Abstract:The concept of teacher professional vision suggests that experienced teachers, compared to novice teachers, might be better at making accurate judgments of students' learning characteristics, which can be explained by their advanced reasoning in diagnostic situations. This study examines experienced and novice teachers' diagnoses of different student characteristic profiles: three inconsistent profiles (overestimating, uninterested, and underestimating) and two consistent profiles (strong and struggling). We examined both experienced ( n = 19 in-service mathematics teachers) and novice teachers ( n = 24 pre-service mathematics teachers) to determine the extent of differences in their judgment accuracy and their diagnostic reasoning about observable cues when diagnosing student profiles while watching a lesson video. ANOVA results indicate that experienced teachers generally achieved a higher judgment accuracy in diagnosing student profiles compared to novice teachers. Moreover, epistemic network analysis of observable cues in experienced and novice teachers' diagnostic reasoning showed that, compared to novice teachers, experienced teachers make more relations between a broader spectrum of both surface cues (e.g., a student's hand-raising behavior) and deep cues (e.g., a student being interested in the subject). Experienced teachers thereby construct more comprehensive and robust reasoning compared to novice teachers. The findings highlight how professional experience shapes teachers' professional skills, such as diagnosing, and suggest strategies for enhancing teacher training.

The Effects of Temporal Distance on Confirmatory Information Processing

Abstract:The present study investigates the influence of temporal distance on confirmatory information processing. Confirmatory information processing principally refers to a tendency of individuals to search and overestimate information that supports with their decision rather than looking for information conflicts (Fischer, Fischer, Englich, Aydin,&Frey, 2011;Fischer, 2011). According to the theoretical viewpoint of Construal Level Theory (CLT, see Trope & Liberman, 2010), temporal distance or perceived temporal proximity exerts an important discount effect on confirmatory information processing under individual and organizational decision making context. It is hypothesized that low temporal distance will increase confirmatory information processing and high temporal distance will decrease it. This discount effect is moderated by construal level mindset and decision’s desirable and feasible representation. We have conducted three studies and our results have found to support our prediction. In Study 1, we have conducted two one-factor between-subjects experiments in which we manipulated temporal distance (near vs. future) and employed a classic selective exposure paradigm to explore the confirmatory information processing. Fifty college students participate in the experiment 1a and sixty college students join in the experiment 1b. The results have shown that participants in a near-temporal decision scenario strengthen their confirmatory information processing, while participants in a future-temporal scenario reduce their confirmatory information processing. Further, we have found that perceived decision certainty plays a partial mediating effect in the process. In study 2, we have conducted a 2×2 between-subjects experiment to investigate the moderating effect of construal level mindset in which we manipulated temporal distance (near vs. future) and construal level mindset (low vs. high). Ninety undergraduate students from a university in Shanghai participate this experiment. The finding have confirmed the temporal discount effect as study 1 did, and also supports a negative impact of construal level mindset on confirmatory information processing. As expected, the moderating effect is testified as well. The negative relationship between temporal distance and confirmatory information processing is stronger when construal level mindset is lower. And the negative relationship between them is weaker when construal level mindset is higher. In study 3, we have explored the moderating effect of decision desirable/feasible representation on the relationship between temporal distance and confirmatory information processing. We conducted a 2×2 between-subjects experiment to verify the hypothesis. Fifty-eight participants take part in this experiment and are randomly assigned into different treatments. Apart from a main effect of decision representation, the results also exhibits its moderating mechanism that the negative relationship between temporal distance and confirmatory information processing is stronger when the decision scenario is represented as high-feasibility/low-desirability, the negative relationship is much weaker when the decision scenario is represented as high-desirability/low-feasibility. The result is shown to support our hypotheses. Our results support the temporal discount effect of confirmatory information processing which shades some light on a new cognitive mechanism for biased information processing. This research brings an insight to the underlying process in mediating role of perceived decision certainty, which responds to the cognitive economy model of confirmatory information processing. Moreover, the abstract and concrete mental representation primed in both mindset and decision context exert a significant boundary mechanism for this temporal discount effect. The negative effects of temporal distance on confirmatory information processing could vary significantly with different level of abstract representation. Finally, our findings also have some practical implications for managing and reducing confirmation biases in the process of information search and evaluation when making a decision.
Clinical reasoning performance assessment: using situated cognition theory as a conceptual framework

