Abstract:Objective To investigate the clinical application of prenatal screening and diagnosis for trisomy 18.Methods The concentrations of alpha-fetoprotein(AFP) and free beta-human chorionic gonadotropin(hCGβ) in serum of pregnant women were detected by time-resolved fluoroimmunoassay.The risk rate of fetal trisomy 18 with maternal age,weight and biochemical indicators was calculated.The prenatal diagnosis was carried out by traditional amniotic fluid cell culture karyotype analysis.Results 145 658 pregnant women were enrolled in prenatal screening,and 588 women were detected and showed at high risk for trisomy 18.The positive rate was 0.40%.From 318 pregnant women with high risk for trisomy 18,15 trisomy 18 fetuses were identified,and the positive rate was 4.72%.Screening risk values(n) of trisomy 18 were classified to 7 groups: n≥1/50,1/50n≥1/100,1/100n≥1/150,1/150n≥1/200,1/200n≥1/250,1/250n≥1/300 and 1/300n≥1/350 groups.After grading,the positive rates of 7 groups were 17.10%,4.44%,0.00%,0.00%,0.00%,0.00% and 0.00%,respectively.Trisomy 18 in pregnant women with high risk value and the identified pregnant women were significantly concentrated at n≥1/100.The distribution of pregnant women with high risk value showed average in other n value groups,and the positive rates were all 0.Furthermore,there were 477 low risk and 1 220 without screening advanced age pregnant women accepting prenatal diagnosis voluntarily.Every group found 2 trisomy 18 fetuses.Conclusions Trisomy 18 calculating with age and biochemical indicators can be applied into prenatal risk screening.Trisomy 18 in pregnant women with high risk value and the identified fetuses are significantly concentrated at n≥1/100.These data are helpful for genetic counseling.

Abstract:ObjectiveThe aim of this study is to report the clinical application of noninvasive prenatal testing (NIPT) to detect chromosomal aneuploidies, especially trisomies 21, 18, and 13 in Chinese singleton pregnancies. MethodsPilot validation between NIPT with full karyotyping was conducted blindly on 306 cases. Subsequently, 7705 pregnancies were offered with NIPT. Follow-up data was obtained in all cases. ResultsIn the validation stage, a total of five NIPT positive cases were observed for trisomies 21 and 18, and results were confirmed by karyotyping; there were no cases of trisomy 13. Thus, the overall sensitivity and specificity in the validation stage was 100%. In 7705 cases, NIPT results were obtained in 7701 cases; 66 cases were classified as positive, including 48 cases of trisomy 21, 14 cases of trisomy 18, and 4 cases of trisomy 13. Subsequent karyotyping documented two false positive diagnoses for trisomies 21, 18, and 13, respectively. Sensitivity and specificity for detection of trisomies 21 and 18 and 13 were 100% and 99.9%, respectively. Additionally, prenatal chromosomal detection for pregnancies with NIPT has shown a gradual increase since its implementation. ConclusionNoninvasive prenatal testing allows a more suitable and efficient workflow for our patients' needs, together with invasive procedures allows a higher prenatal detection of chromosomal aneuploidies. Abstract:ABSTRACTObjectiveThe objective of this study is to assess the performance of noninvasive prenatal testing for trisomies 21 and 18 on the basis of massively parallel sequencing of cell‐free DNA from maternal plasma in twin pregnancies.MethodA double‐blind study was performed over 12 months. A total of 189 pregnant women carrying twins were recruited from seven hospitals. Maternal plasma DNA sequencing was performed to detect trisomies 21 and 18. The fetal karyotype was used as gold standard to estimate the sensitivity and specificity of sequencing‐based noninvasive prenatal test.ResultsThere were nine cases of trisomy 21 and two cases of trisomy 18 confirmed by karyotyping. Plasma DNA sequencing correctly identified nine cases of trisomy 21 and one case of trisomy 18. The discordant case of trisomy 18 was an unusual case of monozygotic twin with discordant fetal karyotype (one normal and the other trisomy 18). The sensitivity and specificity of maternal plasma DNA sequencing for fetal trisomy 21 were both 100% and for fetal trisomy 18 were 50% and 100%, respectively.ConclusionOur study further supported that sequencing‐based noninvasive prenatal testing of trisomy 21 in twin pregnancies could be achieved with a high accuracy, which could effectively avoid almost 95% of invasive prenatal diagnosis procedures. © 2013 John Wiley & Sons, Ltd.
Meta-analysis of Prenatal Screening for Trisomy 21 Using Nuchal Translucency Measurement with Serum Markers

