Abstract:Background: Complex motor stereotypies (CMS) typically begin before age three years and include rhythmic, repetitive, fixed movements that last for seconds to minutes and can be interrupted with distraction. Objective: We evaluated the effectiveness of a home-based, parent-provided therapy accompanied by scheduled telephone calls with a therapist, in five- to seven-year old children with primary CMS. Methods: Eligible families received an instructional digital versatile disk (DVD) written instructions, and scheduled telephone contacts with a therapist at baseline (DVD receipt), one, three, and eight weeks later. At each call, parents completed outcome measures and received feedback. Outcome scales Stereotypy Severity Scale (SSS) Motor and Impairment scales and a Stereotypy Linear Analogue Scale (SLAS) were also completed via the Iinternet (REDCap)-at screening, one and two months post-baseline call. At study conclusion, participants were divided into an intent-to-treat (ITT; had at least one call) or a lost-to-follow-up (LTF) group. Results: Thirty-eight children (mean = 6 years ± 11 months) were enrolled. The LTF group (n = 14) had significantly higher scores than the ITT (n = 24) group on all attention-deficit/hyperactivity disorder ratings (P < 0.01), but not stereotypy severity. Primary outcome scores, acquired by telephone and REDCap, showed a significant reduction in SSS Motor and Impairment scores between the initial and the last completed evaluation (P ≤ 0.001). Calculated change ratios were SSS Motor -0.23/-0.30 (cal/REDCap); SSS Impairment -0.31/-0.32; and SLAS -0.54 (REDCap). Clinical improvement was further supported by results from a parent improvement scale and end of study questionnaires. Conclusions: Home-based, parent-administered behavioral therapy supplemented by telephone contact with a therapist is effective in reducing complex motor stereotypies in children.

Abstract:Autism spectrum disorder (ASD) is a neurodevelopmental condition characterized by symptoms in two domains: Social communication/social interaction and restricted, repetitive patterns of behavior, interests, or activities. The impairments associated with ASD often become challenging in the clinical setting due to their broader impact across developmental domains. Therefore, recent evidence suggests a combination of behavioral and sensory integration strategies in managing ASD. Several carefully designed interventional studies have also provided information about the effects of caregiver training and direct instruction at the interventional level. The current study presents the case of a four years and six months old boy who was referred with complaints of poor socialization and communication and repetitive behaviors. This study was based on a single case-ABA design. Initial assessment of the client involved several steps including a clinical interview, behavioral observation, administration of Sensory Screening Checklist followed by a diagnostic assessment based on the Childhood Autism Rating Scale (CARS 2) and diagnostic criteria provided by DSM-5 TR. Based on the assessment, the diagnosis of ASD was confirmed and a management plan was formulated to address the presenting issues. A total of 26 therapy sessions were carried out with the client that particularly focused on managing repetitive behaviors using a combination of behavioral and sensory integration strategies. Outcome analysis revealed that the client demonstrated a reduction in behaviors including teeth grinding and echolalia. The therapy sessions proved beneficial in addressing the client's challenges, albeit with further work needed to achieve optimal outcomes.
Treatment of bruxism in individuals with developmental disabilities: a systematic review

Abstract:We reviewed studies involving the treatment of bruxism (i.e., teeth clenching or teeth grinding) in individuals with developmental disabilities. Systematic searches of electronic databases, journals, and reference lists identified 11 studies meeting the inclusion criteria. These studies were evaluated in terms of: (a) participants, (b) procedures used to assess bruxism, (c) intervention procedures, (d) results of the intervention, and (e) certainty of evidence. Across the 11 studies, intervention was provided to a total of 19 participants aged 4-43 years. Assessment procedures included dental screening under sedation and interviews with caregivers. Intervention approaches included prosthodontics, dental surgery, injection of botulinum toxin-a, behavior modification, music therapy, and contingent massage. Positive outcomes were reported in 82% of the reviewed studies. Overall, the evidence base is extremely limited and no definitive statements regarding treatment efficacy can be made. However, behavior modification and dental or medical treatment options (e.g., prosthodontics) seem to be promising treatment approaches. At present, a two-step assessment process, consisting of dental screening followed by behavioral assessment, can be recommended.

Abstract:This case study concerned clinical safety and behavioral-pharmacological intervention with an 18-year-old man who had autism spectrum disorder and high-frequency aggression toward care providers at a specialized school. A multicomponent behavior support plan included antecedent, differential reinforcement, environmental modification, and physical management procedures subsequently combined with psychotropic medication (aripiprazole). Clinical safety components emphasized comprehensive care provider training, continuous supervision, function-based treatment, and prevention-focused strategies. The behavioral-pharmacological intervention eliminated implementation of aggression-contingent physical restraint and was associated with increased transition compliance during the school day. Throughout the 7.5 months duration of the study, there were no injuries to the participant or other students and a single injury was sustained by one care provider. Intervention effects were long-standing, and care providers rated their training, implementation fidelity, and therapeutic outcome favorably.

