Abstract:Introduction Transthyretin Amyloid cardiomyopathy (ATTR-CM) usually presents as heart failure with preserved ejection fraction (HFpEF). Its diagnosis has a significant clinical impact, as specific treatment is currently available. Purpose To assess the prevalence of ATTR-CM in patients hospitalised for HFpEF and increased septal thickness in our institution of general community hospital in Latin America (LATAM). Methods Cross-sectional study. Patients hospitalised for HFpEF (>50%) and septal thickness ≥ 12 mm during the period from 8/2019 to 1/2023 were prospectively included. Patients with pacemaker, significant left valve disease and end-stage renal failure (creatinine clearance < 30 ml/min/BSA) were excluded. A pyrophosphate bone scintigraphy (PYP) was planned to be performed in order to assess ATTR-CM. The prevalence of ATTR-CM and its 95% confidence interval were calculated. Results A PYP was performed in 59/82 patients hospitalised for HFpEF. Median age was 85 [IQR 78-88] years, and 54% were women. At admission, 61% (n=36) had atrial fibrillation and the median NT Pro-Bnp was 3536 [1700-7748] pg/mL. The mean left ventricle ejection fraction was 57% (+/- 5%). The prevalence of ATTR-CM diagnosed by PYP was 19% (95% CI 9.7-30.1, n=11). There were no significant clinical differences between patients who performed a PYP and did not. Patients with ATTR-CM had lower blood pressure and higher septal thickness, NT Pro-Bnp and troponin levels compared to patients without ATTR-CM (TABLE). Conclusion In patients admitted for HFpEF and increased septal thickness, the diagnosis of ATTR-CM was relatively common (one of five had ATTR-CM) in our region (LATAM). We believe that systemic assessment with PYP should be performed in patients hospitalised with HFpEF and increased wall thickness.

Abstract:Introduction Transthyretin amyloid cardiomyopathy (ATTR-CM) is an underdiagnosed cause of AHF that benefits from a specific approach. The aim was to determine the prevalence of ATTR-CM among patients hospitalized for AHF. Methods A prospective study was conducted on consecutive patients aged 60 or older admitted for acute AHF without cardiogenic shock. Results The study included 103 patients, a total of 16 patients (15.5 %) were compatible with ATTR-CM. The ATTR-CM group showed a higher septal wall thickness (18.1 mm vs. 11.8 mm; P = 0.001), lower systolic excursion of the tricuspid annular plane (15 mm vs. 18.3 mm, P = 0.014), and S wave of the right ventricle (8 cm/s vs. 9.2 cm/s P=0.032). Conclusion ATTR-CM is an underdiagnosed condition, there are some variables associated with its diagnosis. The coexistence with other comorbidities causing AHF, highlights the importance of considering screening for this cardiomyopathy in adults hospitalized for AHF.

Abstract:Aims Transthyretin amyloid cardiomyopathy (ATTR‐CM) is an increasinglyrecognized cause of heart failure with preserved ejection fraction (HFpEF), which may be diagnosed non‐invasively using 99mTc 3,3‐diphosphono‐1,2‐propanodicarboxylic acid (DPD) scintigraphy‐based diagnostic criteria. Our aim was to determine the prevalence of ATTR‐CM in an undifferentiated HFpEF cohort with a DPD scintigraphy‐based screening protocol. Methods Patients with HFpEF [ejection fraction (EF) ≥50%] aged ≥60 years and no prior evaluation for cardiac amyloidosis or known monoclonal gammopathy attending a regional cardiology network were screened with DPD scintigraphy. Patients with positive myocardial uptake (Perugini grade 2 or 3) were tested for a monoclonal protein and transthyretin gene variant. Results Eighty‐six subjects were prospectively enrolled: 56% female, mean age 77 ± 8 years, 63% New York Heart Association (NYHA) Class III and median N‐terminal pro‐brain natriuretic peptide (NT‐proBNP) 1766 ng/L [inter‐quartile range (IQR) 731–3703]. DPD scintigraphy was positive in seven patients (8%). Monoclonal gammopathy of undetermined significance was present in one out of seven patients, and no pathogenic TTR gene variant was identified. The prevalence of wild‐type ATTR‐CM was 8% of this cohort. Compared with the HFpEF DPD scintigraphy‐negative cohort, DPD scintigraphy‐positive patients were older (86 ± 3 vs. 76 ± 8 years), more frequently male (16% vs. 2%, P = 0.02), and had significantly greater left ventricular (LV) wall thickness (16 vs. 12 mm; P = 0.002) and higher high‐sensitivity troponin levels at diagnosis [78 ng/L (IQR 21–116) vs. 11 ng/L (IQR 9–17); P

