Abstract:Online adaptive radiotherapy (ART) enables accommodation for variations in patient setup and anatomical changes, allowing for fractional replanning for target coverage, organ at risk (OAR) sparing, and application of CT simulation-free (SF) workflows. SF workflows bypass the conventional simulation CT scan at the potential trade-off in dosimetric uncertainty. ART can alleviate many of these uncertainties, and this work extends previous experience with an Ethos adaptive cone-beam computed tomography (CBCT)-based SF process to treating a unique bony and soft tissue case with stereotactic body radiation therapy (SBRT). The patient is an 83-year-old male with metastatic prostate cancer, presenting with metastases near the right posterior ischium and a right perirectal lymph node. The patient's history includes multiple radiation treatments and androgen deprivation therapy (ADT). Rising prostate-specific antigen(PSA) levels and new metastases identified via positron emission tomography (PET)/CT prostate-specific membrane antigen (PSMA) led to SBRT re-irradiation, considered safe due to the time lapse since previous treatments. Using a HyperSight-equipped Ethos ART system, an SF SBRT workflow utilized the patient's recent PET/CT images for target and OAR delineation. A nine-field adaptive intensity-modulated radiotherapy(IMRT) treatment plan was generated to deliver 3600 Gy in three fractions with a primary focus to limit the dose to proximal OARs and the previously treated region. At the adaptive treatment, the patient is positioned based on anatomical marks, and axial images from HyperSight CBCT are used to contour the OARs and targets. These modified contours accommodate daily variations and are used to recalculate the reference plan and generate a new adapted plan. The adapted plan is selected if coverage improvement and OAR sparing are achieved. For each newly adapted plan, Ethos-generated synthetic CT is reviewed prior to treatment to verify no errors occurred in the deformable propagation between the reference image and the fractional CBCT. For this patient, the adapted plan was selected for all fractions due to improved target coverage, particularly of the soft tissue target, and OAR sparing. The patient tolerated the treatment well and demonstrated a good response on three-month follow-up PSMA PET/CT imaging. This case highlights the efficacy of CBCT-driven SF ART in complex re-irradiation scenario. Future enhancements in the Ethos treatment planning system, including direct dose computation on HyperSight CBCT images, will streamline SF workflows and expand their applicability. Careful consideration of potential on-unit OAR changes and target motion remains crucial for successful SF ART applications.

Abstract:Oropharyngeal candidiasis is associated with defects in cell-mediated immunity, and is commonly seen in immunocompromised patients. We have previously shown that T-cell-deficient BALB/c nude (nu/nu) mice are extremely susceptible to oropharyngeal candidiasis, and that recovery from a chronic infection is dependent on CD4 T lymphocytes. In this study we describe the local tissue cytokine profile in lymphocyte-reconstituted immunodeficient mice and their euthymic counterparts. Mice were infected orally with 108 cells of the yeast Candida albicans, and oral tissues sampled on days 0, 4, 8, and 14. Nude mice were reconstituted with 3 x 107 naïve lymphocytes following oral inoculation. Interleukin (IL)-6, interferon (IFN)-gamma and tumour necrosis factor (TNF)-alpha were identified in the oral tissues of infected euthymic mice recovering from oral infection, as well as naïve controls. TNF-alpha was identified in nude oral tissue on days 4 and 8, but only after lymphocyte reconstitution. No IL-2, IL-4 or IL-10 was detected in either euthymic or athymic mice at any time-point throughout the experiment. This study confirms the functional activity of T lymphocytes in reconstituted nude mice, and suggests that TNF-alpha may be an important mediator in the recovery from oropharyngeal candidiasis.
Clinical Workflow of Cone Beam Computer Tomography-Based Daily Online Adaptive Radiotherapy with Offline Magnetic Resonance Guidance: The Modular Adaptive Radiotherapy System (MARS)

