Abstract:The goal of our study was to describe the availability of community child and adolescent mental health services, trauma-informed care, and the geographic accessibility of these services for juvenile justice-involved (JJ) youth who received mental health services while in secure detention. Data collection occurred through direct contact with the child and adolescent outpatient clinics listed on the New York State Office of Mental Health website. Zip codes were collected from the juvenile secure detention census. Of the clinics contacted, 88.5 percent accepted JJ youth; however, 43.5 percent accepted them on a conditional basis. Only 62.1 percent offered trauma-informed care, including evidence-based interventions and unspecified care. Although 84.5 percent of the clinics that would accept this population reported currently accepting new patients, reported wait times were as high as six or more months. When JJ residents' home zip codes and those of the clinics were geographically mapped, there were few clinics in the zip codes where most residents lived. The clinics that accepted youth on a conditional basis often refused high-risk patients, essentially ruling out a large majority of this population. The geographical inaccessibility of these clinics limits their ability to provide care for this vulnerable population.
Abusive Head Trauma: Epidemiology, Pathogenesis, Diagnosis and Outcome

Abstract:While accidental trauma is the most common cause of death in childhood, abusive head trauma is the most common cause of traumatic death in infancy. The incidence of abusive head trauma in the United States is estimated to be 15 per 1000 children each year, though this may be an underestimation. Nearly 25% of children under 2 years of age who are hospitalized for head trauma have been abused. There are several clinical and radiological features that are helpful in distinguishing accidental from abusive head trauma in young infants, such as retinal hemorrhages, which are present in 60% to 95% of patients with abusive head trauma. Computed tomographic scanning (CT) is the most rapid, reliable tool in the diagnosis of abusive head trauma. Acute subdural and subarrachnoid hemorrhage, the most common brain injuries seen in children with abusive head trauma, can be readily appreciated on CT by experienced clinicians. Long-term outcome of survivors of abusive head trauma tends to be poor, and is dependent on the severity of symptoms on initial presentation. Such clinical features are helpful in distinguishing accidental from abusive head trauma in young children.
Prevalence Rates and Evolution of Psychiatric Disorders Among Incarcerated Youths in Comparison With Non-incarcerated Youths

Abstract:Background: Our main objective was to provide estimates of the prevalence rates of psychiatric disorders and comorbidities among youths in a juvenile detention center in Geneva, Switzerland. We also aimed to investigate potential positive effects of intensive psychotherapeutic and educational services this center provides. Finally, we examined psychiatric care prior to and after custody as well as the evolution of the youths' mental health during detention. Methods: We conducted a longitudinal study including a group of incarcerated (n = 86) and a group of non-incarcerated (n = 169) youths (12-18 years old). Measures included diagnoses of psychiatric disorders, cognitive functions, trauma, psychopathic traits and the Youth Self-Report (aggressive behavior, attentional disorders, criminal behavior, social withdrawal, anxiety, depression and somatic complaints) collected at baseline and at discharge for the incarcerated group. Data were analyzed using mixed-effect models. Results: Psychiatric disorders were prevalent in the incarcerated group (82.6, 95% CI: 71.6-90.7%), but young people also often suffered from several disorders simultaneously. Two-thirds of the incarcerated participants had a diagnosis of two or more psychiatric disorders. Regarding health care, most incarcerated participants (79.1%) had psychiatric care prior to detention. The planned care after detention was associated with psychiatric comorbidities, care being more likely planned for those with comorbidities (p = 0.030). Compared to the non-incarcerated group, the incarcerated group had lower scores on cognitive functioning (p < 0.001) and higher scores on trauma (p < 0.021) and psychopathic traits (p < 0.034). The youths' stay in the detention center was associated with a positive change of mental health, with externalized problems being significantly reduced at the end of their stay (p = 0.017). Conclusion: Our findings showed that youths in conflict with the law are characterized by (1) their internal vulnerabilities: a high prevalence of psychiatric disorders and psychiatric comorbidities, lower cognitive functions, externalized problems and psychopathic traits; (2) environmental factors: victims of violence and sexual abuse; and (3) their psychiatric history. Besides, the evolution of the most prevalent issues was favorable over time, which puts into question the usual perspective about the deleterious effect of detention.
Non-suicidal Self-Injury, Suicidal Behaviors, and Mental Health Symptoms among Sexual Minority Youth with Juvenile Justice System Involvement

