Abstract:Globally, in 2016, 1 out of 11 adults suffered from diabetes mellitus. Diabetic foot ulcers (DFU) are a major complication of this disease, which if not managed properly can lead to amputation. Current clinical approaches to DFU treatment rely on patient and clinician vigilance, which has significant limitations, such as the high cost involved in the diagnosis, treatment, and lengthy care of the DFU. We collected an extensive dataset of foot images, which contain DFU from different patients. In this DFU classification problem, we assessed the two classes as normal skin (healthy skin) and abnormal skin (DFU). In this paper, we have proposed the use of machine learning algorithms to extract the features for DFU and healthy skin patches to understand the differences in the computer vision perspective. This experiment is performed to evaluate the skin conditions of both classes that are at high risk of misclassification by computer vision algorithms. Furthermore, we used convolutional neural networks for the first time in this binary classification. We have proposed a novel convolutional neural network architecture, DFUNet, with better feature extraction to identify the feature differences between healthy skin and the DFU. Using 10-fold cross validation, DFUNet achieved an AUC score of 0.961. This outperformed both the traditional machine learning and deep learning classifiers we have tested. Here, we present the development of a novel and highly sensitive DFUNet for objectively detecting the presence of DFUs. This novel approach has the potential to deliver a paradigm shift in diabetic foot care among diabetic patients, which represent a cost-effective, remote, and convenient healthcare solution.

Abstract:Diabetes mellitus (DM) can cause chronic foot issues and severe infections, including Diabetic Foot Ulcers (DFUs) that heal slowly due to insufficient blood flow. A recurrence of these ulcers can lead to 84% of lower limb amputations and even cause death. High-risk diabetes patients require expensive medications, regular check-ups, and proper personal hygiene to prevent DFUs, which affect 15–25% of diabetics. Accurate diagnosis, appropriate care, and prompt response can prevent amputations and fatalities through early and reliable DFU detection from image analysis. We propose a comprehensive deep learning-based system for detecting DFUs from patients' feet images by reliably localizing ulcer points. Our method utilizes innovative model ensemble techniques—non-maximum suppression (NMS), Soft-NMS, and weighted bounding box fusion (WBF)—to combine predictions from state-of-the-art object detection models. The performances of diverse cutting-edge model architectures used in this study complement each other, leading to more generalized and improved results when combined in an ensemble. Our WBF-based approach combining YOLOv8m and FRCNN-ResNet101 achieves a mean average precision (mAP) score of 86.4% at the IoU threshold of 0.5 on the DFUC2020 dataset, significantly outperforming the former benchmark by 12.4%. We also perform external validation on the IEEE DataPort Diabetic Foot dataset which has demonstrated robust and reliable model performance on the qualitative analysis. In conclusion, our study effectively developed an innovative diabetic foot ulcer (DFU) detection system using an ensemble model of deep neural networks (DNNs). This AI-driven tool serves as an initial screening aid for medical professionals, augmenting the diagnostic process by enhancing sensitivity to potential DFU cases. While recognizing the presence of false positives, our research contributes to improving patient care through the integration of human medical expertise with AI-based solutions in DFU management.

Abstract:Every 20 seconds, a limb is amputated somewhere in the world due to diabetes. This is a global health problem that requires a global solution. The MICCAI challenge discussed in this paper, which concerns the automated detection of diabetic foot ulcers using machine learning techniques, will accelerate the development of innovative healthcare technology to address this unmet medical need. In an effort to improve patient care and reduce the strain on healthcare systems, recent research has focused on the creation of cloud-based detection algorithms. These can be consumed as a service by a mobile app that patients (or a carer, partner or family member) could use themselves at home to monitor their condition and to detect the appearance of a diabetic foot ulcer (DFU). Collaborative work between Manchester Metropolitan University, Lancashire Teaching Hospital and the Manchester University NHS Foundation Trust has created a repository of 4,000 DFU images for the purpose of supporting research toward more advanced methods of DFU detection. Based on a joint effort involving the lead scientists of the UK, US, India and New Zealand, this challenge will solicit original work, and promote interactions between researchers and interdisciplinary collaborations. This paper presents a dataset description and analysis, assessment methods, benchmark algorithms and initial evaluation results. It facilitates the challenge by providing useful insights into state-of-the-art and ongoing research. This grand challenge takes on even greater urgency in a peri and post-pandemic period, where stresses on resource utilization will increase the need for technology that allows people to remain active, healthy and intact in their home.

