Abstract:Objective: To evaluate the progress of cataract surgery in a training institution providing tertiary care since the removal of pandemic restrictions. To evaluate ocular and demographic characteristics of cataract cases in a tertiary care teaching institution since the lifting of pandemic restrictions. Methods: Patients who underwent cataract surgery in our clinic in the pre-pandemic period September 2019-March 2020 (group 1, n=353) and in the post-pandemic period September 2021-March 2022 (group 2, n=459) were retrospectively screened. The cases were operated by two educator ophthalmologists and residents. The main parameters evaluated were cataract morphology, surgical parameters, and posterior capsular rupture complications. Results: The case rate increased by 30% in group 2 once pandemic restrictions were relaxed. Preoperative best corrected visual acuity (BCVA) in groups 1 and 2 was 0.840±0.63 and 1.26±0.75 log MAR, respectively (p<0.001). The percentage of mature cataracts was 15.3% in group 1 and 31.2% in group 2 (p<0.001). Significantly higher cumulative dissipated energy (CDE), total aspiration time, and fluid amount (p<0.001 for all) were found in group 2. During the training phase, 25.2% of the cases in group 1 and 24.6% in group 2 were performed by resident doctors (p=0.870). Residents in groups 1 and 2 had mature cataract case rates of 6.7% and 13.3%, respectively (p<0.001). The incidences of posterior capsule rupture in the instances of residents was 3.4% in group 1 and 4.4% in group 2 (p=0.498). A negative correlation (r=-0.424, p<0.001) between CDE and BCVA and a positive correlation (r=0.40, p<0.001) between cataract hardness and CDE were both found. Conclusions: The number of cataract surgeries increased after the COVID-19 pandemic. Poor vision and increasing rates of mature cataracts are other effects of this backlog. Residents have to deal with challenging cases. Our results are just the tip of the iceberg. Urgent planning is needed to deal with the remaining cases. Abstract:PURPOSE: To determine the visual outcome and postoperative complications of cataract surgery in patients with ocular surface disorders. SETTING: Tertiary eye care centre in North India DESIGN: Retrospective observational study METHODS: Patients with various ocular surface disorders with stabilized ocular surfaces who underwent cataract surgery during this period and had a minimum postoperative follow-up of six weeks were included. The primary outcome measures were post-operative best corrected visual acuity at six weeks, best BCVA and postoperative complications. RESULTS: The study included 20 men and 24 women. A total of 55 eyes were evaluated: Steven-Johnson syndrome (SJS) 35 eyes, Ocular Cicatricial Pemphigoid (OCP) four eyes, eight eyes with Dry Eye Disease (DED), six eyes with chemical injury and two eyes with Vernal Keratoconjunctivitis (VKC). The mean duration of ocular surface disorder was 33.9 ± 52.17 months. The median preoperative BCVA was 2.0 (IQR, 1.45 to 2.0). The median BCVA ever achieved was 0.50 (IQR, 0.18 to 1.45) at 2 months and the median BCVA at 6 weeks was 0.6 (IQR, 0.3 to 1.5). Maximum improvement in BCVA was noted in patients with DED and SJS and the least in OCP. Phacoemulsification was performed in 47.27% eyes with intraoperative complications noted in 9% eyes. Post-operative surface complications occurred in 12 (21.82%) eyes. Other postoperative complications occurred in 9 (16%) eyes. CONCLUSION: Cataract surgery outcome can be visually rewarding in patients with ocular surface disorders provided ocular surface integrity is adequately maintained preoperatively and postoperatively.