Abstract:Sexual minority youth have long remained an understudied population within the juvenile justice system, despite emerging evidence suggesting that sexual minority youth are overrepresented. Recent literature indicates that system-involved sexual minority youth may have more behavioral health concerns than their heterosexual counterparts. Even so, more work is needed to elucidate the unique needs of sexual minority youth who become involved in the system, especially as it pertains to suicidal behaviors and non-suicidal self-injury (NSSI). The present study provided a descriptive evaluation of the prevalence of sexual minority youth making early system contact and their mental health concerns. Of the 218 justice-involved youth from a Northeastern family court in the sample, over a quarter identified as a sexual minority (n = 58). Our findings indicated that these youth evidenced significantly higher scores on a mental health screener compared to their heterosexual counterparts, evidencing greater symptomology on scales indicative of depression/anxiety, thought disturbance, etc. Further, sexual minority youth reported higher rates of lifetime, past year, and past month NSSI as well as lifetime and past year suicide attempts. These findings highlight the need to support the well-being of sexual minority youth in the juvenile justice system by screening for and addressing their mental health needs in this unique setting.

Abstract:An estimated 1.8 million juveniles were arrested in the United States for delinquency in 2009. Previous studies indicate high rates of exposure to traumatic events and posttraumatic stress (PTS) symptoms for these youths. This study examined PTS in a sample of 170 youths in juvenile detention. The results of this study reveal higher rates of PTS symptoms (21%) compared to national rates (6%). The data also suggest youths suffering from more PTS symptoms also report higher depression, anxiety, anger, family relationship worries, thought problems, and attention problems. These factors provide a direction for continued practice targeting these youths.

Administration and Policy in Mental Health and Mental Health Services Research

Abstract:The Brief Jail Mental Health Screen (BJMHS) is one of the most well-known and frequently used tools to conduct routine mental health screening at jail intake. In prior research, the BJMHS results typically have been evaluated overall (i.e., yes/no positive screen). However, there is heterogeneity in symptom presentation and treatment histories among people with serious mental illness, and there are potential consequences of this heterogeneity for mental health administration and policy in jails. We conducted a latent class analysis of BJMHS item-level results using administrative data for 37,998 people booked into a southeastern, metropolitan, U.S. county jail over a 3.5-year period. A 4-class solution provided the best fitting and most interpretable model. The largest class (89.5%) comprised people unlikely to report symptoms or treatment histories ( limited symptoms ). The next class comprised people who were unlikely to report ongoing symptoms but reported medication and hospitalization ( managed symptoms ). The third class (2.5%) included people likely to report feeling useless/sinful, prior hospitalization, and current psychiatric medication ( depressive symptoms ). The fourth class (1.0%) comprised people likely to report thought control, paranoia, feeling useless/sinful, medication, and hospitalization ( psychotic symptoms ). Controlling for sociodemographic and booking characteristics, people in the managed, depressive, and psychotic symptoms classes had significantly longer jail stays compared to those in the limited symptoms class. People in the managed and depressive symptoms classes were at heightened risk of re-arrest compared to the limited symptoms class. Findings can inform case prioritization and the allocation of resources to support efficient and effective jail-based mental health services.

Abstract:Incarcerated youth have high levels of mental disorders. However, there are no up-to-date reviews examining the prevalence rates of a broad range of mental disorders and symptoms across youth justice populations. The current review aims to bridge this gap. We conducted a systematic search of the literature using PsycINFO, Medline, Embase, and Web of Science databases. We used meta-analyses to produce pooled prevalence figures for each mental health disorder and symptoms, and meta-regression to test for the moderating effects of covariates, such as gender. Thirty studies were included involving 8,153 participants. Meta-regression analysis showed that females had higher prevalence rates for depression, separation anxiety disorder and suicide. Males had higher prevalence rates for conduct disorder and emerging antisocial personality disorder. Emerging personality disorders (borderline personality disorder: 21%; 95% CI: 13–28%; antisocial personality disorder: 62%; 95% CI: 39–82%) were relatively common in both genders. The findings of this meta-analysis show the need for robust mental health services in custody settings. Adopting a developmentally focused approach would increase understanding of incarcerated youths' needs and help to early detection of emerging personality symptoms. To improve young people's mental health, we need to ensure that services do not misidentify young people's needs due to diagnostic limitations.