Abstract:Purpose: The Ethos (Varian Medical Systems) radiotherapy device combines semi-automated anatomy detection and plan generation for cone beam computer tomography (CBCT)-based daily online adaptive radiotherapy (oART). However, CBCT offers less soft tissue contrast than magnetic resonance imaging (MRI). This work aims to present the clinical workflow of CBCT-based oART with shuttle-based offline MR guidance. Methods: From February to November 2023, 31 patients underwent radiotherapy on the Ethos (Varian, Palo Alto, CA, USA) system with machine learning (ML)-supported daily oART. Moreover, patients received weekly MRI in treatment position, which was utilized for daily plan adaptation, via a shuttle-based system. Initial and adapted treatment plans were generated using the Ethos treatment planning system. Patient clinical data, fractional session times (MRI + shuttle transport + positioning, adaptation, QA, RT delivery) and plan selection were assessed for all fractions in all patients. Results: In total, 737 oART fractions were applied and 118 MRIs for offline MR guidance were acquired. Primary sites of tumors were prostate (n = 16), lung (n = 7), cervix (n = 5), bladder (n = 1) and endometrium (n = 2). The treatment was completed in all patients. The median MRI acquisition time including shuttle transport and positioning to initiation of the Ethos adaptive session was 53.6 min (IQR 46.5–63.4). The median total treatment time without MRI was 30.7 min (IQR 24.7–39.2). Separately, median adaptation, plan QA and RT times were 24.3 min (IQR 18.6–32.2), 0.4 min (IQR 0.3–1,0) and 5.3 min (IQR 4.5–6.7), respectively. The adapted plan was chosen over the scheduled plan in 97.7% of cases. Conclusion: This study describes the first workflow to date of a CBCT-based oART combined with a shuttle-based offline approach for MR guidance. The oART duration times reported resemble the range shown by previous publications for first clinical experiences with the Ethos system.

Abstract:This article presents a case of a patient with advanced head and neck cancer, characterized by a large and protruding tumor. The patient was treated with an innovative on-demand online adaptive radiotherapy (ART) technology, guided by cone beam computed tomography (CBCT), on the Ethos adaptive radiotherapy platform (version 1.0, Varian Medical Systems, Palo Alto, CA). A solution was provided for this special case to address the issue where part of the target volume could not participate in the optimization due to exceeding the external contour boundary during online adaptive radiotherapy. The treatment outcome was satisfactory in terms of tumor regression, while only grade 1 radiodermatitis and grade 2 oral mucositis at the end of radiotherapy. This article discusses the clinical diagnosis, treatment process, and follow-up of this case, aiming to provide clinical references for a broader application of this technology.

Abstract:Purpose/Objective(s) The advent of commercially-available CBCT-guided online adaptive radiotherapy (oART) has made daily replanning possible in our center. We investigated the dosimetric impact and clinical feasibility of a treatment radiographer (RTT)-led oART with daily plan re-optimization service for muscle invasive bladder cancer (MIBC) at our center. Materials/Methods Between January 2021 and February 2022 ten patients with MIBC were treated with oART. The patients underwent a radiotherapy (RT) planning CT scan with an empty bladder. RT was prescribed to a dose of 55 Gy in 20 daily fractions to the planning target volume (PTV) and delivered using either 7- or 9-field IMRT. CTV to PTV margins were 1.5cm superiorly/anteriorly, 1cm posteriorly and 0.8cm inferiorly/laterally. In the oART workflow, artificial intelligence (AI) was utilized during daily plan generation for: organs at risk (OAR) contour delineation; structure-guided target contour propagation; and automated RT plan generation. RTTs were trained to perform the tasks of the oART workflow and a local competency program for RTTs was applied. For the first five patients a radiation oncologist (RO) was present at each treatment, for the second five patients, the RTTs performed the process independently with a RO present if needed. A RO reviewed the images weekly. A comparison of dose delivered using the daily adaptive plan (ADP) versus the original scheduled plan (SP) was performed using Wilcoxon signed rank test, for both PTV coverage and OAR doses. Timing measurements were recorded for the workflow. Results Seven male and three female patients with a median age of 79 years (range 66-82) were treated with oART for MIBC. Four patients received concomitant chemotherapy. A total of 186 oART treatments were planned. Of these, the SP was delivered on 10 occasions as a result of technical issues with the online ADP quality assurance. Using the ADP, PTV D98% was increased by a mean of 3.3% (p<0.05), and bowel and rectal V52 Gy were reduced by a mean of 9.1cc (p<0.05) and 2.6% (p<0.05), respectively, compared to the SP. For both the RO and RTT-led workflows, the median adaptive process duration from CBCT acquisition to beam-on was 19 minutes (range 9-35 minutes). Conclusion The use of AI has enabled routine clinical implementation of daily CBCT-guided oART which is RTT led. The use of oART increased PTV coverage and reduced OAR doses for MIBC. The development of appropriately trained RTTs to run this service increases the likelihood that this technological advancement can be maintained. Steps to reduce PTV margins are to be explored which should further enhance the dosimetric benefits of oART.