Abstract:AIM:To evaluate the value of screening trisomy 21 by nuchal translucency(NT) measurement together with maternal serum biochemistry markers in first trimester. METHODS: A search was performed in Cochrane Library, Pub Med, OVID, Springer, China National Knowledge Infrastructure(CNKI) and Chinese Bio-medicine Database(CBM)to obtain relevant English and Chinese literature between January 1990 and August 2008. Criteria for inclusion were established based on the validity criteria for diagnostic research. The criteria-meeting data were collected and analyzed using Meta-DiSc 1.4. RESULTS: Fourteen research articles were included, with a total of 111 535 fetuses. These articles were analyzed for the value of screening of trisomy 21 using nuchal translucency(NT) measurement together with maternal serum biochemistry markers in first trimester. The pooled sensitivity and specificity were 0.89(95%CI:0.87-0.91) and 0.96(95%CI:0.96-0.96) respectively. The SROC curve was drawn and the area under the curve (AUC) was calculated(AUC=0.971). CONCLUSION: The results suggest that the screening of trisomy 21 by elevated NT together with maternal serum biochemistry markers has a high sensitivity, specificity and accuracy and is suitable for prenatal screen.
Detection of trisomies 13, 18 and 21 using non-invasive prenatal testing

Abstract:The clinical performance of non-invasive prenatal testing (NIPT) in the Down's syndrome screening based on 1,901 pregnant women in a Chinese hospital was investigated. This was a retrospective analysis of NIPT study in singleton pregnancy (n=1,901). The NIPT test is offered routinely as a prenatal screening test for common fetal aneuploidies, including trisomy 13 (T13), T18 and T21 to pregnant women with risk factors of one or more anomalies. Maternal peripheral blood (5 ml) was collected in an ethylenediaminetetraacetic acid (EDTA) tube at a gestational age of 12+0 to 32+6 weeks. The samples were delivered at -80°C to the certified Shenzhen BGI Clinical Laboratory Center. Sequencing data were analyzed using a proprietary algorithm. Women with positive NIPT results were recommended to receive karyotype analysis and amniotic fluid puncture for further validation. The cases were followed up for 56 days after delivery. All the patients underwent ultrasound examination, and the majority of patients (91.16%) showed normal findings. In contrast, 136 (7.15%) showed ultrasound anomalies. The most common anomaly was echogenic heart focus (n=80), accounting for 4.21% of the patients. Twenty-two cases were classified by the NIPT to be positive for the T21 (n=15), T18 (n=5) and T13 (n=2), respectively, while the others (n=1,879) were classified to be NIPT negative cases. Among these cases, the fetal outcome data were obtained in 1,483 cases, while 396 were lost to follow-up. The majority of cases (75.47%) were normal at birth. Neonatal death was observed in 1 case. Five pregnant women decided termination of pregnancy despite the presence of NIPT negativity. In conclusion, NIPT technique is feasible for the prenatal screening of T18 and T21 with higher sensitivity and specificity compared with conventional methods.

Abstract:Rats experienced inescapable, escapable, or no electric shock 1 day after being implanted with a Walker 256 tumor preparation. Only 27 percent of the rats receiving inescapable shock rejected the tumor, whereas 63 percent of the rats receiving escapable shock and 54 percent of the rats receiving no shock rejected the tumor. These results imply that lack of control over stressors reduces tumor rejection and decreases survival.