Abstract:The study aimed at confirming factor structure of the Strength and Difficulties Questionnaire (SDQ) in a general population of Russian adolescents. The SDQ and other psychopathology measures were administered to a representative sample of 13-18-year-old adolescents from Arkhangelsk, Russia (N = 2892). The original factor structure was examined on two randomly selected split-halves of the sample, using structural equation modeling techniques. The fit for all models was good, and when applying increasing constraints across the models, there was not a significant decrease in fit for the random split model. However, the factor loadings and, correspondingly, the scale reliabilities were low. The study concluded that although the Russian SDQ scales do conform reasonably well to a five-factor oblique model, the scales are unsatisfactory in other respects; in its present form, the Russian version of the instrument has inadequate psychometric characteristics. Future research is needed for further scale development.

Abstract:Self‐injurious behavior is thought to be the product of interactions between deficits related to a primary disability (e.g., limited communication repertoires and adaptive skills) and learning experiences that reinforce problem behavior (e.g., consoling the child, terminating non‐preferred activities, etc.). For these cases, functional communication training (FCT) may be prescribed to decrease challenging behavior and increase the client's communicative repertoire. However, FCT procedures for individuals with multiple physical impairments may require considerable individualization and modifications. Unfortunately, the existing body of research related to the assessment and treatment of behavior of individuals with motor, visual, or other impairments is relatively limited. The purpose of this report is to demonstrate how practitioners can modify assessment and treatment procedures to decrease challenging behavior and promote independence when working with individuals with multiple impairments. We provide an example of effective skill acquisition and behavior reduction interventions for a young girl, Ava, presenting with multiple disabilities—including visual, hearing, and motor impairments. Consistent with the habilitative aims of applied behavior analysis, the intervention was effective at decreasing Ava's challenging behavior and increasing her independent communication and adaptive behavior.

Abstract:When mands and problem behavior co-occur within an individual's repertoire, a functional analysis of precurrent contingencies helps to identify any relation between the two responses, as well as the function of problem behavior. Repetitive behaviors may function similarly to mands and also co-occur with problem behavior; particularly when repetitive behavior is blocked, or when caregivers refrain from participating in repetitive behavior episodes (e.g., the repetitive behavior involves a verbal or physical interaction with a caregiver). The current study presents assessment and treatment results for two participants diagnosed with autism, who demonstrated repetitive speech and problem behavior. Informal observations suggested that problem behavior occurred when an adult failed to emit a specific response to the participant's repetitive speech. Functional analysis results confirmed the informal observations and suggested that problem behavior functioned as a precurrent response to increase the probability of reinforcement for repetitive speech. We report treatment results and discuss the application of precurrent contingency analyses for problem behavior and repetitive behavior.

Abstract:This study evaluated the assessment and treatment of problem behaviors related to rituals for children with autism. After functional analyses, we used a multiple-probe design to examine the effects of functional communication training (FCT) plus extinction and schedule thinning as a treatment package for problem behavior and appropriate communication for 3 children. Results of the functional analyses suggested that problem behavior was maintained by reinstatement of the interrupted routine for all participants, and the treatment package reduced problem behavior. Generalization across activities was observed for 1 participant.

Abstract:Background: Gestures or manual signing are valid options for augmentative and alternative communication. However, the data in the literature are limited to a few neurodevelopmental disorders, and less is known about its application in the community setting. Objectives: This case report explores the feasibility and preliminary efficacy of tact training with augmentative gestural support intervention for a child affected by a language disorder with challenging behaviors in a community setting. Methods: Baseline assessments were conducted using the Verbal Behavior Milestone Assessment and Placement Program (VB-MAPP) and Griffiths Mental Developmental Scale-III (GMDS-III). The patient received six months of standard treatment, consisting of neuropsychomotor and speech therapy each twice a week, with improved cooperation in proposed activities, but no improvement in language. Afterward, a total of 24 sessions of tact training with augmentative gestural support interventions were performed. Data were collected by two independent observers and analyzed to measure language and behavioral outcomes. Results: VB-MAPP scores increased form minimal communication and social interaction at T0 (baseline) to improved compliance but unchanged language skills at T1 (after standard therapy). After tack training with augmentative gestural support (T2), VB-MAPP scores showed significant improvements, with notable increases in verbal operants, independence in communication, and intersubjectivity skills. GMDS-III scores at T2 also demonstrated growth in social, communicative, and cognitive skills. Additionally, challenging behaviors were reduced by more than 70% and nearly resolved by the end of the intervention. Conclusions: Personalized approaches appear to be essential for interventions tailored to developmental age. Further research is needed to determine the effectiveness of these approaches for other neurodevelopmental disorders, identify patient characteristics that may be predictors of outcomes to tailor the intervention, and explore the generalization of the results obtained with these strategies.