Abstract:Background Transthyretin cardiac amyloidosis (ATTR-CA) is increasingly recognized as a treatable form of heart failure. Atrial fibrillation (AF) is common in patients with ATTR-CA. Whether recent-onset AF can be used as an early marker to identify patients with ATTR-CA has not been elucidated. Methods This was a prospective study conducted at 3 Spanish centers. ATTR-CA noninvasive screening was offered to patients ≥65 years old recently diagnosed (<1 year) with non-valvular AF and who had ≥1 echocardiographic, electrocardiographic or clinical sign suggestive of ATTR-CA. Results A total of 121 patients were included (75% male, mean age 77±7 years). Ten patients (8.3%, 95%CI:4-14.7%), were diagnosed with cardiac amyloidosis (CA): 5 with definite wild-type ATTR-CA (ATTRwt), 4 with likely ATTRwt and one with undetermined CA. Compared to patients without CA, patients with CA were older (84±4 vs. 76±7 years;p<0.001), more frequently men (90% vs. 59%; p=0.047), presented higher median NTproBNP (3800 pg/L, IQR:1682-6101 vs. 1048 pg/mL, IQR:427-3154;p=0.017) and higher left ventricular hypertrophy (LVH) (14 mm, IQR:13-17 vs. 12 mm, IQR:12-13;p=0.003). Patients with CA also showed higher rate of permanent AF (90% vs. 49.5%;p=0.018) and a greater need for pacemaker implantation during follow-up (30% vs. 7.3%;p=0.049). No differences in mortality were observed between patients with and without CA after a median follow-up of 13 months (IQR:11-16 months). Conclusions Routine DPD scanning in elderly patients with recent-onset AF, LVH and an additional red flag may help to identify patients with ATTR-CA. However, larger studies evaluating this strategy in more diverse clinical settings would be required.

Abstract:The diagnosis and awareness of transthyretin amyloidosis cardiomyopathy (ATTR-CM) in heart failure with left ventricular ejection fraction (LVEF) > 40% remains under-recognized. This study aimed to investigate the prevalence and characteristics of ATTR-CM in patients with heart failure with LVEF > 40%. Patients with LVEF > 40% and maximal left ventricular wall thickness (MWT) > 10 mm who underwent bone scintigraphy were retrospectively investigated. Patients with a definite cause of heart failure were excluded. ATTR-CM was diagnosed when grade 2 or 3 myocardial uptake was observed on scintigraphy. Among 97 patients (male, 62.5%; median age, 69 years), 13 (13.4%) were diagnosed with ATTR-CM (wild type, 69.2%; hereditary type, 30.8%). Age or biomarker levels did not differ significantly; however, all patients with ATTR-CM were male. The ATTR-CM group had a significantly higher prevalence of polyneuropathy or carpal tunnel syndrome than the non-ATTR-CM group, accompanied by a longer PR interval, thicker MWT, larger left atrial volume index, and higher E/e′. Accordingly, ATTR was present in a substantial number, particularly among men. Clinicians should suspect ATTR when a male patient exhibits neurologic symptoms, diastolic dysfunction, and a long PR interval.

Abstract:Background Transthyretin Cardiac Amyloidosis (ATTR) is a type of restrictive cardiomyopathy, which typically manifests as Heart Failure with Preserved Ejection Fraction (HFpEF). The presence of unexplained left ventricular hypertrophy (LVH) associated with HF and red flag manifestations could increase the diagnostic probability. However, the diagnostic prevalence of this triad by cardiac scintigraphy remains uncertain. Methods From August 1st to December 31st, 2021, 22 consecutive patients diagnosed with a HF (ejection fraction more than 40%), LVH with unexplained etiology and at least one red flag clinical manifestation, underwent pyrophosphate scintigraphy (99mTc-PYP). The patients were divided into two groups: “Positive” and “Negative” (as defined by grade 2 or 3 uptake). Multiple logistic models were made with variable 99mTC-PYP and explanatory variables. Results Among 22 patients, 15 had a positive 99mTc-PYP study for ATTR. The prevalence of ATTR using the triad of HFpEF, unexplained LVH and at least one red flag was 68% (CI 95%; 45-86%). Patients with 99mTc-PYP positive tended to be male, older, and with an aortic mean gradient and interventricular septum higher, as compared to the group with a negative study. The most frequent red flag clinical manifestations were proteinuria (55%) and pseudoinfarction pattern (55%). The presence of 2 or more red flags could increase the diagnostic probability of the test (OR 1.6 (CI95% 0.52-4.89). Conclusions The diagnostic probability of ATTR by 99mTc-PYP scan could increased when a clinical manifestation of a red flag was added to the suspected diagnosis of heart failure and left ventricular hypertrophy. The use of non-invasive techniques allows early identification and treatment of this underdiagnosed disease.