Abstract:Objective: This study aimed to determine the proportion of poor visual outcome and associated factors among adult patients who underwent cataract surgery at Debre Markos and Felege Hiwot Comprehensive Specialized Hospitals in northwestern Ethiopia in 2023. Methods: A hospital based cross-sectional study was conducted on 418 adult patients who had undergone cataract surgery from June 07 to August 07, 2023. Patients were recruited using systematic random sampling with an interval of 2. A pre-tested semi-structured questionnaire, medical record review, and ophthalmologic examination were used to collect data. The collected data was entered into Epi-info version 7 and exported to SPSS version 25 software for analysis. Binary logistic regression was used to determine the factors associated with poor visual outcomes of cataract surgery. Variables with a p-value of less than 0.05 in the multivariable binary logistic regression were considered statistically significant. Results: A total of 408 study participants with a median age of 65 years and a response rate of 97.6% took part. The proportion of poor visual outcomes of cataract surgery from 4 weeks to one year was 25.7% (95%CI: 21.6%, 30.3%). Factors responsible for poor visual outcomes of cataract surgery were intraocular lens implantation without a posterior chamber (AOR = 2.91, 95%CI:1.46,5.80), per-existing central corneal opacity (AOR = 3.83, 95%CI:1.52,9.69), pseudoexfoliation (AOR = 3.91,95%CI:1.39,11.88), age-related macular degeneration(AOR = 3.75, 95%CI:1.22, 11.88), glaucoma (AOR = 3.11, 95%CI:1.06,9.17) and striate keratopathy(AOR = 3.4, 95%CI: 1.11, 10.88). Conclusion: In this study, the proportion of poor visual outcomes of cataract surgery is higher than the World Health Organization recommendation. The study found that implantation of an intraocular lens without a posterior chamber, pre-existing central corneal opacity, pre-existing age-related macular degeneration, pre-existing glaucoma, pseudoexfoliation, and striate keratopathy were significantly associated with poor visual outcomes of cataract surgery. We recommend that ophthalmologists and cataract surgeons prioritize the reduction of surgical complications and pre-existing ocular co-morbidities to enhance post-operative visual acuity. Improving pre-operative assessment and refining surgical techniques like phacoemulsification will aid in achieving this goal.

Abstract:Objective This study quantifies change in best visual acuity (BVA) over the preoperative period and assesses factors associated with postoperative outcomes. Design Retrospective chart review conducted at a single institution. Participants A total of 691 patients underwent cataract surgery and had a preoperative assessment 0–30 days prior to surgery following their surgical evaluation. Methods Baseline demographics and past medical and clinical data were collected through electronic medical record query. BVA was noted at initial surgical evaluation, preoperative assessment, and nearest postoperative assessment. Results A total of 691 patients (911 eyes) were included with mean BVAs at the initial evaluation, preoperative assessment, and postoperative assessment of 68.3 ± 16.8, 64.6 ± 18.5, and 81.1 ± 12.0 ETDRS letters, respectively. Mean BVA was significantly higher postoperatively compared with the preoperative assessment and initial evaluation ( p 3 eyes and 0.9% experienced improvement of BVA by >3 lines. Time to surgery was significantly associated with change in postoperative BVA (effect size, –0.03 ETDRS letters; p = 0.015) but was not significant on multiple linear regression. BVA at initial evaluation, glaucoma, and glaucoma surgery were all significantly associated with postoperative outcomes. Conclusion Most eyes experienced stable vision in the preoperative period for cataract surgery. On average, patients with high BVAs at the time of initial surgical evaluation may be able to defer surgery without the risk of poorer surgical outcomes.

Visual Outcome and Complications in White Mature Cataracts after Phacoemulsification

Abstract:Purpose: To identify risk factors and perioperative complications and assess postoperative visual outcome in patients of white mature cataracts undergoing phacoemulsification surgery. Methods: This cross-sectional study was conducted on 46 patients of white mature cataract undergoing phacoemulsification. Preoperatively, a detailed ocular examination was done, and anterior chamber depth (ACD) and lens thickness (LT) were measured on ultrasound biomicroscopy. Intraoperative and postoperative complications (on days 1, 7, and 30) and best-corrected visual acuity on postoperative day 30 were noted. Results: The mean age of patients was 60.78 ± 9.001 years. The study group consisted of 41.3% of males and 58.7% of females. 8 (19.56%) cases encountered complications during phacoemulsification surgery. The intraoperative complications were capsulorhexis-related in 5 (10.86%) patients, posterior capsular rent in 2 (4.3%) patients, and 1 (2.17%) case had zonular dialysis. There was no difference in mean ACD and LT in cases who developed complications and those who underwent uneventful surgery (P > 0.05). The commonly observed postoperative complications on day 1 were corneal edema (41.3%), anterior chamber flare (45.65%), and cells (39.13%) which resolved with routine topical medications. All patients showed a significant reduction in postoperative intraocular pressure (P < 0.001). A total of 37 (80.43%) eyes regained vision between 6/6 and 6/9 postoperatively (P < 0.001). Conclusion: Phacoemulsification surgery is safe in white mature cataract that results in significant visual improvement. Common intraoperative complications are capsule related, while frequently encountered postoperative complications are corneal edema and anterior chamber reaction. Preoperative ACD and LT have no role in predicting intraoperative complications.