Abstract:Abstract Developing valid assessment approaches to clinical reasoning performance has been challenging. Situated cognition theory posits that cognition (e.g. clinical reasoning) emerges from interactions between the clinician and situational (contextual) factors and recognizes an opportunity to gain deeper insights into clinical reasoning performance and its assessment through the study of these interactions. The authors apply situated cognition theory to develop a conceptual model to better understand the assessment of clinical reasoning. The model highlights how the interactions between six contextual factors, including assessee, patient, rater, and environment, assessment method, and task, can impact the outcomes of clinical reasoning performance assessment. Exploring the impact of these interactions can provide insights into the nature of clinical reasoning and its assessment. Three significant implications of this model are: (1) credible clinical reasoning performance assessment requires broad sampling of learners by expert raters in diverse workplace-based contexts; (2) contextual factors should be more explicitly defined and explored; and (3) non-linear statistical models are at times necessary to reveal the complex interactions that can impact clinical reasoning performance assessment.
Misconceptions die hard: prevalence and reduction of wrong beliefs in topics from educational psychology among preservice teachers

Abstract:Abstract Endorsement of educational psychological misconceptions among preservice teachers can be a threat for reaching educational goals. Therefore, it is of societal interest whether preservice teachers hold educational psychological misconceptions and, if they do, whether these misconceptions can be reduced through confrontation with empirical evidence. Prevalence and refutability of misconceptions were analyzed among N = 937 German preservice teachers who participated in an online survey. Results indicated a high prevalence of educational psychological misconceptions but also the possibility of a reduction through refutation-style texts. However, only few preservice teachers shifted their opinions from (rather) endorsing a misconception to (rather) not endorsing it after reading the text. We conclude that educational psychological misconceptions are common among German preservice teachers and that merely presenting empirical evidence is insufficient to effectively counteract misconceptions. Future research should deepen the understanding of why and wherefrom these misconceptions occur and develop efficient interventions to counteract misconceptions among preservice teachers.

Abstract:Teachers routinely observe and interpret student behavior to make judgements about whether and how to support their students' learning. Simulated cases can help pre-service teachers to gain this skill of diagnostic reasoning. With 118 pre-service teachers, we tested whether participants rate simulated cases presented in a serial-cue case format as more authentic and become more involved with the materials compared to cases presented in a whole case format. We further investigated whether participants with varying prior conceptual knowledge (what are symptoms of ADHD and dyslexia) gain more strategic knowledge (how to detect ADHD and dyslexia) with a serial-cue versus whole case format. We found that the case format did not impact authenticity ratings but that learners reported higher involvement in the serial-cue case format condition. Bayes factors provide moderate evidence for the absence of a case format effect on strategic knowledge and strong evidence for the absence of an interaction of case format and prior knowledge. We recommend using serial-cue case formats in simulations as they are a more authentic representation of the diagnostic reasoning process and cognitively involve learners. We call for replications to gather more evidence for the impact of case format on knowledge acquisition. We suggest a further inquiry into the relationship of case format, involvement, and authenticity but think that a productive way forward for designing authentic simulations is attention to aspects that make serial-cue cases effective for diverse learners. For example, adaptive feedback or targeted practice of specific parts of diagnostic reasoning such as weighing evidence.

Abstract:It is important for teachers to be able to accurately assess students' performance. Such judgments can be influenced by characteristics of the student Südkamp et al. (J Educ Psychol 104:743–762, 2012. <a href="https://doi.org/10.1037/a0027627">https://doi.org/10.1037/a0027627</a>). Besides students' actual performance, students' group characteristics (e.g., gender or immigration background) may effect teachers' judgments. In addition, judgment accuracy might be different for various student groups. We conducted an online study of 168 preservice teachers. We presented within a virtual classroom mathematics test results of 12 fictitious second-grade students who differed in their actual performance in a mathematical test, immigration background, and gender. Preservice teachers made a judgment about the students' current performance. Students' actual performance, immigration background, and gender showed statistically significant main effects on the judgment. Students with (vs. without) an immigration background and female (vs. male) students were evaluated less favorably. These effects were qualified by a statistically significant three-way interaction between actual performance, immigration background, and gender. The joint examination of student characteristics in terms of judgment accuracy shows that it is precisely the interaction of student characteristics that makes a difference: female students with and without an immigration background as well as students without an immigration background are assessed more accurately, while male students with an immigration background are assessed significantly more inaccurately. In sum, the judgment made by preservice teachers about students' performance differed in terms of student characteristics that were unrelated to performance such as immigration background and gender in addition to differing on performance-related variables.