Abstract:Aims: Transthyretin cardiac amyloidosis (ATTR-CA) is most often associated with heart failure with preserved ejection fraction (HFpEF). However, patients may present with impaired systolic function at the time of diagnosis, which has not been widely investigated. We sought to explore the prevalence of various heart failure (HF) phenotypes and their associated clinical characteristics at the time of ATTR-CA diagnosis. Methods: We performed a single-centre retrospective cohort study of consecutive patients with ATTR-CA evaluated between February 2016 and December 2022. Data on patient demographics, comorbidities, imaging and laboratory findings were compared across HF phenotypes (age: 78.1 ± 8.6 years, with 91.1% male). A total of 21.6% (n = 46) presented with heart failure with reduced ejection fraction (HFrEF), 17.8% (n = 38) with heart failure with mildly reduced ejection fraction (HFmrEF) and 60.6% (n = 129) with HFpEF at the time of diagnosis with ATTR-CA. Those presenting with HFrEF or HFmrEF were more likely to be African American and had significantly worse New York Heart Association (NYHA) functional class, higher N-terminal pro-brain natriuretic peptide (NT-proBNP) and higher serum creatinine levels as compared with those with HFpEF. Conclusions: Although ATTR-CA is traditionally thought to be seen primarily among patients with HFpEF, our data suggest that ATTR-CA has a higher prevalence among patients with HFrEF, which underscores the importance of heightened clinical suspicion regardless of ejection fraction when considering ATTR-CA. Furthermore, although comorbidities are similar, patients with HFmrEF and HFrEF had a worse symptom burden.
Trends in diagnosis of cardiac transthyretin amyloidosis: 3-year analysis of scintigraphic studies: Prevalence of myocardial uptake and its predictor factors

Abstract:To determine the prevalence of myocardial uptake (MU) and to identify predictors of MU in patients undergoing scintigraphy. Retrospective single-center series of technetium-99 m-labelled 3,3-diphosphono-1,2-propanodicarboxylic acid (99mTc-DPD) scans performed from March 2017 to March 2020. All patients undergoing scintigraphy were included, except those with preexisting amyloidosis. The features of MU, patients' characteristics and comorbidities were documented. Multivariate analysis was used to find items predicting MU. A total of 3,629 99mTc-DPD scans (total 11,444) were performed in patients aged > 70. The overall prevalence of MU was 2.7% (82/3,629); 1.2% in 2017-2018, to 2% in 2018-2019, and to 3.7% in 2019-2020. The prevalence of MU in patients without suspected cardiomyopathy was 1.2%; 1.1% in 2017-2018, 1.5% in 2018-2019 and 1% in 2019-2020. There is an increase in the number of requests due to suspected cardiomyopathy from 0.2% in 2017-2018 to 1.4% in 2018-2019 and to 4.8% in 2019-2020. Age, male sex, hypertension, heart failure, atrial fibrillation, atrioventricular block, aortic stenosis, and carpal tunnel syndrome were found to be predictors of MU. In patients without heart failure, only age, atrial fibrillation, and carpal tunnel syndrome were predicted MU. The prevalence of MU in scintigraphic studies surged over time due to the incremental referrals under the indication of cardiomyopathy workup. Atrial fibrillation and carpal tunnel syndrome were predictors for MU in patients without heart failure. Identifying patients with MU and no heart failure for extended screening for ATTR can lead to an earlier diagnosis and application of novel treatments.

Abstract:Introduction Cardiac amyloidosis is a rare and progressive form of restrictive cardiomyopathy which is associated with a poor prognosis. Two types of amyloid proteins commonly infiltrate the heart: i) immunoglobulin light-chain (AL or primary systemic) amyloid and ii) transthyretin (ATTR) amyloid. ATTR is the predominant hereditary form of cardiac amyloidosis, characterized by abnormal deposition of insoluble misfolded transthyretin protein fibrils in the myocardium resulting in organ dysfunction. The actual prevalence of ATTR amyloidosis is unknown, although recent data suggest that it is overlooked as a cause of common cardiovascular disease (CVD) especially in older people, including patients with heart failure with preserved ejection fraction (HFpEF), low-flow aortic stenosis, and atrial fibrillation. Objectives to identify patients with high-risk features for cardiac amyloidosis and assess for screening of transthyretin amyloidosis cardiomyopathy (ATTR-CM) among these patients at a safety net hospital Atlanta, GA. Methods We performed a systematic chart review of patients who had an echocardiogram between January 2018 and December 2021 at our institution. Adults aged 18 years or older with a diagnosis of cardiomyopathy or heart failure, and a left ventricular posterior wall (LVPWd) or interventricular septal wall (IVSd) thickness in diastole greater than or equal to 1.4 cm on echocardiogram were included. Patients with diagnosed AL amyloidosis and those with aortic stenosis were excluded. Variables collected included LVPWd, IVSd, gender, race, data on diagnostic modalities such as cardiac MRI (CMR), technetium pyrophosphate 99mTc scintigraphy (PYP scan), endomyocardial biopsy (EMB), and a diagnosis of cardiac amyloidosis. Results A total of 594 patients met the inclusion criteria. Of those, 215 (36.1%) were females, and 379 (63.9%) were males. Race analysis showed 91.6% were African American, 5% White, 2.1% Other and 1.1% Asian. Of the 594 included patients, only 16 (2.7%) had CMR, 68 (11.4%) had PYP scan, and 2 (0.33%) had EMB. 10 patients (1.7%) were positive for ATTR (8 of these diagnosed by PYP scan, and 2 by EMB). Discussion Less than 15% of the patients with high risk features for ATTR-CM at our institution had additional screening for this disease. These results demonstrate underutilization of imaging modalities, specifically PYP scan and CMR for the diagnosis of cardiac amyloidosis for at-risk patients at our institution. ATTR-CM is a re-emerging clinical entity that will require a high index of clinical suspicion in routine cardiac care for early diagnosis. Another project is underway focused on assessing healthcare provider and patient gaps in knowledge on the clinical features of ATTR-CM, as well as diagnosis and management of patients with cardiac amyloidosis.