Abstract:Background: The most common preventable cause is cataract for the loss of vision, and pseudophakic cystoid macular edema, also called Irvine-Gass syndrome, is a well-known complication after cataract surgery. Objective: To determine changes in total retinal thickness after cataract uncomplicated surgery. Objective: To determine changes in total retinal thickness after cataract uncomplicated surgery. Methodology: A prospective quasi experimental study was done at the department of Ophthalmology of Combined Military Hospital (CMH), Rawalakot AJK between September and December 2022. Total 28 patients (28 eyes) who underwent phacoemulsification uncomplicated surgery and grafting of intraocular lens in unilateral eye were included. The effective phacoemulsification time and total energy used during phacoemulsification cataract surgery was noted. The optical coherence tomography (OCT) scan was performed to analyze the retina thickness. Data were collected preoperatively, which serves as a baseline measurement. This baseline measurement was used to compare with measurements taken at postoperative day 1, end of week 1, end of month 1, and follow-up period month 3 to assess changes in various parameters such as total retinal thickness. Results: The patient’s mean age comprising of 20 (71.4%) males and 8 (28.6%) females was 54.73±7.6 years. Typically, there is a transient decrease in retinal thickness, immediately after surgery (day 1), followed by a gradual increase in thickness over the weeks (first week) and months (1st & 3rd month) following surgery. Overall, a positive correlation was seen between phacoemulsification time, total energy, and changes in the retinal thickness (1mm & 1-3mm grids) (p ? 0.05). Conclusion: The study concluded that a follow-up period of at least 3 months is generally recommended after surgery of cataract to assess the whether preoperative levels return of total retina. Abstract:Cataract surgery is a widely performed and highly effective procedure that significantly improves vision in elderly patients. This narrative review examines the impact of cataract surgery on mental health outcomes in the elderly, focusing on conditions such as depression, anxiety, and cognitive decline. The review highlights the prevalence of cataracts in older adults and the importance of mental health in this demographic, emphasizing the interconnectedness of visual and mental health. Improved vision following cataract surgery has been associated with enhanced quality of life, increased independence, and better psychological well-being. Mechanisms linking visual improvement to mental health benefits include biological pathways, psychosocial factors, and overall health improvements. However, socioeconomic factors, access to healthcare, and patient education play crucial roles in achieving positive outcomes. This review also compares cataract surgery with other interventions, providing a cost-benefit analysis and discussing the long-term sustainability of mental health benefits. Practice recommendations include pre-surgical mental health screening, integrative care approaches, and guidelines for postoperative care focusing on mental health. The review concludes with suggestions for future research to further explore the relationship between cataract surgery and mental health in the elderly, aiming to enhance clinical practice and public health strategies.
Vision-related Quality of Life Outcomes of Cataract Surgery in Advanced Glaucoma Patients