Abstract:Many decision-making tasks are characterized by a combination of diagnostic and non-diagnostic information, yet models of responding and confidence almost exclusively focus on the contribution of diagnostic information (e.g., evidence associated with stimulus discriminability), largely ignoring the contribution of non-diagnostic information. An exception is Baranski and Petrusic's Journal of Experimental Psychology: Human Perception and Performance, 24 (3), 929-945, (1998) doubt-scaling model, which predicts a negative relationship between non-diagnostic information and confidence, and between non-diagnostic information and accuracy. In two perceptual-choice tasks, we tested the effects of manipulating non-diagnostic information on confidence, accuracy and response time (RT). In Experiment 1, participants viewed a dynamic grid consisting of flashing blue, orange and white pixels and indicated whether the stimulus was predominantly blue or orange (using a response scale ranging from low-confidence blue to high-confidence orange), with the white pixels constituting non-diagnostic information. Increasing non-diagnostic information reduced both confidence and accuracy, generally slowed RTs, and led to an increase in the speed of errors. Experiment 2 replicated these results for a decision-only task, providing further support for the doubt-scaling model of confidence.

Abstract:In science education, context-based learning is mostly based on problem-oriented tasks [Gilbert, J. K. (2006). On the nature of “context” in chemical education. International Journal of Science Education, 28(9), 957–976]. Therefore, a relevant question is, how do students integrate the information given in the task into their problem-solving process? The basic assumption is that there is a transition from the situation described in the task to a science model needed to solve the problem [Mestre, J. (2002). Probing adults’ conceptual understanding and transfer of learning via problem posing. Journal of Applied Developmental Psychology, 23(1), 9–50]. The transition needs to be described by parameters of the situation, the science model and the transition process itself. This investigation focuses on the influence of these three elements on the problem-solving process to understand variations in performance [Bennett, J., Lubben, F., & Hogarth, S. (2007). Bringing science to life: A synthesis of the research evidence on the effects of context-based and STS approaches to science teaching. Science Education, 91(3), 347–370]) and the interaction with interest. Despite the large body of research on the mutual influence of context-based problem-solving and interest, research attempting to examine their interplay regarding performance is still lacking. We conducted a hierarchical regression analysis with 178 participants from German high-track schools to investigate three parameters with regard to their influence on affective variables and successful problem-solving: contextualisation of the situation described in the task; the complexity of the scientific model underlying the task; and transparency, which assesses whether and how the learner can identify this model [Löffler, P., & Kauertz, A. Our findings suggest that these parameters have different effects on how interest is triggered and maintained. Aspects of transparency exhibit small effects on successful problem-solving processes. The results support the assumption that the transition process is the main aspect of context-based problem-solving and can therefore be operationalised as the use of elements of the scientific model in students’ statements. Surprisingly, the usage of such elements cannot be sufficiently explained by pre-knowledge or cognitive abilities.

The influence of context on diagnostic reasoning: A narrative synthesis of experimental findings