Abstract:Objectives: Non-invasive prenatal testing (NIPT) has been widely used in recent years. According to clinical experience from all hospitals providing prenatal screening services in Beijing, we explored the feasibility of using NIPT for the analysis of common foetal aneuploidies among pregnancies. Methods: In total, 68,763 maternal blood samples were collected from January 2020 to December 2020 at the Beijing prenatal diagnosis agency. Cases with positive screening results by NIPT detection were validated using prenatal diagnosis. Results: In total, 920 cases had a high-risk NIPT result, and 755 cases were shown to be truly positive by a chromosome karyotyping analysis; the prenatal diagnosis rate was 82.07% (755/920). Of the920 cases, there were 164 cases of T21, 70 cases of T18, 38 cases of T13, 360 cases of SCAs and 288 cases of other chromosomal abnormalities. The positive rates of T21, T18, T13, and SCAs were 0.24% (164/68,763), 0.10% (70/68,763), 0.06% (38/68,763) and 0.52% (360/68,763), respectively. The sensitivity and specificity were 98.17% and 99.92% for T21, 96.15% and 99.93% for T18, and 100% and 99.95% for T13, respectively. The PPVs of T21,T18,T13 and SCAs were65.24% (107/164), 35.71% (25/70), 18.42% (7/38) and 31.39% (113/360), respectively. For all indications, there were more higher T21/18/13 in the high-risk group than in the low-risk group (comprising only cases of voluntary request), with a positive rate of 0.46% vs. 0.27% ( p < 0.001), sensitivity of 99.16% vs. 91.30% ( p = 0.02) and PPV of 56.73%vs.32.81% ( p = 0.001), but there was no significant difference in specificity between the groups ( p = 0.71). The detection indication with the highest PPV (100%) by NIPT was ultrasound structural abnormalities and ultrasound soft marker abnormalities for T21 and ultrasound structural abnormalities and NT thickening for T18 and T13. The PPVs of different clinical indications of T21 ( p = 0.002), T13 ( p = 0.04) and SACs ( p = 0.02) were statistically significant. Conclusion: The high specificity, efficiency and safety (non-invasiveness) of NIPT can effectively improve the detection rate of common chromosomal aneuploidy, thereby reducing the occurrence of birth defects. We should encourage pregnant women with NIPT-high-risk results to undergo a prenatal diagnosis to determine whether the foetus has chromosomal abnormalities. More importantly, the screening efficiency of NIPT in the low-risk group was significantly lower than that in the high-risk group. Therefore, the use of NIPT in low-risk groups should be fully promoted, and socioeconomic benefits should be considered.

Maternal Prenatal Screening Programs That Predict Trisomy 21, Trisomy 18, and Neural Tube Defects in Offspring

Abstract:OBJECTIVE:To determine the efficacy of three different maternal screening programs (first-trimester screening [FTS], individual second-trimester screening [ISTS], and first- and second-trimester combined screening [FSTCS]) in predicting offspring with trisomy 21, trisomy 18, and neural tube defects (NTDs). METHODS:A retrospective cohort involving 108,118 pregnant women who received prenatal screening tests during the first (9-13+6 weeks) and second trimester (15-20+6 weeks) in Hangzhou, China from January-December 2019, as follows: FTS, 72,096; ISTS, 36,022; and FSTCS, 67,631 gravidas. RESULT:The high and intermediate risk positivity rates for trisomy 21 screening with FSTCS (2.40% and 5.57%) were lower than ISTS (9.02% and 16.14%) and FTS (2.71% and 7.19%); there were statistically significant differences in the positivity rates among the screening programs (all P < 0.05). Detection of trisomy 21 was as follows: ISTS, 68.75%; FSTCS, 63.64%; and FTS, 48.57%. Detection of trisomy 18 was as follows; FTS and FSTCS, 66.67%; and ISTS, 60.00%. There were no statistical differences in the detection rates for trisomy 21 and 18 among the 3 screening programs (all P > 0.05). The positive predictive values (PPVs) for trisomy 21 and 18 were highest with FTS, while the false positive rate (FPR) was lowest with FSTCS. CONCLUSION:FSTCS was superior to FTS and ISTS screening and substantially reduced the number of high risk pregnancies for trisomy 21 and 18; however, FSTCS was not significantly different in detecting fetal trisomy 21 and 18 and other confirmed cases with chromosomal abnormalities.