Abstract:Abstract We explored the effectiveness of a sensory-based, family-centered coaching approach to changing problematic routines for young children with autism. Three mothers of young children with autism, atypical sensory processing, and global developmental delay each participated in a single-case experimental ABA design study. Mothers selected a problematic daily routine linked to sensory challenges as the focus of four intervention sessions provided in the home. Changes in mothers’ perceptions of the children’s behavior were the primary outcome, measured daily on a visual analog scale. Visual and descriptive analyses were undertaken. The sensory-based, family-centered coaching approach showed promise for changing sensory-related problem behaviors of young children with autism, but the degree and maintenance of the intervention effect varied among participants.

Abstract:Purpose: The purpose of this study is to investigate the effectiveness of an augmentative and alternative communication (AAC) intervention package consisting of systematic instruction and aided modeling with speech-output technologies on the acquisition, maintenance, and generalization of socio-communicative behaviors—initiating a request for a turn, answering questions, and commenting—in four, minimally speaking (MS) autistic children between the ages of 6 and 9 years. Method: A multiple–probe design across behaviors replicated across participants was implemented to evaluate the effects of systematic instruction and aided modeling on initiating requests for a turn, answering questions, and commenting behaviors. Additionally, a pre- and posttreatment multiple-generalization-probes design was used to assess generalization across peers. Results: Visual analyses demonstrated experimental control for two participants (i.e., Derek, Ajay) showing a functional relationship between the intervention and outcomes across all social communicative behavior. For one participant (i.e., Matthew), experimental control could not be established because he did not reach the learning criterion for commenting. The fourth participant (i.e., John) transferred to a different school after making some progress on requesting. Effect size indicator analyses corroborated these findings, indicating medium-to-strong effects for initiating requests for a turn strong effects for answering questions, and medium-to-strong effects for commenting. Generalization of socio-communicative behaviors from researcher to a typically developing peer was variable across participants. Participants maintained socio-communicative behaviors 3 weeks after the last intervention session with varying degrees of success. Conclusion: The outcomes of this study suggest that aided modeling and systematic instruction using speech-output technologies may lead to gains in socio-communicative behaviors in some MS autistic children. Abstract:Problem behaviors like self-injury, aggression, or disruption will likely require intervention at some point in the life of a person diagnosed with autism. Behavioral intervention has been proven to be effective for addressing these problems, especially when a functional assessment is conducted. Comprehensive treatment for problem behavior is, however, often fractured across studies, resulting in a dearth of studies that show socially validated improvements in these problem behaviors or illustrate the assessment and treatment process from start to finish. In this article, we describe an effective, comprehensive, and parent-validated functional assessment and treatment process for the severe problem behaviors of 3 children with autism. After an 8- to 14-week outpatient clinic consultation, no problem behavior was observed at the clinic and in the home. Furthermore, behavior that did not occur during baseline (e.g., functional communication, delay and denial tolerance, and compliance with instructions) occurred with regularity.

Abstract:Behavior Modification, Ahead of Print. Competing stimulus assessments (CSA) are effective tools for identifying stimuli that compete with automatically reinforced behavior. However, Jennett et al. suggests there are cases for which non-contingent access to competing stimuli are insufficient at decreasing target responding and additional treatment components may be necessary. The purpose of the current study was to examine procedural variations (i.e., rotating competing items and prompted engagement) when presenting competing stimuli on increasing functional engagement and decreasing stereotypy. Following a functional analysis, a CSA was conducted to identify competing stimuli for four individuals with autism. Items identified were then used with two procedural variations. Levels of stereotypy, functional engagement, and item contact were measured. Results showed that for two participants both treatments were effective, while for the other two participants prompting functional engagement was more effective. Prompting functional engagement is likely a productive strategy for enhancing engagement with competing stimuli for automatically reinforced problem behavior as it may result in functional engagement becoming reinforcing in and of itself.

Periodontal treatment by scaling and root planning resulted in behavior modification of a patient with autistic spectrum disorders: A 14-year follow-up case

