Abstract:AIMS It is well established that leukotrienes (LTs), products of the 5-lipoxygenase (5-LO) pathway, participate in inflammatory tissue reactions and immune responses. In the present study, the impact of the 5-LO pathway on vasculogenesis of mouse embryonic stem (ES) cells was investigated. METHODS AND RESULTS Immunohistochemistry studies demonstrated that 5-LO(+) cells first appeared at day 6 of embryoid body (EB) formation from ES cells. 5-LO(+)/CD68(+) as well as 5-LO(+)/CD45(+) cells were prominent at day 10 of EB differentiation. Real-time PCR and western blot analysis revealed all constituents of the 5-LO pathway. High performance liquid chromatography analyses indicated the synthesis of LTB(4) and LTD(4) in conformity with induction of the 5-LO pathway. Furthermore, Flk-1(+)/CD105(+) cells displayed calcium- (Ca(2+)) transients in response to LTB(4), whereas CD11b(+) cells responded to LTD(4). Treatment of EBs with LTB(4) and LTD(4) resulted in phosphorylation of the mitogen-activated protein kinase ERK1/2. Pharmacological inhibition of the 5-LO pathway and stable shRNA targeting of 5-LO-activating protein decreased capillary cell areas positive for PECAM-1. CONCLUSION Our data demonstrate that the 5-LO pathway emerges early during ES cell differentiation into cells of the myeloid lineage and that LTs play an until now unrecognized role in vascular development of ES cells.
Using machine learning to predict bacteremia in urgent care patients on the basis of triage data and laboratory results

Abstract:Background: Despite advancements in antimicrobial therapies, bacteremia remains a life-threatening condition. Appropriate antimicrobials must be promptly administered to ensure patient survival. However, diagnosing bacteremia based on blood cultures is time-consuming and not something emergency department (ED) personnel are routinely trained to do. Methods: This retrospective cohort study developed several machine learning (ML) models to predict bacteremia in adults initially presenting with fever or hypothermia, comprising logistic regression, random forest, extreme gradient boosting, support vector machine, k-nearest neighbor, multilayer perceptron, and ensemble models. Random oversampling and synthetic minority oversampling techniques were adopted to balance the dataset. The variables included demographic characteristics, comorbidities, immunocompromised status, clinical characteristics, subjective symptoms reported during ED triage, and laboratory data. The study outcome was an episode of bacteremia. Results: Of the 5063 patients with initial fever or hypothermia from whom blood cultures were obtained, 128 (2.5 %) were diagnosed with bacteremia. We combined 36 selected variables and 10 symptoms subjectively reported by patients into features for analysis in our models. The ensemble model outperformed other models, with an area under the receiver operating characteristic curve (AUROC) of 0.930 and an F1-score of 0.735. The AUROC of all models was higher than 0.80. Conclusion: The ML models developed effectively predicted bacteremia among febrile or hypothermic patients in the ED, with all models demonstrating high AUROC values and rapid processing times. The findings suggest that ED clinicians can effectively utilize ML techniques to develop predictive models for addressing clinical challenges.
Development and validation of a machine learning model integrated with the clinical workflow for early detection of sepsis

Abstract:Background Sepsis is a life-threatening condition caused by a dysregulated response to infection, affecting millions of people worldwide. Early diagnosis and treatment are critical for managing sepsis and reducing morbidity and mortality rates. Materials and methods A systematic design approach was employed to build a model that predicts sepsis, incorporating clinical feedback to identify relevant data elements. XGBoost was utilized for prediction, and interpretability was achieved through the application of Shapley values. The model was successfully deployed within a widely used Electronic Medical Record (EMR) system. Results The developed model demonstrated robust performance pre-operations, with a sensitivity of 92%, specificity of 93%, and a false positive rate of 7%. Following deployment, the model maintained comparable performance, with a sensitivity of 91% and specificity of 94%. Notably, the post-deployment false positive rate of 6% represents a substantial reduction compared to the currently deployed commercial model in the same health system, which exhibits a false positive rate of 30%. Discussion These findings underscore the effectiveness and potential value of the developed model in improving timely sepsis detection and reducing unnecessary alerts in clinical practice. Further investigations should focus on its long-term generalizability and impact on patient outcomes.