Abstract:Background: Non-invasive prenatal screening (NIPS) is widely used as the alternative choice for pregnant women at high-risk of fetal aneuploidy. However, whether NIPS has a good detective efficiency for pregnant women at advanced maternal age (AMA) has not been fully studied especially in Chinese women. Methods: 29,343 pregnant women at AMA with singleton pregnancy who received NIPS and followed-up were recruited. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV), receiver operating characteristic (ROC) curves and the Youden Index for detecting fetal chromosomal aneuploidies were analyzed. The relationship between maternal age and common fetal chromosomal aneuploidy was observed. Results : The sensitivity, specificity, PPV, NPV of NIPS for detecting fetal trisomy 21 were 99.11%, 99.96%, 90.98%, and 100%, respectively. These same parameters for detecting fetal trisomy 18 were 100%, 99.94%, 67.92%, and 100%, respectively. Finally, these parameters for detecting trisomy 13 were 100%, 99.96%, 27.78%, and 100%, respectively. The prevalence of fetal trisomy 21 increased exponentially with maternal age. The high-risk percentage incidence rate of fetal trisomy 21 was significantly higher in the pregnant women at 37 years old or above than that in pregnant women at 35 to 37 years old. ( Youden index =37). Conclusion: It is indicated that NIPS is an effective prenatal screening method for pregnant women at AMA.
Clinical Efficiency of Non-invasive Prenatal Screening for Common Trisomies in Low-Risk and Twin Pregnancies

Abstract:To evaluate the clinical efficiency of non-invasive prenatal screening (NIPS) for fetal aneuploidies in low-risk and twin pregnancies, patients who received NIPS in a tertiary university hospital were enrolled, and their clinical data, NIPS results and pregnancy outcomes were collected. Patients were divided into singleton and twin pregnancies, and then those with singleton pregnancies were divided into low- and high-risk pregnancies. Sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) were estimated. Comparisons were made on the clinical efficiency of NIPS between singleton and twin pregnancies, as well as between low- and high-risk pregnancies. Of 66,172 patients enrolled, 59,962 were eligible for analysis. The sensitivity, specificity and NPV were >= 99% in singleton and twin pregnancies. The PPVs were 90.4, 56.6, and 13.0% in singleton pregnancies, while 100, 33.3, and 0% in twin pregnancies for trisomy 21 (T21), trisomy 18 (T18) and trisomy 13 (T13), respectively (P > 0.05 for all). The PPVs were 97.4 and 90.0% in high-risk pregnancies, while 78.6 and 16.7% in low-risk pregnancies for T21 and T18, respectively (P < 0.05 for all). In summary, the performance of NIPS in singleton pregnancies was similar to that in twin pregnancies. NIPS can be recommended for all pregnancies regardless of the risks.

Abstract:BACKGROUND:Non-invasive prenatal testing (NIPT) as alternative screening method had been proven to have very high sensitivity and specificity for detecting common aneuploidies such as T21, T18, and T13, with low false positive and false negative rates. Unfortunately, recent studies suggested that the NIPT achieved lower accuracy in sex chromosomal aneuploidies (SCAs) detection than autosomal aneuploidies detection. BGISEQ-500 powered by Combinatorial Probe-Anchor Synthesis (CPAS) and DNA Nanoballs (DNBs) technology that combined linear amplification and rolling circle replication to reduce the error rate while enhancing the signal. Therefore, NIPT based on CPAS might be a good method for SCAs screening in routine clinical practice. In the study, we intended to evaluate the clinical utility of NIPT based on CPAS on screening for fetal SCAs.RESULTS:A total of 570 pregnant women were included in the retrospective study. Maternal blood samples were collected for NIPT; amniocentesis was performed on all pregnant women. NIPT was carried out by BGISEQ-500 sequencing platform based on CPAS. Karyotype analysis of amniotic cells was performed by standard G-banding techniques. 43 out of the total 570 pregnant women tested by NIPT showed fetal SCAs (19 of 45,X, 12 of 47,XXY, 10 of 47,XXX, and 2 of 47,XYY). The following amniocentesis confirmed that 26 cases were true positive (7 of true positive 45,X, 9 of true positive 47,XXY, 9 of true positive 47,XXX as well as 1 of 47,XYY) and the positive predictive value (PPV) for fetal SCAs was 60.47%. In addition, the PPV of advanced maternal age group (67.74%) was higher than the other indications group (45.45%) or serological screening high-risk /critical-risk group (0%).CONCLUSIONS:NIPT based on CPAS could be a potential method for SCAs screening. However, it still had high false positive rates, especially for 45,X. The pregnant women with fetal SCAs detected by NIPT, especially those with non-age-related prenatal diagnostic indications, should be advised to accept invasive prenatal karyotype analysis.