Abstract:Aims Transthyretin 'wild‐type' amyloid cardiomyopathy (ATTRwt‐CM) is a differential diagnosis of heart failure with preserved ejection fraction (HFpEF). The clinical work‐up for ATTRwt‐CM is challenging. Considering a combination of clinical variables specific for ATTRwt‐CM might aid in identifying patients at risk. Methods and results Sixty patients (78 ± 6 years, 8% female) were diagnosed with ATTRwt‐CM by endomyocardial biopsy. Preserved ejection fraction (LVEF >45%) was present in 41 of the patients. Those were 1:1 propensity score age‐ and sex‐matched to a cohort of patients with HFpEF. ATTRwt‐CM patients had less obesity (P = 0.01) and higher septal thickness (IVSd, P 14 mm, E/e′ > 14 and absence of obesity (P > 0.01 for all) were identified as predictors for ATTRwt‐CM. A weighted point‐based score was derived with IVSd > 14 mm = 1 point; absence of obesity = 2 points; and E/e′ > 14 = 3 points. Abstract:Aims Recognition of transthyretin amyloid cardiomyopathy is increasing due to advances in cardiac imaging and diagnostic strategies, but questions remain regarding disease frequency and characteristics. We examined the prevalence and characteristics of transthyretin amyloid cardiomyopathy in older patients with hypertrophic cardiomyopathy of unascertained aetiology. Methods and results TTRACK was a multicentre, non‐interventional, cross‐sectional epidemiologic study funded by Pfizer and conducted in 20 hospitals and medical centres in 11 countries (NCT03842163). Eligible patients were aged ≥50 years, had hypertrophic cardiomyopathy (maximal end‐diastolic left ventricular wall thickness ≥15 mm on echocardiogram) without an identified genetic or alternative origin at study enrolment, and underwent 99mTechnetium bone scintigraphy, with or without single photon emission computed tomography (SPECT). Cardiac‐versus‐bone uptake on scans was visually scored from 0 to 3 (Perugini scoring). Patients with grades 1–3 underwent monoclonal protein and laboratory testing and transthyretin (TTR) gene sequencing. Of 766 eligible patients, 691 (90.2%) had scintigraphy alone and 75 (9.8%) scintigraphy plus SPECT. Two hundred and eight patients (27.2%) had grade 2 or 3 cardiac uptake on scintigraphy; 144 (18.8%) had grade 2 or 3 cardiac uptake and no evidence of plasma cell dyscrasia and were diagnosed with transthyretin amyloid cardiomyopathy. Of patients with transthyretin amyloid cardiomyopathy, 11 (7.6%) had a pathogenic TTR gene variant and 34 (23.8%), 74 (51.7%), and 35 (24.5%) had New York Heart Association class I, II, and III/IV heart failure (HF) symptoms, respectively. Clinical and laboratory diagnostic characteristics were observed in ≥90% of patients with transthyretin amyloid cardiomyopathy. Abstract:AIMS: Transthyretin cardiac amyloidosis (ATTR-CA) has been reported in patients with aortic stenosis (AS) but its prevalence and phenotype are not known. We examine elderly patients with severe symptomatic AS undergoing transcatheter aortic valve replacement (TAVR) and determine the prevalence and phenotype of ATTR-CA non-invasively.METHODS AND RESULTS: We performed technetium-99m pyrophosphate (99mTc-PYP) cardiac scintigraphy prospectively on patients who underwent TAVR, to screen for ATTR-CA. Transthoracic echocardiography and speckle-strain imaging were performed. We assessed the association of several parameters with ATTR-CA using multivariable logistic regression and constructed receiver operating curves to evaluate the best predictors of ATTR-CA. Among 151 patients (mean age 84 ± 6 years, 68% men), 16% (n = 24) screened positive for ATTR-CA with 99mTc-PYP scintigraphy. Compared with patients without ATTR-CA, ATTR-CA patients had a thicker interventricular septum (1.3 vs. 1.1 cm, P = 0.007), higher left ventricular (LV) mass index (130 vs. 98 g/m2, P = 0.002), and lower stroke volume index (30 vs. 36 mL/m2, P = 0.009). ATTR-CA patients had advanced diastolic dysfunction with higher E/A ratio (2.3 vs. 0.9, P = 0.001) and lower deceleration time (176 vs. 257 ms, P < 0.0001); impairment in systolic function with lower ejection fraction (48% vs. 56%, P = 0.011), myocardial contraction fraction (26 vs. 41, P < 0.0001), and average of lateral and septal mitral annular tissue Doppler S' (4.0 vs. 6.6 cm/s, P < 0.0001). While ATTR-CA patients had more impaired global longitudinal strain (-12 vs. -16%, P = 0.007), relative apical longitudinal strain was the same regardless of ATTR-CA diagnosis (0.98 vs. 0.98, P = 0.991). Average S' best predicted ATTR-CA in multivariable logistic regression (odds ratio 16.67 per 1 cm/s decrease with AUC 0.96, 95% confidence interval 0.90-0.99, P = 0.002) with a value ≤6 conferring 100% sensitivity for predicting a positive 99mTc-PYP amyloid scan.CONCLUSIONS: Transthyretin cardiac amyloidosis is prevalent in 16% of patients with severe calcific AS undergoing TAVR and is associated with a severe AS phenotype of low-flow low-gradient with mildly reduced ejection fraction. Average tissue Doppler mitral annular S' of < 6 cm/s may be a sensitive measure that should prompt a confirmatory 99mTc-PYP scan and subsequent testing for ATTR-CA. Prospective assessment of outcomes after TAVR is needed in patients with and without ATTR-CA.