Using Machine Learning to Develop and Validate an In-Hospital Mortality Prediction Model for Patients with Suspected Sepsis

Abstract:Background: Early recognition of sepsis and the prediction of mortality in patients with infection are important. This multi-center, ED-based study aimed to develop and validate a 28-day mortality prediction model for patients with infection using various machine learning (ML) algorithms. Methods: Patients with acute infection requiring intravenous antibiotic treatment during the first 24 h of admission were prospectively recruited. Patient demographics, comorbidities, clinical signs and symptoms, laboratory test data, selected sepsis-related novel biomarkers, and 28-day mortality were collected and divided into training (70%) and testing (30%) datasets. Logistic regression and seven ML algorithms were used to develop the prediction models. The area under the receiver operating characteristic curve (AUROC) was used to compare different models. Results: A total of 555 patients were recruited with a full panel of biomarker tests. Among them, 18% fulfilled Sepsis-3 criteria, with a 28-day mortality rate of 8%. The wrapper algorithm selected 30 features, including disease severity scores, biochemical parameters, and conventional and few sepsis-related biomarkers. Random forest outperformed other ML models (AUROC: 0.96; 95% confidence interval: 0.93-0.98) and SOFA and early warning scores (AUROC: 0.64-0.84) in the prediction of 28-day mortality in patients with infection. Additionally, random forest remained the best-performing model, with an AUROC of 0.95 (95% CI: 0.91-0.98, p = 0.725) after removing five sepsis-related novel biomarkers. Conclusions: Our results demonstrated that ML models provide a more accurate prediction of 28-day mortality with an enhanced ability in dealing with multi-dimensional data than the logistic regression model.

Abstract:Bloodstream infection (BSI) is associated with increased morbidity and mortality in the pediatric intensive care unit (PICU) and high healthcare costs. Early detection and appropriate treatment of BSI may improve patient's outcome. Data on machine-learning models to predict BSI in pediatric patients are limited and neither study included time series data. We aimed to develop a machine learning model to predict an early diagnosis of BSI in patients admitted to the PICU. This was a retrospective cohort study of patients who had at least one positive blood culture result during stay at a PICU of a tertiary-care university hospital, from January 1 st to December 31 st 2019. Patients with positive blood culture results with growth of contaminants and those with incomplete data were excluded. Models were developed using demographic, clinical and laboratory data collected from the electronic medical record. Laboratory data (complete blood cell counts with differential and C-reactive protein) and vital signs (heart rate, respiratory rate, blood pressure, temperature, oxygen saturation) were obtained 72 hours before and on the day of blood culture collection. A total of 8816 data from 76 patients were processed by the models. The machine committee was the best-performing model, showing accuracy of 99.33%, precision of 98.89%, sensitivity of 100% and specificity of 98.46%. Hence, we developed a model using demographic, clinical and laboratory data collected on a routine basis that was able to detect BSI with excellent accuracy and precision, and high sensitivity and specificity. The inclusion of vital signs and laboratory data variation over time allowed the model to identify temporal changes that could be suggestive of the diagnosis of BSI. Our model might help the medical team in clinical-decision making by creating an alert in the electronic medical record, which may allow early antimicrobial initiation and better outcomes.

Machine learning algorithms for early sepsis detection in the emergency department: A retrospective study

Abstract:Background: Early recognition and treatment of sepsis are crucial for improving patient outcomes. However, the diagnosis of sepsis remains challenging because of vague clinical presentations. Objectives: We aim to developed novel sepsis screening tools with machine learning models and compared their performance with traditional methods. Methods: We used machine learning algorithms to develop models for early risk prediction of sepsis based on retrospective single-center electronic health record data from adult patients who presented to the emergency department (ED) from June 2018 through May 2020. Available triage data including vital signs, baseline characteristics, and chief complaints served as predictors. In our study, 80% and 20% of the data were randomly split into training and testing sets, respectively. Derived from the training set, we built the models based on four machine learning algorithms: logistic regression, gradient boosting, random forest, and neural network. Our primary outcome was the model performance that predicted the final diagnosis of sepsis determined by the area under the receiver operating characteristic curve (AUROC), sensitivity, specificity, and predictive performance compared with those of the reference models (quick sequential organ failure assessment [qSOFA], modified early warning score [MEWS], and systemic inflammatory response syndrome [SIRS]) using the testing dataset. Results: In total, 133,707 ED visits were analyzed. All machine learning models outperformed the reference models by achieving a higher AUROC (e.g., AUROC 0.931 [95% CI 0.921-0.944] in our best model (random forest algorithm) vs 0.635 [95% CI 0.613-0.660] in qSOFA, 0.688 [95% CI 0.662-0.715] in MEWS, and 0.814 [95% CI 0.794-0.833] in SIRS). Conclusion: The machine learning models demonstrated superior performance in prediction of sepsis diagnosis among emergency patients compared with that using the traditional screening tools. Further studies are needed to determine whether the models will enhance physicians' judgments and improve patient outcomes.
Machine learning for the prediction of urosepsis using electronic health record data