Managing Inconsistent Bladder Volumes in the Prostate Cancer Patient Using Daily Online Adaptive RT: A Case Report

Abstract:Purpose: We report the use of online adaptive radiotherapy (OART) aiming to improve dosimetric parameters in the prostate cancer patient who had lower urinary tract symptoms that caused him not to adhere to the standard bladder filling protocol. Methods and materials: The reference treatment plan for adaptive radiotherapy plan was generated for the pelvis and the solitary bony lesion using the Ethos treatment planning system. For each treatment session, high-quality iterative reconstructed cone beam CT (CBCT) images were acquired, and the system automatically generated an optimal adaptive plan after verification of contours. Image-guided RT (IGRT) plans were also created using the reference plan recalculated on the CBCT scan and were compared with adaptive plans. Results: The reference bladder volume in the planning CT scan was 173 cc, and the mean bladder volume difference over the course was 25.4% ± 16.6%. The ART offered superior target coverage for PTV 70 Gy over online IGRT (V95: 90.5 ± 3.2 % Vs 97.3 ± 0.4%; p=0.000) and the bladder was also better spared from the high dose (V65 Gy: 17.9 ± 9.1% vs 14.8 ± 3.6%; p=0.03). However, the mean rectum V65 doses were very similar in both plans. Conclusion: Managing the inconsistent bladder volume was feasible in the prostate cancer patient using the CBCT-guided OART and our analysis confirmed that adaptive plans offered better target coverage while sparing the bladder from high radiation doses in comparison to online IGRT plans. Abstract:Pancreatic cancer is one of the most challenging tumor sites to treat safely and effectively with radiotherapy due to the anatomical location and aggressiveness of the disease. One modality that has shown promising results, which our institution has been employing, is online adaptive stereotactic radiotherapy using a magnetic resonance-guided linear accelerator (MR-linac). However, due to unforeseen circumstances regarding our MR-linac, it was necessary for our institution to use an alternative treatment technique. In this case report, we describe our experience using our ring-gantry linac equipped with an advanced cone-beam computed tomography (CBCT) system to treat a 61-year-old patient with advanced pancreatic cancer with CBCT-guided online adaptive stereotactic radiotherapy. In a short time period of four weeks, we prepared for this case by training with the surface scanning motion management system and developing procedures for planning and adaptation. The patient was prescribed 24 Gy in three fractions, with a risk-adapted approach using strict organ-at-risk (OAR) constraints. Daily CBCT was used for online adaptation of the plan, and the superior plan (non-adapted or adapted) was selected for treatment. For this patient, the adapted plan was chosen for all three fractions, due to OAR constraints being violated in the non-adapted plan. In summary, we found that for this patient, high-quality CBCT guidance for daily re-contouring, in combination with motion management, enabled the use of daily adaptive radiotherapy to safely deliver stereotactic radiotherapy. The results from this case report are promising, and CBCT-guided online adaptive stereotactic radiotherapy for pancreatic cancer warrants further investigation in more patients.
Daily Online Adaptive Radiation Therapy of Postoperative Endometrial and Cervical Cancer With PTV Margin Reduction to 5 mm: Dosimetric Outcomes, Acute Toxicity, and First Clinical Experience

Abstract:Purpose: This study evaluated the first clinical implementation of daily iterative cone beam computed tomography (iCBCT)-guided online adaptive radiation therapy (oART) in the postoperative treatment of endometrial and cervical cancer. Methods and materials: Seventeen consecutive patients treated with daily iCBCT-guided oART were enrolled in this prospective study, with a reduced uniform 3-dimensional PTV margin of 5 mm. Treatment plans were designed to deliver 45 or 50.4 Gy in 1.8 Gy daily fractions to PTV. Pre- and posttreatment ultrasound and iCBCT scans were performed to record intrafractional bladder and rectal volume changes. The accuracy of contouring, oART procedure time, dosimetric outcomes, and acute toxicity were evaluated. Results: The average time from first iCBCT acquisition to completion of treatment was 22 minutes and 26 seconds. During this period, bladder volume increased by 44 cm3 using iCBCT contouring, whereas rectal volume remained stable (62.9 cm3 pretreatment vs 61.9 cm3 posttreatment). A total of 91.6% of influencers and 88.1% of CTVs required no or minor edits. The adapted plan was selected in all (434) fractions and significantly improved the dosimetry coverage for CTV and PTV, especially the vaginal PTV coverage by nearly 7% (P < .05). The adapted bladder Dmean was 104.61 cGy, and the rectum Dmean was 123.67 cGy, significantly lower than the scheduled plan of 108.24 and 128.19 cGy, respectively. The bone marrow and femur head left and right dosimetry were also improved with adaptation. Grade 2 acute gastrointestinal and genitourinary toxicities were 24% and 0, respectively. There was a grade 3 acute toxicity of decreased white blood cell count in 1 patient. Conclusions: Daily oART was associated with favorable dosimetry improvement and low acute toxicity, supporting its safety and efficacy for postoperative treatment of endometrial and cervical cancer. These results need to be validated in a larger prospective randomized controlled cohort.