Abstract:Purpose:To demonstrate longitudinal postoperative vision-related quality of life and visual acuity changes after cataract surgery in advanced glaucoma patients. Materials and Methods:In total, 93 patients who underwent cataract surgeries were collected from June 2010 through June 2013 in Shanghai First People’s Hospital, Shanghai Jiao Tong University, and were followed up for at least 3 months. Patients with advanced glaucoma, here defined as near total cupping of the optic nerve with severe visual field loss within 10 degrees of fixation, were enrolled. A standard phacoemulsification technique with the insertion of an intraocular lens was used. No complications occurred intraoperatively or postoperatively. Preoperative and 3-month postoperative vision-related quality of life were assessed in face-to-face interviews. The Chinese-version of the Low Vision Quality of Life Questionnaire (CLVQOL) was used. Wilcoxon signed-rank test was used to compare the differences in the best-corrected visual acuity (BCVA), weighted average LogMAR (WMAR), and CLVQOL scores. A binary logistic regression analysis was conducted to explore the potential factors associated with the change in CLVQOL scores. Results:The overall changes in the CLVQOL composite scores ranged between 4 and 42 (median=19). Statistically significant increases occurred in the composite scores of all of the subscales, the total CLVQOL, the BCVA in the surgery eye, and the WMAR (all P<0.001). Greater increases in the CLVQOL composite scores was associated with superior preoperative WMAR (odds ratio: 9.920, P<0.05), and age below 60 years (odds ratio: 9.905, P<0.05). Conclusions:Cataract surgery should be recommended for advanced glaucoma patients with stable intraocular pressure.
Outcome of pediatric cataract surgery including color vision, stereopsis, and cognition in a tertiary eye care center in India

Abstract:Background: Pediatric cataract is an important cause of avoidable childhood blindness in India. Although there are studies on clinical features and visual acuity outcomes of pediatric cataract surgery, there are no studies that look into aspects of color vision, stereopsis, and cognitive functions in children with cataracts. Studying these parameters pre- and postsurgery will help us assess the overall components of visual function that are critical for the child's development. With this in mind, we aimed to study the clinical features, outcome of treatment, color vision, stereopsis, and cognitive functions in children with cataracts. Materials and methods: Design: Hospital-based prospective descriptive study. Data of 68 children (0 < 18 years) were collected, on the clinical features, visual acuity, color vision, stereopsis, and social and intelligence quotient before and after surgery. Data analysis was performed using Graph Pad Prism software version 9.4.0 (673). Results: Ninety eyes of 68 children (36-bilateral/32-unilateral) were operated. A total of 46 children were available at 6 months' follow-up (28 bilateral cataracts (46 eyes) and 18 unilateral cataracts). Nineteen (67.8%) of the bilateral, had a best corrected visual acuity of 6/6-6/18 in the better eye and nine (50%) of the unilateral had a visual acuity of 6/18-6/6 in the operated eye. Preoperatively, 61.5% of the children had a stereopsis of 100-600 arc-secs. Post-operatively, 84.4% of the children had a stereopsis in the range <50-600 arc-secs. All children had color vision deficiency. Preoperatively, 62% of the children had strong and moderate deficiency, whereas postoperatively, 66.6% had mild deficiency. We found negative correlation (r = -0.503) between social quotient and percentage of visual impairment indicating that when visual impairment increases social quotient decreases. The mean IQ preoperatively was 94.36 ± 2.98 and postoperatively 101.38 ± 2.77, with 95% confidence interval P value was statistically significant (P = 0.0001). This shows better and faster performance postoperatively. Conclusion: Majority of the children had improved visual acuity after surgery. This outcome can be extrapolated to the overall improvement in stereopsis, color vision, and higher cognitive functions.
Cataract surgery in AMD patients

Abstract:Cataract and age‐related macular degeneration (AMD) are leading causes of visual loss and they occur in the similar age group, thus co‐existence of both diseases is real clinical problem and challenge. Theoretical mechanisms considered to be involved in possible relations between cataract surgery and AMD progression include blue light toxicity and the role of post‐operative inflammation. None of these mechanisms was found to play a role in clinical conditions. Some earlier a recent population‐based studies reported an increased risk of late AMD after cataract surgery; however, this was not confirmed by recent cross‐sectional studies (clinical‐based) from South Korea and Australia. Systematic reviews and meta‐analyses reported a moderate quality of evidence to suggest that VA was better by approximately 7 letters in eyes with AMD that underwent cataract surgery vs. eyes with AMD that did not undergo cataract surgery for up to 12 months after surgery and the same quality of evidence did not find an increased risk of progression to exudative AMD for at least 12 months after surgery. The risks and possible advantages of cataract surgery in visual rehabilitation of wet and dry AMD patients will be discussed.