Abstract:Urosepsis, a medical condition resulting from the progression of urinary tract infection (UTI), is a leading cause of death in hospitals in the United States. Urosepsis commonly occurs due to complicated UTI and constitutes approximately 25% of all sepsis cases. Early prediction of urosepsis is critical in providing personalized care, reducing diagnostic uncertainty, and ultimately lowering mortality rates. While machine learning techniques have the potential to aid healthcare professionals in identifying potential risk factors, and high-risk patients, and recommending treatment options, no existing study has been developed so far to predict the development of urosepsis in patients with a suspected UTI presenting to an outpatient setting. In this research study, we develop and evaluate the utility of multiple machine learning models to predict the likelihood of hospital admission and urosepsis diagnosis for patients with an outpatient UTI encounter, leveraging de-identified electronic health records sourced from a large health care system encompassing a wide range of encounters spanning primary to quaternary care. Inclusion criteria included a positive diagnosis of urinary tract infection indicated by ICD-10 code N30 or N93.0 and positive bacteria result via urinalysis in an ambulatory setting (primary or emergent care settings). For these patients, we extracted demographic information, urinalysis findings, and any antibiotics prescribed for each instance of UTI. Reencounters we defined as all encounters within seven days of the initial UTI encounter. The reencounters were considered urosepsis-related if matching positive blood and urine cultures were found with a sepsis ICD-10 code of A41, R78, or R65. A variety of machine learning models were trained on this rich feature set and were evaluated on two tasks: the prediction of a reencounter leading to hospitalization, and the prediction of Urosepsis. Model performances were stratified by the patient ethnicities. Our models demonstrated high predictive performance with an area under the ROC curve (AUC) of 79.5% AUC and an area under the precision-recall curve (APR) of 13% APR for reencounters, and 90% ROC and 31% APR for Urosepsis. We computed shapley values to interpret our model predictions and found the patient age, sex, and urinary WBC count were the top three predictive features. Our study has the potential to assist clinicians in the identification of high-risk patients, making more informed decisions about antibiotic prescription and providing improved patient care.

Abstract:Abstract Background Accurate diagnosis of acute bacterial lower respiratory tract infections (LRTI) is often elusive. The lack of diagnostic gold standard, exacerbated by overlapping syndrome with non-infective/non-bacterial respiratory conditions, have contributed to antibiotic over-prescribing. This in turn drives drug resistance, healthcare costs and patient morbidity. We explored feasibility of developing a classification model to identify instances where antibiotics should be initiated or withheld when bacterial LRTI is suspected. Methods All inpatients in Singapore General Hospital during Y2019-2020 and had ICD-10 codes for LRTI or mimicking conditions were included. Only cases with ICD-10 codes for LRTI and manually assessed to be true by the hospital antimicrobial stewardship unit (ASU) were labelled as ground truth “Yes” (i.e., antibiotics should be initiated). Cases with ICD-10 codes for other conditions were labelled as ground truth “No” (i.e., antibiotics could be withheld). Patient information (e.g., clinical notes, radiology reports, vital signs) were extracted from our hospital’s electronic data repository. Gradient boosting algorithm was applied for model building and evaluated on a 20% held-out test set. A sub-analysis on incorrect classification was also performed. Results There were a total of 24,399 episodes. After exclusion by ASU review and under-sampling, the ground truth “yes” and “no” groups had 2,887 and 4,888 cases respectively. The model achieved a good performance of 80% accuracy, 83% sensitivity and 79% specificity, corresponding to an AUC of 0.89. Chest imaging findings, procalcitonin, c-reactive protein, cough, neutrophil count and oxygen saturation at start of episode were the 5 most important features. Incorrect classifications had lower confidence scores, and were more likely to occur among those with severe illness or radiological reports that were not suggestive of LRTI. Conclusion We developed a machine learning model to support decision-making on antibiotic initiation for suspected bacterial LRTI. Interpretable Machine Learning Models for Predicting Critical Outcomes in Patients with Suspected Urinary Tract Infection with Positive Urine Culture

