Abstract:Introduction and Objectives Metabolic associated fatty liver disease (MAFLD) is a metabolic and liver disorder with a prevalence of 30% and with significant potential to progress to hepatic steatosis, nonalcoholic steatohepatitis (NASH), fibrosis, cirrhosis and hepatocellular carcinoma. Visceral obesity is a risk factor for MAFLD and associated with disease severity. Previous studies have shown that anthropometric measures such as body mass index (BMI), neck circumference (NC), waist circumference (WC), waist-hip ratio (WHR) and body fat percentage (%BF) are predictors of MAFLD. We aimed to assess the prevalence of MAFLD and the role of anthropometric measurements as predictors and its association with liver fibrosis. Materials and Methods Adults over 18 years old assisted in Antonio Pedro´s University Hospital, with risk of MAFLD (pre-diabetes, diabetes mellitus, metabolic syndrome, and obesity). Patient's clinical information, anthropometric, metabolic profiles were assessed. Non-invasive assessment of MAFLD was performed by ultrasound, elastography and bioelectrical impedance analysis (BIA). Results The group consisted of 73 subjects with 80.8% females. All data are presented as median (IQR) or n (%). Median age was 63 (53-67) years. The prevalence of obesity was 57.5%. Higher diabetes and dyslipidemia (69.8% and 65.7%, respectively). Hepatic steatosis was present in 79.4% of patients. Higher averages for the anthropometric measures that reflect visceral body fat were observed: NC 37.1 cm; WC 104.5 cm; BMI 31.4 kg/m2 in individuals with hepatic steatosis than those without the disease (NC median 35.7 cm; WC 95.5 cm; BMI 25.7 kg/m2). The frequency of liver fibrosis (F 2) was 23.2%. Anthropometric measures had no association with fibrosis, except % BF measure calculated by the BIA (p< 0.02). Conclusions Anthropometric measurements of visceral obesity, demonstrate to be an important risk factor for MAFLD. BIA a non-invasive and easily carried out method, could be useful as a screening test to identify individuals with MAFLD.

Abstract:Background/Objectives: Approximately 25% of the world's population and more than 60% of patients with type 2 diabetes (T2D) have metabolic-dysfunction-associated steatotic liver disease (MASLD). The association between these pathologies is an important cause of morbidity and mortality in Brazil and worldwide due to the high frequency of advanced fibrosis and cirrhosis. The objective of this study was to determine the epidemiologic and clinical-laboratory profile of patients with T2D and MASLD treated at an endocrinology reference service in a state in northeastern Brazil, and to investigate the association of liver fibrosis with anthropometric and laboratory measurements. Methods: A cross-sectional study was performed in a specialized outpatient clinic with 240 patients evaluated from July 2022 to February 2024, using a questionnaire, physical examination, laboratory tests, and liver elastography with FibroScan®. Results: Estimates showed that women (adjusted OR = 2.69, 95% CI = 1.35–5.35, p = 0.005), obesity (adjusted OR = 2.23, 95% CI = 1.22–4.07, p = 0.009), high GGT (adjusted OR = 3.78, 95% CI = 2.01–7.14, p < 0. 001), high AST (adjusted OR = 6.07, 95% CI = 2.27–16.2, p < 0.001), and high ALT (adjusted OR = 3.83, 95% CI = 1.80–8.11, p < 0.001) were associated with the risk of liver fibrosis even after adjusted analysis. Conclusions: The study findings suggested that female sex and BMI were associated with an increased risk of liver fibrosis, highlighting the importance of comprehensive evaluation of these patients. In addition, FIB-4 and MAF-5 provided a good estimate of liver fibrosis in our population and may serve as a useful tool in a public health setting with limited resources.

Abstract:Metabolic dysfunction-associated steatotic liver disease (MASLD) is often an asymptomatic condition found in high prevalence in patients with type 2 diabetes. The fibrosis 4 (FIB-4) score, a validated tool for risk stratifying patients for fibrosis, can aid in early recognition and diagnosis of MASLD. However, FIB-4 is underutilized in the outpatient endocrinology setting. The aim of our study was to assess current rates of recognition and referral of diabetic patients at risk for MASLD at an outpatient endocrinology clinic, and to enhance these rates via an education-based multi-faceted implementation strategy to improve FIB-4 usage and referral to Hepatology in high-risk patients.