Abstract:Diabetes is a chronic condition caused by an uncontrolled blood sugar levels in the human body. Its early diagnosis may prevent severe complications such as diabetic foot ulcers (DFUs). A DFU is a critical condition that can lead to the amputation of a diabetic patient's lower limb. The diagnosis of DFU is very complicated for the medical professional as it often goes through several costly and time-consuming clinical procedures. In the age of data deluge, the application of deep learning, machine learning, and computer vision techniques have provided various solutions for assisting clinicians in making more reliable and faster diagnostic decisions. Therefore, the automatic identification of DFU has recently received more attention from the research community. The wound characteristics and visual perceptions with respect to computer vision and deep learning, especially convolutional neural network (CNN) approaches, have provided potential solutions for DFU diagnosis. These approaches have the potential to be quite helpful in current medical practices. Therefore, a detailed comprehensive study of such existing approaches was required. The article aimed to provide researchers with a detailed current status of automatic DFU identification tasks. Multiple observations have been made from existing works, such as the use of traditional ML and advanced DL techniques being necessary to help clinicians make faster and more reliable diagnostic decisions. In traditional ML approaches, image features provide signification information about DFU wounds and help with accurate identification. However, advanced DL approaches have proven to be more promising than ML approaches. The CNN-based solutions proposed by various authors have dominated the problem domain. An interested researcher will successfully be able identify the overall idea in the DFU identification task, and this article will help them finalize the future research goal.

Abstract:At present, the ubiquity method to diagnose the severity of diabetic feet (DF) depends on professional podiatrists. However, in most cases, professional podiatrists have a heavy workload, especially in underdeveloped and developing countries and regions, and there are often insufficient podiatrists to meet the rapidly growing treatment needs of DF patients. It is necessary to develop a medical system that assists in diagnosing DF in order to reduce part of the workload for podiatrists and to provide timely relevant information to patients with DF. In this paper, we have developed a system that can classify and locate Wagner ulcers of diabetic foot in real-time. First, we proposed a dataset of 2688 diabetic feet with annotations. Then, in order to enable the system to detect diabetic foot ulcers in real time and accurately, this paper is based on the YOLOv3 algorithm coupled with image fusion, label smoothing, and variant learning rate mode technologies to improve the robustness and predictive accuracy of the original algorithm. Finally, the refinements on YOLOv3 was used as the optimal algorithm in this paper to deploy into Android smartphone to predict the classes and localization of the diabetic foot with real-time. The experimental results validate that the improved YOLOv3 algorithm achieves a mAP of 91.95%, and meets the needs of real-time detection and analysis of diabetic foot Wagner Ulcer on mobile devices, such as smart phones. This work has the potential to lead to a paradigm shift for clinical treatment of the DF in the future, to provide an effective healthcare solution for DF tissue analysis and healing status.

Abstract:Among all diabetic‐related complications, the diabetic foot ulcer (DFU) is severe, demanding serious attention, and timely treatment. The purpose of the present study was to conduct features fusion of machine learning (ML) based handcrafted low‐level and convolutional neural networks (CNNs) based high‐level features for improving automatic diagnosis of DFU. A standard image dataset containing 1038 abnormal (DFU) and 641 normal color skin patches has been used for experimental evaluation. Handcrafted features are extracted to uncover the edge, color, shape, and texture information from the images. Also, a new CNN architecture has been proposed based on deeper residual blocks for extracting high‐level features. Features fusion with several ML classifiers like logistic regression classifier, support vector machine, gradient boosting, and artificial neural network have shown improved DFU identification results compared to the individual feature categories. Thereby, LRC outperformed the state‐of‐the‐art results for all evaluation metrics by achieving 95.23% sensitivity, 95.37% F1‐score, and 96.50% area under the curve (AUC) value. Such impressive experimental observations suggest that the proposed approach is expected to provide efficient decision support to medical practitioners, thereby improving patient care.