Abstract:Background: Online adaptive stereotactic radiotherapy allows for improved target and organ at risk (OAR) delineation and inter-fraction motion management via daily adaptive planning. The use of adaptive SBRT for the treatment of pancreatic cancer (performed until now using only MRI or CT on rails-guided adaptive radiotherapy), has yielded promising outcomes. Herein we describe the first reported case of cone beam CT-guided stereotactic adaptive radiotherapy (CT-STAR) for the treatment of pancreatic cancer. Case presentation: A 61-year-old female with metastatic pancreatic cancer presented for durable palliation of a symptomatic primary pancreatic mass. She was prescribed 35 Gy/5 fractions utilizing CT-STAR. The patient was simulated utilizing an end-exhale CT with intravenous and oral bowel contrast. Both initial as well as daily adapted plans were created adhering to a strict isotoxicity approach in which coverage was sacrificed to meet critical luminal gastrointestinal OAR hard constraints. Kilovoltage cone beam CTs were acquired on each day of treatment and the radiation oncologist edited OAR contours to reflect the patient's anatomy-of-the-day. The initial and adapted plan were compared using dose volume histogram objectives, and the superior plan was delivered. Use of the initial treatment plan would have resulted in nine critical OAR hard constraint violations. The adapted plans achieved hard constraints in all five fractions for all four critical luminal gastrointestinal structures. Conclusions: We report the successful treatment of a patient with pancreatic cancer treated with CT-STAR. Prior to this treatment, the delivery of ablative adaptive radiotherapy for pancreatic cancer was limited to clinics with MR-guided and CT-on-rails adaptive SBRT technology and workflows. CT-STAR is a promising modality with which to deliver stereotactic adaptive radiotherapy for pancreatic cancer.

Abstract:Purpose Conventional image-guided radiotherapy (IGRT) of the prostate bed is challenged by the varying anatomy due to dynamic changes of surrounding organs such as the bladder and rectum. This leads to changed dose coverage of target and surrounding tissue. The novel online adaptive radiotherapy (oART) aims to improve target coverage as well as reduce dose exposure to surrounding healthy tissues by daily reoptimization of treatment plans. Here we set out to quantify the resulting changes of this adaptation for patients and treatment team. Methods A total of 198 fractions of radiotherapy of the prostate bed (6 patients) were treated using oART with the Ethos accelerator (Varian Medical Systems, Palo Alto, CA, USA). For each fraction, volumes and several dose–volume parameters of target volumes and organs at risk were recorded for the scheduled plan (initial plan, recalculated based on daily cone beam computed tomography [CBCT]), the adapted plan, and the verification plan, which is the dose distribution of the applied plan recalculated on the closing CBCT after the adaptation process. Clinical acceptability for all plans was determined using given dose–volume parameters of target volumes. Additionally, the time needed for the adaptation process was registered and compared to the time required for the daily treatment of five conventional IGRT patients. Results Volumes of target and organs at risk (OAR) exhibited broad variation from day to day. The differences in dose coverage D 98% of the clinical target volume (CTV) were significant through adaptation ( p < 0.0001; median D 98% 97.1–98.0%) and further after verification CBCT ( p < 0.001; median D 98% 98.1%). Similarly, differences in D 98% of the planning target volume (PTV) were significant with adaptation ( p < 0.0001; median D 98% 91.8–96.5%) and after verification CBCT ( p < 0.001; median D 98% 96.4%) with decreasing interquartile ranges (IQR). Dose to OAR varied extensively and did not show a consistent benefit from oART but decreased in IQR. Clinical acceptability increased significantly from 19.2% for scheduled plans to 76.8% for adapted plans and decreased to 70.7% for verification plans. The scheduled plan was never chosen for treatment. The median time needed for oART was 25 min compared to 8 min for IGRT. Conclusion Target dose coverage was significantly improved using oART. IQR decreased for target coverage as well as OAR doses indicating higher repeatability of dose delivery using oART. Differences in doses after verification CBCT for targets as well as OAR were significant compared to adapted plans but did not offset the overall dosimetric gain of oART. The median time required is three times higher for oART compared to IGRT.