Abstract:(1) Background: Urinary tract infection (UTI) is a leading cause of emergency department visits and hospital admissions. Despite many studies identifying UTI-related risk factors for bacteremia or sepsis, a significant gap remains in developing predictive models for in-hospital mortality or the necessity for emergent intensive care unit admission in the emergency department. This study aimed to construct interpretable machine learning models capable of identifying patients at high risk for critical outcomes. (2) Methods: This was a retrospective study of adult patients with urinary tract infection (UTI), extracted from the Medical Information Mart for Intensive Care IV Emergency Department (MIMIC-IV-ED) database. The critical outcome is defined as either in-hospital mortality or transfer to an intensive care unit within 12 h. ED visits were randomly partitioned into a 70%/30% split for training and validation. The extreme gradient boosting (XGBoost), random forest (RF), and support vector machine (SVM) algorithms were constructed using variables selected from the stepwise logistic regression model. The XGBoost model was then compared to the traditional model and clinical decision rules (CDRs) on the validation data using the area under the curve (AUC). (3) Results: There were 3622 visits among 3235 unique patients diagnosed with UTI. Of the 2535 patients in the training group, 836 (33%) experienced critical outcomes, and of the 1087 patients in the validation group, 358 (32.9%) did. The AUCs for different machine learning models were as follows: XGBoost, 0.833; RF, 0.814; and SVM, 0.799. The XGBoost model performed better than others. (4) Conclusions: Machine learning models outperformed existing traditional CDRs for predicting critical outcomes of ED patients with UTI. Future research should prospectively evaluate the effectiveness of this approach and integrate it into clinical practice.
Machine learning for early detection of sepsis: an internal and temporal validation study

Abstract:Abstract Objective Determine if deep learning detects sepsis earlier and more accurately than other models. To evaluate model performance using implementation-oriented metrics that simulate clinical practice. Materials and Methods We trained internally and temporally validated a deep learning model (multi-output Gaussian process and recurrent neural network [MGP–RNN]) to detect sepsis using encounters from adult hospitalized patients at a large tertiary academic center. Sepsis was defined as the presence of 2 or more systemic inflammatory response syndrome (SIRS) criteria, a blood culture order, and at least one element of end-organ failure. The training dataset included demographics, comorbidities, vital signs, medication administrations, and labs from October 1, 2014 to December 1, 2015, while the temporal validation dataset was from March 1, 2018 to August 31, 2018. Comparisons were made to 3 machine learning methods, random forest (RF), Cox regression (CR), and penalized logistic regression (PLR), and 3 clinical scores used to detect sepsis, SIRS, quick Sequential Organ Failure Assessment (qSOFA), and National Early Warning Score (NEWS). Traditional discrimination statistics such as the C-statistic as well as metrics aligned with operational implementation were assessed. Results The training set and internal validation included 42 979 encounters, while the temporal validation set included 39 786 encounters. The C-statistic for predicting sepsis within 4 h of onset was 0.88 for the MGP–RNN compared to 0.836 for RF, 0.849 for CR, 0.822 for PLR, 0.756 for SIRS, 0.619 for NEWS, and 0.481 for qSOFA. MGP–RNN detected sepsis a median of 5 h in advance. Temporal validation assessment continued to show the MGP–RNN outperform all 7 clinical risk score and machine learning comparisons. Conclusions We developed and validated a novel deep learning model to detect sepsis. Using our data elements and feature set, our modeling approach outperformed other machine learning methods and clinical scores.
Deep learning-based prediction of Clostridioides difficile infection caused by antibiotics using longitudinal electronic health records