Abstract:In recent times, DFU (diabetic foot ulcer) has become a universal health problem that affects many diabetes patients severely. DFU requires immediate proper treatment to avert amputation. Clinical examination of DFU is a tedious process and complex in nature. Concurrently, DL (deep learning) methodologies can show prominent outcomes in the classification of DFU because of their efficient learning capacity. Though traditional systems have tried using DL-based models to procure better performance, there is room for enhancement in accuracy. Therefore, the present study uses the AWSg-CNN (Adaptive Weighted Sub-gradient Convolutional Neural Network) method to classify DFU. A DFUC dataset is considered, and several processes are involved in the present study. Initially, the proposed method starts with pre-processing, excluding inconsistent and missing data, to enhance dataset quality and accuracy. Further, for classification, the proposed method utilizes the process of RIW (random initialization of weights) and log softmax with the ASGO (Adaptive Sub-gradient Optimizer) for effective performance. In this process, RIW efficiently learns the shift of feature space between the convolutional layers. To evade the underflow of gradients, the log softmax function is used. When logging softmax with the ASGO is used for the activation function, the gradient steps are controlled. An adaptive modification of the proximal function simplifies the learning rate significantly, and optimal proximal functions are produced. Due to such merits, the proposed method can perform better classification. The predicted results are displayed on the webpage through the HTML, CSS, and Flask frameworks. The effectiveness of the proposed system is evaluated with accuracy, recall, F1-score, and precision to confirm its effectual performance.

Abstract:The World Health Organization (WHO) has identified that diabetes mellitus (DM) is one of the most prevalent disease worldwide. Individuals with DM have a higher risk of mortality, and it is crucial to prioritize the treatment of foot ulcers, which is a significant complication associated with the disease, as they lead to the development of plantar ulcers, which results in the need to amputate part of the foot or leg. People with diabetes are at risk of experiencing various complications, such as heart disease, eye problems, kidney dysfunction, nerve damage, skin issues, foot ulcers, and dental diseases. Unawareness of the risk associated with diabetic foot ulcers (DFU) is a significant contributing factor to the mortality of diabetic patients. Evolving technological advancements such as deep learning techniques can be used to predict the symptoms of diabetic foot ulcers as early as possible, which helps to provide effective treatment to DM patients. This research introduces a methodology for analyzing images of foot ulcers in diabetic patients, focusing on feature extraction and classification. The dataset used in this study was collected from historical medical records and foot images of patients with diabetes, who commonly experience foot ulcers as a major complication. The dataset was pre-processed and segmented, and features were extracted using a deep recurrent neural network (DRNN). Image and numerical/text data were extracted separately, and the normal and abnormal diabetes ranges were identified. Foot images of patients with abnormal diabetes ranges were separated and classified using a pre-trained fast convolutional neural network (PFCNN) with U++net. The classification procedure involves the analysis of foot ulcers to predict their pathogenesis. To assess the effectiveness of the proposed technique, the study presented simulation results, including a confusion matrix and receiver operating characteristic curve. These results specifically focused on predicting two classes: normal and abnormal diabetes foot ulcerations. The analysis yielded various parameters, including accuracy, precision, recall curve, and area under the curve. The main goal of the study was to introduce an novel technique for assessing the risk of foot ulceration development in patients with diabetes, leveraging the analysis of foot ulcer images. The researchers collected a dataset of foot images and medical data from historical records of patients with diabetes and pre-processed and segmented the data. They then used a deep recurrent neural network to extract features from the segmented data and identified normal and abnormal diabetes ranges based on numerical and text data. Foot images of patients with abnormal diabetes ranges were classified using a pre-trained fast convolutional neural network with U++net to examine foot ulcers and forecast the development of the risk of diabetic foot ulcers (DFU). The study assessed the accuracy of the proposed technique as 99.32% by simulating results for feature extraction and the classification of diabetic foot ulcers. A comparison was made between this proposed technique and existing approaches.