Abstract:We report the case of a patient with autistic spectrum disorders for whom it was difficult to establish home care and to have the patient adapt to dental treatment for long periods. We undertook periodontal treatment by scaling and root planning(SRP) for this patient, which resulted in favorable behavior modification. A 25-year-old male patient was referred to our clinic with the chief complaint of bleeding of the gingiva. Oral examination revealed voluminous dental plaques and calculi, and reddening and swelling of the gingiva. Over 10 years since the first visit, we were not able to perform a diagnostic examination of the periodontal condition, because of the maladaptive behavior of our patient and our lack of experience of periodontal treatment in such cases. Therefore, we had been provided oral hygiene instruction, and performed supragingival scaling and professional mechanical tooth cleaning. However, there were some indications, such as moving teeth and acute symptoms, that the periodontal disease was advanced. In accordance with the shift of attending, we performed periodontal examinations by applying behavior modification techniques and rethought the therapeutic principle. With the SRP, his periodontal condition improved and favorable behavior modification was recognized, with the patient showing better adaptation to dental treatment and getting to request voluntarily to subjective symptoms in oral conditions. Furthermore, it eased the maladaptive behavior during assisted tooth brushing by the mother, and she became keen to play an active role in his improvement. Through the treatment of this case, we have demonstrated that improvement of periodontal disease in autistic spectrum disorders may lead to favorable behavior modification of these patients. Furthermore, it is suggested that the patientsʼ behavior modification may also affect the patientsʼ motivation to undergo treatment. Abstract:Background: Autism is a neurological disorder which can be associated with impaired behavioural patterns, expressive and receptive communication difficulties and challenges with social interaction. When combined these challenges can make dental treatment difficult. Therefore, dentists need to be skilled in appropriate dental management strategies for children with autism who attend their clinics for treatment. Rationale: Previous literature reviews have identified the use of traditional behavioural management techniques, such as physical restraints and general anaesthesia. This study aims to update the existing reviews and report effective contemporary techniques. Aim of the review: This literature review aims to document and record current dental strategies and techniques in treating children with autism. Methods: A systematic literature search within PubMed and EBSCO Host using the search terms “Dental strategies” OR dental techniques OR Dental manag* OR dental treat* OR dentist AND autism AND Child* was carried out. The search yielded 10 articles, once exclusion and inclusion criteria were applied and each article was assessed using the qualitative and quantitative CASP tools. Results: Four main themes emerged from a thematic analysis of the 10 articles: 1) Desensitization strategies. 2) Educational strategies. 3) Behavioural approaches. 4) Anaesthesia interventions. Evidence showed that current dental strategies like inter- professional collaboration and some visual strategies using the electronic screen media were particularly effective in treating children with autism within the clinical setting. Conclusion: A range of strategies in providing dental treatment to children with autism were identified. These strategies include desensitization, by various techniques (e.g. visual strategies and social stories, desensitization appointments), educational strategies (e.g. inter-professional collaboration), behavioural strategies and local anaesthesia. However, there is limited evidence regarding the effectiveness of these techniques particularly local anaesthesia and communication supports. Future research is needed to ascertain the effectiveness of these techniques, both in their own right and comparatively with each other.

Relationship between hand stereotypies and purposeful hand use and factors causing skin injuries and joint contractures in individuals with Rett syndrome

Abstract:Background: Skin injuries and joint contractures in the upper limbs are observed in approximately 50 % of individuals with Rett syndrome, respectively. Aims: To investigate the relationship between stereotypic hand movements and purposeful hand skills, items related to these, and factors that cause upper extremity skin injuries and joint contractures in individuals with Rett syndrome. Study design: We conducted a cross-sectional observational study in 2020 with families belonging to either of the two largest Rett syndrome organizations in Japan. Subjects: In 2020, we sent a questionnaire to 194 Japanese families. Outcome measures: We used descriptive statistics to indicate frequency in each question. We analysed the association between hand stereotypies and purposeful hand use, their associations with each questionnaire item, and the relationship between the occurrence of skin injuries and joint contractures. Results: We acquired information from 72 cases. We found correlations between stereotypy frequency with reaching and between purposeful hand use with intellectual development grade and hand function. Hand and finger skin injuries and elbow and finger joint contractures were associated with wringing/washing, grasping, locomotion, reaching, and intellectual development grade. We identified cut-off points for the occurrence of elbow and finger joint contractures of 10 years 6 months, ability to roll over, finger feeds only, and understanding of simple words. Conclusions: Direct interventions can reduce hand stereotypies and increase purposeful hand use, while related items can be addressed with indirect interventions. Evaluations of factors that cause skin injuries and joint contractures can prevent their occurrence.

Main Outcome Measure: Our hypothesis is that sensory-motor-oral stimulation, including sucking during the DOM process, do not negatively affect surgical outcomes.ResultsTwenty-nine children were included. Eight (72.7%) of the 11 patients in the intervention group and 13 (72.2%) of the 18 controls had MDO complications, with no significant difference between the groups (p = 1.000). The most common surgical outcome was antibiotic therapy for surgical site infection (76.2%). Six months after MDO, 22 (75.9%) children attained full oral feeding or associated with alternative feeding methods.ConclusionThe intervention group did not have higher complication rates, from a surgical point of view, than control group. The protocol adopted by some centers that contraindicates sucking during MDO should be revised to consider the benefits of such stimulation.