Abstract:Clostridioides difficile infection (CDI) is a major cause of antibiotic-associated diarrhea and colitis. It is recognized as one of the most significant hospital-acquired infections. Although CDI can develop severe complications and spores of Clostridioides difficile can be transmitted by the fecal-oral route, CDI is occasionally overlooked in clinical settings. Thus, it is necessary to monitor high CDI risk groups, particularly those undergoing antibiotic treatment, to prevent complications and spread. We developed and validated a deep learning-based model to predict the occurrence of CDI within 28 days after starting antibiotic treatment using longitudinal electronic health records. For each patient, timelines of vital signs and laboratory tests with a 35-day monitoring period and a patient information vector consisting of age, sex, comorbidities, and medications were constructed. Our model achieved the prediction performance with an area under the receiver operating characteristic curve of 0.952 (95% CI: 0.932–0.973) in internal validation and 0.972 (95% CI: 0.968–0.975) in external validation. Platelet count and body temperature emerged as the most important features. The risk score, the output value of the model, exhibited a consistent increase in the CDI group, while the risk score in the non-CDI group either maintained its initial value or decreased. Using our CDI prediction model, high-risk patients requiring symptom monitoring can be identified. This could help reduce the underdiagnosis of CDI, thereby decreasing transmission and preventing complications.

Abstract:Antimicrobial resistance (AMR) is a risk for patients and a burden for the healthcare system. However, AMR assays typically take several days. This study develops predictive models for AMR based on easily available clinical and microbiological predictors, including patient demographics, hospital stay data, diagnoses, clinical features, and microbiological/antimicrobial characteristics and compares those models to a naive antibiogram based model using only microbiological/antimicrobial characteristics. The ability to predict the resistance accurately prior to culturing could inform clinical decision-making and shorten time to action. The machine learning algorithms employed here show improved classification performance (area under the receiver operating characteristic curve 0.88-0.89) versus the naive model (area under the receiver operating characteristic curve 0.86) for 6 organisms and 10 antibiotics using the Philips eICU Research Institute (eRI) database. This method can help guide antimicrobial treatment, with the objective of improving patient outcomes and reducing the usage of unnecessary or ineffective antibiotics.

Abstract:Candidemia often poses a diagnostic challenge due to the lack of specific clinical features, and delayed antifungal therapy can significantly increase mortality rates, particularly in the intensive care unit (ICU). This study aims to develop a machine learning predictive model for early candidemia diagnosis in ICU patients, leveraging their clinical information and findings. We conducted this study with a cohort of 334 patients admitted to the ICU unit at Ji Ning NO.1 people's hospital in China from Jan. 2015 to Dec. 2022. To ensure the model's reliability, we validated this model with an external group consisting of 77 patients from other sources. The candidemia to bacteremia ratio is 1:1. We collected relevant clinical procedures and eighteen key examinations or tests features to support the recursive feature elimination (RFE) algorithm. These features included total bilirubin, age, platelet count, hemoglobin, CVC, lymphocyte, Duration of stay in ICU and so on. To construct the candidemia diagnosis model, we employed random forest (RF) algorithm alongside other machine learning methods and conducted internal and external validation with training and testing sets allocated in a 7:3 ratio. The RF model demonstrated the highest area under the receiver operating characteristic (AUC) with values of 0.87 and 0.83 for internal and external validation, respectively. To evaluate the importance of features in predicting candidemia, Shapley additive explanation (SHAP) values were calculated and results revealed that total bilirubin and age were the most important factors in the prediction model. This advancement in candidemia prediction holds significant promise for early intervention and improved patient outcomes in the ICU setting, where timely diagnosis is of paramount crucial.

Abstract:Background: Interventions to better prevent or manage Clostridioides difficile infection (CDI) may significantly reduce morbidity, mortality, and healthcare spending. Methods: We present a retrospective study using electronic health record data from over 700 United States hospitals. A subset of hospitals was used to develop machine learning algorithms (MLAs); the remaining hospitals served as an external test set. Three MLAs were evaluated: gradient-boosted decision trees (XGBoost), Deep Long Short Term Memory neural network, and one-dimensional convolutional neural network. MLA performance was evaluated with area under the receiver operating characteristic curve (AUROC), sensitivity, specificity, diagnostic odds ratios and likelihood ratios. Results: The development dataset contained 13,664,840 inpatient encounters with 80,046 CDI encounters; the external dataset contained 1,149,088 inpatient encounters with 7,107 CDI encounters. The highest AUROCs were achieved for XGB, Deep Long Short Term Memory neural network, and one-dimensional convolutional neural network via abstaining from use of specialized training techniques, resampling in isolation, and resampling and output bias in combination, respectively. XGBoost achieved the highest AUROC. Conclusions: MLAs can predict future CDI in hospitalized patients using just 6 hours of data. In clinical practice, a machine-learning based tool may support prophylactic measures, earlier diagnosis, and more timely implementation of infection control measures. (c) 2021 The Author(s). An Interpretable Machine Learning Model for Accurate Prediction of Sepsis in the ICU

