Abstract:Objective To investigate the cause and significance of reintubation or not in patients of unplanned endotracheal extubation(UEE).Methods Compare and analyze of patients who were reintubated and who were not reintubated after UEE.Results There were significant differences between the two groups for the ventilator modes.The average reintubation rate when SIMV (synchronized intermittent mandatory ventilation) and CMV (control mandatory ventilation) were used (80.8%) in patients was significantly higher than that of CPAP(continous positive airway pressure) and T-piece used(31.8%) ( P 0 01).The airway secretion scale was higher while the oxygenation index (PaO 2/FiO 2) was lower in the reintubated group( P 0 01).Glasgow Coma Scale showed no significant difference.Conlusion Reintubation in UEE patients should not be considered mandatory.It is important to assess the general condition of patients,especially the ventilator modes,airway secretion,oxygenation index prior to determination of reintubation.GCS have little significance. [

Abstract:Purpose: Although several studies assess unplanned extubation (UE) in children, few have addressed determinants of UE and factors associated with reintubation in a case-controlled manner. We aimed to identify the risk factors and outcomes associated with UE in a pediatric intensive care unit. Methods: Cases of UE were randomly matched with control patients at a ratio of 1:4 for age, severity of illness, and admission diagnosis. For cases and controls, we also collected data associated with UE events, reintubation, and outcomes. Results: We analyzed 94 UE patients (0.75 UE per 100 intubation days) and found no differences in demographics between the 2 groups. Logistic regression revealed that patient agitation (odds ratio [OR]: 2.44; 95% confidence interval [CI]: 1.28-4.65), continuous sedation infusion (OR: 3.27; 95% CI: 1.70-6.29), night shifts (OR: 9.16; 95% CI: 4.25-19.72), in-charge nurse experience 5 (OR: 76.9; 95% CI: 16.79-352.47) were associated with UE. Risk factors for reintubation after UE included prior level of sedation (COMFORT score 5 (OR: 9.32; 95% CI: 2.45-35.48). Conclusions: This case–control study showed that both patient- and nurse-associated risk factors were related to UE. Risk factors associated with reintubation included lower levels of consciousness, copious secretions, and higher OI. Further evidence-based studies, including a larger sample size, are warranted to identify predisposing factors in UEs.

Abstract:Background: Unplanned extubations (UEs) continue to be one of the most common adverse events in the neonatal intensive care unit (NICU). Management of endotracheal tubes (ETTs) can be particularly challenging in neonates due to the unique needs and physical characteristics of this patient population. Purpose: The purpose of this quality improvement project was to decrease the rate of UEs from 0.76 to less than 0.5 per 100 ventilator days in an urban level III NICU in the Midwest, United States. Methods: A newly formed interprofessional team created an evidence-based, standardized, bedside nurse-led care bundle for intubations and ETT care in the NICU. This project also created standardized, clear, closed-loop communication for the transition of bedside staff at shift change. Results: The UE rate decreased from 0.76 to 0 per 100 ventilator days, reaching the goal of less than 0.5 per 100 ventilator days, during the 10-week project implementation period from December 2021 to February 2022. Implications for practice and research: Many NICUs focus on reducing UEs due to the impact on healthcare resource utilization, acute complications, and long-term outcomes for infants. The development of a standardized, nurse-led care bundle for ETTs decreased the rate of UEs. Future research is needed to study the potential for generalization to different units and beyond the scope of the neonatal population.
The Current Status of Unplanned Extubation in Pediatric Intensive Care Units

Abstract:Objectives: To describe the current status of unplanned extubation(UEX) in pediatric intensive care units(ICUs), to analyze their causes and to explore the coutermeasures. Methods: A prospective study by convenience sampling was delivered and self-designed record form was used to investigate patients with tracheal intubation in three ICUs in a Grade Three Class A Children's hospital in Shanghai from October 2010 to May 2011. Results: Totally 247 patients were recorded effectively and 27 children (10.9%)underwent 33 UEXs. The UEX rate was 11.2% and 1.77 events per 100 intubation days. The reintubation rates of neonates and general patients was 66.7% and 88.9%, respectively. The risk factors of UEX were associated with working years of on duty nurses, nursing manpower, children's age, physical constrains and sedation usage. Conclusions: We should strengthen young nurses' training to improve their assessment and predictability abilities, increase nursing manpower, use sedation properly, and restrict body effectively to reduce UEX.

