Abstract:<p>Superb microvascular imaging (SMI) is an innovative Doppler technique for vascular examination. It uses an intelligent algorithm that efficiently separates low-speed flow signals from motion artifacts so that it can assess microvessels and the vessel distribution in detail. This article reviews the clinical applications of SMI in the disorders of superficial tissues and organs including thyroid nodules, breast tumors and lymph node diseases etc. More information of diseases that are closely associated with angiogenesis can be shown by SMI than other noninvasive examinations. Although some limitations exist, this safe and convenient technique is becoming acceptable and would play a more important role in disease diagnosis and therapeutic responses evaluation.</p>

Abstract:Objective To discuss the diagnostic value of superb micro-vascular imaging(SMI) technology in thyroid microcarcinoma.Methods The routine ultrasound and SMI of 140 micro-nodule in 122 cases of papillary thyroid micro carcinoma were analyzed retrospectively.All were confirmed by surgical pathology or fine needle aspiration (FNA) cytology.Results With SMI technology.It had special advantage in display the micro vessels and perforators inside the thyroid micro-carcinoma.There was significant difference(P =0.001) between CDFI and SMI in vessels showing rate and amounts.In 140 thyroid micro-carcinoma nodules,the diagnostic coincidence rate by routine ultrasound was 71.4％.After using SMI,the diagnostic coincidence rate was 90.0％,Contrasting SMI with routine ultra sound respectively(P=0.001).Conclusions SMI technology can obviously improve the diagnostic accuracy in diagnosing thyroid micro-carcinoma.The technology will provide a new approach to make an early accurate diagnosis of thyroid micro-carcinoma.

Abstract:Superb microvascular imaging (SMI) as the latest Doppler ultrasound method uses an advanced clutter filter to eliminate artifacts caused by breathing, movement and so on and retains the low‐speed blood signals in microvessels. This review offers the application of SMI in various clinical filed.Superb microvascular imaging (SMI) is among the latest doppler ultrasound methods. It uses an advanced clutter filter to eliminate artifacts caused by breathing, movement and retains the low‐speed blood signals in microvessels. The great advantage of SMI is that it can intuitively detect very slow blood signals in microvessels, providing clinicians with more significant information about flow distribution in the target area. Therefore, it is speculated that SMI has important application value. The purpose of this article is to outline the application of SMI in different parts of the body.

Abstract:Objective To compare the visualization ability for thyroid nodular among superb micro-vascular imaging (SMI), power Doppler flow imaging (PDFI), and color Doppler flow imaging(CDFI), and determine optimal vascular findings for the diagnosis of thyroid nodules. Methods A total of 58 thyroid nodules from 50 patients were enrolled, including 27 benign nodules (benign group) and 31 malignant nodules (malignant group). Vascular patterns of these nodules were detected by SMI, PDFI and CDFI, respectively. Results Thyroid cancer was characterized by type III vascular pattern. Only central vascularity performed best in detecting thyroid cancers. When using the criterion, SMI performed a better specificity (96.3%) and sensitivity (77.4%) than PDFI (92.5%, 41.9%) and CDFI (88.9%, 48.3%); and the specificity of SMI was significantly higher than the others (all P<0.01). Conclusions The sign of only central is of value in distinguishing malignant thyroid nodules. SMI is better on presenting thyroid nodular than CDFI and PDFI, and it could be used to differentiate thyroid nodules. Abstract:Background As a novel ultrasonic technique, superb microvascular imaging (SMI) can quickly, simply and noninvasively observe the microvascular distribution in a tumor and evaluate the microvascular perfusion. Previous studies have shown that SMI can detect the blood flow signals of neovascularization in tumors and increase the sensitivity for detecting thyroid cancer. However, the results of these studies have been contradictory, and the sample sizes were too small. Therefore, the present meta-analysis will aim at evaluating the value of a thyroid imaging report and data system (TI-RADS) combined with SMI in distinguishing between benign and malignant thyroid nodules. Methods We will search PubMed, Web of Science, Cochrane Library, Google Scholar, and Chinese biomedical databases from their inceptions to the June 31, 2020. Two authors will independently carry out searching literature records; scanning titles, abstracts, and full texts; collecting data; and assessing risk of bias. Review Manager 5.2 and Stata14.0 software will be used for data analysis. Results This systematic review will evaluate the value of TI-RADS combined with SMI in distinguishing between benign and malignant thyroid nodules. Systematic review registration INPLASY202070113.