Abstract:Objectives: Sepsis is among the leading causes of morbidity, mortality, and cost overruns in critically ill patients. Early intervention with antibiotics improves survival in septic patients. However, no clinically validated system exists for real-time prediction of sepsis onset. We aimed to develop and validate an Artificial Intelligence Sepsis Expert algorithm for early prediction of sepsis. Design: Observational cohort study. Setting: Academic medical center from January 2013 to December 2015. Patients: Over 31,000 admissions to the ICUs at two Emory University hospitals (development cohort), in addition to over 52,000 ICU patients from the publicly available Medical Information Mart for Intensive Care-III ICU database (validation cohort). Patients who met the Third International Consensus Definitions for Sepsis (Sepsis-3) prior to or within 4 hours of their ICU admission were excluded, resulting in roughly 27,000 and 42,000 patients within our development and validation cohorts, respectively. Interventions: None. Measurements and Main Results: High-resolution vital signs time series and electronic medical record data were extracted. A set of 65 features (variables) were calculated on hourly basis and passed to the Artificial Intelligence Sepsis Expert algorithm to predict onset of sepsis in the proceeding T hours (where T = 12, 8, 6, or 4). Artificial Intelligence Sepsis Expert was used to predict onset of sepsis in the proceeding T hours and to produce a list of the most significant contributing factors. For the 12-, 8-, 6-, and 4-hour ahead prediction of sepsis, Artificial Intelligence Sepsis Expert achieved area under the receiver operating characteristic in the range of 0.83–0.85. Performance of the Artificial Intelligence Sepsis Expert on the development and validation cohorts was indistinguishable. Conclusions: Using data available in the ICU in real-time, Artificial Intelligence Sepsis Expert can accurately predict the onset of sepsis in an ICU patient 4–12 hours prior to clinical recognition. A prospective study is necessary to determine the clinical utility of the proposed sepsis prediction model.

Abstract:The aim of the study was to build a machine learning-based predictive model to discriminate between hospitalized patients at low risk and high risk of bloodstream infection (BSI). A Data Mart including all patients hospitalized between January 2016 and December 2019 with suspected BSI was built. Multivariate logistic regression was applied to develop a clinically interpretable machine learning predictive model. The model was trained on 2016–2018 data and tested on 2019 data. A feature selection based on a univariate logistic regression first selected candidate predictors of BSI. A multivariate logistic regression with stepwise feature selection in five-fold cross-validation was applied to express the risk of BSI. A total of 5660 hospitalizations (4026 and 1634 in the training and the validation subsets, respectively) were included. Eleven predictors of BSI were identified. The performance of the model in terms of AUROC was 0.74. Based on the interquartile predicted risk score, 508 (31.1%) patients were defined as being at low risk, 776 (47.5%) at medium risk, and 350 (21.4%) at high risk of BSI. Of them, 14.2% (72/508), 30.8% (239/776), and 64% (224/350) had a BSI, respectively. The performance of the predictive model of BSI is promising. Computational infrastructure and machine learning models can help clinicians identify people at low risk for BSI, ultimately supporting an antibiotic stewardship approach.