Abstract:Aims Transthyretin amyloid cardiomyopathy (ATTR‐CM) is characterized by increased wall thickness, diastolic dysfunction and progressive heart failure symptoms. The disease may infiltrate the conduction system leading to conduction disturbances requiring an implantation of permanent cardiac pacemaker (PM), but the extent is unknown. Here, we report the prevalence of ATTR‐CM in patients ≥65 years with PM. Methods and results In this prospective, cross‐sectional single‐centre study patients were recruited from our out‐patient pacemaker clinic. Eligibility criteria were age above 65 years, permanent cardiac pacemaker and competent to give informed consent. Patients underwent echocardiography at the pacemaker visit and were referred to 99mTc‐DPD‐scintigraphy (DPD) and blood samples if septum thickness was ≥12 mm, defined as left ventricular hypertrophy (LVH). Fifty eight of the 128 patients had LVH on echocardiography. Eleven patients had a DPD‐scintigraphy based diagnosis of ATTR‐CM, which represent 19% of patients with LVH and 9% of the total cohort. Abstract:Background: Transthyretin cardiac amyloidosis (ATTR-CA) is a life-threatening heart condition due to mutations in the transthyretin (TTR) gene. This study aimed to investigate ventricular global longitudinal strain (GLS) and global circumferential strain (GCS) alongside traditional echocardiographic measures in TTR carries with preserved ejection fraction (EF). Methods: Individuals with a family history of ATTR-CA, positive genetic testing for TTR, no clinical cardiac involvement and preserved EF were matched with healthy controls. Results: Sixty-five patients were included: 31 TTR+ and 34 controls. No significant differences in chamber dimensions or EF were observed between groups. However, septal velocities were markedly lower in TTR+ than controls (9 [7;12] cm/s vs. 12 [10;13] cm/s, p=0.003). Additionally, both GCS and GLS were significantly reduced in the TTR+ group compared to controls (-21 ± 4% vs. -28 ± 4%, p<0.001 and -19 ± 3% vs. -25 ± 2%, p<0.001, respectively). Furthermore, right ventricular strain showed a similar trend, being lower in the TTR+ group compared to controls (-18 ± 4% vs. -25 ± 5%, p<0.005). Logistic regression analysis identified GLS (OR = 2.46; 95% CI 1.30-4.62) and E/e' mean ratio (OR = 2.04; 95% CI 1.09-3.91) as independent predictors of positive genetic testing for TTR. Conclusion: Asymptomatic individuals with a family history of ATTR-CA and positive genetic testing for TTR, despite preserved EF, exhibit lower GLS and GCS compared to controls. GLS and the E/e' ratio were identified as independent predictors of positive genetic testing for TTR.

