Abstract:Introduction: Idiopathic granulomatous mastitis (IGM), also known as nonpuerperal mastitis or granulomatous lobular mastitis, is a rare, benign, chronic inflammatory breast disease first characterized in 1972. IGM is characterized by sterile noncaseating lobulocentric granulomatous inflammation, usually affecting parous premenopausal women with a history of lactation. Case presentation: We present a 38-year-old African American woman presenting to our continuity clinic complaining of a painful right breast mass discovered during a self-breast examination. An initial right breast ultrasound demonstrated an area of concern with asymmetry, architectural distortion, and a questionable mass correlating to a 5.1 cm hypoechoic mass at the 12:30 position. Additionally, a mammogram showed a suspicious, indeterminate, large hypoechoic region or mass with angular margins measuring 5.1 x 1.7 x 3.7 cm with slight internal vascularity. An ultrasound-guided stereotactic biopsy revealed extensive mixed inflammatory and focal granulomatous lobular mastitis. Conclusion: Idiopathic granulomatous mastitis is a rare disorder that often mimics breast malignancies and infectious processes. This case highlights the need for further research on IGM pathogenesis to prevent unnecessary life-altering therapeutic procedures and bring awareness to the medical community.
Malignant Granular Cell Tumor with Breast Metastasis: A Case Report and Review of the Literature

Abstract:Malignant granular cell tumors (MGCT) are rare mesenchymal soft tissue neoplasms of Schwann cell origin without adequate follow-up. This study describes a case of MGCT with right breast metastasis following two local recurrences. The patient consented to a right breast lumpectomy with right axillary dissection, a right abdominal wall lumpectomy and a right inguinal lumpectomy with dissection. Pathological examination revealed that the two initial lesions were consistent with benign histological performance. The later lesions were classified as malignant due to the observation of spindling of the tumor cells, vesicular nuclei with large nucleoli and increased mitotic rate. Immunohistochemical study of the lesions revealed positivity for S100 protein. The Ki-67 proliferation index increased from 1 to 10%. Twenty-seven months after surgery, the patient was in good health with no sign of further tumor development. We recommend wide local excision with regional lymph node dissection as the first choice of treatment for MGCT.

Abstract:Granulomatous mastitis (GM) is a rare, benign inflammatory breast disease that predominantly affects women of childbearing age and often mimics breast carcinoma. The diagnosis requires histopathological examination due to nonspecific imaging findings. Treatment includes antibiotics, corticosteroids, and surgery, but no standardized protocols exist. This autobiographical case report describes a 34-year-old woman with a tender breast lump following trauma, initially misdiagnosed as a simple abscess. Despite incision and drainage, she developed erythema nodosum, persistent fever, and arthritis, which responded to corticosteroids. Further investigation, including an ultrasound-guided biopsy and MRI, confirmed GM. Recurrent symptoms were managed with prednisolone and doxycycline, leading to significant improvement. This case report aims to highlight the diagnostic challenges associated with GM, emphasizing the necessity for a detailed histopathological examination to achieve an accurate diagnosis. It also brings attention to the significant emotional impact on patients facing a rare and complex diagnosis. By presenting this case, we aim to highlight the critical importance of a comprehensive and multidisciplinary approach to patient care in managing GM effectively.
Differential Diagnosis of Imaging Fmdings Between Patients with Triple-Negative Breast Cancer and Patients with Idiopathic Granulomatous Mastitis