Abstract:Diabetic foot ulcers (DFUs) are a prevalent and serious complication of diabetes, often leading to severe morbidity and even amputations if not timely diagnosed and managed. The increasing prevalence of DFUs poses a significant challenge to healthcare systems worldwide. Accurate and timely classification of DFUs is crucial for effective treatment and prevention of complications. In this paper, we present "DFootNet", an innovative and comprehensive classification framework for the accurate assessment of diabetic foot ulcers using a dense neural network architecture. Our proposed approach leverages the power of deep learning to automatically extract relevant features from diverse clinical DFU images. The proposed model comprises a multi-layered dense neural network designed to handle the intricate patterns and variations present in different stages and types of DFUs. The network architecture integrates convolutional and fully connected layers, allowing for hierarchical feature extraction and robust feature representation. To evaluate the efficacy of DFootNet, we conducted experiments on a large and diverse dataset of diabetic foot ulcers. Our results demonstrate that DFootNet achieves a remarkable accuracy of 98.87%, precision—99.01%, recall—98.73%, F1-score as 98.86%, and AUC-ROC as 98.13%, outperforming existing methods in distinguishing between ulcer and non-ulcer images. Moreover, our framework provides insights into the decision-making process, offering transparency and interpretability through attention mechanisms that highlight important regions within ulcer images. We also present a comparative analysis of DFootNet's performance against other popular deep learning models, showcasing its robustness and adaptability across various scenarios.

Abstract:<p>Recognition and analysis of Diabetic Foot Ulcers (DFU) using computerized methods is an emerging research area with the evolution of image-based machine learning algorithms. Existing research using visual computerized methods mainly focuses on recognition, detection, and segmentation of the visual appearance of the DFU as well as tissue classification. According to DFU medical classification systems, the presence of infection (bacteria in the wound) and ischaemia (inadequate blood supply) has important clinical implications for DFU assessment, which are used to predict the risk of amputation. In this work, we propose a new dataset and computer vision techniques to identify the presence of infection and ischaemia in DFU. This is the first time a DFU dataset with ground truth labels of ischaemia and infection cases is introduced for research purposes. For the handcrafted machine learning approach, we propose a new feature descriptor, namely the Superpixel Color Descriptor. Then we use the Ensemble Convolutional Neural Network (CNN) model for more effective recognition of ischaemia and infection. We propose to use a natural data-augmentation method, which identifies the region of interest on foot images and focuses on finding the salient features existing in this area. Finally, we evaluate the performance of our proposed techniques on binary classification, i.e. ischaemia versus non-ischaemia and infection versus non-infection. Overall, our method performed better in the classification of ischaemia than infection. We found that our proposed Ensemble CNN deep learning algorithms performed better for both classification tasks as compared to handcrafted machine learning algorithms, with 90% accuracy in ischaemia classification and 73% in infection classification.</p>