Abstract:Objective Ultrasound imaging provides a fast and safe examination of thyroid nodules. Recently, the introduction of super-resolution imaging technique shows the capability of breaking the Ultrasound diffraction limit in imaging the micro-vessels. The aim of this study was to evaluate its feasibility and value for the differentiation of thyroid nodules. Methods In this study, B-mode, contrast-enhanced ultrasound, and color Doppler flow imaging examinations were performed on thyroid nodules in 24 patients. Super-resolution imaging was performed to visualize the microvasculature with finer details. Microvascular flow rate (MFR) and micro-vessel density (MVD) within thyroid nodules were computed. The MFR and MVD were used to differentiate the benign and malignant thyroid nodules with pathological results as a gold standard. Results Super-resolution imaging (SRI) technique can be successfully applied on human thyroid nodules to visualize the microvasculature with finer details and obtain the useful clinical information MVD and MFR to help differential diagnosis. The results suggested that the mean value of the MFR within benign thyroid nodule was 16.76 ± 6.82 mm/s whereas that within malignant thyroid was 9.86 ± 4.54 mm/s. The mean value of the MVD within benign thyroid was 0.78 while the value for malignant thyroid region was 0.59. MFR and MVD within the benign thyroid nodules were significantly higher than those within the malignant thyroid nodules respectively ( p < 0.01). Conclusions This study demonstrates the feasibility of ultrasound super-resolution imaging to show micro-vessels of human thyroid nodules via a clinical ultrasound platform. The important imaging markers, such as MVD and MFR, can be derived from SRI to provide more useful clinical information. It has the potential to be a new tool for aiding differential diagnosis of thyroid nodules.

Added Value of Superb Microvascular Imaging and Virtual Touch Imaging Quantification in Assisting Thyroid Cancer Classification

Abstract:This prospective study determined the value added by superb microvascular imaging (SMI) and Virtual Touch imaging quantification (VTIQ) to conventional ultrasonography in classifying malignant and benign thyroid nodules. One hundred eighty-three thyroid nodules (TNs) in 120 patients (112 benign and 71 malignant TNs) were evaluated. SMI revealed noticeable variance between benign and malignant TNs (p < 0.001). Malignant nodules tended to have rich vascularity (grade 3: 38/71, 53.5%) compared with benign nodules (grade 3: 33/112, 29.5%). There is a statistically significant difference between malignant and benign TNs with respect to shear-wave speed (SWS) values (all p values <0.001). The SWS mean, maximum and ratio of malignant nodules were 3.97 ± 1.34, 4.79 ± 1.70 and 1.25 ± 0.39, respectively. The SWS mean, maximum and ratio of benign nodules were 2.65 ± 0.42, 2.97 ± 0.46 and 1.15 ± 0.35, respectively. With respect to area under the curve values, the combined use of SMI or VTIQ improved the diagnostic performance of classifying malignant and benign TNs compared with that of ultrasonography alone. The combination of three modalities achieved the greatest area under the curve values (0.9811, 95% confidence interval: 0.95529–1.000), followed by US + VTIQ (0.9747, 0.94543–1.000), US + SMI (0.9032, 0.85345–0.95391) and ultrasonography (0.8291, 0.76417–0.89403).