Abstract:Objective: To analyze the imaging findings of triple-negative breast cancer(TNBC) and to improve the understanding and diagnostic performance of the disease,by comparing it with idiopathic granulomatous mastitis(IGM).Methods: The imaging findings in 63 randomly assigned women with surgically and pathologically confirmed TNBC(n=45) and IGM(n=18) were retrospectively analyzed.All cases underwent a mammographic examination.An ultrasonic examination was performed in 37 cases of TNBC and 18 cases of IGM.MR examination was performed in 33 cases of TNBC and 14 cases of IGM.The mammographic imaging findings included lesion shape,density,margin,calcification,axillary adenopathy and thickening of the skin.The ultrasound imaging findings included shape,margin,size,echo,axillary adenopathy and thickening of the skin.The MR imaging findings included tumor shape,size,margin and internal enhancement,signal intensity within the tumor as well as peripheral signal intensity on T2-weighted,then the type of time-signal intensity curve(TIC) was analyzed.Results: The results of comparing TNBC group with IGM group showed as follows: ①Mammography imaging findings: mass type accounts for 67%(30/45) in TNBC group and 39%(7/18) in IGM group.Smooth margin of the lesion accounts for 60%(18/30) in TNBC group,while 29%(2/7) in IGM group.②Ultrasound imaging findings: mass type accounts for 92%(34/37) in TNBC group and 89%(16/18) in IGM group.Smooth margin of the lesion accounts for 62%(21/34) in TNBC group,while 37%(6/16) in IGM group.③MR imaging findings: mass type accounts for 91%(30/33) in TNBC group and 86%(12/14) in IGM group.Rim enhancement accounts for 67%(20/30) in TNBC group and 75%(9/12) in IGM group.For the rim enhancement mass type,smooth margin of the lesion accounts for 70%(14/20) in TNBC group,while 11%(1/9) in IGM group;peripheral hyperintense pattern on T2-weighted accounts for 85%(17/20) in TNBC group and 21%(2/9) in IGM group.Conclusion: We believe that through careful observation of the lesion site,shape,edge of the tumor mass and peripheral signal intensity on T2-weighted,enhancement pattern and the TIC types,might be useful for the identification of TNBC and IGM.

Abstract:Background: Granulomatous lobular mastitis (GLM) is a rare chronic inflammatory condition of the breast. The purpose of this study was to describe antituberculous treatment of GLM and the long-term follow-up outcome. Methods: This retrospective study included 22 patients who had been histopathologically diagnosed with GLM at the Second Hospital of Shandong University from January 2011 to March 2015. Clinical characteristics, ultrasonography and mammography findings, laboratory tests, treatment regimens, follow-up information, and recurrences were recorded. Results: All patients were female with a median age of 29 (range 23-44) years. The most common symptom was a breast mass with or without pain. Large irregular hypoechoic masses could be found in the breast ultrasounds of 13 patients. All patients received triple antituberculous therapy. During a median follow-up period of 40 months, 3 patients were lost to follow-up; of the remaining 19 patients, 18 achieved clinical complete remission and no recurrences were observed. Conclusion: GLM is an unusual benign breast condition that mimics breast carcinoma in its clinical and imaging presentation. Antituberculous therapy seems to be an effective alternative option in the treatment of GLM.

Abstract:BACKGROUND Polyacrylamide hydrogel (PAAG) injections were once common in breast augmentation and have been prohibited for augmentation mammaplasty in China since a large number of patients who underwent breast augmentation with PAAG injections have continued to seek medical advice as a result of related complications. Among all these complications, distant migration is relatively rare. CASE SUMMARY A 49-year-old female presented at the hospital with a one-year history of a vulvar lump. The sonography of the lump showed several subcutaneous fluid-filled regions from the left vulva to the pubic symphysis, which suggested possible fat liquefaction. An enhanced magnetic resonance imaging (MRI) revealed a cystic area, which was considered a benign lesion. Intraoperative observations showed that the mass did not have an obvious capsule, the subcutaneous tissue presented as a cavity, and some yellow material came out of this cavity. A culture of the drainage did not show bacterial contamination. Histopathology revealed a foreign body granuloma. After resection and closed drainage, lumps were successively observed in the left lower abdomen and the bilateral hypochondriac region with infections. Sonography found that the hypoechoic areas in the bilateral hypochondriac region seemed continuous with deep in the breasts. The patient reported that she had undergone surgery with PAAG injections 20 years ago after she was repeatedly asked about her past history. Finally, a diagnosis of distant migration of PAAG was made. CONCLUSION PAAG gel can migrate after long periods of time. A diagnosis should not be limited to the area where the symptom develops.
Recurrent Idiopathic Granulomatous Mastitis: A Case Report