Abstract:This paper introduces the Diabetic Foot Ulcers dataset (DFUC2021) for analysis of pathology, focusing on infection and ischaemia. We describe the data preparation of DFUC2021 for ground truth annotation, data curation and data analysis. The final release of DFUC2021 consists of 15,683 DFU patches, with 5,955 training, 5,734 for testing and 3,994 unlabeled DFU patches. The ground truth labels are four classes, i.e. control, infection, ischaemia and both conditions. We curate the dataset using image hashing techniques and analyse the separability using UMAP projection. We benchmark the performance of five key backbones of deep learning, i.e. VGG16, ResNet101, InceptionV3, DenseNet121 and EfficientNet on DFUC2021. We report the optimised results of these key backbones with different strategies. Based on our observations, we conclude that Efficient-NetB0 with data augmentation and transfer learning provided the best results for multi-class (4-class) classification with macro-average Precision, Recall and F1-Score of 0.57, 0.62 and 0.55, respectively. In ischaemia and infection recognition, when trained on one-versus-all, EfficientNetB0 achieved comparable results with the state of the art. Finally, we interpret the results with statistical analysis and Grad-CAM visualisation.
Towards a Remote Patient Monitoring Platform for Comprehensive Risk Evaluations for People with Diabetic Foot Ulcers

Abstract:Diabetic foot ulcers (DFUs) significantly affect the lives of patients and increase the risk of hospital stays and amputation. We suggest a remote monitoring platform for better DFU care. This system uses digital health metrics (scaled from 0 to 10, where higher scores indicate a greater risk of slow healing) to provide a comprehensive overview through a visual interface. The platform features smart offloading devices that capture behavioral metrics such as offloading adherence, daily steps, and cadence. Coupled with remotely measurable frailty and phenotypic metrics, it offers an in-depth patient profile. Additional demographic data, characteristics of the wound, and clinical parameters, such as cognitive function, were integrated, contributing to a comprehensive risk factor profile. We evaluated the feasibility of this platform with 124 DFU patients over 12 weeks; 39% experienced unfavorable outcomes such as dropout, adverse events, or non-healing. Digital biomarkers were benchmarked (0–10); categorized as low, medium, and high risk for unfavorable outcomes; and visually represented using color-coded radar plots. The initial results of the case reports illustrate the value of this holistic visualization to pinpoint the underlying risk factors for unfavorable outcomes, including a high number of steps, poor adherence, and cognitive impairment. Although future studies are needed to validate the effectiveness of this visualization in personalizing care and improving wound outcomes, early results in identifying risk factors for unfavorable outcomes are promising.

Abstract:Diabetic foot ulcers (DFUs) constitute a serious complication for people with diabetes. The care of DFU patients can be substantially improved through self-management, in order to achieve early-diagnosis, ulcer prevention, and complications management in existing ulcers. In this paper, we investigate two categories of image-to-image translation techniques (ItITT), which will support decision making and monitoring of diabetic foot ulcers: noise reduction and super-resolution. In the former case, we investigated the capabilities on noise removal, for convolutional neural network stacked-autoencoders (CNN-SAE). CNN-SAE was tested on RGB images, induced with Gaussian noise. The latter scenario involves the deployment of four deep learning super-resolution models. The performance of all models, for both scenarios, was evaluated in terms of execution time and perceived quality. Results indicate that applied techniques consist a viable and easy to implement alternative that should be used by any system designed for DFU monitoring.

Abstract:Diabetic Foot Ulcer (DFU) is a condition requiring constant monitoring and evaluations for treatment. DFU patient population is on the rise and will soon outpace the available health resources. Autonomous monitoring and evaluation of DFU wounds is a much-needed area in health care. In this paper, we evaluate and identify the most accurate feature extractor that is the core basis for developing a deep-learning wound detection network. For the evaluation, we used mAP and F1-score on the publicly available DFU2020 dataset. A combination of UNet and EfficientNetb3 feature extractor resulted in the best evaluation among the 14 networks compared. UNet and Efficientnetb3 can be used as the classifier in the development of a comprehensive DFU domain-specific autonomous wound detection pipeline.
Real-time diabetic foot ulcer classification based on deep learning & parallel hardware computational tools