Abstract:Background: The early detection of transthyretin cardiac amyloidosis (ATTR-CM) is essential, with Tc-99m pyrophosphate scintigraphy (PYP scan) being a key diagnostic tool. Although a previously validated score has shown promise in predicting PYP scan positivity among patients with HFpEF, further evaluation in diverse cohorts is necessary. Objectives: To assess the effectiveness of the ATTR-CM score in predicting PYP scan positivity within our patient population. Methods: We analyzed patients referred for PYP with SPECT at the Cleveland Clinic from January 2012 to January 2020, all of whom had undergone echocardiography within the previous year. The ATTR-CM score was determined using the following criteria: Age (60–69, +2; 70–79, +3; ≥80, +4), sex (male, +2), hypertension (present, −1), left ventricular ejection fraction (LVEF 0.57, +2). A score of ≥6 indicated high risk. Results: Among the 540 patients (32% female, 33% black), 27% had an LVEF <40%. The score demonstrated good discrimination by AUC, with consistent performance across different racial groups, sexes, and LVEF categories. For scores ≥6, sensitivity was lower in women and black patients; however, lowering the cutoff to 5 markedly improved sensitivity. Conclusions: The ATTR-CM score displayed consistently good performance by AUC across our cohort, including patients with HFrEF. Nevertheless, its sensitivity was reduced in black patients and women. Efforts to scale ATTR-CM diagnosis tools should be mindful of demographic differences in risk prediction models.

Abstract:Background/Introduction Cardiac amyloidosis (CA) is characterized by extracellular deposition of misfolded proteins in the heart. The data suggest that cardiac amyloidosis is underappreciated as a cause of common cardiac diseases or syndromes. More than 98% of currently diagnosed cardiac amyloidosis result from fibrils consisting of monoclonal immunoglobulin light chains (AL) or transthyretin (ATTR). The natural history of ATTR cardiomyopathy commonly includes both the conduction system disease and arrhythmias, which may occur years before the onset of heart failure. Conduction abnormalities are commonly encountered among patients with CA and constitute an important cause of morbidity and mortality. Most reports agree that pacemakers are most common in wild type ATTR-CA, followed by hereditary ATTR-CA and, finally, AL-CA. Purpose As unexplained LV wall thickness ≥ 12 mm is a screening sign of possible cardiac ATTR, systematicscreening for CA in patients requiring permanent pacemakers with unexplained LV wall thickness of ≥ 12 mm seems to be rational and could possibly lead to early diagnosis of cardiac ATTR. To the best of our knowledge, the usefulness of such screening has not been investigated before. This project, therefore, aims to determine the prevalence of cardiac ATTR and its subtypes in this preselected group, and to propose the most suitable battery of diagnostic procedures for this purpose. Methods Population to be tested: Consecutive patients ≥50 years referred for permanent pacemaker implantation with unexplained LV wall thickness ≥12 mm determined by echocardiography. The research protocol was that of an open-label prospective study. Biochemistry samples including serum troponin I and NT-proBNP were collected before pacemaker implantation. All patients were screened for ATTR-CA according to the recommendations of the Position statement of the ESC Working group on myocardial and pericardial diseases. Tc-DPD scintigraphy scan served as a gold standard for determining the diagnosis of cardiac ATTR. Results Out of 100 consecutive patients meeting inclusion criteria, 15 % suffered from amyloidosis (12 patients were diagnosed with ATTR and 3 with AL cardiac amyloidosis). Table 1 shows a variety of parameters, many of which differed between patient groups with and without amyloidosis, confirming the wide range of parameters significantly associated with cardiac amyloidosis; our ongoing research in this field could lead to the development of a scoring system for detection of cardiac amyloidosis. Conclusions The prevalence of cardiac amyloidosis in the investigated group was 15 %. Systematic screening for ATTR in patients with unexplained LV wall thickness ≥ 12 mm requiring permanent pacemaker implantation seems to be rational and could possibly lead to an earlier diagnosis of cardiac amyloidosis, especially ATTR.