Abstract:Idiopathic granulomatous mastitis is a rare benign breast disease of unknown aetiology mostly presenting as a breast abscess but not responding to usual conservative management with incision and drainage and frequently mimics breast cancer. We present a case 31-year-old female presented with complaints of right breast pain and redness who was initially diagnosed and treated as a case of breast abscess with repeated incision and drainage and antibiotics but did not improve. Later histopathology revealed granuloma with giant cell reaction and the patient was given a trial of steroids which showed no improvement. Wide local excision with a long course of broad-spectrum antibiotics was performed which led into remission. This case report highlights the importance of considering idiopathic granulomatous mastitis as differentials in non-responding breast abscesses. Histopathology for diagnosis and trial of wide local excision with a long course of broad-spectrum antibiotics as treatment may be done for management.

Abstract:Granular cell tumor of the breast (GCTB) is a rare tumor, particularly in lactating women. This tumor can clinically and radiologically mimic breast carcinoma, which poses particular problems. The association between GCTB and sex hormones should receive particular attention. The present study reports a case of GCTB in a lactating patient. In this tsudy, the case of a 29-year old female who presented with a mass in the right breast is decribed. Immunohistochemical and cytological analysis revealed a GCT and subsequently wide local excision was performed. At 15 months following surgery, the patient is well and no tumor recurrence has been identified. A comprehensive review of the literature was also performed to assess and compare all cases of GCTB, with particular attention to hyperestrogenic and hyperprolactinemic states. Further studies are required to explore the association between granular cell tumors and hyperestrogenic and hyperprolactinemic states.

Abstract:Background Idiopathic granulomatous lobular mastitis (IGLM) is a rare, chronic inflammatory breast disease without a known etiology. Even though the current literature proposes several treatment strategies, there is no universal consensus for long-term management. Case Presentation A 43-year-old White woman (gravida 5, para 4) presented with a 2-week history of right lower outer quadrant breast tenderness, heaviness, warmth, and redness. Mammography and ultrasound were concerning for inflammatory breast cancer. Biopsies returned as granulomatous mastitis without malignancy. After 8 months of unsuccessful therapy with prednisone and methotrexate, surgeons excised the breast tissue. Cultures and special stains were negative for other organisms. At the 7-month follow-up, no evidence of recurrence was seen. Conclusions As there remains no consensus behind the etiology or management of IGLM, our case demonstrates a reasonable and successful stepwise treatment beginning with medical therapy before proceeding to surgical cure. Because of possible malignancy risk with chronic IGLM, patients should not delay surgical excision if their condition remains refractory to medical therapy alone.

Abstract:Objective To investigate the diagnosis and treatment of idiopathicgranulomatous mastitis.Methods This study was to retrospectively review the clinical presentation,radiological investigation,histopathological features,treatment and outcome of idiopatbic granulomatoos mastitis of women presenting to Xuanwu Hospital between January 2002 and June 2010.Results Twenty-four patients with a mean age of 34.5 years presented with a diagnosis of idiopathic granulomatous mastitis.Patients presented with a palpable breast lump,breast abscess,fistula formation in different periods of the disease; the role of radiological imagings was found to be limited in differentiating idiopathic granulomatous mastitis from other inflammatory and maliguant conditions of the breast.All patients underwent a surgical procedure as the main treatment; in the form of excision or incision and drainage of the breast lesions. Mean follow-up was 47.38 ( range 6-96 ) months with recurrence in 3(12.5％) patients.Conclusions Idiopathic granulomatous mastitis presents clinically with a palpable breast lump.The diagnosis is often only made histopathologically after surgical excision or core biopsy.Wide excision of the lesions or incision and drainage of the lesion are the main treatment modalities.
Idiopathic granulomatous mastitis: imaging update and review