Abstract:Cataract surgery is aimed at restoring sight to near normal vision. This study, conducted at the Department of Ophthalmology, Government Medical College, Srinagar, is an attempt to determine the causes of subnormal vision in patients following cataract surgery at a tertiary hospital in Kashmir. One hundred patients who underwent cataract surgery with an unaided visual acuity of <6/9 at 16 weeks postoperatively were included in the study. Postoperative follow-up examinations were conducted until the 16th week. Intraoperative and postoperative complications were recorded to determine the cause of subnormal vision. Of 100 patients, 40 underwent extracapsular cataract extraction (ECCE), 30 underwent small incision cataract surgery (SICS) and 30 underwent phacoemulsification. Seventy-five percent of the patients who underwent ECCE had postoperative astigmatism with a mean astigmatism of 2.2 ± 0.81 diopters at 16 weeks, with the majority having with-the-rule astigmatism. In the SICS group, 17 (56.6 %) patients had a mean postoperative astigmatism of 0.75 ± 0.40 diopters, with the majority (82.3 %) having against-the-rule (ATR) astigmatism. In the phacoemulsification group, 13 (43.3 %) of the patients had a mean postoperative astigmatism of 0.48 ± 0.23 diopters with the majority having ATR astigmatism. Other causes of subnormal vision were pseudophakic ametropia, posterior capsular opacity and intraoperative complications like posterior capsular rent and vitreous loss. Postoperative astigmatism was the major cause of subnormal vision with greater astigmatism seen in the ECCE group. Therefore, procedures like smaller incision sutureless surgery and careful biometry are advocated to improve visual outcome and patient satisfaction.

Abstract:TOPIC: This systematic review and meta-analysis aims to clarify the association of cataract surgery with cognitive impairment and dementia in adults with cataracts.CLINICAL RELEVANCE: The association between vision impairment and cognitive decline is well established. However, the cognitive benefit of cataract surgery is less clear. Given the lack of cure, identifying potentially modifiable risk factors is key in caring for patients with cognitive deficits.METHODS: The study was conducted following Preferred Reporting Items for Systematic Review and Meta-analyses (PRISMA) guidelines. PubMed, Embase and the Cochrane Library were searched from inception to 11 October 2022 for studies reporting the association of cataract surgery with cognitive impairment and dementia. We pooled maximally adjusted Hazards Ratios (HR) for dichotomous outcomes and Ratio of Means (RoM) for continuous outcomes using a random effects model. Heterogeneity was examined using sensitivity and subgroup analyses. The quality of evidence was evaluated using the Newcastle-Ottawa Scale (NOS), Cochrane Risk-of-Bias Tool for Randomised Trials (RoB 2) and Grading of Recommendations, Assessment, Development and Evaluations (GRADE).RESULTS: This review included 24 articles comprising of 558,276 participants, of which 19 articles were qualitatively analysed. The bias of included studies ranged from low-moderate while GRADE extended from very low-low. Cataract surgery was associated with a 25% reduced risk of long-term cognitive decline compared to those with uncorrected cataracts (HR:0.75, 95%CI:0.72-0.78). This cognitive benefit was seen across various cognitive outcomes, andremained robust to comprehensive sensitivity analyses. Participants who underwent cataract surgery also had a similar risk of long-term cognitive decline as healthy controls without cataracts (HR:0.84, 95%CI:0.66-1.06). Additionally, cataract surgery was associated with a 4% improvement in short-term cognitive test scores assessing general cognitive function among participants with normal cognition (RoM:0.96, 95%CI:0.94-0.99), but no significant association was observed among participants with pre-existing cognitive impairment.CONCLUSION: Cataract surgery may be associated with a lower risk of cognitive impairment and dementia and cataract-associated vision impairment may be a modifiable risk factor for cognitive decline. Physicians should be aware of the cognitive sequelae of cataracts and the possible benefit of surgery. The cognitive benefit of cataract surgery should be further investigated in future randomized trials.