Abstract:Meeting the rising global demand for healthcare diagnostic tools is crucial, especially with a shortage of medical professionals. This issue has increased interest in utilizing deep learning (DL) and telemedicine technologies. DL, a branch of artificial intelligence, has progressed due to advancements in digital technology and data availability and has proven to be effective in solving previously challenging learning problems. Convolutional neural networks (CNNs) show potential in image detection and recognition, particularly in healthcare applications. However, due to their resource-intensiveness, they surpass the capabilities of general-purpose CPUs. Therefore, hardware accelerators such as application-specific integrated circuits (ASICs), field-programmable gate arrays (FPGAs), and graphics processing units (GPUs) have been developed. With their parallelism efficiency and energy-saving capabilities, FPGAs have gained popularity for DL networks. This research aims to automate the classification of normal and abnormal (specifically Diabetic Foot Ulcer—DFU) classes using various parallel hardware accelerators. The study introduces two CNN models, namely DFU_FNet and DFU_TFNet. DFU_FNet is a simple model that extracts features used to train classifiers like SVM and KNN. On the other hand, DFU_TFNet is a deeper model that employs transfer learning to test hardware efficiency on both shallow and deep models. DFU_TFNet has outperformed AlexNet, VGG16, and GoogleNet benchmarks with an accuracy 99.81%, precision 99.38% and F1-Score 99.25%. In addition, the study evaluated two high-performance computing platforms, GPUs and FPGAs, for real-time system requirements. The comparison of processing time and power consumption revealed that while GPUs outpace FPGAs in processing speed, FPGAs exhibit significantly lower power consumption than GPUs.

Classification of Diabetic Foot Ulcers from Images Using Machine Learning Approach

Abstract:Diabetic foot ulcers (DFUs) represent a significant and serious challenge associated with diabetes. It is estimated that approximately one third of individuals with diabetes will develop DFUs at some point in their lives. This common complication can lead to serious health issues if not properly managed. The early diagnosis and treatment of DFUs are crucial to prevent severe complications, including lower limb amputation. DFUs can be categorized into two states: ischemia and infection. Accurate classification is required to avoid misdiagnosis due to the similarities between these two states. Several convolutional neural network (CNN) models have been used and pre-trained through transfer learning. These models underwent evaluation with hyperparameter tuning for the binary classification of different states of DFUs, such as ischemia and infection. This study aimed to develop an effective classification system for DFUs using CNN models and machine learning classifiers utilizing various CNN models, such as EfficientNetB0, DenseNet121, ResNet101, VGG16, InceptionV3, MobileNetV2, and InceptionResNetV2, due to their excellent performance in diverse computer vision tasks. Additionally, the head model functions as the ultimate component for making decisions in the model, utilizing data collected from preceding layers to make precise predictions or classifications. The results of the CNN models with the suggested head model have been used in different machine learning classifiers to determine which ones are most effective for enhancing the performance of each CNN model. The most optimal outcome in categorizing ischemia is a 97% accuracy rate. This was accomplished by integrating the suggested head model with the EfficientNetB0 model and inputting the outcomes into the logistic regression classifier. The EfficientNetB0 model, with the proposed modifications and by feeding the outcomes to the AdaBoost classifier, attains an accuracy of 93% in classifying infections.

Abstract:Diabetic foot ulcer is a severe condition that requires close monitoring and management. For training machine learning methods to auto-delineate the ulcer, clinical staff must provide ground truth annotations. In this paper, we propose a new diabetic foot ulcers dataset, namely DFUC2022, the largest segmentation dataset where ulcer regions were manually delineated by clinicians. We assess whether the clinical delineations are machine interpretable by deep learning networks or if image processing refined contour should be used. By providing benchmark results using a selection of popular deep learning algorithms, we draw new insights into the limitations of DFU wound delineation and report on the associated issues. This paper provides some observations on baseline models to facilitate DFUC2022 Challenge in conjunction with MICCAI 2022. The leaderboard will be ranked by Dice score, where the best FCN-based method is 0.5708 and DeepLabv3+ achieved the best score of 0.6277. This paper demonstrates that image processing using refined contour as ground truth can provide better agreement with machine predicted results. DFUC2022 will be released on the 27th April 2022.