Abstract:CT-guided online adaptive radiotherapy (OART) is a novel and robust treatment technique in radiotherapy. Thanks to its excellent accommodation of inter-fraction variations, OART is characterized by superior accuracy compared to other contemporary treatment techniques in radiotherapy such as image-guided radiotherapy (IGRT). Planning target volume (PTV), which takes into account interfraction movements, could therefore be reduced while utilizing OART with a consequent dose reduction for adjacent healthy tissue. Herein we report our successful experience in treating a patient with retroperitoneal sarcoma after previous radiotherapy and surgery. The tumor was in close proximity to the spinal canal and abutted a large bowel segment and the last portions of the duodenum. Radiotherapy regimen consisted of 30 Gy in five fractions. Treatment implementation and delivery were feasible and the treatment was given without any interruptions. After a follow-up period of nine months so far, the patient reported no radiation-related adverse effects. Furthermore, her post-treatment magnetic resonance imaging (MRI) scans demonstrate good radiographic response. Our case highlights the use of OART to treat a challenging case in radiotherapy as it was given in the setting of re-irradiation to a mesenchymal tumor, which is considered exquisitely radio-resistant.

Abstract:The prostate and post-prostatectomy surgical bed can shift in anatomical position due to changes in the bladder and rectum size. This mobility of the prostate and prostatic bed, along with that of the bladder and rectum, poses a challenge in devising a single radiation therapy plan capable of delivering the desired dose to each organ across all treatment fractions. Adaptive radiation therapy (ART) represents a significant advancement in cancer treatment. The EthosTM ART system (Varian Medical Systems, Inc., Palo Alto, CA) streamlines the adaptive therapy workflow, enabling the efficient creation of superior radiation treatment plans based on anatomical orientation at the time of treatment. This case report aims to discuss how the online ART workflow was utilized in a 72-year-old male with recurrent prostate cancer post-prostatectomy. Our results demonstrated the advantage of having the flexibility to choose between scheduled and adapted plans based on daily images, providing improved radiotherapy plan quality for prostate cancer treatment post-prostatectomy.

Abstract:Stereotactic body radiotherapy (SBRT) is increasingly being prescribed for treating patients with multiple metastases, especially in the setting of oligometastatic disease. Treating multiple targets presents unique challenges in radiotherapy planning and delivery, including practical considerations relating to treatment time, resource allocation, and treatment planning complexity. Treating targets in a common isocenter reduces the time required for treatment and simplifies planning, but historically, it has often not been feasible due to inter- and intra-fractional variation in relative target positions. With online adaptation, individual targets can be re-contoured on each treatment fraction to obviate inter-fractional variation, and with appropriate margin selection intra-fractional motion can be managed. In this case report, we describe single-isocenter, multiple-target treatment via online adaptation of a 93-year-old man with a history of metastatic hepatocellular carcinoma. He initially presented with a 9.1 cm liver mass, suspicious lung lesions, and an enlarged porta hepatis lymph node, which were biopsy proven to be hepatocellular carcinoma. Following 18 months of systemic immunotherapy, he demonstrated a favorable response, including a reduction in primary liver mass to 5.1 cm and resolution of pulmonary lesions; however, recent serial imaging demonstrated oligoprogression of two peripancreatic lymph node conglomerates that were biopsy proven to be poorly differentiated carcinoma. The patient was offered adaptive SBRT to a dose of 35-40 Gy in five fractions as a consolidative approach for treating both the primary liver mass and oligoprogressive lymph nodes. He tolerated treatment without any grade 2 or higher acute toxicity and had stable disease on three-month post-treatment imaging. By leveraging online adaptation, especially for the daily re-definition of target volumes, we were able to treat three targets in the abdomen accurately in a common isocenter. Treating in this manner vastly shortened and simplified the patient's radiation course. Quantitative evaluation of re-contoured targets and post-treatment imaging highlighted the value of online adaption with careful margin specification and alignment instructions.

