Abstract:Mycoplasma pneumoniae(MP)remains to be an important pathogen of childhood community-acquired pneumonia,and also has close relationship to asthma exacerbation. There is a much higher incidence of MP infection in older children than younger children. The latter,although with a lower infection rate,usually show a more severe clinical picture,and is prone to have tachypnea and retractions. High fever and irritative cough are characteristics of MP infections in older children. Serological method is the standard one for diagnosing MP infection,while PCR is more suitable for early diagnosis of MP infection in younger and immunocompromised patients. Macrolide is still the drug of choice for treatment of childhood MP pneumonia,but how to manage the refractory MP pneumonia has become a big challenge to clinical researchers.

Abstract:The conventional wisdom is that the normal human upper respiratory tract has microorganisms. However, under normal conditions, the lower respiratory tract is sterile, especially the distal airway and alveolar tissue. This conclusion is mainly based on the traditional detection method of culturing microorganisms, which has certain limitations. With the rapid development of technologies such as second-generation sequencing, more and more studies have proved the existence of microorganisms in human lungs. By interacting with the organism and the environment, it forms a dynamic and balanced lung microecosystem, which is closely related to human health and respiratory diseases. So far, the concept of pulmonary "sterile" has been gradually replaced. Many studies have also shown that the pulmonary microbes of healthy people and people with respiratory diseases are different, and the microbial composition of people in different disease states is also different. In this paper, the development and research status of pulmonary microbiome, the differences between healthy and diseased pulmonary microbiome, and the relationship between respiratory tract and intestinal microecology were reviewed.

Abstract:Background. Mycoplasma pneumoniae (M. pneumoniae), an atypical pathogen, is increasingly recognized as a common and important pathogen. Previous studies showed that M. pneumoniae infection may play a role in asthmatic mechanisms based on evidence collected from peripheral blood or sputum of patients or animal models. However, evidence reported from the airways of patients has been rare. Objective. To estimate the role of M. pneumoniae infection in asthma by measuring the immunological parameters from peripheral blood and bronchoalveolar lavage fluid (BALF) in pediatric patients with mycoplasma pneumonia. Methods. A total of 30 patients with mycoplasma pneumonia and 37 patients without M. pneumoniae infection undergoing fiberoptic bronchoscopy were reviewed. The peripheral blood cell count, immunoglobulins (Ig), BALF cell count, and other clinical and laboratory data were reviewed and analyzed. Results. There were significantly more patients with raised basophil counts in the M. pneumoniae group than that in the control group (p = 0.033). Serum immunoglobulin (Ig) A, IgM, and IgG levels in the M. pneumoniae group were significantly higher than those in the control group (p = 0.008, p = 0.011, and p = 0.019, respectively). The percentage of eosinophils in BALF cells was in the range 0 to 10% in M. pneumoniae patients, while it ranged between 0 and 4% in the control group with a significant difference (p = 0.043). In the M. pneumoniae group, we found that the percentage of eosinophils in the BALF cells was positively correlated with age, the percentage of peripheral eosinophils, and BALF lymphocytes (r = 0.298, p = 0.030; r = 0.341, p = 0.014; r = 0.387, p = 0.006; respectively) and negatively correlated with total peripheral white blood cell (r = -0.387, p = 0.005). Conclusion. These results suggest that M. pneumonia infection is associated with the asthma mechanism, especially in older children.