Abstract:This research proposes a mobile and cloud-based framework for the automatic detection of diabetic foot ulcers and conducts an investigation of its performance. The system uses a cross-platform mobile framework that enables the deployment of mobile apps to multiple platforms using a single TypeScript code base. A deep convolutional neural network was deployed to a cloud-based platform where the mobile app could send photographs of patient’s feet for inference to detect the presence of diabetic foot ulcers. The functionality and usability of the system were tested in two clinical settings: Salford Royal NHS Foundation Trust and Lancashire Teaching Hospitals NHS Foundation Trust. The benefits of the system, such as the potential use of the app by patients to identify and monitor their condition, are discussed.

Enhancing diabetic foot ulcer prediction with machine learning: A focus on Localized examinations

Abstract:Background: diabetices foot ulcer (DFU) are serious complications. It is crucial to detect and diagnose DFU early in order to provide timely treatment, improve patient quality of life, and avoid the social and economic consequences. Machine learning techniques can help identify risk factors associated with DFU development. Objective: The aim of this study was to establish correlations between clinical and biochemical risk factors of DFU through local foot examinations based on the construction of predictive models using automated machine learning techniques. Methods: The input dataset consisted of 566 diabetes cases and 50 DFU risk factors, including 9 local foot examinations. 340 patients with Class 0 labeling (low-risk DFU), 226 patients with Class 1 labeling (high-risk DFU). To divide the training group (consisting of 453 cases) and the validation group (consisting of 113 cases), as well as preprocess the data and develop a prediction model, a Monte Carlo cross-validation approach was employed. Furthermore, potential high-risk factors were analyzed using various algorithms, including Bayesian BYS, Multi-Gaussian Weighted Classifier (MGWC), Support Vector Machine (SVM), and Random Forest Classifier (RF). A three-layer machine learning training was constructed, and model performance was estimated using a Confusion Matrix. The top 30 ranking feature variables were ultimately determined. To reinforce the robustness and generalizability of the predictive model, an independent dataset comprising 248 cases was employed for external validation. This validation process evaluated the model's applicability and reliability across diverse populations and clinical settings. Importantly, the external dataset required no additional tuning or adjustment of parameters, enabling an unbiased assessment of the model's generalizability and its capacity to predict the risk of DFU. Results: The ensemble learning method outperformed individual classifiers in various performance evaluation metrics. Based on the ROC analysis, the AUC of the AutoML model for assessing diabetic foot risk was 88.48 % (74.44-97.83 %). Other results were found to be as follows: 87.23 % (63.33 %-100.00 %) for sensitivity, 87.43 % (70.00 %-100.00 %) for specificity, 87.33 % (76.66 %-95.00 %) for accuracy, 87.69 % (75.00 %-100.00 %) for positive predictive value, and 87.70 % (71.79 %-100.00 %) for negative predictive value. In addition to traditional DFU risk factors such as cardiovascular disorders, peripheral artery disease, and neurological damage, we identified new risk factors such as lower limb varicose veins, history of cerebral infarction, blood urea nitrogen, GFR (Glomerular Filtration Rate), and type of diabetes that may be related to the development of DFU. In the external validation set of 158 samples, originating from an initial 248 with exclusions due to missing labels or features, the model still exhibited strong predictive accuracy. The AUC score of 0.762 indicated a strong discriminatory capability of the model. Furthermore, the Sensitivity and Specificity values provided insights into the model's ability to correctly identify both DFU cases and non-cases, respectively. Conclusion: The predictive model, developed through AutoML and grounded in local foot examinations, has proven to be a robust and practical instrument for the screening, prediction, and diagnosis of DFU risk. This model not only aids medical practitioners in the identification of potential DFU cases but also plays a pivotal role in mitigating the progression towards adverse outcomes. And the recent successful external validation of our DFU risk prediction model marks a crucial advancement, indicating its readiness for clinical application. This validation reinforces the model's efficacy as an accessible and reliable tool for early DFU risk assessment, thereby facilitating prompt intervention strategies and enhancing overall patient outcomes.