Abstract:Objective: To investigate the ability of superb microvascular imaging (SMI) to improve the differential diagnosis of mummified thyroid nodules (MTNs) and papillary thyroid carcinomas (PTCs) using the 2017 American College of Radiology Thyroid Imaging Reporting and Data System (ACR-TIRADS). Materials and methods: We enrolled 110 cases of MTNs and 110 cases of PTCs confirmed by fine-needle aspiration (FNA) or surgery. Conventional ultrasound (US) and SMI were analyzed for all nodules. Thyroid nodules were initially categorized by ACR-TIRADS and ACR-TIRADS combined with SMI (SMI-TIRADS). We compared the diagnostic performances of ACR-TIRADS and SMI-TIRADS by receiver operating characteristic curve, sensitivity, specificity, accuracy, positive predictive value (PPV), and negative predictive value (NPV). Results: US-detected margin, shape, and echogenic foci differed between MTNs and PTCs (P<0.05). The SMI blood-flow grade was greater in PTCs compared with MTNs (c2=158.78, P<0.05). There was no significant difference in ACR-TIRADS indicators between MTNs and PTCs (c2=1.585, P=0.453); reclassification by SMI-TIRADS showed significant differences between the groups (c2=129.521, P<0.001). The area under the curve was lower for ACR-TIRADS compared with SMI-TIRADS (0.517 vs 0.887, P<0.05). SMI-TIRADS had higher diagnostic value for distinguishing MTNs and PTCs than ACR-TIRADS (sensitivity: 91.82% vs 74.55%, P<0.05; specificity: 84.55% vs 21.82%, P<0.05; accuracy: 88.18% vs 48.18%, P<0.05; PPV: 85.59% vs 48.81%, P<0.05; NPV: 91.18% vs 46.15%, P<0.05). Conclusion: The detection of micrangium and large vessels in thyroid nodules by SMI resulted in high diagnostic specificity and sensitivity. ACR-TIRADS combined with SMI could effectively distinguish between MTNs and PTCs, to avoid unnecessary FNA or surgery.

Abstract:Background: Neovascularity visualization in breast nodules is challenging due to the limitations of conventional Doppler imaging methods. This study aims to assess the performance of superb microvascular imaging (SMI) in evaluating the microvascularity of breast nodules (diameter ≤2 cm). The comparison of performances of SMI with color Doppler flow imaging (CDFI) and power Doppler imaging (PDI) was made by using a three-factor scoring system of vascularity. This study also investigated the common features of microvascularity in small malignant nodules on SMI for early differentiating from benign nodules. Methods: Ninety-one female patients (with 125 breast nodules) were enrolled in this retrospective study. All the breast nodules were examined by grayscale ultrasonography (US), CDFI, PDI, and SMI. The number, morphologic features, and distribution of blood vessels were scored to evaluate the nodular vascularity in light of the three-factor scoring system. The diagnostic value of SMI for microvascularity in breast nodules was analyzed and compared with CDFI and PDI. Results: Histological analysis showed 53 malignant and 72 benign nodules. The vascularity grades detected by SMI were significantly different from those of CDFI and PDI (P<0.05). SMI detected 47 grade-IV nodules of the total 125 nodules (37.6%), which was more than those detected by CDFI (10.4%, 13/125) and PDI (12.8%, 16/125), while more grade-I nodules were detected by CDFI (42.4%, 53/125) and PDI (36.8%, 46/125) compared with SMI (21.6%, 27/125). Differences in the vessel number, morphologic features, and distribution between benign and malignant breast nodules were significant on SMI (P<0.05). The vessel number ≥6, penetrating vessels, and a mixed distribution of vessels in peripheral and central nodular tissues were the common features of microvascularity in the grade-IV malignant nodules on SMI, whereas the blood vessels in the benign nodules were straight and branching and peripherally distributed. Conclusions: In comparison with CDFI and PDI, SMI enhances microvascularity detection, depicts the microvascular architecture in breast nodules and has potential in the differential diagnosis of malignant nodules from benign nodules.