Abstract:Background Cardiac amyloidosis (CA) is an infiltrative disorder caused by protein deposition, being transthyretin amyloidosis (ATTR) and primary amyloidosis (AL) the most common forms of CA. ATTR can occur in a hereditary form (ATTRv) or wild-type (ATTRwt). Our area is endemic for ATTRv due to the Val30Met mutation. Purpose To analyse the usefulness of scintigraphy with [99mTc]Tc-DPD (DPD-CS) in an endemic area and its relationship with the diagnostic of CA. Methods We conduct a retrospective study (04/2013-12/2022) of DPD-CS performed for suspected/screening of CA. Our acquisition protocol consists of performing planar images after 3h p.i.(anterior, left anterior oblique and left lateral) +/- SPECT/CT. The assessment of myocardial uptake was evaluated using the Perugini scale (PS: 0-3), considering 2 and 3 as positive scans. DPD-CS results were classified based on the suspicion of CA, according to the 2021 ESC guidelines (we considered cardiomyopathy if: left ventricle hypertrophy ≥12 mm + ≥3 typical signs/symptoms), as well as according to their final diagnosis: ATTRv, asymptomatic carriers, ATTRwt, AL, AA and other non-amyloid (NA) causes. Clinical findings were correlated with the results of the DPD-CS. Results 509 DPD-CS were performed in 478 patients (median age:73 years [22-95], 62% men). Median follow-up: 4.17 years. 127/478 patients were classified with cardiomyopathy (131/509 DPD-CS) and 351/478 without cardiomyopathy (378/509 DPD-CS). 115/509 positive DPD-CS (PS-2: 17, PS-3: 98): 113 had cardiomyopathy (55 ATTRv, 60 ATTRwt) and 2 without cardiomyopathy (1 ATTRv, 1 asymptomatic carrier). These latter two patients developed cardiomyopathy during the follow-up. 394/509 negative DPD-CS (PS 0: 387, PS 1: 7): 18 with cardiomyopathy (1 ATTRv, 1 carrier, 3 AL and 13 NA) and 376 without cardiomyopathy (30 asymptomatic carriers, 88 ATTRv, 2 AA, 9 AL and 247 NA). One patient with cardiomyopathy and ATTRv had initial negative DPD-CS, but became positive DPD-CS on follow-up. All ATTRv without cardiomyopathy presented polyneuropathy. Final diagnoses (according to DPD-CS number): 143 ATTRv (140 Val30Met, 2 Val122Ile, 1 Glu495Gln), 32 carriers (29 Val30Met, 2 Val122Ile, 1 His51Asn), 61 ATTRwt,12 AL, 2 AA and 259 NA. The analysis obtained after correlating the results of the DPD-CS regarding the presence of cardiomyopathy was: sensitivity 86.25%, specificity 99.47%, PPV 98.26% and NPV 95.43%. Conclusion Our study reaffirms the usefulness of DPD-CS in the non-invasive diagnosis of transthyretin CA. DPD-CS has an additional value in endemic areas for the early detection of CA in asymptomatic patients.

Abstract:Introduction Amyloid transthyretin amyloid cardiomyopathy (ATTR-CM) is a rare, systemic, life-threatening disease, characterized by both cardiac and extra-cardiac symptoms and signs. Its diagnosis remains challenging, however it is increasingly being pursued due to the availability of non-invasive diagnostic means, namely bone scintigraphy. Data regarding the prevalence of ATTR-CM in Greece are scarce. Purpose We aimed to study the prevalence and characteristics of ATTR-CM, both wild-type (ATTRwt-CM) and hereditary (ATTRv-CM), in patients with left ventricular hypertrophy (LVH) of undetermined etiology in Greece. Methods RETTRACE is an on-going, low- risk, prospective, multicenter, nationwide descriptive study, in 14 major outpatient Cardiology centers throughout Greece. Its primary endpoint is the prevalence of ATTR-CM in adults ≥ 50 years, with: a) Heart Failure with preserved Ejection Fraction (HFpEF) or Heart Failure with mildly reduced Ejection Fraction (HFmrEF) and a left ventricular wall thickness (LVWT) of ≥12mm, or b) a LVWT ≥14 mm of unknown etiology. Secondary endpoints include the prevalence of ATTRv-CM, the description of clinical characteristics of ATTR-CM patients, and their quality of life, as evaluated with the Minnesota Living with Heart Failure Questionnaire (MLHFQ) and the EuroQol-5-Dimensions Questionnaire (EQ-5D-3L). Patients were screened and managed according to local clinical practice. Screening for light chain (AL) amyloidosis was done by immunofixation electrophoresis (serum and urine) and serum free light-chain (sFLC) assay, while biopsy was performed at the investigator's discretion. TTR genotyping was done centrally. Results The RETTRACE interim analysis (data-cut-off: May 31st, 2023), includes 153 patients (men: 52,9%), with a mean (SD) age of 77 (10.3) years. Basic demographics are shown in Table 1. Amongst patients who had bone scintigraphy results available, 71 (51.4%) were grade 0 according to the Perugini score. Furthermore, 62 tests of grades 1-3 were assessed by two independent reviewers and their final consensus was used to estimate the primary endpoint. In total, 24 out of 134 eligible patients were deemed to have ATTR-CM (17.9%), per applied criteria. Two of them carried mutations (Val114Ala and Ala101Thr respectively). As compared to non-ATTR-CM patients, those with ATTR-CM were older (mean age of 82.9 vs 76.0 years, p=0.004), less frequently hypertensive (54.2% vs 78.2%, p=0.021) and had more frequently undergone carpal tunnel surgery (29.2% vs 10.0%, p=0.021). Moreover, they had a poorer quality of life by MLHFQ (median 19.5 vs 40.0, p=0.05), while no significant differences were found in the EQ-5D-3L questionnaire. Conclusions ATTR-CM has a significant prevalence in a Greek population of adults ≥ 50 years with LVH of undetermined etiology. RETTRACE study results will provide valuable information for this underdiagnosed, life-threatening condition.