Abstract:Mycoplasma pneumoniae , an obligate parasitic pathogen without cell wall, can cause severe upper and lower respiratory tract symptoms. It is the pathogen of human bronchitis and walking pneumonia , and named community-acquired pneumonia. In addition to severe respiratory symptoms, there are clinical extrapulmonary manifestations in the skin, brain, kidney, musculoskeletal, digestive system, and even blood system after M . pneumoniae infection. Hereby, we comprehensively summarized and reviewed the intrapulmonary and extrapulmonary pathogenesis of M. pneumoniae infection. The pathogenesis of related respiratory symptoms caused by M. pneumoniae is mainly adhesion damage, direct damage including nutrient predation, invasion and toxin, cytokine induced inflammation damage and immune evasion effect. The pathogenesis of extrapulmonary manifestations includes direct damage mediated by invasion and inflammatory factors, indirect damage caused by host immune response, and vascular occlusion. The intrapulmonary and extrapulmonary pathogenic mechanisms of M. pneumoniae infection are independent and interrelated, and have certain commonalities. In fact, the pathogenic mechanisms of M. pneumoniae are complicated, and the specific content is still not completely clear, further researches are necessary for determining the detailed pathogenesis of M. pneumoniae . This review can provide certain guidance for the effective prevention and treatment of M. pneumoniae infection.

Abstract:Mycoplasma pneumoniae is an important cause of respiratory tract infections in children as well as adults that can range in severity from mild to life-threatening. Over the past several years there has been much new information published concerning infections caused by this organism. New molecular-based tests for M. pneumoniae detection are now commercially available in the United States, and advances in molecular typing systems have enhanced understanding of the epidemiology of infections. More strains have had their entire genome sequences published, providing additional insights into pathogenic mechanisms. Clinically significant acquired macrolide resistance has emerged worldwide and is now complicating treatment. In vitro susceptibility testing methods have been standardized, and several new drugs that may be effective against this organism are undergoing development. This review focuses on the many new developments that have occurred over the past several years that enhance our understanding of this microbe, which is among the smallest bacterial pathogens but one of great clinical importance.

Abstract:Mycoplasma pneumoniae( MP) is a frequent cause of community-acquired bacterial respiratory infections in children and adult. There are three kinds of diagnosis methods for MP infection: culture,serology and PCR assay. At the present,serology and PCR assay are the two main laboratory tests for MP infection. Tetracyclines,fluoroquinolones and macrolides are three main drugs for MP treatment in clinics. In recent years,it has been reported that some MP strains are acquired resistance to these antimicrobials.The strains with resistance to macrolides and fluoroquinolones have been reported in vitro. Macrolides-resistant isolates are significantly increasing in clinics. However,no clinical isolates resistant to fluoroquinolones have been reported in the literature.

Abstract:Mycoplasma pneumoniae is a major cause of communityacquired respiratory infections in children and adults. A number of severe pulmonary complications have been reported with M. pneumoniae infection. These complications include obliterative bronchitis, bronchiectasis, pleural effusion, interstitial fibrosis, lung abscess, acute respiratory distress syndrome (ARDS) and necrotizing pneumonitis. Necrotizing pneumonitis, which has been described as a suppurative complication of Streptococcus pneumoniae infection, is a rare manifestation of M. pneumoniae infection. Herein we report on five pediatric patients with necrotizing pneumonitis due to M. pneumoniae and review the relevant literature.

Abstract:Mycoplasmas, the smallest self-replicating prokaryotes without a cell wall, are the most prevalent and extensively studied species in humans. They significantly contribute to chronic respiratory tract illnesses and pneumonia, with children and adolescents being particularly vulnerable. Mycoplasma pneumoniae (M. pneumoniae) infections typically tend to be self-limiting and mild but can progress to severe or even life-threatening conditions in certain individuals. Extrapulmonary effects often occur without pneumonia, and both intrapulmonary and extrapulmonary complications operate through separate pathological mechanisms. The indirect immune-mediated damage of the immune system, vascular blockages brought on by vasculitis or thrombosis and direct harm from invasion or locally induced inflammatory cytokines are potential causes of extrapulmonary manifestations due to M. pneumoniae. Proteins associated with adhesion serve as the primary factor crucial for the pathogenicity of M. pneumoniae, relying on a specialized polarized terminal attachment organelle. The type and density of these host receptors significantly impact the adhesion and movement of M. pneumoniae, subsequently influencing the pathogenic mechanism and infection outcomes. Adjacent proteins are crucial for the proper assembly of the attachment organelle, with variations in the genetic domains of P1, P40 and P90 surfaces contributing to the variability of clinical symptoms and offering new avenues for developing vaccines against M. pneumoniae infections. M. pneumoniae causes oxidative stress within respiratory tract epithelial cells by adhering to host cells and releasing hydrogen peroxide and superoxide radicals. This oxidative stress enhances the vulnerability of host cells to harm induced by oxygen molecules. The lack of superoxide dismutase and catalase of bacteria allows it to hinder the catalase activity of the host cell, leading to the reduced breakdown of peroxides. Lung macrophages play a significant role in managing M. pneumoniae infection, identifying it via Toll-like receptor 2 and initiating the myeloid differentiation primary response gene 88-nuclear factor κΒ signaling cascade. However, the precise mechanisms enabling M. pneumoniae to evade intracellular host defenses remain unknown, necessitating further exploration of the pathways involved in intracellular survival. The present comprehensive review delves into the pathogenesis of M. pneumoniae infection within the pulmonary system and into extrapulmonary areas, outlining its impact.