Abstract:Thyroid disease has always been a common and frequent disease in clinical medicine, and its disease detection rate has been increasing year by year. Thyroid diseases are mainly divided into two categories: thyroid diseases treated by medical treatment and thyroid diseases treated by surgery. Thyroid cancer has also become one of the most common malignant secretory tumor diseases today. Ultrasound examination is a commonly used method for diagnosing thyroid diseases. During the diagnosis process, doctors need to observe the characteristics of ultrasound images and combine professional knowledge and clinical experience to give the patient's disease status. With the improvement of people's living standards and health awareness, thyroid disease has become an important issue that plagues the health of Chinese residents. Therefore, people and medical workers are paying more attention to thyroid disease. In recent years, various ultrasound technologies have been applied in the differential diagnosis of benign and malignant thyroid nodules and have played an important role in the diagnosis. This article aims to study the application value of SMI technology (ultra-microvascular imaging technology) and contrast-enhanced ultrasound in the differential diagnosis of thyroid benign and malignant nodules. It conducts diagnostic experiments and analysis on some cases of benign and malignant thyroid nodules through the use of SMI diagnostic methods and contrast-enhanced ultrasound examination methods. And the ROC curve was used to calculate the sensitivity of SMI technology and ultrasound for the identification and diagnosis of thyroid benign and malignant nodules, and the results were 0.83 and 0.81, respectively. It is concluded that SMI technology and contrast-enhanced ultrasound examination have good diagnostic efficiency and application value for the identification and diagnosis of thyroid benign and malignant nodules.

Abstract:AIM To explore the ability of superb microvascular imaging (SMI) in differential diagnosis of focal liver lesions (FLLs) and to compare SMI morphology findings to those of color Doppler ultrasound and enhanced imaging. METHODS Twenty-four patients with 31 FLLs were included in our study, with diagnoses of hemangioma (HE) (n = 17), hepatocellular carcinoma (HCC) (n = 5), metastatic lesions (n = 5), primary hepatic lymphoma (n = 1), focal nodular hyperplasia (FNH) (n = 2), and adenoma (n = 1). Nine lesions were pathologically diagnosed, and 22 lesions were radiologically confirmed, all of which were evaluated by at least two types of enhanced imaging techniques. All patients had undergone SMI. Patients were divided into subgroups based on pathological and radiological diagnoses to analyze SMI manifestations. We also compared the SMI manifestations of the most common malignant FLLs (HCCs and metastatic lesions) with those of the most common benign FLLs (HEs). RESULTS HEs were classified into three SMI subgroups: diffuse dot-like type (n = 6), strip rim type (n = 8), and nodular rim type (n = 3). The sizes of the three types of HEs were significantly different (P = 0.00, < 0.05). HCCs were classified into two subgroups: diffuse honeycomb type (n = 2) and non-specific type (n = 3). Four of the metastatic lesions were the strip rim type, and the other metastatic lesion was the thick rim type, which is the same as that of lymphoma. FNH was described as a spoke-wheel type, and adenoma as a diffuse honeycomb type. The SMI types of HCCs and metastatic lesions were significantly different from those of HEs (P = 0.048, < 0.05). CONCLUSION SMI technology enables microvascular evaluation of FLLs without using any contrast agent. For HEs, lesion size may affect SMI performance. SMI is able to provide useful information for differential diagnosis of HCCs and metastatic lesions from HEs.

Abstract:This is a summary of a consensus statement on the introduction of "Ultrasound microvasculomics" produced by The Chinese Artificial Intelligence Alliance for Thyroid and Breast Ultrasound. The evaluation of microvessels is a very important part for the assessment of diseases. Super-resolution ultrasound (SRUS) microvascular imaging surpasses traditional ultrasound imaging in the morphological and functional analysis of microcirculation. SRUS microvascular imaging relies on contrast microbubbles to gain sensitivity to microvessels and improves the spatial resolution of ultrasound blood flow imaging for a more detailed depiction of vascular structures and hemodynamics. This method has been applied in preclinical animal models and pilot clinical studies, involving areas including neurology, oncology, nephrology, and cardiology. However, the current quantitative parameters of SRUS images are not enough for precise evaluation of microvessels. Therefore, by employing omics methods, more quantification indicators can be obtained, enabling a more precise and personalized assessment of microvascular status. Ultrasound microvasculomics - a high-throughput extraction of image features from SRUS images - is one novel approach that holds great promise but needs further validation in both bench and clinical settings.