Abstract:Aims and Objectives Contextual information which is implicitly available to physicians during clinical encounters has been shown to influence diagnostic reasoning. To better understand the psychological mechanisms underlying the influence of context on diagnostic accuracy, we conducted a review of experimental research on this topic. Method We searched Web of Science, PubMed, and Scopus for relevant articles and looked for additional records by reading the references and approaching experts. We limited the review to true experiments involving physicians in which the outcome variable was the accuracy of the diagnosis. Results The 43 studies reviewed examined two categories of contextual variables: (a) case‐intrinsic contextual information and (b) case‐extrinsic contextual information. Case‐intrinsic information includes implicit misleading diagnostic suggestions in the disease history of the patient, or emotional volatility of the patient. Case‐extrinsic or situational information includes a similar (but different) case seen previously, perceived case difficulty, or external digital diagnostic support. Time pressure and interruptions are other extrinsic influences that may affect the accuracy of a diagnosis but have produced conflicting findings. Conclusion We propose two tentative hypotheses explaining the role of context in diagnostic accuracy. According to the negative‐affect hypothesis, diagnostic errors emerge when the physician's attention shifts from the relevant clinical findings to the (irrelevant) source of negative affect (for instance patient aggression) raised in a clinical encounter. The early‐diagnosis‐primacy hypothesis attributes errors to the extraordinary influence of the initial hypothesis that comes to the physician's mind on the subsequent collecting and interpretation of case information. Future research should test these mechanisms explicitly. Possible alternative mechanisms such as premature closure or increased production of (irrelevant) rival diagnoses in response to context deserve further scrutiny. Implications for medical education and practice are discussed.

Exploring medical students' metacognitive and regulatory dimensions of diagnostic problem solving

Abstract:Solving clinical problems requires an individual to apply not only domain-specific medical knowledge and cognitive skills for reasoning, but also to be consciously aware of, monitor, and evaluate their thinking processes (i.e., metacognition). The purpose of this study was to map critical metacognitive dimensions of clinical problem solving and to explore the structural relationships among them, which may help frame a conceptual framework and better pedagogy for effective intervention. A context-specific inventory was adapted and modified from a domain-general instrument to capture essential metacognitive skills for learning and solving clinical problems. This inventory was administered to 72 undergraduate medical students to survey their capabilities in five dimensions: knowledge of cognition, objectives, problem representation, monitoring, and evaluation. The interplay among these dimensions was further examined using partial least squares structural equation modeling. Our findings revealed that the medical students fell short of some expert-like, metacognitive, and regulatory competence, even after receiving years of medical education and on-site training. In particular, they did not know when a holistic understanding of a problem had been reached. Many of them often do not have a set of clear diagnostic procedures in mind, nor do they concurrently monitor their thinking during diagnostic reasoning. Moreover, their lack of self-improving approaches seemed to worsen their learning. Finally, the structural equation model indicated that knowledge of cognition and objectives significantly predicted problem representation, suggesting that medical learners' knowledge of and goals for learning are influential in framing the clinical problems at hand. A significant linear prediction path was observed from problem representation, monitoring, to evaluation, signifying a possible sequenced process of clinical problem solving. Metacognitive-based instruction can help improve clinical problem-solving skills and awareness of potential biases or errors.

Confirmatory information seeking is robust in psychologists' diagnostic reasoning

Abstract:Objective: Across two experiments, we examined three cognitive biases (order effects, context effects, confirmatory bias) in licensed psychologists' diagnostic reasoning. Hypotheses: Our main prediction was that psychologist-participants would seek confirming versus disconfirming information after forming an initial diagnostic hypothesis, even given multiple opportunities to seek new information in the same case. We also expected that individual differences would affect diagnostic reasoning, such that psychologists with lower (vs. higher) cognitive reflection tendencies and larger (vs. smaller) bias blind spots would be more likely to demonstrate confirmatory bias. Method: In Study 1, we recruited 149 licensed psychologists (M = 18 years of experience; 44% women; 71% White) and exposed them to one of four randomly assigned vignettes that varied order effects (one set of symptoms in reversed orders) and context effects (court referral vs. employer referral). They rank ordered a list of four possible initial diagnostic hypotheses and received a piped follow-up choice of which of two pieces of information (confirmatory or disconfirmatory) they wanted to test their initial hypothesis. Study 2 (n = 131; M = 21 years of experience; 53% men; 68% White) replicated and extended Study 1, following the same procedure except offering three sequential choice opportunities. Results: Both studies found robust confirmatory information seeking: 92% sought confirmatory information in Study 1, and confirmation persisted across three opportunities in Study 2 (90%, 84%, 77%), although it lowered with each opportunity (generalized logistic mixed regression model), F(2, 378) = 3.85, p = .02, ηp² = .02. Conclusion: These findings expand a growing body of research on bias in expert judgment. Specifically, psychologists may engage in robust confirmation bias in the process of forming diagnoses. Although further research is needed on bias and its impact on accuracy, psychologists may need to take steps to reduce confirmatory reasoning processes, such as documenting evidence for and against each decision element. Exploring Knowledge of Cognitive Disposition to Respond in Clinical Decision-Making among Early Clinical Learners