Abstract:Objective To analyze the impact on quality of life after cataract surgery related factors. Methods Selected265 cases of patients underwent routine phacoemulsification and intraocular lens implantation and surveyed preopera-tive and postoperative quality of life with low vision quality of life scale Chinese version(CLVQOL). Results After 3months CLVQOL score and distance vision, movement and light perception(A), regulation(B), reading and fine work(C), activities of daily living(D) compared with preoperative score were significantly improved( P < 0.05). Visual acuity improved values of different classification groups CLVQOL score and distance vision, movement and light perception(A), regulation(B), reading and fine work(C), activities of daily living(D) score, the differences were statistically sig-nificant(P <0.05), by Spearman correlation analysis, CLVQOL grading score and visual acuity values were positively correlated(r = 0.621, P < 0.05). Eye surgery, visual acuity improved value, glaucoma, diabetic retinopathy and cataract surgery were closely related to the quality of life(P < 0.05). Conclusion Cataract surgery can significantly improve patients' low vision quality of life, visual acuity improved value and quality of life is closely related to higher quality of life in patients glaucoma, diabetic retinopathy will affect the quality of life of patients improved.

Abstract:Objective To analyze impact factors for visual acuity improvement of senile cataract surgery in Tibet prospectively. Methods Of 278 cases were included. To study relations of visual acuity improvement between pre- and post-cataract surgery with patient age, nucleus classification, small incision cataract surgery (SICS) or Phacoemulsification (PHACO) and corneal edema. LogMAR chart was used in visual acuity analysis. Results 1. Univariate analysis: There was no significant difference between PHACO group and SICS group on visual acuity improvement (PHACO 0.92± 0.48, SICS 0.83± 0.46, P ＞0.05). Age group for 70- and 80- years-old had less improvement on visual acuity (P ＜0.05). Diffuse corneal edema had much more influence than central corneal edema on visual acuity improvement (0.52± 0.42, 0.70± 0.44, diffuse vs central, respectively). 2. Multivariate analysis: There were significant differences in age and location of corneal edema on visual acuity improvement (P ＜0.05). Conclusions There is no significant difference on visual acuity improvement between PHACO and SICS. Ophthalmologist can choose either PHACO or SICS for better rehabilitation of visual acuity, depending on area economics, his own experience and so on in Tibet. Visual acuity improvement in cataract surgery is significantly affected by the age of the patients. Since medication is deficient in Tebit, screening of cataract in Tibet is very important for detecting and treating cataract as early as possible. Diffuse corneal edema has much more influence than central edema on visual acuity improvement when corneal edema degrees are not different. It's important to avoid corneal endothelium damage in cataract surgery.

Abstract:Objective:To measure the restoration of dynamic visual acuity (DVA) after phacoemulsification and intraocular lens (IOL) implantation.Methods:In this prospective cohort study,23 cases of age-related cataract (46 eyes) were recruited from May 2012 to August 2013.The control group comprised 26 elder volunteers (52 eyes) with clear crystalline lenses.Binocular DVA at speeds of 15,30,60,and 90 degrees per second (dps) was measured by logarithmic visual acuity chart at baseline and 1 month postoperatively.Binocular contrast sensitivity (CS) at 1.5,3,6,12,and 18 cycles per degree (cpd) was tested under photopic conditions.Comparisons between groups were made with Wilcoxon signed ranks tests,paired t-tests,or Mann-Whitney U tests.Pearson correlation analysis was used to detect the correlation between DVA and CS.Results:Preoperative DVA at 15,30,60,and 90 dps was significantly lower in the cataract group than in the control group (Z=-5.692,-5.601,-5.639,-5.556;P＜0.001 each).Postoperatively,DVA at the four speeds improved significantly (Z=-4.110,-4.109,-4.110,-4.110;P＜0.001 each),which was comparable to the control group at each speed.Compared with static visual acuity (SVA),the improvements of DVA at 15,30,60,and 90 dps were significant (t=-3.596,-5.304,-6.220,-6.170;P＜0.01 each).Improvements of SVA and DVA at all speed levels correlated with improvements of CS at low spatialfrequency (rsvA-1.5 cpd=0.780,rSVA-3 cpd=0.839,r15 dps-1.5 cpd=0.746,r15 dps-3 cpd=0.692,r30 dps-1.5 cpd=0.775,r30 dps-3 cpd=0.719,r60 dps-1.5 cpd=0.718,r60 dps-3 cpd=0.590,r90 dps-1.5 cpd=0.636,r90 dps-3 cpd=0.561;P＜0.01).Improvements of DVA at 30 and 60 dps correlated with improvements of CS at medium spatial frequency (r30 dps-12cpd=0.537,r60dps-12 cpd=0.554;P＜0.01).Improvements of DVA at 90 dps correlated with improvements of CS at medium to high spatial frequency (r90 dps-12cpd=0.562,r90 dps-18cpd=0.542;P＜0.01).Conclusions:Cataract surgery effectively restored DVA in age-related cataract patients.There were significant correlations between clinical outcomes in DVA and CS based on temporal-spatial frequency.