Abstract:Purpose/objectives: Retrospective analysis suggests that dose escalation to a biologically effective dose of more than 70 Gy may improve overall survival in patients with pancreatic ductal adenocarcinoma (PDAC), but such treatments in practice are limited by proximity of organs at risk (OARs). We hypothesized that CT-guided online adaptive radiotherapy (OART) can account for interfraction movement of OARs and allow for safe delivery of ablative doses. Materials/methods: This is a single institution retrospective analysis of patients with PDAC treated with OART on the Ethos platform (Varian Medical Systems, a Siemens Healthineers Company, Palo Alto). All patients were treated to 40 Gy in 5 fractions. PTV overlapping with a 5 mm planning risk volume expansion on the stomach, duodenum and bowel received 25 Gy. Initial treatment plans were created conventionally. For each fraction, PTV and OAR volumes were recontoured with AI assistance after initial cone beam CT (CBCT). The adapted plan was calculated, underwent QA, and then compared to the scheduled plan. A second CBCT was obtained prior to delivery of the selected plan. Total treatment time (first CBCT to end of radiation delivery) and active physician time (first to second CBCT) were recorded. PTV_4000 V95 %, PTV_2500 V9 5%, and D0.03 cc to stomach, duodenum and bowel were reported for scheduled (S) and adapted (A) plans. CTCAEv5.0 toxicities were recorded. Statistical analysis was performed using a two-sided T test and α of 0.05. Results: 21 patients with unresectable or locally-recurrent PDAC were analyzed, with a total of 105 fractions. Average total time was 29 min and 16 s (16:36-49:40) and average active physician time was 19:41 min (9:25-39:34). All fractions were treated with adapted plans. 97 % of adapted plans met PTV_4000 V95.0 % >95.0 % coverage goal and 100 % of adapted plans met OAR dose constraints. Median follow up was 6.6 months. Only 1 patient experienced acute grade 3+ toxicity directly attributable to radiation. Only 1 patient experienced late grade 3+ toxicity directly attributable to radiation. Conclusions: Daily CT-based OART was associated with significantly reduced dose OARs while achieving superior PTV coverage. Given the relatively quick total treatment time, radiation delivery was generally well tolerated and easily incorporated into the clinic workflow. Our initial clinical experience demonstrates OART allows for safe dose escalation in the treatment of PDAC.

The period of assessment varies from one day to three weeks; the advantages of prolonged assessment are discussed. Abstract:Purpose A workflow/planning strategy delivering low‐dose radiation therapy (LDRT) (1 Gy) to all polymetastatic diseases using conventional planning/delivery (Raystation/Halcyon = "conventional") and the AI‐based Ethos online adaptive RT (oART) platform is developed/evaluated. Methods Using retrospective data for ten polymetastatic non‐small cell lung cancer patients (5‐52 lesions each) with PET/CTs, gross tumor volumes (GTVs) were delineated using PET standardized‐uptake‐value (SUV) thresholding. A 1 cm uniform expansion of GTVs to account for setup/contour uncertainty and organ motion‐generated planning target volumes (PTVs). Dose optimization/calculation used the diagnostic CT from PET/CT. Dosimetric objectives were: Dmin,0.03cc ≥ 95% (acceptable variation (Δ) ≥ 90%), V100% ≥ 95% (Δ ≥ 90%), and D0.03cc ≤ 120% (Δ ≤ 125%). Additionally, online adaptation was simulated. When available, subsequent diagnostic CT was used to represent on‐treatment CBCT. Otherwise, the CT from PET/CT used for initial planning was deformed to simulate clinically representative changes. Results All initial plans generated, both for Raystation and Ethos, achieved clinical goals within acceptable variation. For all patients, Dmin,0.03cc ≥ 95%, V100% ≥ 95%, and D0.03cc ≤ 120% goals were achieved for 84.8%/99.5%, 97.7%/98.7%, 97.4%/92.3%, in conventional/Ethos plans, respectively. The ratio of 50% isodose volume to PTV volume (R50%), maximum dose at 2 cm from PTV (D2cm), and the ratio of the 100% isodose volume to PTV volume (conformity index) in Raystation/Ethos plans were 7.9/5.9; 102.3%/88.44%; and 0.99/1.01, respectively. In Ethos, online adapted plans maintained PTV coverage whereas scheduled plans often resulted in geographic misses due to changes in tumor size, patient position, and body habitus. The average total duration of the oART workflow was 26:15 (min:sec) ranging from 6:43 to 57:30. The duration of each oART workflow step as a function of a number of targets showed a low correlation coefficient for influencer generation and editing (R2 = 0.04 and 0.02, respectively) and high correlation coefficient for target generation, target editing and plan generation (R2 = 0.68, 0.63 and 0.69, respectively). Conclusions This study demonstrates feasibility of conventional planning/treatment with Raystation/Halcyon and highlights efficiency gains when utilizing semi‐automated planning/online‐adaptive treatment with Ethos for immunostimulatory LDRT conformally delivered to all sites of polymetastatic disease.