Abstract:Objective: To compare reintubation rates after planned extubation and unplanned extubation (UE) in patients in neonatal intensive care units (NICUs), to analyse risk factors for reintubation after UE and to compare outcomes in patients with and without UE. Design: Prospective, observational study nested in a randomised controlled trial (SEPREVEN/Study on Epidemiology and PRevention of adverse EVEnts in Neonates). Outcomes were expected to be independent of the intervention tested. Setting: 12 NICUs in France with a 20-month follow-up, starting November 2015. Patients: n=2280 patients with a NICU stay >2 days, postmenstrual age ≤42 weeks on admission. Interventions/exposure: Characteristics of UE (context, timing, sedative administration in the preceding 6 hours, weaning from ventilation at time of UE) and patients. Main outcome measures: Healthcare professional-reported UE rates, reintubation/timing after extubation, duration of mechanical ventilation, mortality and bronchopulmonary dysplasia (BPD). Results: There were 162 episodes of UE (139 patients, median gestational age (IQR) 27.3 (25.6-31.7) weeks). Cumulative reintubation rates within 24 hours and 7 days of UE were, respectively, 50.0% and 57.5%, compared with 5.5% and 12.3% after a planned extubation. Independent risk factors for reintubation within 7 days included absence of weaning at the time of UE (HR, 95% CI) and sedatives in the preceding 6 hours (HR 1.93, 95% CI 1.04 to 3.60). Mortality at discharge did not differ between patients with planned extubation or UE. UE was associated with a higher risk of BPD. Conclusion: In the SEPREVEN trial, reintubation followed UE in 58% of the cases, compared with 12% after planned extubation. Trial registration number: NCT02598609.

Abstract:In order to determine the risk factors and outcomes of ventilator-associated pneumonia (VAP) in the neonatal intensive care unit (NICU), a retrospective cohort study was conducted on 259 patients who were ventilated > 48 h. Clinical characteristics and risk factors were compared and non-conditional logistic regression analysis was performed to determine independent predictors for VAR There were 52 episodes of VAP (20.1 %). The main pathogens were G- bacterium (82.1%, 23/28). Hospital stay in the VAP group was 19.9 +/- 5.9 vs. 16.7 +/- 7.2 days in controls (P < 0.01). The mortality rate of the VAP group was 13.5% (7/52) vs. 12.1 % in controls (P > 0.05). By logistic regression analysis the following independently predicted VAP: re-intubation (OR 5.3, 95% Cl 2.0, 14.0), duration of mechanical ventilation (OR 4.8, 95% CI 2.2, 10.4), treatment with opiates (OR 3.8, 95% Cl 1.8, 8.5) and endotracheal suctioning (OR 3.5, 95% Cl 1.6, 7.4). VAP occurred at significant rates among mechanically ventilated NICU patients and is associated with care procedures. The risk factors of neonatal VAP were re-intubation, duration of mechanical ventilation, treatment with opiates and endotracheal suctioning. Additional studies are necessary to develop interventions to prevent neonatal VAP.

Abstract:Background: We aimed to determine the epidemiology and outcomes of unplanned extubation (UE), both accidental and self-extubation, in ICU. Methods: A multicentre prospective cohort study was conducted in 47 French ICUs. The number of mechanical ventilation (MV) days, and planned and unplanned extubation were recorded in each center over a minimum period of three consecutive months to evaluate UE incidence. Patient characteristics, UE environmental factors, and outcomes were compared based on the UE mechanism (accidental or self-extubation). Self-extubation outcomes were compared with planned extubation using a propensity-matched population. Finally, risk factors for extubation failure (re-intubation before day 7) were determined following self-extubation. Results: During the 12-month inclusion period, we found a pooled UE incidence of 1.0 per 100 MV days. UE accounted for 9% of all endotracheal removals. Of the 605 UE, 88% were self-extubation and 12% were accidental-extubations. The latter had a worse prognosis than self-extubation (34% vs. 8% ICU-mortality, p < 0.001). Self-extubation did not increase mortality compared with planned extubation (8% vs. 11%, p = 0.075). Regardless of the type of extubation, planned or unplanned, extubation failure was independently associated with a poor outcome. Cancer, higher respiratory rate, lower PaO2/FiO2 at the time of extubation, weaning process not-ongoing, and immediate post-extubation respiratory failure were independent predictors of failed self-extubation. Conclusion: Unplanned extubation, mostly represented by self-extubation, is common in ICU and accounts for 9% of all endotracheal extubations. While accidental extubations are a serious and infrequent adverse event, self-extubation does not increase mortality compared to planned extubation.