Abstract:Introduction: Mycoplasma pneumoniae (MP) is a major pathogen of community-acquired pneumonia in children. However, the specific pathogenesis of the progression of Mycoplasma pneumoniae pneumonia (MPP) is unclear. We aimed to reveal the landscape of microbiota and the host immune response in MPP. Methods: This self-controlled study analyzed the microbiome and transcriptome of bronchoalveolar lavage fluid (BALF) from the severe side (SD) and opposite side (OD) of 41 children with MPP from January to December 2021 and revealed the differences of the peripheral blood neutrophil function among children with mild MPP, severe MPP, and healthy children through transcriptome sequencing. Results: The MP load or the pulmonary microbiota had no significant difference between the SD group and OD group, and the deterioration of MPP was related to the immune response, especially the intrinsic immune response. Discussion: The immune response plays a role in MPP, which may inform treatment strategies for MPP.

Abstract:Abstract Background This study aimed to investigate the interactions among three core elements of respiratory infection—pathogen, lung microbiome, and host response—and their avocation with the severity and outcomes of Mycoplasma pneumoniae pneumonia (MPP) in children. Methods We prospectively collected bronchoalveolar lavage fluid from a cohort of 41 children with MPP, including general MPP (GMPP) and complicated MPP (CMPP), followed by microbiome and transcriptomic analyses to characterize the association among pathogen, lung microbiome, and host response and correlate it with the clinical features and outcomes. Results The lung microbiome of patients with CMPP had an increased relative abundance of Mycoplasma pneumoniae (MP) and reduced alpha diversity, with 76 differentially expressed species. Host gene analysis revealed a key module associated with neutrophil function and several inflammatory response pathways. Patients with a high relative abundance of MP, manifested by a specific lung microbiome and host response type, were more prone to CMPP and had a long imaging recovery time. Conclusion Patients with CMPP have a more disrupted lung microbiome than those with GMPP. MP, lung microbiome, and host response interacts with each other and are closely related to disease severity and outcomes in children with MPP. Graphical Abstract

Abstract:Mycoplasma pneumoniae is a common pathogen causing respiratory infections in children and adults. In addition to respiratory diseases, Mycoplasma pneumoniae is also involved in numerous extrapulmonary diseases. Thrombosis is an extrapulmonary manifestation associated with Mycoplasma pneumoniae infection. In recent years, an increasing number of case reports have been published identifying thrombosis secondary to Mycoplasma pneumoniae infection. In the present study, the available relevant literature in English available on PubMed, Medline and Web of Science was consulted. The results of the present study demonstrated that in patients with thrombosis caused by Mycoplasma pneumoniae infection, some of the factors causing thrombosis are transient and some are due to hereditary thrombophilia. Following timely treatment, the majority of patients recovered completely but some patients had a poor prognosis. The present review focuses on the pathogenesis, clinical features, treatment and prognosis of this crucial issue, which contributes toward the understanding of the disease.