Abstract:Objectives: The purpose of this study was to review the imaging features of idiopathic granulomatous mastitis (IGM) with clinical and pathology correlation. Methods: With institutional review board (IRB) approval, a retrospective search of the surgical pathology database from January 2000 to July 2015 was performed. Clinical, imaging and histology findings were reviewed. Cases of granulomatous mastitis without a known source, diagnosed with percutaneous or surgical biopsy, were included in our analysis. Results: Seventeen cases of IGM were identified with imaging available for review. The majority of patients presented with a palpable abnormality, whereas a minority were asymptomatic with an abnormal screening mammogram. At imaging, IGM most often demonstrated a focal asymmetry at mammography, a hypoechoic mass with irregular or angular margins at ultrasound, and robust enhancement with mixed progressive and plateau kinetics at magnetic resonance imaging (MRI). Axillary lymph nodes were reactive in appearance at ultrasound. Molecular breast imaging performed in one case showed mild focal asymmetric radiotracer uptake. Conclusion: IGM is a rapidly progressive rare inflammatory condition of the breast resulting in non-necrotizing granuloma formation. Imaging features mimic breast carcinoma and diagnosis can be difficult. Radiologists' awareness of this condition is essential to prevent delayed or unnecessary treatment. Teaching points: • Idiopathic granulomatous mastitis is rapidly progressive inflammatory condition. • Imaging features may mimic breast carcinoma or infection. • Ultrasound shows irregular hypoechoic masses with increased vascularity and sinus tracts. • MRI shows irregular, enhancing masses or non-mass enhancement with microabscesses. • MRI is useful for assessment of breast involvement and response to treatment.
Steroid refractory granulomatous mastitis treated by top surgery: A case report

Abstract:Rationale: Granulomatous mastitis (GM) is a rare inflammatory disease and the presentation mimics infectious mastitis or breast cancer. The disease usually develops at the unilateral breast in women with breast-feeding history at their child-bearing age. Systemic steroids had been proposed as the first-line treatment, the combination of surgery was also recommended for complicated disease. However, recurrence might still happen in some rare cases. Few studies have addressed the management of such difficult situations. Patient concerns: We report the case of a 33-year-old androgynous and nulliparous woman who initially presented left breast erythematous swelling and was treated as infectious mastitis with debridement and antibiotics. Diagnosis: After wider excision for pathology, the diagnosis of GM was confirmed. Interventions: Steroids combined with methotrexate were prescribed. However, the symptoms only subsided temporarily and progressed to the contralateral side within 3 months. She finally underwent double-incision mastectomy and free nipple grafting. Outcomes: The surgery was completed uneventfully, and she had a satisfactory result with no more recurrence at the 6-month follow-up. Lesson: This GM case with the refractory treatment courses brought out the importance of surgical resection and was the first case report of treating GM with top surgery in the literature. Total mastectomy facilitated a highest complete remission rate of GM and may be advantageous for selected patients, especially in cases where steroids are intolerable.

Abstract:Granulomatous Lobular Mastitis (GLM), also known as idiopathic granulomatous mastitis s, is a type of non-puerperal mastitis. It is characterized by a palpable breast mass, which can rapidly progress to abscess formation and rupture. In some cases, patients may also experience accompanying symptoms such as low-grade fever, dry cough, lower limb erythema, and arthritis. [ 1 ] GLM was previously considered a rare chronic benign inflammatory breast disease, but its incidence has been rapidly increasing in the past 20 years. As of 2021, the Chinese Society of Breast Surgery has published Clinical Practice Guidelines for patients with non-lactational mastitis. [ 2 ] Various therapies such as steroids, [ 3 ] immunosuppressants, [ 4 ] antibiotics, [ 5 ] and surgical excision [ 6 ] are the main approaches. The choice of treatment method primarily depends on the clinical physician diagnostic habits and experience. In our hospital, a systematic and standardized treatment process has been developed in the long-term clinical treatment. In the retrospective analysis of 1309 patients who received standardized treatment, the recurrence rate was found to be approximately 3.8%. However, with an increase in the number of cases accumulated, a gradual increase in the number of patients with bilateral morbidity was observed.