Abstract:This paper investigates the suitability of two-dimensional (2D) finite-element method (FEM) simulations to model the quasi-static bending-mode response of cantilever magnetoelectric (ME) sensors. We compute the deformation of the cantilever due to an applied magnetic field across the magnetostrictive (MS) layer as well as the generated voltage across the piezoelectric (PE) layer by solving a system of coupled linear elastostatic/elastodynamic and electrostatic/magnetostatic equations. In the 2D FEM formulation a plane-stress boundary condition is employed. Both 2D and 3D FEM results are compared for varying cantilever length and width. For cantilevers with length >> height >> width good agreement is obtained between 2D and 3D results. For cantilevers obeying the condition length >> width >> height, a systematic deviation occurs between 2D and 3D results as the plane-stress condition does not model the situation in the center of the cantilever. For a length:width:height ratio of 25:5:1, a difference in generated voltage of ~12% is obtained for the Terfenol-D-PZT-Si material system. Especially for cantilevers with high aspect ratios, we show that 2D FEM simulations are sufficient to investigate the ME properties.

Abstract:Journal of Intensive Care Medicine, Ahead of Print. Unplanned extubations (UE) frequently occur in critical care units. These events are precipitated by many risk factors and are associated with adverse outcomes for patients. We reviewed the current literature to examine factors related to UE and presented the analysis of 41 articles critical to the topic. Our review has identified specific risk factors that we discuss in this review, such as sedation strategies, physical restraints, endotracheal tube position, and specific nursing care aspects associated with an increased incidence of UE. We recommend interventions to reduce the risk of UE. However, we recommend that bundled rather than a single intervention is likely to yield higher success, given the heterogeneity of factors contributing to increasing the risk of UE.

Situations and Risk Factors of Unplanned Extubation of Nasogastric Tubes in Inpatients: A Retrospective Study

Abstract:Objective: The aim of the study is to understand whether the risk factors of the unplanned extubation (UE) of nasogastric (NG) tubes vary among different inpatient situations. Methods: Inpatients who experienced UE between 2009 and 2022 at a medical center were selected, and electronic medical records were used to collect patient background data and their conditions during UE. A total of 302 patients were included in our analysis. Results: Conscious patients were at greater risk of UE when coughing, scratching their nose, blowing their nose, or sneezing than those who were confused (odds ratio [OR] = 0.07, P < 0.001) and those who were drowsy or comatose (OR = 0.15, P = 0.026). During activity, repositioning, bathing, or changing incontinence pads, the risk of UE was higher in patients whose hands were not restrained at the time of UE than in those whose hands were restrained (OR = 0.05, P = 0.004), higher in those with companions than in those without companions (OR = 7.78, P = 0.002), and higher in those with longer NG tube placement time (OR = 1.05, P = 0.008). Accidental extubation (OR = 2.62, P = 0.007) occurred more frequently during activity, repositioning, bathing, or changing incontinence pads. Conclusions: There is an increased risk of UE in conscious patients during activity, repositioning, bathing, or changing incontinence pads. Patients inserted with an NG tube for a longer period of time were at greater risk of accidental extubation during activity, repositioning, bathing, or changing incontinence pads irrespective of whether a companion could aid them if their hands were or were not restrained.

Abstract:Background Unplanned extubation is the removal of an endotracheal tube accidently during procedural activities or by the action of the patient. It is one of the commonly reported complications among mechanically ventilated patients in the intensive care unit. This study aimed to assess the magnitude and associated factors of unplanned extubation in intensive care units at referral hospitals in Addis Ababa, Ethiopia, 2021. Methods Institutional based prospective observational study was conducted on 317 intubated patients in the intensive care unit at referral hospitals of Addis Ababa, Ethiopia, from January 8, 2021–May 9, 2021. Data were collected using a structured questionnaire. Descriptive statics were expressed in percentages and presented with tables and figures. Both Bivariable and multivariable logistic analysis was done to identify factors associated with unplanned extubation in intensive care unit. P < 0.05 with 95% CI was set as Statistical significance. Result The prevalence of unplanned extubation in this study was 19.74%. Being male (AOR = 3.132, 95%CI: 1.276–7.69), duration of intubation <5days (AOR = 2.475, 95% CI: 1.039–5.894), managed by junior resident (AOR = 5.25, 95% CI: 2.125–12.969), being physically restrained (AOR = 4.356, 95%CI: 1.786–10.624), night shift (AOR = 3.282, 95%CI:1.451–7.424)and agitation (AOR = 4.934,95%CI:1.934–12.586) were significantly contribute to the occurrence of unplanned extubation. Conclusion and recommendation: This study showed that the prevalence of unplanned extubation was high in the intensive care unit. We suggest to intensive care unit staff to give special attention to early intubated patients, especially male individuals and the stakeholders of hospitals should rearrange the time of shift and physician schedules in the intensive care unit.
The incidence and influencing factors of unplanned extubation of gastric tube in neonates