Abstract:Introduction and Objectives Metabolic fatty liver disease is the most important cause of chronic liver disease with estimated global prevalence of 25% and a latin America prevalence of about 44%. This study aimed to evaluate frequency of fibrosis in risk patients for metabolic liver disease in tertiary center, stratifying grades and presence of risk factors . Material and Methods Cross sectional study after informed consent. Clinical examination, bioimpedance and non invasive approach for fibrosis diagnosis by biomarkers and ultrasonography elastography were performed. Results Sixty nine patients were included, with 82% women and median age of 63 years. The BMI median was 31 kg/m². Significative fibrosis was present in 30,4% of patients by ultrasonography elastography. Twenty five percent of patients were classified as indeterminated zone fibrosis by FIB 4 and 8,7% with definitely advanced fibrosis risk by Fib4. APRI test results were, respectively, 18,5% and 2,1 %. Fibrosis risk was not associated to steatosis grade but to body corporal fat percentage and serum ferritin. Conclusions Our study evidenced approximately a tier of tertiary hospital patients at risk for metabolic fatty liver disease to have fibrosis using ultrasonography elastography. Biomarkers (FIB4 and APRI tests) had a lower frequency for fibrosis identification. Risk factors for fibrosis identified were body fat corporal percentage and serum ferritin.

Abstract:Abstract Background and Aims Severity of fibrosis is the driver of liver-related outcomes in metabolic dysfunction-associated steatotic liver disease (MASLD), and non-invasive testing such as fibrosis-4 (FIB-4) score is utilized for risk stratification. We aimed to determine if primary care patients at risk for MASLD and advanced fibrosis were evaluated with subsequent testing. A secondary aim was to determine if at-risk patients with normal aminotransferases had advanced fibrosis. Methods Primary care patients at increased risk for MASLD with advanced fibrosis ( n = 91,914) were identified using previously established criteria. Patients with known alternative/concomitant etiology of liver disease or cirrhosis were excluded. The study cohort included patients with calculated FIB-4 score in 2020 ( n = 52,006), and stratified into low, indeterminate, and high likelihood of advanced fibrosis. Among those at indeterminate/high risk, rates of subsequent testing were measured. Results Risk stratification with FIB-4 characterized 77% ( n = 40,026) as low risk, 17% ( n = 8847) as indeterminate, and 6% ( n = 3133) as high risk. Among indeterminate/high-risk patients ( n = 11,980), 78.7% ( n = 9433) had aminotransferases within normal limits, 0.95% ( n = 114) had elastography, and 8.2% ( n = 984) were referred for subspecialty evaluation. Conclusion In this cohort of primary care patients at risk for MASLD with fibrosis, the FIB-4 score identified a substantial proportion of indeterminate/high-risk patients, the majority of which had normal aminotransferase levels. Low rates of subsequent testing were observed. These data suggest that a majority of patients at increased risk for liver-related outcomes remain unrecognized and highlight opportunities to facilitate their identification.

Abstract:Background: Metabolic dysfunction-associated steatotic liver disease (MASLD), formerly known as non-alcoholic fatty liver disease (NAFLD), is a global cause of chronic liver disease. The prevalence of MASLD is high in patients with type 2 diabetes mellitus (T2DM). Various non-invasive tools such as the fibrosis-4 index (FIB-4) and NAFLD fibrosis score (NFS), liver ultrasound, and FibroScan can aid in the detection of liver fibrosis in MASLD, while the Hamaguchi ultrasound-based liver grading system has demonstrated high sensitivity and specificity comparable to liver biopsy. Objective: We assessed the frequency of MASLD in patients with T2DM using the liver ultrasound Hamaguchi score and the accuracy of NFS and Fib-4 in identifying MASLD. Patients and methods: We retrospectively collected data and reviewed the charts of all patients with T2DM who underwent liver ultrasound and laboratory tests during the past 5 years. Results: A total of 6,214 medical records were screened, and only 153 patients (68.6% women; mean age, 59 ± 12.2 years) fulfilled the selection criteria. MASLD was diagnosed using the Hamaguchi grading criteria in 45.1% of patients. A high/intermediate NFS had a higher sensitivity (79.7%) for diagnosing MASLD with a specificity of 10.7%, while a high/intermediate Fib-4 score showed only 30.4% sensitivity but a higher specificity of 54.8%. Conclusion: Our study indicates that MASLD is frequent in patients with T2DM, and clinical prediction tools such as NFS and Fib-4 can be applied in clinic/primary care settings with variable results.