Abstract:Introduction: Background: Errors in clinical medicine are a constant occurrence leading to litigations. Cognitive disposition to respond (CDR) was considered the underlying cause of errors in the decision-making process. The rapid thinking system used by early clinical learners leads to cognitive errors. The scenario gets compounded by a limited awareness and lack of exposure to cognitive dispositions to respond (CDRs) in the formal medical graduate curriculum. Variation in the frequency of occurrence in a real-life context, contradictory claims over the influence of clinical expertise, specialty, and methods of teaching about cognitive biases led to this exploratory study among early clinical learners. Methods:In the present research, subjects' recruitment was based on voluntary participation of medical graduates, interns, and residents, who were assigned to equal groups of 30. All study groups were first exposed to six commonly occurring CDRs as role-play-based videos, followed by a case scenario-based exposure to 13 uncommon CDRs in the clinical setting. Participants were then asked to reflect on the experience. Data was analyzed by descriptive and thematic analysis. Results:Among the 19 tested biases, 75.6% of participants identified Hindsight bias. The sunk cost was found out by 34.4% of participants only. Information processing bias was identified more frequently than psychological processing bias. Intergroup variability showed that residents were not identifying biases as frequently as medical graduates and interns. The role play video with reflective writing was voiced as the suitable training method for cognitive bias. Conclusion:The study concludes that medical graduates must be sensitized to CDRs using role-play-based reflection methods.
HatA and HatR, implicated in the uptake of inorganic carbon in Synechocystis PCC6803, contain WD40 domains

Abstract:In a recent publication, Bédu et al. (1995, Mol Microbiol 18: 559–568) identified two adjacent genes (designated hatA and hatR) believed to function in the uptake of inorganic carbon (CO2 HCO3 ) in the cyanobacterium Synechocystis PCC6803. Inactivation of hatA yielded a clone that was unable to grow with limiting amounts of inorganic carbon, while inactivation of hatR yielded a strain with nearly normal growth properties but lacking the high-affinity inorganiccarbon-uptake system. The HatA protein was reported to lack homology with all proteins in the protein sequence databases but to possess three hydrophobic domains including a putative N-terminal signal peptide. The deduced HatR protein was reported to exhibit the features characteristic of a response regulator of a bacterial two-component regulatory system (Stock et al., 1990, Nature 344: 395–400). It was postulated that HatR might control the activity of an inorganic-carbon-transport system. Various potential roles for HatA were proposed. We have examined the HatA and HatR amino acid sequences and report here that: (i) HatR lacks significant sequence similarity to response regulators; (ii) HatA possesses only one sequence that appears to be sufficiently long and hydrophobic to span the membrane as an a-helix; and (iii) HatA and HatR both possess at least two recognizable WD40 domains (Neer et al., 1994, Nature 371: 297–300). Janda et al. (1996, J Bacteriol 178: 1487–1489) recently reported the presence of WD40 repeats in a chromosomally encoded protein from the actinomycete, Thermomonospora curvata. This protein also possesses a domain that exhibits sequence similarity to eukaryotic serine/threonine protein kinases, and it was therefore designated PkwA. WD40 domains were also noted in a putative Synechocystis protein with GenBank Accession Number D63999. These were the first WD40 domains to be identified in prokaryotic proteins. BLAST searches (Altschul et al., 1990, J Mol Biol 215: 403–410) of both HatA and HatR revealed that these proteins exhibit domains that are similar in sequence to the bacterial WD40 domains noted above, as well as to many eukaryotic WD40 sequences. No response-regulator protein in the database exhibited significant sequence similarity to HatR. Two putative WD40 domains of HatA and those of HatR are shown multiply aligned in Fig. 1 with two WD40 sequences of PkwA, two WD40 domains of the Synechocystis protein noted by Janda et al. (1996, ibid.) and two recognized eukaryotic WD40 sequences, one of bovine origin and the other of yeast origin. All 10 sequences align without the introduction of a single gap. Abstract:Online information repositories increasingly serve as memory aids in people's lives. Access to such information stores, however, can result in false perceived equivalencies between web-based information and personal knowledge, which can in turn influence judgments of oneself, of information search tasks, and of the Internet itself. Cognitive processing fluency, access to reliable web-based information , and actively searching for information are shown in a series of experiments to be associated with judgments related to metacognition and task performance. In the context of online information repositories accessed via web search activities, people are shown to (a) overemphasize the degree to which they find the web to be a ready source of relevant information, (b) overestimate their future task performance and the ease of tasks, and (c) inflate their own perceived cognitive and memory abilities. Results also show that those who are least competent in task completion overestimate their relative performance, whereas the most competent underestimate theirs, and that the availability of web-based information can inflate people's estimated performance, particularly among the more competent. Collectively, three interrelated studies add considerable new insight regarding the impacts of near-ubiquitous access to contemporary information-saturated environments.