Abstract:Criminal Justice and Behavior, Ahead of Print. There is evidence that youth in the justice system are more likely to experience trauma and mental health problems than the general population. Trauma histories may exacerbate mental health problems, and mental health problems may increase the likelihood of continued offending. While prior research has examined the effects of adverse childhood experiences (ACEs) on recidivism, it has yet to consider the prevalence of trauma among youth in a juvenile mental health court. Data from an urban juvenile mental health court (N = 203) was used to assess the prevalence of ACEs. The average ACE score of participants was 3.6% and 46% had a high ACE score. Furthermore, those who did not complete the program and/or were rearrested following participation had significantly higher ACE scores. Controlling for other factors, ACEs were not significantly associated with program noncompletion or rearrest. Policy implications surrounding trauma-informed practices are provided.

Abstract:Abstract Objectives This study aims to systematically review screening and assessment approaches and therapeutic interventions for young people with neurodevelopmental disorders (NDDs) in the youth justice system (YJS). Method Five databases were searched (PubMed, Embase, CINAHL, Web of Science and PsycINFO) for studies evaluating effectiveness of screening, assessment or interventions, targeting people aged 10–18 years in youth justice with a NDD. Results Eighteen peer-reviewed articles (9 screening/assessment; 9 intervention studies) were eligible. More comprehensive assessment appeared to be superior to brief screening for identifying NDDs. Interventions addressed social and emotional wellbeing, educational, employment, language and communication and behavioural outcomes, community engagement, quality of life factors and recidivism. Limited consideration was given to cultural diversity in evaluations of screening, assessment or interventions. Conclusions The limited amount of evidence retrieved suggests further research is urgently needed in the provision of culturally competent and holistic screening, assessment and treatment for young people with NDDs in the YJS.

Abstract:The double ionization of He by either photon impact or bare ion impact are simple examples of the quantal three-body Coulomb breakup problem. For example, the double photoionization of He near threshold produces two slow moving continuum electrons in the Coulomb field of an alpha particle, which is extremely difficult for perturbation theory to handle. On the other hand, various non-perturbative theoretical approaches, including R-matrix methods [1–5], the converged close-coupling method [6], the time-dependent close-coupling (TDCC) method [7], and the exterior complex scaling (ECS) method [8] have yielded double photoionization cross sections for He in good agreement with experiment [9]. Another simple example is found in neutron scattering from the nucleus of the He atom [10, 11]. Depending on the energy acquired by the target nucleus, the atomic electrons can either follow its movement and remain in bound states or be released to the continuum. When both electrons are released to the continuum, we again have to solve the three-body breakup problem. Recently a non-perturbative ECS method was used to calculate neutron-impact single and double ionization cross sections for He [12]. They found significant differences in the ratio of double to single ionization cross sections from those found in photon impact and bare ion impact processes. With almost two dozen neutron scattering facilities in the world [13], a better understanding of the motion of atomic electrons following neutron-impact could provide insight into many neutron research projects in not only physics, but astronomy, biology, chemistry, and engineering. For example, new detectors are currently being designed to search for weakly interacting massive particles (dark matter candidates) based on neutron scattering experiments [14]. In this paper we present a theoretical study of the single and double ionization of He over a broad energy range. The non-perturbative TDCC method allows us to incorporate the interaction of two continuum electrons in the presence of a positive ion field in an accurate manner. We present total cross sections for the single, single with excitation, and double ionization of He. We compare cross section ratios of double to single ionization with previous ECS results [12]. We also present energy and angle differential cross sections for the single and double ionization of He. The rest of the paper is organized as follows. In section 2 we present the non-perturbative TDCC method for neutron-impact ionization of atoms. In section 3 we present total and differential cross sections for the neutron-impact single and double ionization of He. In section 4 we conclude with a brief summary and future plans. Unless otherwise stated, we will use atomic units.

Abstract:Criminal Justice and Behavior, Ahead of Print. Few studies have examined the impact of trauma-specific interventions in juvenile justice settings, and juvenile justice-related outcomes are infrequently assessed. This study examined the impact of implementing youth skills groups (Skills Training in Affective and Interpersonal Regulation [STAIR]) and trauma training for staff (Think Trauma) on individual-level assaults in two secure detention facilities. The impact of the intervention was primarily evidenced in one facility among Black youth. In the same facility, the staff training (pre-skills groups) cohort and skills group cohort (post-staff training) evidenced a significantly greater reduction in violent incident rates as compared to the pre-intervention cohort. These findings highlight the importance of trauma-focused training for staff as a promising step toward reducing violence and improving staff members' responses to a large subset of vulnerable youth.