Abstract:Objective To investigate the incidence of unplanned extubation(UEX) of gastric tube in the neonates,and to analyze the causes and explore the countermeasures.Methods The clinical data of 220 neonates with gastric tube from October 2010 to March 2011 in a children hospital in Shanghai were recorded.Results The incidence of UEX was 18.6 percent and 1.86 per 100 tube days in the neonates with gastric tube.The reintubation rate was 80.5 percent.Logistic regression analysis showed that body restraint and the fixation method of gastric tube were the influencing factors of UEX.Conclusion It is suggested that frequent observation,effective body restraint,proper fixation method and standardized nursing care are helpful to reduce the incidence of unplanned extubation in neonates with gastric tube.

Abstract:Patients require admission to intensive care unit with endotracheal intubation and mechanical ventilation for various reasons. It can last for a short duration or could be prolonged. The endotracheal tube is removed once the underlying problem is resolved and is called planned extubation. Sometimes extubation occurs unintentionally and is called unplanned extuabtion. Unplanned extubation could be accidental or self extubation. There is a potential risk of unplanned extubation as long as the patient remains intubated. As unplanned extubation can lead to catastrophic events, it is ideal to have a weaning protocol in place so that the patients are extubated as soon as they satisfy the extubation criteria. This CME article mainly focuses on the risk factors for unplanned extubation in the adult intensive care unit and the complications associated with it. The strategies for prevention of unplanned extubation has been highlighted at the end.

Abstract:EXECUTIVE SUMMARY Background: Adverse events of mechanically ventilated neonates such as unplanned extubations may be associated with serious negative outcomes. Unplanned extubation rates have been monitored by many neonatal intensive care units as a quality of care metric. Objectives: The objective was to implement evidence-based best practice and assess the effects of these strategies on minimizing unplanned extubation in the neonatal intensive care unit in a large tertiary children's hospital. Methods: Evidence-based audit criteria were used to conduct an audit in the neonatal intensive care unit, Children's Hospital of Fudan University, Shanghai. The program included three phases and was conducted from May 2016 to October 2016. The Joanna Briggs Institute Practical Application of Clinical Evidence System and Getting Research into Practice audit tools for promoting change in health practice were used to ascertain compliance with the criteria before and after the implementation of best practice. Results: Compared with the baseline audit, the follow-up audit results demonstrated increased compliance rates for securement procedures, documentation of position and security of the endotracheal tube. Compliance for standardized care practice documentation increased from 0% to 100%; compliance for standard care practice implementation increased from 0% to 54.9%; and compliance for staff education increased from 66.7% to 100%. The neonatal intensive care unit also achieved the benchmark of less than one UE per 100 intubation days. Conclusions: This implementation project achieved a significant improvement in establishing evidence-based prevention of unplanned extubations in the neonatal intensive care unit of Children's Hospital of Fudan University, China. Standardizing the procedures represented an important step toward refining the quality improvement process.

Abstract:Unplanned extubation is any removal of an endotracheal tube from the airway during mechanical ventilation that is not planned, premeditated, or controlled. The frequency of unplanned extubation is one of the most important indicators of the quality of care in the pediatric intensive care unit, and according to the literature, it ranges from 0.11 to 6.4 per 100 days of mechanical ventilation. Unplanned extubation leads to severe complications, worsens outcomes, and increases treatment costs, which determine the urgency of the problem. The search for relevant publications was conducted in PubMed, Google Scholar, and eLibrary.ru. The review included 37 articles published from June 1997 to July 2023. The articles contained information on the incidence of unplanned extubations in pediatric and neonatal intensive care units, their complications, and associated factors. It also presented measures aimed at improving the quality and safety of medical care and reducing the frequency of unplanned extubations, with an analysis of their effectiveness. No studies have focused on the frequency of unplanned extubations in Russia, which is a good motivator for conducting research on unplanned extubations in the national healthcare system. The authors hope to draw the attention of clinicians and healthcare managers to this problem as one of the criteria for the quality and safety of medical care for children.
Reducing NICU Unplanned Extubations From Tube Dislodgement