Abstract:Most patients (96.8%) in the nonalcoholic fatty liver disease (NAFLD) cohort fulfilled the new criteria for metabolic dysfunction‐associated steatotic liver disease (MASLD). The diagnostic performance of FIB‐4 and the NAFLD fibrosis score (NFS) for advanced fibrosis between MASLD and NAFLD was comparable. Aims We aimed to explore the extent to which individuals previously diagnosed with nonalcoholic fatty liver disease (NAFLD) meet the criteria fulfilled with the new nomenclature, metabolic dysfunction‐associated steatotic liver disease (MASLD), within an Asian primary clinic cohort. Additionally, we assessed the reliability of the diagnostic performance of FIB‐4 and NAFLD fibrosis score (NFS) for MASLD within the primary clinic cohort. Methods This retrospective cross‐sectional study included participants who underwent magnetic resonance elastography and abdominal ultrasonography during their health checkups at nationwide health promotion centers (n = 6740). Results The prevalence rates of NAFLD and MASLD diagnosed based on ultrasonography results were 36.7% and 38.0%, respectively. Notably, 96.8% of patients in the NAFLD cohort fulfilled the new criteria for MASLD. A small proportion of patients with NAFLD (n = 80, 3.2%) did not meet the MASLD criteria. Additionally, 168 patients (6.6%) were newly added to the MASLD group. The areas under the receiver operating characteristic curves for diagnosing advanced hepatic fibrosis for FIB‐4 (0.824 in NAFLD vs. 0.818 in MASLD, p = 0.891) and NFS (0.803 in NAFLD vs. 0.781 in MASLD, p = 0.618) were comparable between the MASLD and NAFLD groups. Furthermore, the sensitivity, specificity, positive predictive value, and negative predictive value of FIB‐4 and NFS for advanced fibrosis in MASLD were also comparable to those in NAFLD. Conclusions Most patients (96.8%) previously diagnosed with NAFLD fulfilled the new criteria for MASLD in an Asian primary clinic cohort. Diagnostic performance of FIB‐4 in the MASLD cohort demonstrated satisfactory results.

Abstract:A dogma in the field of circadian rhythms is that in order to keep accurate time, pacemakers that generate such rhythms must be relatively independent of changes in the external and internal environment. While it is true that the period of circadian oscillators is conserved within a narrow range, regardless of alterations in the external and internal envi ronment, numerous perturbations have now been found that can change the period and/or induce a phase shift in circadian pacemakers. Many of these perturbations also alter the overall level of activity and/or metabolic state of the organism. In 1960, Aschoff suggested that alterations in the "level of excitement" may induce changes in circadian clocks. Although little attention has been given to this hypothesis over the past three decades, recent findings support its validity and open new avenues for studying the function and organization of circadian clock systems.

Abstract:Metabolic dysfunction-associated steatotic liver disease (MASLD) is the fastest-growing indication for liver transplantation in Western countries, including the US. With the escalating obesity epidemic, MASLD incidence is projected to continue rising. Due to its often asymptomatic nature, MASLD frequently goes undiagnosed. Early detection is critical for improving outcomes, particularly with the emergence of newly FDA-approved agents. To facilitate early detection, GI and endocrine societies advocate for MASLD screening in outpatient settings using non-invasive tools such as the FIB4 score and liver elastography. This study aimed to evaluate and improve the adherence of primary care clinic (PCP) providers to MASLD screening guidelines.