Abstract:Objective The aim of this study was to describe how specific mental health–trained social workers can assist in the evaluations and follow-up of patients presenting with mental health concerns in the pediatric emergency department (ED). Methods Work was performed at a quaternary children's hospital ED with 95,000 annual ED visits across 2 locations. Patients requiring mental health services identified based on presenting complaint or from universal suicide screen were included. Emergency department team first evaluates the patients for medical screening and then consults a team of social workers specialized in acute mental health screening (AMHS). The team evaluates and provides recommendation for disposition and assists in plan completion. For patients not admitted, AMHS team makes 24- and 48-hour calls to ensure safety. We collected and analyzed the data on all eligible patients from September 2015 through June 2019 for (1) demographic information, (2) trends in number of consults to AMHS, (3) disposition plans and trends by year, and (4) frequency of follow-up phone calls. Results A total of 5950 patient visits were reviewed, for 4454 distinct patients. Most patients were 12 to 17 years of age, female, and White, with Medicaid being the predominant insurance. The most common chief complaint was suicidal ideation/plan/attempt. Self-referrals were the majority of assessments, and 59% of patients were already receiving mental health services. Median team response time was 19 minutes. There was an upward trend in consults. Psychiatric hospitalization was the most common disposition; more than 95% of the other visits had timely follow-up phone calls. Conclusion Despite an increasing number of patients presenting to the ED with mental health crisis, safe and efficient management is possible with ED staff–social worker partnership. This approach can ensure that eligible patients receive consistent and evidence-based evaluations and can allow ED clinicians to respond to medical emergencies that require their attention.

Abstract:Background Mental health services are available for young people involved with the criminal justice system. However, they have unmet mental health needs after the expiration of criminal justice supervision. Objective To determine the incidence rate and identify predictors of psychiatric hospitalisations within 24 months after the expiration of criminal justice supervision among young people involved with the New South Wales (NSW) criminal justice system. Methods Retrospective data from 1556 individuals aged 14–22 years who participated in four surveys of justice-involved young people in NSW were harmonised and linked to four NSW data collections. We calculated the incidence rates of psychiatric hospitalisations within 24 months postsupervision and identified predictors of these hospitalisations using a competing risks regression analysis. Results Within 24 months postsupervision, 11.4% had a psychiatric hospitalisation compared with 3.5% during supervision. 20.7% of those admitted had a known history of mental illness and engaged with community-based and outpatient mental health services postsupervision. Predictors of psychiatric hospitalisations were: female sex (adjusted subdistribution HR (asHR) 1.84, 95% CI 1.24 to 2.73); previous incarceration (highest asHR for ≥4 episodes 1.67, 95% CI 1.01 to 2.78); head injury (asHR 1.63, 95% CI 1.20 to 2.21); personality disorder (asHR 3.66, 95% CI 2.06 to 6.48) and alcohol and substance use disorder (asHR 1.89, 95% CI 1.29 to 2.77). Conclusion Justice-involved youth have higher rates of psychiatric admissions after criminal justice supervision. Engagement with mental health services postsupervision is important in addressing emerging or persisting mental health needs.

Abstract:Aim Young offenders experience higher rates of neurodevelopmental and mental health disorders than the general population, and significant access barriers to health treatment. Treatment combining Cognitive Remediation Therapy (CRT) and Social Cognition Remediation Therapy (SCRT) has demonstrated benefits for functional improvements and social development. However, there is limited information regarding group treatment programs in custodial settings for young offenders. This pilot study explores the effectiveness and feasibility of a group treatment program for youth offenders with cognitive deficits and mental health concerns in youth detention. Methods The School‐Link Advantage pilot study designed and tested a 10‐week group treatment program combining CRT and SCRT for young offenders in custody. The closed groups incorporated interactive activities focussed on emotional recognition and regulation skills, optimizing executive functioning, understanding values, exploring belief systems, improving relationships, and safety planning. Results Of the 22 male participants recruited in an Australian Youth Justice Centre, 12 completed all aspects of the treatment program, reflecting a 54.5% completion rate in a typically challenging to engage population cohort. Results demonstrated significant improvements in the ability to store and retrieve information, recognize information, and control emotions. Planning and organizing skills also showed considerable development. Conclusions This pilot study suggests that a combined CRT and SCRT group treatment program has the potential to effectively target cognitive challenges associated with mental health disorders in young offenders in custody. These promising outcomes suggest exploring randomized controlled trials with increased cultural sensitivity for diverse populations.

