensive experimental comparisons, it is found that the innovations proposed in this paper improve the performance of all CNN architectures and achieve state-of-the-art results in the field.
Discriminative kernel convolution network for multi-label ophthalmic disease detection on imbalanced fundus image dataset

Abstract:It is feasible to recognize the presence and seriousness of eye disease by investigating the progressions in retinal biological structures. Fundus examination is a diagnostic procedure to examine the biological structure and anomalies present in the eye. Ophthalmic diseases like glaucoma, diabetic retinopathy, and cataracts are the main cause of visual impairment worldwide. Ocular Disease Intelligent Recognition (ODIR-5K) is a benchmark structured fundus image dataset utilized by researchers for multi-label multi-disease classification of fundus images. This work presents a Discriminative Kernel Convolution Network (DKCNet), which explores discriminative region-wise features without adding extra computational cost. DKCNet is composed of an attention block followed by a Squeeze-and-Excitation (SE) block. The attention block takes features from the backbone network and generates discriminative feature attention maps. The SE block takes the discriminative feature maps and improves channel interdependencies. Better performance of DKCNet is observed with InceptionResnet backbone network for multi-label classification of ODIR-5K fundus images with 96.08 AUC, 94.28 F1-score, and 0.81 kappa score. The proposed method splits the common target label for an eye pair based on the diagnostic keyword. Based on these labels, over-sampling and/or under-sampling are done to resolve the class imbalance. To check the bias of the proposed model towards training data, the model trained on the ODIR dataset is tested on three publicly available benchmark datasets. It is observed that the proposed DKCNet gives good performance on completely unseen fundus images also.

Abstract:Deep neural networks (DNNs) have been widely applied in the medical image community, contributing to automatic ophthalmic screening systems for some common diseases. However, the incidence of fundus diseases patterns exhibits a typical long-tailed distribution. In clinic, a small number of common fundus diseases have sufficient observed cases for large-scale analysis while most of the fundus diseases are infrequent. For these rare diseases with extremely low-data regimes, it is challenging to train DNNs to realize automatic diagnosis. In this work, we develop an automatic diagnosis system for rare fundus diseases, based on the meta-learning framework. The system incorporates a co-regularization loss and the ensemble-learning strategy into the meta-learning framework, fully leveraging the advantage of multi-scale hierarchical feature embedding. We initially conduct comparative experiments on our newly-constructed lightweight multi-disease fundus images dataset for the few-shot recognition task (namely, FundusData-FS). Moreover, we verify the cross-domain transferability from miniImageNet to FundusData-FS, and further confirm our method's good repeatability. Rigorous experiments demonstrate that our method can detect rare fundus diseases, and is superior to the state-of-the-art methods. These investigations demonstrate that the potential of our method for the real clinical practice is promising.

Self-Supervised Cross-Level Consistency Learning for Fundus Image Classification

Abstract:The rapid development of intelligent systems for eye disease diagnosis decreases the risk of people suffering from vision impairment. However, the superior discrimination ability of existing retinal disease diagnosis methods heavily relies on the large-scale high-quality annotations. In this work, we adapt the self-supervised technique for fundus image classification with the merits of bypassing the over-dependence of labeled data. Unlike most current self-supervised approaches, which only learn global pre-text representations from view-level, our method further incorporates the region-level representations into the learning process, since the pathological changes in fundus images are usually subtle and scattered. Specifically, we propose a novel self-supervised cross-level consistency learning scheme (S2C2L), which leverages both view-level and region-level representations of a vision Transformer to improve the robustness of extracted self-supervised representation. A diagnosis perception module (DPM) is constructed to enhance the activation of local pathological regions from both region and view levels, and a cross-level consistency loss is dedicated to align the representations from both levels. Extensive experiments on iChallenge-AMD, LAG and APTOS2019 datasets validate the state-of-the-art performance of our method for three common eye diseases.