Abstract:Background: Central line-associated bloodstream infections (CLABSIs) are associated with significant morbidity, mortality, and increased healthcare costs. Despite the high prevalence of CLABSIs in the U.S., there are currently no tools to stratify a patient's risk of developing an infection as the result of central line placement. To this end, we have developed and validated a machine learning algorithm (MLA) that can predict a patient's likelihood of developing CLABSI using only electronic health record data in order to provide clinical decision support. Methods: We created three machine learning models to retrospectively analyze electronic health record data from 27,619 patient encounters. The models were trained and validated using an 80:20 split for the train and test data. Patients designated as having a central line procedure based on International Statistical Classification of Diseases and Related Health Problems 10 codes were included. Results: XGBoost was the highest performing MLA out of the three models, obtaining an AUROC of 0.762 for CLABSI risk prediction at 48 hours after the recorded time for central line placement. Conclusions: Our results demonstrate that MLAs may be effective clinical decision support tools for assessment of CLABSI risk and should be explored further for this purpose. (c) 2021 The Author(s). Abstract:Abstract Background Bacterial bloodstream infection is responsible for the majority of cases of sepsis and septic shock. Early recognition of the causative pathogen is pivotal for administration of adequate empiric antibiotic therapy and for the survival of the patients. In this study, we developed a feasible machine learning (ML) model to predict gram-positive and gram-negative bacteremia based on routine laboratory parameters. Methods Data for 2118 patients with bacteremia were obtained from the Medical Information Mart for Intensive Care dataset. Patients were randomly split into the training set and test set by stratified sampling, and 374 routine laboratory blood test variables were retrieved. Variables with missing values in more than 40% of the patients were excluded. Pearson correlation test was employed to eliminate redundant features. Five ML algorithms were used to build the model based on the selected features. Additionally, 132 patients with bacteremia who were treated at Qilu Hospital of Shandong University were included in an independent test cohort to evaluate the model. Results After feature selection, 32 variables remained. All the five ML algorithms performed well in terms of discriminating between gram-positive and gram-negative bacteremia, but the performance of convolutional neural network (CNN) and random forest (RF) were better than other three algorithms. Consider of the interpretability of models, RF was chosen for further test (ROC-AUC = 0.768; 95%CI = 0.715–0.798, with a sensitivity of 75.20% and a specificity of 63.79%). To expand the application of the model, a decision tree (DT) was built utilizing the major variables, and it achieved an AUC of 0.679 (95%CI = 0.632–0.723), a sensitivity of 66%, and a specificity of 67.82% in the test cohort. When tested in the Qilu Hospital cohort, the ROC-AUC of the RF and DT models were 0.666 (95%CI = 0.579–0.746) and 0.615 (95%CI = 0.526–0.698), respectively. Finally, a software was developed to make the RF- and DT-based prediction models easily accessible. Conclusion The present ML-based models could effectively discriminate between gram-positive and gram-negative bacteremia based on routine laboratory blood test results. This simple model would be beneficial in terms of guiding timely antibiotic selection and administration in critically ill patients with bacteremia before their pathogen test results are available.

Abstract:In most classical scheduling models, it is assumed that a job is dispatched to a customer immediately after its processing completes. In many practical situations, however, a set of delivery dates may be fixed before any jobs are processed. This is particularly relevant where delivery is an expensive or complicated operation, for example, as with heavy machinery. A similar situation arises where customers find deliveries disruptive and thus require them to be made within a limited time interval that repeats periodically. A third possibility is that a periodic business function, for example, the supplier's billing cycle, effectively defines a delivery date, and includes all jobs that have been completed since the previous billing cycle. These situations are not adequately represented by classical scheduling models. We consider a variety of deterministic scheduling problems in which a job is dispatched to a customer at the earliest fixed delivery date that is no earlier than the completion time of its processing. Problems where the number of delivery dates is constant, and others where it is specified as part of data input, are studied. For almost all problems considered, we either provide an efficient algorithm or establish that such an algorithm is unlikely to exist. By doing so, we permit comparisons between the solvability of these fixed delivery date problems and of the corresponding classical scheduling problems.

Abstract:Background Artificial intelligence (AI) holds significant potential to be a valuable tool in healthcare. However, its application for predicting bacteremia among adult febrile patients in the emergency department (ED) remains unclear. Therefore, we conducted a study to provide clarity on this issue. Methods Adult febrile ED patients with blood cultures at Chi Mei Medical Center were divided into derivation (January 2017 to June 2019) and validation groups (July 2019 to December 2020). The derivation group was utilized to develop AI models using twenty-one feature variables and five algorithms to predict bacteremia. The performance of these models was compared with qSOFA score. The AI model with the highest area under the receiver operating characteristics curve (AUC) was chosen to implement the AI prediction system and tested on the validation group. Results The study included 5,647 febrile patients. In the derivation group, there were 3,369 patients with a mean age of 61.4 years, and 50.7% were female, including 508 (13.8%) with bacteremia. The model with the best AUC was built using the random forest algorithm (0.761), followed by logistic regression (0.755). All five models demonstrated better AUC than the qSOFA score (0.560). The random forest model was adopted to build a real-time AI prediction system integrated into the hospital information system, and the AUC achieved 0.709 in the validation group. Conclusion The AI model shows promise to predict bacteremia in adult febrile ED patients; however, further external validation in different hospitals and populations is necessary to verify its effectiveness.