Abstract:Purpose: We performed a systematic review and meta-analysis of studies that investigated the diagnostic performance of Superb Microvascular Imaging (SMI) in differentiating between benign and malignant breast tumors. Methods: Studies published between January 2010 and March 2022 were retrieved by online literature search conducted in PubMed, Embase, Cochrane Library, Web of Science, China Biology Medicine Disc, China National Knowledge Infrastructure, Wanfang, and Vip databases. Pooled sensitivity, specificity, and diagnostic odd ratios were calculated using Stata software 15.0. Heterogeneity among the included studies was assessed using I2 statistic and Q test. Meta-regression and subgroup analyses were conducted to investigate potential sources of heterogeneity. Influence analysis was conducted to determine the robustness of the pooled conclusions. Deeks' funnel plot asymmetry test was performed to assess publication bias. A summary receiver operating characteristic curve (SROC) was constructed. Results: Twenty-three studies involving 2749 breast lesions were included in our meta-analysis. The pooled sensitivity and specificity were 0.80 (95% confidence interval [CI], 0.77–0.84, inconsistency index [I2] = 28.32%) and 0.84 (95% CI, 0.79–0.88, I2 = 89.36%), respectively. The pooled diagnostic odds ratio was 19.95 (95% CI, 14.84–26.82). The area under the SROC (AUC) was 0.85 (95% CI, 0.81–0.87). Conclusion: SMI has a relatively high sensitivity, specificity, and accuracy for differentiating between benign and malignant breast lesions. It represents a promising supplementary technique for the diagnosis of breast neoplasms.

Abstract:The TI‐RADS classification standard scores suspicious nodules that are difficult to determine benign and malignant as grade 4, and further pathological puncture is recommended clinically, which may lead to a large number of unnecessary biopsies and operations. Thyroid nodule is a common and frequently occurring disease in the neck in recent years, and ultrasound has become the preferred imaging diagnosis method for thyroid nodule due to its advantages of noninvasive, nonradiation, real‐time, and repeatable. The thyroid imaging, reporting and data system (TI‐RADS) classification standard scores suspicious nodules that are difficult to determine benign and malignant as grade 4, and further pathological puncture is recommended clinically, which may lead to a large number of unnecessary biopsies and operations. Including conventional ultrasound, ACR TI‐RADS, shear wave elastography, super microvascular imaging, contrast enhanced ultrasound, "firefly," artificial intelligence, and multimodal ultrasound imaging used in combination. In order to identify the most clinically significant malignant tumors when reducing invasive operations. This article reviews the application and research progress of multimodal ultrasound imaging in the diagnosis of TI‐RADS 4 thyroid nodules.

The Use of Superb Microvascular Imaging in Evaluating Rheumatic Diseases: A Systematic Review

Abstract:Background and Objectives: Superb microvascular imaging is an advanced Doppler algorithm that seems to be useful in detecting low-velocity blood flow without using a contrast agent. Increasing evidence suggests that SMI is a more sensitive tool than conventional Doppler techniques for evaluating rheumatic diseases, especially inflammatory arthritis. We aimed to assess the use of SMI in evaluating joints and extraarticular structures. Materials and Methods: Two reviewers independently reviewed the literature to provide a global overview of the possibilities of SMI in rheumatology. Original English-language articles published between February 2014 and November 2022 were identified through database (PubMed, Medline, Ebsco, the Cochrane Library, and ScienceDirect) searching, and analysed to summarise existing evidence according to PRISMA methodology. Inclusion criteria covered original research articles reporting applications of SMI on rheumatic diseases and musculoskeletal disorders secondary to rheumatic conditions. Qualitative data synthesis was performed. Results: A total of 18 articles were included. No systematic reviews fulfilled our inclusion criteria. Most studies focused on characterising the synovial vascularity of rheumatoid arthritis. There have been several attempts to demonstrate SMI's value for evaluating extra-articular soft tissues (fat pads or salivary glands) and large-diameter vessels. The quantitative importance of SMI vascular indices could become a useful non-invasive diagnostic marker. Studies on therapeutic applications are still scarce, and the majority of studies have gaps in reporting the methodology (ultrasound performance technique and settings) of the research. Conclusions: SMI has proved to be useful in characterising low-flow vascularity, and growing evidence indicates that SMI is a non-invasive and lower-cost tool for prognostic assessment, especially in inflammatory arthritis. Preliminary findings also suggest potential interest in evaluating the effect of treatment.
Superb Micro-Vascular Imaging, A Novel Ultrasound Technique in Focal Liver Lesions Evaluation