Abstract:PURPOSE: To determine whether there is a need for second eye cataract surgery or whether cataract surgery in one eye provides sufficiently adequate vision.METHODS: The vision of 43 patients was assessed using a battery of clinical vision tests, performance-based functional vision tests, and quality of life questionnaires, both before and a few months after cataract surgery. Twenty-five patients underwent second eye surgery and 18 patients underwent first-eye surgery. To determine whether cataract surgery returned vision to normal levels, a control group of 25 subjects of a similar age with normal, healthy eyes was also assessed.RESULTS: Overall, greater improvements occurred in most aspects of vision after first eye surgery than after second eye surgery. However, second eye surgery provided similar improvements in mobility orientation and self-reported night driving to those after first eye surgery, and substantially greater improvements in stereoacuity and reductions in anisometropia.CONCLUSIONS: The study provides additional evidence to support the need for second eye cataract surgery. Second eye surgery may be particularly important to improve mobility orientation and the avoidance of falls.

Importance of optical coherence tomography before cataract surgery

Abstract:Purpose: To determine the importance of optical coherence tomography (OCT) in patients scheduled for cataract surgery who present with no pathologies in biomicroscopic fundus examination. Design: Retrospective study. Methods: In this study, the routine ophthalmologic examination of patients who were recommended cataract surgery was performed.Occult retinal pathologies were evaluated using OCT in patients without any pathologies in biomicroscopic fundus examination.According to whether retinal pathologies were detected on OCT, the patients were divided into two groups: normal and abnormal OCT.The findings of patients with retinal pathologies on OCT and their distribution according to age were also evaluated. Results: A total of 271 eyes from 271 patients were evaluated.The number of patients with retinal pathologies on OCT despite normal fundoscopic examination findings was 38(14.0%).Of these patients,15(39.4%) had an epiretinal membrane,10(26.3%) had age-related macular degeneration, eight(21%) had vitreomacular traction, two(5.2%) had a lamellar hole, and 1(2.6%) patient each had a full-thickness macular hole, an intraretinal cyst, and photoreceptor layer damage.The age distribution of the patients with retinal pathologies was as follows: two patients,<60 years; six patients,60-70 years;14 patients,70-80 years; and 16 patients,>80 years.The rate of patients aged > 70 years and above was 78.9%.There was no statistically significant difference between the normal and abnormal OCT groups in terms of age, gender, the presence of systemic diseases, visual acuity, central macular thickness, and cataract type or density(p > 0.05 for all). Conclusion: In one of seven patients evaluated, retinal pathologies were detected on OCT despite normal fundoscopic examination findings.OCT can be used to detect occult retinal pathologies that cannot be detected by biomicroscopic fundus examination before cataract surgery.