Abstract:Few studies have examined the prevalence of mental illness in young people over time within the same jurisdiction. In the current study, we compared data from three large surveys of youth in custody in New South Wales, conducted in 2003, 2009 and 2015. We examined rates of mental illness, self-harm and suicidal behaviours, substance use and childhood trauma and found little consistent change over time, though some fluctuations were observed regarding certain mental illnesses and substance use. We also descriptively compared findings with observed rates for the general population and found that young people in custody showed higher levels of all examined variables. In sum, these data suggest little improvement in the well-being of young people in custody in New South Wales over time. Better identification and treatment of these issues are vital if young people are to be prevented from becoming enmeshed in the criminal justice system.

Abstract:Purpose We sought to identify trends and characteristics associated with the availability of tailored mental health services for individuals involved in the criminal justice system and ordered to treatment by a court, nationally in the US and by state. Methods We used National Mental Health Services Survey to identify outpatient mental health treatment facilities in the US (2016 n = 4744; 2018 n = 4626; 2020 n = 4869). We used clustered multiple logistic regression to identify changes over time as well as facility- and state-level factors associated with the availability of specialty court-ordered services. Results Slightly more than half of the outpatient mental health treatment facilities offered specialized services for individuals ordered to treatment by a court, with wide variation between states. Nationally, there was a significant increase in the odds of offering court-ordered treatment in 2020 compared to 2016 (aOR = 1.16, 95% CI = 1.06–1.27, p < 0.01). Notable associations included offering integrated substance use treatment (versus none, aOR = 2.95, 95% CI = 2.70–3.22, p < 0.0001) and offering trauma therapy (versus none, aOR = 2.05, 95% CI = 1.85–2.27, p < 0.0001). Conclusion The availability of mental health services for individuals ordered to treatment by a court is growing nationally but several states are lagging behind. Court ordered treatment is a promising strategy to improve health and reduce reliance on the carceral system as a healthcare provider. At the same time, we express caution around disparities within behavioral health courts and advocate for equity in access to incarceration alternatives.

Administration and Policy in Mental Health and Mental Health Services Research

Abstract:Previous research indicates that youth exhibiting antisocial behavior are at risk for utilizing a disproportionate amount of health services compared to youth without these problems. The present study investigates whether being processed by the juvenile justice system and showing callous-unemotional (CU) traits independently predict health service utilization (medical and mental health service use and out-of-home placement) over and above the severity of antisocial behavior across adolescence. A total of 766 participants who had been arrested for the first time in adolescence provided data at ten appointments over a period of seven years. Results showed that self-reported antisocial behavior at the time of arrest predicted increased use of most health service use types over the next seven years (i.e. medicine prescriptions, tests for sexually transmitted infections, mental health service appointments, and out-of-home placements). All except prescription medication use remained significant when controlling for justice system processing and CU traits. Further, justice system processing added significantly to the prediction of medical service appointments. Whereas CU traits were associated with mental health service appointments and out-of-home placements, these did not remain significant when controlling for severity of antisocial behavior. These findings are consistent with prior research documenting the health care costs of antisocial behavior.

Abstract:Objective: The objective of this study was to explore the acceptability and feasibility of a therapeutic assessment protocol for the Screening and Support of Youth (SASY). SASY provides brief but comprehensive community-based screening and support for diverse youth in the community. Methods: SASY screening evaluates symptoms, functioning and clinical risk. The Kiddie Computerized Adaptive Test was used to evaluate seven different diagnoses and symptom severity. The Weiss Functional Impairment Rating Scale-Self was used to measure functional impairment. Measures were scored according to nationally developed norms. An algorithm was developed to aggregate symptom and function ratings into an overall score for clinical risk. The results are discussed with participants in a motivational interview designed to promote insight, followed by the opportunity for the participant to engage in an online intervention. Protocol changes necessitated by social distancing during the pandemic led to innovative methods including the use of a QR code for recruitment, integration of both online and offline participation, and expansion from in-person recruitment within the schools to virtual engagement with youth throughout the community. The final sample included disproportionately more Black or African American and Hispanic youth as compared to school and community statistics, suggesting that optimization of online and offline methods in research may facilitate the recruitment of diverse populations. Qualitative interviews indicated that the screening and feedback raised youth awareness of their wellbeing and/or distress, its impact on their functioning, and engagement with options for improved wellbeing. Conclusions: The emergence of innovative methods optimizing the advantages of both online and offline methods, developed as a necessity during the pandemic, proved advantageous to the feasibility and acceptability of community-based recruitment of at-risk, minoritized youth.