Abstract:Background & Aims Metabolic dysfunction-associated steatotic liver disease (MASLD) is a growing public health concern. The disease is silent, and its diagnosis is often delayed. Inflammatory markers constitute an interesting tool to act as subrogate, non-invasive markers. This study aimed to evaluate the changes of inflammatory markers throughout a two-year dietary intervention in subjects presenting MASLD, to determine which of the markers are suitable to predict the disease, and act as a customizing tool for MASLD ́s dietary treatment. Methods Ninety-eight subjects with MASLD and forty-five controls from the Fatty Liver in Obesity (FLiO) Study were analyzed. MASLD was diagnosed and graded by ultrasound. The MASLD subjects were randomly assigned to two different dietary strategies, the American Heart Association (AHA diet) or a dietary strategy based on the Mediterranean pattern, which was specially designed for the study (FLiO diet), and then followed for two years. Hepatic status was additionally assessed through Magnetic Resonance Imaging (MRI), elastography, and determination of transaminases. Results & Discussion Inflammatory markers improved throughout the intervention in the MASLD subjects and managed to reach similar levels to controls, especially at 6 and 12 months. Additionally, leptin, adiponectin, M30, and LECT2 managed to significantly diagnose the disease at all time marks of the intervention, making them candidates for subrogate non-invasive markers of the disease. Moreover, baseline chemerin, leptin, LECT2, and M65 were used to build a predictive score to achieve greater weight loss, and therefore, which strategy could be more useful for MASLD ́s treatment. The predictive score was significantly able assign a specific diet to 55% of the study participants, meaning that the remaining 45% could achieve the same amount of weight loss following either diet equally. Conclusion Inflammatory markers constitute a potential non-invasive tool to be used in MASLD screening and could also constitute an interesting tool for MASLD's treatment customization, being able to predict the effectiveness of a dietary strategy based on the initial inflammatory state of each subject. Abstract:Metabolic dysfunction-associated fatty liver disease (MASLD), with a prevalence of 30% of adults globally, is considered a multifactorial disease. There is a lack of effective non-invasive methods for accurate diagnosis and monitoring. Therefore, this study aimed to explore associations between changes in circulating miRNA levels, inflammatory markers, and depressive symptoms with hepatic variables in MASLD subjects and their combined potential to predict the disease after following a dietary intervention. Biochemical markers, body composition, circulating miRNAs and hepatic and psychological status of 55 subjects with MASLD with obesity and overweight from the FLiO study were evaluated by undergoing a 6-, 12- and 24-month nutritional intervention. The highest accuracy values of combined panels to predict the disease were identified after 24 months. A combination panel that included changes in liver stiffness, high-density lipoprotein cholesterol (HDL-c), body mass index (BMI), depressive symptoms, and triglycerides (TG) yielded an AUC of 0.90. Another panel that included changes in hepatic fat content, total cholesterol (TC), miR15b-3p, TG, and depressive symptoms revealed an AUC of 0.89. These findings identify non-invasive biomarker panels including circulating miRNAs, inflammatory markers, depressive symptoms and other metabolic variables for predicting MASLD presence and emphasize the importance of precision nutrition in MASLD management and the sustained adherence to healthy lifestyle patterns.

Abstract:Background: Inflammatory bowel disease (IBD) is characterized by persistent inflammation and is often associated with metabolic dysfunction-associated steatotic liver disease (MASLD). IBD patients are at risk of developing MASLD due to shared risk factors such as gut dysbiosis and systemic inflammation. The new MASLD nomenclature emphasizes the link between liver steatosis and cardiometabolic comorbidities. However, the prevalence of MASLD in IBD patients remains poorly explored. The main aim of this cross-sectional study is to assess the prevalence of ultrasound (US) and the clinical features of MASLD in patients with IBDs. Materials and Methods: We conducted a retrospective study enrolling 272 Italian IBD patients attending Renato Dulbecco Teaching Hospital in a period between 1 January 2021 and 31 December 2023. MASLD was diagnosed based on the presence of liver steatosis with cardiometabolic risk factors, using established guidelines. Demographic, clinical, and laboratory data were collected and analyzed. Statistical significance was determined at a p-value < 0.05. Results: Of the 272 IBD patients, 6% had non-alcoholic fatty liver disease (NAFLD), while 18% had MASLD. Patients with IBD-MASLD were significantly older, had higher body mass index, waist circumference, and triglyceride levels, and were more likely to have type 2 diabetes mellitus and hypertension compared to those with IBD-NAFLD. IBD-MASLD patients also showed higher disease activity scores and required more frequent surgical interventions. Bivariate logistic regression revealed triglyceride levels as a significant predictor of MASLD in IBD patients. Conclusions: MASLD is more prevalent in IBD patients, highlighting the importance of early detection of liver steatosis in this at-risk population. The association between MASLD and cardiometabolic risk factors underscores the need for a multidisciplinary approach to manage these patients effectively. Further studies in larger cohorts are necessary to confirm these findings and explore the pathophysiological mechanisms involved.