Abstract:Purpose To evaluate the relationship between cataract types and subjective visual function among patients with age-related cataract. Methods This was a prospective, multicenter, 831 Chinese patient-based, cross-sectional study. Patients were administered the Visual Function Index-14 (VF-14) and the 25-item National Eye Institute Visual Functioning Questionnaire (NEI-VFQ-25) to evaluate their subjective visual function. Lens Opacities Classification System III (LOCS III) was used to evaluate the type of cataract. Relationships among these parameters were analyzed. Results LOCS III cortical (C) and posterior subcapsular scores are negatively associated with VF-14 ( r = − 0.188, P < 0.01; r = − 0.146, P < 0.01) and total score of NEI-VFQ-25 ( r = − 0.223, P < 0.01; r = − 0.160, P < 0.01), respectively; LOCS III nuclear opalescence (NO) score is positively associated with VF-14 ( r = 0.087, P < 0.05) and total score of NEI-VFQ-25 ( r = 0.097, P < 0.05). In multiple linear regression, a decrease in the LOCS III C score is a significant predictor for improvement of the total score of NEI-VFQ-25 ( β = − 1.286, P < 0.05). In contrast, an increase in LOCS III NO score is a significant predictor for improvement of VF-14 ( β = 3.826, P < 0.01) and total score of NEI-VFQ-25 ( β = 4.618, P < 0.01). Patients with LOCS III C score ≤ 2 have higher VF-14 (49.38 versus 43.74, P < 0.01), total (80.73 versus 71.58, P < 0.01) and subscale scores of NEI-VFQ-25 than patients with LOCS III C score > 2. Conclusion Cortical cataract has adverse effects on subjective visual function, while mild-to-moderate nuclear cataract has positive effects. Furthermore, “LOCS III C score > 2” can be a potential cutoff as a reference for cataract surgery without self-assessing questionnaires.
Predictive Factors of Complications and Visual Outcomes after Pediatric Cataract Surgery: A Single Referral Center Study from Türkiye

Abstract:Objectives: To evaluate the predictive factors of complications and visual acuity outcomes in pediatric cataract patients. Materials and methods: This retrospective, observational clinical study included 80 eyes of 50 patients treated for pediatric cataracts between 2010 and 2020. The eyes were divided into Group I (congenital cataracts, n=38) and Group II (developmental cataracts, n=42). Group II was also divided into Group IIA (aphakic, n=21) and Group IIB (pseudophakic, n=21). The effects of the age, laterality, cataract morphology, intraocular lens implantation, preoperative nystagmus/strabismus, and intraoperative anterior hyaloid rupture on complications and final best-corrected visual acuity (BCVA; logMAR) were evaluated. Results: The median (interquartile range) age and follow-up time were 28 (5-79) months and 60 (29-84) months, respectively. There was a significant difference in mean final BCVA between Group I (0.79±0.46) and Group II (0.57±0.51) (p=0.047); however, no difference was observed between Group IIA and Group IIB (p=0.541). Having congenital cataract (p=0.045), preoperative nystagmus/strabismus (p=0.042), total/mature cataract (p<0.001), and postoperative complications (p=0.07) were significantly associated with final BCVA. However, in multivariate analysis, only total/mature cataract (β: 0.52, p<0.001) and having any complication (β: 0.24, p=0.018) were associated with final BCVA. Congenital cataract and intraoperative anterior hyaloid rupture were the only significant risk factors of postoperative complications on univariate (p=0.027 and p=0.003, respectively) and binary logistic regression analysis (odds ratio [OR]: 2.95 [95% confidence interval: 1.07-8.15], p=0.036 and OR: 4.28 [95% confidence interval: 1.55-11.77], p=0.005, respectively). Conclusion: Total/mature cataract and the presence of any postoperative complication adversely affected the final BCVA. Having a congenital cataract and intraoperative anterior hyaloid membrane rupture increased the risk of complications.

Abstract:Purpose: To evaluate small incision cataract surgery visual outcomes and associated risk factors in a tertiary eye care center in Bhutan. Methods: This was a hospital-based prospective longitudinal study. A total of 310 patients who met the eligibility criteria were included in the study. Results: The mean age of the patients was 68.36 (SD: 12.74 years), and 157 (50.6%) were male. 285 (91.94%) of the cataract-operated eyes had best-corrected vision greater than 6/60, while 25 (8.06%) of the eyes had vision less than 6/60. 47 individuals (15.16%) had pre-existing ocular co-morbidity. Intraoperative complications were seen in 19 eyes (6%). At the 6-week follow-up visit, based on best-corrected visual acuity (BCVA), 90.65% had a good outcome. Analysis of multiple logistic regression showed that patients with preoperative ocular comorbidities (OR 50.92; 95% CI 10.23, 253.37) and those with operative complications (OR 16.59; 95% CI 3.54, 77.70) were significantly associated with poor visual outcome. Conclusions: This study shows that cataract surgery can restore good visual acuity. However, patients should be followed up on for a longer period, and greater emphasis should be paid to monitoring outcomes after cataract surgery.