Abstract:Abstract Background Cell-free fetal DNA (cffDNA) has opened up new approaches for non-invasive prenatal testing (NIPT), and it is often used as the second-tier test for high-risk pregnant women in detecting trisomy (T) 21, T18, and T13 after serum biochemistry screening. This study aims to discuss the clinical performance of NIPT as an alternative first-tier screening test for pregnant women in detecting T21, T18, T13, and sex chromosome aneuploidies (SCAs) in China. Methods A total of 42,924 samples were recruited. The cell-free plasma DNA was directly sequenced. Each of the chromosome aneuploidies of PPV was analyzed. A total of 22 placental samples were acquired, including 14 FP and 8 TP samples. The placental verification of FP NIPT results was performed. Results Among 42,924 samples, 281 (0.65%) positive cases, including 87 of T21, 31 of T18, 22 of T13, and 141 of SCAs were detected. For the detection of T21, the positive predictive value (PPV) was 78.46%, for trisomy 18, 62.96%, for trisomy 13, 10.00%, for SCAs, 47.22% in the total samples. For trisomy 21, the PPV was 86.67%, for trisomy 18, 80.00%, for trisomy 13, 20.00%, for SCAs, 56.52% in advanced maternal age (AMA) women. The PPV of T21 increased with age. For T18, the PPV showed an overall upward trend. For T13 and SCAs, PPV was raised first and then lowered. Placental verification of false positive (FP) NIPT results confirmed confined placental mosaicism(CPM) was the reason for false positives. Conclusions This study represents the first time that NIPT has been used as a first-tier screening test for fetal aneuploidies in a pilot city with large clinical samples in China. We propose that NIPT could replace serum biochemistry screening as a first-tier test.

Abstract:OBJECTIVES:To report the clinical performance of massively parallel sequencing-based non-invasive prenatal testing (NIPT) in detecting trisomies 21, 18 and 13 in over 140,000 clinical samples and to compare its performance in low-risk and high-risk pregnancies.METHODS:Between 1 January 2012 and 31 August 2013, 147,314 NIPT requests to screen for fetal trisomies 21, 18 and 13 using low-coverage whole-genome sequencing of plasma cell-free DNA were received. The results were validated by karyotyping or follow-up of clinical outcomes.RESULTS:NIPT was performed and results obtained in 146,958 samples, for which outcome data were available in 112,669 (76.7%). Repeat blood sampling was required in 3213 cases and 145 had test failure. Aneuploidy was confirmed in 720/781 cases positive for trisomy 21, 167/218 cases positive for trisomy 18 and 22/67 cases positive for trisomy 13 on NIPT. Nine false negatives were identified, including six cases of trisomy 21 and three of trisomy 18. The overall sensitivity of NIPT was 99.17%, 98.24% and 100% for trisomies 21, 18 and 13, respectively, and specificity was 99.95%, 99.95% and 99.96% for trisomies 21, 18 and 13, respectively. There was no significant difference in test performance between the 72,382 high-risk and 40,287 low-risk subjects (sensitivity, 99.21% vs. 98.97% (P = 0.82); specificity, 99.95% vs. 99.95% (P = 0.98)). The major factors contributing to false-positive and false-negative NIPT results were maternal copy number variant and fetal/placental mosaicism, but fetal fraction had no effect.CONCLUSIONS:Using a stringent protocol, the good performance of NIPT shown by early validation studies can be maintained in large clinical samples. This technique can provide equally high sensitivity and specificity in screening for trisomy 21 in a low-risk, as compared to high-risk, population.
A Risk Model for Predicting Fetuses with Trisomy 21 Using Alpha-Fetoprotein Variants L2 Combined with Maternal Serum Biomarkers in Early Pregnancy