Abstract:Objectives: Unplanned extubations (UEs) can be a frequent problem and are associated with adverse outcomes. This quality improvement initiative sought to reduce UEs from tube dislodgement in a level IV NICU utilizing methods applicable to other ICUs and performed with minimal monetary funds. Methods: From January 2019 to July 2023, an interdisciplinary quality improvement team used the Model for Improvement and performed sequential interventions to improve the outcome measure of UEs per 100 ventilator days. Process measures included adherence to a modified, site-specific UE care bundle derived from the Solutions for Patient Safety network, whereas the number of endotracheal tube-related pressure injuries was used as a balancing measure. Statistical process control charts and established rules for special cause variation were applied to analyze data. Results: Sequential interventions reduced the rate of UEs from a baseline of 2.3 to 0.6 UEs per 100 ventilator days. Greater than 90% adherence with the UE care bundle and apparent cause analysis form completion occurred since December 2020. There were no endotracheal tube-related pressure injuries. Conclusions: A sustained reduction in UEs was demonstrated. Leveraging a multidisciplinary team allowed for continuous UE analysis, which promoted tailored consecutive interventions. UE care bundle audits and the creation of a postevent debrief guide, which helped providers share a common language, were the most impactful interventions. Next steps include disseminating these interventions to other ICUs across our hospital enterprise. These low-cost interventions can be scalable to other NICUs and PICUs.
[The Risk Factors of Failed Extubation after Cardiac Surgery in Infants].

Abstract:OBJECTIVE:To investigate the risk factors of failed extubation (FE) after cardiac surgery in infants.METHODS:227 infants (< 1 year of age) who underwent congenital heart surgery in the period between January 2001 and December 2002 were included in this study. Logistic regression analysis was used to assess the risk factors of failed extubation. Odds Ratio was used to suppose the degree of relationship between FE and risk factors.RESULTS:Out of the 227 infants undergoing congenital heart operations, 30 (13.22%) cases failed at the extubation. Risk factors for failed extubation included postoperative duration of ventilation (EOR = 12.0; 95% CI = 4.04 - 35.71; P = 0.000 9), postoperative pneumonia (EOR = 5.33; 95% CI = 1.81 - 15.68; P = 0.002) and preoperative pulmonary hypertension (EOR = 2.80; 95% CI = 1.21 - 10.45; P = 0.041). Postoperative pneumonia and preoperative pulmonary hypertension were the 2 independent risk factors for failed extubation (P < 0.05).CONCLUSIONS:Postoperative pneumonia and preoperative pulmonary hypertension are major risk factors for failed extubation after congenital heart surgery in infants. To prevent and to treat postoperative pneumonia and pulmonary hypertensive crises will be beneficial to the successful extubation.

Abstract:Tremendous advancements in neonatal respiratory care have contributed to the improved survival of extremely preterm infants (gestational age ≤ 28 weeks). While mechanical ventilation is often considered one of the most important breakthroughs in neonatology, it is also associated with numerous short and long-term complications. For those reasons, clinical research has focused on strategies to avoid or reduce exposure to mechanical ventilation. Nonetheless, in the extreme preterm population, 70% to 100% of infants born 22-28 weeks of gestation are exposed to mechanical ventilation, with nearly 50% being ventilated for ≥ 3 weeks. As contemporary practices have shifted towards selectively reserving mechanical ventilation for those patients, mechanical ventilation weaning and extubation remain a priority yet offer a heightened challenge for clinicians. In this review, we will summarize the evidence for different strategies to expedite weaning and assess extubation readiness in preterm infants, with a particular focus on extremely preterm infants.

Abstract:Objective To describe the current status of unplanned extubation(UEX) of gastric tube in pediatrics,and to analyze the related reasons.Methods A prospective study was used to investigate the status of UEX in patients with gastric tube at one grade A tertiary children′s hospital in Shanghai from April to September 2011.The record form was self-designed.Results A total of 249 cases were recorded,among whom 19 underwent UEX.The UEX rate was 7.6%.The UEX rate determined by intubation days was 1.03 per 100 intubation days.The reintubation rate was 63.2%.UEX was associated with the type of intubation and body restraint by logistic regression analysis.Conclusion The improvement in patient inspection and nurse prediction and management of high-risk tubes as well as proper use of restraints is helpful to reduce the occurrence of UEX.