Abstract:The review discusses the recurrence of Mycoplasma pneumoniae (M. pneumoniae), a bacterium causing atypical pneumonia, primarily affecting Europe and Asia due to climate change, immunity decline, antibiotic resistance, and genetic heterogeneity. The COVID-19 pandemic initially reduced M. pneumoniae cases due to preventative measures, but its reemergence suggests different transmission dynamics and exacerbates clinical severity with co-infections with other viruses. The pathogenicity of M. pneumoniae is attributed to its intracellular changes, toxin release, and adhesion processes, which can result in a variety of symptoms and problems. Antibiotics and immunomodulators are used in treatment, and attempts are being made to create vaccines. Effective management of its reappearance necessitates surveillance and preventative measures, especially in the context of co-infections and potential outbreaks. M. pneumoniae's resurgence highlights its reliance on a polarized cytoskeletal architecture for host cell attachment and pathogenicity through cytoadherence and cytotoxic agent synthesis. M. pneumoniae has returned even though the COVID-19 pandemic originally reduced incidence; this might be because of things like declining immunity and particular pathogenic characteristics. Meteorological factors like temperature and humidity, along with air quality, including pollutants like PM2.5 and NO2, increase susceptibility to environmental hazards. During the pandemic, non-pharmaceutical measures decreased transmission but did not eradicate the infection. Epidemics typically occur three to five years apart, emphasizing the need for ongoing study and observation. Antimicrobial resistance is a serious issue, necessitating caution and alternative therapies, especially in macrolides. COVID-19 pandemic lessons, such as mask use and hand hygiene, may help limit M. pneumoniae transmission.

Abstract:The emergency setting has undergone significant changes in recent years. Notably, the throughput and acuity of patients has increased, with a concomitant improvement in the clinical and technical management of these patients (Dolan, 1998; Coughlan and Corry, 2007). However, there is evidence to suggest that the increase in workload and proliferation of technology, at such a fast pace, has potentially threatened the caring component of nursing, including spiritual care (Wilkin and Selvin, 2004). During hospitalization, the majority of patients tend to become anxious because of the fear of the unknown, an uncertain future, and possible resultant complications of their respective illnesses. In this regard, patients being treated in emergency departments are at vulnerable periods of their lives. Consequently, while the emergency department can be physically demanding, nurses spend considerable time in intense interactions with patients. In spite of this, changes have brought associated pressures on both nurses and patients (Bailie, 2005). Therefore, although advances in technology can enable nurses to objectively measure responses to care; conversely, it can supersede the premise of holistic health care. Nonetheless, it has been empirically shown that caring and the provision of spiritual care is not only possible within the technological world of emergency nursing, but it can be positively enhanced by the mastery of the technological environment (Locin, 1995; Little, 2000).

Abstract:Mycoplasma pneumoniae is a major causative agent of community-acquired pneumonia which can lead to both acute upper and lower respiratory tract inflammation, and extrapulmonary syndromes. Refractory pneumonia caused by M. pneumonia can be life-threatening, especially in infants and the elderly. Here, based on a comprehensive review of the scientific literature related to the respective area, we summarize the virulence factors of M. pneumoniae and the major pathogenic mechanisms mediated by the pathogen: adhesion to host cells, direct cytotoxicity against host cells, inflammatory response-induced immune injury, and immune evasion. The increasing rate of macrolide-resistant strains and the harmful side effects of other sensitive antibiotics (e.g., respiratory quinolones and tetracyclines) in young children make it difficult to treat, and increase the health risk or re-infections. Hence, there is an urgent need for development of an effective vaccine to prevent M. pneumoniae infections in children. Various types of M. pneumoniae vaccines have been reported, including whole-cell vaccines (inactivated and live-attenuated vaccines), subunit vaccines (involving M. pneumoniae protein P1, protein P30, protein P116 and CARDS toxin) and DNA vaccines. This narrative review summarizes the key pathogenic mechanisms underlying M. pneumoniae infection and highlights the relevant vaccines that have been developed and their reported effectiveness.