Abstract:Objective Metabolic dysfunction‐associated steatotic liver disease (MASLD) is closely associated with obesity. We aimed to assess the impact of obesity on the performance of different noninvasive tests, including liver stiffness measurement (LSM) and Agile3+ (A3+), to detect advanced fibrosis (AF) in a population of patients with MASLD encompassing a wide range of BMI values. Abstract:Introduction Liver fibrosis is the strongest predictor of liver-related events (LRE) in patients with MASLD. The enhanced liver fibrosis (ELF) score is a composite of direct fibrosis biomarkers that reflect extracellular matrix turnover. ELF test shows high diagnostic accuracy for advanced liver fibrosis in patients with MASLD but its utility as a prognostic biomarker is still unclear. Aim The aim of our study is to compare the prognostic effectiveness of ELF, FIB4 and liver histology in patients with MASLD. Materials and Methods We retrospectively enrolled 289 patients with MASLD. ELF score was determined using a serum sample collected at baseline. FIB4 computation and liver biopsy were performed at baseline. The primary outcome was a composite endpoint of all-cause mortality, hepatocellular carcinoma, liver transplantation or cirrhosis complications (ascites, variceal bleeding, hepatic encephalopathy, MELD≥15). Considering existing literature cut-offs, subjects were stratified accordingly to ELF (≤9.8, 9.8-11.3, ≥11.3), FIB-4 (<1.3, 1.3-2·67, >2.67) and histology (F0-2, F3, F4) to assess risk of occurrence of primary outcome. Results We included data of 289 patients (30.4% female, median age 50y [IQR 39-58]). After a median follow-up of 41 months [IQR 21-68], the composite endpoint was observed in 11.8% of patients. There was a stepwise increase in the incidence of primary outcome according with ELF ≤9.8 (0.5%), 9.8-11.2 (14.5%), ≥11.3 (69.7%). Survival curves for pairwise comparisons between groups showed significant differences according to pre-defined histological and NITs stratification (p<0.05). At multivariate Cox regression analysis, ELF and liver histology were significant predictors of the primary outcome after adjusting for gender, diabetes, age, BMI. (ELF >11.2vs<9.8 HR 135.4 [95%CI 15.9-1149.0 p<0.01], 9.8-11.2vs<9.8 HR 22.5 [95%CI 2.7-183.9 p<0.01]) (F4vs0-2 HR 216.6 [95%CI 23.5-1999.2 p<0.01], F3vs0-2 HR 5.44 [95%CI 1.0-30.9 p=0.05]). Conclusions ELF, a simple non-invasive blood test, performed as well as histologically assessed fibrosis in predicting clinical outcomes and should be considered as alternative to liver biopsy for prognostic assessment in patients with MASLD.

Abstract:Current guidelines recommend screening for significant fibrosis using Fibrosis-4 (FIB-4) testing and/or liver stiffness measurement (LSM) by transient elastography (TE) in patients at risk for metabolic dysfunction associated steatotic liver disease (MASLD); i.e. those with obesity and diabetes mellitus (DM). Strategies to promote MASLD related fibrosis screening are understudied. We designed a quality improvement initiative to examine the impact of availability of LSM as a point-of-care (POC) test in an urban, safety-net weight management and diabetes clinic on MASLD related fibrosis screening.

Abstract:Fractional thyroidal and renal radioiodine clearance and renal 127I excretion were measured in 152 euthyroid subjects. Thyroidal 127I accumulation (A) was calculated from the basic formula of Stanbury et al. with short-term thyroidal and extrathyroidal radioiodine clearance measurements substituted for 48-hr thyroidal radioiodine uptake. Appropriate corrections were included for diurnal variations in renal radioiodine clearance, lag time in urine collections, and losses of iodine in uncollected urine and in sweat and stools. Thyroxine 131I degradation (H4) was measured by whole body counting techniques and compared with calculated thyroidal 127I accumulation in 29 subjects on a mean calculated daily iodine intake of 387 μg. Significant negative correlation of thyroidal radioiodine uptake and renal iodine excretion was observed (p < 0.001). Abstract:Metabolic dysfunction-associated steatotic liver disease (MASLD) is defined by the presence of hepatic steatosis, detected on ultrasonography (US) imaging or histology, and at least one of criteria for Metabolic Syndrome diagnosis. Simple non-invasive tests (NITs) have been proposed as an acceptable alternative when US and biopsy are not available or feasible but have not been validated for MASLD.