Abstract:To establish a risk prediction model and the clinical value of trisomy 21 using alpha-fetoprotein variants L2 (AFP-L2) combined with maternal serum biomarkers and nuchal translucency (NT) thickness in early pregnancy. A retrospective case-control study was conducted. The subjects were divided into the case group (n = 40) or the control group (n = 40). An enzyme-linked immunosorbent assay was used to measure the maternal serum AFP-L2 level in both groups. The AFP-L2 single-index or multi-index combined risk model was used to predict the efficiency of trisomy 21. The best cut-off value and area under the curve (AUC) were determined to evaluate the predictive efficacy of different risk models constructed by AFP-L2. The maternal serum AFP-L2 level in the case group was 1.59 (0.61-3.61) Multiple of medium (MoM), which was higher than 1.00 (0.39-2.12) MoM in the control group (P < 0.001). The free beta-human chorionic gonadotropin (free β-hCG) level and NT in the case group were significantly higher than those in the control group (P < 0.001). The pregnancy-associated plasma protein A (PAPP-A) level in the case group was lower than that in the control group (P < 0.001). The AUC of AFP-L2 in predicting trisomy 21 was 0.797. After considering the maternal serum AFP-L2 level, the AUC, detection rate (DR), positive predictive value (PPV), negative predictive value (NPV), falsepositive rate (FPR), false negative rate (FNR), positive likelihood ratio (+LR), and negative likelihood ratio (-LR) were significantly improved. In this study, PAPP-A + free β-hCG + NT + AFP-L2 and PAPP-A + free β-hCG + AFP-L2 increased the integrated discrimination improvement (IDI) and net classification improvement (NRI) of predicting fetuses with trisomy 21 (1.10% and 5.27%; 11.07% and 2.78%) (1.10% and 5.27%; 11.07% and 2.78%), respectively, after considering the maternal serum AFP-L2 level. The maternal serum AFP-L2 level in early pregnancy had high sensitivity and specificity, and it was a good biomarker to predict fetuses with trisomy 21.
Association of maternal risk factors with fetal aneuploidy and the accuracy of prenatal aneuploidy screening: a correlation analysis based on 12,186 karyotype reports

Abstract:Background: NIPT is becoming increasingly important as its use becomes more widespread in China. More details are urgently needed on the correlation between maternal risk factors and fetal aneuploidy, and how these factors affect the accuracy of prenatal aneuploidy screening. Methods: Information on the pregnant women was collected, including maternal age, gestational age, specific medical history and results of prenatal aneuploidy screening. Additionally, the OR, validity and predictive value were also calculated. Results: A total of 12,186 analysable karyotype reports were collected with 372 (3.05%) fetal aneuploidies, including 161 (1.32%) T21, 81 (0.66%) T18, 41 (0.34%) T13 and 89 (0.73%) SCAs. The OR was highest for maternal age less than 20 years (6.65), followed by over 40 years (3.59) and 35-39 years (2.48). T13 (16.95) and T18 (9.40) were more frequent in the over-40 group (P < 0.01); T13 (3.62/5.76) and SCAs (2.49/3.95) in the 35-39 group (P < 0.01). Cases with a history of fetal malformation had the highest OR (35.94), followed by RSA (13.08): the former was more likely to have T13 (50.65) (P < 0.01) and the latter more likely to have T18 (20.50) (P < 0.01). The sensitivity of primary screening was 73.24% and the NPV was 98.23%. The TPR for NIPT was 100.00% and the respective PPVs for T21, T18, T13 and SCAs were 89.92, 69.77, 53.49 and 43.24%, respectively. The accuracy of NIPT increased with increasing gestational age (0.81). In contrast, the accuracy of NIPT decreased with maternal age (1.12) and IVF-ET history (4.15). Conclusions: ①Pregnant patients with maternal age below 20 years had higher risk of aneuploidy, especially in T13; ②A history of fetal malformations is more risky than RSA, with the former more likely to have T13 and the latter more likely to have T18; ③Primary screening essentially achieves the goal of identifying a normal karyotype, and NIPT can accurately screen for fetal aneuploidy; ④A number of maternal risk factors may influence the accuracy of NIPT diagnosis, including older age, premature testing, or a history of IVF-ET. In conclusion, this study provides a reliable theoretical basis for optimizing prenatal aneuploidy screening strategies and improving population quality.
A Cost-Effectiveness Analysis of Different Screening Strategies Involving Noninvasive Prenatal Testing for Trisomy 21