Abstract:A 74-year-old woman with pathologic T4a N1 M0 adenocarcinoma of the cecum, initially treated with right hemicolectomy, developed rising serum carcinoembryonic antigen levels while receiving adjuvant chemotherapy. Re-staging investigations demonstrated two soft tissue metastases in the right abdomen comprised of a retrocolic lesion immediately posterior to the colon and a retroperitoneal lesion with no other sites of metastases. The patient was treated with stereotactic ablative radiotherapy (SABR) to a dose of 40 Gy in five daily fractions to both pericolonic soft tissue metastases simultaneously. A standard volumetric modulated arc therapy (VMAT) plan had suboptimal dose coverage of the retrocolic metastasis adjacent to the colon, so cone-beam computed tomography (CBCT)-guided online adaptive radiotherapy (ART) was employed to maximize radiation dose to the tumors due to the radioresistant histology. An intensity-modulated radiotherapy (IMRT) plan was created using artificial intelligence tools integrated with the treatment unit. Median contouring and plan creation for each fraction was 21.5 minutes (range 14.9-28.1). For the retrocolic metastasis, compared to the standard VMAT plan, the CBCT-guided online ART plan improved coverage of the gross target volume by the prescription dose from 80.0% to 99.7%. SABR to pericolonic soft tissue metastases was feasible using CBCT-guided online ART and can significantly improve target volume coverage when targets are adjacent to mobile normal organs, which may be particularly important for radioresistant histologies for local control.

Abstract:In the context of oligometastatic renal cell carcinoma (RCC), local treatment with stereotactic body radiotherapy (SBRT) may improve oncologic outcomes. However, the location and size can often pose a technical challenge in standard SBRT delivery, and the dose is potentially limited by nearby organs at risk (OARs). Online adaptive radiotherapy (oART) improves radiation delivery by personalizing high-dose fractions to account for daily stochastic variations in patient anatomy or setup. The oART process aims to maximize tumor control and enhances precision by tailoring to a more accurate representation of a patient in near-real time. The proceeding re-optimization can mitigate the uncertainty inherent in the traditional radiation delivery workflow and precludes the need for larger margins that account for anatomical variations and setup errors. Here, we describe a case of oligometastatic RCC with a bulky (>300 cm3) pleural-based left lower lobe mass extending into the upper abdomen treated via personalized ultrafractionated stereotactic adaptive radiotherapy (PULSAR). Three fractions were delivered four weeks apart allowing for tumor shrinkage of these bulky lesions, and oART permitted on-table adaptation of the plan without traditional re-simulation and re-planning required during off-line adaptive radiotherapy. The plan was designed for the Ethos linear accelerator (Varian Medical Systems, Inc., Palo Alto, CA, USA). The prescription dose was 36 Gray (Gy) in three fractions, and the adapted plan was selected in each treatment over the scheduled plan due to better target coverage and reversal of OAR dose violations. The adapted plan met all OAR dose constraints, and it achieved higher target coverage in the first two PULSAR fractions compared to the scheduled plan. In the third fraction, the cumulative point dose was approaching the maximum heart tolerance, and target coverage was accordingly compromised based on clinical judgment. There was evidence of tumor regression throughout the course of treatment, and the patient did not develop any significant radiation-related toxicities. Follow-up imaging has demonstrated the overall stable size of her lesion without any evidence of disease progression. Our case reflects the benefit of adaptive SBRT delivery to a bulky mass near multiple OARs in the setting of oligometastatic RCC. The adapted plan allowed for prioritization of critical structures on a fraction-by-fraction basis while preserving the therapeutic intent of SBRT. Further integration of advanced imaging techniques, optimal disease-specific systemic immunotherapies or targeted therapies, and refinement of patient selection will be crucial in identifying which patients would most benefit from an adaptive approach.