Atrial fibrillation in transthyretin amyloidotic cardiomyopathy: prevalence and echocardiographic predictors

Abstract:Background: Conduction disorders and arrhythmias frequently accompany cardiac amyloidosis (CA), with atrial fibrillation (AF) being the most prevalent manifestation. The prevalence of AF varies across different types of CA, with transthyretin (TTR) type showing the highest prevalence upon diagnosis. Methods: A retrospective, observational analysis was conducted to evaluate the prevalence of AF and to identify echocardiographic predictors related to the development of AF in our population of patients with transthyretin cardiac amyloidosis (TTR-CA). Results: A total of 99 patients with TTR-CA were identified, with a median age of 82 (75-85) years, a median ejection fraction of 50% (43-60) and 97 of them wild type. At the time of cardiomyopathy diagnosis, 55% had AF, and during follow-up, 43% developed new AF. Among the latter group, there was a non-significant tendency to have a smaller diastolic diameter, lower left ventricular ejection fraction, increased septal thickness, higher pulmonary pressure, and lower tissue velocities, with statistical significance only found in the right ventricular S wave velocity: 8.5 cm/s (7.7-9) vs. 9.7 cm/s (8.4-10) (P=0.046). Conclusions: The high prevalence and incidence of AF in TTR-CA is demonstrated in our series. Doppler echocardiography might help to identify patients with signs of more advanced cardiomyopathy, such as lower right ventricle tissue velocity, who might be at higher risk of developing AF and gain the benefit of prompt diagnosis and treatment.

Abstract:Aims Transthyretin cardiac amyloidosis (ATTR‐CM) may be an underestimated cause of heart failure among geriatric patients and represent a unique phenotype and prognostic profile. Methods and results This retrospective, observational, cohort study characterizes cardiac and extracardiac disorders at diagnosis and assesses prognosis among ATTR‐CM patients based on age (geriatric vs. non‐geriatric) and amyloidosis subtype (wild type, ATTRwt and hereditary, ATTRv). In total, 943 patients with ATTR‐CM were included, of which 306 had ATTRv and 637 had ATTRwt. Among these, 331 (35.1%) were non‐geriatric (<75 years), and 612 (64.9%) were geriatric (≥75 years). The population exhibited conduction abnormalities, atrial fibrillation and ischaemic heart disease that progressively deteriorated with age. Among ATTRwt patients, peripheral neuropathy, neurovegetative symptoms, and hearing loss were present across all age groups, but reports of carpal tunnel symptoms or surgery decreased with age. Conversely, among ATTRv patients, reports of extracardiac symptoms increased with age and Val122ILe mutation was highly prevalent among geriatric patients. The 3‐year survival was higher among non‐geriatric ATTR‐CM patients (76%) than geriatric patients (55%) and predictors of 3‐year mortality differed. Notably, predictors identified among geriatric patients were alkaline phosphatase (ALP) (HR = 1.004, 95% CI: [0.001–1.100)], troponin T hs (HR = 1.005, 95% CI: [1.001–1.120)] and tricuspid insufficiency (HR = 1.194, 95% CI: [1.02–1.230)]. Whereas, among non‐geriatric patients, NT‐proBNP (HR = 1.002, 95% CI: [1.02–1.04], global longitudinal strain (HR = 0.95, 95% CI: [0.922–0.989], and glomerular filtration rate (HR = 0.984, 95% CI: [0.968–1.00) were identified. We propose a 3‐stage prognostic staging system combining troponin T hs (≥44 ng/L) and ALP levels (≥119 UI/L). In the geriatric population, this model discriminated survival more precisely than the National Amyloidosis Centre staging, particularly for classifying between stage 1 (82%), stage 2 (50%) and stage 3 (32%) for ATTRv and ATTRwt. Conclusions These diagnostic and prognostic indicators, along with ATTR subtype, highlight the distinct characteristics of this important, geriatric ATTR‐CM patient group. Recognizing these mortality markers can be valuable for geriatricians to improve the prognostic quality management of geriatric patients with ATTR‐CM.