Common features of low-temperature spin–charge separation and superlattice formation in multiferroic manganites and antiferromagnetic cuprates

Abstract:The paper reports on new results obtained for tetragonal quasi-2D antiferromagnetic Eu2CuO4 at temperatures below 30–40 K. A set of ferromagnetic resonance lines similar to ones obtained earlier for a number of multiferroic manganites (Sanina et al 2012 J. Phys.: Condens. Matter 24 346002), with similar electric polarization and local conductivity, were measured, i.e., a multiferroic behavior was revealed in Eu2CuO4 at low temperatures. We attribute these observations to the 1D superlattices that are formed due to phase separation and self-organization of charge carriers and occupy a small crystal volume, as in the multiferroic manganites studied earlier. Characteristic features of these superlattices include a spin–charge separation and conductivity in their ferromagnetic layers. It is supposed that superlattices are domain walls separating the volume antiferromagnetic domains. A similarity between the superlattices in a number of crystals having different symmetries and bulk properties suggests that a topological order similar to that of the topological insulator exists in them at low temperatures.
Granulomatous Mastitis Due to Non-Tuberculous Mycobacteria: A Diagnostic and Therapeutic Dilemma

Abstract:Non-tuberculous mycobacterial (NTM) infections of the breast are rare. These infections present as cellulitis of the breast or breast abscess. Their diagnosis poses a challenge as they manifest signs of acute inflammation, unlike tuberculous mycobacterial infections which present in a chronic pattern. However, on aspiration of pus from the site of infection, primary smear may show acid fast bacilli. This poses a diagnostic dilemma. The present case is that of a 34-year-old woman who presented with recurrent mastitis. She had history of right breast swelling, for which surgical excision had been performed three months prior at another facility. Her histopathology had showed cystic granulomatous neutrophilic mastitis (CNGM). The patient again presented with right breast abscess which was confirmed on ultrasonography. Incision and drainage along with removal of necrotic tissue was done. Primary smear of pus showed acid fast bacilli on Ziehl-Neelson staining. Bacterial culture and line probe speciation revealed non-tuberculous mycobacterium M. abscessus, which responded well to prolonged anti-microbial therapy. These rapidly growing NTM require prolonged treatment and are quite often recurrent. M. abscessus is a rare cause of CNGM, with this being only the third reported case in literature. A brief case report with a review of literature is presented.

Abstract:BACKGROUND Granulomatous mastitis (GM) an inflammatory disease of the breast that usually affects women of childbearing age, occurs very rarely in males. CASE SUMMARY We present a case study of a 50-year-old male patient with GM. The patient developed a breast lump following the cleaning of a previously embedded dirt-filled nipple. While an initial improvement was noted with antibiotic therapy, a recurrence occurred a year later, showing resistance to the previously effective antibiotics. Subsequently, the lesion was excised. The histopathological examination confirmed the diagnosis of GM. CONCLUSION GM should be considered a possible diagnosis of male breast masses.