Abstract:Background Diagnostic errors have been attributed to reasoning flaws caused by cognitive biases. While experiments have shown bias to cause errors, physicians of similar expertise differed in susceptibility to bias. Resisting bias is often said to depend on engaging analytical reasoning, disregarding the influence of knowledge. We examined the role of knowledge and reasoning mode, indicated by diagnosis time and confidence, as predictors of susceptibility to anchoring bias. Anchoring bias occurs when physicians stick to an incorrect diagnosis triggered by early salient distracting features (SDF) despite subsequent conflicting information. Methods Sixty-eight internal medicine residents from two Dutch university hospitals participated in a two-phase experiment. Phase 1: assessment of knowledge of discriminating features (ie, clinical findings that discriminate between lookalike diseases) for six diseases. Phase 2 (1 week later): diagnosis of six cases of these diseases. Each case had two versions differing exclusively in the presence/absence of SDF. Each participant diagnosed three cases with SDF (SDF+) and three without (SDF−). Participants were randomly allocated to case versions. Based on phase 1 assessment, participants were split into higher knowledge or lower knowledge groups. Main outcome measurements: frequency of diagnoses associated with SDF; time to diagnose; and confidence in diagnosis. Results While both knowledge groups performed similarly on SDF- cases, higher knowledge physicians succumbed to anchoring bias less frequently than their lower knowledge counterparts on SDF+ cases (p=0.02). Overall, physicians spent more time (p<0.001) and had lower confidence (p=0.02) on SDF+ than SDF− cases (p<0.001). However, when diagnosing SDF+ cases, the groups did not differ in time (p=0.88) nor in confidence (p=0.96). Conclusions Physicians apparently adopted a more analytical reasoning approach when presented with distracting features, indicated by increased time and lower confidence, trying to combat bias. Yet, extended deliberation alone did not explain the observed performance differences between knowledge groups. Success in mitigating anchoring bias was primarily predicted by knowledge of discriminating features of diagnoses.

Abstract:A significant portion of diagnostic errors arises through cognitive errors resulting from inadequate knowledge, faulty data gathering, and/or faulty verification. Experts estimate that 75% of diagnostic failures can be attributed to clinician diagnostic thinking failure. The cognitive processes that underlie diagnostic thinking of clinicians are complex and intriguing, and it is imperative that clinicians acquire explicit appreciation and application of different cognitive approaches to make decisions better. A dual-process model that unifies many theories of decision-making has emerged as a promising template for understanding how clinicians think and judge efficiently in a diagnostic reasoning process. The identification and implementation of strategies for decreasing or preventing such diagnostic errors has become a growing area of interest and research. Suggested strategies to decrease diagnostic error incidence include increasing clinician's clinical expertise and avoiding inherent cognitive errors to make decisions better. Implementing Interventions focused solely on avoiding errors may work effectively for patient safety issues such as medication errors. Addressing cognitive errors, however, requires equal effort on expanding the individual clinician's expertise. Providing cognitive support to clinicians for robust diagnostic decision-making serves as the final strategic target for decreasing diagnostic errors. Clinical guidelines and algorithms offer another method for streamlining decision-making and decreasing likelihood of cognitive diagnostic errors. Addressing cognitive processing errors is undeniably the most challenging task in reducing diagnostic errors. While many suggested approaches exist, they are mostly based on theories and sciences in cognitive psychology, decision-making, and education. The proposed interventions are primarily suggestions and very few of them have been tested in the actual practice settings. Collaborative research effort is required to effectively address cognitive processing errors. Researchers in various areas, including patient safety/quality improvement, decision-making, and problem solving, must work together to make medical diagnosis more reliable.
Can degrading information about patient symptoms in vignettes alter clinical reasoning in paramedics and paramedic students? An experimental application of fuzzy trace theory