Abstract:Background: To investigate the benefits and cost effectiveness of NIPT as primary or contingent strategies within high risk population, in accordance with increasing advanced maternal age (AMA) and NIPT rapid application.Methods: A theoretical model involving 1,000,000 single pregnancies was established. NIPT together with or without traditional triple serum screening constitutes five screening scenarios. All related parameters were referred to publication or on-site verification. Strategy 1, NIPT is primarily offered to all pregnant women. Strategy 2, NIPT is offered to all pregnancies with risks higher than 1:300 (Strategy 2-1) or 1:1000 (Strategy 2-2) after triple serum screening. Strategy 3 is stratified based on maternal age in which 1) for age 40 and more, prenatal diagnosis is directly offered, 2) for population of maternal age for 35~39, primary NIPT is introduced, 3) if younger than 35y, contingent strategy with risk higher than 1:300 (Strategy 3-1) or 1:1000 (Strategy 3-2) as the indication for NIPT. The primary outcome is incremental cost analysis on baseline and alterative assumptions. The second outcomes are total cost, cost-effect and cost-benefit analysis. The strategy is defined as "cost-effective" if the incremental cost is less than 0.16million US$, needed for raising one T21 livebirth or the benefit-to-cost ratio is above 1.Findings: With the advantage of lower false positive rate of NIPT, anticipated prenatal diagnosis are greatly reduced. In cost-effectiveness analysis, strategy 2-1 costs least with 112.9million US$ in total, 33.4 thousand US$ in cost-effect analysis and ratioed as 3.53 in cost-benefit analysis. Setting 2-1 as baseline, incremental costs of strategy 1, 3-2, 2-2 and 3-1 are 0.67million US$, 0.25million US$, 0.22million US$ and 0.17million US$, which are all more than costs for raising one visible T21. In sensitivity analysis of age proportion, when AMA account for more than 15.5% in population, incremental costs of strategy 3-1 are lower than 0.16million US$. All strategies are considered as "appropriate" if NIPT costs less than 106 US$. When it's less than 309 US$, the incremental cost of strategy 3-1 is least and lower than 0.16million US$. Strategy 1 needs least money when it's lower than 47 US$.Interpretation: Strategy 2-1 is optimal in cost-effectiveness analysis. AMA proportion and NIPT costs are two important factors. Strategy 3-1 is more prominent as AMA increases. Abstract:OBJECTIVETo analyze the results of noninvasive prenatal screening (NIPS) for fetal chromosome aneuploidy in twin pregnancy.METHODSA total of 2057 women with twin-pregnancy between 12-26+6 weeks were recruited from Women's Hospital, Zhejiang University School of Medicine, Hangzhou Municipal Women's Hospital and Jiaxing Maternal and Child Health Hospital during February 2015 to August 2018. The cell-free DNA was extracted from the peripheral blood sample for DNA library, and non-invasive prenatal testing (NIPT) was performed by high-throughput sequencing technique. The fetal karyotype analysis or neonatal karyotype analysis was performed in pregnant women with fetal chromosome aneuploidy, and all subjects were followed up. The efficiency of NIPS testing for twin aneuploidy was calculated.RESULTSNIPS revealed chromosome abnormalities in 11 out of 2057 twin pregnant women, 9 cases were confirmed chromosome abnormalities, 2 cases were normal and no false negative cases. In this screening, the detection rate, sensitivity, specificity, positive predictive value, false positive rate of NIPS were 100.00%, 100.00%, 99.90%, 81.82%, 0.10%. Those were 100.00%, 100.00%, 99.95%, 87.50% and 0.05% for trisomy 21, 100.00%, 100.00%, 100.00%, 100.00%, 0.00% for trisomy18, and the specificity and false positive rate for trisomy13 were 99.95% and 0.05%, respectively.CONCLUSIONSNIPS can detect fetal chromosomal aneuploidy rapidly and accurately in twin pregnancies,and it is of value in clinical application.

Abstract:Objective To assess the effectiveness of prenatal screening with maternal serum markers and the sonographic characteristics of the foetus for detecting trisomy 18. Methods Amniotic fluid or the fetal blood samples were obtained and cultured in women with positive results in serum screening (n=36) and with abnormal findings in sonography (n=92) at 18~32 weeks of gestation, and then fetal karyotyping was performed. Results Nine cases of trisomy 18 and 2 cases of other abnormal karyotype were found among the 128 pregnant women (8.59%, 11/128). Four cases were diagnosed as trisomy 18 among the 36 women with positive screening results. The rate of abnormal karyotype among those women with abnormal sonographic findings was 7.61%(7/92,5 cases of trisomy 18 and 2 cases of other abnormal karyotype). Two cases of trisomy 18 had both positive result in screening and abnormality in sonography. Conclusions Prenatal screening for maternal serum markers combined with sonographic examinations for the fetus is effective in detecting foetus with trisomy 18.