Background: Granulomatous mastitis (GM) is a rare, benign inflammatory breast disease of unclear etiology with clinical presentation that is similar to inflammatory breast cancer and periductal lobular mastitis. It occurs mainly in younger females and during the postpartum period. Prolactin (PRL) is an essential hormone of pituitary and extrapituitary origin that promotes lactation and the development of mammary glands within breast tissues. In addition to its lactogenic effect, it has over 300 other effects, several of which are immunomodulatory, including the expression and secretion of proinflammatory cytokines from macrophages. Clinical Case: A 37-year-old African American woman presented with right breast pain and swelling for a few weeks. Her medical history includes amenorrhea for five years. She is gravida 1, para 1 with normal spontaneous vaginal delivery at the age of 20. Physical exam showed non-bloody, non-purulent nipple discharge bilaterally, otherwise unremarkable. A breast ultrasound showed a 1 cm complex cystic mass. Biopsy showed benign breast tissue, with noncaseating granulomas formed by macrophages (CD68 positive on immunohistochemistry). Ziehl-Neelsen staining for acid-fast bacilli was negative. Staining for fungi was also negative. Prolactin level was 237 ng/mL. TSH, IGF-1, and ACTH were within normal. Pituitary MRI showed a focus of abnormal signal and diminished enhancement measuring 2.8x3.2x4.2 mm within the left side of the pituitary gland is compatible with a microadenoma. The patient was started on bromocriptine 2.5mg daily. Discussion: Our patient represents a rare case of granulomatous mastitis in the setting of microprolactinoma, likely long-standing, given the five years history of amenorrhea. The immunomodulatory effect of PRL on several immune system cells, including B-cells, T-cells, and macrophages, might be an underlying mechanism. In a recent study of 25 samples of GM, metagenomics studies identified unusual organisms in 68% of the samples. Of these, Corynebacterium kroppenstedtii was the most common (60%), followed by Pseudomonas oleovorans (16%), Epstein-Barr virus (4%), Acinetobacter baumannii (4%), and Tepidiphilus thermophilus (4%). In addition, 50% of the patients had elevated PRL, and elevated immunological markers, such as C3 and IgA [1]. This suggests that GM results from an interaction between the presence of certain pathogens in a host with elevated PRL that mediates proinflammatory response. References: 1. Abstract:Background Needle tract metastasis is a rare complication following percutaneous procedures for malignancy. Case Summary This report describes a 49-year-old female with a lump on her right breast. Mass core needle biopsy showed the specimen was an invasive carcinoma, and mastectomy with sentinel lymph node biopsy was performed. What is special about this case was that the patient reported a history of lung cancer and the position of the breast mass was the puncture site of computed tomography-guided core needle biopsy for lung cancer. Immunostaining of paraffin specimen findings indicated the breast mass as a result of lung carcinoma metastasis. The patient's medical history indicated that the malignant tumor in the breast was a core needle tract pulmonary metastasis. The patient underwent the examination and received therapy based on the lung cancer metastasis principle. At 9 months from breast surgery, the patient is alive, in good condition, and with stability of the disease. Conclusions This patient was misdiagnosed. Careful medical history review and multidisciplinary team discussions are important, especially for patients with a history of cancer or invasive operation.

Abstract:Granulomatous lobular mastitis (GLM) is a rare inflammatory breast disease with unknown etiology, characterized by non-caseous granulomatous inflammation of the lobules, which infiltrate lymphocytes, neutrophils, plasma cells, monocytes, and eosinophils may accompany. GLM is often misdiagnosed as breast cancer due to the lack of specificity in clinical and imaging examinations, and therefore histopathology is the main basis for confirming the diagnosis. This review provides an overview of the pathological features of granulomatous lobular mastitis and cystic neutrophil granulomatous mastitis (CNGM, a pathologic subtype of GLM). As well as pathologic manifestations of other breast diseases that need to be differentiated from granulomatous lobular mastitis such as breast tuberculosis, lymphocytic mastopathy/diabetic mastopathy, IgG4-related sclerosing mastitis (IgG4-RSM), nodular disease, Wegener’s granulomatosis, and plasma cell mastitis. Besides, discusses GLM and CNGM, GLM and breast cancer, emphasizing that their relationship deserves further in-depth exploration. The pathogenesis of GLM has not yet been clearly articulated and needs to be further explored, pathology enables direct observation of the microscopic manifestations of the disease and contributes to further investigation of the pathogenesis.