Abstract:Background: Research has shown paramedics form rapid intuitive impressions on first, meeting a patient and these impressions subsequently affected their clinical reasoning. We report an experiment where theory-based interventions are developed with the goal of reducing reliance on intuitive reasoning by paramedics and paramedic students in simulated patients. Method: Australian paramedics (n = 213; 49% female) and paramedicine students (n = 83; 55% female) attending paramedic conferences completed a 2 × 2 fully between participants experiment. They saw a written clinical vignette designed to be representative of Acute Coronary Syndrome (ACS) in which key clinical information was precise or degraded (stimulus), they then either chose the single most likely diagnosis from a list, or ranked competing diagnoses (response). Outcome variables were diagnostic rate and response time. Results: There were no differences in the proportion of participants choosing ACS across the four stimulus-response conditions (0.75 [0.65, 0.84] vs 0.79 [0.68, 0.87] vs, 0.78 [0.65, 0.87] vs 0.72 [0.59, 0.82], p = 0.42) CONCLUSION: This is the first study attempting to experimentally examine clinical reasoning in paramedics using a theory-based intervention. Neither of the interventions tested succeeded in altering measures of clinical reasoning. Similar to previous research on physicians, paramedic reasoning appears robust to manipulation.

Abstract:Rationale Research on diagnostic reasoning has been conducted for fifty years or more. There is growing consensus that there are two distinct processes involved in human diagnostic reasoning: System 1, a rapid retrieval of possible diagnostic hypotheses, largely automatic and based to a large part on experiential knowledge, and System 2, a slower, analytical, conscious application of formal knowledge to arrive at a diagnostic conclusion. However, within this broad framework, controversy and disagreement abound. In particular, many authors have suggested that the root cause of diagnostic errors is cognitive biases originating in System 1 and propose that educating learners about the types of cognitive biases and their impact on diagnosis would have a major influence on error reduction. Aims and Objectives In the present paper, we take issue with these claims. Method We reviewed the literature to examine the extent to which this theoretical model is supported by the evidence. Results We show that evidence derived from fundamental research in human cognition and studies in clinical medicine challenges the basic assumptions of this theory—that errors arise in System 1 processing as a consequence of cognitive biases, and are corrected by slow, deliberative analytical processing. We claim that, to the contrary, errors derive from both System 1 and System 2 reasoning, that they arise from lack of access to the appropriate knowledge, not from errors of processing, and that the two processes are not essential to the process of diagnostic reasoning. Conclusions The two processing modes are better understood as a consequence of the nature of the knowledge retrieved, not as independent processes.

Abstract:Teachers’ subjective understandings of their students’ cognitive abilities have important implications for classroom interactions, children’s access to resources and opportunities, and educational equity more broadly. Using nationally representative data and three-level hierarchical linear models, this study explored the links between teacher perceptions and children’s sociodemographic backgrounds. The authors find that teachers perceive substantial racial-ethnic, socioeconomic, and gender differences in children’s literacy skills. Roughly half of these disparities are explained by actual between-group differences. The remaining perceptual inaccuracies flow more from classroom characteristics than from teachers’ professional or personal backgrounds (e.g., their own race or ethnicity). Specifically, holding students’ social and academic backgrounds constant, the authors find that teachers in lower-socioeconomic-status and lower-achieving contexts more often underestimate their students’ abilities. These results highlight the importance of recent policy efforts to avoid isolating traditionally disadvantaged children.