Abstract:The purpose of this study is to describe the characteristic of SMI and compare the clinical value of SMI to contrast-enhanced ultrasound/CT/MRI in evaluating focal liver lesions. Twenty-four patients with 31 focal liver lesions were involved in our study, the lesions consisted of hepatocellular carcinoma (n=5), Diffuse large B lymphoma (n=1), adenoma (n=1), metastatic lesions (n=5), focal nodular hyperplasia (n=2), hemangioma (n=17). Nine lesions were pathologically diagnosed and 22 lesions were radiologically confirmed. The mean value of the largest diameter of the lesions was 3.9cm (1.1cm ∼ 8.5cm). All patients had been done SMI examination for the liver lesions, and at least had been done one of the listed imaging technology (contrast enhanced ultrasound, enhanced CT or enhanced MRI). The patients were divided into subgroups based on the characteristic of SMI results and compared the clinical value of SMI in diagnoses of focal liver lesions to the above three imaging technology. The SMI manifestations of hemangiomas were described as three subgroups: diffuse dot-like type (n=6); strip rim type (n=8); nodular rim type (n=3), the largest diameters of the three groups were obviously different (P=0.0, <0.05) and these findings were corresponding to the enhanced ultrasound/CT/MRI. The hepatocellular carcinoma were described as two subgroup: diffuse honeycomb type (n=2); strip trunk with tiny branches (n=3). Four of the metastatic lesions were from breast cancer in one patient, the SMI manifestation was like the second type of hemangioma with strip rim; the other metastatic lesion was from pancreatic cancer had the same SMI type of lymphoma with thick rim type. All of the FNH was described as spoke-wheel type, the adenoma was described as diffuse honeycomb type. SMI technology made the evaluation of micro-vascular of focal liver lesions possible without any enhanced agents. The SMI type were obviously different from different lesions, which could provide additional diagnostic information. While some lesions had the same type, this result was need further research with large sample size.
Correlation and agreement between superb micro-vascular imaging and contrast-enhanced ultrasound for assessing radiofrequency ablation treatment of thyroid nodules: a preliminary study

Abstract:Abstract Background To evaluate the correlation and agreement between superb micro-vascular imaging (SMI) mode and the contrast-enhanced ultrasound (CEUS) mode for the ablative completeness and the volumes of ablation lesions to determine the clinical application value of SMI in follow-up after radiofrequency ablation. Methods From April 2020 to June 2020, two radiologists used SMI and CEUS mode to measure the volume of the ablation lesion. We use intra-class correlation coefficient (ICC), scatter plots and Bland–Altman plots to evaluate the correlation and agreement of the two techniques. In addition, intra- and inter-observer reliability in volume measurement of ablation lesions with SMI mode was assessed. Results SMI mode and CEUS mode have good agreement in the evaluation of ablative completeness. The ICC was 0.876 and 0.928 of reader A and reader B between SMI mode and CEUS mode in terms of ablation lesions volume measurement. There was a strong correlation between the two modes in both reader A and reader B (r A = 0.808; r B = 0.882). The ICC was 0.836 for the inter-observer reliability of SMI technique. The scatter plot showed a good linear relation (r = 0.715). In the Bland–Altman plot, 4.35% (1/23) of the points was outside the 95% limits of agreement. The ICC was 0.965 for the intra-observer reliability of SMI technique, the scatter plot also showed a strong linear correlation (r = 0.965). In the Bland–Altman plot, 8.70% (2/23) of the points was outside the 95% limits of agreement. Conclusions SMI and CEUS have good agreement and correlation in the ablation volume measurement. SMI technology is expected to be applied as an alternative to CEUS in the clinical follow-up of ablation lesions.
